ABSTRACT Background: Increasing population size and the presence of bottlenecks in access to specialized health care demonstrate the importance of developing measures for better clinical management. The implementation of teleregulation is expected to bring greater resolution in the system. Objective: The objective of the present study was to evaluate the results of teleregulation in gastroenterology for the resolution of referrals in a large Brazilian city. Methods: We carried out a retrospective cross-sectional study of primary health care teleregulation requests for gastroenterology in a Brazilian city. Data were collected from October 2022 to June 2023 in patients aged >18 years. Patient demographic data, the reason for requesting screening, and screening outcomes were collected. Requests for reassessment were excluded. Results: Of the 3,000 teleregulation sessions screened in the study period, 71.1% were included, of which 68.17% were for women with a mean age of 54.32±16.19 years. Among the reasons for referral, 1,368 (64.13%) were to request examinations, 568 (26.63%) to discuss conduct and 197 (9.24%) to request a referral to a specialist. Ten percent of cases required referral to a specialist, 6.61% were incorrect requests and 14.95% were prioritized. Conclusion: The present study highlights that teleregulation represents an important tool in health management, being able to bring resolution in 89.9% of cases.

RESUMO Contexto: O aumento populacional e a presença de gargalos no acesso à atenção especializada em saúde demonstram a importância do desenvolvimento de medidas para melhor gestão. Com a implantação da telerregulação, espera-se uma maior resolutividade no sistema. Objetivo: O objetivo do presente estudo é avaliar os resultados da telerregulação em gastroenterologia nos encaminhamentos de uma grande cidade brasileira. Métodos: Estudo transversal retrospectivo das solicitações de telerregulação da atenção primária à saúde para gastroenterologia realizadas na cidade de Curitiba-PR-Brasil pela equipe de Gastroenterologia do Complexo Hospital de Clínicas da Universidade Federal do Paraná. Os dados foram coletados entre os meses de outubro de 2022 e junho de 2023 em pacientes maiores de 18 anos. Foram coletados dados demográficos dos pacientes, bem como o motivo da solicitação da triagem e seu desfecho. Solicitações de reavaliação foram excluídas. Resultados: Das 3000 telerregulações triadas no período, 71,1% foram incluídas, sendo 68,17% do sexo feminino e com média de idade de 54,32±16,19 anos. Dos motivos de encaminhamento, 1368 (64,13%) foram para solicitação de exames, 568 (26,63%) para discussão de conduta e 197 (9,24%) para solicitação de encaminhamento para especialista. Dez por cento dos casos necessitaram de encaminhamento para especialista, 6,61% foram solicitações incorretas e 14,95% foram priorizadas. Conclusão: O presente estudo destaca que a telerregulação representa uma importante ferramenta na gestão da saúde, sendo capaz de trazer resolutividade em 89,9% dos casos.

ABSTRACT Background: In patients with cystic fibrosis (pwCF) acid suppression therapy, gastrointestinal dysmotility, and post-operative bowel status, may predispose to the development of small intestinal bacterial overgrowth (SIBO). SIBO may continue to be present in the progression of the disease even on modulators. Breath testing is the most simple, non-invasive and available method for diagnosing SIBO. There are some divergencies over the operational procedures used to carry out and interpret breath tests in pwCF. Objective: We performed a systematic review of SIBO in pwCF to assess the methods used in breath tests and the existence of causal relationship between SIBO and following CF co-morbidities: liver disease, fat absorption, and eating disorders. Methods: We searched the PubMed, Cochrane Library, Embase, LILACS, MEDLINE, OpenGray, medRxiv, Google Scholar, and CAPES databases up to March 20, 2024. We selected clinical cohort and case-control studies to assess SIBO in cwCF. We selected studies that met the following criteria: (1) participants - children and adolescents diagnosed with CF; (2) intervention - assessment of SIBO using H2 and CH4 breath tests; (3) control - patients without SIBO; and (4) outcome - assessment of breath tests for SIBO diagnosis and the causal relationship between SIBO and CF co-morbidities. The PRISMA statement was used to report the search. QUADAS 2 tool was used for assessing the quality of each study methodology. The protocol for this review was registered in the Prospective Registration of Systematic Review Database (CRD42024503593). Results: The search strategy identified 279 studies. After screening titles and abstracts, 36 studies were selected for full-text review and 27 were excluded; nine studies involving 206 pwCFs were reviewed. All nine studies used H2 breath tests as a diagnostic method for SIBO, and five of them used a combined H2/CH4 test. There was no consistency in the timing of cessation of antibiotic therapy prior to testing. All patients performed the test after an overnight fast. A basal sample was collected prior to substrate (glucose or lactulose) ingestion, which ranged from 7 to 20 ppm. There was great variability between respiratory sample collection times, being times 0, 15, 30, 45, 60, 90, and 120 minutes the most used protocol. The methods for performing breath tests varied widely, making it difficult to reach conclusions on the role of SIBO as a co-morbidity in pwCF. There was no association between increased serum AST, ALT, and GGT levels and positive breath tests. There was no agreement regarding the role of SIBO and nutritional deficiency, but a reduction in fat absorption and the presence of hyporexia have been described under this condition. Conclusion: Data on assessment of SIBO in pwCF is limited by the small number of studies available, the lack of appropriate controls in some studies, and the varying test methodology and diagnostic cut-offs applied. Protocols to investigate and diagnosing SIBO in pwCF need to be developed.

