RESUMO RACIONAL: O conhecimento anatômico da artéria cística e de suas variações é essencial para a realização de colecistectomias seguras. A artéria cística possui origem na artéria hepática direita, passando posteriormente ao ducto hepático comum, anteriormente ao ducto cístico, ramificando-se em dois ramos no colo da vesícula biliar. Todavia, variações de posição, tamanho e relação com estruturas adjacentes são comuns. OBJETIVOS: Este artigo apresenta uma revisão de literatura em relação às variações da artéria cística e sua frequência durante colecistectomias. MÉTODOS: Os artigos selecionados desta revisão foram escolhidos a partir das bases de dados PubMed e SciELO. Utilizou-se os descritores padronizados: Anatomic variation e cholecystectomy. Estes, foram escolhidos usando o “Medical Subject Headings” e combinados com o operador booleano AND e o descritor não padronizado Cystic artery. RESULTADOS: Verificou-se em 54,5% dos estudos que o padrão anatômico da artéria cística foi o tipo de maior ocorrência. Citou-se uma origem diferente do padrão em 63,6% dos artigos. Encontrou-se a irrigação dupla da vesícula biliar em 59,1%. Em 36,4% a artéria cística estava anteriormente ao ducto hepático comum ou ao ducto cístico. Encontrou-se artérias císticas fora do triângulo de Calot em 36,4%. Artérias císticas curtas foram encontradas em 13,6%. Relatou-se a ausência ou não identificação da artéria em 9,1%. CONCLUSÕES: Variações da artéria cística são comuns, sendo frequentemente relatadas. Um aspecto de uma colecistectomia segura é o conhecimento anatômico e de possíveis variações. Assim, cirurgiões devem estar familiarizados com este ponto a fim de reduzir lesões vasculares e biliares.

ABSTRACT BACKGROUND: Knowledge of the cystic artery and its variations is essential to perform safe cholecystectomies. The cystic artery originates from the right hepatic artery, passing posterior to the common hepatic duct, anterior to the cystic duct, and branching into two branches at the neck of the gallbladder. However, variations in position, size, and relationship with adjacent structures are common. AIMS: This article presents a literature review regarding cystic artery variations and their frequency during cholecystectomies. METHODS: The articles selected for this review were chosen from the PubMed and SciELO databases. The standardized descriptors used were anatomic variation and cholecystectomy. These were chosen using the “Medical Subject Headings” and combined with the Boolean operator AND and the non-standard descriptor cystic artery. RESULTS: It was found in 54.5% of the studies that the anatomical pattern of the cystic artery was the most frequent type. A different origin from the standard was cited in 63.6% of the articles. Double irrigation of the gallbladder was found in 59.1%. In 36.4%, the cystic artery was anterior to the common hepatic duct or the cystic duct. Cystic arteries outside Calot’s triangle were found in 36.4%. Short cystic arteries were found in 13.6%. The absence or non-identification of the artery was reported in 9.1%. CONCLUSIONS: Variations of the cystic artery are common and are frequently reported. One aspect of a safe cholecystectomy is anatomical knowledge and its possible variations. Thus, surgeons must be familiar with this point in order to reduce vascular and biliary injuries.

RESUMO RACIONAL: Diferenças na cor da pele têm implicações socioeconômicas e de saúde, no entanto, ainda existem lacunas na compreensão da percepção da qualidade de vida relacionada à saúde (HRQoL). OBJETIVOS: Examinar se as diferenças na cor da pele influenciam a HRQoL em pacientes com obesidade submetidos à cirurgia de bypass gástrico Roux-en-Y. MÉTODOS: Estudo transversal com participantes de ambos os sexos, idades entre 18 e 60 anos e 3 a 6 meses de pós-operatório. Os dados foram coletados de outubro de 2018 a julho de 2019 em uma clínica bariátrica em Salvador, Bahia. A cor da pele de cada participante, escores do Questionário de Qualidade de Vida Moorehead-Ardelt II (MAQOL-II), medidas antropométricas, status socioeconômico, atividade física e percepções da imagem corporal foram registrados. RESULTADOS: Dos 196 pacientes, 67,35% eram negros. A “Autoestima” no MAQOL-II demonstrou a melhoria pós-cirúrgica mais significativa, com 62,8% relatando resultados “Muito melhores”. Resíduos ajustados associaram respostas “Muito melhores” e “Inalteradas” com a cor da pele. O escore geral do MAQOL-II indicou escores de HRQoL mais baixos (M=1,65; DP=0,98) para indivíduos com pele negra em comparação com aqueles com pele branca. Ao analisar as respostas do questionário, ambos os grupos raciais apresentaram percentuais iguais (45,3%) relatando progresso “Muito melhor” e “Melhor” pós-cirúrgico. No entanto, não foram observadas diferenças estatisticamente significativas na HRQoL ao comparar a cor da pele. CONCLUSÕES: A cor da pele parece não impactar significativamente na HRQoL de pacientes com obesidade submetidos ao bypass gástrico Roux-en-Y.

ABSTRACT BACKGROUND: Differences in skin color have socioeconomic and health implications; however, gaps persist in understanding health-related quality of life (HRQoL) perception. AIMS: To examine whether skin color differences influence HRQoL in obese patients undergoing Roux-en-Y gastric bypass surgery. METHODS: Cross-sectional study with participants of both genders, aged 18 to 60, and three to six months postoperatively. Data were collected from October 2018 to July 2019 at a bariatric clinic in Salvador, Bahia. Skin color, Moorehead-Ardelt II Quality of Life Questionnaire (MAQOL-II) scores, anthropometric measurements, socioeconomic status, physical activity, and body image perceptions were recorded. RESULTS: Of 196 patients, 67.35% were Black. “Self-esteem” in MAQOL-II demonstrated the most significant post-surgical improvement, with 62.8% reporting “much better” outcomes. Adjusted residuals associated “much better” and “unchanged” responses with skin color. The overall MAQOL-II score indicated lower HRQoL scores (M=1.65; standard deviation - SD=0.98) for individuals with black skin compared to those with white skin. Analyzing questionnaire responses, both racial groups exhibited equal percentages (45.3%) reporting “much better” and “better” post-surgery progress. However, no statistically significant differences in HRQoL were observed when comparing skin color. CONCLUSIONS: Skin color appears not to significantly impact the HRQoL of obese patients undergoing Roux-en-Y gastric bypass.

