RESUMO Objetivo: O objetivo principal desse estudo foi comparar a saúde gengival após o uso de dois tipos de contenção: uma feita com fio redondo multifilamentado e outra feita com fio retangular Ortho-Flex-Tech™. Os objetivos secundários foram avaliar o acúmulo de placa/cálculo, determinar a eficácia dessas contenções em manter o alinhamento dos dentes, bem como sua taxa de falha. Material e Métodos: Esse foi um estudo unicêntrico do tipo ensaio clínico randomizado paralelo de dois braços, conduzido nas clínicas ortodônticas do Centro de Ensino Odontológico da Jordan University of Science and Technology (Jordânia). Foram selecionados aleatoriamente sessenta pacientes com contenção colada no segmento anterior inferior após tratamento ortodôntico fixo. A amostra foi composta por pacientes caucasianos com apinhamento pré-tratamento leve a moderado na região anterior inferior, relação de Classe I, tratados sem extração de dentes anteriores inferiores. Além disso, foram incluídos apenas os pacientes que apresentavam sobressaliência e sobremordida normais após o tratamento. Intervenção: Um grupo recebeu contenção com fio redondo multifilamentado (30 pacientes, idade média: 19,7 ± 3,8 anos), enquanto o outro grupo recebeu contenção com fio retangular Ortho-Flex-Tech™ (30 pacientes; idade média: 19,3 ± 3,2 anos). Em ambos os grupos, as contenções foram coladas em todos os dentes anteriores inferiores de canino a canino. Todos os pacientes retornaram um ano após a descolagem dos braquetes. A sequência de randomização foi criada no Excel 2010, com uma alocação de 1:1, usando tamanho de bloco aleatório 4. A sequência de alocação foi ocultada em envelopes numerados sequencialmente, opacos e lacrados. Apenas os participantes desconheciam o tipo de contenção usada. O resultado primário foi a comparação dos dois grupos quanto à condição gengival. Os resultados secundários foram a avaliação dos índices de placa/cálculo, índice de irregularidade dos dentes anteriores inferiores e taxa de falha das contenções. As comparações foram realizadas usando o teste U de Mann-Whitney ou o teste qui-quadrado. A significância estatística foi predeterminada ao nível de p≤ 0,05 para todos os testes. Resultados: Dados completos foram coletados para 46 pacientes (grupo de contenção com fio multifilamentado redondo, n=24 pacientes; grupo de contenção com fio retangular Ortho-Flex-Tech™, n=22 pacientes). Nenhuma diferença significativa foi encontrada entre os dois grupos para os parâmetros de saúde gengival (p>0,05). As contenções Ortho-Flex-Tech™ mantiveram mais o alinhamento dos dentes anteriores inferiores do que as contenções com fio multifilamentado (p<0,05). Nenhuma diferença significativa foi encontrada entre os dois grupos quanto à taxa de falha (p>0,05). Conclusões: Os parâmetros de saúde gengival e a taxa de falhas não foram diferentes entre os dois grupos. No entanto, as contenções Ortho-Flex-Tech™ foram mais eficientes em conter os incisivos inferiores do que as contenções com fio multifilamentado; no entanto, essa diferença não foi clinicamente significativa.

ABSTRACT Objective: The primary objective was to compare round multi-strand wire and Ortho-Flex-Tech™ rectangular wire retainers in terms of gingival health. The secondary objectives were to assess plaque/calculus accumulation, and to determine the effectiveness of these retainers in maintaining tooth alignment and their failure rate. Material and Methods: This single-center study was a two-arm parallel randomized clinical trial and was conducted at the Orthodontic clinics in Dental Teaching Center/Jordan University of Science and Technology. Sixty patients, with bonded retention for the mandibular anterior segment after fixed orthodontic treatment, were randomly selected. The sample comprised Caucasian patients with mild to moderate pretreatment crowding in the mandibular anterior region, Class I relationship, treated without extraction of mandibular anterior tooth. In addition, only patients presenting normal overjet and overbite after treatment were included. Intervention: One group received round multi-strand wire retainer (30 patients, average age: 19.7 ± 3.8 years), while the other group received Ortho-Flex-Tech™ retainer (30 patients; average age: 19.3 ± 3.2 years). In both groups, the retainers were bonded to all mandibular anterior teeth from canine to canine. All patients were recalled one year after bracket debonding. Randomization sequence was created using Excel 2010, with a 1:1 allocation, using random block size 4. The allocation sequence was concealed in sequentially numbered, opaque and sealed envelopes. Only participants were blinded to the type of bonded retainer used. The primary outcome was to compare the gingival condition between the two groups. The secondary outcomes were to assess plaque/calculus indices, irregularity index of the mandibular anterior teeth and retainers’ failure rate. Comparisons were conducted using Mann-Whitney U test or chi-square test. Statistical significance was predetermined at the p≤ 0.05 level for all tests. Results: Complete data were collected for 46 patients (round multi-strand wire retainer group, n=24 patients; rectangular Ortho-Flex-Tech™ retainer group, n=22 patients). No significant differences were found in the gingival health parameters between the two groups (p>0.05). Ortho-Flex-Tech™ retainers maintained the alignment of mandibular anterior teeth more than multi-strand retainer (p<0.05). No significant difference was found in the failure rate between the two groups (p>0.05). Conclusions: Gingival health parameters and failure rate were not different in both groups. However, Ortho-Flex-Tech™ retainers were more efficient to retain the mandibular incisors than the multi-strand retainers; nevertheless, the difference was not clinically significant.