RESUMO Contexto: Em pacientes com fibrose cística (FC), a terapia de supressão ácida, dismotilidades gastrointestinais e o status pós-operatório de cirurgias intestinais podem predispor ao desenvolvimento de supercrescimento bacteriano do intestino delgado (SIBO). O SIBO pode continuar presente na progressão da doença, mesmo com o uso de moduladores. O teste respiratório é o método mais simples, não invasivo e amplamente disponível para diagnosticar SIBO. Contudo, há algumas divergências sobre os procedimentos operacionais usados para realizar e interpretar esses testes de pacientes com FC. Objetivo: Realizamos uma revisão sistemática do SIBO em pessoas com FC para avaliar os métodos usados nos testes respiratórios e a existência de relação causal entre SIBO e as seguintes comorbidades no paciente com FC: doença hepática, absorção de gordura e distúrbios nutricionais/alimentares. Métodos: Realizamos buscas nos bancos de dados PubMed, Cochrane Library, Embase, LILACS, MEDLINE, OpenGray, medRxiv, Google Scholar e CAPES incluindo artigos publicados até 20 de março de 2024. Selecionamos estudos clínicos de coorte e caso-controle para avaliar a SIBO em pacientes com FC. Selecionamos estudos que atendiam aos seguintes critérios: (1) participantes - crianças e adolescentes diagnosticados com FC; (2) intervenção - avaliação de SIBO usando testes respiratórios com dosagens de H2 ou CH4; (3) controle - pacientes sem SIBO; e (4) resultado - avaliação dos procedimentos operacionais de realização dos testes respiratórios para diagnóstico de SIBO e a relação causal entre SIBO e comorbidades em paciente com FC. A estratégia PRISMA foi usada para relatar a pesquisa. A ferramenta QUADAS 2 foi utilizada para avaliar qualidade da metodologia de cada estudo. O protocolo para essa revisão foi registrado no Prospective Registration of Systematic Review Database (CRD42024503593). Resultados: A estratégia de busca identificou 279 estudos. Após a triagem de títulos e resumos, 36 estudos foram selecionados para revisão de texto completo e 27 foram excluídos; nove estudos envolvendo 206 pacientes com FC foram revisados. Todos os nove estudos usaram testes respiratórios com H2 e cinco associaram H2/CH4 expirados. Não houve uniformidade quanto ao tempo de suspensão de antibioticoterapia previamente a realização dos testes. Todos os pacientes realizaram o teste em jejum. Uma amostra basal foi coletada antes da ingesta do substrato (glicose ou lactulose), variando de 7 a 20 ppm. Houve grande variabilidade entre o tempo de coleta das amostras respiratórias, sendo o protocolo de tempos 0, 15, 30, 45, 60, 90 e 120 minutos, o mais utilizado. Os métodos para a realização de testes respiratórios apresentaram grande variabilidade, dificultando a obtenção de conclusões sobre o papel do SIBO como uma comorbidade em pessoas com FC. Não houve associação entre o aumento dos níveis séricos de AST, ALT e GGT e testes respiratórios positivos. Não houve concordância com relação ao papel do SIBO na deficiência nutricional, mas uma redução na absorção de gordura e a presença de hiporexia foram descritas nessa condição. Conclusão: Os dados sobre a avaliação do SIBO em pessoas com FC são limitados pelo pequeno número de estudos disponíveis, pela falta de controles pareados e pela variação da metodologia dos testes diagnósticos aplicados. É necessário desenvolver protocolos para investigar e diagnosticar SIBO em pessoas com FC.

ABSTRACT Background: The incidence of nephrolithiasis has increased significantly, diet, obesity, and high animal protein intake having been reported to be risk factors. Nephrolithiasis has a great economic impact on society related to work absenteeism, recurrent attacks of renal colic, and urological interventions. Nephrolithiasis can also progress to renal failure. It is therefore important to identify the risk factors for nephrolithiasis. Inflammatory bowel diseases, which include Crohn’s disease, represent a risk factor for the formation of renal calculi, due to the disease itself and to the use of drugs that can influence the metabolism of substances related to nephrolithiasis. In the past, Crohn’s disease patients were often submitted to surgery, which is known to contribute to nephrolithiasis. New drugs have changed the clinical course of Crohn’s disease, whose incidence has increased worldwide. Specialists should be on the alert not only for the complications of Crohn’s disease but also for its extraintestinal manifestations, which can dramatically affect the quality of life of these patients and lead to renal failure. It is therefore important to screen this population for nephrolithiasis. Objective: To determine the prevalence of nephrolithiasis in a population of patients with Crohn’s disease; to determine whether drugs, previous surgery, location of the disease, and clinical activity are risk factors for nephrolithiasis; and to alert specialists to the importance of screening for nephrolithiasis (through simple methods) in order to prevent chronic kidney disease. Methods: Were analyzed the electronic medical records of 93 Crohn’s disease patients treated between 2009 and 2010 at the Inflammatory Bowel Disease Center of the Scope Clinic, located in the city of Campo Grande. All of the patients underwent ultrasound at the first medical appointment. Results: Of the 93 patients, 37 developed nephrolithiasis at some point during the study period. Risk factors for nephrolithiasis were disease location (P=0.023) and the use of ciprofloxacin (P=0.0001), corticosteroids (P=0.005), immunomodulators (P=0.001), or metronidazole (P=0.0005). Surgical status, age, and gender were not found to predispose to the formation of renal calculi. Conclusion: This study demonstrates the importance of using imaging methods to screen Crohn’s disease patients for nephrolithiasis, regardless of their surgical status.

RESUMO Contexto: A incidência de nefrolitíase aumentou significativamente, tendo sido relatado que dieta, obesidade e alta ingestão de proteína animal são fatores de risco. A nefrolitíase tem um grande impacto econômico na sociedade relacionado com o absenteísmo laboral, crises recorrentes de cólica renal e intervenções urológicas. A nefrolitíase também pode evoluir para insuficiência renal. Portanto, é importante identificar os fatores de risco para nefrolitíase. As doenças inflamatórias intestinais, que incluem a doença de Crohn, representam fator de risco para a formação de cálculos renais, devido à própria doença e ao uso de medicamentos que podem influenciar o metabolismo de substâncias relacionadas à nefrolitíase. No passado, os pacientes com doença de Crohn eram frequentemente submetidos a cirurgia, o que é conhecido por contribuir para a nefrolitíase. Novos medicamentos alteraram o curso clínico da doença de Crohn, cuja incidência aumentou em todo o mundo. Os especialistas devem estar atentos não apenas às complicações dela, mas também as manifestações extraintestinais da doença, que podem afetar dramaticamente a qualidade de vida desses pacientes e levar à insuficiência renal. Sendo assim, é importante rastrear esta população para nefrolitíase. Objetivo: Determinar a prevalência de nefrolitíase numa população de doentes com doença de Crohn; determinar se medicamentos, cirurgia prévia, localização da doença e atividade clínica são fatores de risco para nefrolitíase; e alertar os especialistas para a importância do rastreio da nefrolitíase (através de métodos simples) para prevenir a doença renal crônica. Métodos: Foram analisados os prontuários eletrônicos de 93 pacientes com doença de Crohn atendidos entre 2009 e 2010, no Centro de Doenças Inflamatórias Intestinais da Clínica Scope, localizado na cidade Campo Grande. Todos os pacientes foram submetidos à ultrassonografia na primeira consulta médica. Resultados: Dos 93 pacientes, 37 desenvolveram nefrolitíase em algum momento durante o período do estudo. Os fatores de risco para nefrolitíase foram localização da doença (P=0,023) e uso de ciprofloxacino (P=0,0001), corticoide (P=0,005), imunomoduladores (P=0,001) ou metronidazol (P=0,0005). O estado cirúrgico, a idade e o sexo não predispuseram à formação de cálculos renais. Conclusão: Este estudo demonstra a importância do uso de métodos de imagem para triagem de nefrolitíase em pacientes com doença de Crohn, independentemente do estado cirúrgico.

