Abstract Objective: To describe the process of translation into Brazilian Portuguese and cross-cultural adaptation of the French Reflux Symptom Score-12 questionnaire used for the diagnosis of laryngopharyngeal reflux. Methods: This was across-cultural translation and adaptation study of a health instrument, with a cross-sectional design. It was carried out in eight stages: translation from French into Brazilian Portuguese, cultural adaptation by a panel of experts, application of the first version (pilot test 1), adaptation by a panel of experts, application of the second version (pilot test 2), back translation, reviewing by a committee in conjunction with the author of the original instrument and, application of the final version. The Brazilian Portuguese versions of the questionnaire were applied to individuals with symptoms and signs of laryngopharyngeal reflux who underwent pHmetry and esophageal manometry at the study site. Results: In pilot test 1, the first version of the RSS-12 in Brazilian Portuguese was applied to 30 patients. The patients had no difficulty to understand any of the 12 symptom items, but 15 patients (50%) had difficulty interpreting the symptom frequency score. After adapting the format of the frequency score, a version 2 of the RSS-12 in Brazilian Portuguese was applied to another 23 patients, who completed the questionnaire in full without any difficulty. Along with the review committee, the author of the original RSS-12 considered the version 2 to be adequate and did not propose any changes, so it was approved as the final version of the Brazilian Portuguese RSS-12. Conclusion: The Brazilian Portuguese version of the instrument, called Reflux Symptom Score-12 PT-BR, shows good understanding and linguistic, conceptual and content equivalence, in relation to the original Reflux Symptom Score-12.

ABSTRACT The aim of this study was to characterize the growth and nutrient absorption of ‘Debora Victory’ tomato grown in organic system. The treatments consisted of nine sampling dates, at 2, 32, 46, 60, 77, 91, 109, 122 and 137 days after transplantation (DAT). In the last sampling, plants reached an estimated accumulation of dry and fresh weight in plant shoots of 550 and 9,528 g/plant, respectively. At the end of the cycle, the distribution of dry matter was 73% in the fruits, 16% in the leaves and 11% in the stem. The yield of total fresh fruits was estimated at 154.7 t/ha. The decreasing order of nutrient accumulation in plant shoots was K>N>Ca>P>S>Mg, with estimated values of 22.6; 10.4; 5.0; 2.3; 2.1 and 1.6 g/plant, respectively, corresponding to 431; 198; 95; 44; 40 and 30 kg/ha of K, N, Ca, P, S and Mg, respectively. The decreasing order of nutrient accumulation in the fruits was K>N>P>S>Mg>Ca, with estimated values of 16.6; 8.4; 1.7; 0.8; 0.7 and 0.2 g/plant, respectively, which corresponded to an estimated total extraction of 315; 153; 32; 16; 14 and 3 kg/ha of K, N, P, S, Mg and Ca, respectively. In the last sampling, the greatest accumulation of N, P and K occurred in the fruits and Ca, S and Mg in the vegetative organs (stem + leaves).
RESUMO O objetivo desta pesquisa foi caracterizar o crescimento e absorção de nutrientes pelo tomateiro ‘Debora Victory’ cultivado em sistema orgânico. Os tratamentos constituíram-se por nove épocas de amostragem, aos 2, 32, 46, 60, 77, 91, 109, 122 e 137 dias após o transplante (DAT). As plantas atingiram acúmulo estimado de massa seca e fresca na parte aérea na última amostragem de 550 e 9528 g/planta, respectivamente. Ao final do ciclo, a distribuição da massa seca foi de 73% nos frutos, 16% nas folhas e 11% no caule. A produtividade de frutos frescos totais foi estimada em 154,7 t/ha. A ordem decrescente do acúmulo de nutrientes na parte aérea das plantas foi K>N>Ca>P>S>Mg, com valores estimados de 22,6; 10,4; 5,0; 2,3; 2,1 e 1,6 g/planta, respectivamente, correspondendo a 431; 198; 95; 44; 40 e 30 kg/ha de K, N, Ca, P, S e Mg, respectivamente. A ordem decrescente do acúmulo de nutrientes nos frutos foi K>N>P>S>Mg>Ca, com valores estimados de 16,6; 8,4; 1,7; 0,8; 0,7 e 0,2 g/planta, respectivamente, o que correspondeu a uma extração estimada total de 315; 153; 32; 16; 14 e 3 kg/ha de K, N, P, S, Mg e Ca, respectivamente. Na última amostragem, o maior acúmulo de N, P e K ocorreu nos frutos e de Ca, S e Mg na parte vegetativa (caule + folhas).