RESUMO RACIONAL: A doença inflamatória intestinal (DII), que engloba a doença de Crohn (DC) e a colite ulcerativa (CU), carece de etiologia conhecida. Embora sintomas clínicos, imagens e colonoscopia sejam ferramentas diagnósticas comuns, a calprotectina fecal (CalF) atua como biomarcador amplamente utilizado para monitorar a atividade da doença. A metabolômica, no âmbito das ciências “omics,” apresenta promissoras perspectivas para identificação de biomarcadores de progressão da doença. Este método envolve o estudo de metabólitos em meios biológicos para revelar fatores patológicos. OBJETIVOS: Explorar a metabolômica fecal em pacientes com DII, avaliando seu potencial na diferenciação de subtipos e na avaliação da atividade da doença usando CalF. MÉTODOS: Estudo transversal incluindo pacientes com DII, dados clínicos e medições de CalF (=200μg/g como indicador de doença ativa). RESULTADOS: A metabolômica fecal utilizou cromatografia de massa/extração em fase sólida com o software MetaboAnalyst 5.0 para análise. Dos 52 pacientes (29 CU, 23 DC), 36 (69,2%) apresentaram atividade inflamatória. Foram identificados 56 metabólitos fecais, sendo os ácidos hexadecanoico, esqualeno e octadecanoico notáveis na distinção entre CD e CU. Para CU, os ácidos octadecanoico e hexadecanoico correlacionaram-se com a atividade da doença, enquanto o ácido octadecanoico foi mais relevante em CD. CONCLUSÕES: Os achados destacam o potencial da metabolômica como um complemento não invasivo para avaliar a doença inflamatória intestinal, auxiliando no diagnóstico e na avaliação da atividade da doença.

ABSTRACT BACKGROUND: Inflammatory bowel disease (IBD), comprising Crohn’s disease (CD) and ulcerative colitis (UC), lacks a known etiology. Although clinical symptoms, imaging, and colonoscopy are common diagnostic tools, fecal calprotectin (FC) serves as a widely used biomarker to track disease activity. Metabolomics, within the omics sciences, holds promise for identifying disease progression biomarkers. This approach involves studying metabolites in biological media to uncover pathological factors. AIMS: The purpose of this study was to explore fecal metabolomics in IBD patients, evaluate its potential in differentiating subtypes, and assess disease activity using FC. METHODS: Cross-sectional study including IBD patients, clinical data, and FC measurements (=200 μg/g as an indicator of active disease). RESULTS: Fecal metabolomics utilized chromatography mass spectrometry/solid phase microextraction with MetaboAnalyst 5.0 software for analysis. Of 52 patients (29 UC, 23 CD), 36 (69.2%) exhibited inflammatory activity. We identified 56 fecal metabolites, with hexadecanoic acid, squalene, and octadecanoic acid notably distinguishing CD from UC. For UC, octadecanoic and hexadecanoic acids correlated with disease activity, whereas octadecanoic acid was most relevant in CD. CONCLUSIONS: These findings highlight the potential of metabolomics as a noninvasive complement for evaluating IBD, aiding diagnosis, and assessing disease activity.

RESUMO Racional: Pesquisas indicam que pacientes submetidos à cirurgia bariátrica enfrentam um risco seis a sete vezes maior de desenvolver transtorno por uso de álcool (TUA) em comparação à população de indivíduos obesos que não passam por intervenção cirúrgica. Estudos sugerem que o consumo problemático de álcool, incluindo depressão, aumenta gradualmente após a cirurgia. OBJETIVOS: Avaliar o impacto da cirurgia bariátrica na incidência de transtorno do uso de álcool (TUA) e depressão durante o período pós-operatório. MÉTODOS: Estudo prospectivo avaliando 68 pacientes submetidos à gastrectomia vertical (GV) ou bypass gástrico em Y-de-Roux (BGYR). A presença de TUA e depressão foi avaliada tanto no pré quanto no pós-operatório. A avaliação de TUA utilizou o escore AUDIT-C, enquanto a depressão foi avaliada pelo Inventário de Depressão de Beck (IDB). RESULTADOS: A idade média da amostra foi de 42,81±9,28 anos, sendo 85,3% do sexo feminino. O acompanhamento médio foi de 16,54±7,41 meses. Na avaliação pré-operatória, 92,6% da amostra estava na categoria de baixo risco para TUA. Não houve diferença significativa entre os grupos BGYR e GV. No pós-operatório, 89,7% da amostra foi classificada como de baixo risco para TUA, sem diferenças significativas em relação à avaliação pré-operatória. Em relação à depressão, não houve diferença significativa entre os períodos pré e pós-operatório para todos os pacientes. No entanto, foi observada uma tendência significativa de redução da “depressão severa” no período pós-operatório para os pacientes submetidos à GV (pré: 14,0% vs. pós: 7,0%, p=0,013, p<0.05). CONCLUSÕES: Não há diferença significativa na presença de transtorno do uso de álcool e depressão entre as avaliações pré e pós-operatórias em pacientes submetidos à cirurgia bariátrica.

