Abstract Objective: To describe study protocol and initial results of research project COVID-Inconfidentes. Method: This paper described the methodological procedures adopted and the prevalence of the SARS-CoV-2 infection in the population. A household survey was conducted between October and December 2020, in two historic cities of Brazil's mining region. Anti-SARS-CoV-2 antibody was detected using the Wondfo® rapid test. The face-to face interview consisted of administration of a questionnaire containing registration data, sociodemographic and economic variables, living habits, general health condition, mental health, sleep habits, and eating and nutrition. Results: We evaluated 1,762 residents, of which 764 (43.4 %) were in Mariana and 998 (56.6 %) in Ouro Preto. For both cities, 51.9 % of the interviewees were female, with a predominance of the age range 35 to 59 years old (47.2 %). The prevalence of the SARS-CoV-2 infection was 5.5 % in both cities, 6.2 % in Ouro Preto, and 4.7 % in Mariana (p-value > 0.05). Conclusion: The study was effective to estimate the seroprevalence of infection by the virus and its findings will enable further analyses of the health conditions of the population related to social isolation and the risk of infection with SARS-CoV-2.
Resumen Describir el protocolo de estudio y los resultados iniciales del proyecto de investigación COVID Inconfidentes. Método: Este artículo describe los procedimientos metodológicos adoptados y la prevalencia de la infección por SARS-CoV-2 en la población. Se realizó una encuesta domiciliaria entre octubre y diciembre de 2020, en dos ciudades históricas de la región minera de Brasil. El anticuerpo anti-SARS-CoV-2 se detectó mediante la prueba rápida Wondfo®. La entrevista cara a cara consistió en la administración de un cuestionario que contenía datos de registro, variables sociodemográficas y económicas, hábitos de vida, estado general de salud, salud mental, hábitos de sueño, alimentación y nutrición. Resultados: Se evaluaron 1.762 habitantes, de los cuales 764 (43,4 %) estaban en Mariana y 998 (56,6 %) en Ouro Preto. Para ambas ciudades, el 51,9 % de los entrevistados eran del sexo femenino, con predominio del rango de edad de 35 a 59 años (47,2 %). La prevalencia de la infección por SARS-CoV-2 fue del 5,5 % en ambos municipios, del 6,2 % en Ouro Preto y del 4,7 % en Mariana (p > 0,05). Conclusión: El estudio fue efectivo para estimar la seroprevalencia de infección por el virus y sus hallazgos permitirán analizar más las condiciones de salud de la población relacionadas con el aislamiento social y el riesgo de infección por SARS-CoV-2.

RESUMO A partir de referenciais do feminismo negro, da perspectiva interseccional e dos estudos étnicoraciais no Brasil, problematizam-se o racismo e o sexismo na academia brasileira com base na caracterização e análise da presença/ausência de professoras negras em programas de pós-graduação em ciências da saúde de duas universidades federais fluminenses, UFRJ e UFF. Utilizando informações de sites de 31 Programas de Pós-Graduação (PPG), reconstruíram-se quantitativamente os perfis de gênero e étnico-raciais por universidade e área de avaliação. Identificaram-se 23 professoras negras que ocupam 26 vagas docentes nos PPG analisados. Com base em informações da Plataforma Lattes, também se abordou longitudinalmente a dimensão de estudo. Os resultados assinalam que a presença de professoras negras é de 2% na UFRJ e de 6% na UFF; que ela é maior em áreas relativas aos cuidados e ínfima em áreas de maior prestígio científico e socioeconômico, como medicina. Constata-se o racismo como principal sistema de poder, operando no contexto institucional e disciplinar. Neste último, associado ao sexismo que determina as hierarquias de gênero nas áreas de saúde. Observa-se, também, que as desigualdades de raça se sobrepõem às de gênero no contexto desta pesquisa, confirmando as teses que apontam o epistemicídio dos saberes negros.

Background: A population study is an important tool that can be used to understand the actual epidemiological scenario of the Covid-19 in different territories, identify its magnitude, understand its transmission dynamics, and its demographic, geographical, and social distribution.
Objective: The aim of this study was to determine the prevalence and dynamics of SARS-CoV-2 infection in the population of two Brazilian cities during the pandemic first wave and subsequent socioeconomic and health effects.

