ABSTRACT Dementia poses a significant societal and health challenge in the 21st century, with many hospitalized patients experiencing dementia without a documented diagnosis. Objective: To evaluate the prevalence of dementia and its associated risk factors among older patients admitted to hospitals. Methods: The study included older patients (≥ 60 years) admitted to medical departments of a general hospital in three major Iranian cities. Researchers utilized the Activities of Daily Living-Instrumental Activities of Daily Living (ADL-IADL) scale, the Geriatric Depression Scale (GDS), the Mini-Cog test, the 4 A’s test (4AT), and the Abbreviated Mental Test Score (AMTS). Among the 420 recruited older inpatients, 228 (54.3%) were female. Results: The mean age of participants was 71.39 years (standard deviation ±7.95), with 30.7% diagnosed with major neurocognitive disorder (dementia). The likelihood of dementia exhibited statistically significant correlations with gender, age, number of children, and occupation. ABSTRACT. Given the importance of dementia syndrome and its impacts on the population, interest in studying modifiable risk factors for dementia is growing. Objective: To compare the prevalence of risk factors for dementia in middle-aged and older adults over a two-year period and to identify what variables in baseline were predictive of cognitive decline in the follow-up. Methods: Longitudinal and quantitative study, with follow-up evaluation after two years, conducted with 200 participants aged 45 years or more, registered in Primary Care Units. In the baseline (2018/2019) and follow-up (2021) assessments, sociodemographic data were collected, and cognitive performance and risk factors for dementia were evaluated (education, hearing loss, head trauma, high blood pressure, alcohol use, obesity, smoking, depressive symptoms, social isolation, physical inactivity, and diabetes mellitus). Data were compared using the McNemar’s test. Individual multinomial logistic regression models were performed to identify the factors associated with cognitive decline after two years. Results: The percentages of low education, traumatic brain injury, and smoking remained the same in both assessments. There was a significant increase in the prevalence of high blood pressure (from 55.0 to 62.0%) and physical inactivity (from 58.5 to 74.5%) and a significant reduction in social isolation (from 25.0 to 18.0%). Participants with depressive symptoms in baseline had a higher risk of cognitive decline in follow-up. RESUMO. Dada a importância da síndrome demencial e de seus impactos na população, cresce o interesse em estudar os fatores de risco modificáveis para a demência. Objetivo: Comparar a prevalência de fatores de risco para demência em adultos de meia-idade e pessoas idosas, em um período de dois anos, e identificar quais fatores de risco na avaliação de base predizem o declínio cognitivo na avaliação de acompanhamento. Métodos: Estudo longitudinal e quantitativo, com avaliação de acompanhamento após dois anos, realizado com 200 participantes com 45 anos ou mais, cadastrados na Atenção Básica. Nas avaliações de base (2018/2019) e de acompanhamento (2021) foram coletados dados sociodemográficos, foram feitas avaliação cognitiva e avaliação dos fatores de risco para demência (escolaridade, perda auditiva, traumatismo craniano, hipertensão arterial, uso de álcool, obesidade, tabagismo, sintomas depressivos, isolamento social, inatividade física, diabetes mellitus). Os dados foram comparados pelo teste de McNemar. Modelos de regressão logística multinominal individuais foram conduzidos para identificar quais fatores estavam associados ao declínio cognitivo após dois anos. RESUMO. A demência frontotemporal (DFT) é uma doença neurodegenerativa progressiva definida pela deterioração seletiva dos lobos frontal e temporal do cérebro. Esta revisão abrangente explorou o efeito da DFT na linguagem, na fala e no comportamento. ABSTRACT. The repercussions of Long COVID demand specialised rehabilitation care. Interdisciplinary interventions in a 58-year-old patient were implemented to improve the clinical, motor and cognitive complaints associated with COVID-19. The rehabilitation team performed quantitative and qualitative evaluations in the initial phase and after 12 months of follow-up. The patient’s neuropathic pain, ankle and foot muscle strength, gait pattern, general cognitive functioning, initiative, emotional expressiveness, processing speed, neuropsychiatric symptoms and quality of life improved. She demonstrated gains in metacognition and expanded the use of compensatory strategies, resuming her routine and professional activities, although still with signs of executive dysfunction. It is concluded that a rehabilitation program calibrated to