ABSTRACT Background: Steatotic liver disease (SLD) affects about 1 billion people globally, making its proper management essential to prevent progression to more severe stages. Objective: The aim of this study was to evaluate medical management concerning hepatic steatosis incidentally identified by ultrasound in patients undergoing elective cholecystectomy. Methods: This observational, cross-sectional, and retrospective study included patients aged 18 years or older who underwent elective cholecystectomy at Hospital do Trabalhador, in Curitiba/PR, between 2018 and 2022. Patients with external ultrasound reports or incomplete data in their medical records were excluded. Medical records, laboratory tests, and ultrasound reports were analyzed to evaluate the prevalence of steatosis in these patients. Results: The study sample consisted of 355 patients, and 103 (29.01%) of them presented steatosis on ultrasound. Older age (P=0.0022), male sex (P=0.03009), higher body mass index (P<0.001), obesity (P<0.001), hypertension (P<0.001), dyslipidemia (P=0.0072), and elevated levels of oxaloacetic and pyruvic aminotransferases (P=0.02112) were associated with the presence of this finding. No action was taken regarding the presence of steatosis in 60.19% of patients. Approximately 39.81% had the finding recorded in their medical records, 6.80% received lifestyle change counseling, and 4.85% were investigated for the stage of steatosis. Conclusion: A significant prevalence of hepatic steatosis was incidentally identified in the ultrasound of patients undergoing cholecystectomy. However, the approach to this finding was insufficient, highlighting the need for substantial improvements on its management and investigation.

RESUMO Contexto: A doença hepática esteatótica (DHE) afeta cerca de 1 bilhão de pessoas globalmente, sendo fundamental seu manejo adequado para evitar sua progressão para estágios mais graves. Objetivo: O objetivo do estudo foi avaliar a conduta médica referente à esteatose hepática identificada incidentalmente no ultrassom de pacientes submetidos à colecistectomia eletiva. Métodos: Estudo observacional, transversal e retrospectivo, que incluiu pacientes com idade igual ou superior a 18 anos submetidos à colecistectomia eletiva no Hospital do Trabalhador, em Curitiba/PR, entre 2018 e 2022. Foram excluídos pacientes com ultrassonografia externa e com dados incompletos no prontuário. Foram analisados prontuários, exames laboratoriais e laudos ultrassonográficas, com avaliação da prevalência da esteatose nesses pacientes. Resultados: A amostra do estudo foi composta por 355 pacientes, e 103 (29,01%) deles apresentaram esteatose ao ultrassom. Idade elevada (P=0,0022), sexo masculino (P=0,03009), índice de massa corporal elevado (P<0,001), obesidade (P<0,001), hipertensão arterial (P<0,001), dislipidemia (P=0,0072) e maiores níveis de transaminases oxalacética e pirúvica (P=0,02112) associaram-se à presença desse achado. Não houve conduta em relação à presença de esteatose em 60,19% dos pacientes. Cerca de 39,81% tiveram o achado anotado em seu prontuário, 6,80% receberam orientações sobre mudanças no estilo de vida e 4,85% foram investigados quanto ao estágio da esteatose. Conclusão: Uma prevalência notável de esteatose hepática foi identificada incidentalmente no ultrassom de pacientes submetidos à colecistectomia. Contudo, a abordagem desse achado revelou-se insuficiente, com necessidade de melhorias substanciais no seu manejo e investigação.

ABSTRACT Background: Metabolic dysfunction-associated steatotic liver disease (MASLD) is currently the most prevalent cause of chronic hepatic disease worldwide. Recently, the association between MASLD and renal injury has emerged as an additional factor impacting the clinical course of MASLD. Objective: The present study evaluated the clinical association in patients with obesity. Methods: This study enrolled patients classified as having obesity class II and III (BMI >35 kg/m2) and MASLD from an obesity surgical treatment center. The diagnosis criteria for MASLD included the presence of hepatic steatosis as indicated by histology or imaging assessments. We use Fibrosis-4 (FIB-4) and NAFLD fibrosis score (NSF) to assess and determine the presence of liver fibrosis. The glomerular filtration rate (GRF) was determined using CKD-EPI (chronic kidney disease epidemiology collaboration) equation, with GFR levels ≥90 and <120 mL/min/1,73 m2 considered within the normal range. Results: The study comprised a total of 560 individuals with obesity grade II and III, 325 individuals with MASLD. Among these, 422 (75.4%) patients were female, and the mean age was 36±10 years. Systemic arterial hypertension (SAH) was present in 162 (41.1%) patients, and 218 (42.8 %) were diagnosed with type 2 Diabetes Mellitus (T2DM). A total of 286 individuals (51.1%) had a GFR below 114 mL/min, with 183 (64%) of them exhibiting a higher degree of liver fibrosis, as indicated by FIB-4 >0.54. Conclusion: In patients with obesity classified as grades II and III, age emerged as the primary determinant leading to decline in GFR. Furthermore, glomerular hyperfiltration could be an early sign of progression to chronic kidney disease. Nonetheless, the progression of hepatic fibrosis could also be a significant factor contributing to impaired renal function.

RESUMO Contexto: A Doença hepática gordurosa associada à disfunção metabólica (DHGADM) é atualmente a causa mais prevalente de doença hepática crônica no mundo. Recentemente, a associação entre DHGADM e injúria renal surgiu como um fator adicional que impacta o curso clínico da DHGADM. Objetivo: O presente estudo avaliou a associação clínica em pacientes com obesidade. Métodos: Este estudo incluiu pacientes classificados com obesidade de classe II e III (IMC >35 kg/m²) e DHGADM, de um centro de tratamento cirúrgico para obesidade. Os critérios diagnósticos para DHGADM incluíram a presença de esteatose hepática indicada por histologia ou exames de imagem. Utilizamos o Fibrosis-4 (FIB-4) e o escore de fibrose NAFLD (NSF) para avaliar e determinar a presença de fibrose hepática. A taxa de filtração glomerular (TFG) foi determinada usando a equação CKD-EPI (chronic kidney disease epidemiology collaboration), com níveis de TFG ≥90 e <120 mL/min/1,73 m² considerados dentro do intervalo normal. Resultados: O estudo incluiu um total de 560 indivíduos com obesidade de grau II e III, sendo 325 com DHGADM. Entre estes, 422 (75,4%) eram mulheres, e a idade média foi de 36±10 anos. Hipertensão arterial sistêmica (HAS) estava presente em 162 (41,1%) pacientes, e 218 (42,8%) foram diagnosticados com Diabetes Mellitus tipo 2 (DM2). Um total de 286 indivíduos (51,1%) apresentou TFG abaixo de 114 mL/min, com 183 (64%) deles exibindo um grau mais elevado de fibrose hepática, conforme indicado por FIB-4 >0,54. Conclusão: Em pacientes com obesidade classificado como graus II e III, a idade surgiu como o principal determinante para o declínio na TFG. Além disso, a hiperfiltração glomerular pode ser um sinal precoce de progressão para doença renal crônica. No entanto, a progressão da fibrose hepática também pode ser um fator significativo que contribui para a função renal comprometida.