Abstract During a survey of mucoralean fungi in soil from an upland forest area located in Pernambuco, Brazil, a strain of Backusella (URM 8637) was isolated. Based on morphological, physiological, and molecular data [internal transcribed spacer (ITS) and large subunit (LSU) ribosomal DNA regions], it was recognized that this Backusella differed from all other species in the genus. Morphologically, the new species is characterized as forming varied-shaped columellae, including elongated, basally constricted, unisporate (rare) and multisporate sporangiola, and ellipsoidal sporangiospores. The maximum temperature growth of URM 8637 on malt extract agar and potato dextrose agar was 36 °C. In the phylogram, it was closely related to B. constricta. Based on the evidence from the analyzed datasets, a new species of Backusella is proposed. An updated identification key for Backusella from the Americas is provided.

Alterations on magnetic resonance imaging of the neonatal brain: correlations with prenatal risk factors and transfontanellar ultrasound findings

Resumo Objetivo: Avaliar alterações encontradas nas ressonâncias magnéticas (RMs) encefálicas neonatais, correlacionando com a ultrassonografia transfontanelar (USTF), e descrever os principais fatores de risco encontrados. Materiais e Métodos: Foram avaliados exames de 51 pacientes que realizaram RM utilizando protocolo neonatal durante internação hospitalar, correlacionando com resultados da USTF prévia, sendo utilizada, para minimizar as chances de viés, a última USTF realizada. Os dados foram obtidos de prontuário médico e as imagens foram revisadas por médico radiologista especialista em neuroimagem. Resultados: A população foi composta majoritariamente de recém-nascidos prematuros extremos (21; 41,2%) e de extremo baixo peso (22; 43,1%). Foram encontradas alterações em 16 (31,4%) das USTFs e em 30 (58,8%) das RMs, sendo a hemorragia da matriz germinativa o achado mais frequente. Os valores preditivos positivo e negativo da USTF em relação à RM foram de 87% e 54%, respectivamente. Conclusão: A USTF mostrou-se importante na distinção entre os graus de hemorragia da matriz germinativa leve e moderada (I e II) dos graus acentuados (III e IV), sendo considerada um bom exame de rastreio e acompanhamento, principalmente em pacientes mais graves e com fatores de risco.
Abstract Objective: To describe the alterations seen on magnetic resonance imaging (MRI) of the brain in newborns, correlating those alterations with the transfontanellar ultrasound (TFUS) findings, and to describe the main risk factors identified. Materials and Methods: We evaluated the examinations of 51 patients who were submitted to brain MRI with a neonatal protocol during hospitalization. We evaluated the MRI findings and correlated them with previous TFUS findings, using the last TFUS performed in order to minimize the risk of bias. Data were obtained from medical records, and the images were reviewed by a radiologist specializing in neuroimaging. Results: Of the 51 patients evaluated, 21 (41.2%) were extremely preterm infants and 22 (43.1%) were extremely-low-birth-weight infants. Alterations were seen on 16 (31.4%) of the TFUS examinations and on 30 (58.8%) of the brain MRI scans, the most common finding being germinal matrix hemorrhage. The positive and negative predictive values of TFUS in relation to MRI were 87% and 54%, respectively. Conclusion: Because TFUS proved to be capable of distinguishing mild and moderate (grade I and II) germinal matrix hemorrhage from the severe forms (grades III and IV), it can be considered a good tool for screening and follow-up, especially in infants with severe disease and risk factors.