the profile of the patient with Long COVID had positive effects on functionality and satisfaction with quality of life. RESUMO. As repercussões da COVID Longa demandam atendimentos especializados de reabilitação. Intervenções interdisciplinares em paciente de 58 anos foram implementadas com o objetivo de melhorar as queixas clínicas, motoras e cognitivas associadas à COVID-19. A equipe de reabilitação realizou avaliações quantiqualitativas na fase inicial e após 12 meses de acompanhamento. A paciente evoluiu com melhora da dor neuropática, da força muscular de tornozelo e pé, do padrão de marcha, do funcionamento cognitivo geral, da iniciativa, da expressividade emocional, da velocidade de processamento, dos sintomas neuropsiquiátricos e da qualidade de vida (domínios físico e psicológico). ABSTRACT. This review is based on a conference presented in June 2023. Its main objective is to explain the cognitive differences between humans and non-human primates (NHPs) focusing on characteristics of their brains. It is based on the opinion of a clinical neurologist and does not intend to go beyond an overview of this complex topic. As language is the main characteristic differentiating humans from NHPs, this review is targeted at their brain networks related to language. NHPs have rudimentary forms of language, including primitive lexical/semantic signs. Humans have a much broader lexical/semantic repertory, but syntax is the most important characteristic, which is probably unique to Homo sapiens. ABSTRACT. The immediate early gene exhibits activation markers in the nervous system consisting of ARC, EGR-1, and c-Fos and is related to synaptic plasticity, especially in the hippocampus. Immediate early gene expression is affected by physical exercise, which induces direct ARC, EGR-1, and c-Fos expression. Objective: To assess the impact of exercise, we conducted a literature study to determine the expression levels of immediate early genes (ARC, c-Fos, and EGR-1). Methods: The databases accessed for online literature included PubMed-Medline, Scopus, and ScienceDirect. The original English articles were selected using the following keywords in the title: (Exercise OR physical activity) AND (c-Fos) AND (Hippocampus), (Exercise OR physical activity) AND (ARC) AND (Hippocampus), (Exercise OR physical activity) AND (EGR-1 OR zif268) AND (Hippocampus). ABSTRACT This article aims to bridge the gap between drug development and lifestyle modification strategies for dementia prevention. First, it focuses on three lessons from drug development to inform lifestyle changes: dementia treatment-resistance, the importance of maximizing therapeutic dose, and obtaining actionable feedback from drug trials. Moreover, epidemiology reveals a mismatch between promising associations and later interventions, the specificity of different dementias, and the ethics of health promotion. The article will argue that lifestyle choices for dementia risk should be understood through the lens of a life philosophy rather than a testable scientific hypothesis. Em primeiro lugar, ele se concentra em três lições do desenvolvimento de medicamentos para informar as mudanças no estilo de vida: resistência ao tratamento da demência, a importância de maximizar a dose terapêutica e a obtenção de informações acionáveis a partir de testes de medicamentos. ABSTRACT Huntington's disease (HD) is a hereditary, neurodegenerative disease characterized by motor, cognitive, and behavioral issues. As the disease progresses, people become dependent on their caregivers leading to a significant burden. There is a lack of effective neuropsychological intervention to support this population. Objective: To develop an online neuropsychological intervention and evaluate its effects in decreasing the HD caregiver burden. Methods: This qualitative pilot study was conducted online during the COVID-19 pandemic in Brazil. We selected a convenience sample of five informal caregivers of HD patients (mean 46.2 years old). At baseline, participants were assessed via an individual online neuropsychology interview/screening, covering demographic data. Subsequently, we conducted 20 online 90-minute sessions of the psychological consultation over 12 months. Each session was structured in three stages: 1) Welcoming; 2) Breathing and mindfulness practice; and 3) Discussion of themes about the group's needs based on the main complaints. A second intervention assessment was applied. Results: None of the caregivers were provided with details about HD at the time of diagnosis and later care. The necessity to care for a person with a rare genetic disease was added to caregivers' already-existing working routine and home services, causing a symptom of overload. After the neuropsychological intervention, all participants