RESUMO Contexto: O supercrescimento bacteriano no intestino delgado (SIBO) é uma condição caracterizada por um aumento anormal da população bacteriana no intestino delgado, levando a sintomas como dor e inchaço abdominal, diarreia e, eventualmente, má absorção. O diagnóstico e o manejo do SIBO permanecem desafiadores devido à sobreposição de sintomas com outros distúrbios gastrointestinais, como doença inflamatória intestinal (DII), síndrome do intestino irritável (SII) e doença celíaca. Objetivo: Este artigo tem como objetivo revisar as evidências atuais sobre o diagnóstico e o tratamento do SIBO, com foco em estratégias adequadas ao sistema de saúde brasileiro. Métodos: Foi realizada uma revisão abrangente da literatura, com ênfase em diretrizes clínicas, ensaios clínicos randomizados e estudos de coorte relacionados ao SIBO. Métodos diagnósticos, incluindo testes respiratórios e técnicas de aspiração direta, foram analisados criticamente. Abordagens terapêuticas, como antibióticos, modificações dietéticas e probióticos, foram revisadas. As recomendações foram formuladas com base em um painel de gastroenterologistas, membros da Federação Brasileira de Gastroenterologia (FBG), com aprovação da maioria dos membros. Resultados: Os testes respiratórios com glicose e lactulose continuam sendo as ferramentas diagnósticas não invasivas mais utilizadas, embora apresentem limitações como falsos positivos e falsos negativos. O tratamento com rifaximina é eficaz na maioria dos casos de SIBO, enquanto antibióticos sistêmicos como metronidazol e ciprofloxacino são alternativas. Probióticos e intervenções dietéticas, especialmente dietas com baixo teor de FODMAP, podem complementar a terapia antibiótica. O acompanhamento de longo prazo é essencial devido à alta taxa de recorrência, que é comum em pacientes com SIBO. Conclusão: Padronizar o diagnóstico e o tratamento do SIBO no Brasil é essencial para reduzir atrasos diagnósticos e otimizar os cuidados, especialmente considerando as disparidades e a heterogeneidade na prática clínica no país. Este artigo apresenta recomendações baseadas em evidências para orientar a prática clínica. Mais pesquisas são necessárias para aprimorar os métodos diagnósticos, explorar novas estratégias de tratamento e compreender melhor as características específicas da população brasileira.

ABSTRACT Background: Small intestinal bacterial overgrowth (SIBO) is a condition characterized by an abnormal increase in bacterial population in the small intestine, leading to symptoms such as bloating, abdominal pain, distension, diarrhea, and eventually malabsorption. The diagnosis and management of SIBO remain challenging due to overlapping symptoms with other gastrointestinal disorders such as inflammatory bowel disease (IBD), irritable bowel syndrome (IBS), and coeliac disease. Objective: This article aims to review current evidence on the diagnosis and treatment of SIBO, with a focus on strategies suitable for the Brazilian healthcare system. Methods: A comprehensive literature review was performed, focusing on clinical guidelines, randomized controlled trials, and cohort studies concerning SIBO. Diagnostic methods, including breath tests and direct aspiration techniques, were critically analyzed. Treatment approaches, including antibiotics, dietary modifications, and probiotics, were reviewed. The recommendations were formulated based on a panel of gastroenterologists, members of the Brazilian Federation of Gastroenterology (FBG), with approval from the majority of the members. Results: Breath tests using glucose and lactulose remain the most commonly used non-invasive diagnostic tools, though they are subject to limitations such as false positives and false negatives. Treatment with rifaximin is effective in most cases of SIBO, while systemic antibiotics like metronidazole and ciprofloxacin are alternatives. Probiotics and dietary interventions, particularly low FODMAP diets, can complement antibiotic therapy. Long-term follow-up is essential due to the recurrence rate, which is common in SIBO patients. Conclusion: Standardizing SIBO diagnosis and treatment in Brazil is essential to reduce diagnostic delays and optimize care, especially given the disparities and heterogeneity in clinical practice across the country. This article provides evidence-based recommendations to guide clinical practice. Further research is needed to refine diagnostic methods, explore novel treatment strategies, and better understand the specific characteristics of the Brazilian population.

RESUMO Contexto: A colelitíase, caracterizada por depósitos endurecidos na vesícula biliar, apresenta sintomas como dor abdominal, icterícia, náuseas e complicações potenciais como colecistite e coledocolitíase. Apesar do aumento dos diagnósticos, a literatura sobre colelitíase pediátrica é limitada, com protocolos indefinidos. Objetivo: Este estudo tem como objetivo avaliar as características clínicas, laboratoriais e desfechos dos casos de colelitíase pediátrica, identificando fatores associados à colecistectomia. Métodos: Foi realizado um estudo retrospectivo de série de casos em pacientes tratados em um serviço terciário, diagnosticados com colelitíase por ultrassonografia de 2007 a 2021. Perfis clínicos, comorbidades, exames, procedimentos e evolução dos pacientes foram avaliados. Os pacientes foram categorizados em dois grupos: Grupo NC (sem colecistectomia) e Grupo C (colecistectomia). Resultados: Trinta e cinco pacientes foram incluídos, com 51% do sexo feminino e 60% com comorbidades, sendo a dor abdominal foi o sintoma predominante. 33 pacientes foram tratados ambulatorialmente, enquanto que 2 seguiram o acompanhamento em outro serviço. Doze optaram por manejo expectante (Grupo NC), enquanto 21 foram submetidos à colecistectomia (Grupo C). Foi realizada colecistectomia laparoscópica eletiva no Grupo C, com idade mediana de 11 anos e 3 meses. O Grupo C apresentou maior frequência de dor abdominal quando comparado ao Grupo NC, sendo esta diferença significativa (p=0,04). Não foram observadas disparidades quanto ao sexo, comorbidades, icterícia, febre, achados laboratoriais, duração dos sintomas, tempo de acompanhamento ou idade no diagnóstico. O tempo mediano de acompanhamento no Grupo NC foi de 1 ano e 7 meses. Conclusão: A dor abdominal foi o sintoma predominante nos pacientes submetidos à colecistectomia, enquanto comorbidades e anormalidades laboratoriais não apresentaram associações significativas. Embora a intervenção cirúrgica seja tipicamente recomendada, o manejo expectante mostrou-se viável em casos selecionados sem complicações subsequentes durante o período de avaliação.

