Resumo A doença cerebrovascular extracraniana tem sido intensamente investigada em todo o mundo, sendo tema de suma importância para os cirurgiões vasculares. A presente Diretriz foi elaborada pela Sociedade Brasileira de Angiologia e Cirurgia Vascular (SBACV) em sucessão à Diretriz de 2015. As doenças de etiologia não ateroscleróticas não foram incluídas nesse documento. O objetivo desta Diretriz é congregar as evidências mais robustas nessa área para auxiliar os especialistas no processo decisório do tratamento. Foi utilizada a metodologia AGREE II e o sistema da Sociedade Europeia de Cardiologia para as recomendações e níveis de evidências. As recomendações foram graduadas de I a III, e os níveis de evidência classificados em A, B e C. Abstract Extracranial cerebrovascular disease has been the subject of intense research throughout the world, and is of paramount importance for vascular surgeons. This guideline, written by the Brazilian Society of Angiology and Vascular Surgery (SBACV), supersedes the 2015 guideline. Non-atherosclerotic carotid artery diseases were not included in this document. The purpose of this guideline is to bring together the most robust evidence in this area in order to help specialists in the treatment decision-making process. The AGREE II methodology and the European Society of Cardiology system were used for recommendations and levels of evidence. The recommendations were graded from I to III, and levels of evidence were classified as A, B, or C. ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. ABSTRACT The SISBIOTA-BRASIL was a three-year multimillion-dollar research program of the Brazilian government to document plants and animals in endangered/understudied areas and biomes in Brazil. Distributional patterns and the historical events that generated them are extensively unknown regarding Brazilian fauna and flora. This deficiency hinders the development of conservation policies and the understanding of evolutionary processes. Conservation decisions depend on precise knowledge of the taxonomy and geographic distribution of species. Given such a premise, we proposed to research the diversity of Diptera of the Brazilian western arc of Amazon, Cerrado, and Pantanal in the states of Mato Grosso, Mato Grosso do Sul, and Rondônia. Three important biomes of the South American continent characterize these Brazilian states: Amazon forest, Cerrado (Brazilian Savannah), and Pantanal. Besides their ecological relevance, these biomes historically lack intensive entomological surveys. Therefore, they are much underrepresented in the Brazilian natural history collections and in the scientific literature, which is further aggravated by the fact that these areas are being exponentially and rapidly converted to commercial lands. Our project involved over 90 collaborators from 24 different Brazilian institutions and one from Colombia among researchers, postdocs, graduate and undergraduate students, and technicians. We processed and analyzed nearly 300,000 specimens from ~60 families of Diptera collected with a large variety of methods in the sampled areas. Here, we provide a detailed overview of the genera and species diversity of 41 families treated. Our results point to a total of 2,130 species and 514 genera compiled and identified for the three states altogether, with an increase of 41% and 29% in the numbers of species and genera known for the three states combined, respectively. Overall, the 10 most species-rich families were Tachinidae, Cecidomyiidae, Tabanidae, Psychodidae, Sarcophagidae, Stratiomyidae, Bombyliidae, Syrphidae, Tephritidae, and Asilidae. The 10 most diverse in the number of genera were Tachinidae, Stratiomyidae, Asilidae, Mycetophilidae, Syrphidae, Tabanidae, Muscidae, Dolichopodidae, Sarcophagidae, and Chloropidae. So far, 111 scientific papers were published regarding taxonomic, phylogenetic, and biogeographical aspects of the studied families, with the description of 101 new species and three new genera. We expect that additional publications will result from this investigation because several specimens are now curated and being researched by specialists. ABSTRACT Introduction: Although endovascular correction is a promising perspective, the gold-standard treatment for thoracoabdominal aortic aneurisms and type-B dissections with visceral involvement remains open surgery, particularly due to its well-established long-term durability. This study aims to describe and evaluate public data from patients treated for thoracoabdominal aortic aneurism in the Brazilian public health system in a 12-year interval. Methods: Data from procedures performed between 2008 and 2019 were extracted from the national public database (Departamento de Informática do Sistema Único de Saúde, or DATASUS) using web scraping techniques. Procedures were evaluated regarding the yearly frequency of elective or urgency surgeries, in-hospital mortality, and governmental costs. All tests were done with a level of significance P<0.05. Results: A total of 812 procedures were analyzed. Of all surgeries, 67.98% were elective cases. There were 328 in-hospital deaths (mortality of 40.39%). In-hospital mortality was lower in elective procedures (26.92%) than in urgency procedures (46.74%) (P=0.008). Total governmental expenditure was $3.127.051,56 — an average of $3.774,22 for elective surgery and $3.791,93 for emergency surgery (P=0.999). Conclusion: The proportion of urgency procedures is higher than that recommended by international literature. Mortality was higher for urgent admissions, although governmental costs were equal for elective and urgent procedures; specialized referral centers should be considered by health policy makers.

