Abstract Aiming to test the capacity of retention of carotenoids in tissues, Lophiosilurus alexandri juveniles were fed diets containing 0, 25, 50, 100, 200, and 400 mg/kg of synthetic astaxanthin for 62 days. The inclusion of astaxanthin did not result in significant differences in growth, weight gain, apparent feed conversion, and feed efficiency of the fish. Blood biochemistry and liver histology did not change with the different treatments. At the level of 100 mg/kg of inclusion, there were the highest levels of carotenoids in the blood, and muscle and the smallest difference between the muscle x integument ratio.

RESUMO Adequada nutrição potássica e cálcica é uma estratégia para reduzir o estresse salino sobre o tomateiro, pois reduz o desbalanço nutricional nas plantas. Com o objetivo de avaliar a produção do tomate utilizando irrigação com águas salinas, e fertigação com diferentes proporções potássio-cálcio, realizou-se um experimento em ambiente protegido em Mossoró, RN. O delineamento experimental utilizado foi em blocos casualizados, em esquema fatorial 5 x 4, com quatro repetições. Os tratamentos consistiram em combinações de quatro condutividade elétricas da solução nutritiva (ECns) (1,75; 3,25; 4,75 e 6,25 dS m-1) com cinco proporções iônicas (m/m) de potássio e cálcio (F1 = 2.43:1; F2 = 2.03:1; F3 = 1.62:1; F4 = 1.30:1 and F5 = 1.08:1). As variáveis resposta foram: número de frutos, peso médio de fruto, produção de frutos por planta e produção relativa. Foi possível identificar resultados de produção satisfatórios quando se usa uma maior salinidade. O uso de fertigação com baixas proporções K+/Ca2+ potencializa o efeito da salinidade sobre a cultura do tomateiro.
ABSTRACT Adequate potassium and calcium nutrition is a strategy to reduce salt stress on tomatoes, as it reduces nutritional imbalance in plants. With the objective of evaluating tomato production using irrigation with saline waters and fertigation with different potassium-calcium proportions, an experiment was carried out in a protected environment in Mossoró, RN, Brazil. The experimental design used was randomized blocks, in a 5 x 4 factorial scheme, with four replicates. The treatments consisted of the combination of four electrical conductivity of nutrient solution (ECns) (1.75; 3.25; 4.75; and 6.25 dS m-1) combined with five ionic proportions (m/m) of potassium and calcium (F1 = 2.43:1; F2 = 2.03:1; F3 = 1.62:1; F4 = 1.30:1 and F5 = 1.08:1). The response variables were: number of fruits, mean fruit weight, fruit production per plant and relative yield. It was possible to identify satisfactory results of production when higher salinity was used. Fertigation with low K+/Ca2+ proportions intensifies the effect of salinity on tomato crop.

OBJECTIVES: Strategic planning for coronavirus disease (COVID-19) care has dominated the agenda of medical services, which have been further restricted by the need for minimizing viral transmission. Risk is particularly relevant in relation to endoscopy procedures. This study aimed to describe a contingency plan for a tertiary academic cancer center, define a strategy to prioritize and postpone examinations, and evaluate the infection rate among healthcare workers (HCWs) in the endoscopy unit of the Cancer Institute of the State of São Paulo (ICESP). METHODS: We created a strategy to balance the risk of acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection and to mitigate the effects of postponing endoscopic procedures in oncological patients. A retrospective analysis of prospectively collected data on all endoscopies between March and June 2020 compared with those during the same period in 2019 was carried out. All HCWs were interviewed to obtain clinical data and SARS-CoV-2 test results. RESULTS: During the COVID-19 outbreak, there was a reduction of 55% in endoscopy cases in total. Colonoscopy was the most affected modality. The total infection rate among all HCWs was 38%. None of the senior digestive endoscopists had COVID-19. However, all bronchoscopists had been infected. One of three fellows had a serological diagnosis of COVID-19. Two-thirds of all nurses were infected, whereas half of all technicians were infected. CONCLUSIONS: In this pandemic scenario, all endoscopy services must prioritize the procedures that will be performed. It was possible to maintain some endoscopic procedures, including those meant to provide nutritional access, tissue diagnosis, and endoscopic resection. Personal protective equipment (PPE) seems effective in preventing transmission of COVID-19 from patients to digestive endoscopists. These measures can be useful in planning, even for pandemics in the future.

