Abstract Objective: To analyze the morbidity trend and space-time distribution clusters of confirmed COVID-19 cases in children and adolescents. Method: An ecological study of COVID-19 cases confirmed in the Information System from 2020 to 2022 in the age group from 0 to 19 years old, residents in Mato Grosso municipalities, Brazilian Midwest region. A trend analysis of the monthly morbidity rate of cases/100,000 inhabitants was used, following Prais-Winsten’s regression. A space-time distribution of the Bayesian incidence rate per 100,000 inhabitants was performed, in addition to a space-time scan to identify high-risk clusters. Results: Of all 79,592 COVID-19 cases studied, 51.6% were in females and 44.21% in people aged 15-19 years old. The mean monthly rate was 265.87 cases per 100,000 inhabitants, with a stationary trend in the period analyzed (Monthly Percentage Variation [MPV]) = 12.15; CI95%[MPV]: -0.73;26.70). The morbidity rate due to COVID-19 was higher in the female gender (283.14/ 100,000 inhabitants) and in the age group from 15 to 19 years old (485.90/100,000 inhabitants). An increasing trend was observed with a greater monthly time variation of 14.42% (CI95%[MPV]: 1.28;29.28)] among those aged from 10 to 14 years old. The primary cluster, which was also the one with the highest Relative Risk (RR = 5,16, p-value = 0.001), included 19 municipalities located in the North health macro-region. Conclusion: The findings indicated a monthly stationary trend in the study population, an increase in the age group from 10 to 14 years old, and areas at a higher risk for the disease in the North health macro-region of the state. Abstract This study aimed to characterize the spatial distribution and composition of living Benthic Foraminifera (BF) and to comprehend how environmental conditions (e.g., organic matter) can affect communities of these protozoa in the northern and southern sectors of the Santos Basin (SB), in the continental slope and São Paulo Plateau. In this context, 23 stations (65 samples including replicates at each station) were collected between 400 and 2,400 m water depth. Multivariate analyses revealed that the ecological structure of the community changes mainly along the bathymetric gradients. Stations located between 400 and 700 m, both in northern and southern sectors, are characterized by the presence of indicator species of high intensity of currents, such as Globocassidulina subglobosa and Trifarina bradyi. These stations are also mainly marked by the occurrence of Epistominella exigua, a phytodetritivore species. The stations at 1,000 and 1,300 m depth, in both sectors, are characterized by high accumulation of organic matter in the sediments, which favors the development of agglutinated foraminifera species, such as those of the genus Reophax. Finally, the lower slope and the São Paulo Plateau, in both sectors, are oligotrophic regions, with pulses of labile organic carbon, probably low current velocities and the presence of Alabaminella weddellensis. The quantity and quality of food, which are closely related to hydro-sedimentary dynamics and bentho-pelagic coupling in the slope and São Paulo Plateau, are the main factors that influence the distribution of living BF assemblages in the SB. ABSTRACT Objective: To describe the IMPACTO-MR, a Brazilian nationwide intensive care unit platform study focused on the impact of health care-associated infections due to multidrug-resistant bacteria. Methods: We described the IMPACTO-MR platform, its development, criteria for intensive care unit selection, characterization of core data collection, objectives, and future research projects to be held within the platform. Results: The core data were collected using the Epimed Monitor System® and consisted of demographic data, comorbidity data, functional status, clinical scores, admission diagnosis and secondary diagnoses, laboratory, clinical, and microbiological data, and organ support during intensive care unit stay, among others. From October 2019 to December 2020, 33,983 patients from 51 intensive care units were included in the core database. Conclusion: The IMPACTO-MR platform is a nationwide Brazilian intensive care unit clinical database focused on researching the impact of health care-associated infections due to multidrug-resistant bacteria. This platform provides data for individual intensive care unit development and research and multicenter observational and prospective trials. RESUMO Objetivo: Descrever o IMPACTO-MR, um estudo brasileiro de plataforma nacional em unidades de terapia intensiva focado no impacto das infecções por bactérias multirresistentes relacionadas à assistência à saúde. Métodos: Descrevemos a plataforma IMPACTO-MR, seu desenvolvimento, critérios para seleção das unidades de terapia intensiva, caracterização da coleta de dados, objetivos e projetos de pesquisa futuros a serem realizados na plataforma. Resultados: Os dados principais foram coletados por meio do Epimed Monitor System® e consistiram em dados demográficos, dados de comorbidades, estado funcional, escores clínicos, diagnóstico de internação e diagnósticos secundários, dados laboratoriais, clínicos e microbiológicos e suporte de órgãos durante a internação na unidade de terapia intensiva, entre outros. De outubro de 2019 a dezembro de 2020, 33.983 pacientes de 51 unidades de terapia intensiva foram incluídos no banco de dados principal. Conclusão: A plataforma IMPACTO-MR é um banco de dados clínico brasileiro de unidades de terapia intensiva focado na pesquisa do impacto das infecções por bactérias multirresistentes relacionadas à assistência à saúde. The role of probiotics in the immune response and intestinal microbiota of children with celiac disease: a systematic review