ABSTRACT BACKGROUND: Research indicates that patients undergoing bariatric surgery face a six to seven times higher risk of developing alcohol use disorder (AUD) compared with the population of obese individuals not undergoing surgical intervention. Studies suggest that problematic alcohol consumption encompassing depression escalates gradually after surgery. AIMS: The purpose of this study was to evaluate the impact of bariatric surgery on the incidence of AUD and depression during the postoperative period. METHODS: Prospective study that evaluated 68 patients who underwent either sleeve gastrectomy (SG) or Roux-en-Y gastric bypass (RYGB). The presence of AUD and depression was assessed both pre- and post-operatively. AUD assessment utilized the AUD identification test-C score, whereas depression assessment employed the Beck Depression Inventory (BDI). RESULTS: The average age of the sample was 42.81±9.28 years, with 85.3% being female. The mean follow-up was 16.54±7.41 months. In the preoperative assessment, 92.6% of the sample fell into the low-risk category for AUD. No significant difference was observed between the RYGB and SG groups. Postoperatively, 89.7% of the sample was classified as low risk for AUD, with no significant differences compared with the preoperative assessment. Regarding depression, there was no significant difference between pre- and post-operative periods for all patients. However, a notable trend toward a reduction in “severe depression” was observed in the postoperative period for patients undergoing SG (pre: 14.0% vs. post: 7.0%, p=0.013). CONCLUSIONS: There is no significant difference in the presence of AUD and depression between pre- and post-operative assessments in patients who have undergone bariatric surgery.

RESUMO RACIONAL: A colonoscopia é um procedimento endoscópico amplamente utilizado para investigar doenças do cólon e do reto. O procedimento de colonoscopia apresenta dificuldades para o paciente e o endoscopista. OBJETIVOS: Investigar se o uso de um espartilho abdominal pode tornar o procedimento de colonoscopia mais fácil e rápido. MÉTODOS: Este é um estudo prospectivo controlado e randomizado. Foram avaliados pacientes com mais de 18 anos de idade que se submeteram à colonoscopia eletiva em nossa clínica. Os pacientes foram divididos em dois grupos de acordo com o uso do espartilho. As variáveis foram comparadas entre os grupos. RESULTADOS: O total de 204 pacientes foram incluídos no estudo. Os espartilhos foram usados em 97 pacientes e não foram usados em 107 pacientes. Verificou-se que a necessidade de compressão manual diminuiu no grupo que usou o espartilho. Não houve efeito do uso do espartilho no tempo de intubação cecal na população em geral. Verificou-se que o tempo de intubação cecal diminuiu com o uso do espartilho em pacientes com IMC<30 e do sexo masculino. CONCLUSÕES: A necessidade de compressão manual pode ser reduzida com o uso de um espartilho abdominal durante a colonoscopia. O uso de um espartilho abdominal pode tornar o procedimento de colonoscopia mais rápido e fácil para o endoscopista e o paciente.

ABSTRACT BACKGROUND: Colonoscopy is a widely used endoscopic procedure to investigate diseases of the colon and rectum. Colonoscopy procedure has difficulties for the patient and endoscopist. AIMS: To investigate whether the use of an abdominal corset can make the colonoscopy procedure easier and faster. METHODS: This is a prospective randomised controlled study. Patients over 18 years of age who underwent elective colonoscopy in our clinic were evaluated. Patients were divided into two groups according to the use of the corset. Variables were compared between the groups. RESULTS: A total of 204 patients were included in the study. Corsets were used in 97 patients and not used in 107 patients. The need for manual compression was found to be decreased in the corset use group. There was no effect of corset use on cecal intubation time in the general population. It was found that cecal intubation time decreased with corset use in patients with body mass index - BMI<30 and male gender. CONCLUSIONS: The need for manual compression can be reduced by the use of an abdominal corset during colonoscopy. The use of an abdominal corset may make the colonoscopy procedure faster and easier for the endoscopist and the patient.

RESUMO RACIONAL: A pancreaticoduodenectomia (PD) é uma intervenção importante na cirurgia digestiva. Embora sua mortalidade seja atualmente baixa em centros experientes, a morbidade permanece alta, dominada pela fístula pancreática. OBJETIVOS: Analisar os fatores de risco para fístula pancreática pós-operatória (FPO) após pancreaticoduodenectomia. MÉTODOS: Foi realizado um estudo retrospectivo no Departamento de Cirurgia Geral do Hospital Universitário Habib Thameur, em Túnis, durante 12 anos (2010–2021). Todos os pacientes submetidos à pancreaticoduodenectomia foram incluídos, independentemente da indicação. RESULTADOS: Nossa série incluiu 50 pacientes, sendo 27 homens e 23 mulheres. A taxa de fístula pancreática foi de 32% (16 pacientes) com um tempo médio de início de 5 dias (1-12 dias). Foi um vazamento bioquímico (grau A) em 1 paciente (2%), fístula pancreática grau B em 5 pacientes (10%) e grau C em 10 pacientes (20%). A fístula pancreática foi responsável por 10% da mortalidade pós-operatória (5 pacientes). A análise univariada demonstrou uma correlação significativa entre a fístula pancreática pós-operatória e os seguintes fatores: diâmetro do ducto pancreático principal ≤3 mm (p=0,036, p<0,05), textura macia do pâncreas (p=0,025, p<0,05), pancreaticojejunostomia por 2 suturas sobrepostas pela metade (p=0,049, p<0,05), glicemia de jejum ≤8 mmol/l (p=0,025, p<0,05). A análise multivariada mostrou que a textura macia do pâncreas foi o único fator de risco independente para fístula pancreática pós-operatória (p=0,02, p<0,05). CONCLUSÕES: A textura macia do pâncreas é o único fator de risco independente para FPO. Ainda são necessários estudos prospectivos e randomizados para determinar com precisão os verdadeiros fatores de risco para fístula pancreática após a PD.

