Abstract Introduction In clinical practice, patients with the same degree and configuration of hearing loss, or even with normal audiometric thresholds, present substantially different performances in terms of speech perception. This probably happens because other factors, in addition to auditory sensitivity, interfere with speech perception. Thus, studies are needed to investigate the performance of listeners in unfavorable listening conditions to identify the processes that interfere in the speech perception of these subjects. Objective To verify the influence of age, temporal processing, and working memory on speech recognition in noise. Methods Thirty-eight adult and elderly individuals with normal hearing thresholds participated in the study. Participants were divided into two groups: The adult group (G1), composed of 10 individuals aged 21 to 33 years, and the elderly group (G2), with 28 participants aged 60 to 81 years. They underwent audiological assessment with the Portuguese Sentence List Test, Gaps-in-Noise test, Digit Span Memory test, Running Span Task, Corsi Block-Tapping test, and Visual Pattern test. Results The Running Span Task score proved to be a statistically significant predictor of the listening-in-noise variable. This result showed that the difference in performance between groups G1 and G2 in relation to listening in noise is due not only to aging, but also to changes in working memory. RESUMO Objetivo Avaliar a audibilidade de fala em crianças usuárias de próteses auditivas e correlacionar o Índice de Inteligibilidade de Fala à detecção de fonemas. Método 22 crianças e adolescentes usuários de próteses auditivas passaram por avaliação audiológica básica, verificação in situ (e consequente obtenção do Índice de Inteligibilidade de Fala - SII - para condições com e sem próteses auditivas) e pesquisa dos limiares de detecção para fonemas por meio do teste Ling-6(HL). Resultados O SII médio foi 25,1 sem próteses auditivas e 68,9 com amplificação (p <0,001*). Os limiares de detecção de fonemas em campo livre, em dBNA, foram, sem amplificação /m/=29,9, /u/=29,5, /a/=35,5, /i/=30,8, /∫/=44,2 e /s/=44,9, e com amplificação /m/=13,0, /u/=11,5 /a/=14,3, /i/=15,4, /∫/=20,4 e /s/=23,1 (p <0,001*). Houve correlação negativa entre SII e os limiares de todos os fonemas na condição sem próteses (p≤0,001*) e entre SII e o limiar do /s/ com próteses (p = 0,036*). Conclusão Os limiares de detecção de todos os fonemas são menores do que na condição sem próteses. Há correlação negativa entre SII e os limiares de todos os fonemas na situação sem próteses e entre SII e o limiar de detecção do fonema /s/ na situação com próteses auditivas.

ABSTRACT Purpose To evaluate speech audibility in schoolchildren hearing aids users and correlate the Speech Intelligibility Index to phonemes detecion. Methods 22 children and adolescents hearing aids users, underwent audiological evaluation, in situ verification (and consequent obtaining the Speech Intelligibility Index - SII - for conditions with and without hearing aids) and detection thresholds for phonemes by Ling-6 (HL) test. Results The average value for the SII was 25.1 without hearing aids and 68.9 with amplification (p <0.001 *). The phoneme detection thresholds in free field, in dBHL, were, without amplification /m/ = 29.9, /u/ = 29.5, /a/ = 35.5, /i/ = 30.8, /∫/ = 44.2 e /s/ = 44.9, and with amplification /m/ = 13.0, /u/ = 11.5 /a/ = 14.3, /i/ = 15.4, /∫/ = 20.4 e /s/ = 23.1 (p<0.001*). There was a negative correlation between SII and the thresholds of all phonemes in the condition without hearing aids (p≤0.001*) and between SII and the /s/ threshold with hearing aids (p = 0.036*). Conclusion The detection thresholds for all phonemes are lower than without hearing aids. There is a negative correlation between SII and the thresholds of all phonemes in the situation without hearing aids and between SII and the detection threshold of the phoneme / s / in the situation with hearing aids.

Resumo A miopatia mitocondrial é causada pela ausência e/ou insuficiência de uma enzina quaternária, L-carnitina, responsável por transportar ácidos graxos livres para a parte interna da mitocôndria. A função da mitocôndria é produzir energia, contribuindo para o bom funcionamento das células. A Lipidose Muscular é uma doença que provoca anomalias em enzimas que metabolizam gordura e por consequência causa acúmulo de toxinas de subprodutos com gordura nos tecidos. O objetivo deste trabalho é apresentar o estudo de caso da paciente B.D., 37 anos, diagnosticada com Lipidose Muscular aos seis anos, com deficiência de L-Carnitina e relatar o acompanhamento fonoaudiológico realizado em um serviço de saúde auditiva. A abertura de prontuário da paciente foi realizada em 05/03/1989. Foi prescrito pelo neurologista o uso contínuo de 2g/dia de L-carnitina. A mãe relatou que B.D. apresentava dificuldades em ouvir, pois era muito desatenta, o que foi mais evidente quando começou a frequentar a escola. Em 1988, a paciente foi diagnosticada com perda auditiva neurossensorial de grau moderado bilateral e começou a fazer uso de aparelhos de amplificação sonora individual retroauriculares em 1989. O desempenho escolar e comunicação melhoraram. Em 1998, passou a utilizar aparelhos tipo micro canal, o que a favoreceu esteticamente, promoveu melhora da localização sonora e maior ganho em altas frequências. Os limiares de audibilidade apresentaram uma leve piora e a paciente atualmente é pós-graduada e trabalha em uma grande instituição financeira. Conclui-se que o diagnostico neurológico e a intervenção fonoaudiológica precoces possibilitaram o adequado desenvolvimento de linguagem da paciente.

