ABSTRACT Introduction: Coarctation of the aorta (CoA) is a narrowing of the thoracic aorta that often manifests as discrete stenosis but may be tortuous or in long segment. The study aimed to evaluate pre and post-surgical aspects of pediatric patients submitted to CoA surgical correction and to identify possible predisposing factors for aortic recoarctation. Methods: Twenty-five patients were divided into groups according to presence (N=8) or absence (N=17) of recoarctation after surgical correction of CoA and evaluated according to clinical-demographic profile, vascular characteristics via computed angiotomography (CAT), and other pathological conditions. Results: Majority of males (64%), ≥ 15 days old (76%), ≥ 2.5 kg (80%). There was similarity between groups with and without recoarctation regarding sex (male: 87% vs. 53%; P=0.277), age (≥ 15 days: 62.5 vs. 82%; P=0.505), and weight (≥ 2.5 kg: 87.5 vs. 76.5; P=0,492). Altered values of aortic root/Valsalva diameter, proximal transverse arch, and distal isthmus, and normal values for aorta prevailed in preoperative CAT. Normal values for the aortic root/Valsalva sinus diameter were observed with and without recoarctation, the same for both groups regarding ascending and descending aorta in postoperative CAT. No significant difference for altered values of proximal transverse arch and alteration in distal isthmus was observed. Conclusion: No predictive risk for recoarctation was observed. CTA proved to be important in CoA diagnosis and management, since CoA is mainly related with altered diameter of aortic root/sinus of Valsalva and proximal and distal aortic arch/isthmus, however, it failed to show predictive risk for recoarctation. Resumo Este estudo investigou pela primeira vez, a eficácia de banhos terapêuticos com óleo essencial (OE) de Piper hispidum contra monogenéticos Anacanthorus spathulatus, Notozothecium janauachensis, Mymarothecium boegeri e Linguadactyloides brinkmanni das brânquias de Colossoma macropomum, bem como os efeitos hematológicos e histológicos neste peixe. Foram usados 100 mg/L de óleo essencial de P. hispidum e dois grupos controles (água do tanque de cultivo e água do tanque de cultivo com álcool 70%) expostos durante 1 hora por dia, com intervalos de 48 horas durante 3 dias, três repetições cada. Banhos terapêuticos com 100 mg/L de OE de P. hispidum tiveram uma eficácia de 78,6% contra monogenéticos. A toxicidade foi baixa, pois ocorreram poucas alterações fisiológicas e histopatológicas que não comprometeram o funcionamento das brânquias dos peixes. Abstract This study investigated for the first time the effectiveness of therapeutic baths with essential oil (EO) of Piper hispidum against monogeneans Anacanthorus spathulatus, Notozothecium janauachensis, Mymarothecium boegeri and Linguadactyloides brinkmanni from the gills of Colossoma macropomum, as well as the hematological and histological effects on this fish. In therapeutic baths, 100 mg/L of P. hispidum essential oil and two control groups (water from the culture tank and water from the culture tank with 70% alcohol) were exposed for 1 h/day, with intervals of 48 hours for 3 days, and three replicates each were used. Therapeutic baths with 100 mg/L of P. hispidum essential oil had an efficacy of 78.6% against monogeneans. Abstract The aim of this study was to investigate the effect of acid challenge on the activation of matrix metalloproteinases (MMPs) in the Dentinoenamel junction of primary and permanent teeth submitted to radiotherapy. For this purpose, a total of 178 dental fragments obtained from molars were used, and randomly divided into 2 groups (primary and permanent teeth) / 4 experimental subgroups (irradiated and non-irradiated, demineralized and non-demineralized). The fragments were exposed to radiation, with a dose fraction of 2 Gy, for 5 consecutive days, until a total dose of 60 Gy was reached, with a total of 30 cycles, for 6 weeks. To determine the activity of MMPs on the dentinoenamel junction (DEJ), in situ zymography assays on 0.6mm dental fragments were performed. To assess whether MMP activity would be impacted by an acidic environment, the fragments were placed in a demineralizing solution (pH of 4.8). The finding was that irradiation