ABSTRACT BACKGROUND: Pancreaticoduodenectomy (PD) is a major intervention in digestive surgery. Although its mortality is currently low in experienced centers, morbidity remains high, dominated by a pancreatic fistula. AIMS: The aim of this study was to analyze the risk factors for postoperative pancreatic fistula (POPF) after PD. METHODS: A retrospective study was conducted at the General Surgery Department of Habib Thameur University Hospital in Tunis for 12 years (2010–2021). All patients who underwent PD were included regardless of indications. RESULTS: Our series comprised 50 patients, consisting of 27 men and 23 women. The rate of a pancreatic fistula was 32% (16 patients) with an average time of onset of 5 days (1–12 days). It was observed as a biochemical leak (grade A) in 1 patient (2%), pancreatic fistula grade B in 5 patients (10%), and pancreatic fistula grade C in 10 patients (20%). Pancreatic fistula was responsible for 10% of postoperative mortality (five patients). Univariate analysis showed a statistically significant correlation between POPF and the following factors: diameter of the main pancreatic duct ≤3 mm (p=0.036, p<0.05), soft texture of the pancreas (p=0.025, p<0.05), pancreaticojejunostomy by two semi-overlapping sutures (p=0.049, p<0.05), and fasting blood glucose level ≤8 mmol/l (p=0.025, p<0.05). Multivariate analysis showed that soft pancreatic texture was the only independent risk factor for POPF (p=0.02, p<0.05). CONCLUSION: The soft texture of the pancreas is the only independent risk factor for POPF. Prospective randomized studies are still needed to accurately determine the true risk factors for a pancreatic fistula after PD.

RESUMO RACIONAL: A doença Coronavírus 2019 (COVID-19) impactou fortemente os pacientes submetidos à cirurgia bariátrica devido às medidas prolongadas de quarentena e bloqueio. OBJETIVOS: Este estudo teve como objetivo investigar qualitativamente o impacto das medidas de quarentena e bloqueio da COVID-19 na saúde mental e emocional de mulheres pós-cirurgia bariátrica. MÉTODOS: Foi realizado um estudo qualitativo, com entrevistas individuais realizadas por meio de chamada de vídeo, utilizando um serviço de vídeo comunicação (Google Meet®). Um guia moderador com perguntas sobre três temas pré-estabelecidos com base na literatura foi realizado: saúde mental e emocional; relacionamento social; e uso de tecnologia em saúde. RESULTADOS: Participaram deste estudo 12 mulheres, com idade média de 43±9,83 anos, massa corporal de 82,33±13,83 quilogramas, altura de 1,62±0,06 metros, índice de massa corporal de 26,32±2,97 kg/m2 e tempo pós-operatório de 12,83±4,37 meses. As entrevistas tiveram duração média de 50,71±7,26 minutos. Nossos resultados sugeriram um impacto negativo da pandemia de COVID-19 em aspectos da saúde mental e emocional, como aumento da ansiedade, sintomas depressivos, medo, estresse e angústia, que foram de alguma forma diminuídos em pacientes mais próximos de familiares. A cirurgia bariátrica foi citada como aspecto positivo pelos pacientes para o enfrentamento das condições clínicas de risco. CONCLUSÕES: O estudo mostrou um impacto negativo da pandemia de COVID-19 em aspectos da saúde mental e emocional, principalmente devido a medidas de confinamento que levaram ao isolamento social e a um aumento da carga de tarefas domésticas.

ABSTRACT BACKGROUND: The coronavirus disease 2019 (COVID-19) greatly impacted patients undergoing bariatric surgery due to prolonged quarantine and lockdown measures. AIMS: The aim of this study was to qualitatively investigate the impact of the COVID-19 quarantine and lockdown measures on the mental and emotional health of post-bariatric surgery women. METHODS: A qualitative study was carried out, with individual interviews conducted via video calls using a video-communication service (Google Meet®). The moderator guide inquired about three pre-established topics based on the literature: mental and emotional health, social relationship, and the use of health technology. RESULTS: A total of 12 women participated in this study, with an average age of 43±9.83 years, a body mass of 82.33±13.83 kg, a height of 1.62±0.06 m, a body mass index of 26.32±2.97 kg/m2, and post-surgery time of 12.83±4.37 months. The interviews had an average duration of 50.71±7.26 min. Our results suggested a negative impact of the COVID-19 pandemic on aspects of mental and emotional health, such as increased anxiety, depressive symptoms, fear, stress, and anguish, which were somehow diminished in patients who were closer to family members. Bariatric surgery was mentioned as a positive aspect by the patients for coping with clinical risk conditions. CONCLUSIONS: The study showed a negative impact of the COVID-19 pandemic on aspects of mental and emotional health mostly due to lockdown measures, which led to social isolation and an increased burden with household chores.

RESUMO RACIONAL: A análise microscópica de brotamento peri-tumoral (BT) pode ser uma importante ferramenta preditiva de metástases linfonodais regionais em casos de câncer colorretal, principalmente entre pacientes em estágios intermediários, os quais apresentam marcante variabilidade prognóstica. OBJETIVOS: Avaliar o poder preditivo do BT em relação à presença de metástases linfonodais, bem como sua associação com outras características relacionadas à progressão do carcinoma colorretal. MÉTODOS: Trata-se de estudo transversal, retrospectivo com abordagem quantitativa, com revisão de prontuários médicos e laudos anatomopatológicos de pacientes submetidos a cirurgia oncológica por câncer colorretal. RESULTADOS: Foram analisados 153 prontuários de pacientes, predominando a faixa etária de 61-70 anos, com uma maioria masculina (50,98%). O adenocarcinoma sem especificação foi o tipo histológico mais comum (60,78%), com a maioria mostrando moderada diferenciação (87,58%). Da amostra total, 97 casos (63,39%) apresentaram BT, com 51,55% classificados como alto escore de brotamento. A invasão em tecido adiposo/subserosa foi a mais prevalente, ocorrendo em 46,41% dos casos. Metástases em linfonodos regionais e embolização angiolinfática foram observadas em 66 e 101 pacientes, respectivamente. A análise cruzada indicou uma associação estatisticamente significativa entre BT e metástase linfonodal (p<0,05). CONCLUSÕES: A relação entre BT e metástase linfonodal destaca a importância deste fator histológico na estratificação de risco e prognóstico em pacientes com câncer colorretal, complementando o estadiamento TNM. Portanto, a avaliação do brotamento tumoral é crucial em laudos anatomopatológicos, podendo influenciar decisões terapêuticas adicionais.