reported feeling less burdened by caring, improved self-care and conflict resolution with the person with HD through expanding behavioral repertoire focused on emotions and communication. RESUMO A doença de Huntington (DH) é hereditária e neurodegenerativa, caracterizada por problemas motores, cognitivos e comportamentais. À medida que avança, os pacientes dependem cada vez mais de cuidadores, gerando uma sobrecarga significativa. Há uma carência de intervenções neuropsicológicas eficazes para apoiar esta população. Objetivo: Desenvolver uma intervenção neuropsicológica online e avaliar seus efeitos na redução da sobrecarga do cuidador em DH. Métodos: Este estudo piloto qualitativo ocorreu online, durante a pandemia de COVID-19 no Brasil. Selecionamos uma amostra de conveniência de cinco cuidadores informais de pacientes com DH (média de 46,2 anos). Inicialmente, realizamos triagens neuropsicológicas online, abrangendo dados demográficos e entrevistas. ABSTRACT Active aging is based on four pillars: health, safety, participation, and lifelong learning. These pillars help individuals improve their quality of life throughout the aging process. Objective: To analyze the effectiveness of the Active Aging Program; identify the sociodemographic profile of the participants; identify prevalent diseases in the initial evaluation; and assess the results of the program after one year of follow-up. Methods: This is a quantitative, cross-sectional, exploratory, desk research, and descriptive study. Data from 545 employees of University of São Paulo participating in the Active Aging Program of the University Teaching Hospital of the University of São Paulo (HU-USP) between 2015 and 2018 were analyzed using the Minitab Program. For data analysis, Pearson's chi-square test was used to determine the association between Groups A and B. For continuous measures, the paired t-test was used to verify differences in means, adopting a 95% confidence interval and significance level of 0.05. Results: Statistically significant correlations were found when crossing sex with smoking; sex with triglycerides; age with risk factors for cardiovascular diseases, being the age group 40–59 statistically more significant; physical activity with risk factors for cardiovascular diseases; and body mass index at program entry with the one-year result. RESUMO O envelhecimento ativo se apoia em quatro pilares: saúde, segurança, participação e aprendizagem ao longo da vida. Esses pilares auxiliam os indivíduos na qualidade de vida ao longo do processo de envelhecimento. Objetivo: Analisar a efetividade do Programa Envelhecimento Ativo; identificar o perfil sociodemográfico dos participantes; identificar patologias predominantes na avaliação inicial; e verificar os resultados do programa após um ano de acompanhamento. Métodos: Trata-se de pesquisa quantitativa, de caráter transversal, exploratória, documentária e descritiva. Foram analisados os dados de 545 funcionários da Universidade de São Paulo que participaram do Programa do Envelhecimento Ativo do Hospital Universitário da Universidade de São Paulo (HU-USP) no período de 2015 a 2018. RESUMO. O envelhecimento da população mundial tem levado a um aumento da epidemiologia e da carga da Doença de Alzheimer e outras Demências. Objetivo: Descrever a Carga Global da Doença de Alzheimer de Início Precoce e outras Demências (DAIPoD) por região do mundo e nível de renda por meio de uma análise secundária do Estudo Global de Carga de Doenças (Global Burden of Disease Study – GBD), 2019. Métodos: Estudo ecológico transversal descritivo. Os dados de cinco anos por sexo e faixa etária de 40 a 64 anos foram extraídos da ferramenta de resultados do GBD. Realizou-se uma análise descritiva de prevalência, incidência, óbitos, Anos de Vida Ajustados por Incapacidade (Disability Adjusted Life Years – DALY), Anos de Vida Perdidos (Years of Life Lost – YLL) e Anos Vividos com Incapacidade (Years Lived with Disability – YLD). Resultados: Em 2019, a DAIPoD apresentou uma prevalência de 2,67 casos e uma incidência de 0,44 casos por 1.000. Foi observada uma carga significativa, resultando em 1,16 DALY por 1.000, principalmente devido aos YLL e, em menor grau, aos YLDs. As regiões mais afetadas foram o Leste Asiático e Pacífico, a América Latina e Caribe, e a América do Norte. As taxas de carga são consistentemente mais elevadas entre as mulheres; não foi observado um gradiente por nível de renda dos países. ABSTRACT. The aging of the world population has led to an increase in the epidemiology and burden of Alzheimer's disease and other dementias. Objective: To describe the global burden of young-onset Alzheimer's disease and other dementias by world region and income through a secondary analysis of the Global Burden of Disease Study 