ABSTRACT Background: Knowledge about COVID-19 in pregnancy is limited, and evidence on the impact of the infection during pregnancy and postpartum is still emerging. Aim: To analyze maternal morbidity and mortality due to severe acute respiratory infections (SARI), including COVID-19, in Brazil. Methods: National surveillance data from the SIVEP-Gripe (Sistema de Informação de Vigilância Epidemiológica da Gripe) was used to describe currently and recently pregnant women aged 10-49 years hospitalized for SARI from January through November, 2020. SARI cases were grouped into: COVID-19; influenza or other detected agent SARI; and SARI of unknown etiology. Characteristics, symptoms and outcomes were presented by SARI type and region. Binomial proportion and 95% confidence intervals (95% CI) for outcomes were obtained using the Clopper-Pearson method. Results: Of 945,460 SARI cases in the SIVEP-Gripe, we selected 11,074 women aged 10-49 who were pregnant (7964) or recently pregnant (3110). COVID-19 was confirmed in 49.4% cases; 1.7% had influenza or another etiological agent; and 48.9% had SARI of unknown etiology. The pardo race/ethnic group accounted for 50% of SARI cases. Hypertension/Other cardiovascular diseases, chronic respiratory diseases, diabetes, and obesity were the most common comorbidities. A total of 362 women with COVID-19 (6.6%; 95%CI 6.0-7.3) died. Mortality was 4.7% (2.2-8.8) among influenza patients, and 3.3% (2.9-3.8) among those with SARI of unknown etiology. The South-East, Northeast and North regions recorded the highest frequencies of mortality among COVID-19 patients. Conclusion: Mortality among pregnant and recently pregnant women with SARIs was elevated among those with COVID-19, particularly in regions where maternal mortality is already high.

ABSTRACT Croton zehntneri Pax & K. Hoffm., Euphorbiaceae, or “canela-de-cunhé” is used in the Northeast Brazil to treat several diseases. Leaves and aerial parts of C. zehntneri are rich in volatile oil of high potential therapeutic. This study aimed to investigate volatile oil systemic toxicity after per oral treatment in rats. Volatile oil characterization (gas chromatography and mass spectrometry) showed 85.7% anethole and 4.8% estragole. Male Wistar rats (116–149 g) were treated with volatile oil (250 mg/kg p.o.) during ten weeks and evaluated for the following parameters: survival; food and water intake; body mass; absolute/relative organs weight; hemogram; plasma biochemical dosage; organs morphology. Volatile oil did not alter animal water and food consumption or the relative/absolute weight of most organs, but animals gained less weight. Volatile oil did not alter function biomarkers of pancreas, kidney, heart or liver, but increased plasma gamma-glutamyltranspeptidase (liver biomarker) and decreased uric acid (kidney biomarker). Although volatile oil had caused discrete morphological alterations in some organs, it did not induce architectural changes in these organs. In conclusion, the sub-acute per oral treatment with volatile oil no longer than ten weeks in rats offers small toxicity at doses below 250 mg/kg.