RESUMO Objetivo: O objetivo desse estudo randomizado controlado paralelo de dois braços foi avaliar os efeitos do tratamento e as mudanças no perfil labial em pacientes esqueléticos Classe II submetidos a tratamento com extração de pré-molares (EP) versus tratamento funcional fixo (FF). Métodos: Quarenta e seis indivíduos que preencheram os critérios de inclusão foram distribuídos aleatoriamente em Grupo EP (idade média 13,03±1,78 anos) e Grupo FF (idade média 12,80±1,67 anos) (n=23 cada). O grupo EP foi tratado com extração dos primeiros pré-molares superiores e segundos pré-molares inferiores, seguida de fechamento do espaço com ancoragem em mini-implantes; e o Grupo FF, com tratamento usando aparelhos funcionais fixos. As alterações esqueléticas, dentárias e de tecidos moles foram analisadas usando cefalogramas laterais pré e pós-tratamento. Os dados obtidos desse estudo aberto foram submetidos a análise estatística cega. Resultados: O tratamento com extrações resultou em maior aumento do ângulo nasolabial (ANL: 3,1 [IC 95% 2,08, 4,19], p<0,001), melhora significativa do lábio superior (Ls-Linha E: -2,91 [IC 95% -3,54, -2,28], p<0,001, Ls-Linha S: -2,50 [IC 95% -2,76, -2,24], p<0,001, Ls-SnPog’: -2,32 [IC 95% -2,90, -1,74], p<0,01) e posição do lábio inferior (Li-Linha E: -0,68 [IC 95% -1,36, 0,00], p<0,01, Li-Linha S: -0,55 [IC 95% -1,11, 0,02], p<0,01, e Li-SnPog’: -0,64 [IC 95% -1,20, -0,07], p<0,01), espessura dos lábios (espessura Ls: 2,27 [IC 95% 1,79, 2,75], p<0,001; espessura Li: 0,41 [IC 95% -0,16, 0,97], p<0,01), tensão do lábio superior (tensão Ls: -2,68 [IC 95% -3,32, -2,04], p<0,001) e perfil de tecidos moles (N’-Sn-Pog’: 2,68 [IC 95% 1,87, 3,50], p<0,01). Nenhuma diferença significativa foi observada entre os grupos quanto às alterações esqueléticas na maxila e mandíbula, padrão de crescimento, sobressaliência, sobremordida, ângulo interincisal e posição dos tecidos moles do mento (p>0,05). O tratamento com extração de pré-molares demonstrou significativa intrusão-retração dos incisivos superiores, melhor manutenção da inclinação dos incisivos superiores e protração significativa dos molares inferiores; enquanto o tratamento funcional resultou em efeito retrusivo e intrusivo nos molares superiores, proclinação acentuada dos dentes anteriores inferiores e extrusão significativa dos molares inferiores. Ambas as modalidades de tratamento tiveram duração de tratamento semelhante. A falha do mini-implante foi observada em 7,9% dos casos, enquanto a falha do aparelho funcional fixo foi observada em 9,09% dos casos. Conclusões: O tratamento com extração de pré-molares é uma modalidade de tratamento melhor do que os aparelhos funcionais fixos para pacientes Classe II com discrepância esquelética moderada, sobressaliência aumentada, incisivos superiores protruídos e lábios protruídos, pois produz melhor resposta dentoalveolar e permite maior melhora do perfil dos tecidos moles e relacionamento labial.

ABSTRACT Objective: The objective of this two-arm parallel randomized controlled trial was to evaluate the treatment effects and lip profile changes in skeletal Class II patients subjected to premolars extraction treatment versus fixed functional treatment. Methods: Forty six subjects fulfilling inclusion criteria were randomly distributed into Group PE (mean age 13.03±1.78 years) and Group FF (mean age 12.80±1.67 years) (n=23 each). Group PE was managed by therapeutic extraction of maxillary first premolars and mandibular second premolars, followed by mini-implant-supported space closure; and Group FF, by fixed functional appliance therapy. Skeletal, dental, and soft-tissue changes were analyzed using pre and post-treatment lateral cephalograms. Data obtained from this open label study was subjected to blind statistical analysis. Results: Extraction treatment resulted in greater increase of nasolabial angle (NLA: 3.1 [95% CI 2.08, 4.19], p<0.001), significant improvement of upper lip (UL-E line: -2.91 [95% CI -3.54, -2.28], p<0.001, UL-S line: -2.50 [95% CI -2.76, -2.24], p<0.001, UL-SnPog’: -2.32 [95% CI -2.90, -1.74], p<0.01) and lower lip position (LL-E line: -0.68 [95% CI -1.36, 0.00], p<0.01, LL-S line: -0.55 [95% CI -1.11, 0.02], p<0.01, and LL-SnPog’: -0.64 [95% CI -1.20, -0.07], p<0.01), lip thickness (UL thickness: 2.27 [95% CI 1.79, 2.75], p<0.001; LL thickness: 0.41 [95% CI -0.16, 0.97], p<0.01), upper lip strain (UL strain: -2.68 [95% CI -3.32, -2.04], p<0.001) and soft tissue profile (N’-Sn-Pog’: 2.68 [95% CI 1.87, 3.50], p<0.01). No significant difference was observed between the groups regarding skeletal changes in the maxilla and mandible, growth pattern, overjet, overbite, interincisal angle and soft tissue chin position (p>0.05). Premolar extraction treatment demonstrated significant intrusion-retraction of maxillary incisors, better maintenance of maxillary incisor inclination, and significant mandibular molar protraction; whereas functional treatment resulted in retrusive and intrusive effect on maxillary molars, marked proclination of mandibular anterior teeth, and significant extrusion of mandibular molars. Both treatment modalities had similar treatment duration. Implant failure was seen in 7.9% of cases, whereas failure of fixed functional appliance was observed in 9.09% of cases. Conclusions: Premolar extraction therapy is a better treatment modality, compared to fixed functional appliance therapy for Class II patients with moderate skeletal discrepancy, increased overjet, protruded maxillary incisors and protruded lips, as it produces better dentoalveolar response and permits greater improvement of the soft tissue profile and lip relationship.

RESUMO Introdução: A má oclusão de Classe III deve ser interceptada e tratada em idade precoce, a fim de evitar uma futura necessidade de procedimentos complexos e invasivos. O tratamento com máscara facial ortopédica tem o objetivo de obter alterações esqueléticas, minimizando os efeitos colaterais na dentição. O uso de ancoragem óssea em mini-implantes, associada ao protocolo Alt-RAMEC (Alternate Rapid Maxillary Expansion and Constriction) pode ser eficaz no tratamento de um grande número de pacientes Classe III em crescimento. Objetivo: Realizar uma síntese da literatura baseada em evidência sobre o tratamento da má oclusão de Classe III em pacientes adultos jovens, e ilustrar sua aplicação e eficácia por meio do relato de um caso emblemático. Conclusão: A resolução e o acompanhamento em longo prazo do caso apresentado, juntamente com estudos conduzidos em uma amostra maior, demonstram a eficácia da combinação estratégica dos tratamentos ortopédico e ortodôntico usando um expansor palatal híbrido e o protocolo Alt-RAMEC para corrigir a má oclusão de Classe III em pacientes adultos.