ABSTRACT Background: Cholelithiasis, characterized by hardened deposits in the gallbladder, presents symptoms such as abdominal pain, jaundice, nausea, and potential complications like cholecystitis and choledocholithiasis. Despite increasing diagnoses, literature on pediatric cholelithiasis is limited, with undefined protocols. Objective: This study aims to evaluate the clinical, laboratory characteristics, and outcomes of pediatric cholelithiasis cases, identifying factors associated with cholecystectomy. Methods: A retrospective case series study was conducted on patients treated at a tertiary service, diagnosed with cholelithiasis via ultrasound from 2007 to 2021. Clinical profiles, comorbidities, examinations, procedures, and patient evolution were assessed. Patients were categorized into two groups: Group NC (no cholecystectomy) and Group C (cholecystectomy). Results: Thirty-five patients were included, with 51% females and 60% having comorbidities with abdominal pain was the predominant symptom. Thirty-three patients were managed outpatient while two patients continued follow-up at another facility. Twelve opted for expectant management (Group NC), while 21 underwent cholecystectomy (Group C). Elective laparoscopic cholecystectomy was performed in Group C, with a median age of 11 years and 3 months. Group C showed a higher frequency of abdominal pain compared to Group NC, and this difference was significant (P=0.04). No differences were observed in gender, comorbidities, jaundice, fever, laboratory findings, symptom duration, follow-up time, or age at diagnosis. The median follow-up duration in Group NC was 1 year and 7 months. Conclusion: Abdominal pain was the predominant symptom in patients undergoing cholecystectomy, while comorbidities and laboratory abnormalities showed no significant associations. Although surgical intervention is typically recommended, expectant management proved viable in select cases without ensuing complications during the evaluation period.

RESUMO Contexto: A diarreia é um distúrbio do trânsito gastrointestinal e, segundo relatório da OMS, é frequentemente observada em crianças desnutridas. Indivíduos desnutridos apresentam imunidade comprometida e falta de nutrientes, tornando-os mais suscetíveis à diarreia. Objetivo: Para manter a saúde intestinal, uma nutrição adequada e equilibrada é essencial. Neste estudo, foi formulado e avaliado um suplemento nutricional, fermentado e não fermentado, contra a diarreia induzida por óleo de rícino. Métodos: Dois grupos de ratos foram inicialmente alimentados com uma dieta deficiente em proteínas (2%) por dez semanas. Após esse período, um grupo recebeu uma dieta enriquecida com componentes nutricionais misturados com colostro bovino fermentado, enquanto o outro grupo recebeu uma dieta com componentes nutricionais não fermentados. Ao final da 20ª semana, foi administrado óleo de rícino a todos os animais, exceto ao grupo controle, para induzir diarreia. Subsequentemente, esses ratos foram submetidos a várias avaliações, incluindo o tempo de início do primeiro episódio de fezes diarreicas (em minutos), peso fecal, escore fecal, número de fezes úmidas, acúmulo de fluidos intestinais e exame histopatológico. A atividade sequestradora de radicais DPPH da mistura nutricional também foi determinada. Resultados: Os ratos desnutridos alimentados com dietas fermentadas e não fermentadas mostraram atraso no início da diarreia, redução do peso das fezes, diminuição na frequência e gravidade da defecação, bem como proteção significativa contra o acúmulo de fluidos intestinais em comparação aos grupos controle negativo. Os resultados mostraram que ambas as composições misturadas, fermentadas e não fermentadas, exibiram atividade antioxidante. O intestino dos ratos desnutridos alimentados com a dieta nutricional fermentada mostrou ausência de infiltração e melhora na estrutura das vilosidades. Conclusão: O estudo apresenta evidências promissoras dos benefícios potenciais das composições nutricionais formuladas no alívio da diarreia induzida por óleo de rícino em ratos Wistar desnutridos. A atividade antioxidante, os efeitos antidiarreicos e as melhorias na histologia intestinal sugerem que essas composições nutricionais podem ser exploradas como intervenções naturais para a saúde gastrointestinal.

ABSTRACT Background: Diarrhea is a gastrointestinal transit disorder and mostly seen in malnourished children’s as per WHO report. Malnourished individuals are found to be associated with compromised immunity and lack of nutrients, which makes person susceptible to diarrhoea. Objective: For maintaining the gut health adequate and balance nutrition is essential. In this study, both fermented and non-fermented nutritional supplement was formulated and evaluated against castor-oil induced diarrhoea. Methods: Two groups of rats initially fed a 2% protein-deficient diet for ten weeks. After this period, one group received a diet enriched with nutritional components blended with fermented bovine colostrum, while the other group received a diet with non-fermented nutritional components. At the end of 20th week, castor oil was given to all animals except the control group to induce diarrhoea. Subsequently, these rats were subjected to various assessments including time of onset of the first diarrheal stool (min), faecal weight, faecal score, number of wet stools, intestinal fluid accumulation and Histopathological examination. DPPH radical-scavenging activity of nutritional blend was also determined. Results: The undernourished rats fed with non-fermented and fermented diet showed delayed the onset of diarrhea and reduction of weight stool, the decrease in the frequency and severity of defecation as well as significantly protected against the intestinal fluid accumulation as compare to negative control groups. The results showed that both the fermented and non-fermented blended composition exhibited antioxidant activity. The intestine of undernourished rats fed with fermented nutritional diet showed the absence of infiltration and improved villi structure. Conclusion: The study presents promising evidence of the potential benefits of the formulated nutritional compositions in alleviating the castor oil-induced diarrhea in undernourished wistar rats. The antioxidant activity, anti-diarrheal effects and improvements in gut histology suggest that, the nutritional compositions could be explored further as natural interventions for gastrointestinal health.