Conclusion: The study was effective in verifying the seroprevalence of infection by the virus and its findings will enable further analyses of the health conditions of the population related to social isolation and the risk of infection with SARS-CoV-2.

RESUMO Objetivo: Descrever as práticas de ressuscitação volêmica em unidades de terapia intensiva brasileiras e compará-las com as de outros países participantes do estudo Fluid-TRIPS. Métodos: Este foi um estudo observacional transversal, prospectivo e internacional, de uma amostra de conveniência de unidades de terapia intensiva de 27 países (inclusive o Brasil), com utilização da base de dados Fluid-TRIPS compilada em 2014. Descrevemos os padrões de ressuscitação volêmica utilizados no Brasil em comparação com os de outros países e identificamos os fatores associados com a escolha dos fluidos. Resultados: No dia do estudo, foram incluídos 3.214 pacientes do Brasil e 3.493 pacientes de outros países, dos quais, respectivamente, 16,1% e 26,8% (p < 0,001) receberam fluidos. A principal indicação para ressuscitação volêmica foi comprometimento da perfusão e/ou baixo débito cardíaco (Brasil 71,7% versus outros países 56,4%; p < 0,001). No Brasil, a percentagem de pacientes que receberam soluções cristaloides foi mais elevada (97,7% versus 76,8%; p < 0,001), e solução de cloreto de sódio a 0,9% foi o cristaloide mais comumente utilizado (62,5% versus 27,1%; p < 0,001). A análise multivariada sugeriu que os níveis de albumina se associaram com o uso tanto de cristaloides quanto de coloides, enquanto o tipo de prescritor dos fluidos se associou apenas com o uso de cristaloides. Conclusão: Nossos resultados sugerem que cristaloides são usados mais frequentemente do que coloides para ressuscitação no Brasil, e essa discrepância, em termos de frequências, é mais elevada do que em outros países. A solução de cloreto de sódio 0,9% foi o cristaloide mais frequentemente prescrito. Os níveis de albumina sérica e o tipo de prescritor de fluidos foram os fatores associados com a escolha de cristaloides ou coloides para a prescrição de fluidos.
Abstract Objective: To describe fluid resuscitation practices in Brazilian intensive care units and to compare them with those of other countries participating in the Fluid-TRIPS. Methods: This was a prospective, international, cross-sectional, observational study in a convenience sample of intensive care units in 27 countries (including Brazil) using the Fluid-TRIPS database compiled in 2014. We described the patterns of fluid resuscitation use in Brazil compared with those in other countries and identified the factors associated with fluid choice. Results: On the study day, 3,214 patients in Brazil and 3,493 patients in other countries were included, of whom 16.1% and 26.8% (p < 0.001) received fluids, respectively. The main indication for fluid resuscitation was impaired perfusion and/or low cardiac output (Brazil: 71.7% versus other countries: 56.4%, p < 0.001). In Brazil, the percentage of patients receiving crystalloid solutions was higher (97.7% versus 76.8%, p < 0.001), and 0.9% sodium chloride was the most commonly used crystalloid (62.5% versus 27.1%, p < 0.001). The multivariable analysis suggested that the albumin levels were associated with the use of both crystalloids and colloids, whereas the type of fluid prescriber was associated with crystalloid use only. Conclusion: Our results suggest that crystalloids are more frequently used than colloids for fluid resuscitation in Brazil, and this discrepancy in frequencies is higher than that in other countries. Sodium chloride (0.9%) was the crystalloid most commonly prescribed. Serum albumin levels and the type of fluid prescriber were the factors associated with the choice of crystalloids or colloids for fluid resuscitation.