RESUMO Objetivo: Avaliar mudanças na resposta imunológica (diminuição de citocinas pró-inflamatórias no sangue) e na microbiota fecal (principalmente Bacteroidetes e Firmicutes) após a administração de probióticos em crianças com doença celíaca que seguiam uma dieta livre de glúten. Fontes de dados: Foram utilizadas para esta revisão as bases de dados Medical Literature Analysis and Retrieval System Online (MEDLINE), Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS), Springer e Scientific Electronic Library Online (SciELO), com os descritores “celiac disease AND probiotics”. No fim da busca, foram recuperados 168 artigos, dos quais quatro foram incluídos na síntese qualitativa final. Os critérios de inclusão foram ensaios clínicos randomizados e população pediátrica (1–19 anos) e os critérios de exclusão, intervenções que não fossem com probióticos, estudos com pacientes portadores de outras doenças associadas à doença celíaca, ou que não preenchessem critérios para o diagnóstico. Todos os estudos eleitos foram publicados até setembro de 2020, sem restrição de idioma, com pacientes recebendo cepas de Bifidobacterium breve ou B. longum e em uma dieta livre de glúten. Síntese dos dados: Os estudos demonstram que a administração de probióticos associada a uma dieta livre de glúten pode aproximar a microbiota fecal desses pacientes às condições típicas de um indivíduo saudável ao restaurar a abundância de algumas comunidades microbianas que caracterizam uma condição tipicamente fisiológica. Além disso, a administração de probióticos pode reduzir as citocinas pró-inflamatórias (principalmente TNF-alfa). Conclusões: Apesar da correlação positiva entre probióticos e microbiota fecal/marcadores sorológicos nos pacientes pediátricos portadores de doença celíaca, ressalta-se a necessidade de estudos multicêntricos futuros que abranjam um maior número de pacientes e maior tempo de acompanhamento.%
ABSTRACT Objective: To evaluate changes in peripheral immunological response (decrease in blood proinflammatory cytokines) and fecal microbiota (especially Bacteroidetes and Firmicutes) after administration of probiotics in children with celiac disease on a gluten-free diet. Data source: The databases MEDLINE, LILACS, Springer and SciELO were used for this review, with the descriptors “celiac disease AND probiotics”. At the end of the search, 168 articles were retrieved, four of which were included in the final qualitative synthesis, having as inclusion criteria randomized clinical trials and pediatric population (1–19 years) and, as exclusion criteria, interventions other than probiotics, studies with patients with other diseases associated with celiac disease, or patients who did not meet the diagnostic criteria. All elected studies were published until September 2020, without language restriction, with patients receiving strains of Bifidobacterium breve or B. longum and on a gluten-free diet. Data synthesis: The studies show that the administration of probiotics along with a gluten-free diet, can approximate the fecal microbiota of celiac patients to typical conditions of healthy individuals, by restoring the abundance of some microbial communities that characterize the typical physiological condition. In addition, the administration of probiotics can reduce serum proinflammatory cytokines (mainly TNF-alpha). Conclusions: Despite the positive correlation between probiotics and fecal microbiota/serological markers in pediatric patients with celiac disease, we emphasize the need for future multicentric studies that should include a larger number of patients and a longer follow up period.%

Abstract Background: There is a lack of information on the role of chronic use of hydroxychloroquine during the SARS-CoV-2 outbreak. Our aim was to compare the occurrence of COVID-19 between rheumatic disease patients on hydroxychloroquine with individuals from the same household not taking the drug during the first 8 weeks of community viral transmission in Brazil. Methods: This baseline cross-sectional analysis is part of a 24-week observational multi-center study involving 22 Brazilian academic outpatient centers. All information regarding COVID-19 symptoms, epidemiological, clinical, and demographic data were recorded on a specific web-based platform using telephone calls from physicians and medical students. COVID-19 was defined according to the Brazilian Ministry of Health (BMH) criteria. Mann-Whitney, Chi-square and Exact Fisher tests were used for statistical analysis and two binary Final Logistic Regression Model by Wald test were developed using a backward-stepwise method for the presence of COVID-19. Results: From March 29th to May 17st, 2020, a total of 10,443 participants were enrolled, including 5166 (53.9%) rheumatic disease patients, of whom 82.5% had systemic erythematosus lupus, 7.8% rheumatoid arthritis, 3.7% Sjögren’s syndrome and 0.8% systemic sclerosis. In total, 1822 (19.1%) participants reported flu symptoms within the 30 days prior to enrollment, of which 3.1% fulfilled the BMH criteria, but with no significant difference between rheumatic disease patients (4.03%) and controls (3.25%). After adjustments for multiple confounders, the main risk factor significantly associated with a COVID-19 diagnosis was lung disease (OR 1.63; 95% CI 1.03-2.58); and for rheumatic disease patients were diagnosis of systemic sclerosis (OR 2.8; 95% CI 1.19-6.63) and glucocorticoids above 10 mg/ day (OR 2.05; 95% CI 1.31-3.19). In addition, a recent influenza vaccination had a protective effect (OR 0.674; 95% CI 0.46-0.98). Conclusion: Patients with rheumatic disease on hydroxychloroquine presented a similar occurrence of COVID-19 to household cohabitants, suggesting a lack of any protective role against SARS-CoV-2 infection.
