Abstract: Brazil and Uruguay have a historical tradition of welcoming refugees, especially those who are victims of armed conflicts around the world, and, due to their proximity to neighboring regions, those forced to leave their countries due to political and financial crises. With these premises, this academic paper will explore the landscape of fundamental rights for refugees, focusing on the legislation of Brazil and Uruguay. It will also delve into specific developments related to new issues of protection concerning gender and sexuality vulnerabilities, as well as collective issues, such as the irreversible impacts of climate change. Finally, the paper aims to demonstrate the possibility of adopting new technical and legislative criteria and measures to strengthen refugee protection. The importance of implementing public policies that reach a greater number of people in need and that update the legal concepts and instruments to address the emerging challenges is highlighted
Resumen: Brasil y Uruguay son países con tradición histórica en la acogida de refugiados, especialmente aquellos que son víctimas de conflictos armados en el mundo y, por las regiones continentales cercanas, aquellos que se ven obligados a abandonar sus países debido a crisis políticas y financieras. Con estas premisas, el presente trabajo académico abordará el panorama de los derechos fundamentales de los refugiados, destacando la legislación de Brasil y Uruguay, y explorando los desarrollos específicos relacionados con nuevas cuestiones de protección de vulnerabilidades de género y sexualidad, así como cuestiones colectivas, como los impactos irreversibles causados por el cambio climático. Finalmente, se pretende demostrar la posibilidad de adoptar nuevos criterios técnicos y legislativos y medidas para fortalecer la protección de los refugiados, destacando la importancia de implementar políticas públicas que abarquen a un mayor número de necesitados y actualicen los conceptos e instrumentos jurídicos que aborden las nuevas dificultades del escenario
Resumo: O Brasil e o Uruguai são países com tradição histórica na acolhida de refugiados, especialmente daqueles que são vítimas de conflitos armados ao redor do mundo e, devido à proximidade das regiões continentais, daqueles que são forçados a abandonar seus países devido a crises políticas e financeiras. Com base nessas premissas, o presente trabalho acadêmico abordará o panorama dos direitos fundamentais dos refugiados, destacando a legislação do Brasil e do Uruguai, e explorando os desenvolvimentos específicos relacionados a novas questões de proteção de vulnerabilidades de gênero e sexualidade, assim como questões coletivas, como os impactos irreversíveis causados pelas mudanças climáticas. Por fim, pretende-se demonstrar a possibilidade de adotar novos critérios técnicos e legislativos e medidas para fortalecer a proteção dos refugiados, destacando a importância de implementar políticas públicas que atendam a um maior número de necessitados e atualizem os conceitos e instrumentos jurídicos que tratam das novas dificuldades desse cenário

RESUMO A pandemia de covid-19 gerou um numeroso volume de produções científicas com diferentes níveis de qualidade. A velocidade com que o conhecimento era produzido e compartilhado a nível mundial impôs à gestão em saúde o desafio de buscar meios de identificar as melhores evidências disponíveis para subsidiar suas decisões. Em resposta a este desafio, o Departamento de Ciência e Tecnologia do Ministério da Saúde do Brasil estabeleceu um serviço para elaborar e disponibilizar conhecimento científico abordando questões prioritárias de saúde pública no cenário da pandemia. Entre os temas abordados estão tratamentos medicamentosos, medidas não farmacológicas, testagem, reinfecção e resposta imunológica, imunização, fisiopatologia, síndrome pós-covid e eventos adversos. Neste artigo, discute-se os pontos fortes e lições aprendidas, bem como os desafios e perspectivas que fornecem um exemplo real sobre como disponibilizar as melhores evidências científicas, em tempo hábil e de forma oportuna, para auxiliar o processo decisório durante uma emergência em saúde pública.