Resumo Em um levantamento de espécies de fungos Mucorales no Brejo da Serra do Benedito, um Brejo de altitude do semi-árido pernambucano, Brasil, um espécime de Backusella gigacellularis foi isolado. Até o momento, B. gigacellularis (holótipo) só havia sido isolada de solo no Domínio Cerrado, no estado de São Paulo, sudeste do Brasil. Este é segundo registro mundial de B. gigacellularis e o primeiro para o nordeste do Brasil, especificamente em um a área de Brejo de altitude. Nesse trabalho, são apresentadas uma descrição detalhada e ilustração do espécime isolado.
Abstract In a survey mucoralean fungi in the Brejo da Serra do Benedito, a fragment of Upland Forest in the semi-arid region of Pernambuco, Brazil, a specimen of Backusella gigacellularis was isolated. So far, B. gigacellularis (holotype) had only been isolated from soil in the Cerrado domain, in São Paulo state, southeastern Brazil. This is the second report of B. gigacellularis worldwide, and the first record to northeastern Brazil. A detailed description, as well as the illustration of the specimen, are presented.

Resumo Objetivo analisar o processo de apreensão e transformação do uso do brincar e brinquedo pela equipe de enfermagem de uma unidade pediátrica. Método estudo apoiado na pesquisa convergente assistencial, envolvendo rodas de conversas com 11 profissionais de uma equipe de enfermagem, atuantes em uma unidade de internação pediátrica de um hospital de ensino. Foi desenvolvido entre Dezembro de 2018 e Maio de 2019. Resultados Houve reconhecimento do brincar como inerente à criança e benéfico na interação com esta, quando hospitalizada. Os participantes percebem a não apropriação do brincar estruturado em suas práticas, prospectam ampliação, porém identificam entraves associados ao pouco apoio institucional. Dos desdobramentos, decidiram por inserção do fantoche e capacitação para o uso do brincar estruturado. Conclusões e implicações para a prática a inserção do brincar no hospital demanda ruptura com o modelo biomédico em saúde e apostas em uma cultura institucional de reconhecimento do brincar. O brincar ao longo da hospitalização integra um cuidado justo, humano e integral, que deve ser para a equipe de enfermagem motivo de luta e garantia, correspondendo às instituições, o dever de dar suporte, tanto no âmbito dos processos de trabalho, quanto no da educação permanente.
Abstract Objective to analyze the process of apprehension and transformation of the use of play and toys by the Nursing staff of a pediatric unit. Method a study supported by convergent care research, developed between December 2018 and May 2019 with 11 professionals of the Nursing team, through conversation circles. Results the participants recognize playing as inherent to the child and beneficial in the interaction with the child when hospitalized. They perceive the non-appropriation of structured play in their practices, look for expansion, but identify obstacles associated with little institutional support. From the developments, they decided to insert a puppet and get trained to the use structured play. Conclusions and implications for the practice The insertion of play in the hospital demands a break with the biomedical model in health and betting on an institutional culture of the recognition of playing. Playing throughout hospitalization integrates fair, humane and comprehensive care, which should be a reason for the Nursing team to fight and guarantee and, for institutions, the duty to provide support, both within the scope of work and permanent education.
Resumen Objetivo analizar el proceso de aprehensión y transformación del uso de juegos y juguetes por parte del personal de Enfermería de una unidad pediátrica. Método estudio sustentado en investigación de cuidados convergentes, desarrollado entre diciembre de 2018 y mayo de 2019 con 11 profesionales del equipo de Enfermería, a través de círculos de conversación. Resultados los participantes reconocen el juego como algo inherente al niño y beneficioso en la interacción con el niño cuando está hospitalizado. Perciben la no apropiación del juego estructurado en sus prácticas, buscan expandirlo, pero identifican obstáculos asociados al limitado apoyo institucional. A partir de los desarrollos, decidieron insertar un títere y entrenarse para usar el juego estructurado. Conclusiones e implicaciones para la práctica la inserción del juego en el hospital exige romper con el modelo biomédico en salud y apostar por una cultura institucional de reconocimiento del juego. Jugar durante toda la hospitalización integra una atención justa, humana e integral, que debe ser motivo de lucha y garantía del equipo de Enfermería, correspondiendo a las instituciones el deber de brindar apoyo, ambos en el ámbito del trabajo y de la educación permanente.