RESUMO O manejo da fertirrigação pode ser realizado com doses fixas de fertilizantes ou por meio da avaliação da concentração iônica na solução do solo ao longo do período de cultivo. Objetivou-se avaliar diferentes formas de manejo de fertirrigação na cultura do pimentão, com ênfase para a produção e qualidade de frutos. O experimento foi conduzido em casa de vegetação e as plantas foram cultivadas em vasos. Testaram-se três formas de manejo de fertirrigação (M1) realizado de acordo com a marcha de absorção da cultura, (M2) por meio do monitoramento da condutividade elétrica da solução do solo e (M3) por meio do monitoramento da concentração de N e K. Utilizaram-se seis doses de N e K (0, 50, 100, 150, 200 e 300% da dose recomendada para a cultura) em delineamento experimental de blocos casualizados, com esquema fatorial 3x6 e quatro repetições. Avaliou-se o acúmulo de matéria seca, produção e a classificação de frutos. As formas de manejo de fertirrigação com monitoramento da solução do solo (M2 e M3) proporcionam desenvolvimento vegetativo até 25% maior, e acréscimo de produtividade de até 20% quando comparados com o manejo que segue a marcha de absorção (M1). Os maiores rendimentos de frutos nos manejos M1, M2 e M3 foram obtidos para os níveis de N e K correspondentes a 127,6% (1,33 kg/planta), 222,5% (1,60 kg/planta) 215% (1,48 kg/planta), respectivamente. O fornecimento de N e K na cultura do pimentão pode ser realizado por intermédio do monitoramento da concentração ou da condutividade elétrica da solução do solo, pois essas formas propiciam melhor qualidade dos frutos e eficiências até 47% maiores no uso desses nutrientes. Sugere-se utilizar como referencial a concentração de N e K equivalente a 200% daquela recomendada para o cultivo hidropônico.
ABSTRACT Fertigation can be managed using fixed fertilizer doses or by assessing the ionic concentration of soil solution throughout the growing period. This work studied how different fertigation management systems affected sweet pepper yield and quality. The experiment was carried out in greenhouse, in pots. Fertigation was managed according to the crop uptake rate (M1) or by monitoring either the electrical conductivity (M2) or the N and K concentration (M3) in the soil solution. Fertigation management was combined with six N and K doses (0, 50, 100, 150, 200 and 300% of the recommended dose for sweet pepper in the region), in complete blocks at random, with four replications, and treatments in 3x6 factorial. Dry matter accumulation, yield and fruit grading were evaluated. Fertigation managed by means of monitoring the soil solution improved the vegetative growth in up to 25% and increased yield in up to 20% when compared to management according to the uptake rate. Highest fruit yields in M1, M2 and M3 were achieved with N and K levels corresponding to 127.6% (1.33 kg/plant), 222.5% (1.60 kg/plant) and 215% (1.48 kg/plant), respectively. N and K can be supplied successfully to sweet peppers using electrical conductivity or concentration of ions in the soil solution to manage fertigation. These management systems resulted in high quality fruits and up to 47% increase in N and K use efficiency. N and K concentration equivalent to 200% of the recommended for growing sweet pepper in hydroponics should be taken as reference.

RESUMO Swietenia macrophylla é uma espécie que fornece madeira de alta qualidade, no entanto, sua vulnerabilidade à extinção somada aos desafios para seu cultivo tem despertado a atenção para novas alternativas, como a Khaya anthotheca. A espécie foi recentemente introduzida na América do Sul, mostrando potencial para a produção de madeira de alta qualidade. No entanto, informações sobre o melhoramento são limitadas, assim como estratégias para a propagação vegetativa. No presente trabalho foram obtidos resultados significativos da aplicação da enxertia por garfagem e da mergulhia aérea na propagação de K. anthotheca. Primeiramente, foi analisada a compatibilidade entre enxerto e porta-enxerto em combinações intra e interespecíficas entre K. anthotheca e S. macrophylla. Adicionalmente, a mergulhia aérea foi realizada em plantas de K. anthotheca irrigadas com três soluções nutritivas (100%, 50% e 25% da concentração original) combinadas com a aplicação de ácido indolbutírico (AIB) para avaliação do enraizamento. Da enxertia, foi verificada compatibilidade de 48% após 200 dias. No entanto, a combinação interespecífica entre K. anthotheca (porta-enxerto) e S. macrophylla (enxerto) não apresentou compatibilidade, enquanto que a enxertia recíproca resultou em 52% de compatibilidade. Através da mergulhia aérea, a irrigação com pelo menos 50% da concentração de nutrientes e combinada com a aplicação de AIB (3,0 ou 8,0 g.L-1) resultou na melhor performance de enraizamento. No geral, recomenda-se a enxertia por garfagem, exceto para a combinação S. macrophylla (enxerto) com K. anthotheca (porta-enxerto). A mergulhia aérea pode ser útil para a propagação de genótipos selecionados de K. anthotheca em programas de melhoramento genético.
