Abstract Introduction: Methylene Blue (MB) has been shown to attenuate oxidative, inflammatory, myocardial, and neurological lesions during ischemia-reperfusion and has great potential during cardiac arrest. This study aimed to determine the effects of MB combined with epinephrine during cardiac arrest on myocardial and cerebral lesions. Method: Thirty-eight male Wistar rats were randomly assigned to four groups: the sham group (SH, n = 5), and three groups subjected to cardiac arrest (n = 11 /group) and treated with EPI 20 μg.kg−1 (EPI), EPI 20 μg.kg−1 + MB 2 mg.kg−1 (EPI + MB), or saline 0.9% 0.2 ml (CTL). Ventricular fibrillation was induced by direct electrical stimulation in the right ventricle for 3 minutes, and anoxia was maintained for 5 minutes. Cardiopulmonary Resuscitation (CPR) consisted of medications, ventilation, chest compressions, and defibrillation. After returning to spontaneous circulation, animals were observed for four hours. Blood gas, troponin, oxidative stress, histology, and TUNEL staining measurements were analyzed. Groups were compared using generalized estimating equations. Results: No differences in the Returning of Spontaneous Circulation (ROSC) rate were observed among the groups (EPI: 63%, EPI + MB: 45%, CTL: 40%, p = 0.672). The mean arterial pressure immediately after ROSC was higher in the EPI+MB group than in the CTRL group (CTL: 30.5 [5.8], EPI: 63 [25.5], EPI+MB: 123 [31] mmHg, p = 0.007). Serum troponin levels were high in the CTL group (CTL: 130.1 [333.8], EPI: 3.70 [36.0], EPI +MB: 43.7 [116.31] ng/mL, p < 0.05). Conclusion: The coadministration of MB and epinephrine failed to yield enhancements in cardiac or brain lesions in a rodent model of cardiac arrest. A metallic surface can exhibit low wettability through the formation of a rough structure with micro/nanometric dimensions combined with a surface energy-reducing agent. When the contact and sliding angles are ≥ 150° and ≤ 10°, respectively, the surface is classified as superhydrophobic (SHS). The study aimed to electrodeposit a microstructure with micronanometric roughness of nickel, stearic acid (Al/Ni-SA), and nickel, stearic acid with multi-layered carbon nanotubes (Al/Ni-SA-MWCNT) on aeronautical aluminum 7050. The obtained contact angles were close to 160°, and sliding angles were less than 1°, characterizing the superhydrophobic and self-cleaning nature. Freezing tests were conducted, and the deposit with MWCNT showed longer freezing times at -22°C. The Al/Ni-SA-MWCNT condition also exhibited the best corrosion resistance with corrosion current superior by two orders of magnitude compared to the Al/Ni-Sa condition. In this investigation, we synthesized magnetic nanoparticles coated with silica and functionalized with p-sulfonic acid calix[4]arene (CX4), to be used as heterogeneous acid catalysts for acetal synthesis. We evaluated the catalytic efficiency of these nanoparticles for acetal synthesis using various aldehydes and glycerol under microwave irradiation at atmospheric pressure, with and without solvents. Two distinct synthesis methods, Pechini (Fe/pch/Si/CX4) and precipitation (Fe/ppt/Si/CX4), resulted in variations in the physical and chemical properties of the solids produced. Structural characterization involved thermogravimetry analysis (TG), X-ray diffraction (XRD), Mössbauer spectroscopy, vibrating sample magnetometry, scanning and transmission electron microscopy (SEM and TEM), infrared spectroscopy (FTIR), and titration for acidic site quantification. Fe/ppt/Si/CX4 exhibited goethite phases, while Fe/pch/Si/CX4 showed a substantial proportion of maghemite. Both catalysts demonstrated significant activity in acetal synthesis, achieving approximately 80% conversion without solvents. ABSTRACT Biological soil crusts (BSC) are commonly found in soils in the drylands regions, which can influence stabilization, water retention, nutrient cycling (particularly carbon (C) and nitrogen (N) dynamics), and several ecological processes. However, the composition of BSC in Brazilian soils undergoing the desertification process remains poorly understood. This study aimed to characterize the bacterial community in BSC formed