activated MMPs in DEJ and these effects were more evident in permanent when compared with primary teeth. When the effect of an acid challenge on MMPs activity was investigated, demineralization was observed not to increase MMPs activity in non-irradiated teeth, but it did increase MMPs activity in irradiated teeth. RESUMO Para práticas de gestão e tomada de decisões eficazes, as incertezas associadas aos modelos climáticos regionais (MCR) e aos seus cenários precisam ser avaliadas no contexto das alterações climáticas. Este estudo analisa tendências de longo prazo em conjuntos de dados de precipitação e temperatura (valores máximos e mínimos) oriundos da NASA, da Earth Exchange Global Daily Downscaled Prediction (NEX-GDDP), por meio do Caminho de Concentração Representativa (RCP 4.5 e 8.5), na Bacia do Rio São Francisco utilizando o método REA (Reliable Ensemble Average). Em cada grade, o multimodelo construído foi corrigido para períodos anuais, secos, chuvosos e pré-estação – para períodos históricos (1961-2005) e futuros (2006-2100), usando o software CMhyd. Para detectar tendências de precipitação e temperatura mínima e máxima na bacia, foram utilizados quatro diferentes métodos juntamente com o conjunto de dados que originou o multimodelo. Os quatro métodos utilizados foram: Mann-Kendall, teste de pré-clareamento de Mann-Kendall, pré-clareamento com correção de viés e correlação de Spearman. Na análise das métricas de precipitação e temperatura, os resultados do NRMSD mostraram que, de forma geral, o modelo CSIRO apresentou resultados mais satisfatórios em todas as regiões fisiográficas. O coeficiente de correlação de Person mostrou melhor ajuste da precipitação para os modelos MIROC5, EC.EARTH e NORESMI1, nas áreas do submédio e alto São Francisco. Para a temperatura mínima, os modelos CSIRO e NORESMI1 apresentaram o melhor ajuste, em geral. Na temperatura máxima, os modelos EC.EARTH e CSIRO apresentaram resultados mais satisfatórios. No que diz respeito à análise de tendência, os resultados indicaram uma tendência crescente na temperatura média anual e na precipitação em toda a bacia. Quando analisados por sub-região, os resultados mostram uma tendência crescente nas temperaturas médias mínimas e máximas mensais no médio e baixo SFRB, enquanto a precipitação média mensal aumenta durante a estação chuvosa e pré-estação no Alto São Francisco. ABSTRACT For effective management practices and decision-making, the uncertainties associated with regional climate models (RCMs) and their scenarios need to be assessed in the context of climate change. This study analyzes long-term trends in precipitation and temperature data sets (maximum and minimum values) from the NASA, Earth Exchange Global Daily Downscaled Prediction (NEX-GDDP), under the São Francisco River Basin Representative Concentration Path (RCP) 4.5 and 8.5, using the REA (Reliable Ensemble Average) method. In each grid, the built multi-model was bias-corrected using the CMhyd software for annual, dry, wet, and pre-season periods – for historical (1961-2005) and future (2006-2100) periods. The multi-model and four different methods, namely: The Mann-Kendall, Mann-Kendall pre-brightening test, bias-corrected pre-brightening, and Spearman correlation, were used to detect trends in precipitation, and minimum and maximum temperature. In the analysis of precipitation and temperature metrics, the results for the NRMSD showed that, in general, the CSIRO model presented more satisfactory results in all physiographic regions. Person's correlation coefficient showed a better adjustment of precipitation for the MIROC5, EC.EARTH and NORESMI1 models, in areas of sub-medium and upper São Francisco. For the minimum temperature, the CSIRO and NORESMI1 models showed the best fit, in general. At maximum temperature, the EC.EARTH and CSIRO models showed more satisfactory results. With regard to trend analysis, the results indicated an increasing trend in mean annual temperature and precipitation across the basin. When analyzed by subregion, the results show an increasing trend in monthly average minimum and maximum temperatures in the middle and lower SFRB, while average monthly rainfall increases during the rainy season and preseason in Upper São