ABSTRACT Introduction: Class III malocclusion should be intercepted and treated at early age, to prevent the necessity of future complex and expensive procedures. The orthopedic facemask therapy has the goal to achieve skeletal changes, minimizing side effects on dentition. The use of skeletal anchorage, combined with Alternate Rapid Maxillary Expansion and Constriction (Alt-RAMEC) protocol, may be effective in treating a greater number of growing Class III patients. Objective: To summarize the existing evidence-based literature on Class III malocclusion treatment in young adult patients, and to illustrate its application and effectiveness, by presenting an emblematic case report. Conclusion: The resolution of the present case, its long-term follow up, along with the studies conducted on a larger sample, demonstrate the effectiveness of the strategic combination of orthopedic and orthodontic treatments by using an hybrid rapid palatal expander and Alt-RAMEC protocol for treating Class III malocclusions in adult patients.

RESUMO Objetivo: Comparar o aumento na temperatura da câmara pulpar (ATCP) na colagem de braquetes com fotoativação usando ou não primer, em dentes hígidos versus dentes restaurados, em incisivos centrais inferiores (ICI), primeiros pré-molares superiores (1PMS) e terceiros molares inferiores (3MI). Métodos: Foram incluídos noventa dentes humanos: ICI (n=30), 1PMS (n=30) e 3MI (n=30). A colagem de braquetes com fotoativação foi realizada em dentes hígidos (n=60) ou dentes restaurados (n=30), com primer (n=60) ou sem (n=30) primer. O ATCP foi definido como a diferença entre as temperaturas inicial (T0) e máxima (T1) registradas com um termômetro durante a colagem com fotoativação. As diferenças no ATCP entre as técnicas de colagem (com primer vs. sem primer), diferentes dentes (ICIvs.1PMSvs.3MI) e condição dos dentes (hígidos vs. restaurados) foram estimadas por ANCOVA, com α=5%. Resultados: O ATCP foi significativamente maior com o uso de primer (2,05 ± 0,08oC) do que sem primer (1,65 ± 0,14oC) (p=0,02), e no ICI (2,23 ± 0,22oC) do que no 1PMS (1,56 ± 0,14oC) (p<0,01). Não houve diferença no ATCP do 3MI (1,77 ± 0,28oC) em comparação com ICI ou 1PMS (p>0,05), e não houve diferença entre dentes hígidos (1,78 ± 0,14oC) e restaurados (1,92 ± 0,08oC) (p=0,38). Não houve influência da espessura da dentina e do esmalte no ATCP (p=0,19). Conclusão: O ATCP foi maior na colagem de braquetes com fotoativação usando primer, principalmente no ICI. A colagem com fotoativação parece ser menos invasiva sem primer.

ABSTRACT Objective: To evaluate the pulp chamber temperature rise (PCTR) in light-cure bonding of brackets with and without primer, in intact and restored mandibular central incisors (M1), maxillary first premolars (Mx4), and mandibular third molars (M8). Material and Methods: Ninety human teeth were included: M1 (n=30), Mx4 (n=30), and M8 (n=30). Light-cure bonding of brackets was performed in intact (n=60) and restored (n=30) teeth, with primer (n=60) or without (n=30) primer. PCTR was defined as the difference between initial (T0) and peak temperatures (T1), recorded with a thermocouple during light-cure bonding. Differences on PCTR between bonding techniques (primer vs. no primer), teeth types (M1 vs. Mx4 vs. M8), and teeth condition (intact vs. restored) were estimated by ANCOVA, with α=5%. Results: PCTR was significantly higher with the use of primer (2.05 ± 0.08oC) than without primer (1.65 ± 0.14oC) (p=0.02), and in M1 (2.23 ± 0.22oC) compared to Mx4 (1.56 ± 0.14oC) (p<0.01). There was no difference in the PCTR in M8 (1.77 ± 0.28oC) compared to M1 or Mx4 (p>0.05), and no difference between intact (1.78 ± 0.14oC) and restored (1.92 ± 0.08oC) teeth (p=0.38). There was no influence of dentin enamel thickness in the PCTR (p=0.19). Conclusion: PCTR was higher in light-cure bonding of brackets with primer, especially in M1. Light-cure bonding seems less invasive without primer.

RESUMO Objetivo: Estudar a viabilidade do método Time trade-off (TTO) para quantificar escores de valoração da saúde em diferentes tipos de má oclusão. Material e Métodos: Neste estudo transversal, foram incluídos e entrevistados 70 pacientes ortodônticos com idade igual ou superior a 18 anos, que compareceram para tratamento/consulta. A valoração da saúde em relação à má oclusão foi avaliada por meio do método TTO e a qualidade de vida relacionada à saúde bucal foi medida com a ajuda do Questionário de Qualidade de Vida Ortognática (Orthognathic Quality of Life Questionnaire, OQLQ). A classificação da má oclusão segundo Angle foi registrada, e análises bivariadas e regressão multivariada de Poisson foram feitas para verificar qualquer associação entre os escores de valoração da saúde bucal, OQLQ e características demográficas e clínicas. Resultados: Os pacientes com má oclusão esquelética de Classe III apresentaram escores de valoração da saúde mais baixos do que aqueles com má oclusão de Classe I e Classe II (p=0,013). A regressão de Poisson mostrou que a Classe II de Angle divisão 1 (0,90, IC 0,84 a 0,97), Classe III (0,68, IC 0,59 a 0,95), má oclusão esquelética (0,79, IC 0,71 a 0,87) e os escores do OQLQ (1,0, IC 1 a 1,003) foram considerados preditores significativos dos escores de valoração pelo método TTO. Conclusões: Os escores do TTO foram considerados válidos e bem correlacionados com os achados clínicos, e podem servir como marcadores úteis e confiáveis da qualidade de vida relacionada à saúde (health-related quality of life, HRQL) entre indivíduos ou comunidades, e ajudar no planejamento de programas de prevenção ou de intervenção, com uma boa relação custo-benefício.