in a Brazilian semiarid region under the desertification process. Thus, a highly desertified region was selected from which 34 BSC samples were collected. The total DNA of the BSC was extracted from 0.5 g samples, and the bacterial community was sequenced by a Next Generation Sequencing (NGS) platform (Miseq – Illumina®) using universal primers (515F and 806R). Bioinformatic analysis was carried out in QIIME (v.1.9), and the Operational Taxonomic Units (OTU) table was constructed following the Sumaclust methodology. The pH of BSC, C, N, and phosphorus contents was analyzed. Our study identified a diverse bacterial community in the BSCs. Cyanobacteria, Chloroflexi, and Proteobacteria phyla presented the greatest relative abundance (%) across the samples. Cyanobacteria were dominated by the orders Nostocales and Leptolyngbyales. The prediction of the putative functions found that mostf OTU were related to phototrophy, photosynthetic cyanobacteria, and photoautotrophy. The study found correlations between bacterial phyla and BSC properties, with Cyanobacteria positively related to C. Chloroflexi, Armatimonadetes, and WPS-2 were negatively correlated with C and N contents. These results suggest the critical roles bacteria communities play in BSCs from the Caatinga biome and highlight the potential impact of environmental factors on their diversity and functions. Abstract This study aimed to evaluate the phenophases of Cenostigma pyramidale (Tul.) Gagnon & G.P.Lewis in an area of Caatinga in the semi-arid region of Paraíba, Brazil. Field research was carried out in the municipality of Sumé, in the Experimental Space reserved for Studies on Ecology and Dynamics of the Caatinga - Area I of the Laboratory of Ecology and Botany - LAEB/CDSA/UFCG (7°39'38.8'' S and 36°53'42.4'' W; 538 m altitude). A total of 96 plots of 10 x 10 m were arranged in the study area, seeking to identify one specimen of C. pyramidale per plot. Monitoring was performed monthly from March 2019 to February 2020, recording budding, senescence, flower bud, flowering, and fruiting data. The Fournier intensity percentage and the activity index were analyzed. The highest budding intensity index was observed when there was a gradual increase in precipitation. Senescence had the highest peaks of intensity in the months with the absence or reduced levels of precipitation. Flower bud and flowering were practically equal throughout the monitoring period. The highest fruiting intensity occurred in the month with the highest precipitation. Regarding the budding activity index, all specimens expressed this phenophase as soon as the precipitation levels increased. In the dry period, all specimens showed senescence. Flower bud and flowering phenophases occurred synchronously. Fruiting had a higher activity index in periods of higher precipitation. Abstract Extracranial cerebrovascular disease has been the subject of intense research throughout the world, and is of paramount importance for vascular surgeons. This guideline, written by the Brazilian Society of Angiology and Vascular Surgery (SBACV), supersedes the 2015 guideline. Non-atherosclerotic carotid artery diseases were not included in this document. The purpose of this guideline is to bring together the most robust evidence in this area in order to help specialists in the treatment decision-making process. The AGREE II methodology and the European Society of Cardiology system were used for recommendations and levels of evidence. The recommendations were graded from I to III, and levels of evidence were classified as A, B, or C. Abstract Background: The relationship between viral infections and host factors holds high hopes for identifying the role of Interferon Lambda 3 (IFNL3) and Interleukin 6 (IL-6) polymorphisms in the development of Chronic Liver Disease (CLD) in patients infected with hepatitis Delta virus (HDV) in the Western Brazilian Amazon. Methods: Cross-sectional study conducted with a cohort of 40 chronic HDV patients, 27 with CLD and 13 without evident liver damage. Biological samples from the participants were analyzed using the polymerase chain reaction (PCR) technique, followed by sequencing by the automated