ABSTRACT BACKGROUND: Microscopic analysis of tumor budding (TB) may be an essential predictive tool for regional lymph node metastases in colorectal cancer, especially among patients in intermediate stages, who exhibit considerable prognostic variability. AIMS: The aim of this study was to assess the predictive power of BT regarding the presence of lymph node metastases and its association with other characteristics related to colorectal carcinoma progression. METHODS: This is a cross-sectional, retrospective study with a quantitative approach, focusing on the review of medical records and histopathological reports of patients who underwent oncologic surgery for colorectal cancer. RESULTS: A total of 153 patient records were examined, with a predominance of the 61-70 age group and a male majority (50.98%). Adenocarcinoma not otherwise specified was the most common histological type (60.78%), with the majority exhibiting moderate differentiation (87.58%). From the total sample, 97 cases (63.39%) exhibited TB, with 51.55% classified as a high budding score. Invasion of adipose tissue/subserosa was the most prevalent, occurring in 46.41% of cases. Regional lymph node metastases and angiolymphatic invasion were observed in 66 and 101 patients, respectively. Cross-tabulation analysis showed a statistically significant association between TB and lymph node metastasis (p<0.05). CONCLUSIONS: The relationship between TB and lymph node metastasis highlights the significance of this histological factor in the risk stratification and prognosis of patients with colorectal cancer, complementing TNM staging. Therefore, the assessment of tumor budding is crucial in histopathological reports, potentially influencing additional therapeutic decisions.

RESUMO RACIONAL: Fístula biliar é uma das complicações mais comuns após ressecção hepática e é associado a significativa morbimortalidade. Um dos métodos empregados para avaliar fístulas biliares é o white test, que consiste na injeção de uma emulsão lipídica na via biliar. No entanto, nenhuma técnica padrão para realização do white test foi publicada. OBJETIVOS: Padronização da técnica para realização do white test em pacientes submetidos a hepatectomias, com e sem colecistectomia prévia e os resultados preliminares. MÉTODOS: Paciente acima de 18 anos e submetidos a hepatectomia aberta foram incluídos na pesquisa. O desfecho primário foi a taxa de fístula biliar. Desfechos secundários foram a incidência de pancreatite aguda e morbidade geral, medida pela classificação de Clavien-Dindo. RESULTADOS: A técnica padrão para o white test foi realizada em 17 pacientes. Três pacientes tinham colecistectomia prévia e 2 tinham inserção baixa do ducto cístico, necessitando canulação do ducto hepatocolédoco. Nenhum dos pacientes desenvolveu fístula biliar clinicamente significativa. Pancreatite aguda não ocorreu em nenhum paciente. Um paciente desenvolveu pneumonia necessitando ventilação mecânica (Clavien-Dindo IV). Todos os outros tiveram complicações menores ou não tiveram complicações. CONCLUSÕES: A técnica padronizada para realização do white test sugere uma estratégia adequada para maximizar a detecção de vazamentos biliares no intraoperatório.

ABSTRACT BACKGROUND: Biliary fistula is one of the most common complications after liver resection and is associated with significant morbidity and mortality. One of the methods used to evaluate biliary fistulas is the White test, which consists of injecting a lipid emulsion into the bile duct. However, no standard technique for performing the White test has been published. AIMS: The aim of this study was to standardize the technique for performing the White test in patients undergoing hepatectomies, with and without previous cholecystectomy, and to assess the preliminary results. METHODS: Patients over 18 years of age who were submitted to open hepatectomy were included in the study. The primary outcome was the rate of biliary fistula. Secondary outcomes were the incidence of acute pancreatitis and overall morbidity, measured by the Clavien-Dindo classification. RESULTS: The standard technique for the White test was performed on 17 patients. In total, three patients had previous cholecystectomy, and two had low insertion of the cystic duct, requiring cannulation of the hepatocholedochal duct. None of the patients developed clinically significant biliary leaks. Acute pancreatitis did not occur in any patient. One patient developed pneumonia requiring mechanical ventilation (Clavien-Dindo IV). All others had minor or no complications. CONCLUSIONS: The standardized technique for performing the White test suggests an appropriate strategy to maximize the detection of intraoperative biliary leaks.

RESUMO Os óbitos relacionados ao câncer colorretal são geralmente associadas às suas metástases que afetam o fígado (50%), por via hematogênica. Aproximadamente 20-25% desses pacientes já apresentam metástases sincrônicas no fígado no momento do diagnóstico do tumor primário. Em outros, as metástases hepáticas ocorrerão durante o curso da doença e são chamadas de metacrônicas. Acredita-se que as metástases metacrônicas tenham um melhor prognóstico, no entanto, 20%-25% dos casos metastáticos podem ser ressecados durante o curso da doença. Não há consenso sobre o intervalo de tempo diagnóstico para que uma metástase seja considerada metacrônica na literatura consultada. O tratamento cirúrgico das metástases e dos linfonodos é indicado, e a doença neoplásica extra-hepática deve ser cuidadosamente avaliada. O transplante de fígado pode beneficiar o paciente, deve ser avaliado e é indicado em algumas situações especiais.

ABSTRACT Deaths related to colorectal cancer are generally associated with its metastases that affect the liver (50%) through the hematogenous route. Approximately 20-25% of these patients already have synchronous metastases in the liver at the time of primary tumor diagnosis. In others, liver metastases will occur during the course of the disease and are called metachronous. Metachronous metastases are believed to have a better prognosis; however, 20-25% of metastatic cases can be resected during the course of the disease. There is a lack of consensus on the diagnostic time interval for metastases to be considered metachronous in the consulted literature. Surgical treatment of metastases and lymph nodes is indicated, and extrahepatic neoplastic disease must be carefully evaluated. Liver transplantation can benefit the patient, should be evaluated, and is indicated in some special situations.