ABSTRACT The COVID-19 pandemic generated a large volume of scientific productions with different quality levels. The speed with which knowledge was produced and shared worldwide imposed on health management the challenge of seeking ways to identify the best available evidence to support its decisions. In response to this challenge, the Department of Science and Technology of the Brazilian Ministry of Health started offering a service to produce and provide scientific knowledge addressing priority public health issues in the pandemic scenario. Drug treatments, non-pharmacological measures, testing, reinfection and immunological response, immunization, pathophysiology, post-COVID syndrome and adverse events are among the topics covered. In this article, we discuss the strengths and lessons learned, as well as the challenges and perspectives that present a real example of how to offer the best scientific evidence in a timely manner in order to assist the decision-making process during a public health emergency.

ABSTRACT Hematopoietic stem cell transplant (HSCT) recipients are at -increased risk for severe COVID-19. The aim of this study was to evaluate the burden of COVID-19 in a cohort of HSCT recipients. This retrospective study evaluated a cohort of adult hospitalized HSCT recipients diagnosed with COVID-19 in two large hospitals in São Paulo, Brazil post-HSCT, from January 2020 to June 2022. The primary outcome was all-cause mortality. Of 49 cases, 63.2% were male with a median age of 47 years. Allogeneic-HSCT (51.2%) and autologous-HSCT (48.9%) patients were included. The median time from HSCT to COVID-19 diagnosis was 398 days (IQR: 1211-134), with 22 (44.8%) cases occurring within 12 months of transplantation. Most cases occurred during the first year of the pandemic, in non-vaccinated patients (n=35; 71.4%). Most patients developed severe (24.4%) or critical (40.8%) disease; 67.3% received some medication for COVID-19, primarily corticosteroids (53.0%). The probable invasive aspergillosis prevalence was 10.2%. All-cause mortality was 40.8%, 51.4% in non-vaccinated patients and 14.2% in patients who received at least one dose of the vaccine. In the multiple regression analyses, the variables mechanical ventilation (OR: 101.01; 95% CI: 8.205 – 1,242.93; p = 0.003) and chest CT involvement at diagnosis ≥50% (OR: 26.61; 95% CI: 1.06 – 664.26; p = 0.04) remained associated with all-cause mortality. Thus, HSCT recipients with COVID-19 experienced high mortality, highlighting the need for full vaccination and infection prevention measures. (HSCT increased COVID19. Abstract This study aimed to evaluate the influence of the Er,Cr:YSGG irradiation and 980-nm diode lasers on the surface roughness (SR) and volume loss (VL) of dentin subjected to cariogenic challenge. Subsequently, 130 specimens of bovine dentin were divided into the following 13 groups: NT: no treatment; FG: fluoride gel; FV: fluoride varnish; Di: 980-nm diode; Di + FG; Di + FV; FG + D; FV + Di; Er: Er,Cr:YSGG; Er + FG; Er + FV; FG + Er and FV + Er. Er,Cr:YSGG laser parameters were as follows: 0.25 W; 5.0 Hz; 4.46 J/cm2 without water and 55% air. Furthermore, the 980-nm diode laser parameters were 2.0 W; 2.0 Hz; 21.41 J/cm2. The samples from each group were subjected to pH cycling. A confocal laser scanning microscope was used to evaluate SR and VL. Difference between the volume of the reference and treated areas + DES/RE was used to determine SR and VL. The mean values of the different groups were subjected to analysis of variance and Tukey’s post-hoc test. The VL values were analyzed using the Kruskal–Wallis and Dunn post-hoc test (p < 0.05). The SR of the reference area did not show a statistically significant 1807-3107-bor-38-e025treatment and cariogenic challenge (p > 0.05). Moreover, VL in the FV + Di and FV + Er groups showed a statistically significant difference compared with areas submitted to different types of treatment and cariogenic challenge (p > 0.05). Resumo: O objetivo deste estudo qualitativo foi caracterizar a estrutura e as funções das redes sociais significativas no processo de luto antecipatório de familiares cuidadores no contexto dos cuidados paliativos. Utilizou-se a entrevista semiestruturada e o Mapa de Redes com 14 familiares cuidadores que estavam acompanhando pacientes em um hospital oncológico do Sul do Brasil. Os dados foram organizados e analisados com base na Grounded Theory e com auxílio do software ATLAS.ti 7.5. Os resultados indicaram a prevalência de redes de tamanho médio e grande, além do predomínio dos membros da rede familiar. As principais funções desempenhadas pelos membros das redes foram de ajuda material e de serviços e apoio