ABSTRACT Objective: To report outcomes from the largest multicenter series of penile cancer patients undergoing video endoscopic inguinal lymphadenectomy (VEIL). Materials and Methods: Retrospective multicenter analysis. Authors of 21 centers from the Penile Cancer Collaborative Coalition-Latin America (PeC-LA) were included. All centers performed the procedure following the same previously described standardized technique. Inclusion criteria included penile cancer patients with no palpable lymph nodes and intermediate/high-risk disease and those with non-fixed palpable lymph nodes less than 4 cm in diameter. Categorical variables are shown as percentages and frequencies whereas continuous variables as mean and range. Results: From 2006 to 2020, 210 VEIL procedures were performed in 105 patients. Mean age was 58 (45-68) years old. Mean operative time was 90 minutes (60-120). Mean lymph node yield was 10 nodes (6-16). Complication rate was 15.7%, including severe complications in 1.9% of procedures. Lymphatic and skin complications were noted in 8.6 and 4.8% of patients, respectively. Histopathological analysis revealed lymph node involvement in 26.7% of patients with non-palpable nodes. Inguinal recurrence was observed in 2.8% of patients. 10y- overall survival was 74.2% and 10-y cancer specific survival was 84.8%. CSS for pN0, pN1, pN2 and pN3 were 100%, 82.4%, 72.7% and 9.1%, respectively. Conclusion: VEIL seems to offer appropriate long term oncological control with minimal morbidity. In the absence of non-invasive stratification measures such as dynamic sentinel node biopsy, VEIL emerged as the alternative for the management of non-bulky lymph nodes in penile cancer.

ABSTRACT The expression ‘you need to know to conserve’ is a well-known cliche among biologists. Documenting the richness of a group of organisms is the first step towards understanding biodiversity and preparing efficient conservation plans. In this context, many efforts have been made to quantify the number of species on Earth and estimate the number of species still unknown to science. A few countries have complete and integrated databases estimating the approximate number of species recorded for their territory, particularly in the Global South. In Brazil, a country of continental dimensions, revealing the richness of the second most diverse clade of invertebrates (=Mollusca) has been a goal of taxonomists. Recently, in an unprecedented, collective, and integrated effort among Brazilian malacologists, it was possible to estimate how many valid species of molluscs are there in Brazil. In this effort, more than 30 mollusc experts joined together to update the Taxonomic Catalogue of the Brazilian Fauna (TCBF), a governmental website that allows a quick and real-time updating of all Metazoan. So far, more than 5,000 updates have been made in TCBF, indicating the presence of 3,552 valid species of molluscs in Brazil, distributed among the main clades as follows: Caudofoveata (10 spp.), Solenogastres (6 spp.), Polyplacophora (35 spp.), Scaphopoda (43 spp.), Cephalopoda (92 spp.), Bivalvia (629 spp.) and Gastropoda (2,737 spp.). The present study, in addition to demonstrating for the first time the richness of Brazilian molluscs, also presents the state of the art of this important phylum of invertebrates highlighting its most representative and neglected groups.

Abstract A new lectin from marine sponge Ircinia strobilina, denominated IsL, was isolated by combination of affinity chromatography in Guar gum matrix followed by size exclusion chromatography. IsL was able to agglutinate native and enzymatically treated rabbit erythrocytes, being inhibited by galactosides, such as α-methyl-D-galactopyranoside, β-methyl-D-galactopyranoside and α-lactose. IsL hemagglutinating activity was stable at neutral to alkaline pH, however the lectin loses its activity at 40° C. The molecular mass determinated by mass spectrometry was 13.655 ± 5 Da. Approximately 40% of the primary structure of IsL was determined by mass spectrometry, but no similarity was observed with any protein. The secondary structure of IsL consists of 28% α-helix, 26% β-sheet, and 46% random region, as determined by dichroism circular. IsL was a calcium-dependent lectin, but no significant variations were observed by circular dichroism when IsL was incubated in presence of calcium and EDTA. IsL was not toxic against Artemia nauplii and did not have antimicrobial activity against bacterial cells. However, the IsL was able to significantly inhibit the biofilm formation of Staphylococcus aureus and Staphylococcus epidermidis.

Infectious diseases are caused by pathogenic microorganisms and can spread through different ways. Mathematical models and computational simulation have been used extensively to investigate the transmission and spread of infectious diseases. In other words, mathematical model simulation can be used to analyse the dynamics of infectious diseases, aiming to understand the effects and how to control the spread. In general, these models are based on compartments, where each compartment contains individuals with the same characteristics, such as susceptible, exposed, infected, and recovered. In this paper, we cast further light on some classical epidemic models, reporting possible outcomes from numerical simulation. Furthermore, we provide routines in a repository for simulations.

