ABSTRACT Dopamine agonists are the first line of treatment for patients with symptomatic hyperprolactinemia due to prolactinomas and in those with idiopathic hyperprolactinemia. Treatment with these agents is effective in 80%-90% of the cases. Infertility treatment of patients with hyperprolactinemia is also carried out with dopamine agonists, aiming for the normalization of prolactin levels. The risk of symptomatic growth of prolactinomas during pregnancy is dependent on the tumor's size, duration of previous treatments, and prolactin levels. Notably, the corresponding risk is relatively low in cases of microprolactinomas (<5%). Remission of hyperprolactinemia occurs in about 30% of the patients after drug treatment and may also occur after pregnancy and menopause. The use of some drugs, such as antidepressants and antipsychotics, is a frequent cause of hyperprolactinemia, and managing this occurrence involves unique considerations. ABSTRACT Hyperprolactinemia is a frequent cause of menstrual irregularity, galactorrhea, hypogonadism, and infertility. The most common etiologies of hyperprolactinemia can be classified as physiological, pharmacological, and pathological. Among pathological conditions, it is essential to distinguish prolactinomas from other tumors and pituitary lesions presenting with hyperprolactinemia due to pituitary stalk disconnection. Proper investigation considering clinical data, laboratory tests, and, if necessary, imaging evaluation, is important to identify the correct cause of hyperprolactinemia and manage the patient properly. RESUMO: Objetivo: avaliar a qualidade de vida, funcionalidade e autoestima de pessoas no pós-alta por cura de hanseníase. Métodos: estudo epidemiológico transversal, desenvolvido numa instituição de referência para hanseníase na Paraíba-Brasil, de maio a agosto de 2021. Utilizaram-se quatro instrumentos para caracterização sociodemográfica e clínica, avaliação da qualidade de vida, da funcionalidade e da autoestima. Os dados foram analisados pelos testes qui-quadrado de Pearson, exato de Fisher, Mann-Whitney e Kruskal-Wallis. Resultados: participaram 131 pessoas, dos quais 86 (65,6%) eram do sexo masculino e 72 (55%) com 41 a 60 anos. Houve impacto negativo da qualidade de vida relacionada à saúde em não escolarizados e com baixa renda familiar. Limitação da funcionalidade em quem não pratica atividade física, afetado por neurite e com algum grau de incapacidade física. Contudo, elevada autoestima. Conclusão: torna-se imprescindível capacitar profissionais e organizar medidas assistenciais frente ao cuidado longitudinal e interdisciplinar nessa fase terapêutica. RESUMEN: Objetivo: evaluar la calidad de vida, la funcionalidad y la autoestima de las personas después del alta de la lepra. Métodos: Se trató de un estudio epidemiológico transversal realizado en una institución de referencia para lepra en Paraíba, Brasil, de mayo a agosto de 2021. Se utilizaron cuatro instrumentos para caracterización sociodemográfica y clínica, evaluación de la calidad de vida, funcionalidad y autoestima. Los datos se analizaron mediante las pruebas chi-cuadrado de Pearson, exacta de Fisher, Mann-Whitney y Kruskal-Wallis. Resultados: Participaron 131 personas, de las cuales 86 (65,6%) eran hombres y 72 (55%) tenían entre 41 y 60 años. Se observó un impacto negativo en la calidad de vida relacionada con la salud entre las personas sin estudios y con bajos ingresos familiares. Limitación de la funcionalidad en los que no practican actividad física, afectados por neuritis y con algún grado de discapacidad física. Sin embargo, alta autoestima. ABSTRACT Objective: to evaluate the quality of life, functionality, and self-concept of people after discharge from leprosy. Methods: This was a cross-sectional epidemiological study carried out in a reference institution for leprosy in Paraíba, Brazil, from May to August 2021. Four instruments were used for sociodemographic and clinical characterization, assessment of quality of life, functionality, and self-concept. The data was analyzed using Pearson’s chi-square, Fisher’s exact, Mann-Whitney, and Kruskal-Wallis tests. Results: 131 people took part, of whom 86 (65.6%) were male and 72 (55%) were aged between 41 and 60. There was a negative impact on health-related quality of life among those with no schooling and low family income. Limitation of functionality in those who do not practice physical activity, are affected by neuritis, and have some degree of physical disability. However, high self-concept. ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. Abstract The aim of the present study was to compare the oral conditions of children with congenital Zika syndrome (CZS)-associated microcephaly, non-CZS-associated microcephaly, and normotypical children, as well as to characterize their sociodemographic aspects and medical history. A paired cross-sectional study was carried out on 14 children with CZS-associated microcephaly and 24 age-matched controls, in Belo Horizonte, in southeastern Brazil. Children’s oral conditions were assessed: dental caries experience (dmft/DMFT indices); developmental defects of enamel (DDE) index; dental anomalies; mucosal changes; lip sealing, and malocclusion (overjet, overbite, and/or posterior crossbite alterations). The quality of oral hygiene was analyzed by the simplified oral hygiene index. The children’s mothers also answered a questionnaire about sociodemographic and medical history data. The variables were analyzed descriptively. Female participants were more prevalent (60.5%), and the mean age of the participants was 4.9 years (±1.4) (range: 2–8 years) and 92.1% of their exhibited some oral condition. All participants with CZS-associated microcephaly showed absence of lip sealing and had malocclusion (100.0%). When compared to the other groups, children with CZS had a higher percentage of dental anomalies (35.7%), mucosal changes (71.4%), and unsatisfactory oral hygiene (64.3%). In a sample composed mainly of female participants aged less than 5 years, the prevalence of oral conditions and unsatisfactory oral hygiene was higher in the group with CZS-associated microcephaly, followed by the group with non-CZS-associated microcephaly. Resumo: O objetivo do presente artigo é compreender como é construído o debate em torno do conceito nas publicações vinculadas à International Society for Knowledge Organization Brasil. Foi realizado levantamento de literatura na série “Estudos Avançados em Organização do Conhecimento”, publicada em seis volumes pelo capítulo brasileiro da organização, da qual foram selecionados 23 trabalhos sobre os quais se realizou uma Revisão Bibliográfica de Literatura de caráter não exaustivo. Identificou-se a presença de importantes debates em relação ao conceito na literatura, tanto em sentido mais estritamente teórico, quanto numa dimensão bastante ligada à prática, com diversos trabalhos apresentando objetos relacionados, como a modelagem conceitual e mapas conceituais. Além disso, pode-se notar a grande influência exercida pela Teoria do Conceito de Dahlberg na produção consultada. Tal amplitude temática e teórica demonstra o caráter incontornável da discussão a respeito dos fundamentos teóricos do conceito para pensar os processos de Organização e Representação, tanto do Conhecimento quanto da Informação. Abstract: The objective of this article is to understand how the debate around the idea of concept is being constructed in publications linked to the International Society for Knowledge Organization Brazil. To this end, we carried out a literature survey in the series “Advanced Studies in Knowledge Organization”, published in six volumes by the Brazilian chapter of the organization, from which 23 works were selected on which a non-exhaustive Bibliographic Literature Review was carried out. The presence of important debates regarding the concept in the literature was identified, both in a more strictly theoretical sense and in a dimension closely linked to practice, with several works presenting related objects, such as conceptual modeling and conceptual maps, for example. Furthermore, the great influence exerted by Dahlberg's Concept Theory on the consulted production can be noted. Abstract: This study assessed the influence of sociodemographic conditions, oral hygiene habits, and the socio-psychological need for orthodontic treatment on the emotions of caregivers of children with and without autism spectrum disorder (ASD). We conducted a comparative cross-sectional study with families of individuals aged 6 to 14 years at a reference center for neurodevelopmental disorders and two dental school clinics in northeastern Brazil. Caregiver emotions were assessed using the ‘Parental Emotions’ domain of the Family Impact Scale (FIS). We analyzed sociodemographic variables, oral hygiene habits, and sociopsychological need for orthodontic treatment using the esthetic component of the Index of Orthodontic Treatment Need (IOTN). We used descriptive and hierarchical Poisson regression analyses with robust variance (α = 5%). The study included 144 families evenly distributed across the groups. The caregiver group with ASD demonstrated a higher total score for parental emotions (p < 0.001). Factors associated with this factor included caregiver responsible