Two new species of the industrially relevant genus Absidia (Mucorales) from soil of the Brazilian Atlantic Forest

ABSTRACT During a survey of Mucorales in soil from an upland forest area in Pernambuco, Brazil, two specimens were isolated and characterized based on their morphological, physiological, and molecular data (ITS and LSU rDNA). Phylogenetic analyses of the isolates revealed that the strains URM 8209 and URM 8210 are closely related to species of Absidia. URM 8209 forms conical, subglobose, and strawberry-shaped columellae and the sporangiospores are cylindrical and ellipsoid. URM 8210 produces hemispheric, subglobose, and strawberry-shaped columellae and the sporangiospores are globose, subglobose, ellipsoid, and short cylindrical. Based on evidence obtained through analysis of datasets (LSU and ITS rDNA regions), A. saloaensis sp. nov. (URM 8209) and A. multispora sp. nov. (URM 8210) are proposed here as novel species of Absidia. A table with morphological characteristics of Neotropical Absidia spp. is provided.

RESUMO Objetivo: Definir o perfil epidemiológico e os principais determinantes de morbimortalidade dos pacientes cirúrgicos não cardíacos de alto risco no Brasil. Métodos: Estudo prospectivo, observacional e multicêntrico. Todos os pacientes cirúrgicos não cardíacos admitidos nas unidades de terapia intensiva, ou seja, considerados de alto risco, no período de 1 mês, foram avaliados e acompanhados diariamente por, no máximo, 7 dias na unidade de terapia intensiva, para determinação de complicações. As taxas de mortalidade em 28 dias de pós-operatório, na unidade de terapia intensiva e hospitalar foram avaliadas. Resultados: Participaram 29 unidades de terapia intensiva onde foram realizadas cirurgias em 25.500 pacientes, dos quais 904 (3,5%) de alto risco (intervalo de confiança de 95% - IC95% 3,3% - 3,8%), tendo sido incluídos no estudo. Dos pacientes envolvidos, 48,3% eram de unidades de terapia intensiva privadas e 51,7% de públicas. O tempo de internação na unidade de terapia intensiva foi de 2,0 (1,0 - 4,0) dias e hospitalar de 9,5 (5,4 - 18,6) dias. As taxas de complicações foram 29,9% (IC95% 26,4 - 33,7) e mortalidade em 28 dias pós-cirurgia 9,6% (IC95% 7,4 - 12,1). Os fatores independentes de risco para complicações foram Simplified Acute Physiology Score 3 (SAPS 3; razão de chance − RC = 1,02; IC95% 1,01 - 1,03) e Sequential Organ Failure Assessment Score (SOFA) da admissão na unidade de terapia intensiva (RC =1,17; IC95% 1,09 - 1,25), tempo de cirurgia (RC = 1,001; IC95% 1,000 - 1,002) e cirurgias de emergências (RC = 1,93; IC95% 1,10 - 3,38). Em adição, foram associados com mortalidade em 28 dias idade (RC = 1,032; IC95% 1,011 - 1,052) SAPS 3 (RC = 1,041; IC95% 1,107 - 1,279), SOFA (RC = 1,175; IC95% 1,069 - 1,292) e cirurgias emergenciais (RC = 2,509; IC95% 1,040 - 6,051). Conclusão: Pacientes com escores prognósticos mais elevados, idosos, tempo cirúrgico e cirurgias emergenciais estiveram fortemente associados a maior mortalidade em 28 dias e mais complicações durante permanência em unidade de terapia intensiva.
ABSTRACT Objective: To define the epidemiological profile and the main determinants of morbidity and mortality in noncardiac high surgical risk patients in Brazil. Methods: This was a prospective, observational and multicenter study. All noncardiac surgical patients admitted to intensive care units, i.e., those considered high risk, within a 1-month period were evaluated and monitored daily for a maximum of 7 days in the intensive care unit to determine complications. The 28-day postoperative, intensive care unit and hospital mortality rates were evaluated. Results: Twenty-nine intensive care units participated in the study. Surgeries were performed in 25,500 patients, of whom 904 (3.5%) were high-risk (95% confidence interval - 95%CI 3.3% - 3.8%) and were included in the study. Of the participating patients, 48.3% were from private intensive care units, and 51.7% were from public intensive care units. The length of stay in the intensive care unit was 2.0 (1.0 - 4.0) days, and the length of hospital stay was 9.5 (5.4 - 18.6) days. The complication rate was 29.9% (95%CI 26.4 - 33.7), and the 28-day postoperative mortality rate was 9.6% (95%CI 7.4 - 12.1). The independent risk factors for complications were the Simplified Acute Physiology Score 3 (SAPS 3; odds ratio - OR = 1.02; 95%CI 1.01 - 1.03) and Sequential Organ Failure Assessment Score (SOFA) on admission to the intensive care unit (OR = 1.17; 95%CI 1.09 - 1.25), surgical time (OR = 1.001, 95%CI 1.000 - 1.002) and emergency surgeries (OR = 1.93, 95%CI, 1.10 - 3.38). In addition, there were associations with 28-day mortality (OR = 1.032; 95%CI 1.011 - 1.052), SAPS 3 (OR = 1.041; 95%CI 1.107 - 1.279), SOFA (OR = 1.175, 95%CI 1.069 - 1.292) and emergency surgeries (OR = 2.509; 95%CI 1.040 - 6.051). Conclusion: Higher prognostic scores, elderly patients, longer surgical times and emergency surgeries were strongly associated with higher 28-day mortality and more complications during the intensive care unit stay.