ABSTRACT Objective Despite the development of endovascular procedures, open repair remains the gold standard for the treatment of aortic thoracoabdominal aneurysms and some type B dissections, with well-established good outcomes and long-term durability at high-volume centers. The present study described and analyzed public data from patients treated in the public system in a 12-year interval, in a city where more than 5 million inhabitants depend on the Public Health System. Methods Public data from procedures performed between 2008 and 2019 were extracted using web scraping techniques. The variables available in the database include sex, age, elective or emergency hospital admission, number of surgeries, in-hospital mortality, length of stay, and information on reimbursement values. Results A total of 556 procedures were analyzed. Of these, 60.79% patients were men, and 41.18% were 65 years of age or older. Approximately 60% had a residential address registered in the municipality. Of all surgeries, 65.83% were elective cases. There were 178 in-hospital deaths (mortality of 32%). In the elective context, there were 98 deaths 26.78% versus 80 deaths (42.10%) in the emergency context (p=0.174). Mortality was lower in the hospitals that performed more surgeries. A total of USD 3,038,753.92 was paid, an average of USD 5,406.95 for elective surgery and USD 5,074.76 for emergency surgery (p=0.536). Conclusion Mortality was no different between groups, and hospitals with higher volume presented more favorable outcomes. Specialized referral centers should be considered by health policy makers.

Abstract: The description of experiences in self-managed networks of collaboration between schools located in contexts of high social vulnerability is a scarcely explored field. In this context, the objective is to report the experience of a self-managed school improvement network with such characteristics, which has been organized since the beginning of 2017 by five principals belonging to municipal schools with a high School Vulnerability Index. The experience was systematized based on the reflections of the directors and the methodological support of two external researchers. The motivations that originate the network were analyzed, emerging intentions such as the need to compensate for technical support, generate emotional support, apply knowledge and achieve improvement. The main practices of the network are the meetings of reflections between principals and internships of teachers to other educational establishments. Likewise, the facilitators, obstacles and contributions that contextualize and influence the work of the network are examined. Finally, the network operation is thoughtfully discussed, and a projective analysis is carried out from the point of view of its trajectory and its characteristics, such as its self-managed nature, the managerial leadership and the collaboration. In parallel, this last section considers the tensions between the experiences of collaborative self-management and the current Chilean educational model.
Resumen: La descripción de experiencias en redes autogestionadas de colaboración entre escuelas ubicadas en contextos de alta vulnerabilidad social es un campo escasamente explorado. Bajo este contexto, se establece como objetivo reportar la experiencia de una red autogestionada de mejoramiento escolar con tales características, la cual se encuentra organizada desde comienzos del año 2017 por cinco directoras pertenecientes a escuelas municipales con alto Índice de Vulnerabilidad Escolar. Se realizó la sistematización de la experiencia en base a las reflexiones de las directoras y el apoyo metodológico de dos investigadores externos. Destaca entre la información analizada las motivaciones que originan la red, emergiendo intenciones como la necesidad de compensar el soporte técnico, generar apoyo emocional, aplicar conocimientos y lograr mejoramiento. Las principales prácticas de la red constituyen las reuniones de reflexiones entre directoras y las pasantías de los docentes a otros establecimientos educacionales. Asimismo, se examinan los facilitadores, obstaculizadores y aportaciones que contextualizan e influyen en el trabajo de la red. Finalmente, se discute reflexivamente el funcionamiento de red y se realiza un análisis proyectivo desde el punto de vista de su trayectoria y sus características, tales como su naturaleza autogestionada, el liderazgo directivo y la colaboración. Paralelamente, en este último apartado se consideran las tensiones entre las experiencias de autogestión colaborativa y el actual modelo educacional chileno.