for brushing (PR = 1.34; 95%CI: 1.12–1.59), mandatory need for orthodontic treatment (PR = 1.25; 95%CI: 1.07–1.46), and caregivers’ education up to 8 years (PR = 1.45; 95%CI: 1.02–2.07). Caregivers with lower income showed a lower prevalence of parental emotions (PR = 0.57; 95%CI: 0.35–0.93). Caregivers of children with ASD exhibited a higher emotional burden. Factors associated with parental emotions included responsibility for tooth brushing attributed to caregivers, sociopsychological need for orthodontic treatment, and family income. Objetivo: analisar os efeitos das tecnologias educativas na prevenção e tratamento da úlcera diabética. Método: revisão sistemática conduzida em sete bases de dados, um índice bibliográfico, uma biblioteca eletrônica e na literatura cinzenta. A amostra foi constituída de 11 ensaios clínicos controlados randomizados. A síntese dos resultados foi descritiva e por meio de metanálise. Resultados: as tecnologias educativas predominantes foram os treinamentos e as orientações verbais, destacando-se as tecnologias leve-duras. Na comparação com o cuidado usual, as tecnologias educativas apresentaram fator de proteção para prevenção da incidência de úlcera diabética (RR=0,40; IC 95%=0,18-0,90; p=0,03) e a avaliação de certeza da evidência foi baixa. As tecnologias educativas também tiveram fator de proteção para prevenção da incidência de amputação em membros inferiores (RR=0,53; IC 95%=0,31-0,90; p=0,02) e a certeza da evidência foi muito baixa. Conclusão: as tecnologias educativas leve-duras, como orientações verbais estruturadas, jogos educativos, aula expositiva, treinamentos teórico-práticos, vídeo educativo, folder, álbum seriado e desenhos lúdicos, e as tecnologias duras, a exemplo do calçado terapêutico, palmilhas, termômetro digital de infravermelho, kits de cuidados com os pés, aplicativo de telemedicina e telefone móvel, foram efetivas para prevenção e tratamento da úlcera diabética, porém, estudos mais robustos são necessários. Objective: to analyze the effects of educational technologies in the prevention and treatment of diabetic ulcers. Method: a systematic review conducted in seven databases, a bibliographic index, an electronic library and the Gray Literature. The sample consisted of 11 randomized controlled clinical trials. The synthesis of the results was descriptive and through meta-analysis. Results: the predominant educational technologies were training sessions and verbal guidelines, with soft-hard technologies standing out. When compared to usual care, the educational technologies presented a protective factor to prevent the incidence of diabetic ulcers (RR=0.40; 95% CI=0.18-0.90; p=0.03) and the certainty of the evidence assessment was low. The educational technologies also had a protective factor to prevent the incidence of lower limb amputations (RR=0.53; 95% CI=0.31-0.90; p=0.02) and certainty of the evidence was very low. Conclusion: soft-hard educational technologies such as structured verbal guidelines, educational games, lectures, theoretical-practical training sessions, educational videos, folders, serial albums and playful drawings, and hard technologies such as therapeutic footwear, insoles, infrared digital thermometer, foot care kits, Telemedicine app and mobile phone use, were effective for the prevention and treatment of diabetic ulcers, although more robust studies are required. Objetivo: analizar los efectos de las tecnologías educativas en la prevención y tratamiento de la úlcera diabética. Método: revisión sistemática realizada en siete bases de datos, un índice bibliográfico, una biblioteca electrónica y literatura gris. La muestra estuvo compuesta por 11 ensayos clínicos controlados aleatorizados. La síntesis de los resultados fue descriptiva y mediante metaanálisis. Resultados: las tecnologías educativas predominantes fueron la capacitación y la orientación verbal, se destacaron las tecnologías blandas-duras. En comparación con la atención habitual, las tecnologías educativas demostraron ser un factor protector para prevenir la incidencia de úlcera diabética (RR=0,40; IC 95%=0,18-0,90; p=0,03) y la evaluación de certeza de evidencia fue baja. Las tecnologías educativas también demostraron ser un factor protector para prevenir la incidencia de amputación en miembros inferiores (RR=0,53; IC 95%=0,31-0,90; p=0,02) y la certeza de evidencia fue muy baja. Conclusión: tecnologías educativas blandas-duras, como orientación verbal estructurada, juegos educativos, clase expositiva, capacitación teórico-práctica, video educativo, folder, rotafolio educativo y dibujos lúdicos, y tecnologías duras, como calzado terapéutico, plantillas, termómetro