RESUMO RACIONAL: Cirurgias estão associadas a um alto risco de morbidade e mortalidade quando realizadas em pacientes críticos. OBJETIVOS: investigar os fatores de risco associados a resultados adversos em uma coorte de pacientes admitidos em unidade de terapia intensiva (UTI) de um único centro de referência após cirurgia abdominal. MÉTODOS: Pacientes admitidos em UTI cirúrgica entre janeiro 2016 e dezembro 2022 foram avaliados retrospectivamente. Dados referentes a variáveis demográficas, clínicas e perioperatórias foram comparados à mortalidade hospitalar. RESULTADOS: Foram avaliados 1.717 pacientes (1.096 mulheres, idade 61+17 anos). A maioria dos pacientes foi submetida a cirurgia colorretal (n=499), pancreática (n=148), biliar (n=147), gástrica (n=145), hepática (n=131) e vários procedimentos ginecológicos/obstétricos (n=250). Um total de 52,3% desses procedimentos cirúrgicos foram eletivos. Os escores médios do Índice de Comorbidade de Charlson (CCI) e do APACHE II foram 4,4±2,8 e 10,1±5,6, respectivamente. Mortalidade foi observada em 158 (9,2%) pacientes. Idade (70,4±14,3 vs. 60,6±17,1 anos nos sobreviventes, p=0,002), CCI (6,1±2,5 vs. 4,3±2,8 nos sobreviventes, p=0,005), tipo de cirurgia (13,6% cirurgias emergenciais ou urgentes vs. 5,5% cirurgias eletivas, p<0,001) e APACHE II (16,7±8,4 vs. 9,4±4,7 nos sobreviventes, p=0,001) foram associados à mortalidade na análise univariada, mas somente o índice de comorbidade de Charlson, o tipo de cirurgia e APACHE II foram independentemente correlacionados a um maior risco de morte na análise multivariada. CONCLUSÕES: Atualmente, a mortalidade após cirurgia abdominal em pacientes que necessitam de suporte pós-operatório em UTI é inferior a 10%, sendo independentemente associada a cirurgias urgentes/emergenciais, gravidade da doença e comorbidades.

ABSTRACT BACKGROUND: Surgery is associated with a high risk for morbidity and mortality, particularly when performed in critical patients requiring intensive care unit (ICU) admission. AIM: The aim of this study was to investigate risk factors associated with adverse outcomes in a large cohort of patients admitted to a single-center ICU after abdominal surgery. METHODS: All patients admitted to a surgical ICU for postoperative care, from January 2016 to December 2022, were retrospectively evaluated. Data concerning demographics and clinical and perioperative variables were compared to in-hospital mortality. RESULTS: A total of 1,717 patients (1,096 women, mean age: 61±17 years) were evaluated. Most of the patients underwent colorectal (n=499), pancreatic (n=148), biliary tract (n=147), and gastric surgeries (n=145); liver resection (n=131); and several gynecological or obstetric procedures (n=250). Only 52.3% of these surgical procedures were elective. The mean Charlson Comorbidity Index (CCI) and Acute Physiology and Chronic Health Evaluation II (APACHE II) scores were 4.4±2.8 and 10.1±5.6, respectively. Mortality was observed in 158 (9.2%) patients. Age (70.4±14.3 vs. 60.6±17.1 years in survivors, p=0.002), CCI (6.1±2.5 vs. 4.3±2.8 in survivors, p=0.005), type of surgery (13.6% in emergent/urgent vs. 5.5% in elective surgeries, p<0.001), and APACHE II score (16.7±8.4 vs. 9.4±4.7 in survivors, p<0.0001) were associated with mortality on univariate analysis, but only CCI, type of surgery, and APACHE II score were independently correlated with a higher risk of death on multivariate analysis. CONCLUSIONS: Mortality after abdominal surgery in patients requiring postoperative ICU support is less than 10% nowadays, and it is independently associated with urgent or emergent surgeries, disease severity, and comorbidity.

RESUMO RACIONAL: A obesidade representa um estado pró-inflamatório crônico que acelera a aterosclerose e o envelhecimento celular. Além da proteína C-reativa (PCR) e da ferritina, outros marcadores têm ganhado atenção, como a razão neutrófilos/linfócitos (RNL) e a razão plaquetas/linfócitos (RPL), que são relacionados com o grau de inflamação sistêmica em várias patologias, incluindo a obesidade e suas comorbidades. OBJETIVOS: Comparar os valores de RNL e RPL antes e depois da gastrectomia vertical (GV). MÉTODOS: Estudo retrospectivo que incluiu 622 pacientes portadores de obesidade submetidos à GV em nosso centro. Dados relacionados à presença de comorbidades, incluindo diabetes tipo 2 (DT2), hipertensão arterial sistêmica (HAS) e doença hepática gordurosa não alcoólica (DHGNA), variações de peso e índice de massa corporal (IMC) e marcadores bioquímicos de inflamação sistêmica, incluindo RNL, RPL e PCR, foram coletados. Os valores da RNL e RPL foram correlacionados com a perda de peso e o prognóstico das comorbidades no período pós-operatório. RESULTADOS: A amostra foi predominantemente feminina (79,3%), com idade média de 36.91±10.04 anos, com a presença das seguintes comorbidades: HAS (25,1%), DT2 (8,0%), e DHGNA (80,1%). Pacientes com HAS apresentaram redução nos valores de RNL e PCR após o procedimento, enquanto os pacientes com DT2 apresentaram redução de PCR e aumento da RPL. Não houve correlação das variações do IMC com a RNL, porém houve correlação significativa com a RPL. Após a cirurgia, a RNL reduziu e a RPL aumentou nos pacientes com DHGNA. CONCLUSÕES: Pacientes com obesidade apresentam redução significativa da RNL e aumento da RPL após a realização da GV.