There have been many Cuban doctors who have devoted themselves to the study of Medical Microbiology, including well-known ones as the Matanzasan Juan Guiteras Gener. With this work the authors pretended to highlight the figure of the Colombinan microbiologist Florencio Angel Duque Pérez, the first microbiologist trained after the Revolution, with an important work. Deceased several years ago, this excellent professional is scarcely known and has been virtually forgotten. This biography is intended to all those who knew him and to all those who did not have that great pleasure.
Muchos han sido los médicos cubanos que se han dedicado al estudio de la microbiología médica, entre ellos algunos muy conocidos como el matancero Juan Guiteras Gener. Con este trabajo los autores pretendieron resaltar la figura del microbiólogo colombino Ángel Florencio Duque Pérez, primer microbiólogo matancero formado por la revolución y con una esmerada labor. Fallecido hace varios años, este excelente profesional es poco conocido y prácticamente ha quedado en el olvido. Para todos aquellos que lo conocieron y para los que no tuvieron ese enorme placer se les hace llegar esta síntesis biográfica.

Linguistic competence develops mainly during the first years of a child and influences the way in which he gets along with her family, peers, other adults and in school contexts, and have a significant impact in the acquisition of formal and informal learning skills. This research studies the influence that the parent’s mentalization and the socio-economic status have on the level of language skills attained by that child when he is 30 months old. Subjects were eighty-five children and their respective care providers from different socio-economic status, all residents of Santiago de Chile. We applied the Bayley III Language Scale and an instrument to assess mentalization. Our results show that parent’s mentalization assessed at the age of 12 months and the socio-economic status influence the language skills of the same children when they become 30 months old. Language receptive ability at the age of 12 months is also a predictor of the same at the age of 30 months, but not so of language expressive ability. We discuss the results obtained and their implications
Las competencias lingüísticas se desarrollan principalmente en los primeros años de vida del niño e influyen en la forma en que este se desenvuelve con su familia, pares, otros adultos y en contextos escolares, afectando significativamente la adquisición de aprendizajes formales e informales. La presente investigación estudia la influencia de la mentalización de los cuidadores principales y el nivel socioeconómico, sobre el nivel de desarrollo del lenguaje alcanzado por el niño a los 30 meses de edad. La muestra se compuso de 85 niños y sus cuidadores, de diferentes niveles socioeconómicos y residentes en la ciudad de Santiago de Chile. Se aplicó la Escala de Lenguaje del Bayley III y un instrumento para medir la mentalización. Los resultados indican un efecto significativo de la mentalización de los padres evaluada a los 12 meses y del nivel socioeconómico, sobre el lenguaje de los niños a los 30 meses de edad. El lenguaje receptivo a los 12 meses también es un factor predictor del mismo a los 30 meses, no así el lenguaje expresivo. Se discuten los resultados y sus implicancias

RESUMOO objetivo é documentar um caso de persistência hiperplásica do vítreo primário (PHPV) com papila de Bergmeister atípica e artéria hialóide pérvia que teve seu diagnóstico conclusivo feito pelo exame ultrassonográfico com Doppler colorido. Relatamos um caso de uma criança com leucocoria unilateral, apresentando massa branca sobre o disco óptico observada à fundoscopia, que se estendia à retina adjacente. Após a realização do exame ultrassonográfico foi mantida a dúvida com as seguintes hipóteses diagnósticas: persistência hiperplásica do vítreo primário, granuloma (toxocaríase) e retinoblastoma. O diagnóstico foi estabelecido após a utilização do Doppler colorido que evidenciou fluxo sanguíneo no interior da membrana, confirmando o diagnóstico de persistência hiperplásica do vítreo primário associada à papila de Bergmeister.
ABSTRACTThe purpose is to document a case of persistent hiperplastic primary vitreous (PHPV) with atypical Bergmeister's papilla and patent hialoid artery that had its conclusive diagnosis done by doppler ultrasound. We report a case of child, who had unilateral leucokoria. This child's fundoscopic examination showed a white mass on the optic nerve that extended over to adjacent retina. Performed ultrasound that remained a doubt with the following diagnostic hypotheses: persistent hiperplastic primary vitreous, granuloma (toxocara), astrocytic hamartoma and retinoblastoma. The diagnosis was only established when the doppler ultrasound showed a blood flow inside of the membrane, thus confirming the diagnosis of persistent hiperplastic primary vitreous associated with the Bergmeister'spapilla.