infrarrojo digital, kits para el cuidado de los pies, aplicación de telemedicina y teléfono móvil, resultaron efectivas para la prevención y el tratamiento de la úlcera diabética, sin embargo, es necesario que se realicen estudios más robustos. ABSTRACT BACKGROUND AND OBJECTIVES: The type of questionnaire that aims to capture a patient’s perception/view of an aspect to be measured (e.g. pain intensity) is called Patient Reported Outcome Measure (PROM). One of the biggest challenges that clinicians and researchers often face is making a decision about which PROM to use for the assessment of their patient with pain, especially due to the lack of scientific literacy needed to understand the criteria and terms used in the field of measurement properties. Thus, the objectives of this narrative review (part I) were: (I) to introduce basic concepts about PROMs with a focus on the terminology and criteria defined through the COnsensus- based Standards for the selection of health Measurement INstruments (COSMIN), and (2) to describe the measurement properties of the reliability domain. METHODS: This study was produced using a search for articles from the COSMIN initiative. As the subject is very extensive, the authors divided the text into two parts. RESULTS: This study described basic concepts about PROMs (purposes and constructs), the process of cross-cultural adaptation and the measurement properties of the reliability domain (reliability, error measure and internal consistency). In general, an instrument with adequate reliability quality should meet certain criteria, such as: intraclass correlation coefficient ≥0.70, error measure < minimal clinically important change and Cronbach’s Cronbach’s α ≥ 0.70. CONCLUSION: The understanding on how to determine the quality of reliability can assist clinicians and researchers in choosing the best PROMs available. A checklist for assessing the quality of the measurement properties of PROMs is described in the part II of the manuscript. RESUMO JUSTIFICATIVA E OBJETIVOS: O tipo de questionário que pretende captar a percepção/visão de um paciente sobre um aspecto a ser medido (ex: intensidade da dor) é chamado de Instrumento de Medida Baseado no Relato do Paciente (Patient Reported Outcome Measure - PROM). Um dos maiores desafios que clínicos e pesquisadores costumam enfrentar é quanto a tomar uma decisão sobre qual PROM utilizar para a avaliação de seu paciente com dor, especialmente devido à falta do letramento científico necessário para entender os critérios e termos empregados na área de propriedades de medida. Assim, os objetivos deste estudo (parte I) foram: (I) introduzir conceitos básicos sobre PROMs com enfoque na terminologia e critérios definidos através do COnsensus-based Standards for the selection of health Measurement INstruments (COSMIN), e (2) descrever as propriedades de medida do domínio confiabilidade. Abstract Geopolymers are a potential alternative substitute for Portland cement binders in some applications. However, there are many unexplored research topics to be conducted on this subject. Thus, this study aimed to investigate the rheological and mechanical properties of geopolymer pastes based on Class F fly ash activated by a solution of NaOH and Na 2 SiO 3 , with activator/precursor ratios of 0.5, 0.6 and 0.7, by mass. Furthermore, microstructural and chemical analyses. Rheological properties were determined from flow tests with shear rates between 0 s -1 and 100 s -1 at 25°C. Mechanical analysis was performed from simple compressive strength tests on the hardened pastes. The results of simple compression indicated better strength values for lower activator/precursor ratios. In flow tests, the material behaved as a pseudoplastic fluid, and, with the increase of the activator/precursor ratio, there was a reduction of apparent viscosity values. Resumo Os geopolímeros figuram como uma alternativa potencial aos ligantes de cimento Portland em algumas aplicações. Contudo, existem muitas lacunas de pesquisa a serem preenchidas dentro dessa temática. Assim, este estudo teve como objetivo investigar as propriedades reológicas e mecânicas de pastas geopoliméricas à base de cinza volante Classe F ativadas por uma solução de NaOH e Na2SiO3, com relações ativador/precursor de 0,5, 0,6 e 0,7, em massa. Além disso, foram realizadas análises microestruturais e químicas. As propriedades reológicas foram determinadas a partir de ensaios de fluxo com taxas de cisalhamento variando de 0 s-1 a 100 s-1 a 25 ºC. A análise mecânica foi realizada a partir de ensaios de resistência à compressão simples nas pastas endurecidas. Os resultados