ABSTRACT Objective: To study the feasibility of time trade-off (TTO) method in quantifying health utility ratings in different types of malocclusion. Material and Methods: In this cross-sectional study, 70 orthodontic patients aged 18 years or above, reporting for treatment/consultation, were included and interviewed. Malocclusion-related health utilities were assessed through the TTO method, and oral health-related quality of life was measured with the help of Orthognathic Quality of Life Questionnaire (OQLQ). Angle’s classification of malocclusion was recorded. Bivariate analyses and multivariate Poisson’s regression were done to find out an association between the oral health utility values, OQLQ and demographic and clinical characteristics. Results: Patients with skeletal Class III malocclusion had lower health utility values than those with Class I and Class II malocclusions (p=0.013). Poisson’s regression showed that Angle’s Class II division 1 (0.90, CI 0.84 to 0.97), Class III (0.68, CI 0.59 to 0.95) and Skeletal malocclusion (0.79, CI 0.71 to 0.87) and OQLQ scores (1.0, CI 1 to 1.003) were found to be significant predictors of TTO utility scores. Conclusions: TTO utilities were found to be valid and well correlated with clinical findings. Health utilities could serve as useful and reliable markers of health-related quality of life (HRQL) among individuals or communities and help cost-effective preventive or intervention programs planning.

RESUMO Objetivos: Este ensaio clínico foi conduzido para avaliar a estabilidade e a taxa de falha de mini-implantes ortodônticos com superfície tratada, e determinar se elas diferem das dos mini-implantes ortodônticos sem superfície tratada. Desenho do estudo: Ensaio clínico randomizado com desenho de boca dividida. Instituição: Department of Orthodontics, SRM Dental College, Chennai/India. Participantes: Pacientes que necessitavam de mini-implantes ortodônticos para retração anterior em ambas as arcadas. Métodos: Mini-implantes ortodônticos autoperfurantes, cônicos, de titânio com ou sem tratamento de superfície, foram colocados em cada paciente, seguindo um desenho de boca dividida. Os torques máximos de inserção e de remoção foram medidos para cada mini-implante, usando um torquímetro digital. As taxas de falha foram calculadas para cada tipo de mini-implante. Resultados: O valor médio do torque máximo de inserção foi de 17,9 ± 5,6 Ncm para mini-implantes com superfície tratada e 16,4 ± 9,0 Ncm para mini-implantes sem superfície tratada. O valor médio do torque máximo de remoção foi de 8,1 ± 2,9 Ncm para mini-implantes com superfície tratada e 3,3 ± 1,9 Ncm para mini-implantes sem superfície tratada. Entre os implantes que falharam, 71,4% eram mini-implantes sem superfície tratada e 28,6% eram mini-implantes com superfície tratada. Conclusão: O torque de inserção e a taxa de falha não diferiram significativamente entre os grupos; porém, o torque de remoção foi significativamente maior no grupo com superfície tratada. Assim, o tratamento de superfície com jateamento e condicionamento ácido pode melhorar a estabilidade secundária dos mini-implantes ortodônticos autoperfurantes. ABSTRACT Objectives: This clinical trial was conducted to evaluate the stability and failure rate of surface-treated orthodontic mini-implants and determine whether they differ from those of non-surface-treated orthodontic mini-implants. Trial Design: Randomized clinical trial with a split-mouth study design. Setting: Department of Orthodontics, SRM Dental College, Chennai. Participants: Patients who required orthodontic mini-implants for anterior retraction in both arches. Methods: Self-drilling, tapered, titanium orthodontic mini-implants with and without surface treatment were placed in each patient following a split-mouth design. The maximum insertion and removal torques were measured for each implant using a digital torque driver. The failure rates were calculated for each type of mini-implant. Results: The mean maximum insertion torque was 17.9 ± 5.6 Ncm for surface-treated mini-implants and 16.4 ± 9.0 Ncm for non-surface-treated mini-implants. The mean maximum removal torque was 8.1 ± 2.9 Ncm for surface-treated mini-implants and 3.3 ± 1.9 Ncm for non-surface-treated mini-implants. Among the failed implants, 71.4% were non-surface-treated mini-implants and 28.6% were surface-treated mini-implants. Conclusion: The insertion torque and failure rate did not differ significantly between the groups, whereas the removal torque was significantly higher in the surface-treated group. Thus, surface treatment using sandblasting and acid etching may improve the secondary stability of self-drilling orthodontic mini-implants. ABSTRACT Objective: To measure enamel thickness at the proximal surfaces of the mandibular incisors, using micro-computed tomography (micro-CT) scans. Material and Methods: Forty-one single-rooted mandibular incisors were selected and analyzed according to anatomical characteristics, to form three groups: Group 1 - central incisors (n = 18); Group 2 - right lateral incisors (n = 10); and Group 3 - left lateral incisors (n = 13). First, enamel thickness at the proximal contact areas of the mandibular incisors was measured. Second, the mesial and distal surfaces of the lateral incisors were compared. Finally, the relationship between the tooth width and the mean enamel thickness was determined. Each tooth was scanned with a micro-CT scanner, and the image was processed with SCANCO micro-CT onboard analysis software. Results: There were no statistically significant differences in mean enamel thickness between the mesial and distal surfaces for each lateral incisor, or between contralateral lateral incisors. In all surfaces analyzed, the upper zones had statistically significantly thinner enamel (0.52 ± 0.10 mm) when compared to the middle and lower zones (0.60 ± 0.08 mm and 0.59 ± 0.08 mm, respectively). There was no correlation (r =0.07) between enamel thickness of the mandibular incisor and the tooth width. Conclusions: The enamel thickness of the mandibular incisors is similar on the mesial and distal surfaces, with the thinnest layer located at the upper zone.