OBJECTIVES: To compare demographic/clinical/laboratory/treatments and outcomes among children and adolescents with laboratory-confirmed coronavirus disease 2019 (COVID-19). METHODS: This was a cross-sectional study that included patients diagnosed with pediatric COVID-19 (aged <18 years) between April 11, 2020 and April 22, 2021. During this period, 102/5,951 (1.7%) of all admissions occurred in neonates, children, and adolescents. Furthermore, 3,962 severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) detection samples were processed in patients aged <18 years, and laboratory-confirmed COVID-19 occurred in 155 (4%) inpatients and outpatients. Six/155 pediatric patients were excluded from the study. Therefore, the final group included 149 children and adolescents (n=97 inpatients and 52 outpatients) with positive SARS-CoV-2 results. RESULTS: The frequencies of sore throat, anosmia, dysgeusia, headache, myalgia, nausea, lymphopenia, pre-existing chronic conditions, immunosuppressive conditions, and autoimmune diseases were significantly reduced in children and adolescents (p<0.05). Likewise, the frequencies of enoxaparin use (p=0.037), current immunosuppressant use (p=0.008), vasoactive agents (p=0.045), arterial hypotension (p<0.001), and shock (p=0.024) were significantly lower in children than in adolescents. Logistic regression analysis showed that adolescents with laboratory-confirmed COVID-19 had increased odds ratios (ORs) for sore throat (OR 13.054; 95% confidence interval [CI] 2.750-61.977; p=0.001), nausea (OR 8.875; 95% CI 1.660-47.446; p=0.011), and lymphopenia (OR 3.575; 95% CI 1.355-9.430; p=0.010), but also had less hospitalizations (OR 0.355; 95% CI 0.138-0.916; p=0.032). The additional logistic regression analysis on patients with preexisting chronic conditions (n=108) showed that death as an outcome was significantly associated with pediatric severe acute respiratory syndrome (SARS) (OR 22.300; 95% CI 2.341-212.421; p=0.007) and multisystem inflammatory syndrome in children (MIS-C) (OR 11.261; 95% CI 1.189-106. 581; p=0.035). CONCLUSIONS: Half of the laboratory-confirmed COVID-19 cases occurred in adolescents. Individuals belonging to this age group had an acute systemic involvement of SARS-CoV-2 infection. Pediatric SARS and MIS-C were the most important factors associated with the mortality rate in pediatric chronic conditions with COVID-19.

RESUMEN El girasol (Helianthus annuus L.) es responsable del 13% de todo el aceite vegetal producido en el mundo. El desarrollo de estas plantas depende de los elementos minerales que tienen funciones esenciales y específicas en su metabolismo. En este sentido, el diagnóstico visual consiste en comparar la apariencia de una planta que recibió todos los nutrientes necesarios con una que sufrió la omisión de uno o más nutrientes. Por lo tanto, este estudio tuvo como objetivo evaluar los efectos de la ausencia de nitrógeno (N), fósforo (P), potasio (K), calcio (Ca), magnesio (Mg), azufre (S) y hierro (Fe) en el crecimiento de girasol, cultivar BRS-122, a fin de identificar y describir los síntomas visuales de la ausencia de tales nutrientes. El experimento fue conducido en invernadero y consistió en un diseño completamente al azar, con tres repeticiones y ocho tratamientos, utilizando una técnica de diagnóstico de sustracción. Los síntomas de las deficiencias fueron observados y evaluados a través de parámetros biométricos (altura de la planta, diámetro del tallo, número de hojas y área foliar), así como por aspectos visuales. La ausencia de N, P, K, Ca y Fe en la solución nutritiva afectaron severamente a las plantas de girasol, impidiendo su crecimiento vegetativo y consecuentemente su desarrollo. La interferencia negativa de la omisión del Mg en el crecimiento del girasol fue más lenta que la observada en N, P, K y Ca, con resultados más significativos a partir de los 40 días después de la siembra (DAS). La ausencia de los nutrientes dio claras evidencias de los distintos efectos que la omisión de cada elemento puede causar en los aspectos visuales de las plantas de girasol.
ABSTRACT Sunflower (Helianthus annuus L.) is responsible for 13% of all vegetable oil produced in the world. These plants’ development depends on the mineral elements that have essential and specific functions in their metabolism. In this sense, visual diagnosis consists of comparing the appearance of a plant that has received all the necessary nutrients with one that has suffered the omission of one or more nutrients. Therefore, this study aimed to evaluate the absence effects of nitrogen (N), phosphorus (P), potassium (K), calcium (Ca), magnesium (Mg), sulfur (S) and iron (Fe) elements on the growth of sunflower, BRS-122 cultivar, in order to identify and describe the visual symptoms caused by the absence of such nutrients. The experiment was carried out in a greenhouse and consisted in a completely randomized design with three replications and eight treatments using a diagnostic subtraction technique. The symptoms of the deficiencies were observed and evaluated through biometric parameters (plant height, stem diameter, number of leaves, and leaf area) as well as by visual aspects. The absence of N, P, K, Ca and Fe in the nutrient solution severely affected the sunflower plants, preventing their vegetative growth and consequently their development. The negative interference of the Mg omission in sunflower growth was slower than the observed for nitrogen, phosphorus, potassium, and calcium giving more significant results from 40 days after sowing (DAS). The absence of nutrients gave clear evidence of the distinct effects that the omission of each element can cause on the visual aspects of sunflower plants.

