RESUMO Objetivo: O estudo teve como objetivo estimar a prevalência de sífilis adquirida e fatores associados em uma pesquisa nacional. Métodos: "TransOdara" foi um estudo transversal compreendendo mulheres trans e travestis (MTT) em cinco grandes cidades do Brasil durante dezembro–2019 e julho–2021. A amostra foi recrutada usando o método respondente-driven sampling (RDS). O desfecho “sífilis ativa” foi definido como um teste treponêmico positivo e título do Venereal-Disease-Research-Laboratory (VDRL) maior ou igual a ⅛. Variáveis sociodemográficas foram descritas. Análises bi- e multivariadas foram realizadas, e odds ratio (OR) e IC95% foram estimados. Todas as análises foram realizadas no R,4.3.1. Resultados: Um total de 1.317 MTT foram recrutadas, com 1.291 sendo testadas para sífilis, das quais 294 (22,8%) preencheram os critérios para sífilis ativa. Na análise bivariada, raça negra/parda (OR=1,41; IC95% 1,01–1,97), nível básico de educação (OR=2,44; IC95% 1,17–5,06), não alteração do nome nos documentos (OR=1,39; IC95% 1,00–1,91) e trabalho sexual (pregresso OR=2,22; IC95% 1,47–3,32; parcial OR=2,75; IC95% 1,78–4,25; período integral OR=3,62; IC95%: 2,36-5,53) foram associados à sífilis ativa. Na análise multivariada, o trabalho sexual foi o único fator associado, 2,07 (IC95%: 1,37-3,13) trabalho sexual passado, 2,59 (IC95% 1,66–4,05) trabalho sexual em tempo parcial e 3,16 (IC95% 2,04–4,92) trabalho sexual como principal fonte de renda. Conclusão: A prevalência de sífilis ativa neste estudo foi elevada em comparação com outros países da América Latina. O trabalho sexual foi um fator associado importante com sífilis ativa, destacando o impacto que essa condição de vulnerabilidade pode ter na saúde das MTT, como membros de uma população-chave marginalizada. ABSTRACT Objective: The study aimed to estimate the prevalence of acquired syphilis and associated factors in a national survey. Methods: TransOdara was a cross-sectional study comprising transgender women and travestis (TGW) in five major cities in Brazil during December of 2019 and July of 2021. The sample was recruited using the respondent-driven sampling (RDS) method. The outcome “active syphilis” was defined as a positive treponemal test and Venereal-Disease-Research-Laboratory (VDRL) title greater than∕ equal to ⅛. Sociodemographic variables were described. Bivariate and multiple logistic regression were performed, and odds ratios (OR) and 95% confidence intervals (95%CI) were estimated. All analyses were performed in R, 4.3.1. Results: A total of 1,317 TGW were recruited, with 1,291 being tested for syphilis, and 294 (22.8%) meeting the criteria for active syphilis. In bivariate analysis, black/mixed race (OR=1.41, 95%CI 1.01–1.97), basic level of education (OR=2.44, 95%CI 1.17–5.06), no name change in documents (OR=1.39, 95%CI 1.00–1.91) and sex work (past only OR= 2.22, 95%CI 1.47–3.32; partial OR=2.75, 95%CI 1.78–4.25; full time OR=3.62, 95%CI 2.36–5.53) were associated with active syphilis. In the multivariate analysis, sex work was the only associated factor, 2.07 (95%CI 1.37–3.13) past sex work, 2.59 (95%CI 1.66–4.05) part-time sex work and 3.16 (95%CI 2.04–4.92) sex work as the main source of income. Conclusion: The prevalence of active syphilis in this study was elevated compared with other countries in Latin America. Sex work was an important associated factor with active syphilis, highlighting the impact that this condition of vulnerability may have in the health of TGW, as members of a key, marginalized population. RESUMO Objetivo: O risco de doença cardiovascular é cerca de quatro vezes maior em pacientes com hipertensão arterial e diabetes mellitus, e sua detecção precoce é importante para diminuir a mortalidade. O objetivo deste estudo foi estimar a ocorrência simultânea de hipertensão arterial e diabetes mellitus, e o uso de medicamentos para tratamento concomitante de ambas as doenças, na população ≥50 anos, segundo posse de plano de saúde/convênio médico, de acordo com características sociodemográficas e comparando os anos de 2011 e 2021. Métodos: Utilizando dados do inquérito Vigitel, foram estimadas as prevalências e razões de prevalência brutas e ajustadas com intervalos de confiança de 95%, pela regressão de Poisson com variância robusta. Resultados: A prevalência de diagnóstico simultâneo dessas condições foi de 11,3% (IC95% 10,5–12,2) e de 14,3% (IC95% 13,2–15,4) em 2011 e 2021, respectivamente. Houve crescimento de 32% entre os usuários com plano de saúde (11,1% em 2011 vs. 13,8% em 2021) e 36% nos usuários do SUS (11,4% em 2011 vs. 14,7% em 2021). No período, houve aumento no uso concomitante de medicamentos para o conjunto da população, de 81,4% (IC95% 78,2–84,2) para 89,8% (IC95% 87,0–92,0). Esse aumento pode ser atribuído ao uso de medicamentos para diabetes, em ambos os estratos. Conclusão: O presente estudo mostrou que a prevalência simultânea de hipertensão e diabetes, assim como o uso de medicamentos para essas doenças, foram semelhantes entre usuários do Sistema Único de Saúde (SUS) e os usuários de planos de saúde/convênio, superando possíveis desigualdades entre os grupos no diagnóstico e tratamento dessas condições.