Sanger method. Results: The rs8099917 T allele, from the IFNL3 gene, showed a higher frequency in both groups; however, it was not possible to establish an association with HDV infection [OR = 1.42 (0.42 - 4.75; p = 0.556 (95% CI). For IL-6, the rs1800795 G allele was superior to rs1800795 C. Analyzing both distributions in the studied groups, any association with HDV was absent (p > 0.05). Conclusion: The results suggest that the rs8099917 T/G (IFNL3) and rs1800795 G/C (IL-6) polymorphisms are not associated with the evolution of HDV in the studied population. ABSTRACT In 2000, Cleide Costa published a paper presenting the state of knowledge of the Neotropical Coleopte ra, with a focus on the Brazilian fauna. Twenty-four years later, thanks to the development of the Coleoptera section of the Taxonomic Catalog of the Brazilian Fauna (CTFB - Catálogo Taxonômico da Fauna do Brasil) through the collaboration of 100 coleopterists from all over the globe, we can build on Costa’s work and present an updated overview of the state of knowledge of the beetles from Brazil. There are currently 35,699 species in 4,958 genera and 116 families known to occur in the country, including representatives of all extant suborders and superfamilies. Our data show that the Brazilian beetle fauna is the richest on the planet, concentrating 9% of the world species diversity, with some estimates accounting to up to 15% of the global total. The most diverse family in numbers of genera is Cerambycidae (1,056 genera), while in number of species it is Chrysomelidae (6,079 species). Conotrachelus Dejean, 1835 (Curculionidae) is the most species-rich genus, with 570 species. The French entomologist Maurice Pic is the author who has contributed the most to the naming of species recorded from Brazil, with 1,794 valid names in 36 families, whereas the Brazilians Ubirajara R. Martins and Maria Helena M. Galileo are the only ones among the top-ten authors to have named species in the 21st century. RESUMO A adubação verde pode melhorar a viabilidade econômica e reduzir os impactos ambientais da agricultura. A pimenta rosa possui ecoplasticidade e desenvolvimento rápido, podendo ser cultivada sob diferentes sistemas de manejo. O objetivo deste estudo foi avaliar como a aplicação de bioestimulante influencia a produção de massa, liberação de nutrientes e produtividade da pimenta rosa em sucessão às culturas de cobertura, com ênfase no sequestro de carbono, saúde do solo e otimização da produção. O delineamento estatístico foi o de blocos casualizados, em arranjo fatorial 4 × 2 × 5, correspondendo a três adubos verdes (crotalária, estilosantes e milheto, além da testemunha - vegetação espontânea), com ou sem aplicação de FertBokashi® sobre o solo e cinco épocas de avaliação (0, 45, 90, 135 e 180 dias após o corte). Dos adubos verdes foram avaliadas as produções de massa e liberação de N, P e K. As plantas de pimenta rosa foram cortadas aos 270 dias após o transplante para mensuração das massas de folhas, caules e frutos. Os maiores teores de P e K na massa remanescente do milheto e da vegetação espontânea no dia e aos 180 dias após o corte sugere seu elevado potencial de ciclagem desses nutrientes. As plantas da pimenta rosa produziram mais massa de folhas (386,46 g por planta) e frutos (80,82 g por planta) quando cultivadas em sucessão à vegetação espontânea e com aplicação do FertBokashi®. ABSTRACT Cover crops have the potential to improve economic viability while simultaneously mitigating the environmental impacts of agriculture. Pink pepper, known for its ecoplasticity and rapid growth, can be cultivated under various management systems. This study aims to assess the influence of biostimulant application on the mass production, nutrient release, and productivity of pink pepper following cover crops, focusing on carbon sequestration, soil health, and production optimization. The experimental design employed a randomized block in a 4 × 2 × 5 factorial arrangement. Three cover crops (crotalaria, stylosanthes, and millet) were compared with a control-group (spontaneous vegetation), with or without FertBokashi® application. Evaluation occurred at five periods (0, 45, 