emocional. Este estudo evidencia a importância da visibilização das redes sociais significativas como recursos de intervenção em saúde, que promovem apoio material, cognitivo e emocional durante o processo dos cuidados paliativos, principalmente quanto à elaboração do luto antecipatório. Résumé : Cette étude qualitative caractérise la structure et les fonctions des réseaux sociaux significatifs dans le processus de deuil anticipé des aidants familiaux dans le cadre des soins palliatifs. Les données ont été recueillies par des entretiens semi-structurés et de la carte des réseaux réalisées avec 14 aidants familiaux de patients dans un hôpital d’oncologie du sud du Brésil, puis organisées et analysées selon la théorie ancrée. Les résultats montrent une prévalence des réseaux moyens et grands, ainsi que la prédominance des membres du réseau familial. L’aide et les services matériels et le soutien émotionnel sont les principales fonctions remplies par les membres du réseau. Abstract: This qualitative study characterizes the structure and functions of significant social networks in the anticipatory mourning process of family caregivers within palliative care. Data were collected by means of semi-structured interviews and the Map of Networks conducted with 14 family caregivers at an Oncology Hospital in southern Brazil, and then organized and analyzed by Grounded Theory using the ATLAS.ti 7.5 software. Results showed a prevalence of medium and large-sized networks, as well as the predominance of family network members. ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. Abstract The economic development of human activities contributes to the discharge of many anthropogenic pollutants. To assess the environmental risks in the Jaguaribe River, the most important river in the hydrographic region of the Eastern Northeast Atlantic, a bibliographic review of scientific articles and a series of ecotoxicological bioassays were conducted. The bioassays were conducted using sediment samples at six collection sites along the river, while the bibliographic review was used to identify the presence of anthropogenic contaminants in sediment and tissue samples of aquatic organisms within two km of each of the sediment collection sites. The bibliographic review showed the presence of thirty-eight anthropogenic pollutants in sediment samples and seven in tissue samples of aquatic organisms. The ecotoxicological bioassays showed that the sediment samples produced lethal and sublethal effects in the four tested representatives of the different trophic levels: Daphnia magna, Artemia salina, Allium cepa and Cucumis sativus. The presence of multiple anthropogenic pollutants in the Jaguaribe River and the observed lethal and sublethal effects in ecotoxicological bioassays suggest potential risks not only to the aquatic ecosystem but also to human health. Humans may be exposed to these contaminants through the consumption of water and aquatic organisms, leading to potential health issues such as increased cancer risk. The findings underscore the urgent need for regular monitoring and effective pollution control measures to mitigate these health risks and protect the well-being of local communities. Resumo Este estudo objetivou compreender o processo de comunicação de más notícias na perspectiva de residentes de medicina. Trata-se de uma pesquisa qualitativa, exploratória e descritiva, realizada com 12 residentes de medicina que estão se especializando em Clínica Médica em um hospital do sul do Brasil. Os dados foram coletados através de entrevista semiestruturada e analisados por meio de análise de conteúdo. Os resultados foram agrupados em quatro categorias: concepção e comunicação de más notícias, formação profissional, critérios utilizados para a comunicação de más notícias e desdobramentos do processo de comunicação. A transmissão de más notícias foi descrita como complexa, devido aos aspectos emocionais associados, exigindo preparação e competência do profissional. A formação médica, na perspectiva dos residentes, foi considerada insuficiente na capacitação para comunicar más notícias. Isso foi justificado pelo enfoque prioritário ao conhecimento técnico-científico no currículo, em detrimento dos aspectos subjetivos da relação médico-paciente-família. Diante dessa dificuldade, a racionalização foi identificada como a estratégia de enfrentamento psicológico comumente usada pelos entrevistados para a manutenção da objetividade e o controle dos próprios sentimentos. Uma formação humanística em medicina busca integrar os aspectos emocionais e relacionais aos conhecimentos biomédicos, o que se configura um investimento complexo e desafiador, mas eminentemente necessário.