ABSTRACT Anthracnose is a crop disease usually caused by fungi in the genus Colletotrichum or Gloeosporium. These are considered one of the main pathogens, causing significant economic losses, such as in peppers and guarana. The current forms of control include the use of resistant cultivars, sanitary pruning and fungicides. However, even with the use of some methods of controlling these cultures, the crops are not free of anthracnose. Additionally, excessive application of fungicides increases the resistance of pathogens to agrochemicals and cause harm to human health and the environment. In order to find natural antifungal agents against guarana anthracnose, endophytic fungi were isolated from Amazon guarana. The compounds piliformic acid and cytochalasin D were isolated by chromatographic techniques from two Xylaria spp., guided by assays with Colletotrichum gloeosporioides. The isolated compounds were identified by spectrometric techniques, as NMR and mass spectrometry. This is the first report that piliformic acid and cytochalasin D have antifungal activity against C. gloeosporioides with MIC 2.92 and 2.46 µmol mL-1 respectively. Captan and difenoconazole were included as positive controls (MIC 16.63 and 0.02 µmol mL-1, respectively). Thus, Xylaria species presented a biotechnological potential and production of different active compounds which might be promising against anthracnose disease.

The Impact of Early Dermatologic Events in the Survival of Patients with Hepatocellular Carcinoma Treated with Sorafenib

ABSTRACT Background and Aims. The presence of dermatologic reaction as an adverse event to sorafenib treatment in patients with unresectable hepatocellular carcinoma has been indicated as a prognostic factor for survival in a recent prospective analysis. To date, this is the only clinical predictor of treatment response, which can be evaluated earlier in the treatment and, therefore, contribute to a better and more individualized patient management. Material and methods. This retrospective study included 127 patients treated with sorafenib under real-life practice conditions in two hepatology reference centers in Brazil. Demographic data, disease/medical history and time of sorafenib administration as well as adverse events related to the medication were recorded in a database. Results. Cirrhosis was present in 94% of patients, 85.6% were Child-Pugh A, 80.3%BCLC-C, 81% had vascular invasion and/or extrahepatic spread and 95% had a performance status 0 to 1.The median duration of treatment was 10.1 months (range: 0.1-47 months).The most common adverse event within the first 60 days of treatment were diarrhea (62.2%) and dermatological reaction (42%).The median overall survival for the cohort was 20 months, and it was higher for patients who developed dermatological reactions within the first 60 days compared to those who did not present this adverse event. Conclusion. This retrospective analysis showed the use of sorafenib in patients selected according to BCLC staging, and it is the first external validation of early dermatologic adverse events as a predictor of overall survival in patients with advanced hepatocellular carcinoma.