For several decades have existed in Mexico large areas impacted by oil spills. The majority of current bioremediation technologies use microorganisms to clean up oil sites. In this work, were prepared and evaluated three doses of Proteus sp. inoculants on clay-loam soil contaminated with hydrocarbons. The soil samples were collected in the municipality of Cunduacán, Tabasco, México. We determined the amount of total petroleum hydrocarbons (TPH) removed and the magnitude of population development of microorganisms during the study, using a factorial experimental design 3x3x3: three concentrations of HTP (treatments T1=50 000 ppm, T2=70 000 ppm, T3=90 000 ppm), three times seed development of the microorganism pre-planted in the contaminated soil (D1=48 h D2=72 h D3=96 h) and three replications. The data were analyzed using ANOVA procedure and mean test using the statistical software SAS-6. The dose that showed the highest degradation within 90 d was D1 in all treat-ments: 70.99 % (T1), 75.58 % (T2) and 67.56 % (T3) with late bacterial population 3.506x10³, 4.100x10³ and 3.867x10³ CFU/g of soil, respectively. Therefore, it is recommended to prepare the inoculum 48 h before being discharged into the clay-loam soil contamined with Istmo crude petroleum, and conditioning with Triple 17 in order to perform bioremediation.
Desde hace varias décadas existen en México extensas áreas impactadas por derrames petroleros. La mayoría de las tecnologías de biorremediación actuales utilizan microorganismos para descontaminar sitios con hidrocarburos. En este trabajo se prepararon y evaluaron tres dosis inoculantes de Proteus sp. sobre un suelo arcilla-limoso contaminado con hidrocarburos. Las muestras de suelo se colectaron en el municipio Cunduacán, Tabasco, México. Se determinaron la cantidad de hidrocarburos totales de petróleo (HTP) removidos y la magnitud del desarrollo poblacional microbiano durante el estudio, empleando un diseño experimental tipo factorial 3x3x3: tres concentraciones de HTP (tratamientos T1=50 000 ppm, T2=70 000 ppm, T3=90 000 ppm), tres tiempos de desarrollo del inóculo del microorganismo previos al sembrado en el suelo contaminado (D1=48 h D2=72 h D3=96 h) y tres repeticiones. Los datos fueron analizados con el procedimiento ANOVA y prueba de medias mediante el paquete estadístico SAS-6.0. La dosis que presentó la mayor degradación en un lapso de 90 d fue la D1 en todos los tratamientos: 70.99 % (T1), 75.58 % (T2) y 67.56 % (T3) con poblaciones bacterianas finales de 3.506x10³, 4.100x10³ y 3.867x10³ UFC/g de suelo, respectivamente. Se recomienda preparar el inoculante 48 h antes de ser vertido al suelo arcilla-limoso, contaminado con petróleo crudo tipo Istmo y acondicionado con Triple 17, para realizarle biorremediación.

In February 2009, we commenced an investigation for WNV in mosquitoes, horses and caimans from the Pantanal, Central-West Brazil. The sera of 168 horses and 30 caimans were initially tested using a flaviviruses-specific epitope-blocking enzyme-linked immunosorbent assay (blocking ELISA) for the detection of flavivirus-reactive antibodies. The seropositive samples were further tested using a plaque-reduction neutralisation test (PRNT90) for WNV and its most closely-related flaviviruses that circulate in Brazil to confirm the detection of specific virus-neutralising antibodies. Of the 93 (55.4%) blocking ELISA-seropositive horse serum samples, five (3%) were seropositive for WNV, nine (5.4%) were seropositive for St. Louis encephalitis virus, 18 (10.7%) were seropositive for Ilheus virus, three (1.8%) were seropositive for Cacipacore virus and none were seropositive for Rocio virus using PRNT90, with a criteria of > four-fold antibody titre difference. All caimans were negative for flaviviruses-specific antibodies using the blocking ELISA. No virus genome was detected from caiman blood or mosquito samples. The present study is the first report of confirmed serological evidence of WNV activity in Brazil.