ABSTRACT Objective: The risk of cardiovascular disease is about four times higher in patients with hypertension and diabetes mellitus, and its early detection is important to reduce mortality. The aim of this study was to estimate the co-occurrence of hypertension and diabetes mellitus, and the use of medications for the concomitant treatment of both diseases, in the population ≥50 years of age, according to health insurance/medical plan ownership, according to sociodemographic characteristics, and comparing the years 2011 and 2021. Methods: Using data from the Vigitel survey, crude and adjusted prevalence and prevalence ratios with 95% confidence intervals were estimated using Poisson regression with robust variance. Results: The prevalence of concurrent diagnosis of these conditions was 11.3% (95%CI 10.5–12.2) and 14.3% (95%CI 13.2–15.4) in 2011 and 2021, respectively. There was an increase of 32% among users with health insurance (11.1% in 2011 vs. 13.8% in 2021) and 46% among SUS users (11.4% in 2011 vs. 14.7% in 2021). In this period, there was an increase in concomitant drug use for the whole population, from 81.4% (95%CI 78.2–84.2) to 89.8% (95%CI 87.0–92.0). This increase can be attributed to the use of diabetes medications in both strata. Conclusion: This study showed that the simultaneous prevalence of hypertension and diabetes and the use of medication for these diseases were similar between SUS users and health insurance users, overcoming possible inequalities between the groups in the diagnosis and treatment of these conditions.

RESUME Objetivo: Apresentar a metodologia utilizada no desenvolvimento de dois produtos para a vigilância da saúde materna e seus determinantes e discutir as suas possíveis utilizações. Métodos: A partir de modelo teórico dos determinantes do óbito materno e bases de dados dos sistemas de informação em saúde brasileiros, foram desenvolvidos dois produtos gratuitos: um painel interativo denominado “Vigilância da saúde materna” e um material educativo chamado “Aparecida: uma história sobre a vulnerabilidade da mulher brasileira à morte materna”, ambos disponíveis no site do Observatório Obstétrico Brasileiro. Resultados: Foram calculados mais de 30 indicadores para o período 2012-2020, contendo informações sobre condições socioeconômicas e de acesso a serviços de saúde, planejamento reprodutivo, assistência pré-natal, assistência ao parto, condições de nascimento e mortalidade e morbidade materna. Destacam-se os indicadores relacionados à morbidade materna grave em internações públicas, calculados pela primeira vez para o país. O painel permite análises por município ou agregadas por região de saúde, unidade da federação, macrorregião e país; análises de série histórica; e comparações entre localidades e com padrões de referência. Dados de qualidade da informação são apresentados e discutidos de forma integrada aos indicadores. No material educativo, visualizações com dados nacionais e internacionais são apresentadas, visando auxiliar na compreensão dos determinantes do óbito materno e facilitar a interpretação dos indicadores. ABSTRACT Objective: To present the methodology used in the development of two products for maternal health surveillance and its determinants and discuss their possible uses. Methods: Based on a theoretical model of the determinants of maternal death and databases of Brazilian health information systems, two free products were developed: an interactive panel “surveillance of maternal health” and an educational material “Aparecida: a story about the vulnerability of Brazilian women to maternal death”, both available on the website of the Brazilian Obstetric Observatory. Results: More than 30 indicators were calculated for the period 2012-2020, containing information on socioeconomic conditions and access to health services, reproductive planning, prenatal care, delivery care, conditions of birth and maternal mortality and morbidity. The indicators related to severe maternal morbidity in public hospitalizations stand out, calculated for the first time for the country. The panel allows analysis by municipality or aggregated by health region, state, macro-region and country; historical series analysis; and comparisons across locations and with benchmarks. Information quality data are presented and discussed in an integrated manner with the indicators. In the educational