ABSTRACT BACKGROUND: Obesity represents a chronic pro-inflammatory status that contributes to accelerated atherosclerosis and cell aging. Besides the widely used C-reactive protein and ferritin, other inflammatory markers have gained attention, such as neutrophil-to-lymphocyte ratio (NLR) and platelet-to-lymphocyte ratio (PLR), which are related with the degree of inflammation in various pathological conditions, including obesity and its comorbidities. AIMS: To compare and monitor the levels of NLR and PLR before and after sleeve gastrectomy (SG). METHODS: Retrospective study that included a total of 622 patients with obesity who underwent SG as primer bariatric surgery in our center. Data regarding the presence of comorbidities, including type 2 diabetes (T2D), high blood pressure (HBP) and non-alcoholic fatty liver disease (NAFLD), variations in body weight and body mass index (BMI), and biochemical markers of inflammation, including neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR) and C-reactive protein (CRP) were gathered. Values of NLR and PLR were correlated with weight loss and prognosis of comorbidities within the postoperative period. RESULTS: The sample was predominantly female (79.3%) with average age 36.91±10.04 years, with comorbidities including HBP (25.1%), T2D (8.0%), and NAFLD (80.1%). Patients with HBP showed reduced NLR and CRP post-intervention, while those with T2D experienced decreased CRP but increased PLR. Correlation analysis found no significant correlation between BMI/weight changes and NLR but significant correlation with PLR. Post-surgery, NLR decreased for previously NAFLD patients, and PLR increased. CONCLUSIONS: According to the results, patients with obesity present a significant decrease in NLR and an increase in PLR after SG.

RESUMO RACIONAL: Os temperamentos afetivos fazem parte do espectro dos transtornos do humor e compreendem os conceitos de hipertimia, distimia e ciclotimia. Vários estudos demonstraram uma forte relação entre obesidade e transtornos do humor. OBJETIVOS: O objetivo do presente estudo foi avaliar a frequência de temperamentos afetivos em indivíduos com obesidade mórbida e controles e estabelecer uma possível associação entre temperamentos afetivos e obesidade mórbida. MÉTODOS: O estudo avaliou 106 casos (obesos mórbidos) e 100 controles (não obesos). Para avaliar os temperamentos afetivos foi aplicada a escala TEMPS-Rio de Janeiro. Os sintomas depressivos foram avaliados pela Escala de Avaliação de Depressão de Hamilton, os sintomas de ansiedade pela Escala de Avaliação de Ansiedade de Hamilton e os sintomas maníacos pela Escala de Avaliação de Mania de Young. Para análise univariada e multivariada foram ajustados modelos de Regressão Logística. RESULTADOS: A presença de pelo menos um temperamento afetivo foi de 74,5% no grupo de obesos mórbidos e de 63% no grupo de não obesos. Na comparação entre os dois grupos, a análise estatística do subgrupo etário dos indivíduos com 50 anos ou mais mostrou uma razão de chances de 2,56 (1,07-6,09) para o temperamento hipertímico. CONCLUSÕES: Na faixa etária de 50 anos ou mais, os casos de obesidade mórbida são significativamente mais prováveis (2,56 vezes) de ocorrer em indivíduos com temperamento hipertímico. Entre os três tipos de temperamentos afetivos avaliados, apenas a hipertimia poderia ser fator de risco para obesidade mórbida.

ABSTRACT BACKGROUND: Affective temperaments are part of the spectrum of mood disorders and comprise the concepts of hyperthymia, dysthymia and cyclothymia. Numerous studies have demonstrated a strong relationship between obesity and mood disorders. AIMS: The objective of the present study was to evaluate the frequency of affective temperaments in morbidly obese individuals and controls and to establish a possible association between affective temperaments and morbid obesity. METHODS: The study evaluated 106 cases (morbidly obese) and one hundred controls (non-obese). To assess affective temperaments, the Temperament Evaluation in Memphis Pisa and San Diego - Rio de Janeiro TEMPS-Rio de Janeiro scale was applied. Depressive symptoms were assessed using the Hamilton Depression Rating Scale, anxiety symptoms using the Hamilton Anxiety Rating Scale and manic symptoms using the Young Mania Rating Scale. For univariate and multivariate analysis, logistic regression models were adjusted. RESULTS: The presence of at least one affective temperament was 74.5% in the morbidly obese group and 63% in the non-obese group. When comparing the two groups, the statistical analysis of the age subgroup of individuals aged 50 years or over showed an odds ratio of 2.56 (1.07-6.09) for hyperthymic temperament. CONCLUSIONS: In the age group of 50 years or more, cases of morbid obesity are significantly more likely (2.56 times) to occur in individuals with a hyperthymic temperament. Among the three types of affective temperaments evaluated, only hyperthymia could be a risk factor for morbid obesity.

RESUMO RACIONAL: Atualmente emergem em nosso meio aplicações mais relevantes dos derivados perinatais, por exemplo a membrana amniótica (MA), como fonte de biomateriais para emprego em diferentes processos cicatriciais. O estudo de cicatrização de anastomoses associadas a drogas antimetabólicas como o 5-fluorouracil (5- FU) é um potencial alvo da MA. OBJETIVOS: Avaliar os efeitos cicatriciais do uso da MA em ratos tratados com 5-FU na dose de 20 mg/kg ao sétimo dia de evolução pós-operatória quanto aos parâmetros porcentagem de colágeno tipo I (maduro), viabilidade celular, densidade microvascular e formação do tecido de granulação. MÉTODOS: Utilizaram-se 32 ratos da linhagem Wistar submetidos a colotomia e colorrafia separados em quatro grupos com oito, que receberam diferentes tratamentos diariamente por via intraperitoneal até o dia do sacrifício: solução fisiológica (C), 20 mg/kg 5-FU, 20 mg/kg 5-FU e MA. RESULTADOS: O tratamento com 20 mg/kg de 5-FU, ao sétimo dia pós-operatório, induziu efeitos adversos no processo cicatricial anastomótico evidenciados pela diminuição da porcentagem de colágeno tipo I (maduro), da viabilidade celular, da densidade microvascular, da formação de crosta fibrinoleucocitária e da proliferação angiofibroblástica; o uso da MA nessas condições induziu melhora da porcentagem de colágeno tipo I (maduro). CONCLUSÕES: O tratamento com 20 mg/kg de 5-FU, ao sétimo dia pós-operatório, induziu efeitos adversos no processo cicatricial anastomótico e o uso da MA nessas condições induziu melhora da porcentagem de colágeno tipo I (maduro).