RESUMO Objetivo: Medir a espessura do esmalte nas superfícies proximais dos incisivos inferiores, usando imagens de microtomografia computadorizada (micro-CT). Material e Métodos: Quarenta e um incisivos inferiores com raiz única foram selecionados e analisados de acordo com as características anatômicas, formando três grupos: Grupo 1 - incisivos centrais (n = 18); Grupo 2 - incisivos laterais direitos (n = 10); e Grupo 3 - incisivos laterais esquerdos (n = 13). Primeiro, foi medida a espessura do esmalte nas áreas de contato proximal dos incisivos inferiores. Em segundo lugar, as faces mesial e distal dos incisivos laterais foram comparadas. Por fim, foi determinada a relação entre a largura do dente e a espessura média do esmalte. Cada dente foi escaneado com um scanner micro-CT, e a imagem foi processada com o software de análise SCANCO micro-CT. Resultados: Não houve diferenças estatisticamente significativas na espessura média do esmalte entre as superfícies mesial e distal de cada incisivo lateral, ou entre os incisivos laterais contralaterais. Em todas as superfícies analisadas, as zonas superiores apresentaram esmalte significativamente mais fino (0,52 ± 0,10 mm) quando comparadas às zonas média e inferior (0,60 ± 0,08 mm e 0,59 ± 0,08 mm, respectivamente). Não houve correlação (r = 0,07) entre a espessura do esmalte do incisivo inferior e a largura do dente. Conclusões: A espessura do esmalte dos incisivos inferiores é semelhante nas faces mesial e distal, com a camada mais fina localizada na zona superior.

ABSTRACT Objective: To compare the initial stress distribution and displacement on mandibular dentition using extra and inter-radicular mini-implants for arch distalization, by means of finite element analysis. Methods: For this study, two finite element models of the mandible were designed. The models consisted of periodontal ligament (PDL) and alveolar bone of all teeth until second molars. In the Case 1, bilateral extra-radicular buccal-shelf stainless steel mini-implants (10.0-mm length; 2.0-mm diameter) were placed between first and second permanent molars. In the Case 2, bilateral inter-radicular stainless steel mini-implants (10.0-mm length; 1.5-mm diameter) were placed between second premolar and first permanent molar. Power hook was attached between canine and first premolar at a fixed height of 8mm. In the two cases, 200g of distalization force was applied. ANSYS v. 12.1 software was used to analyze and compare von Mises stress and displacement in the mandibular dentition, PDL and bone. Results: Higher stresses were observed in mandibular dentition with the inter-radicular implant system. The amount of von Mises stress was higher for cortical bone (85.66MPa) and cancellous bone (3.64MPa) in Case 2, in comparison to cortical bone (41.93MPa) and cancellous bone (3.43MPa) in Case 1. The amount of arch distalization was higher for mandible in Case 1 (0.028mm), in comparison to Case 2 (0.026mm). Conclusion: Both systems were clinically safe, but extra-radicular implants showed more effective and controlled distalization pattern, in comparison to inter-radicular implants, in Class III malocclusion treatment.

RESUMO Objetivo: Comparar a distribuição da tensão inicial e o deslocamento na dentição inferior usando mini-implantes extra e inter-radiculares para distalização da arcada, por meio da análise de elementos finitos. Métodos: Dois modelos de elementos finitos da mandíbula foram criados, os quais consistiram de ligamento periodontal (PDL) e osso alveolar de todos os dentes até os segundos molares. No Caso 1, mini-implantes extra-radiculares de aço inoxidável (10,0 mm de comprimento; 2,0 mm de diâmetro) foram colocados bilateralmente na buccal-shelf entre o primeiro e o segundo molares permanentes. No Caso 2, mini-implantes de aço inoxidável inter-radiculares (comprimento de 10,0 mm; diâmetro de 1,5 mm) foram colocados bilateralmente entre o segundo pré-molar e o primeiro molar permanentes. Um Power hook foi preso entre o canino e o primeiro pré-molar a uma altura fixa de 8mm. Nos dois casos, foi aplicada força de distalização de 200g. O software ANSYS v. 12.1 foi usado para analisar e comparar a tensão de von Mises e o deslocamento na dentição inferior, ligamento periodontal e osso. Resultados: Maiores tensões foram observadas na dentição inferior com o sistema de implantes inter-radiculares. A quantidade de tensões de von Mises foi maior para osso cortical (85,66MPa) e osso esponjoso (3,64MPa) no Caso 2, em comparação com osso cortical (41,93MPa) e osso esponjoso (3,43MPa) no Caso 1. A quantidade de distalização da arcada inferior foi maior no Caso 1 (0,028 mm), em comparação com o Caso 2 (0,026 mm). Conclusão: Ambos os sistemas foram clinicamente seguros, mas os implantes extra-radiculares mostraram um padrão de distalização mais eficaz e controlado, em comparação com os implantes inter-radiculares, para tratamento da má oclusão de Classe III.

ABSTRACT Objective: The aim of this study was to evaluate the volumetric root resorption in maxillary incisors following clear aligner therapy (CAT) with low-intensity pulsed ultrasound (LIPUS), and compare the results to CAT alone. Material and Methods: This retrospective study evaluated pretreatment (T0) and post-treatment (T1) cone-beam computed tomography imaging of 42 adult patients. Twenty-one patients (14 females, 7 males, mean age= 38.1±12.96 years) were treated using CAT with LIPUS device, whereas the other twenty-one matching controls patients (15 females, 6 males, mean age= 35.6±11.7 years) were treated using CAT alone. Images were analyzed and a segmentation protocol was applied on the maxillary incisors. Each segmented tooth volume was exported as a surface mesh in the Visualization Toolkit (VTK) file format. The VTK files for all maxillary incisors were coded and corresponding teeth volumes from T0 and T1 were superimposed. Clipping the crown of each tooth was done, then measurements of root volumes and differences between groups were performed. Changes in root volumes were assessed (p<0.05). Results: Root loss was evident in all teeth in both groups, but was significantly increased in all maxillary incisors of the control group (p<0.001) and in upper left central incisor of LIPUS group (p=0.009). When both groups were compared, there was statistically significant minimal volumetric root loss in LIPUS group (3.50-7.32 mm3), when compared to control group (11.48-12.95 mm3) (p<0.05). Conclusion: LIPUS group showed less volumetric root resorption compared to control group during the studied treatment time using clear aligners.