Francisco. Resumo Este estudo teve como objetivo identificar a influência da substituição do agregado graúdo natural por agregado graúdo reciclado de concreto na espessura e no volume da pasta cimentícia sob a variação da relação água/cimento nas propriedades frescas e endurecidas do concreto permeável. A espessura teórica da pasta determinada foi ineficiente para dosagem de concreto permeável. Porém, constatou-se que o volume da pasta foi o parâmetro que melhor se relacionou com as propriedades mecânicas e de permeabilidade do material estudado. O concreto permeável com agregados naturais apresentou maior resistência à compressão que o concreto com agregados reciclados, mas ambos atenderam às normas vigentes. Todos os concretos produzidos, exceto com agregado reciclado de relação a/c 0,25, atingiram a resistência à tração na flexão mínima de projeto. O concreto permeável com agregados reciclados apresentou perda de massa mais significativa por abrasão e foi mais poroso que o concreto com agregados naturais. Abstract This study aimed to identify the influence of replacing coarse natural aggregate with coarse recycled concrete aggregate on the thickness and volume of the cementitious paste under the variation of the water/cement ratio in the fresh and hardened properties of pervious concrete. The determined theoretical paste thickness was inefficient for pervious concrete dosage. However, it was found that the paste volume was the parameter that was best related to the mechanical and permeability properties of the studied material. Pervious concrete with natural aggregates showed higher compressive strength than concrete with recycled aggregates, although both met the regulations. All the concretes produced, except one with recycled aggregate of w/c ratio 0.25, reached the tensile strength in the minimum design bending. Pervious concrete with recycled aggregates showed more significant mass loss by abrasion and more porosity than concretes with natural aggregates. Resumo Fundamento Dados robustos sobre a curva de aprendizagem (LC) da substituição da válvula aórtica transcateter (TAVR) são escassos nos países em desenvolvimento. Objetivo Avaliar a LC da TAVR no Brasil ao longo do tempo. Métodos Analisamos dados do registro brasileiro de TAVR de 2008 a 2023. Pacientes de cada centro foram numerados cronologicamente em número sequencial de caso (NSC). A LC foi realizada usando um spline cúbico restrito ajustado para o EuroSCORE-II e o uso de próteses de nova geração. Ainda, os desfechos hospitalares foram comparados entre grupos definidos de acordo com o nível de experiência, com base no NSC: 1º ao 40º caso (experiência inicial), 41º ao 80º caso (experiência básica), 81º ao 120º caso (experiência intermediária) e 121º caso em diante (experiência alta). Análises adicionais foram conduzidas de acordo com o número de casos tratados antes de 2014 (>40 e ≤40 procedimentos). O nível de significância adotado foi p <0,05. Resultados Foram incluídos 3194 pacientes de 25 centros. A idade média foi 80,7±8,1 anos e o EuroSCORE II médio foi 7±7,1. A análise da LC demonstrou uma queda na mortalidade hospitalar ajustada após o tratamento de 40 pacientes. Um patamar de nivelamento na curva foi observado após o caso 118. A mortalidade hospitalar entre os grupos foi 8,6%, 7,7%, 5,9%, e 3,7% para experiência inicial, básica, intermediária e alta, respectivamente (p<0,001). A experiência alta foi preditora independente de mortalidade mais baixa (OR 0,57, p=0,013 vs. experiência inicial). Centros com baixo volume de casos antes de 2014 não mostraram uma redução significativa na probabilidade de morte com o ganho de experiência, enquanto centros com alto volume de casos antes de 2014 apresentaram uma melhora contínua após o caso de número 10. Conclusão Observou-se um fenômeno de LC para a mortalidade hospitalar do TAVR no Brasil. Abstract Background Robust data on the learning curve (LC) of transcatheter aortic valve replacement (TAVR) are lacking in developing countries. Objective To assess TAVR’s LC in Brazil over time. Methods We analyzed data from the Brazilian TAVR registry from 2008 to 2023. Patients from each center were numbered chronologically in case sequence numbers (CSNs). LC was performed using restricted cubic splines