Abstract This study aimed to understand the process of communicating bad news from the perspective of medical residents. This is a qualitative, exploratory and descriptive research carried out with 12 medical residents who are specializing in Medical Clinic in a hospital in the south of Brazil. Data were collected through a semi-structured interview and analyzed through content analysis. The results were grouped into four categories: conception and communication of bad news, professional training, criteria used to communicate bad news and unfolding of the communication process. The transmission of bad news was described as complex, due to the emotional aspects associated, requiring professional preparation and competence. Medical training, from the residents' perspective, was considered insufficient in the capacity to communicate bad news. This was justified by the priority focus on technical-scientific knowledge in the curriculum, to the detriment of the subjective aspects of the doctor-patient-family relationship. Faced with this difficulty, rationalization was identified as the psychological coping strategy commonly used by the interviewees, to maintain objectivity and control their own feelings. Humanistic training in medicine seeks to integrate the emotional and relational aspects of biomedical knowledge, which constitutes a complex and challenging investment, but eminently necessary.

RESUMO Objetivo: Traduzir e realizar a adaptação transcultural dos pontos âncoras da escala Cornell Assessment of Pediatric Delirium do inglês para a língua portuguesa do Brasil. Métodos: O processo de tradução e adaptação transcultural dos pontos âncoras seguiu todas as etapas recomendadas internacionalmente após a autorização de uso pela autora principal. As etapas foram: tradução da versão original para língua portuguesa por dois tradutores bilíngues nativos do idioma-alvo; síntese das versões; tradução reversa por dois tradutores nativos do idioma de origem; revisão e síntese da retradução; revisão por um comitê de juízes formado por especialistas e elaboração da versão final. Resultados: O processo de tradução e adaptação transcultural dos pontos âncoras seguiu as recomendações. As questões linguísticas e semânticas que surgiram foram discutidas pelo comitê de juízes, no qual se observou concordância de 91,8% pela escala de Likert com pequenas alterações de forma consensual. Após reanálise dos autores, não houve alterações, resultando na versão final, de fácil compreensão e administração. Conclusão: A tradução e a adaptação transcultural dos pontos âncoras da escala de Cornell Assessment of Pediatric Delirium para a língua portuguesa falada no Brasil foram bem-sucedidas com manutenção das propriedades linguísticas e semânticas do instrumento original. A tabela dos pontos âncoras mostrou ser de fácil compreensão e auxílio durante a avaliação das crianças abaixo de 24 meses por meio da escala de Cornell Assessment of Pediatric Delirium. ABSTRACT Objective: To translate and cross-culturally adapt the Cornell Assessment of Pediatric Delirium anchor points from English to Brazilian Portuguese. Methods: For the translation and cross-cultural adaptation of the anchor points, all steps recommended internationally were followed after authorization for use by the lead author. The stages were as follows: translation of the original version into Portuguese by two bilingual translators who were native speakers of the target language, synthesis of the versions, reverse translation by two translators who were native speakers of the source language, review and synthesis of the back-translation, review by a committee of experts and preparation of the final version. Results: The translation and cross-cultural adaptation of the anchor points was conducted in accordance with recommendations. The linguistic and semantic issues that arose were discussed by a committee of judges, with 91.8% agreement, as determined using a Likert scale, after changes by consensus. After reanalysis by the authors, there were no changes, resulting in the final version, which was easy to understand and administer. Conclusion: The translation and cross-cultural adaptation of the anchor points of the Cornell Assessment of Pediatric Delirium scale into Portuguese spoken in Brazil were successful, maintaining the linguistic and semantic properties of the original instrument. The table of anchor points is easy to understand and will be helpful during the assessment of children younger than 24 months using the Cornell Assessment of Pediatric Delirium scale. Resumo Histórico: O surgimento de cepas multirresistentes de enterobacteriaceae produtoras de NDM-1 tornou-se uma ameaça para pacientes hospitalizados, especialmente para os imunossuprimidos, como os receptores de transplante renal. NDM-1 é uma carbapenemase que torna as bactérias gram-negativas resistentes a muitos tipos de antibióticos. A incidência de infecção por enterobactérias produtoras de carbapenemas em receptores de transplante de órgãos sólidos é de cerca de 3 a 10%, com uma taxa de mortalidade de até 30%. Métodos: Apresentamos uma série de casos de 4 pacientes com enterobactérias produtoras de NDM-1 isoladas em culturas de urina ou esfregaços retais. Também realizamos um estudo transversal 30 dias após a identificação do paciente, coletando culturas de vigilância (esfregaço retal) de todos os pacientes internados para avaliar a extensão de disseminação deste mecanismo de resistência; foram incluídos um total de 101 pacientes. Resultados: Dois pacientes foram tratados adequadamente com culturas de controle negativo. Os outros dois pacientes não foram tratados porque eram assintomáticos e tiveram culturas de urina negativas subsequentes. Não foi identificada nenhuma nova colonização na triagem transversal, e não foram registrados novos casos de infecção urinária por NDM-1 após um acompanhamento de 4 anos. Conclusão: A vigilância de infecções causadas por cepas multirresistentes em hospitais que tratam pacientes imunossuprimidos deve ser continuada e devem ser tomadas medidas imediatas em casos de surtos desses tipos de infecções. Abstract Background: The emergence of multidrug-resistant NDM-1-producing enterobacteriaceae strains has become a threat to inpatients, especially to immunosuppressed ones, such as kidney transplant recipients. NDM-1 is a carbapenemase that makes gram-negative bacteria resistant to many types of antibiotics. The incidence of carbapenemase-producing enterobacteria infection in solid organ transplant recipients is around 3 to 10%, with a mortality rate of up to 30%. Methods: We present a case series of 4 patients with NDM-1-producing enterobacteria isolated in urine cultures or rectal swabs. We also conducted a cross-sectional study 30 days after patient identification, collecting surveillance cultures (rectal swab) from all inpatients to assess the extent of spread of this resistance mechanism; a total of 101 patients were included. Results: Two patients were adequately treated with negative control cultures. The other two patients were not treated because they were asymptomatic and had subsequent negative urine cultures. No new colonization was identified in the cross-sectional screening, and no new cases of urinary NDM-1 infection were recorded after a 4-year follow-up. Conclusion: Surveillance for infections caused by multidrug-resistant strains in hospitals treating immunosuppressed patients should be continued and prompt action should be taken in cases of outbreaks of multidrug-resistant infections. Objectives: To re-evaluate a sample of older adults enrolled in a randomized controlled trial of lithium for amnestic mild cognitive impairment (MCI) after 11 to 15 years, re-assessing their current (or last available) global cognitive and functional state. Methods: We recalled all former participants of the Lithium-MCI trial conducted by our group between 2009 and 2012 to perform a single-blinded, cross-sectional evaluation of their global clinical state to compare the long-term outcome of those who received lithium vs. those who received placebo. Results: Of the original sample (n=61), we were able to reach 36 participants (59% of retention), of whom 22 had previously received lithium (61% of the recall sample) and 14 (39%) had received placebo. Since 30.5% of the recalled sample was deceased, psychometric data were collected only for 69.5% of the participants. We found statistically significant differences in current mean Mini Mental State Examination score according to previous treatment group (25.5 [SD, 5.3] vs. 18.3 [SD, 10.9], p = 0.04). The lithium group also had better performance in the phonemic Verbal Fluency Test than the control group (34.4 [SD, 14.4] vs. 11.6 [SD, 10.10], p < 0.001). Differences in these measures also had large effect sizes, as shown by Cohen’s d values of 0.92 and 1.78, respectively. Conclusion: This data set suggests that older adults with amnestic MCI who had been treated with lithium during a previous randomized controlled trial had a better long-term global cognitive outcome than those from a matched sample who did not receive the intervention. Abstract Background