ABSTRACT Swietenia macrophylla yields high-quality wood; however, its vulnerability to extinction coupled with challenges for its cultivation have brought attention to its replacement for alternative species such as the Khaya anthotheca. The species has been recently introduced to South America, with potential for the production of high-quality wood. However, limited background on breeding and efficient strategies for its vegetative propagation exist. Here, we achieved significant results with the application of cleft grafting and air layering for the propagation of K. anthotheca plants grown from seeds. First, we analyzed the compatibility of scions and rootstocks for intraspecific and interspecific cleft grafting combinations of K. anthotheca and S. macrophylla. Second, air layering was performed in K. anthotheca seedlings irrigated with three nutrient solution (100%, 50% and 25% of the initial concentration of nutrients) combined with the application of indole-3-butyric acid (IBA) to evaluate adventitious rooting. From cleft grafting, we achieved an overall graft compatibility and survival of 48% after 200 days. However, the interspecific combination of S. macrophylla (scion) and K. anthotheca (rootstock) implicated in no compatibility, while the reciprocal resulted in 52% of compatibility. Through air layering, the irrigation with the nutrient solution with at least 50% of the nutrients concentration and with IBA (3.0 or 8.0 g.L-1) resulted in the best adventitious rooting. Overall, we recommend cleft grafting, except for the combination S. macrophylla (scion) and K. anthotheca (rootstock), with no compatibility. Air layering might also be useful for the propagation of K. anthotheca genotypes in breeding programs.

RESUMO O surgimento de novos híbridos de milho pode afetar a tolerância à deficiência nutricional. Objetivou-se, com este trabalho, caracterizar os sintomas visuais de deficiência de macronutrientes e sua relação com o crescimento, os teores e o acúmulo de nutrientes em milho. O experimento foi realizado em delineamento inteiramente casualizado, com sete tratamentos e três repetições. Os tratamentos consistiram na solução completa de Hoagland & Arnon (1950), com omissões de N, P, K, Ca, Mg e S. O início dos sintomas de deficiência de N, P, K, Ca e Mg ocorreram aos 5, 16, 16, 16 e 22 dias após o transplantio, respectivamente. O milho apresentou menor produção de matéria seca com a omissão de N, P, K, Ca e Mg, em comparação com o tratamento completo. Os sintomas de deficiência de N, P, K, Ca e Mg foram observados na parte aérea do milho com teores iguais a 7,8; 1,2; 15,7; 1,8 e 0,7 g kg-1, respectivamente. No tratamento completo, os teores de N, P, K, Ca e Mg na parte aérea foram de 30,6; 5,9; 63,7; 6,0 e 4,5 g kg-1, sem sintomas de deficiência. A produção de matéria seca diminui quando há a omissão dos nutrientes, tanto para parte aérea como para raízes, e os tratamentos que apresentaram menor produção de matéria seca foram a omissão de N e a de K. A omissão de um nutriente, além de diminuir seu teor na parte aérea, causa desequilíbrio entre os demais nutrientes e, consequentemente, provoca sintomas característicos de deficiência de cada nutriente.
ABSTRACT The emergence of new corn hybrids may affect tolerance to nutritional deficiency culture. This study aimed to characterize the visual symptoms of macronutrient deficiency and its relation to the development, content and nutrient accumulation in maize. The experiment was arranged in entirely randomized design, with seven treatments and three replications. The plants were cultivated in nutritive complete solution of Hoagland & Arnon (1950), and the omission of N, P, K, Ca, Mg and S. The onset of symptoms of deficiency of N, P, K, Ca and Mg occurred at 5, 16, 16, 16 and 22, days after the transplant, respectively. The corn had lower dry matter production in the omission of N, P, K, Ca and Mg in relation to the complete treatment. Symptoms of deficiency of N, P, K, Ca and Mg were observed in the shoots of corn with levels equal to 7.8; 1.2; 15.7; 1.8 to 0.7 g kg-1 , respectively. Complete treatment in the N, P, K, Ca and Mg in the shoots were 30.6; 5.9; 63.7; 6.0 and 4.5 g kg-1, in that order, good nutritional status. The dry matter production will decrease in the omission of nutrients in the aerial part and in the roots, and the treatments had lower dry matter production were the omission of N and K. The absence of a nutrient, and reduce its content in the shoot, cause imbalance between the other nutrients, thus providing characteristic symptoms of deficiency of each nutrient.