material, visualizations with national and international data are presented, aiming to help in the understanding of the determinants of maternal death and facilitate the interpretation of the indicators. Conclusion: It is expected that the two products have the potential to expand epidemiological surveillance of maternal health and its determinants, contributing to the formulation of health policies and actions that promote women’s health and reduce maternal mortality. RESUMO Objetivo: Analisar a dinâmica espaço-temporal de COVID-19 no estado do Rio de Janeiro nas nove regiões de saúde, entre março de 2020 e dezembro de 2022. Métodos: Utilizou-se o modelo de Poisson com efeitos aleatórios para suavizar a curva de incidência de hospitalizações por COVID-19 notificadas no Sistema de Informação da Vigilância Epidemiológica da Gripe (Sivep-Gripe) para verificar a sincronicidade da epidemia no estado. Resultados: A epidemia de COVID-19 no estado é caracterizada pela presença de sete picos no período analisado correspondentes a sete variantes encontradas. Identificou-se uma assincronicidade nas hospitalizações, variando de acordo com as diferentes variantes do vírus nas nove regiões de saúde do estado. Os picos de incidência das hospitalizações variaram de 1 a 12 casos por 100 mil habitantes no decorrer da pandemia. Conclusão: Essa análise espaço-temporal é extensível em outros cenários, sendo possível o monitoramento e a tomada de decisões de controle de doenças epidêmicas em várias áreas. ABSTRACT Objective: To analyze the spatio-temporal dynamics of COVID-19 in the Rio de Janeiro state within the nine health regions, between March 2020 and December 2022. Methods: The Poisson model with random effects was used to smooth and estimate the incidence of COVID-19 hospitalizations reported in the Influenza Epidemiological Surveillance Information System (SIVEP-Gripe) to verify the synchronicity of the epidemic in the state. Results: The COVID-19 epidemic in the state is characterized by the presence of seven peaks during the analyzed period corresponding to seven found. An asynchrony in hospitalizations was identified, varying according to the different virus variants in the nine health regions of the state. The incidence peaks of hospitalizations ranged from 1 to 12 cases per 100,000 inhabitants during the pandemic. Conclusion: This spatio-temporal analysis is applicable to other scenarios, enabling monitoring and decision-making for the control of epidemic diseases in different areas. RESUMO Objetivo: Analisar características da visita domiciliar (VD) no Brasil em 2012 e 2017. Métodos: Estudo ecológico com dados em painel cujas unidades de análise foram as equipes de saúde da Atenção Primária à Saúde no Brasil, participantes do 1º e 3º ciclos do Programa de Melhoria do Acesso e Qualidade da Atenção Básica do Sistema Único de Saúde. Foram realizadas análises descritivas, inferenciais e espaciais (alpha=5%). Resultados: Houve aumento na proporção de equipes que realizavam visita domiciliar com periodicidade definida a partir da análise de risco e vulnerabilidade e na de busca ativa de pessoas com sintomáticos respiratórios e de mulheres com exame citopatológico atrasado e alterado. Nordeste, Sudeste e Sul foram as regiões com maior concentração de equipes que realizavam a VD e faziam busca ativa. ABSTRACT Objective: To analyze characteristics of the home visit (HV) in Brazil, 2012 and 2017. Methods: Ecological study, with panel data whose units of analysis were the Primary Health Care teams in Brazil, participants of the 1st and 3rd cycles of the Program to Improve Access and Quality of Primary Care of the Unified Health System. Descriptive, inferential and spatial analyzes (alpha=5%) were performed. Results: There was an increase in the proportion of teams that carried out home visits at a frequency defined based on risk and vulnerability analysis and actively searched for people with respiratory symptoms and women with delayed and altered cytopathological examination. RESUMO Objetivo: Analisar a distribuição espacial dos nascimentos prematuros e sua associação com indicadores maternos, sociais e de serviços de saúde na região metropolitana de São Paulo, Brasil, 2010-2019. Métodos: Estudo ecológico utilizando dados sobre recém-nascidos pré-termo dos 39 municípios da região metropolitana de São Paulo. Utilizou-se o índice de Moran (Im) global univariado para avaliar a associação espacial da prematuridade, e o índice de Moran local univariado por meio do mapa de clusters (LISA) para a identificação de padrões e aglomerados espaciais. Também foi utilizado o índice de Moran global bivariado para analisar a autocorrelação espacial com os indicadores maternos, sociais e de serviços de saúde. Resultados: Foram registrados 3.103.898 