Diabetes mellitus (DM) is a metabolic disorder characterized by persistent hyperglycemia. The main cause of death among diabetics is cardiovascular disease (CVD), which is a precursor to acute myocardial infarction (AMI). Approximately 30% of diabetic patients with AMI require myocardial revascularization surgery (MRS). Objective To evaluate the prevalence of MRS in diabetic and non-diabetic patients after AMI in a hospital in the city Criciúma, Brazil, during the period from 2016 to 2019. Methods A cross-sectional study was carried out with secondary data collection, with analysis of 215 medical records of diabetic and non-diabetic patients with AMI, submitted or not to MRS during the study period. For statistical analysis, the Shapiro-Wilk test, Pearson’s chi-square test and Fisher’s exact test were used, with a significance level of α = 0.05. Results The frequency of diabetic AMI patients with hypertension (88.1%) was significantly higher (p<0.01) than of diabetic AMI patients without hypertension. Although no statistically significant differences were found in the other variables between the groups, smoking, hypertension and dyslipidemia were more prevalent in diabetic than in non-diabetic patients with AMI undergoing MRS. ABSTRACT The effects of diets containing oilseeds were measured to evaluate the productive and economic parameters in the finishing of young, feedlot Nellore bulls. Twenty-four young Nellore bulls were used, with an initial body weight (BW) of 311.46±0.37 kg and 24 months of age, distributed into individual stalls ( 4 × 20 m) in a completely randomized design, totaling four treatments with six repetitions per treatment. Four diets (control, based on corn and soybean meal, and three diets containing cottonseed, soybean, and sunflower) were evaluated. Feed and orts were measured daily to calculate intake and costs. The dry matter intake of the control group was higher than soybean (10.64 kg/day), cotton (9.88 kg/day), and sunflower (9.30 kg/day) treatments, respectively. The cottonseed treatment showed the highest average neutral detergent fiber intake. There was a dietary effect of diets on average daily gain, total weight gain, and final weight. The soybean treatment showed the highest performance, total gain (232.55 kg), and final weight (544.38 kg). Oilseed intake can modify the fatty acids profile in the meat, decreasing its saturated fatty acid content. Whole soybean seed favors performance, improves feed efficiency, fatty acid profile, and fat distribution in the carcass, and can reduce production costs. RESUMO Objetivo: Avaliar sinais radiológicos (gestacionais e perinatais) e neonatais de paciente com síndrome de Patau e holoprosencefalia semilobar, assim como relatar a associação de ambas as patologias. Descrição do caso: Trata-se de um relato de recém-nascido do sexo feminino a termo, que apresentou trissomia do cromossomo 13 e holoprosencefalia semilobar, com fusão talâmica e ventrículo cerebral único, além de várias outras alterações que pioraram o prognóstico da paciente. Comentários: A trissomia do cromossomo 13 é um defeito genético que caracteriza um conjunto de sintomas que compõem a Síndrome de Patau. Nesta síndrome, é comum o acometimento cardiovascular, urogenital, do sistema nervoso central, da estrutura facial e da capacidade intelectual, além de falhas na formação dos membros, como diminuição no comprimento do úmero, fêmur, polidactilia, hipotelorismo e baixa implantação das orelhas. Estima-se, no entanto, que a holoprosencefalia apresente-se nesse grupo de malformações congênitas apenas em 24 a 45% dos casos. ABSTRACT Objective: To evaluate radiological (gestational and perinatal) and neonatal signs of patients with Patau syndrome and semilobar holoprosencephaly, as well as to report the association of both pathologies. Case description: This case report is about a female infant, born at term with trisomy of the chromosome 13 and semilobar holoprosencephaly, with thalamic fusion and a single cerebral ventricle, in addition to several other changes that worsened the patient's prognosis. Comments: Chromosome 13 trisomy is a genetic alteration that leads to the symptoms that determines Patau syndrome. In this syndrome, cardiovascular, urogenital, central nervous system, facial structure and intellectual impairment are common, in addition to problems in limb formation, such as decreased humerus and femur length, polydactyly, hypotelorism and low ear implantation. It is estimated, however, that holoprosencephaly is present in only 24 to 45% of the patients with trisomy 13.