RESUMO Objetivo: O objetivo deste estudo foi avaliar volumetricamente a reabsorção radicular em incisivos superiores após tratamento com alinhadores transparentes (CAT) com e sem uso adjuvante de ultrassom de baixa intensidade (LIPUS). Material e Métodos: Esse estudo retrospectivo avaliou imagens de tomografia computadorizada de feixe cônico pré-tratamento (T0) e pós-tratamento (T1) de 42 pacientes adultos: 21 pacientes (14 mulheres, 7 homens, idade média= 38,1±12,96 anos) foram tratados com CAT e LIPUS, enquanto os outros 21 pacientes controles correspondentes (15 mulheres, 6 homens, idade média= 35,6±11,7 anos) foram tratados apenas com CAT. As imagens foram analisadas e foi aplicado um protocolo de segmentação dos incisivos superiores. Os volumes de cada dente segmentado foram exportados como malhas de superfície, em arquivos no formato Visualization Toolkit (VTK). Os arquivos VTK de todos os incisivos superiores foram codificados e foram sobrepostos os volumes dos dentes correspondentes a T0 e T1. Foi realizada a clipagem da coroa de cada dente e, em seguida, foram realizadas medições dos volumes radiculares e comparadas as diferenças entre os grupos, avaliando-se as alterações nos volumes de raízes (p<0,05). Resultados: A perda radicular foi evidente em todos os dentes em ambos os grupos, mas foi significativamente maior em todos os incisivos superiores do grupo controle (p<0,001) e no incisivo central superior esquerdo do grupo LIPUS (p=0,009). Quando ambos os grupos foram comparados, houve perda volumétrica mínima estatisticamente significativa no grupo LIPUS (3,50-7,32 mm3), em comparação ao grupo controle (11,48-12,95 mm3) (p<0,05). Conclusão: O grupo LIPUS apresentou menor volume de reabsorção radicular, em comparação ao grupo controle, durante o tempo de tratamento estudado usando alinhadores transparentes.

RESUMO Introdução: O uso de esporões linguais tem sido descrito como uma opção eficiente, com grande estabilidade de resultados, mas com poucas informações quanto à tolerância de seu uso nas fases de dentição mista e permanente. Objetivo: O objetivo deste estudo foi avaliar o impacto dos esporões linguais na qualidade de vida relacionada à saúde bucal de crianças e/ou adolescentes durante o tratamento da mordida aberta anterior. Métodos: Essa revisão foi registrada no banco de dados PROSPERO. Oito bases de dados eletrônicas e parte da literatura cinzenta foram pesquisadas, sem restrições, até março de 2022. Uma busca manual também foi realizada nas referências dos artigos incluídos. Estudos avaliando o impacto dos esporões linguais na qualidade de vida relacionada à saúde bucal foram incluídos. O risco de viés foi avaliado usando a ferramenta JBI ou ROBINS-I, de acordo com o desenho do estudo. O nível de evidência foi avaliado por meio do GRADE. Resultados: Cinco estudos preencheram os critérios de elegibilidade. Dois ensaios clínicos não randomizados apresentaram sério risco de viés. Dos estudos de série de casos, dois tiveram baixo risco de viés e o outro, risco moderado de viés. A certeza da evidência foi classificada como muito baixa para todos os resultados avaliados. Em geral, os estudos relataram um impacto negativo inicial com o uso dos esporões linguais; porém, de caráter transitório. Não foi realizada análise quantitativa, devido à grande heterogeneidade entre os estudos. Conclusão: As evidências atuais, embora limitadas, sugerem que os esporões linguais têm um impacto negativo transitório inicial durante o tratamento interceptativo. São necessários mais ensaios clínicos randomizados bem conduzidos.

ABSTRACT Introduction: The use of lingual spurs has been described as one efficient option, with great stability of results, but with scarce information of toleration for use in the mixed and permanent dentition phases. Objective: The purpose of this study was to assess the impact of lingual spurs on the oral health-related quality of life of children and/or adolescents during anterior open bite treatment. Methods: The review was recorded in the PROSPERO database. Eight electronic databases and partial gray literature were searched, without restrictions until march 2022. A manual search was also performed in the references of the included articles. Studies assessing the impact of lingual spurs on the oral health-related quality of life were included. Risk of bias was assessed using JBI or ROBINS-I tool, according to the study design. The level of evidence was assessed through GRADE. Results: Five studies met the eligibility criteria. Two non-randomized clinical trials had a serious risk of bias. Of the case-series studies, two had a low risk of bias and the other, a moderate risk of bias. The certainty of the evidence was classified as very low for all the evaluated results. In general, the studies reported an initial negative impact with the use of lingual spurs, however this was transitory in nature. A quantitative analysis was not performed due to the great heterogeneity between the studies. Conclusion: Current evidence, although limited, suggests that lingual spurs have an initial transient negative impact during interceptive treatment. Additional well-conducted randomized clinical trials are needed.

RESUMO Objetivo: O presente trabalho teve como objetivo comparar parâmetros dentários posicionais e gengivais de dentes anteriores superiores em pacientes com fissura labiopalatina transforame unilateral (FLPTU) após tratamento ortodôntico com substituição de incisivo lateral ausente por canino, devido a agenesia. Métodos: Esse estudo de boca dividida reuniu uma amostra, de um mesmo centro especializado de tratamento, de 57 indivíduos com FLPTU (31 homens, 26 mulheres) e agenesia do incisivo lateral superior no lado da fissura. A substituição do incisivo lateral por um canino foi concluída após o enxerto ósseo alveolar secundário. Os modelos dentários foram obtidos entre dois e seis meses após a remoção do aparelho (idade média: 20,4 anos). Foram mensuradas nos dentes anteriores as variáveis: altura, largura, proporção e simetria da coroa, bem como as distâncias entre as bordas incisais, margens gengivais, angulação mesiodistal e inclinação vestibulolingual. O teste t pareado com correção post-hoc de Bonferroni foi usado para comparações entre os lados fissurados e não fissurados (p<0,05). Resultados: No lado fissurado, os caninos que substituíram os incisivos laterais ausentes apresentaram uma altura de coroa maior (0,77 mm) e uma largura aumentada (0,67 mm). Os primeiros pré-molares apresentaram uma altura de coroa menor (1,39 mm). Assimetrias foram observadas no nível gengival dos incisivos centrais e laterais, com maior coroa clínica no lado fissurado (0,61 e 0,81mm, respectivamente). Os incisivos centrais do lado fissurado estavam mais intruídos do que os contralaterais (2,12 mm). Conclusão: Os dentes anteriores superiores demonstraram diferenças de posição, tamanho e altura gengival entre os lados fissurado e não fissurado, após o fechamento do espaço da agenesia dos incisivos laterais superiores. Leves assimetrias na posição do dente e margem gengival nos dentes anteriores superiores devem ser esperadas após o tratamento ortodôntico em pacientes com fissura labiopalatina transforame unilateral.