90, 135, and 180 days post-cutting). Mass production and N, P, and K release were measured for over crops. Pink pepper plants were harvested at 270 days post-transplanting to assess leaf, stem, and fruit mass productivity. The residual mass of pearl millet and spontaneous vegetation showed the highest P and K levels at cutting and 180 days later, indicating significant potential for nutrient cycling by millet and certain spontaneous vegetation species. Pink pepper plants exhibited higher fresh leaf mass (386.46 g per plant) and fruit mass (80.82 g per plant) when cultivated after spontaneous vegetation and with FertBokashi® application. Resumo Fundamento: A adesão à uma alimentação adequada em macronutrientes é fundamental para a prevenção secundária de doenças cardiovasculares. Objetivo: Avaliar a prevalência de adesão às recomendações de consumo de ácidos graxos para prevenção e tratamento de doenças cardiovasculares, e estimar se a presença de determinados fatores de risco cardiovascular estaria associada à adesão. Métodos: Estudo transversal com os dados de linha de base de 2358 participantes do estudo "Brazilian Cardioprotective Nutritional Program Trial". Dados de consumo alimentar, e fatores de risco cardiovascular foram avaliados. Foi considerada, de acordo com a Sociedade Brasileira de Cardiologia, uma ingestão adequada de ácidos graxos poli-insaturados (AGPI) ≥10% do consumo total de energia diária, para ácidos graxos monoinsaturados (AGM), 20% e para ácidos graxos saturados (AGS), <7%. Na análise estatística foi considerando nível de significância de 5%. Resultados: Nenhum participante aderiu a todas as recomendações de forma simultânea e mais da metade (1482 [62,9%]) não aderiu a nenhuma recomendação. A adesão exclusivamente à recomendação de AGS foi a mais prevalente, sendo cumprida por 659 (28%) dos participantes, seguida da adesão exclusivamente à recomendação de AGP (178 [7,6%]) e de AGM (5 [0,2%]). Não houve associação entre o número de comorbidades e a adesão às recomendações nutricionais (p =0,269). Os participantes da região Nordeste do país apresentaram maior proporção de adesão às recomendações para consumo de AGS (38,42%), e menor para ingestão de AGPI (3,52%) (p <0,001) em comparação às demais. Background: Adhering to a diet adequate in macronutrients is crucial for the secondary prevention of cardiovascular diseases. Objective: To assess the prevalence of adherence to recommendations for the consumption of dietary fatty acids for the prevention and treatment of cardiovascular diseases and to estimate whether the presence of certain cardiovascular risk factors would be associated with adherence. Methods: Cross-sectional study using baseline data from 2,358 participants included in the "Brazilian Cardioprotective Nutritional Program Trial". Dietary intake and cardiovascular risk factors were assessed. Adequate intake of polyunsaturated fatty acids (PUFA) was considered as ≥10% of total daily energy intake; for monounsaturated fatty acids (MUFA), 20%; and for saturated fatty acids (SFA), <7% according to the Brazilian Society of Cardiology. A significance level of 5% was considered in the statistical analysis. Results: No participant adhered to all recommendations simultaneously, and more than half (1,482 [62.9%]) did not adhere to any recommendation. Adherence exclusively to the SFA recommendation was the most prevalent, fulfilled by 659 (28%) participants, followed by adherence exclusively to the PUFA (178 [7.6%]) and MUFA (5 [0.2%]) recommendations. There was no association between the number of comorbidities and adherence to nutritional recommendations (p = 0.269). Participants from the Brazilian Northeast region showed a higher proportion of adherence to SFA consumption recommendations (38.42%) and lower adherence to PUFA intake (3.52%) (p <0.001) compared to other regions. ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. Abstract Maternal endotoxemia disturbs the intrauterine environment, impairs nephrogenesis, and increases the risk of hypertension and kidney disease in adulthood. Here, it was investigated whether maternal treatment with the water extract of Moringa oleifera seeds (WEMoS) or the water-soluble M. oleifera seed lectin (WSMoL) prevents the oxidative stress induced by lipopolysaccharide (LPS) in pregnant rats, and the renal injury and hypertension in the adult offspring. The administration of WEMoS or WSMoL prevented the stimulatory effects of LPS on lipid peroxidation in the maternal-placenta-fetuses environment. The impact of WEMoS was linked to decreased superoxide anions production in the placenta. The effects of WSMoL were parallel to the inhibition of superoxide anion production and NADPH oxidase activity. The WSMoL also prevented increased NADPH oxidase activity in the fetal kidney. The LPS offspring presented higher systolic blood pressure (SBP) and increased lipid peroxidation, reactive oxygen species (ROS), NADPH oxidase activity, and nitrate/nitrite in the kidney; the maternal treatment with WEMoS and WSMoL prevented these changes. RESUMO: O presente estudo objetivou investigar a prevalência de Leishmania spp., Mycoplasma spp. hemotrópico, patógenos transmitidos por carrapatos (PTC), e coinfecção em cães com sinais clínicos sugestivos de leishmaniose visceral (LV), determinar os fatores associados à infecção, e mapear a distribuição de cães coinfectados em uma área endêmica no Nordeste do Brasil. Amostras de sangue de 168 cães foram avaliadas por análises sorológicas para Leishmania spp., Anaplasma spp., Ehrlichia spp., Babesia spp., e ensaio molecular para Leishmania spp., Anaplasma platys, Ehrlichia canis, Babesia spp., e Mycoplasma hemotrópico. Pelas análises sorológicas e moleculares, 29,8% e 5,9% dos cães apresentaram coinfecção, respectivamente. Na análise de regressão, a soropositividade para Ehrlichia spp., Babesia spp., e Leishmania spp. foram significantemente associadas com a presença de petéquias, cães jovens, e perda de peso. O diagnóstico sorológico revelou que a coexposição à Babesia spp. e Ehrlichia spp. está associada com febre e trombocitopenia, havendo associação entre a soropositividade para Ehrlichia spp. e Babesia spp. em cães soropositivos para Leishmania spp. A presença de DNA de Mycoplasma foi associada à anorexia. Desta forma, cães com sinais de LV possuem coinfecção com outros patógenos, reforçando a importância deste estudo para um melhor entendimento dessas coinfecções em cães de áreas endêmicas. ABSTRACT: The present study aimed to investigate the occurrence of Leishmania spp., hemotropic Mycoplasma spp., tick-borne pathogens (TBP), and co-infection in dogs with clinical signs suggestive of visceral leishmaniasis (VL). It also aimed to determine the factors associated with infection and to map the distribution of co-infected dogs in an endemic area in the Northeast region of Brazil. Blood samples from 168 dogs were evaluated for serological analysis to Leishmania spp., Anaplasma spp., Ehrlichia spp., Babesia spp., and molecular assays to Leishmania spp., Anaplasma platys, Ehrlichia canis, Babesia spp., and hemotropic Mycoplasma spp. In serological and molecular analysis, 29.8% and 5.9% of dogs were co-infected. In the regression analysis, seropositivity for Ehrlichia spp., Babesia spp., and Leishmania spp. was significantly associated with the presence of petechiae, young dogs, and weight loss. Serology revealed that co-exposure with Babesia spp. and Ehrlichia spp. was associated with fever and thrombocytopenia, and there was an association between seropositivity for Ehrlichia spp. and Babesia spp. in dogs seropositive for Leishmania spp. The presence of hemotropic Mycoplasma spp. DNA was associated with anorexia. Hypertension is one of the primary modifiable risk factors for morbidity and mortality worldwide, being a major risk factor for coronary artery disease, stroke, and kidney failure. Furthermore, it is highly prevalent, affecting more than one-third of the global population.
Blood pressure measurement is a MANDATORY procedure in any medical care setting and is carried out by various healthcare professionals. However, it is still commonly performed without the necessary technical care. Since the diagnosis relies on blood pressure measurement, it is clear how important it is to handle the techniques, methods, and equipment used in its execution with care.