Abstract Hemostatic and adhesive agents date back to World War II, when homologous fibrin sealant came onto scene. Considering that infectious diseases can be transmitted via human blood, a new heterologous fibrin sealant was standardized in the 1990s. Its components were a serine protease (a thrombin-like enzyme) extracted from the venom of Crotalus durissus terrificus snakes and a fibrinogen-rich cryoprecipitate extracted from the blood of Bubalus bubalis buffaloes. This new bioproduct has been used as a coagulant, sealant, adhesive and recently as a candidate scaffold for mesenchymal stem cells and bone and cartilage repair. This review discusses the composition of a new heterologous fibrin sealant, and cites published articles related to its preclinical applications aiming at repairing nervous system traumas and regenerating bone marrow. Finally, we present an innovative safety trial I/II that found the product to be a safe and clinically promising candidate for treating chronic venous ulcers. A multicenter clinical trial, phase II/III, with a larger number of participants will be performed to prove the efficacy of an innovative biopharmaceutical product derived from animal venom.

RESUMO: O objetivo deste estudo foi investigar a prevalência de anticorpos contra o vírus da arterite viral equina (EVAV) em equinos sadios criados nas mesorregiões Noroeste, Centro Ocidental e Norte Central do estado do Paraná. Após o cálculo do tamanho amostral, foram analisadas, utilizando a técnica de soroneutralização, amostras de soro sanguíneo de 653 equinos. Nenhum animal sororreagente foi encontrado nas mesorregiões Noroeste (0/236) e Centro Ocidental (0/99). Na mesorregião Norte Central a prevalência foi de 0,62% (2/318), totalizando 0,30%. Pode-se concluir que a arterite viral equina (AVE) ainda não representa um problema de importância epidemiológica nos equinos criados nas mesorregiões paranaenses estudadas.
ABSTRACT: The prevalence of antibodies against equine viral arteritis virus (EVAV) in healthy horses raised in Northwest, Western Central and North Central regions of the State of Paraná, Brazil was investigated. According to calculated sample size, serum samples of 653 horses were analyzed through standard virus neutralization test. No horse had antibodies to EVAV in Northwest (0/236) and in Western Central (0/99) regions. In North Central region frequency was 0.62% (2/318), with an overall prevalence of 0.30%. It can be concluded that EVA does not represent a problem of epidemiological importance in the studied regions of Paraná.