ABSTRACT Objective: To compare dental positional and gingival parameters of maxillary anterior teeth in unilateral cleft lip and palate (UCLP) after orthodontic treatment with canine substitution due to lateral incisor agenesis. Methods: This split-mouth study comprised 57 subjects with UCLP (31 male, 26 female) and agenesis of maxillary lateral incisor at the cleft side, from a single center. Canine substitution was completed after the secondary alveolar bone graft. Dental models were taken between 2 to 6 months after debonding (mean age: 20.4 years). The following variables were measured in the maxillary anterior teeth: crown height, width, proportion, and symmetry, as well as steps between incisal edges, gingival margins, tooth mesiodistal angulation and labiolingual inclination. Paired t-tests with Bonferroni post-hoc correction was used for comparisons between cleft and non-cleft sides (p<0.05). Results: At the cleft side, canines replacing missing lateral incisors had a higher crown height (0.77mm) and an increased width (0.67mm), and first premolars showed a shorter crown height (1.39mm). Asymmetries were observed in the gingival level of central and lateral incisors, with a greater clinical crown at the cleft side (0.61 and 0.81mm, respectively). Cleft side central incisors were more upright than their contralaterals (2.12º). Conclusions: Maxillary anterior teeth demonstrated positional, size and gingival height differences between cleft and non-cleft side after space closure of maxillary lateral incisor agenesis. Slight asymmetries in tooth position and gingival margin in the maxillary anterior teeth should be expected after orthodontic treatment in UCLP patients.

RESUMO Introdução: Embora se alegue uma superioridade dos alinhadores transparentes sobre os aparelhos multibraquetes em manter a gengiva saudável, ainda não foram investigados os possíveis benefícios de um modelo de alinhador sobre outro, especialmente no que diz respeito à borda vestibular. Objetivo: O objetivo deste estudo foi medir vários índices periodontais em adolescentes submetidos a tratamento ortodôntico com alinhadores, comparando dois tipos diferentes de borda. Métodos: O estudo envolveu 43 pacientes com idade entre 14 e 18 anos. A saúde periodontal foi avaliada por meio do índice de placa (IP), índice gengival (IG) e índice de sangramento gengival (ISG), ao início do tratamento com alinhadores (T0), com borda vestibular (BV) estendendo-se até 3 mm além da margem gengival. Três meses depois (T1), os alinhadores foram ajustados para se obter uma borda justagengival (BJ) no segundo quadrante e BV no primeiro quadrante. Os índices periodontais foram medidos novamente tanto em T1 quanto três meses depois (T2). Resultados: As comparações intraquadrantes revelaram uma piora estatisticamente significativa nos índices periodontais apenas para o segundo quadrante (p<0,05), em T1 (IG) e especialmente em T2 (IP, IG, ISG), enquanto nenhuma mudança estatisticamente significativa foi encontrada para o primeiro quadrante. Conclusões: Irritações mecânicas mais intensas, principalmente durante a inserção e remoção do alinhador, podem explicar a piora dos índices inflamatórios com a BJ. Além disso, a pressão exercida pela BJ no sulco gengival pareceu facilitar a deposição de placa, enquanto a BV teve um efeito protetor, reduzindo o risco de trauma mecânico.

ABSTRACT Introduction: Although the superiority of clear aligners over multi-bracket appliances in keeping gingiva healthy has been suggested, the possible benefits of one aligner design over another have not yet been investigated, especially with regard to the vestibular edge. Objective: The aim of this study was to measure several periodontal indexes in adolescents undergoing orthodontic treatment with aligners, comparing two different types of rim. Methods: The study involved 43 patients aged between 14 and 18 years. The periodontal health was assessed using plaque index (PI), gingival index (GI), and gingival bleeding index (GBI), at the start of the treatment with aligners (T0), with a vestibular rim (VR) reaching up to 3 mm beyond the gingival margin. Three months later (T1), aligners were set to obtain a juxtagingival rim (JR) on the second quadrant and VR on the first quadrant. The periodontal indexes were measured again, both at T1 and then three months later (T2). Results: Intra-quadrant comparisons revealed a statistically significant worsening of the periodontal indexes only for the second quadrant (p<0.05), at T1 (GI), and especially at T2 (PI, GI, GBI), while no statistically significant changes were found for the first quadrant. Conclusions: More severe mechanical irritation, especially during insertion and removal of the aligner, can explain the worsening inflammatory indexes with the JR. In addition, the pressure exerted by the JR on the gingival sulcus seemed to facilitate plaque deposition, whereas the VR had a protective effect, reducing the risk of mechanical trauma.

RESUMO Introdução: A apneia obstrutiva do sono (AOS) afeta uma importante parcela da população e caracteriza-se pela obstrução total ou parcial recorrente da via aérea superior (VAS) durante o sono, o que afeta negativamente a qualidade de vida dos pacientes no curto prazo e no longo prazo, e constitui importante problema de saúde pública para a sociedade. A área de atuação do ortodontista está em íntima relação com a VAS, o que o coloca em uma posição estratégica para diagnosticar falhas na passagem de ar e intervir quando necessário. É imperativo que o ortodontista, como profissional da saúde, saiba reconhecer problemas respiratórios e manejá-los de maneira apropriada, quando indicado. Objetivo: O objetivo desse artigo é revisar e avaliar criticamente a literatura pertinente, para proporcionar ao ortodontista conhecimento atualizado sobre o diagnóstico e terapêutica relacionados à AOS. Ciência e tecnologia estão em constante evolução; portanto, a literatura também foi revisada considerando as novas tecnologias disponíveis em aplicativos e dispositivos direcionados aos consumidores para o diagnóstico, monitoramento e tratamento dos distúrbios respiratórios do sono.