RESUMO Foi avaliado o padrão de crescimento entre fêmeas e machos de tambaqui por meio do modelo de regressão não linear de Gompertz. Foram mensuradas cinco características de importância econômica em 145 animais durante os três anos, totalizando 981 de dados morfométricos analisados. Curvas diferentes foram ajustadas entre machos e fêmeas para peso corporal, altura e comprimento da cabeça e apenas uma curva para a largura e comprimento do corpo foi ajustada para ambos os sexos. O peso assintótico (a) e taxa de crescimento relativo à maturidade (k) foram diferentes entre os sexos nos animais com ± 5 kg; peso de abate praticado por um nicho específico do mercado, muito rentável. No entanto, não houve diferença entre machos e fêmeas de até ± 2 kg; peso de abate estabelecido para abastecer o maior mercado consumidor. As fêmeas apresentaram maior peso do que os machos (± 280 g), sendo estas mais indicadas para fins de piscicultura definidos para o nicho de mercado de animais maiores. Em geral, os machos apresentaram menor taxa de crescimento (8,66 g / dia) do que as fêmeas (9,34 g / dia), entretanto, alcançaram a taxa máxima de crescimento mais rápido do que as fêmeas, 476 e 486 dias, respectivamente. A altura e comprimento do corpo são as características que mais contribuíram para o peso aos 516 dias (P <0,001).
ABSTRACT Was evaluated the pattern of growth among females and males of tambaqui by Gompertz nonlinear regression model. Five traits of economic importance were measured on 145 animals during the three years, totaling 981 morphometric data analyzed. Different curves were adjusted between males and females for body weight, height and head length and only one curve was adjusted to the width and body length. The asymptotic weight (a) and relative growth rate to maturity (k) were different between sexes in animals with ± 5 kg; slaughter weight practiced by a specific niche market, very profitable. However, there was no difference between males and females up to ± 2 kg; slaughter weight established to supply the bigger consumer market. Females showed weight greater than males (± 280 g), which are more suitable for fish farming purposes defined for the niche market to larger animals. In general, males had lower maximum growth rate (8.66 g / day) than females (9.34 g / day), however, reached faster than females, 476 and 486 days growth rate, respectively. Resumo Apresenta-se uma síntese atualizada de informações sobre algas no Brasil objetivando refinar os dados reunidos até o presente, bem como avaliar os avanços sobre o conhecimento da diversidade de algas no país desde a publicação do Catálogo de Plantas e Fungos do Brasil. Os resultados de 2015 mostraram um acréscimo de 1.250 espécies (35.7%) a um total de 4.747 em relação a 2010. As classes mais diversas em número de espécies foram Bacillariophyceae, Conjugatophyceae, Florideophyceae, Cyanophyceae, Dinophyceae e Euglenophyceae. Bacillariophyceae e Cyanophyceae tiveram o maior acréscimo de espécies no intervalo de cinco anos. A região Sudeste e Sul foram as mais diversas, porém, as regiões Nordeste com os estados do Piauí e Sergipe e Centro-Oeste com os estados de Mato Grosso, Goiás e Distrito Federal destacaram-se no cenário da biodiversidade nacional. Apesar da escassez de taxonomistas e da infraestrutura limitada, os resultados obtidos evidenciaram um avanço significativo no conhecimento da diversidade de algas no país nesse período de cinco anos, iniciando uma mudança quanto as diferenças geográficas regionais.