RESUMO Contexto: A história natural da cirrose é caracterizada por uma fase assintomática (cirrose compensada), seguida por uma fase rapidamente progressiva (cirrose descompensada). A capacidade de prever a sobrevida de pacientes com cirrose é crucial para a tomada de decisões, algumas tão complexas quanto a indicação para transplante hepático. Vários modelos foram desenvolvidos e validados. Objetivo: Analisar e comparar o desempenho de modelos na predição da mortalidade em 90 dias entre pacientes hospitalizados com cirrose descompensada. Métodos: Amostra de 481 pacientes hospitalizados, com média de idade de 59,04 anos, 73% do sexo masculino, diagnosticados com cirrose descompensada e uma média de pontuação Child-Pugh de 9. Os modelos prognósticos foram calculados com base em exames realizados na admissão: MELD-Na, MELD-Plus, MELD 3.0, ReMELD, Refit MELD e Refit MELD-Na. A acurácia dos modelos foi avaliada pelo cálculo da área sob a curva de operadores recebedores de características (AUROC), com seus respectivos intervalos de confiança de 95%. As comparações entre as áreas foram realizadas por meio do teste de DeLong. Foi realizada uma comparação entre todos os escores, com foco principal no MELD 3.0 e MELD-Plus. Estes escores específicos foram os principais pontos de interesse. Resultados: Os escores apresentaram valores de AUROC entre 0,703 e 0,758, indicando uma capacidade moderada de discriminar entre sobreviventes e pacientes falecidos no período analisado. A comparação entre os modelos não estabeleceu, de forma inequívoca, a superioridade de um modelo sobre os demais. Conclusão: Os escores apresentam capacidade preditiva limitada para morte em 90 dias em pacientes com cirrose descompensada. Nosso estudo não foi capaz de estabelecer a superioridade prognóstica de um escore específico.

ABSTRACT Background: The natural history of cirrhosis is characterized by an asymptomatic phase (compensated cirrhosis) followed by a rapidly progressive phase (decompensated cirrhosis). The ability to predict the survival of patients with cirrhosis is crucial for decision-making, some as complex as the indication for a liver transplant. Several models have been developed and validated. Objective: To analyze and compare the performance of models in predicting 90-day mortality among patients hospitalized with decompensated cirrhosis. Methods: A sample of 481 hospitalized patients, with a mean age of 59.04 years 73% male, diagnosed with decompensated cirrhosis and a mean Child-Pugh score of 9. The prognostic models were calculated based on tests performed on admission: MELD-Na, MELD-Plus, MELD 3.0, ReMELD, Refit MELD, and Refit MELD-Na. The accuracy of the models was assessed by calculating the area under the receiver operating characteristic (AUROC) curve, and their respective 95% confidence intervals. Comparisons between the areas were conducted using the DeLong test. A comparison was conducted among all scores, with a primary focus on MELD 3.0 and MELD-Plus. These specific scores were the focal points of interest. Results: The scores presented AUROC curve values of 0.703-0.758, indicating a moderate capacity to discriminate between survivors and deceased patients during the considered period. The comparison between the models did not unequivocally establish the superiority of one model over the other. Conclusion: The scores have a limited predictive ability for death within 90 days in patients with decompensated cirrhosis. Our study is unable to establish the prognostic superiority of a specific scoring system.

RESUMO A sarcopenia é uma síndrome caracterizada pela diminuição da força, quantidade e/ou qualidade da massa muscular esquelética. Quando associada ao câncer, ela correlaciona-se a piores resultados clínicos. Os cânceres do trato gastrointestinal, prevalentes globalmente e no Brasil, estão associados a um maior risco nutricional. É fundamental que haja detecção e intervenção nutricional precoces nessa população. Estudos recentes sobre cúrcuma/curcumina demonstraram efeitos benéficos em pacientes com câncer. Especificamente, a curcumina mostrou-se promissora na redução da depleção muscular, estresse oxidativo e melhora da força e fadiga, fatores relacionados à sarcopenia. Esta revisão visa elucidar a sarcopenia e a sarcopenia secundária ao câncer, enfatizando o manejo nutricional e o papel da suplementação de curcumina. O manejo eficaz do câncer, com ou sem sarcopenia, exige estratégias abrangentes de saúde pública e intervenções multimodais dentro das instituições de saúde. A nutrição é fundamental em toda a jornada de tratamento do câncer, abrangendo triagem, orientação e fornecimento de nutrientes que apoiam a manutenção ou recuperação da composição corporal. A suplementação de curcumina surge como um potencial adjuvante ao tratamento padrão contra o câncer e no manejo da sarcopenia. No entanto, mais estudos clínicos são necessários para consolidar essas descobertas.

ABSTRACT Sarcopenia is a syndrome characterized by decreased strength, quantity and/or quality of skeletal muscle mass. When associated with cancer, it correlates with poorer clinical outcomes. Cancers of the gastrointestinal tract, prevalent globally and in Brazil, are associated with a greater nutritional risk. Early detection and intervention for nutritional risks are critical in this population. Recent studies on turmeric/curcumin have demonstrated beneficial effects in cancer patients. Specifically, curcumin have shown promise in reducing muscle depletion, oxidative stress, and improving strength and fatigue, factors related to sarcopenia. This review aims to elucidate sarcopenia and sarcopenia secondary to cancer, emphasizing nutritional management and the role of curcumin supplementation. Effective cancer management, whether with or without sarcopenia, demands comprehensive public health strategies and multimodal interventions within healthcare institutions. Nutrition is pivotal across the cancer care journey, encompassing screening, guidance, and provision of nutrients that support maintaining or recovering body composition. Curcumin supplementation emerges as a potential adjuvant to the standard cancer treatment and sarcopenia management. Nevertheless, further clinical studies are warranted to substantiate these findings.