O estudo verificou-se o efeito da estimulação cortical no tempo de reação e eficiência hábil-motriz-cognitiva em atletas de futebol, pré e pós-estimulação cortical A amostra de (N=20) atletas masculino de futebol da categoria infantil. Os participantes foram divididos em 2 grupos, controle e experimental, sendo o grupo experimental submetido à estimulação. Foram realizadas 10 sessões, de 20'. Os dados do teste foram tratados através de uma análise de variância paramétrica (ANOVA) two way. A média de acertos / pelo tempo de reação obteve ganho significativo para o grupo experimental quando comparada ao controlo no momento pós-teste (P<0.05). No grupo experimental obteve ganho significativo na melhora do tempo de reação quando comparado ao grupo controle (p<0.05). No intra-grupos, o grupo que recebeu o treinamento combinado, obteve melhoras significativas comparados o pré e pós-teste (p<0,05). Concluiu-se que o treinamento combinado pode-se aprimorar a aprendizagem motora em diversas áreas de conhecimento e esporte.
The study was verified the effect of the cortical stimulation in the time of reaction and skilled motor cognitive efficiency in soccer athletes, before and cortical after stimulation the sample of (N=20) athletes masculine of soccer of the infantile category. The participants were divided in 2 groups, control and experimental, being the experimental group submitted to the stimulation. 10 sessions, of 20'. The data originating from of the test were treated through a parametric variance analysis (ANOVA) two way. The average of successes / for the time of reaction obtained significant earnings for the experimental group when compared to the control in the moment after - tests (P <0.05). In the experimental group obtained significant earnings in the improvement of the time of reaction when compared to the group controls (p <0.05). In the in groups, the group that it received the combined training, obtained significant improvements compared the before and after-test (p <0,05). It was ended that the combined training the motor learning can be perfected in several knowledge areas and sport.

Morphological and hemodynamic patterns of carotid stenosis treated by endarterectomy with patch closure versus stenting: a duplex ultrasound study

OBJECTIVES: A duplex ultrasound study was performed to investigate morphological and hemodynamic patterns of carotid stenoses treated by endarterectomy with patch closure versus stenting. MATERIALS AND METHOD: Twenty-nine carotid stenoses were treated with stenting and 65 with patch closure. Duplex ultrasound parameters (luminal diameter, mm; peak systolic velocity and end-diastolic velocity, cm/s) were measured 24 hours after the procedures and also at 12 months post-procedure. Residual stenoses (immediately postprocedure) and restenoses (within 12 months of procedure) were defined as narrowings of >50% on duplex ultrasound examination. RESULTS: In stented patients, the luminal diameter of the proximal internal carotid artery increased in the interval between the 24-hour and 12-month post-procedure studies, while in the patch closure patients, the diameter decreased. Carotid hemodynamics normalized immediately after both patching and stenting and remained relatively stable thereafter up to 12 months. No statistically elevated flow velocities (in the absence of residual stenosis or restenosis) were observed in the patched or stented carotid arteries. No significant differences in residual stenosis rates were observed between the stenting group (3 cases, 10.34%) and the patch closure group (1 case, 1.53%, P = 0.08). At 12 months, 2 stenting patients (6.88%) and 2 patch closure patients (3.07%) had $50% restenosis (P = 0.58). One case of late stroke due to restenosis was observed in the stenting group; the patient died 12 months postoperatively, before receiving new intervention. CONCLUSION: Measurements over time in luminal diameter signalized differences in arterial remodeling mechanisms between patched and stented carotids. Both stenting and patch closure were associated with carotid patency and flow restoration. This study does not support a general approach to new velocity criteria indiscriminately applied to stented or patched carotids.