Abstract Background: Primary Sjögren's syndrome (pSS) is a systemic immune-mediated disease whose main characteristic is exocrine gland inflammation and, subsequent reduction in tear and saliva production. A delayed diagnosis is common due to the nonspecific clinical manifestations of disease. The aim of the present study was to develop recommendations for the diagnosis of glandular manifestations of pSS based on evidence and expert opinion. Main body of the abstract: We conducted a systematic literature review to retrieve the best evidence available on the accuracy of diagnostic tests for pSS. We also held two in-person meetings with experts (rheumatologists, pathologists, ophthalmologists and dentists) to establish their level of agreement using the Delphi method. Ultimately, we generated 18 recommendations that aim to facilitate the diagnosis of the glandular manifestations of pSS. Conclusion: The diagnosis of glandular manifestations of pSS is complex and multidisciplinary. It requires specific knowledge in the field of ophthalmology, immunology, pathology and imaging, making it compulsory for the rheumatologist to work with professionals from these different areas in order to improve accuracy and early diagnosis. Glandular dysfunction tests, ANA, RF, Anti-Ro, protein electrophoresis, urinalysis, blood count, C-Reactive protein, complement, testing for syphilis and viruses (HCV, HIV) and SGUS should be investigated when dryness or systemic manifestation are present. Minor salivary gland biopsy is recommended for all anti-Ro negative or incomplete criteria cases.

ABSTRACT Objective: To identify incidents defined as notifiable circumstances that occurred in a teaching hospital in Goiás. Method: Descriptive cross-sectional study that uses retrospective analysis of reports made by the nursing team of a general medicine unit made in the January 2010-December 2015 period. Data was collected and recorded in a semi-structured instrument and descriptive analysis was performed. Results: There were 2,718 notifiable circumstances, of which 1,100 (40.5%) were related to Resources/ Organizational Management, 844 (31.1%) to Medical Devices/Equipment, 270 (9.9%) to Documentation, 262 (9.6%) to Infrastructure/Facilities, 109 (4.0%) to Behavior, 80 (3.0%) to Medications/Intravenous Fluids, 28 (1.0%) to Blood/Blood derivatives, 17 (0.6%) to Diet/Food. Conclusion: The present study showed that situations with the potential to cause harm to hospitalized patients were very common, exposing organizational and care weaknesses that must be discussed by the management, so that improvements are implemented in the health care process and incidents are prevented.
RESUMEN Objetivo: identificar los incidentes del tipo circunstancia notificable que ocurrieron en un hospital de enseñanza de Goiás. Método: estudio descriptivo, trasversal, de análisis retrospectivo de informes del equipo de enfermería de la unidad de clínica médica, hecho entre enero de 2010 y diciembre de 2015. Se obtuvieron los datos y estos se registraron por medio de instrumento semi estructurado, procediéndose al análisis descriptivo. Resultados: se apuntaron 2.718 circunstancias notificables, de las cuales 1.100 (40,5%) se relacionaban a Recursos/Gestión Organizacional, 844 (31,1%) a Dispositivos/Equipos Médicos, 270 (9,9%) a Documentación, 262 (9,6%) a Infraestructura/Instalaciones, 109 (4,0%) a Comportamiento, 80 (3,0%) a Medicaciones/Fluidos Endovenosos, 28 (1,0%) a Sangre/Hemoderivados, 17 (0,6%) a Dieta/Alimentación. Conclusión: el estudio posibilitó identificar alta frecuencia de situaciones con potencial para generar daños a los pacientes ingresados, lo que muestra fragilidades organizacionales y asistenciales que se necesitan discutir por la gestión, de manera que se promuevan mejorías en el proceso de cuidado y prevención de la ocurrencia de incidentes.