RESUMO Dentre as inúmeras plantas medicinais utilizadas pela população brasileira está a Solanum paniculatum L. (jurubeba), em que o uso de suas folhas é bastante popular, na forma de chá, como remédio contra ressacas após o consumo exagerado de álcool e comida, além de serem usadas na forma de pomadas como cicatrizantes, e, também, no tratamento de problemas hepáticos e digestivos. As folhas de plantas medicinais, normalmente, possuem elevado teor de água, necessitando a redução do mesmo por meio da secagem, para que o produto possa ser armazenado com segurança até o seu processamento. Objetivou-se com o presente estudo avaliar a cinética de secagem de folhas de jurubeba, bem como determinar o coeficiente de difusão efetiva e a energia de ativação durante a secagem. As folhas de jurubeba foram submetidas a secagem em diferentes temperaturas (30, 40, 50, 60 e 70 °C) e velocidade do ar (0,4 e 0,8 m s-1) . O modelo de Midilli ajustou-se satisfatoriamente aos dados observados da secagem das folhas de jurubeba para todas as condições. O aumento da temperatura e da velocidade do ar de secagem reduziu o tempo de secagem e aumentou o coeficiente de difusão efetiva. Para a faixa de temperatura de 40 a 70 °C, ao aumentar a velocidade do ar de secagem, reduz-se a energia de ativação para a secagem das folhas de jurubeba.
ABSTRACT Solanum paniculatum L. (jurubeba) is among the numerous medicinal plants used by the Brazilian population, and the use of its leaves is quite popular, in the form of tea, as a remedy against hangovers after excessive consumption of alcohol and food, besides being used in the form of ointments as healing agent and also in the treatment of liver and digestive problems. Leaves of medicinal plants usually have a high moisture content, requiring its reduction by drying, so that the product can be stored safely until its processing. The objective of this study was to evaluate the drying kinetics of jurubeba leaves and to determine the effective diffusion coefficient and activation energy during drying. Jurubeba leaves were dried at different air temperatures (30, 40, 50, 60 and 70 °C) and speed (0.4 and 0.8 m s-1). The Midilli model satisfactorily fitted to the observed data of drying of jurubeba leaves for all air conditions. Increasing the drying air temperature and speed reduced the drying time and increased the effective diffusion coefficient. For the air temperature range from 40 to 70 °C, as the drying air speed increases, the activation energy for the drying of the jurubeba leaves is reduced.

RESUMO Os diferentes microhabitats proporcionados pelas pradarias marinhas e o habito das espécies determinam a distribuição vertical de espécies de crustáceos. Este estudo verificou a influência da sazonalidade sobre a distribuição espacial da comunidade de crustáceos associada a uma pradaria de Halodule wrightii. De abril de 2006 a julho de 2007, foram realizadas 15 amostragens na praia das Goiabeiras, estado do Ceará, em cada uma com cinco amostras seccionadas em partes aérea e subterrânea. Cumáceos e o anfípode Hyale media (Dana, 1853) tiveram a maior frequência, densidade e dominância em relação a outros táxons, em ambos os estratos. A comunidade exibiu mudanças graduais ao longo do estudo, em ambos os estratos aéreo e subterrâneo, mas a estrutura da pradaria não foi suficiente para explicar a distribuição vertical da fauna de crustáceos ao longo do tempo.
ABSTRACT The different microhabitats provided by seagrasses and the habit of the species determinate the vertical distribution of crustaceans. This study verified the influence of the seasonality on the spatial distribution of the crustacean community associated with a Halodule wrightii meadow. From April 2006 to July 2007, we performed fifteen samplings in Goiabeiras Beach, Ceará State, each with five sectioned replicates in belowground and aboveground. Cumaceans and the amphipod Hyale media (Dana, 1853) had a higher frequency, density, and dominance than the other taxa, in both strata. The community exhibited gradual changes along the study, in both the belowground and aboveground strata, but the seagrass structure was not sufficient to explain the vertical distribution of the crustacean fauna along the time.