adjusted for EuroSCORE-II and the use of new-generation prostheses. Also, in-hospital outcomes were compared between groups defined according to the level of experience based on the CSN: 1st to 40th (initial-experience), 41st to 80th (early-experience), 81st to 120th (intermediate-experience), and over 121st (high-experience). Additional analysis was performed grouping hospitals according to the number of cases treated before 2014 (>40 and ≤40 procedures). The level of significance adopted was <0.05. Results A total of 3,194 patients from 25 centers were included. Mean age and EuroSCORE II were 80.7±8.1 years and 7±7.1, respectively. LC analysis demonstrated a drop in adjusted in-hospital mortality after treating 40 patients. A leveling off of the curve was observed after case #118. In-hospital mortality across the groups was 8.6%, 7.7%, 5.9%, and 3.7% for initial-, early-, intermediate-, and high-experience, respectively (p<0.001). High experience independently predicted lower mortality (OR 0.57, p=0.013 vs. initial experience). Low-volume centers before 2014 showed no significant decrease in the likelihood of death with gained experience, whereas high-volume centers had a continuous improvement after case #10. Conclusion A TAVR LC phenomenon was observed for in-hospital mortality in Brazil. Resumo O ensino remoto possibilitou o isolamento social de estudantes e profissionais da educação durante a pandemia de Covid-19, mas foi permeado de críticas de diversos setores da sociedade brasileira, entre os quais instituições da educação e de psicologia. Diante da carência de discussão acadêmica sobre o fenômeno, este trabalho objetiva identificar e analisar críticas de entidades dessas duas áreas ao ensino remoto na educação básica. Trata-se de uma pesquisa qualitativa e documental, cujo corpus é constituído de 34 documentos, publicados entre 11 de março e 11 de maio de 2020 por seis entidades. A análise de conteúdo extraiu 143 trechos (recortes), que foram organizados em cinco categorias. Os resultados evidenciam no ensino remoto aspectos prejudiciais ao processo de ensino-aprendizagem: acesso limitado ou inexistente de alunas(os) e suas famílias aos recursos tecnológicos; precarização do trabalho das(os) profissionais; violação do direito à educação; mercantilização da educação; e esvaziamento da função social da escola. O ensino remoto foi desenvolvido de maneira aligeirada e descontextualizada, desconsiderando marcadores socioeconômicos e raciais, e promoveu a intensificação do trabalho docente e o recrudescimento das desigualdades educacionais, que se mostraram estratégias políticas e técnicas de governo. Numa conjuntura de ultraliberalismo obscurantista, são intensificados os ataques à escola pública, não sem resistência popular, democrática e libertária. Essa disputa se estende e se atualiza no contexto pandêmico. As manifestações das instituições evidenciam isso. Se todas elas apresentam elementos críticos, estes não são homogêneos e pautados nos mesmos interesses, que se ocupam ou do mercado ou dos direitos constitucionais das(os) alunas(os), suas famílias e profissionais. Abstract Remote teaching ensured social distancing for students and education professional during the Covid-19 pandemic, but received criticism from various sectors of Brazilian society, including education and psychology institutions. Given the lack of academic discussion concerning the phenomenon, this article identifies and analyzes criticisms made by education and psychology institutes to remote teaching in basic education. A qualitative and documentary research was conducted with 34 documents, published between March 11 and May 11, 2020 by six entities. Content analysis extracted 143 excerpts, which were organized into five categories. Results show that remote teaching is detrimental to the teaching-learning process: limited or non-existent access of students and their families to technological resources; precariousness of teaching work; violation of the right to education; commodification of education; and stripping of the school’s social role. Developed in a rapid and decontextualized manner, disregarding socioeconomic and racial markers, remote teaching increased teaching work and further expanded educational inequalities, which proved to be political and technical government strategies. Attacks on public schools increase in an ultraliberal and obscurantist context, but not without popular, democratic and libertarian resistance. This dispute is extended and updated in the pandemic context, as evinced by the institutional manifestations. Although they all present criticisms, these are neither homogeneous nor based on the same interests, with some showing concern for the market while others focus on constitutional rights. ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. Abstract Background Pathologies involving the heart are still the main causes of death, and acute myocardial infarction (AMI) is consistently present in this index. The two-minute walk test (2MWT) is ideal for assessing the functional capacity of this patient. Objective To describe the feasibility of the 2MWT in older people after AMI. Methods This is a cross-sectional study. At hospital discharge, patients were invited to perform the 2MWT. Before starting the test, systolic blood pressure (SBP), diastolic blood pressure (DBP), peripheral oxygen saturation (SpO2), heart rate (HR) and the Double Product (DP) were checked. After checking the vital signs, the patients were accompanied by an examiner, who was positioned laterally to ensure safety and verbally encouraged during the test; after the completion of the test, all vital signs were reassessed in two moments, at the immediate end and after 20 minutes of rest. ANOVA was used for the comparison of pre and post-test and pre and recovery. A p<0.05 was considered significant. Results We evaluated 51 patients, 4 (80%) males with a mean age of 67±8 years. The distance walked on the 2MWT had a mean of 157 ± 22 meters. The SBPmmHg Pre-Test 112±21 vs 131±15 Post-Test (p=0.24) and 119±22 at Recovery (p = 0.34) and HR (bpm) Pre-Test 75±15 vs 89±19 Post-Test (p=0.15) and 79±15 at Recovery (p = 0.59). Abstract: The minimally invasive endodontic access is not directly associated with tooth discoloration in the presence of bioceramic or epoxy resin-based root canal sealers. This study aimed to evaluate the influence of minimally invasive access and endodontic sealer composition on the color stability of endodontically-treated teeth, the restorative material adaptation, and the presence of remaining filling material in the pulp chamber. Endodontic access surgery was performed in maxillary central incisors, either through conservative or minimally invasive approaches, and the root was filled with AH Plus or Bio-C Sealer. The crown color was measured with a spectrophotometer at baseline and after root obturation, restoration, and specimen storage for one year in an oven. The occurrence of voids in the restoration and the remaining filling material was analyzed using micro-CT scans. The Yellowness Index (YI) and color changes (∆E00) were calculated after each color measurement. Data of micro-CT were submitted to 2-way ANOVA, and YI and ∆E00 were analyzed with repeated-measures ANOVA. Pair-wise comparisons were performed with Tukey's test (α = 0.05). The experimental conditions had no effect on the presence of the remaining material. The minimally invasive access associated with Bio-C Sealer resulted in more voids between the restoration and the remaining filling material. Only the evaluation time affected YI and ∆E00 values. Specimens became more yellow after filling and storage in the oven (the highest ∆E00 values). Resumo Objetivo Avaliar o impacto da raça (negra versus não negra) nos desfechos maternos e perinatais de gestantes com COVID-19 no Brasil. Métodos Esta é uma subanálise da REBRACO, um estudo de coorte multicêntrico brasileiro desenhado para avaliar o impacto da COVID-19 em mulheres grávidas. De fevereiro de 2020 a fevereiro de 2021, 15 maternidades do Brasil coletaram dados de mulheres com sintomas respiratórios. Selecionamos todas as mulheres com teste positivo para COVID-19; em seguida, as dividimos em dois grupos: mulheres negras e não negras. Finalmente, comparamos, entre os grupos, os resultados sociodemográficos, maternos e perinatais. Obtivemos a frequência dos eventos em cada grupo e comparamos usando o teste X2; Valores de p <0,05 foram considerados significativos. Também estimamos