nascidos vivos no período 2010-2019, dos quais 331.174 (10,7%) foram prematuros. O índice de Moran global mostrou independência espacial (Im=0,05; p-valor=0,233) da proporção dos nascimentos prematuros entre municípios. No entanto, na análise espacial local foi possível identificar aglomerado espacial estatisticamente significativo entre os municípios de Biritiba Mirim, Guararema e Salesópolis, com proporções altas de nascimentos pré-termo. Na análise bivariada, identificou-se associação espacial significativa positiva com proporções de mães menores de 20 anos (Im=0,17; p-valor=0,024) e mães com baixa escolaridade (Im=0,17; p-valor=0,020), e associação espacial significativa negativa com IDH (Im=-0,14; p-valor=0,039). Conclusão: A abordagem espacial local identificou agrupamento espacial situado no extremo leste da região metropolitana de São Paulo, onde ações dos gestores de saúde são necessárias para minimizar a ocorrência de partos prematuros. ABSTRACT Objective: To analyze spatial distribution of preterm births and their association with maternal, social, and health services indicators in the metropolitan region of São Paulo, Brazil, 2010-2019. Methods: Ecological study using data on preterm newborns from 39 municipalities in the metropolitan region of São Paulo. Univariate global Moran’s index (Im) was used to evaluate spatial association of prematurity, and univariate local Moran’s index by using the cluster map (LISA) to identify spatial patterns and clusters. Bivariate global Moran’s index was also used to analyze spatial autocorrelation with maternal, social, and health services indicators. Results: A total of 3,103,898 live births were registered in period 2010-2019, of which 331,174 (10.7%) were preterm. The global Moran’s index showed spatial independence (Im=0.05; p-value=0.233) of the proportion of preterm births between municipalities. However, in the local spatial analysis it was possible to identify a statistically significant spatial cluster between the municipalities of Biritiba Mirim, Guararema and Salesópolis, with high proportions of preterm births. In the bivariate analysis, a significant positive spatial association was identified with proportions of mothers under 20 years old (Im=0.17; p-value=0.024) and mothers with low schooling (Im=0.17; p-value=0.020), and a significant negative spatial association with HDI (Im=-0.14; p-value=0.039). Conclusions: The local spatial approach identified a spatial cluster located in the far east of the metropolitan region of São Paulo, where actions by health managers are needed to minimize occurrence of preterm births. RESUMO Objective Identificar o padrão epidemiológico, espacial e temporal da coinfecção TB-HIV no Brasil durante o período de 2001 a 2020. Métodos: Estudo ecológico com técnicas de análise espacial e temporal. Incluiu os casos de coinfecção por TB-HIV registrados no Brasil entre 2001 e 2020. A análise de tendência temporal foi realizada por meio da regressão segmentada, através do Joinpoint regression. Para a análise espacial, foram calculados os índices de Moran e construídos mapas coropléticos através dos softwares TerraView e QGIS. Resultados: Observou-se tendência temporal estável das taxas de incidência da coinfecção por TB-HIV no Brasil durante o período analisado. ABSTRACT Objective To identify the epidemiological, spatial, and temporal pattern of TB-HIV coinfection in Brazil during the period from 2001 to 2020. Methods: Ecological study using space-time analysis techniques. It included cases of TB-HIV coinfection registered in Brazil from 2001 to 2020. RESUMO Objective A Escala de Discriminação Explícita (EDE) foi desenvolvida para avaliar experiências com discriminação em inquéritos epidemiológicos no Brasil. Embora análises prévias tenham revelado boas propriedades configurais, métricas e escalares do instrumento, nenhum estudo examinou sua invariância. Este trabalho objetivou examinar a invariância fatorial de duas versões abreviadas da EDE, considerando cor/raça, sexo, posição socioeconômica e suas intersecções. Métodos: Utilizaram-se dados do Estudo EpiFloripa Adulto, que encerra uma amostra representativa de residentes de uma capital do sul do Brasil (n=1.187). Cerca de 57% da amostra foi constituída por mulheres e 90% dos entrevistados se declararam brancos; a média de idade dos participantes foi de 39 anos. Duas versões abreviadas da EDE, com sete e oito itens, foram examinadas por meio de Análises Fatoriais Confirmatórias Multigrupo e o método Alignment. ABSTRACT Objective The Explicit Discrimination Scale (EDS) was developed to assess experiences with discrimination in Brazilian epidemiologic surveys. Though previous analyses have demonstrated that the EDS has good configural, metric, and scalar properties, its invariance has not