de resistência à compressão simples indicaram melhores valores para menores relações de ativador/precursor. Nos ensaios de fluxo, o material comportou-se como um fluido pseudoplástico e, com o aumento da relação ativador/precursor, houve redução dos valores de viscosidade aparente. Abstract Objective: this study aimed to evaluate if there is an association of intake of coffee and its polyphenols with mammographic findings in women treated at a breast care service unit of the Unified Health System (SUS), Brazil. Research methods and procedures: this was a cross-sectional study with 532 women treated at a health service. The participants were divided according to their mammographic reports into two groups: without and with altered findings. Two 24-h dietary recalls were applied and coffee consumption was categorized into three groups (less than 1 cup, 1 to 3 cups, and more than 3 cups). Phenolic acids were determined using the Phenol Explorer program. The intake of polyphenols was calculated by adding the values obtained from the total amount of coffee consumed during the day. The Multiple Source Method (MSM) was applied to analyze the usual intake. Results: of the 532 women, 178 (33.5 %) had altered mammographic findings. The participants' average daily coffee intake was 193.4 mL. No significant association was found between coffee consumption and mammographic findings. However, it was found that the second tertile of polyphenols was a protective factor for breast changes. Conclusion: coffee polyphenols are protective against breast changes in the group evaluated and, thus, can help prevent breast cancer.
Resumen Objetivo: este estudio tuvo como objetivo evaluar si existe una asociación de la ingesta de café y sus polifenoles con los hallazgos mamográficos de mujeres tratadas en una unidad de patología mamaria del Sistema Unificado de Salud (SUS), Brasil. Métodos y procedimientos de investigación: este fue un estudio transversal con 532 mujeres tratadas en un servicio de salud. Las participantes se dividieron de acuerdo con sus informes mamográficos en dos grupos: sin y con hallazgos alterados. Se aplicaron dos registros dietéticos de 24 horas y el consumo de café se clasificó en tres grupos (menos de 1 taza, 1 a 3 tazas y más de 3 tazas). Los ácidos fenólicos se determinaron utilizando el programa Phenol Explorer. La ingesta de polifenoles se calculó sumando los valores obtenidos de la cantidad total de café consumido durante el día. Se aplicó el método de fuentes múltiples (MSM) para analizar la ingesta habitual. Resultados: de las 532 mujeres, 178 (33,5 %) tenían hallazgos mamográficos alterados. La ingesta diaria promedio de café de los participantes fue de 193,4 ml. No se encontró una asociación significativa entre el consumo de café y los hallazgos mamográficos. Sin embargo, se encontró que el segundo tercil de polifenoles era un factor protector para los cambios mamarios. Conclusión: los polifenoles del café son protectores contra los cambios mamarios en el grupo evaluado y, por lo tanto, podrían ayudar a prevenir el cáncer de mama.

Factors associated with women diagnosed with syphilis who received prenatal care in a primary healthcare unit

ABSTRACT Objective To estimate the prevalence of syphilis and its associated factors in women who were treated at public maternity hospitals and received prenatal care in a primary healthcare unit. Methods This cross-sectional study included 399 postpartum women. Interviews were conducted, and additional data were extracted from the pregnant woman’s booklet, medical records, and printed tests. The dependent variable was a gestational syphilis diagnosis. The independent variables were grouped into socioeconomic and demographic, behavioral, reproductive, and prenatal blocks. The prevalence, prevalence ratios, and 95% confidence intervals (95%CI) were calculated. The χ 2 test was also performed (p≤0.05). Multivariate analysis was performed using Poisson regression models. Results The prevalence of gestational syphilis was 9.61% (95%CI: 7.14-12.83). We identified the following determining factors (adjusted prevalence ratios): history of sexually transmitted infections (2.3), first sexual intercourse by the age of 15 (2.42), partner having a history of syphilis (5.98), partner using crack/cocaine (6.42) and marijuana and others (3.02), not having a partner (3.07), low income (2.85), history of stillbirth (5.21), beginning prenatal care in the third trimester (3.15), and prenatal care received in a primary healthcare unit (without a Family Health Strategy team) (0.35). Conclusion Individual and prenatal factors were associated with gestational syphilis.