RESUMO Contexto: A esteato-hepatite não alcoólica (EHNA) está se tornando uma das principais causas de doença hepática nos EUA, enquanto a artrite reumatoide (AR) afeta uma parcela significativa da população global. Nos últimos tempos, novos medicamentos foram desenvolvidos para retardar a progressão da AR, um dos quais é a hidroxicloroquina (HCQ). Apesar de a HCQ estar ligada à transamina lentamente progressiva nossa pesquisa preenche essa lacuna examinando a prevalência e os fatores de risco do desenvolvimento de EHNA em pacientes com AR em HCQ. Métodos: Este estudo de coorte retrospectivo analisou 619.350 pacientes adultos diagnosticados com AR. Os dados foram obtidos de um banco de dados multicêntrico que abrange mais de 360 hospitais em 26 sistemas de saúde nos EUA de 1999 a setembro de 2022, excluindo grávidas. A análise de regressão multivariada avaliou o risco de EHNA, ajustando para fatores de confusão, incluindo história de tabagismo, sexo masculino, dislipidemia, hipertensão, diabetes mellitus tipo 2, obesidade e uso de hidroxicloroquina. A significância estatística foi estabelecida em P<0,05, com análises conduzidas usando o R versão 4.0.2 (R Foundation for Statistical Computing, Viena, Áustria, 2008). Resultados: Em uma coorte de 79,4 milhões de indivíduos, 619.350 indivíduos não grávidas tinham AR, com 3.080 diagnosticados com EHNA, enquanto 616.270 não. Pacientes com EHNA apresentaram maior prevalência de história de tabagismo, hiperlipidemia, hipertensão, diabetes mellitus tipo 2, obesidade e uso de HCQ. A análise de regressão multivariada identificou aumento do risco de EHNA em fumantes (OR: 1,24; IC95%: 1,14-1,36), homens (OR: 0,88; IC95%: 0,81-0,96), indivíduos com dislipidemia (OR: 1,34; IC95%: 1,21-1,47), hipertensão (OR: 1,11; IC95%: 1,00-1,27), diabetes mellitus tipo 2 (OR: 3,24; IC95%: 2,98-3,54), obesidade (OR: 3,59; IC95%: 3,31-3,89) e uso de hidroxicloroquina (OR: 1,79; IC95%: 1,65-1,94). Conclusão: Pacientes com AR em HCQ apresentaram aumento da prevalência e chances de desenvolver EHNA, mesmo após ajuste para fatores de confusão comuns. Isso indica que a HCQ pode desempenhar um papel no desenvolvimento de esteatose hepática e fibrose. Os médicos devem considerar essa associação para prevenir doença hepática avançada. Pesquisas futuras devem se concentrar na triagem ideal para detecção precoce e melhorar os resultados dos pacientes.

ABSTRACT Background: Non-alcoholic steatohepatitis (NASH) is becoming a leading cause of liver disease in the US, while Rheumatoid arthritis (RA) affects a significant portion of the global population. In recent times, newer drugs have been developed to slow down the progression of RA, one of which is hydroxychloroquine (HCQ). Despite HCQ being linked to slowly progressive transaminitis, its role in the development of NASH remains unclear. Our research fills this gap by examining the prevalence and risk factors of developing NASH in patients with RA on HCQ. Methods: This retrospective cohort study analyzed 619,350 adult patients diagnosed with RA. Data were sourced from a multicenter database covering over 360 hospitals across 26 healthcare systems in the US from 1999 to September 2022, excluding pregnant individuals. Multivariate regression analysis assessed the risk of NASH, adjusting for confounders including smoking history, male gender, dyslipidemia, hypertension, type 2 diabetes mellitus, obesity, and hydroxychloroquine use. Statistical significance was set at P<0.05, with analyses conducted using R version 4.0.2 (R Foundation for Statistical Computing, Vienna, Austria, 2008). Results: In a cohort of 79.4 million individuals, 619,350 non-pregnant subjects had rheumatoid arthritis, with 3,080 diagnosed with NASH, while 616,270 did not. Patients with NASH displayed a higher prevalence of smoking history, hyperlipidemia, hypertension, type 2 diabetes mellitus, obesity, and HCQ use. Multivariate regression analysis identified increased NASH risk in smokers (OR: 1.24; 95%CI: 1.14-1.36), males (OR: 0.88; 95%CI: 0.81-0.96), individuals with dyslipidemia (OR: 1.34; 95%CI: 1.21-1.47), hypertension (OR: 1.11; 95%CI: 1.00-1.27), type 2 diabetes mellitus (OR: 3.24; 95%CI: 2.98-3.54), obesity (OR: 3.59; 95%CI: 3.31-3.89), and hydroxychloroquine use (OR: 1.79; 95%CI: 1.65-1.94). Conclusion: RA patients on HCQ showed an increased prevalence and odds of developing NASH, even after adjusting for common confounding factors. This indicates that HCQ may play a role in the development of hepatic steatosis and fibrosis. Clinicians should consider this association to prevent advanced liver disease. Future research should focus on optimal screening for early detection and enhancing patient outcomes.

ABSTRACT Background: Patients diagnosed with Crohn’s disease (CD) are at high risk of nutritional impairment due to the symptoms and the intense inflammatory response of the disease. The use of indirect calorimetry (IC) to assess resting energy expenditure (REE) proves to be a valuable alternative for more accurately determining the energy requirements of these patients. Objective: The primary objective of this study was to compare the REE, increased by 20% (to account for diet-induced thermogenesis and daily energy expenditure), as measured by IC in patients at different stages of CD, with the total energy expenditure (TEE) calculated using the Simple Weight-Based Equation (30 kcal/kg and 35 kcal/kg). Methods: Sessions for measuring REE were conducted using IC, along with the collection of clinical, biochemical, and anthropometric data. The clinical activity of CD was classified using the Harvey-Bradshaw Index (HBI), while endoscopic classification was assessed through the Simple Endoscopic Score in CD (SES-CD). Results: A total of 60 adult patients diagnosed with CD in different disease phases were randomly evaluated, with 56.7% being male and 43.3% female, and a mean age of 39 years. The majority (76.7%) were of caucasian ethnicity, and 48.3% had completed high school. According to body mass index (BMI), 21.7% were classified as undernourished, 48.3% as eutrophic, 16.7% as overweight, and 13.3% as obese. Regarding disease activity classification based on the HBI, 50% were in the active phase and 50% in remission. Endoscopic classification revealed that 60% had findings indicative of active disease. The patients evaluated were diagnosed with Crohn’s disease (CD) at an average age of 28.2 years, with the majority presenting ileocolonic involvement (61.8%) and a stenosing behavior (45.5%). Regarding biochemical serum analysis, the average values found were 12.5 g/dL for hemoglobin, 38.7% for hematocrit, and 15.7 mg/L for C-reactive protein. 58.3% of patients did not have fistulas. No agreement was found between the energy expenditure results measured by the weight-based formulas (30 kcal/kg and 35 kcal/kg) and IC (ICC <0.4), with the values obtained by simple weight-based equations being higher than those from IC. The result obtained using 30 kcal/kg showed slightly greater concordance with IC, but still with low agreement. In isolation, energy expenditure in male patients was statistically higher than in female patients. There was a statistically significant direct correlation between energy expenditure and hemoglobin levels, as well as statistically significant indirect correlations with age and age at diagnosis. The difference in energy expenditure between the methods was indirectly correlated with age, BMI, and age at diagnosis. No statistically significant correlations were found between energy expenditure and the behavior, location, or activity of Crohn’s disease. Conclusion: This study suggests that the use of IC would be more beneficial for younger patients, with lower BMI, or those diagnosed at a younger age, given the greater discrepancy between the methods evaluated in these cases. For other patients, the 30 kcal/kg weight-based formula remains a practical and more accessible option. However, it is undeniable and increasingly recognized that IC provides a more accurate assessment of energy requirements in different clinical conditions, and when used appropriately, it can enhance nutritional support and care.