Abstract An updated synthesis of cyanobacteria and algae information is presented for Brazil aiming to refine the data gathered to date and evaluate the progress of the biodiversity knowledge about these organisms in the country since the publication of the Catálogo de Plantas e Fungos do Brasil. The results of 2015 showed an increase of 1,250 species (35.7%) when compared to 2010, reaching a total of 4,747 species. The most diverse classes in species number were the Bacillariophyceae, Conjugatophyceae, Florideophyceae, Cyanophyceae, Dinophyceae and Euglenophyceae. Bacillariophyceae and Cyanophyceae had the highest increase in species number in the five-year interval. The Southeast and South regions were the most diverse, however, the Northeast, with the states of Piauí and Sergipe, and the Central-west region, with Mato Grosso, Goiás and Distrito Federal, also stood out in the national algal biodiversity scenario. Despite the shortage of taxonomists and limited infrastructure, the results showed a significant improvement in the knowledge regarding the diversity of cyanobacteria and algae in the country during the study period, starting to even out regional geographical differences caused by subsampling.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Prevalence of dementia subtypes in a developing country: a clinicopathological study

OBJECTIVES: To assess the distribution of dementia subtypes in Brazil using a population-based clinicopathological study. METHOD: Brains from deceased individuals aged ≥50 years old were collected after the next of kin signed an informed consent form and provided information through standardized questionnaires. Post-mortem clinical diagnoses were established in consensus meetings, and only cases with moderate or severe dementia or without cognitive impairment were included in the analysis. Immunohistochemical neuropathological examinations were performed following the universally accepted guidelines. A diagnosis of Alzheimer's disease was made when there were at least both a moderate density of neuritic plaques (Consortium to Establish a Register for Alzheimer's disease B or C) and Braak stage III for neurofibrillary tangle distribution. For the diagnosis of vascular dementia, at least three zones or strategic areas had to be affected by infarcts, lacunae, or microinfarcts. RESULTS: From 1,291 subjects, 113 cases were classified as having moderate or severe dementia, and 972 cases were free of cognitive impairment. The neuropathological diagnoses of the dementia sub-group were Alzheimer's disease (35.4%), vascular dementia (21.2%), Alzheimer's disease plus vascular dementia (13.3%), and other causes of dementia (30.1%). Small-vessel disease, which alone was not considered sufficient for a vascular dementia diagnosis, was present in 38.9% of all of the dementia cases and in 16.8% of the group without cognitive impairment (odds ratio = 2.91; 95% confidence interval, 1.53-5.51), adjusted for age, sex, and education. CONCLUSIONS: The relatively high frequencies of vascular dementia and small-vessel disease in the dementia sub-group constitute relevant findings for public health initiatives because control of vascular risk factors could decrease the prevalence of dementia in developing countries.

INTRODUÇÃO: O aumento da expectativa de vida da população e o risco de sangramento com o uso de trombolíticos têm permitido que pacientes cada vez mais idosos com infarto agudo do miocárdio sejam tratados pela intervenção coronária percutânea (ICP) primária. Analisamos os resultados hospitalares da ICP primária em pacientes com idade > 80 anos. MÉTODO: No período de janeiro de 2002 a outubro de 2008, foram realizadas 4.788 ICPs, sendo 428 ICPs primárias. Destas, 34 foram em pacientes com idade > 80 anos e 394 em pacientes < 80 anos. RESULTADOS: O sexo feminino (47,1% vs. 27,7%; P = 0,017), a classe funcional Killip IV (11,8% vs. 4,1%; P = 0,002) e o fluxo coronário TIMI 2/3 pré-ICP (60% vs. 44,4%; P = 0,032) foram mais frequentes nos idosos. Tabagismo (44,9% vs. 8%; P < 0,001) e uso de inibidores da glicoproteína IIb/IIIa (32,5% vs. 11,7%; P = 0,04) prevaleceram no grupo < 80 anos. Não ocorreram diferenças significativas quanto ao tempo portabalão (117,4 ± 125,7 minutos vs. 179,3 ± 169,8 minutos; P = 0,095), à taxa de utilização de stents (91,2% vs. 98,2%; P = 0,78), e ao uso de stents farmacológicos (0 vs. 2,8%; P = 0,067) ou de dispositivos de aspiração de trombos (11,7% vs. 12,2; P = 0,65). Não houve diferença quanto à taxa de sucesso angiográfico (97,1% vs. 96,5%; P > 0,99), porém o grupo > 80 anos apresentou mortalidade hospitalar mais elevada (11,8% vs. 2,3%; P = 0,014). Reinfarto, acidente vascular cerebral, complicações vasculares maiores e insuficiência renal aguda foram semelhantes entre os grupos. CONCLUSÃO: A ICP apresenta, em geral, altas taxas de sucesso angiográfico e baixo índice de complicações, reforçando seu papel como método preferencial de reperfusão na fase aguda do infarto do miocárdio. Os pacientes muitos idosos, com mais de 80 anos, apresentam maior mortalidade, possivelmente reflexo da maior comorbidade desse grupo.