It should be emphasized that once the diagnosis is made, all short-term, medium-term, and long-term investigations and treatments are based on the results of blood pressure measurement. Therefore, improper techniques and/or equipment can lead to incorrect diagnoses, either underestimating or overestimating values, resulting in inappropriate actions and significant health and economic losses for individuals and nations.
Once the correct diagnosis is made, as knowledge of the importance of proper treatment advances, with the adoption of more detailed normal values and careful treatment objectives towards achieving stricter blood pressure goals, the importance of precision in blood pressure measurement is also reinforced.
Blood pressure measurement (described below) is usually performed using the traditional method, the so-called casual or office measurement. Over time, alternatives have been added to it, through the use of semi-automatic or automatic devices by the patients themselves, in waiting rooms or outside the office, in their own homes, or in public spaces. A step further was taken with the use of semi-automatic devices equipped with memory that allow sequential measurements outside the office (ABPM; or HBPM) and other automatic devices that allow programmed measurements over longer periods (HBPM).
Some aspects of blood pressure measurement can interfere with obtaining reliable results and, consequently, cause harm in decision-making. These include the importance of using average values, the variation in blood pressure during the day, and short-term variability. These aspects have encouraged the performance of a greater number of measurements in various situations, and different guidelines have advocated the use of equipment that promotes these actions. Devices that perform HBPM or ABPM, which, in addition to allowing greater precision, when used together, detect white coat hypertension (WCH), masked hypertension (MH), sleep blood pressure alterations, and resistant hypertension (RHT) (defined in Chapter 2 of this guideline), are gaining more and more importance.
Taking these details into account, we must emphasize that information related to diagnosis, classification, and goal setting is still based on office blood pressure measurement, and for this reason, all attention must be given to the proper execution of this procedure.
La hipertensión arterial (HTA) es uno de los principales factores de riesgo modificables para la morbilidad y mortalidad en todo el mundo, siendo uno de los mayores factores de riesgo para la enfermedad de las arterias coronarias, el accidente cerebrovascular (ACV) y la insuficiencia renal. Además, es altamente prevalente y afecta a más de un tercio de la población mundial.
La medición de la presión arterial (PA) es un procedimiento OBLIGATORIO en cualquier atención médica o realizado por diferentes profesionales de la salud. Sin embargo, todavía se realiza comúnmente sin los cuidados técnicos necesarios. Dado que el diagnóstico se basa en la medición de la PA, es claro el cuidado que debe haber con las técnicas, los métodos y los equipos utilizados en su realización.
Debemos enfatizar que una vez realizado el diagnóstico, todas las investigaciones y tratamientos a corto, mediano y largo plazo se basan en los resultados de la medición de la PA. Por lo tanto, las técnicas y/o equipos inadecuados pueden llevar a diagnósticos incorrectos, subestimando o sobreestimando valores y resultando en conductas inadecuadas y pérdidas significativas para la salud y la economía de las personas y las naciones.
Una vez realizado el diagnóstico correcto, a medida que avanza el conocimiento sobre la importancia del tratamiento adecuado, con la adopción de valores de normalidad más detallados y objetivos de tratamiento más cuidadosos hacia metas de PA más estrictas, también se refuerza la importancia de la precisión en la medición de la PA.
La medición de la PA (descrita a continuación) generalmente se realiza mediante el método tradicional, la llamada medición casual o de consultorio. Con el tiempo, se han agregado alternativas a través del uso de dispositivos semiautomáticos o automáticos por parte del propio paciente, en salas de espera o fuera del consultorio, en su propia residencia o en espacios públicos. Se dio un paso más con el uso de dispositivos semiautomáticos equipados con memoria que permiten mediciones secuenciales fuera del consultorio (AMPA; o MRPA) y otros automáticos que permiten mediciones programadas durante períodos más largos (MAPA).