ABSTRACT BACKGROUND: Nowdays, more relevant applications of perinatal derivatives, such as amniotic membrane (AM), are emerging in our environment as a source of biomaterials for use in different healing processes. The study of anastomosis healing associated with antimetabolic drugs such as 5-fluorouracil (5-FU) is a potential target of AM. AIMS: To evaluate the healing effects of AM in rats treated with 5-FU at a dose of 20 mg/kg on the seventh day of postoperative evolution, regarding the parameters percentage of type I collagen (mature), cell viability, microvascular density and formation of granulation tissue. METHODS: Thirty-two Wistar rats were used, submitted to colotomy and colorraphy, separated into four groups of eight, which received different treatments daily, intraperitoneally, until the day of sacrifice: saline solution (C), 20 mg/kg 5-FU, 20 mg/kg 5-FU and AM. RESULTS: Treatment with 20 mg/kg of 5-FU, on the seventh postoperative day, induced adverse effects on the anastomotic healing process, evidenced by a decrease in the percentage of type I (mature) collagen, cell viability, microvascular density, fibrin-leukocyte scab formation and angiofibroblast proliferation; the use of AM under these conditions induced an improvement in the percentage of type I (mature) collagen. CONCLUSIONS: Treatment with 20 mg/kg of 5-FU on the seventh postoperative day induced adverse effects on the anastomotic healing process, and the use of AM under these conditions induced an improvement in the percentage of type I (mature) collagen.

RESUMO RACIONAL: O volume do Hospital, especialização, disponibilidade serviços oncológicos e experiência na realização de cirurgias complexas têm impacto favorável no tratamento do câncer gástrico (CG). OBJETIVOS: Comparar resultados do tratamento do CG de acordo com o tipo de Hospital oncológico no Estado de São Paulo. MÉTODOS: Pacientes diagnosticados com CG entre 2000 e 2022 foram avaliados por dados extraídos do Registro Hospitalar de Câncer, de acordo com o tipo de Unidade de tratamento: Unidade de Assistência de Alta Complexidade Oncológica (UNACON) e Centro de Assistência de Alta Complexidade Oncológica (CACON), que tem maior complexidade. RESULTADOS: Entre os 33.774 pacientes, 23.387 (69,2%) foram tratados em CACONs e 10.387 (30,8%) em UNACONs. Pacientes no CACON eram mais jovens, tinham maior nível de escolaridade e cTNM mais avançado em comparação ao UNACON (todos p<0,001). O tempo do diagnóstico ao tratamento foi superior a 60 dias em 49,8% dos pacientes do CACON e 39,4% dos do UNACON (p<0,001). O tratamento cirúrgico foi realizado em 18.314 (54,2%) pacientes. A frequência de pN0 (40,3 vs 32,4%) e pTNM I (23 vs 19,5%) foi maior no CACON. Não houve diferença na sobrevida global (SG) entre todos os casos de adenocarcinoma tratados no CACON e no UNACON (9,3 vs 10,3 meses, p=0,462, p>0.05). No entanto, considerando apenas os pacientes submetidos à cirurgia, a SG daqueles tratados no CACON foi melhor (24,4 vs 18 meses, p<0,001, p<0.05). CONCLUSÕES: Pacientes com GC submetidos à gastrectomia em CACONs apresentaram melhores resultados de sobrevida, sugerindo que a centralização de cirurgias oncológicas complexas pode ser benéfica.

ABSTRACT BACKGROUND: The hospitals’ volume, specialization, availability of all oncological services, and experience in performing complex surgeries have a favorable impact on gastric cancer (GC) treatment. AIMS: The aim of this study was to compare the results of GC treatment according to the type of oncological hospital in the State of São Paulo. METHODS: Patients diagnosed with GC between 2000 and 2022 in qualified hospitals for cancer treatment were evaluated by data extracted from the hospital cancer registry. Patients were assessed according to the type of hospital for cancer treatment: Oncology High Complexity Assistance Unit (UNACON) and Oncology High Complexity Care Center (CACON), which has greater complexity. RESULTS: Among the 33,774 patients, 23,387 (69.2%) were treated at CACONs and 10,387 (30.8%) in UNACONs. CACON patients were younger, had a higher level of education, and had a more advanced cTNM stage compared to UNACON (all p<0.001, p<0.05). The time from diagnosis to treatment was over 60 days in 49.8% of CACON’s patients and 39.4% of UNACON’s (p<0.001, p<0.05). Surgical treatment was performed in 18,314 (54.2%) patients. The frequency pN0 (40.3 vs 32.4%) and pTNM stage I (23 vs 19.5%) were higher in CACON. There was no difference in overall survival (OS) between all adenocarcinoma cases treated at CACON and UNACON (9.3 vs 10.3 months, p=0.462, p>0.05). However, considering only patients who underwent curative surgery, the OS of patients treated at CACON was better (24.4 vs 18 months, p<0.001). CONCLUSIONS: Patients with GC who underwent gastrectomy at CACONs had better survival outcomes, suggesting that the centralization of complex cancer surgery may be beneficial.