BACKGROUND: Improvements in life expectancy of the overall population and the risk of bleeding with thrombolytics have enabled very elderly patients with acute myocardial infarction to be treated by primary percutaneous coronary intervention (PCI). We analyzed the primary PCI in-hospital results in patients > 80 years of age. METHOD: From January 2002 to October 2008, 4,788 PCIs were performed, of which 428 were primary PCIs. Of these, 34 were performed in patients > 80 years of age and 394 in patients < 80 years of age. RESULTS: Females (47.1% vs. 27.7%; P = 0.017), Killip IV functional class (11.8% vs. 4.1%; P = 0.002) and pre-procedure TIMI 2/3 flow (60% vs. 44.4%; P = 0.032) were more frequent in elderly patients. Smoking (44.9% vs. 8%; P < 0.001) and use of glycoprotein IIb/IIIa inhibitors (32.5% vs. 11.7%; P = 0.04) were prevalent in the group < 80 years of age. There were no significant differences for door-to-balloon time (117.4 ± 125.7 minutes vs. 179.3 ± 169.8 minutes; P = 0.095), use of stents (91.2% vs. 98.2%; P = 0.78), or drug-eluting stents (0 vs. 2.8%; P = 0.067) and thrombus aspiration devices (11.7% vs. 12.2%; P = 0.65). Angiographic success rate was equal between the groups (97.1% vs. 96.5%; P > 0.99), but patients > 80 years old had a higher mortality rate (11.8% vs. 2.3%; P = 0.014). Reinfarction, stroke, major vascular complications and acute renal failure were similar between groups. CONCLUSION: PCI usually presents high angiographic success and low complication rates, reinforcing its role as a method of choice for reperfusion in the acute phase of myocardial infarction. Very elderly patients (> 80 years of age) have higher mortality rates, possibly due to the greater comorbidity in this group.

CONTEXTUALIZAÇÃO: A incontinência urinária (IU) é de causa multifatorial, sendo atribuída, em parte, à fraqueza da musculatura do assoalho pélvico. Apesar de ser subestimada por muitas mulheres, a avaliação funcional do assoalho pélvico (AFA) pode contribuir para um correto diagnóstico e terapêutica adequada. OBJETIVOS: Comparar a função muscular do assoalho pélvico em mulheres continentes e incontinentes na pós menopausa como fator diagnóstico no tratamento da IU. MÉTODOS: A partir da investigação dos sintomas urinários, 153 mulheres (idade X=66,7±5,4) foram separadas em dois grupos (G1 incontinentes e G2 assintomáticas). Após análise dos critérios de inclusão, as mulheres foram submetidas à AFA por meio da palpação bidigital (classificação de Contreras Ortis, 1994) e à quantificação da pressão de contração perineal por meio do perineômetro (PERINA 996-2® QUARK). RESULTADOS: Observou-se prevalência de IU (54,9%) na amostra estudada, sendo a incontinência urinária de esforço (IUE) (41,7%) o tipo mais presente. Em relação aos sintomas urinários, como a frequência miccional diurna (p=0,004) e noturna (p=0,02), o grupo G1 apresentou um valor significativamente mais alto. A AFA mostrou resultados similares durante a palpação e o perineômetro, com diferenças significativas (p<0,001) entre os dois grupos. Utilizou-se estatística descritiva, teste t de Student para amostras independentes, medidas de prevalência e análise de variância (one-way ANOVA), seguida do post hoc de Bonferroni (p<0,05). O software Statistical Package for the Social Sciences (SPSS) versão 10,0 (SPSS, Chicago, IL) foi utilizado para realização de todas as análises. CONCLUSÕES: A palpação e o perineômetro se mostraram eficientes na avaliação da força e pressão de contração desse grupo muscular.