Algunos aspectos en la medición de la PA pueden interferir en la obtención de resultados confiables y, en consecuencia, causar daños en las decisiones a tomar. Estos incluyen la importancia de usar valores promedio, la variación de la PA durante el día y la variabilidad a corto plazo. Estos aspectos han alentado la realización de un mayor número de mediciones en diversas situaciones, y diferentes pautas han abogado por el uso de equipos que promuevan estas acciones. Los dispositivos que realizan MRPA o MAPA, que además de permitir una mayor precisión, cuando se usan juntos, detectan la hipertensión de bata blanca (HBB), la hipertensión enmascarada (HM), las alteraciones de la PA durante el sueño y la hipertensión resistente (HR) (definida en el Capítulo 2 de esta guía), están ganando cada vez más importancia.
Teniendo en cuenta estos detalles, debemos enfatizar que la información relacionada con el diagnóstico, la clasificación y el establecimiento de objetivos todavía se basa en la medición de la presión arterial en el consultorio, y por esta razón, se debe prestar toda la atención a la ejecución adecuada de este procedimiento.
A hipertensão arterial (HA) é um dos principais fatores de risco modificáveis para morbidade e mortalidade em todo o mundo, sendo um dos maiores fatores de risco para doença arterial coronária, acidente vascular cerebral (AVC) e insuficiência renal. Além disso, é altamente prevalente e atinge mais de um terço da população mundial.
A medida da PA é procedimento OBRIGATÓRIO em qualquer atendimento médico ou realizado por diferentes profissionais de saúde. Contudo, ainda é comumente realizada sem os cuidados técnicos necessários. Como o diagnóstico se baseia na medida da PA, fica claro o cuidado que deve haver com as técnicas, os métodos e os equipamentos utilizados na sua realização.
Deve-se reforçar que, feito o diagnóstico, toda a investigação e os tratamentos de curto, médio e longo prazos são feitos com base nos resultados da medida da PA. Assim, técnicas e/ou equipamentos inadequados podem levar a diagnósticos incorretos, tanto subestimando quanto superestimando valores e levando a condutas inadequadas e grandes prejuízos à saúde e à economia das pessoas e das nações.
Uma vez feito o diagnóstico correto, na medida em que avança o conhecimento da importância do tratamento adequado, com a adoção de valores de normalidade mais detalhados e com objetivos de tratamento mais cuidadosos no sentido do alcance de metas de PA mais rigorosas, fica também reforçada a importância da precisão na medida da PA.
A medida da PA (descrita a seguir) é habitualmente feita pelo método tradicional, a assim chamada medida casual ou de consultório. Ao longo do tempo, foram agregadas alternativas a ela, mediante o uso de equipamentos semiautomáticos ou automáticos pelo próprio paciente, nas salas de espera ou fora do consultório, em sua própria residência ou em espaços públicos. Um passo adiante foi dado com o uso de equipamentos semiautomáticos providos de memória que permitem medidas sequenciais fora do consultório (AMPA; ou MRPA) e outros automáticos que permitem medidas programadas por períodos mais prolongados (MAPA).
Alguns aspectos na medida da PA podem interferir na obtenção de resultados fidedignos e, consequentemente, causar prejuízo nas condutas a serem tomadas. Entre eles, estão: a importância de serem utilizados valores médios, a variação da PA durante o dia e a variabilidade a curto prazo. Esses aspectos têm estimulado a realização de maior número de medidas em diversas situações, e as diferentes diretrizes têm preconizado o uso de equipamentos que favoreçam essas ações. Ganham cada vez mais espaço os equipamentos que realizam MRPA ou MAPA, que, além de permitirem maior precisão, se empregados em conjunto, detectam a HA do avental branco (HAB), HA mascarada (HM), alterações da PA no sono e HA resistente (HAR) (definidos no Capítulo 2 desta diretriz).
Resguardados esses detalhes, devemos ressaltar que as informações relacionadas a diagnóstico, classificação e estabelecimento de metas ainda são baseadas na medida da PA de consultório e, por esse motivo, toda a atenção deve ser dada à realização desse procedimento.