o odds ratio (OR) e os intervalos de confiança (IC). Resultados 729 mulheres sintomáticas foram incluídas no estudo; desses, 285 foram positivos para COVID-19, 120 (42,1%) eram negros e 165 (57,9%) não eram negros. As mulheres negras apresentaram pior escolaridade (p = 0,037). O tempo de acesso ao sistema de saúde foi semelhante entre os dois grupos, com 26,3% incluídos com sete ou mais dias de sintomas. Síndrome respiratória aguda grave (OR 2,22 CI 1,17–4,21), admissão em unidade de terapia intensiva (OR 2,00 CI 1,07–3,74) e dessaturação na admissão (OR 3,72 CI 1,41–9,84) foram mais prováveis de ocorrer entre mulheres negras. A mortalidade materna foi maior entre as negras (7,8% vs. 2,6%, p = 0,048). Os resultados perinatais foram semelhantes entre os dois grupos. Abstract Objective To evaluate the impact of the race (Black versus non-Black) on maternal and perinatal outcomes of pregnant women with COVID-19 in Brazil. Methods This is a subanalysis of REBRACO, a Brazilian multicenter cohort study designed to evaluate the impact of COVID-19 on pregnant women. From February2020 until February 2021, 15 maternity hospitals in Brazil collected data on women with respiratory symptoms. We selected all women with a positive test for COVID-19; then, we divided them into two groups: Black and non-Black women. Finally, we compared, between groups, sociodemographic, maternal, and perinatal outcomes. We obtained the frequency of events in each group and compared them using X2 test; p-values < 0.05 were considered significant. We also estimated the odds ratio (OR) and confidence intervals (CI). Results 729 symptomatic women were included in the study; of those, 285 were positive for COVID-19, 120 (42.1%) were Black, and 165 (57.9%) were non-Black. ABSTRACT Economic development based on cleaner industrial technologies has been increasingly studied in recent years. Cementitious composites reinforced with lignocellulosic fibers can be a sustainable alternative for use in engineering. The study aimed to evaluate the technological properties of cementitious composites produced from natural and industrial coconut fiber to compare the reinforcement with wood particles. Physical tests (density, water absorption, and thickness swelling) and mechanical tests (static bending) were carried out in four treatments, which varied the fiber and cement contents. It was found that treatment with natural coconut fiber had the lowest density value, 0.59 g/cm3, and treatment with industrial coconut fiber had the lowest water absorption value for the times and times of 2 and 24 h, 0.19, and 0.38%, respectively, and the treatment with wood particles, the highest values of modulus of elasticity and rupture, 1,897 MPa and 2.44 MPa. ABSTRACT Background: Chagas disease (CD) is caused by Trypanosoma cruzi and transmitted by triatomines. Historical information from the 20th century demonstrates T. cruzi records in the metropolitan region of Salvador (MRS), the third largest urban agglomeration in the Brazilian Northeast and the eighth largest in Brazil, an area with intense migratory activity from CD-endemic regions. Therefore, this study aimed to evaluate CD indicators (prevalence and mortality) in the MRS. Methods: A mixed ecological and descriptive study was conducted using secondary data. We analyzed data from 2008 to 2015: deaths due to CD, self-reported cases of CD, and blood donors that were non-negative for T. cruzi infection. Results: São Francisco do Conde was one of the municipalities with the highest mortality rates due to CD. The seroprevalence rates varied by year and municipality; those with the highest values were 2008: Vera Cruz, 2009: Mata de São João, 2010: Dias D'Ávila, 2011 and 2015: São Francisco do Conde, 2012: São Sebastião do Passé, and 2013 and 2014: Pojuca. Spatial correlations between the municipalities were not detected. Conclusions: We conclude that CD is present in the MRS. The indicators analyzed in the MRS are below-state-level data. Given the importance of indicator analysis for the surveillance and control of CD at the state and national levels, it is important to strengthen the surveillance program at the municipal level, including the regions classified as low risk for T. cruzi vector transmission.