yet been investigated. In this study, we examined the factorial invariance of two abridged versions of the EDS, according to skin color/ethnicity, sex, socioeconomic status, and their intersections. Methods: Data from the EpiFloripa Adult Study were used, which include a representative sample of adults residing in a state capital of southern Brazil (n=1,187). Over half of the respondents were women, and around 90% identified as white; the mean age of the participants was 39 years. Two abridged versions of the EDS were analyzed, with seven and eight items, using Multigroup Confirmatory Analysis and the Alignment method. Results: The two versions of the scale may be used to provide estimates of discrimination that are comparable across skin color/ethnicity, sex, socioeconomic status, and their intersections. In the seven-item version of the scale, only one parameter lacked invariance (i.e., threshold of item i13 – called by names you do not like), specifically among black respondents with less than 12 years of formal education. RESUMO Objetivo: Investigar a associação entre as dimensões do Modelo de Crenças em Saúde (MCS) e a vacinação completa para hepatite B entre trabalhadores da saúde (TS). Métodos: Estudo epidemiológico de corte transversal com TS da Atenção Primária à Saúde e Média Complexidade. Realizaram-se análises uni e bivariada a fim de testar a associação entre a variável desfecho (vacinação completa para hepatite B a partir do autorrelato) e as variáveis das dimensões do MCS. Foram calculadas razões de prevalência (RP) e seus respectivos intervalos de confiança de 95% (IC95%). Resultados: Participaram do estudo 453 TS. A prevalência de vacinação completa para hepatite B foi de 56,9%. No modelo final de análise, estiveram associadas à vacinação completa para hepatite B as variáveis: chance de pegar hepatite B (RP=1,73) – relativa à dimensão de suscetibilidade; gravidade da doença (RP=0,74) – relativa à dimensão de gravidade; diminuição do risco de absenteísmo (RP=1,29) – relativa à dimensão de benefícios; não dispêndio de tempo para se vacinar (RP=1,41) e não preocupação acerca de eventos atribuíveis à vacinação ou imunização (RP=1,43) – relativas à dimensão de barreiras. ABSTRACT Objective: To investigate the association between the dimensions of the Health Belief Model (HBM) and complete vaccination for hepatitis B among healthcare workers (HCW). Methods: Cross-sectional epidemiological study with HCW in Primary Health and Medium Complexity Care. Univariate and bivariate analyses were performed to test the association between the outcome variable (complete vaccination for hepatitis B based on self-report) and the variables of the HBM dimensions. Prevalence ratio (PR) and its respective 95% confidence intervals (95%CI) were calculated. Results: 453 HCW participated. The prevalence of complete vaccination for hepatitis B was 56.9%. In the final analysis model, the following variables were associated with complete vaccination for hepatitis B: chances of having hepatitis B (PR=1.73) – related to the susceptibility dimension; disease severity (PR=0.74) – related to severity; reduced risk of absenteeism (PR=1.29) – related to benefits; not spending time to get vaccinated (PR=1.41) and not worrying about Events Supposedly Attributable to Vaccination or Immunization (PR=1.43) – related to barriers. RESUMO Objetivo: Estimar a probabilidade da ocorrência de infecção pelos vírus das hepatites B (HBV) e C (HCV) em diferentes estratos socioeconômicos da população de Recife, Nordeste do Brasil. Métodos: Estudo realizado com base em amostras obtidas em um inquérito de residentes de um grande centro urbano que teve base populacional e amostragem estratificada com seleção aleatória dos domicílios por meio do pacote “Amostra Brasil” no software R. A pesquisa do antígeno de superfície do HBV (HBsAg) e do anti-HCV foi realizada por testes imunocromatográficos. Nos casos positivos para HBsAg, foram realizadas pesquisas do anti-HBc e do HBeAg por quimioluminescência, bem como do HBV-DNA, por meio de PCR em tempo real. Para os casos positivos para anti-HCV, foi repetida a pesquisa desse anticorpo por quimioluminescência e do HCV-RNA por PCR em tempo real. A ocorrência de casos HBsAg e anti-HCV na população geral foi estimada com base em uma distribuição teórica binomial negativa. Resultados: Dentre 2.070 amostras examinadas, cinco (0,24%) foram HBsAg e duas (0,1%) anti-HCV positivas. ABSTRACT Objective: To estimate the probability of infection with hepatitis B (HBV) and C (HCV) viruses in different socioeconomic strata of the population of Recife, Northeast Brazil. Methods: Study carried out from samples obtained in a survey of residents of a large urban center that had a population base and stratified sampling with random selection