ABSTRACT Introduction: Obstructive sleep apnea (OSA) affects an important part of the population and is characterized by recurrent total or partial obstruction of the upper airway (UA) during sleep, negatively affecting the quality of life of patients in the short and long terms, and constituting an important public health problem for the society. The field of expertise of orthodontists is closely related to the UA, placing them in a strategic position to diagnose air passage failures and intervene when necessary. Orthodontists, as health professionals, must know how to recognize respiratory problems and manage them appropriately, when indicated. Objective: Thus, this paper aims to review and critically evaluate the related literature, to provide orthodontists with updated knowledge on the diagnosis and therapy related to OSA. Science and technology are constantly evolving; thus, the literature was also reviewed considering new technologies available in consumer-targeted applications and devices for the diagnosis, monitoring, and treatment of sleep-disordered breathing.

RESUMO Introdução: O surgimento dos alinhadores ortodônticos tem proporcionado uma opção estética e confortável para o tratamento ortodôntico. No entanto, o design encapsulado dos alinhadores pode influenciar os músculos mastigatórios e comprometer a segurança do tratamento. Objetivo: Este estudo longitudinal preliminar teve como objetivo investigar se o uso de alinhadores ortodônticos afeta a força de mordida e a atividade mioelétrica dos músculos masseter superficial e temporal anterior. Métodos: Dez indivíduos participaram do estudo e foram submetidos a tratamento durante um período de acompanhamento de 8 meses. A raiz quadrada média (RMS), a frequência mediana de potência (FMP) dos sinais de superfície da eletromiografia (sEMG) e a força de mordida (kgf) foram registradas e normalizadas em relação à condição de pré-tratamento. Os dados foram analisados por análise de variância para medidas repetidas (ANOVA), com nível de significância estabelecido em 5%. Resultados: Tanto o masseter superficial quanto o temporal anterior apresentaram aumento da atividade do sinal sEMG durante o tratamento, com aumento acentuado desse último em comparação ao primeiro (p<0,05). Além disso, foi evidenciada uma diminuição significativa da força de mordida (p<0,05). Conclusões: Esse estudo preliminar observou que os alinhadores ortodônticos afetaram o padrão de recrutamento muscular dos músculos mastigatórios e reduziram o desempenho da mordida durante o período de acompanhamento de oito meses.

ABSTRACT Introduction: The emergence of orthodontic aligners has provided an aesthetic and comfortable option for orthodontic treatment. However, the encapsulated design of the aligners can influence the masticatory muscles, and might compromise safe treatment. Objective: This preliminary longitudinal study aimed to investigate whether the use of orthodontic aligners affects the biting force and myoelectric activity of the superficial masseter and anterior temporal muscles. Methods: Ten subjects participated in the study and underwent treatment during an 8-month follow-up period. The root mean square (RMS), the median power frequency (MPF) of the surface electromyography (sEMG) signals, and the biting force (kgf) were recorded and normalized relative to the pretreatment condition. The data were analyzed by repeated-measure analysis of variance (ANOVA), with the significance level set at 5%. Results: Both the superficial masseter and the anterior temporal muscles presented an increase in sEMG signal activity during the treatment, with a marked increase in the latter compared to the former (p<0.05). Moreover, a significant decrease in bite force was evidenced (p<0.05). Conclusions: This preliminary study observed that the orthodontic aligners affected the muscle recruitment pattern of masticatory muscles, and reduced biting performance during the 8-month follow-up period.

RESUMO Introdução: O número de estudos publicados na área da Ortodontia aumentou consideravelmente nos últimos dez anos. Objetivos: Analisar os dados bibliométricos dos estudos ortodônticos internacionais publicados entre 2011 e 2020 em periódicos ortodônticos da base de dados Scopus, bem como realizar a comparação entre os dados dos períodos 2010-2015 e 2016-2020. Material e Métodos: Uma pesquisa retrospectiva foi realizada em 14 periódicos ortodônticos incluídos na base de dados Scopus de 2011 a 2020. Estudos dos tipos primários e secundários foram incluídos na pesquisa. Foram apresentados o número anual de estudos publicados em 14 revistas e os 20 primeiros países, instituições e seu tipo (público/privado) e autores, respectivamente, em relação ao volume de publicações. Resultados: Nos últimos dez anos, o número de publicações nos periódicos escolhidos chegou a 9.200, e o American Journal of Orthodontics and Dentofacial Orthopedics e o Angle Orthodontist lideraram a lista de periódicos, com 22% e 12% das publicações, respectivamente. Além disso, o volume de publicações ortodônticas apresentou uma tendência de queda no final da década (-9%), sendo que as instituições acadêmicas/públicas produziram a maior parte dos estudos ortodônticos, e os EUA (20%), Brasil (17%) e a Coreia do Sul (8%) lideraram os países com mais estudos ortodônticos. Uma comparação das duas metades da década revelou que a pesquisa ortodôntica exibiu uma tendência crescente nos países em desenvolvimento, especialmente Egito (104%), Arábia Saudita (88%) e Irã (83%). Conclusão: Os estudos ortodônticos publicados nos últimos dez anos nos periódicos escolhidos mostraram uma mudança dinâmica na publicação anual e no ranking de países, instituições e autores.

ABSTRACT Introduction: The number of published orthodontic studies has increased considerably over the past ten years. Objective: To analyze the bibliometric data of international orthodontic studies included in orthodontic journals encompassed by the Scopus database between 2011 and 2020, as well as to undertake data comparison between the period 2010-2015 and the period 2016-2020. Materials and methods: A retrospective search was conducted on 14 orthodontic journals included in the Scopus database from 2011 to 2020. Studies of both primary and secondary types were targeted by the search. The yearly number of studies published in the 14 journals, and the first 20 countries, institutions and their type (public/private), and authors, respectively, regarding publication volume, were presented. Results: Over the past ten years, the number of publications in the chosen journals reached 9200, where the American Journal of Orthodontics and Dentofacial Orthopedics and Angle Orthodontist topped the journal list, with 22% and 12% of the publications, respectively. Furthermore, the orthodontic publication volume showed a declining trend by the end of the decade (-9%), where academic/public institutions produced most of the orthodontic studies, and the US (20%), Brazil (17%), and South Korea (8%) topped the countries with the most orthodontic studies. A comparison of the two halves of the decade revealed that orthodontic research exhibited an increasing trend in developing nations, especially Egypt (104%), Saudi Arabia (88%), and Iran (83%). Conclusion: The orthodontic studies published in the chosen journals over the past ten years showed a dynamic change in yearly publication and ranking of countries, institutions, and authors.