RESUMO Contexto: Os pacientes diagnosticados com doença de Crohn (DC) apresentam alto risco de comprometimento do estado nutricional em decorrência dos sintomas e da intensa resposta inflamatória da doença e a utilização da calorimetria indireta (CI) para a aferição do gasto energético de repouso (GER) se mostra uma boa alternativa para melhor quantificar as necessidades energéticas desses pacientes. Objetivo: O principal objetivo desse estudo foi comparar o GER, acrescido de 20% (termogênese da dieta e dispêndio energético diário), mensurado pela CI, dos pacientes nas diferentes fases da DC, com o Gasto Energético Total (GET) calculado pela Fórmula de Bolso (FB) (30 kcal/kg e 35 kcal/kg). Métodos: Foram realizadas sessões de aferição do GER, utilizando a CI, além da coleta de dados clínicos, bioquímicos e medidas antropométricas. Para classificação de atividade clínica da DC foi utilizado o Índice de Harvey-Bradshaw (HBI), e a classificação endoscópica foi mensurada através do Simple Endoscopic Score in CD (SES-CD). Resultados: Foram avaliados ao acaso, 60 pacientes adultos diagnosticados com DC nas diferentes fases da doença, 56,7% do sexo masculino e 43,3% do sexo feminino, com idade média de 39 anos, sendo a maioria (76,7%) brancos e possuíam o ensino médio completo (48,3%). De acordo com o índice de massa corporal (IMC), 21,7% foram classificados em desnutrição, 48,3% em eutrofia, 16,7% em sobrepeso e 13,3% em obesidade. Quanto a classificação de atividade clínica da doença segundo o HBI, 50% estavam em atividade e 50% em remissão. Para classificação endoscópica, 60% possuíam achados de atividade de doença. Os pacientes avaliados receberam o diagnóstico de DC em média aos 28,2 anos, em sua maioria com acometimento ileocolônico (61,8%) e comportamento estenosante (45,5%). Em relação aos resultados de análises bioquímicas séricas, foi encontrado um valor médio de 12,5 g/dL para hemoglobina, 38,7% para hematócrito e 15,7 mg/L para proteína C reativa. 58,3% dos pacientes não possuíam fístulas. Não foi encontrada concordância entre os resultados do gasto energético aferido pela FB a 30 kcal/kg e 35 kcal/kg com a CI (CCI < 0,4), sendo os valores encontrados pela FB maiores que os da CI. O resultado da FB a 30 kcal/kg foi um pouco mais concordante com a CI, mas ainda com concordância baixa. Isoladamente, o gasto energético nos homens foi estatisticamente maior que nas mulheres. Houve correlação direta estatisticamente significativa do gasto energético com a hemoglobina e correlações indiretas estatisticamente significativas com a idade e a idade de diagnóstico. A diferença no gasto energético entre os métodos foi indiretamente correlacionada com a idade, IMC e idade no diagnóstico. Não foram encontradas correlações estatisticamente significativas entre o gasto energético e comportamento, localização e atividade da DC. Conclusão: Este estudo sugere que o uso da CI seria mais bem aproveitado em pacientes jovens, com menor IMC ou diagnosticados mais novos, considerando a maior diferença entre os métodos avaliados para estes casos. Para os demais pacientes a FB a 30 kcal/kg continua sendo uma opção prática e mais acessível. Entretanto, é inegável e cada vez mais reconhecido que a CI fornece uma avaliação mais precisa da demanda energética nas diferentes condições clínicas e, quando usada adequadamente, pode melhorar o suporte e cuidado nutricional.

ABSTRACT Background: Acute pancreatitis (AP) is a common gastrointestinal disorder, with organ failure being the primary cause of mortality. This study evaluates the combined score of C-reactive protein level and neutrophil-to-lymphocyte ratio (C-NLR score), as a predictor of organ failure. Methods: A retrospective analysis was conducted on AP patients admitted to the Hospital Clínico Universitario de Valladolid between March 2014 and January 2022. The C-NLR score was calculated from admission data: patients with both elevated neutrophil-to-lymphocyte ratio (NLR) and C-reactive protein (CRP) were assigned a score of 2, those with one elevated marker received a score of 1, and a score of 0 was given to those with neither. Results: A total of 778 patients were included: 611 with mild AP (MAP), 123 with moderately severe AP (MSAP), and 44 with severe AP (SAP). A C-NLR score of 2 was most frequently observed in patients with MSAP (56.1%) and SAP (56.85%), while a score of 1 was more common in patients with MAP (46.9%). Multivariable logistic regression analysis revealed that a C-NLR score of 2 significantly increased the likelihood of organ failure by threefold (OR 3.176; 95% CI 1.297-7.775). Conclusion: The C-NLR score could be a useful supplementary tool for predicting organ failure in AP, complementing traditional scoring systems.

RESUMO Contexto: A pancreatite aguda (PA) é um distúrbio gastrointestinal comum, sendo a falência de órgãos a principal causa de mortalidade. Este estudo avalia o escore combinado do nível de proteína C reativa e da relação neutrófilo-linfócito (escore C-NLR), como preditor de falência de órgãos. Métodos: Foi realizada uma análise retrospectiva de pacientes com PA internados no Hospital Clínico Universitário de Valladolid entre março de 2014 e janeiro de 2022. O escore C-NLR foi calculado a partir dos dados de admissão: pacientes com relação neutrófilo-linfócito (NLR) e proteína C reativa (PCR) elevadas receberam uma pontuação de 2, aqueles com um marcador elevado receberam uma pontuação de 1 e uma pontuação de 0 foi dada àqueles sem nenhum dos dois. Resultados: Um total de 778 pacientes foram incluídos: 611 com PA leve (PAM), 123 com PA moderadamente grave (PASM) e 44 com PA grave (PAS). Um escore C-NLR de 2 foi mais frequentemente observado em pacientes com PASM (56,1%) e PAS (56,85%), enquanto um escore de 1 foi mais comum em pacientes com PAM (46,9%). A análise de regressão logística multivariada revelou que uma pontuação C-NLR de 2 aumentou significativamente a probabilidade de falência de órgãos em três vezes (OR 3,176; IC 95% 1,297-7,775). Conclusão: O escore C-NLR pode ser uma ferramenta complementar útil para predizer falência de órgãos na PA, complementando os sistemas de pontuação tradicionais.