Este artigo apresenta os resultados gerais da pesquisa sobre o processo de construção da gestão regional no estado de São Paulo, durante a discussão do COAP/redes de atenção, com o intuito de fornecer subsídios para a compreensão deste processo de pactuação regional da saúde, com enfoque nas regiões de saúde de Bauru, Baixada Santista, Grande ABC e Vale do Ribeira, no estado de São Paulo. Além dos resultados apresentados sobre as regiões de saúde estudadas, a metodologia utilizada no desenvolvimento da construção dos perfis das regiões constitui em si uma proposta metodológica de análise de perfis regionais de saúde. A primeira parte do artigo apresenta a metodologia geral adotada para a análise das regiões de saúde; a segunda, abrange os resultados e a discussão da pesquisa, organizados em dois itens. O primeiro destes itens refere-se à análise dos perfis das cinco regiões de saúde pesquisadas no Estado. O segundo, analisa os principais aspectos do processo de pactuação regional da saúde em São Paulo, destacando potencialidades e limites, a partir de entrevistas realizadas com gestores municipais e apoiadores do Conselho de Secretários Municipais de Saúde de São Paulo dessas regiões.
This article presents the main results of the survey on the regional management building process in the State of São Paulo, during the discussion of the COAP / Care Networks, in order to provide a basis for understanding this process of regional health pact, focusing on the metropolitan areas of Bauru, Santos, Grande ABC and the Ribeira Valley. In addition to the results presented on the health regions studied, the methodology used in the development of the construction of profiles of the regions is itself a proposal for a methodology of analyzing regional health profiles. The first section presents the general methodology for analyzing health regions. The second part covers the results and discussion of the research, organized into two items. The first refers to the analysis of the profiles of the five São Paulo health regions surveyed. The second item analyzes the main aspects of the process of regional health pact in São Paulo, highlighting strengths and limitations, based on interviews with municipal managers and supporters of the Council of Municipal Health Secretaries of São Paulo in these regions.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

The semisynthetic landscape of aphidicolin: inspiration towards leishmanicidal compounds

Recentes estudos demonstraram que afidicolina, metabólito secundário do fungo endofítico Nigrospora sphaerica, apresenta alta atividade contra o protozoário Leishmania major. Apesar do promissor potencial leishmanicida, a afidicolina apresenta propriedades físico-químicas e perfil farmacocinético inadequado para terapêutica. Neste sentido, uma revisão da literatura apresenta as limitadas modificações para este terpeno e ainda traz informações sobre as propriedades moleculares dos compostos já descritos e sua correlação com compostos leishmanicidas. Essa revisão forneceu uma análise racional para o desenvolvimento do derivado oxima. Uma série de derivados da afidicolina bem como o análogo oxima foram sintetizados para avaliação preliminar dos requisitos estruturais para atividade leishmanicida da afidicolina e seus derivados semi-sintéticos. Oito compostos foram sintetizados e testados contra diferentes espécies de Leishmania. A avaliação preliminar demonstrou alta atividade leishmanicida da afidicolina enquanto que o derivado oxima apresenta moderada seletividade contra a espécie L. braziliensis, endêmica em diversos países da América do Sul. Nenhum dos compostos apresentou citotoxicidade contra células de mamíferos.
Recent studies have shown that aphidicolin, a secondary metabolite of the endophytic fungus Nigrospora sphaerica, has high activity against the protozoan Leishmania major. Despite its promising leishmanicidal potential, aphidicolin presents a therapeutically unsuitable physico-chemical and pharmacokinetic profile. In this sense, a review of the literature shows the limited types of modification for this terpene. In addition, it gives an idea about the molecular properties of the compounds produced and which were correlated to leishmanicidal derivatives. This analysis provided us a rationale for the development of an oxime derivative. We synthesized the oxime-aphidicolin and a series of derivatives for a preliminary evaluation of the structural requirements for the leishmanicidal activity of aphidicolin and its semisynthetic derivatives. Eight compounds have been synthesized and tested against different species of the Leishmania protozoa. The preliminary evaluation demonstrated high leishmanicidal activity for aphidicolin, while the oxime derivative shows moderate selectivity for the L. braziliensis species, which is commonly found in several South American countries. None of the compounds showed cytotoxicity against mammalian cells.