of households using the “Brazil Sample” package in the R software. HBV (HBsAg) and anti-HCV was performed using immunochromatographic tests. In cases positive for HBsAg, anti-HBc and HBeAg were tested using chemiluminescence, as well as HBV-DNA using real-time PCR. For cases positive for anti-HCV, the search for this antibody was repeated by chemiluminescence and for HCV-RNA by real-time PCR. The occurrence of HBsAg and anti-HCV cases in the general population was estimated based on a theoretical negative binomial distribution. Results: Among 2,070 samples examined, 5 (0.24%) were HBsAg and 2 (0.1%) anti-HCV positive. The majority of cases had self-reported skin color as black/brown (6/7), education level up to high school (6/7), a steady partner (5/7) and lived in an area of low socioeconomic status (5/7). Conclusion : The occurrence of HBsAg and anti-HCV was lower than those previously found in population-based studies and slightly lower than the most recent estimates. RESUMO Objetivo: Analisar a tendência temporal dos indicadores de qualidade dos serviços de atenção à saúde para redução da hanseníase no Brasil, no período de 20 anos. Métodos: Trata-se de um estudo epidemiológico de tendência temporal, cujos dados foram extraídos do Sistema de Informação de Agravos de Notificação (SINAN). Foram construídos indicadores a partir do Manual Técnico-Operacional, que apresenta as Diretrizes para Vigilância, Atenção e Eliminação da Hanseníase como Problema de Saúde Pública do Ministério da Saúde. Para análise de tendência dos indicadores selecionados, utilizou-se o modelo de Prais-Winsten e calculou-se a Taxa de Incremento Anual (TIA). Resultados: Na série temporal de 20 anos aqui investigada, foram notificados no Brasil 732.959 casos de hanseníase. A tendência mostrou-se estacionária para cura de hanseníase entre os casos novos (β=-0,000; p=0,196; TIA=-0,2); casos de hanseníase em abandono de tratamento entre os casos novos (β=-0,001; p=0,147; TIA=-0,4); contatos examinados de casos novos de hanseníase (β=-0,001; p=0,112; TIA=1,6); casos novos de hanseníase com grau de incapacidade física avaliado no diagnóstico (β=-0,000; p=0,196; TIA=-0,2); e casos curados no ano com grau de incapacidade física avaliada (β=0,002; p=0,265; TIA=0,5); enquanto o indicador casos de recidiva entre os casos notificados no ano (β=0,019; p<0,001; TIA=0,5) apresentou tendência crescente. Indicators were constructed from the Ministry of Health Technical-Operational Manual that presents the Guidelines for Surveillance, Care and Elimination of Leprosy as a Public Health Problem. For trend analysis of the selected indicators, the Prais-Winsten model was used and the Average Annual Growth Rate (AAGR) was also calculated. Results: In the 20-year time series investigated here, 732,959 cases of leprosy were reported in Brazil. The trend was stationary for: new leprosy cases cure rate (β=-0.000; p=0.196; AAGR=-0.2), new leprosy cases drop out rate (β=-0.001; p=0.147; AAGR=-0.4), new leprosy cases contact tracing rate (β=-0.001; p=0.112; AAGR=1.6), new cases of leprosy with degree physical disability assessment rate among new cases (β=-0.000; p=0.196; AAGR=-0.2) and cases cured in the year with the degree of physical disability assessed (β=0.002; p=0.265; AAGR=0.5); while the indicator of recurrence rate among cases reported in the year (β=0.019; p<0.001; AAGR=0.5) showed an increasing trend. Conclusion: Based on the evaluation of indicators to assess the quality of healthcare services to reduce leprosy, it was evident that Brazil has major challenges for its full implementation, with improvements being necessary in the quality of care service offered to the population. RESUMO Objetivo: Este estudo teve como objetivo analisar a evolução espaçotemporal das taxas de incidência do vírus da imunodeficiência humana (HIV) e da síndrome da imunodeficiência adquirida (AIDS) no estado do Paraná, Brasil. Métodos: Foi realizado um estudo ecológico com componente analítico de séries temporais e análise espacial no estado do Paraná, de 2007 a 2022. A fonte de dados foi o Sistema de Informação de Agravos de Notificação. Para estudar a tendência, foi utilizado o modelo de regressão linear generalizada de Prais-Winsten por meio da decomposição de séries temporais e, para a análise espacial, foi aplicado o Índice de Moran. Resultados: A amostra total foi composta de 50.676 registros de HIV/AIDS. A taxa de incidência apresentou tendência crescente, com crescimento médio de 2,14% (intervalo de confiança de 95% — IC95% 1,16–3,13) ao mês. Nos períodos de 2007 a 2014 e 2015 a 2022, a média de casos no estado foi de 105,64 e 159,20 a cada 100 mil habitantes, respectivamente, com importantes variações entre os municípios. Agrupamentos espaciais de alto risco permaneceram na