2019. Methods: This is a descriptive cross-sectional ecological study. Data by sex and five-year age groups from 40 to 64 years were extracted from the Global Burden of Disease Study results tool. We performed a descriptive analysis of prevalence, incidence, deaths, disability-adjusted life years, years of life lost, and years lived with disability. Results: In 2019, young-onset Alzheimer's disease and other dementias presented a prevalence of 2.67 cases and an incidence of 0.44 per 1,000 inhabitants globally. It carried a significant burden, resulting in 1.16 disability-adjusted life years per 1,000 inhabitants, primarily due to years of life lost, and to a lesser extent due to years lived with disability. East Asia & the Pacific, Latin America & the Caribbean, and North America are the most affected regions. Burden rates are consistently higher among women; no gradient was observed by country income. ABSTRACT Today, talking about autism spectrum disorder (ASD) is the same as talking about cases that occur in one in 160 births worldwide. Some of them will be able to live independently when they grow up while others will have less autonomy and will be more dependent, requiring the support of caregivers throughout their lives. Objective: Understanding the emotional burden that this could generate on parents, we sought to analyze the level of stress and coping techniques in caregivers of children with ASD in Chile, 2021. Methods: Interview with a sample composed of 61 parents or guardians of people with ASD. Results: After data analysis, it was possible to perform a statistically significant correlation (p=0.002) between the level of stress and the coping strategies (problem-solving, self-criticism, emotional expression, wishful thinking, social support, cognitive restructuring, problem avoidance, and social withdrawal). In addition, positive strategies that reduce stress levels in parents or caregivers of children with ASD were identified (problem resolution, cognitive restructuring, social support, and emotional expression). Conclusion: Through this research, it was possible to respond to each of the stated objectives, managing to determine what were the characteristics of caregivers and their main difficulties. RESUMO Hoje, falar sobre transtorno do espectro autista (TEA) é falar sobre casos que ocorrem em um a cada 160 nascimentos em todo o mundo. Alguns deles conseguirão viver de forma independente quando crescerem, e outros terão menos autonomia e serão mais dependentes, necessitando do apoio de cuidadores ao longo da vida. Objetivo: Entendendo o gasto emocional que isso pode gerar nos pais, buscamos analisar o nível de estresse e as técnicas de enfrentamento em cuidadores de crianças com TEA no Chile, 2021. Métodos: Entrevistar uma amostra composta de 61 pais ou responsáveis de pessoas com TEA. ABSTRACT Dementia is one of the growing diseases in the world and has different types based on its definition. The Montreal Cognitive Assessment (MoCA) test has been employed to screen patients with dementia, cognitive impairment, and disruption of daily activities. Objective: This study examined the diagnostic value of the total MoCA score and its subscores in differentiating Alzheimer's disease (AD), frontotemporal dementia (FTD), dementia with Lewy body (DLB), and vascular dementia (VaD). Methods: A total of 241 patients (AD=110, FTD=90, DLB=28, and VaD=13) and 59 healthy persons, who were referred to a dementia clinic with memory impairment in Firoozgar Hospital, were included in this study. MoCA tests were performed in all patients and normal persons. Results: By using the receiver operating characteristic (ROC) curve and measuring the area under the curve (AUC) for the total MoCA score in each group, AUC was 0.616, 0.681, 0.6117, and 0.583 for differentiating AD, FTD, DLB, and VaD patients, respectively. Among the groups, just the VaD group showed no significant usefulness in using the total MoCA score to differentiate it. RESUMO A demência é uma das doenças que mais cresce no mundo e possui diferentes tipos de acordo com sua definição. O teste de Avaliação Cognitiva de Montreal (Montreal Cognitive Assessment – MoCA) tem sido empregado para a triagem de pacientes com demência, comprometimento cognitivo e perturbação das atividades diárias. Objetivo: Este estudo examinou o valor diagnóstico da pontuação total do MoCA e seus subescores na diferenciação entre doença de Alzheimer (DA), demência frontotempotal (DFT), demência com corpos de Lewy (DCL) e demência vascular (DV). Métodos: Este estudo incluiu 241 pacientes (DA=110, DFT=90, DCL=28 e DV=13) e 59 pessoas saudáveis, encaminhados à clínica de demência com comprometimento de memória no Hospital Firoozgar. O teste MoCA foi realizado em todos os pacientes e pessoas normais.