CONTEXTO E OBJETIVO:Dados sobre os custos do seguimento ambulatorial pós-transplante de fígado são escassos no Brasil. O objetivo do presente estudo foi estimar os custos diretos médicos do seguimento ambulatorial pós-transplante de fígado a partir da primeira visita ambulatorial pós-transplante até cinco anos após o transplante.TIPO DE ESTUDO E LOCAL:Estudo de descrição de custos realizado em um hospital universitário em São Paulo, Brasil.MÉTODOS:Dados de custos estavam disponíveis para 20 adultos que foram submetidos a transplante de fígado devido a insuficiência hepática aguda (IHA) de 2005 a 2009. Os dados foram retrospectivamente obtidos em prontuários médicos e no sistema de informação contábil hospitalar de dezembro de 2010 a janeiro de 2011.RESULTADOS:A média de custo por paciente/ano foi de R$ 13.569 (US$ 5.824). O primeiro ano de acompanhamento foi o mais caro, R$ 32.546 (US$ 13,968), e medicação foi o principal impulsionador dos custos totais, respondendo por 85% dos custos totais no período de cinco anos e 71,9% dos custos totais do primeiro ano. No segundo ano pós-transplante, os custos médios totais foram cerca da metade do montante de custos do primeiro ano (R$ 15.165 ou US$ 6,509). Medicação foi o maior contribuinte para os custos seguido da internação, no período de cinco anos. No quarto ano, os custos dos testes diagnósticos superam os custos de internação.CONCLUSÃO:Esta análise proporciona uma compreensão significativa dos custos do seguimento ambulatorial pós-transplante de fígado por IHA e a participação de cada componente de custo no cenário brasileiro.
CONTEXT AND OBJECTIVE:Data on the costs of outpatient follow-up after liver transplantation are scarce in Brazil. The purpose of the present study was to estimate the direct medical costs of the outpatient follow-up after liver transplantation, from the first outpatient visit after transplantation to five years after transplantation.DESIGN AND SETTING:Cost description study conducted in a university hospital in São Paulo, Brazil.METHODS:Cost data were available for 20 adults who underwent liver transplantation due to acute liver failure (ALF) from 2005 to 2009. The data were retrospectively retrieved from medical records and the hospital accounting information system from December 2010 to January 2011.RESULTS:Mean cost per patient/year was R$ 13,569 (US$ 5,824). The first year of follow-up was the most expensive (R$ 32,546 or US$ 13,968), and medication was the main driver of total costs, accounting for 85% of the total costs over the five-year period and 71.9% of the first-year total costs. In the second year after transplantation, the mean total costs were about half of the amount of the first-year costs (R$ 15,165 or US$ 6,509). Medication was the largest contributor to the costs followed by hospitalization, over the five-year period. In the fourth year, the costs of diagnostic tests exceeded the hospitalization costs.CONCLUSION:This analysis provides significant insight into the costs of outpatient follow-up after liver transplantation due to ALF and the participation of each cost component in the Brazilian setting.

Devido às mudanças ocorridas pela evolução da Odontologia, a busca pela estética tem se tornado cada vez mais frequente. A constante atualização dos profissionais é uma necessidade. Quando ocorre a perda de um elemento dental, várias técnicas têm sido sugeridas para a correta reabilitação do mesmo. Manutenção do contorno ósseo e gengival já é um fator entendido como imprescindível para que a mesma possa ser alcançada. Em busca dessa manutenção de contorno, técnicas de posicionamentos imediatos de implantes, técnicas de enxertia óssea e/ou gengival e cuidados no correto estabelecimento da posição ideal do implante tem sido descritos. No entanto, alguns casos chegam para reabilitação com implantes já instalados e osseointegrados. Entendendo a importância de se conhecer os recursos descritos para obtenção de contornos gengivais adequados frente a essa condição clínica esse estudo foi idealizado. Foi realizada uma revisão de literatura e alguns dos métodos descritos para obtenção de papilas gengivais e estéticas em maxila anterior, assim como para confecção da prótese foram utilizados para resolução de um caso clínico que é apresentado. A análise do caso finalizado e a observação do grau de satisfação da paciente com o resultado alcançado permitem concluir que os recursos descritos são aplicáveis e permitem a otimização dos resultados.
As dentistry has evolved, the search for good esthetics has become increasingly frequent. Professionals must be constantly up-to-date regarding the latest advances. When a tooth is lost, there are several possible rehabilitation techniques. Adequate contour of the bone and gingiva is an essential factor if good esthetic results are to be achieved. In order to maintain the contour, techniques for immediate placement of implants, bone and gum grafting techniques and care in the correct establishment of the ideal position of the implant have been advocated. Sometimes dentists have to treat a patient whose implants have already been installed and are osseointegrated. This report was carried out in order to highlight the importance of understanding the information that is available to obtain appropriate gingival contours in such clinical conditions. A review of the literature was carried out on methods for obtaining gingival papilla and esthetics in the anterior maxilla, and on methods to construct a prosthesis for the clinical case described. The analysis of the outcome and of the degree of patient satisfaction suggests that the resources described can be successfully applied.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.