BACKGROUND: Urinary incontinence (UI) is multifactorial and attributed, in part, to weakness of the pelvic floor muscles. Despite being underestimated by many women, a functional pelvic floor assessment (FPA) may contribute to a correct diagnosis and appropriate treatment. OBJECTIVES: To compare the function of pelvic floor muscles in continent and incontinent postmenopausal women as a diagnostic factor in UI treatment. METHODS: Based on the investigation of urinary symptoms, 153 women (age X=66.7±5.4) were divided into two groups (G1-incontinent and G2-continent). After analysis of the inclusion criteria, the women were submitted to FPA by means of bidigital palpation according to Contreras Ortiz (1994) and quantification of perineal strength with a perineometer (PERINA 996-2 QUARK®). RESULTS: There was prevalence of UI (54.9%) in the sample, with stress urinary incontinence (41.7%) as the most common. Regarding urinary symptoms such as diurnal (p=0.004) and nocturnal (p=0.02) voiding frequency, G1 had a significantly higher value. The FPA found similar results via palpation and the perineometer, with significant differences (p<0.001) between the two groups. We used descriptive statistics, the Student t test for independent samples, measures of prevalence and one-way ANOVA, followed by Bonferroni's post-hoc test (p<0.05). The software Statistical Package for the Social Sciences (SPSS) version 10.0 (SPSS, Chicago, IL) was used to perform all tests. CONCLUSIONS: Palpation and the perineometer were efficient forms of assessing the force and pressure of the muscle contractions of this muscle group.

Mercury accumulation in sediments along an eutrophication gradient in Guanabara Bay, southeast Brazil

A acumulação de Hg foi investigada em testemunhos sedimentares da Baía de Guanabara, Rio de Janeiro, Brasil, amostrados ao longo de um gradiente de eutrofização, que se refletiu na qualidade física dos sedimentos e na distribuição de carbono orgânico total (COT). Sedimentos da área oeste da baía foram fortemente afetados por aportes de efluentes domésticos e industriais, apresentando concentrações de Hg de até 7500 ng g-1 e inventários de Hg de até 786 mg m-2, em contraste com valores próximos aos de background encontrados na área nordeste (< 65 ng g-1 e 10,6 mg m-2). Uma mudança de uma relação negativa para uma positiva entre Hg e COT ocorreu no estuário mais eutrofizado, no qual um aumento nos níveis de Hg no sentido do topo foi observado. Este aumento pode ser atribuído a (i) um crescente aporte fluvial de Hg e (ii) um impacto da eutrofização sobre a qualidade dos sedimentos, que pode aumentar a sua capacidade de acumular Hg associado à matéria orgânica.
The accumulation of Hg was investigated in sediment cores from Guanabara Bay, Rio de Janeiro, Brazil, sampled along an eutrophication gradient, which was reflected in the sediment physical quality and total organic carbon (TOC) distribution. Sediments from the western bay area are strongly affected by inputs of urban and industrial effluents, showing Hg concentrations up to 7500 ng g-1 and Hg inventories up to 786 mg m-2, in contrast with near-background values found in the northwestern bay area (< 65 ng g-1 and 10.6 mg m-2). A shift from a negative to a positive relationship between Hg and TOC occurred in the most eutrophicated estuary, in which an upward increase in the recorded Hg levels was observed. This increase may be attributed to (i) an increasing riverine Hg input and (ii) an impact of eutrophication on the sediment quality, which may enhance the sediment capacity to accumulate organic matter-bound Hg.