Tegumentary manifestations of Noonan and Noonan-related syndromes

OBJECTIVES: Noonan and Noonan-related syndromes are common autosomal dominant disorders with neuro-cardio-facial-cutaneous and developmental involvement. The objective of this article is to describe the most relevant tegumentary findings in a cohort of 41 patients with Noonan or Noonan-related syndromes and to detail certain aspects of the molecular mechanisms underlying ectodermal involvement. METHODS: A standard questionnaire was administered. A focused physical examination and a systematic review of clinical records was performed on all patients to verify the presence of tegumentary alterations. The molecular analysis of this cohort included sequencing of the following genes in all patients: PTPN1, SOS1, RAF1, KRAS, SHOC2 and BRAF. RESULTS: The most frequent tegumentary alterations were xeroderma (46%), photosensitivity (29%), excessive hair loss (24%), recurrent oral ulcers (22%), curly hair (20%), nevi (17%), markedly increased palmar and plantar creases (12%), follicular hyperkeratosis (12%), palmoplantar hyperkeratosis (10%), café-au-lait spots (10%) and sparse eyebrows (7%). Patients with mutations in PTPN11 had lower frequencies of palmar and plantar creases and palmar/plantar hyperkeratosis compared with the other patients. CONCLUSIONS: We observed that patients with mutations in genes directly involved in cell proliferation kinase cascades (SOS1, BRAF, KRAS and RAF1) had a higher frequency of hyperkeratotic lesions compared with patients with mutations in genes that have a more complex interaction with and modulation of cell proliferation kinase cascades (PTPN11).

Muitos métodos para estimativa de idade de mamíferos têm sido utilizados ultimamente. Entretanto, a maioria deles requerem procedimentos laboratoriais, que geralmente, não estão disponíveis em campo. Neste artigo nós definimos categorias de idade para seis espécies de mamíferos através da análise de desgaste dentário, erupção dentária e massa corporal. Nós analisamos 632 crânios, incluindo Mazama nemorivaga (veado cinza), Mazama americana (veado vermelhor), Tayassu pecari (queixada), Pecari tajacu (cateto), Cuniculus paca (paca) and Dasyprocta leporina (cotia). Usando critérios biológicos e morfológicos nós definimos quatro categorias de idade para cada espécie: jovem, sub-adulto, adulto e adulto senescente. Foram definidos padrões de desgaste e erupção dos molares para cada categoria de idade para cada uma das espécies analisadas. Nós encontramos diferenças significativas das médias de peso entre as categorias de idade para as espécies D. leporina (F = 81.1; df = 3; p < 0.05), C. paca (F = 39.0; df = 3; p < 0.05), P. tajacu (F = 24.6; df = 3; p < 0.05) and M. americana (F = 30.6; df = 3; p < 0.05), sugerindo que a massa corporal pode ser usada como um critério complementar de estimativa de idade. Embora seja um método menos quantitativo do que aqueles citados com mais frequência na literatura, como deposição de bandas de cementum, o método descrito neste artigo mostrou ser uma ferramenta eficiente para pesquisadores e manejadores em trabalhos de campo na Amazônia brasileira. Embora métodos similares já tenham sido utilizados em estudos de caça, é a primeira vez que esta metodologia é descrita em detalhes, podendo ser replicada em outros estudos semelhantes.
Many methods of age determination have been used to date. However, most of them require laboratory procedures which are, in general, not available for wildlife managers in the field. In this paper we attempt to standardize the age categories for six mammal species through the analysis of dental wear, dental eruption and body mass. We analyzed 632 skulls, including Mazama nemorivaga (brown brocket deer), Mazama americana (red brocket deer), Tayassu pecari (white-lipped peccary), Pecari tajacu (collared peccary), Cuniculus paca (paca) and Dasyprocta leporina (agouti). Using biological and morphological criteria we define four age categories: young, sub adult, adult and senescent adult. Scores and variables of molar cusp wear and eruption were defined for each age category within each species. We found significant differences in mean weights among age categories for D. leporina (F = 81.1; df = 3; p < 0.05), C. paca (F = 39.0; df = 3; p < 0.05), P. tajacu (F = 24.6; df = 3; p < 0.05) and M. americana (F = 30.6; df = 3; p < 0.05), suggesting that body mass can be a complementary criterion for estimating age. Despite being less quantitative than methods such as those which use deposition of cementum layers, the method described here proved to be an efficient tool for field researchers and managers in the Brazilian Amazon. Although similar methods have already been applied in the study of hunted mammals in the Peruvian and Brazilian Amazon, it is the first time such a method is described in detail and therefore it can be easily used in future works.