Método: estudo descritivo, transversal, de análise retrospectiva de relatórios da equipe de enfermagem da unidade de clínica médica, entre janeiro de 2010 e dezembro de 2015. Os dados foram coletados e registrados em instrumento semiestruturado e procedeu-se à análise descritiva. Resultados: foram encontradas 2.718 circunstâncias notificáveis, das quais 1.100 (40,5%) estavam relacionadas a Recursos/Gestão Organizacional, 844 (31,1%) a Dispositivos/Equipamentos Médicos, 270 (9,9%) a Documentação, 262 (9,6%) a Infraestrutura/Instalações, 109 (4,0%) a Comportamento, 80 (3,0%) a Medicações/Fluidos Endovenosos, 28 (1,0%) a Sangue/Hemoderivados, 17 (0,6%) a Dieta/Alimentação. Conclusão: o estudo permitiu identificar alta frequência de situações com potencial para gerar danos aos pacientes internados, demonstrando fragilidades organizacionais e assistenciais que precisam ser discutidas pela gestão, de modo a promover melhorias no processo de cuidado e prevenção da ocorrência de incidentes.

ABSTRACT This was a cross-sectional descriptive observational study aimed at analyzing adverse events in a medical clinic unit of a hospital in Goiás. Data were collected from the reading of 12 nursing registry books completed from January 2005 to December 2014. The analysis found 1,360 adverse events, of which 408 (30%) referred to accidental removal of probes, catheters, and cannulas; 288 (21.2%) to medication errors; 157 (11.5%) to errors involving vascular access; 134 (9.8%) to cases of healthcare-associated infections; 115 (8.4%) to allergic processes; 91 (6.7%) to patient falls; 72 (5.3%) to pressure ulcers; 56 (4.1%) to episodes of lack of blood derivatives; and 39 (2.9%) to evasions. The study shows an increase in the registry of adverse events over the decade, pointing out the need to adopt preventive measures and raising nursing personnel’s awareness about the importance of reporting these incidents.
RESUMEN Estudio observacional de tipo descriptivo transversal objetivando analizar los eventos adversos ocurridos en unidad de clínica médica de un hospital de Goiás. Datos recolectados a partir de lectura de 12 libros de registros de enfermería completados entre enero de 2005 y diciembre de 2014. Fueron hallados 1360 eventos adversos, de los cuales 408 (30%) hacían referencia a retiros accidentales de sondas, catéteres y cánulas; 288 (21,2%) errores de medicación; 157 (11,5%) errores involucrando acceso vascular; 134 (9,8%) casos de infección relacionada a atención de salud; 115 (8,4%) procesos alérgicos; 91 (6,7%) caídas de pacientes; 72 (5,3%) úlceras por presión; 56 (4,1%) episodios de falta de hemoderivados y 39 (2,9%) evasiones. El estudio expresa el aumento de registros de eventos adversos a lo largo de la década, indicando necesidad de adopción de medidas preventivas y mayor concientización de los enfermeros respecto de la importancia de informar estos incidentes.

O estudo aponta o aumento dos registros dos eventos adversos ao longo da década, indicando a necessidade de adoção de medidas de prevenção e maior conscientização da enfermagem quanto à importância do relato desse incidente.

Antifungal activity of lectins against yeast of vaginal secretion

Lectins are carbohydrate-binding proteins of non-imune origin. This group of proteins is distributed widely in nature and they have been found in viruses, microorganisms, plants and animals. Lectins of plants have been isolated and characterized according to their chemical, physical-chemical, structural and biological properties. Among their biological activities, we can stress its fungicidal action. It has been previously described the effect of the lectins Dviol, DRL, ConBr and LSL obtained from the seeds of leguminous plants on the growth of yeasts isolated from vaginal secretions. In the present work the experiments were carried out in microtiter plates and the results interpreted by both methods: visual observations and a microplate reader at 530nm. The lectin concentrations varied from 0.5 to 256µg/mL, and the inoculum was established between 65-70% of trammitance. All yeast samples isolated from vaginal secretion were evaluated taxonomically, where were observed macroscopic and microscopic characteristics to each species. The LSL lectin did not demonstrate any antifungal activity to any isolate studied. The other lectins DRL, ConBr and DvioL, showed antifungal potential against yeast isolated from vaginal secretion. These findings offering offer a promising field of investigation to develop new therapeutic strategies against vaginal yeast infections, collaborating to improve women's health.