região metropolitana à capital e litoral e um novo agrupamento foi observado à região norte do estado. ABSTRACT Objective: The aim of this study was to analyze the spatiotemporal evolution of the incidence rates of human immunodeficiency virus (HIV) and acquired immune deficiency syndrome (AIDS) in the state of Paraná, Brazil. Methods: An ecological study with an analytical component of time series analysis was conducted in the state of Paraná from 2007 to 2022. The data source was the Notifiable Diseases Information System. To study the trend, the Prais–Winsten generalized linear regression model was used by decomposing the time series, and for spatial analysis, the Moran's index was applied. Results: The total sample consisted of 50,676 HIV/AIDS records. The incidence rate showed an increasing trend, with an average growth of 2.14% [95% confidence interval – 95%CI 1.16–3.13] per month. From 2007 to 2014 and from 2015 to 2022, the average number of cases in the state was 105.64 and 159.20 per 100,000 inhabitants, respectively, with significant variation among municipalities. Spatial clusters of high risk persisted in the metropolitan region, the capital, and coastal areas, and a new cluster was observed in the northern region of the state. Conclusion: The incidence rates of HIV/AIDS showed an upward trend over time. The number of cases varied considerably in some municipalities, especially in the coastal region. RESUMO Objetivo: Avaliar a violência doméstica (VD) longitudinalmente no período pós-parto, identificando tipos, padrões e determinantes de VD, segundo relatos de mães em Fortaleza, Brasil. Métodos: O estudo de coorte Iracema-COVID entrevistou em casa mães que pariram na primeira onda de COVID-19, aos 18 e 24 meses após o parto. Os padrões de VD relatados foram classificados da seguinte forma: VD inexistente, VD interrompida, VD iniciada e VD persistente. Regressões logísticas multinomiais brutas e ajustadas com variância robusta foram utilizadas para avaliar os fatores associados à VD persistente. Resultados: A VD foi relatada por 19–24% das mães aos 18 e 24 meses pós-parto, respectivamente, mostrando um aumento de 5 pontos percentuais. Em 11% dos domicílios a VD persistente esteve presente no período. As formas de VD incluíram agressão verbal, relatada por 17–20% das mães; embriaguez ou uso de drogas em casa, presente em 3-5% das residências; agressão física, relatada por 1,2–1,6% das mães. Residências com duas ou mais formas de VD aumentaram de 2 para 12% no período. Fatores de risco ajustados associados à VD persistente foram: transtorno mental comum materno, família chefiada pela mãe e baixa escolaridade do chefe de família. Insegurança alimentar esteve associada à VD iniciada. Conclusão: A prevalência de VD foi consideravelmente alta no período pós-parto. Políticas de prevenção de VD devem se basear em intervenções que visem melhorar a atenção à saúde mental das mulheres; combater a insegurança alimentar; e promover o nível educacional de jovens de ambos os sexos. ABSTRACT Objective: To longitudinally assess domestic violence (DV) during the postpartum period, identifying types, patterns and determinants of DV, according to mothers’ reports in Fortaleza, Brazil. Methods: Data from the Iracema-COVID cohort study interviewed at home mothers who gave birth in the first wave of COVID-19, at 18 and 24 months after birth. Patterns of reported DV were classified as follows: no DV, interrupted DV, started DV and persistent DV. Adjusted multinomial logistic regressions were used to assess factors associated with persistent DV. Results: DV was reported by 19 and 24% of the mothers at 18 and 24 months postpartum, respectively, a 5 percentage points increase. Persistent DV was present in 11% of the households in the period. The most frequent forms of DV were verbal aggression, reported by 17–20% of the mothers at 18 and 24 months, respectively; drunkenness or use of drugs at home, present in 3–5% of the households; physical aggression, reported by 1.2–1.6% of the mothers. Households with two or more forms of DV increased from 2 to 12% in the period. Adjusted factors associated with persistent DV were maternal common mental disorder, family headed by the mother and head of family’s poor schooling. Food insecurity was associated with starting DV. Conclusion: Prevalence of DV was considerably high in the postpartum period. DV prevention policies should rely on improving care to women’s mental health; preventing food insecurity; and fostering the educational level of young people of both sexes. RESUMO Objetivo: Analisar a influência dos fatores individuais e contextuais do hospital e do município de assistência sobre a sobrevida de pacientes com Síndrome Respiratória Aguda Grave por COVID-19. Métodos: Estudo de coorte