FUNDAMENTO: Embora o Escore de Risco TIMI seja o mais utilizado em síndromes coronarianas agudas sem supradesnível do segmento ST (SCA), o Escore GRACE tem potencial superioridade prognóstica, pois foi criado a partir de um registro observacional, parte das variáveis são tratadas de forma semiquantitativa e a função renal é computada em seu cálculo. OBJETIVO: Testar a hipótese de que o Escore de Risco GRACE tem superior valor prognóstico hospitalar, comparado ao Escore TIMI em pacientes admitidos com SCA. MÉTODOS: Foram incluídos indivíduos com angina instável ou infarto do miocárdio sem supradesnível do segmento ST, consecutivamente internados em unidade coronariana entre agosto de 2007 e janeiro de 2009. RESULTADOS: Foram estudados 154 pacientes, idade 71 ± 13 anos, 56% do gênero feminino, mediana do GRACE de 117 e mediana do TIMI de 3. Durante o período de internamento, a incidência de eventos foi 8,4% (12 óbitos e 1 infarto não fatal). O teste de Hosmer-Lemeshow aplicado ao Escore GRACE apresentou χ2 de 5,3 (P = 0,72), enquanto Escore TIMI apresentou χ2 de 1,85 (P = 0,60). Desta forma, ambos os escores apresentaram boa calibração. Quanto à análise de discriminação, o Escore GRACE apresentou estatística-C de 0,91 (95% IC = 0,86 - 0,97), significativamente superior à estatística-C de 0,69 do Escore TIMI (95% IC = 0,55 - 0,84) - P = 0,02 para diferença entre os escores. CONCLUSÃO: Em relação à predição de eventos hospitalares em pacientes com SCA, o Escore GRACE tem superior capacidade prognóstica quando comparado ao Escore TIMI.
BACKGROUND: Although the TIMI score is the one most frequently used in acute coronary syndromes (ACS) without ST-segment elevation, the GRACE score has potential prognostic superiority, as it was created based on an observational registry, part of the variables is treated in a semi-quantitative form and renal function is taken into account in its calculation. OBJECTIVE: To test the hypothesis that the GRACE risk score has superior in-hospital prognostic value, when compared to the TIMI score in patients admitted with ACS. METHODS: Individuals with unstable angina or myocardial infarction without ST-segment elevation, consecutively admitted at the Coronary Unit between August 2007 and January 2009, were included in the study. RESULTS: A total of 154 patients aged 71 ± 13 years, of which 56% were females, with a GRACE median of 117 and a TIMI median of 3 were studied. During the hospitalization period, the incidence of events was 8.4% (12 deaths and 1 non-fatal infarction). The Hosmer-Lemeshow test applied to the GRACE score presented an χ2 of 5.3 (P = 0.72), whereas the TIMI score presented an χ2 of 1.85 (P = 0.60). Therefore, both scores presented good calibration. As for the analysis of discrimination, the GRACE score presented a C-statistics of 0.91 (95%CI= 0.86 - 0.97), significantly superior to the C-statistics of 0.69 of the TIMI score (95%CI = 0.55 - 0.84) - P = 0.02 for the difference between the scores. CONCLUSION: Regarding the prediction of hospital events in patients with ACS, the GRACE score has superior prognostic capacity when compared to the TIMI score.
FUNDAMENTO: Aunque el Score de Riesgo TIMI sea el más utilizado en síndromes coronarios agudos sin supradesnivel del segmento ST (SCA), el Score GRACE tiene potencial superioridad pronóstica, pues fue creado a partir de un registro observacional, parte de las variables son tratadas de forma semicuantitativa y la función renal se computa en su cálculo. OBJETIVO: Probar la hipótesis de que el Score de Riesgo GRACE tiene superior valor pronóstico hospitalario, comparado con el Score TIMI en pacientes ingresados con SCA. MÉTODOS: Fueron incluidos individuos con angina inestable o infarto de miocardio sin supradesnivel del segmento ST, consecutivamente internados en Unidad Coronaria entre agosto de 2007 y enero de 2009. RESULTADOS: Fueron estudiados 154 pacientes, edad 71 ± 13 años, el 56% del sexo femenino, mediana de GRACE de 117 y mediana de TIMI de 3. Durante el período de internación, la incidencia de eventos fue del 8,4% (12 fallecimientos y 1 infarto no fatal). El test de Hosmer-Lemeshow aplicado al Score GRACE presentó χ² de 5,3 (P = 0,72), mientras que el Score TIMI presentó χ² de 1,85 (P = 0,60). De esta forma, ambos scores presentaron buena calibración. Em 82 exames parasitológicos de fezes realizados pelo método de sedimentação espontânea, em indivíduos moradores em ruas da Cidade do Rio de Janeiro, foram encontrados ovos de Ascaris lumbricoides em 40 (48,8%), de Trichuris trichiura em 27 (32,9%) e ancilostomídeos 7 (8,5%).
Stool examination by sedimentation method in 82 homeless individuals living in the streets of Rio de Janeiro City, revealed Ascaris lumbricoides eggs in 40 (48.85%), Trichuris trichiura in 27 (32.9%) and hookworm in 7 (8.5%).