hospitalar com dados de 159.948 adultos e idosos com Síndrome Respiratória Aguda Grave por COVID-19 internados de 01 de janeiro a 31 de dezembro de 2022 e notificados no Sistema de Informação de Vigilância Epidemiológica da Influenza. As variáveis contextuais foram relacionadas à estrutura, aos profissionais e equipamentos dos estabelecimentos hospitalares e indicadores socioeconômicos e de saúde dos municípios. O desfecho foi a sobrevida hospitalar em até 90 dias. Árvore de sobrevida e curvas de Kaplan-Meier foram utilizados para analisar a sobrevida. Resultados: A letalidade hospitalar foi de 30,4%. Idosos submetidos à ventilação mecânica invasiva e internados em cidades com baixo percentual de arrecadação de impostos apresentaram menor sobrevida quando comparados aos demais grupos identificados na árvore de sobrevida (p<0,001). ABSTRACT Objective: To analyze the influence of individual and contextual factors of the hospital and the municipality of care on the survival of patients with Severe Acute Respiratory Syndrome due to COVID-19. Methods: Hospital cohort study with data from 159,948 adults and elderly with Severe Acute Respiratory Syndrome due to COVID-19 hospitalized from January 1 to December 31, 2022 and reported in the Influenza Epidemiological Surveillance Information System. The contextual variables were related to the structure, professionals and equipment of the hospital establishments and socioeconomic and health indicators of the municipalities. The outcome was hospital survival up to 90 days. Survival tree and Kaplan-Meier curves were used for survival analysis. Results: Hospital lethality was 30.4%. Elderly patients who underwent invasive mechanical ventilation and were hospitalized in cities with low tax collection rates had lower survival rates compared to other groups identified in the survival tree (p<0.001). Conclusion: The study indicated the interaction of contextual factors with the individual ones, and it shows that hospital and municipal characteristics increase the risk of death, highlighting the attention to the organization, operation, and performance of the hospital network. RESUMO Objetivo: Verificar a evolução da disponibilidade de equipamentos e insumos de tecnologia de informação e comunicação nos serviços de Atenção Primária à Saúde que participaram da avaliação externa do Programa de Melhoria do Acesso e da Qualidade na Atenção Básica e sua distribuição de acordo com características do contexto social e geográfico. Métodos: Estudo transversal, que analisou a distribuição de equipamentos de tecnologia de informação e comunicação nas unidades básicas de saúde do Brasil, durante os três ciclos (2012 a 2018) do Programa de Melhoria do Acesso e da Qualidade na Atenção Básica. As variáveis foram examinadas no nível dos municípios e estratificadas por região geopolítica. Realizou-se análise univariada, com o uso do teste χ2 e teste das distribuições das exposições entre si e entre o desfecho e as exposições. Resultados: A disponibilidade do conjunto de equipamentos de tecnologias de informação e comunicação evoluiu de 9,4% (2012) para 17,5% (2018), com destaque às Regiões Sudeste e Sul, em municípios com porte populacional de até 10 mil hab.², com maior cobertura de saúde da família e índice de desenvolvimento humano municipal — IDH-M alto/muito alto. ABSTRACT Objective: To verify the evolution of the availability of information and communication technology equipment and inputs in primary health care services that participated in the external evaluation of the Access and Quality Improvement Program in Primary Care and its distribution according to context characteristics social and geographic. Methods: Cross-sectional study, analyzed the distribution of information and communication technology equipment in basic health units in Brazil, during the three cycles (2012 to 2018) of the Program for Improving Access and Quality in Primary Care. The variables were examined at the municipal level and stratified by geopolitical region. Univariate analysis was performed, using the chi-square test and testing the distributions of exposures among themselves and between the outcome and exposures. Results: The availability of information and communication technology equipment increased from 9.4% (2012) to 17.5% (2018), with emphasis on the Southeast and South regions, in municipalities with a population size of up to 10,000 inhabitants², with greater family health coverage and high/very high HDI-M. Over the period from 2012 to 2018, basic units joined the program and increased availability of information and communication technologies, such as Internet access, which ranged from 45.2% (n=6,249) to 74.0% (n=21,423), with emphasis on the Northeast region, which increased from 19.1% (n=970) to 58.8% (n=7,087).