Abstract Mitochondrial myopathy is caused by the absence and/or insufficiency of L-carnitine, a quaternary enzyme responsible for transporting free fatty acids into the mitochondria. The primary function of the mitochondria is to produce energy, contributing to proper cell functioning. Muscular lipidosis causes abnormalities in enzymes that metabolize fat, resulting in the accumulation of harmful amounts of fats in tissues. The aim of this study was to present the case study of patient B.D., a 37-year-old woman diagnosed with muscular lipidosis with L-carnitine deficiency at 6 years old, and describe the speech-language follow-up performed at a hearing care clinic. The first entry in the patient’s medical chart was on 03/05/1989, with continuous use of 2g/day of L-carnitine prescribed by a neurologist. The mother reported that B.D. had difficulty hearing and was inattentive, which became more evident when she started school. In 1988 the patient was diagnosed with moderate bilateral sensorineural hearing loss and began using behind-the-ear (BTE) hearing aids in 1989, after which her academic performance and communication improved. In 1998 she switched to Completely in Canal (CIC) hearing aids, which are more discreet, provided better sound localization and greater high frequency gain, although her hearing thresholds worsened slightly. She completed her graduate studies and currently works at a large financial institution. It was concluded that early neurological diagnosis and speech-language intervention enabled adequate language development in the patient.

RESUMO Objetivo Verificar o benefício obtido pela prescrição de ganho acústico baseada nos limiares audiométricos obtidos com tons puros (Warble) e com ruído de faixa estreita (NB). Método Amostra de 30 idosos, com idade igual ou superior a 60 anos, perda auditiva neurossensorial de grau moderado a severo simétrica bilateral de configuração descendente com limiares em 4kHz iguais ou inferiores a 70dBNA. Foram dois grupos. GTP (grupo tom puro): 15 idosos tiveram as próteses auditivas adaptadas com emprego dos limiares obtidos com tom puro e grupo GNB (grupo Narrowband): 15 idosos tiveram as próteses auditivas adaptadas por meio dos limiares obtidos com NB. Os procedimentos realizados antes da adaptação de próteses auditivas e após três meses de uso de amplificação foram: Escala COSI, IPRF (Índice Percentual de Reconhecimento de fala), Relação Sinal/Ruído e análise do tempo de uso do AASI. O Questionário Internacional de Aparelho de Amplificação Sonora Individual (QI-AASI) foi aplicado após três meses. Resultados Os idosos do grupo em que as próteses auditivas foram adaptadas com ganho prescrito com base nos limiares auditivos obtidos com o estímulo Narrow Band apresentaram melhor desempenho nos seguintes testes: IPRF à orelha direita, pontuação total do questionário QI-AASI, escala COSI e maior tempo de uso do AASI em comparação ao grupo GTP. Conclusão Observou-se maior benefício com o uso de próteses auditivas, pela pontuação total do questionário QI-AASI, escala COSI e maior tempo de uso do AASI, no grupo cuja prescrição do ganho acústico baseou-se nos limiares audiométricos obtidos com o Narrowband.

ABSTRACT Purpose To verify the benefit obtained by the prescription of acoustic gain based on the auditory thresholds obtained with pure tones modulated in frequency and with Narrow Band Noise. Methods The sample consisted of 30 elderly people, aged 60 years or over with moderate to severe descending sensorineural symmetrical hearing loss with thresholds at 4kHz equal to or less than 70dBHL. There were two groups. GTP (pure tone group): 15 elderly people had their hearing aids fitted through the auditory thresholds obtained with pure tone and the GNB group (narrow band group): 15 elderly people had their hearing aids fitted through the auditory thresholds obtained with NB. The procedures performed before the fitting of hearing aids and after three months of amplification use were: COSI, WRS (Word Recognition Score), Signal/Noise ratio. The International Outcome Inventory for Hearing Aids (IOI-HA) was applied only after three months of hearing aid fitting. Results The elderly people in the group in which the hearing aids were fitted with a prescribed gain based on the hearing thresholds obtained with the Narrow Band stimulus showed better performance in the following tests: WRS on the right ear, total score of the IOI-HA inventory, COSI and longer use of hearing aids compared to the GTP group. Conclusion There was a greater benefit with the use of hearing aids, due to the total score of the IOI-HA inventory, COSI scale and longer daily use time of hearing aids, in the group whose prescription of acoustic gain was based on the auditory thresholds obtained with narrow band.

Resumo Introdução: Os potenciais evocados auditivos de tronco encefálico para sons complexos, como, por exemplo, sons de fala, investigam a representação neural desses sons em níveis subcorticais e refletem com fidelidade as características do estímulo. No entanto, existem ainda poucos estudos que usam esse tipo de estímulo e para que possa ser usado na prática clínica é necessário estabelecer padrões de normalidade por meio de estudos em diferentes populações. Objetivo: Analisar as latências e as amplitudes das ondas obtidas nos traçados dos potenciais evocados auditivos de tronco encefálico por estímulo de fala em neonatos brasileiros com audição normal e sem risco auditivo. Método: Foram avaliados 21 neonatos com média de 9 dias, sem risco auditivo segundo o Joint Committe on Infant Hearing e com resultado normal para triagem auditiva neonatal. Fizeram-se potenciais evocados auditivos por estímulo de fala (sílaba /da/) na intensidade de 80 dBNA e analisaram-se a latência e a amplitude das ondas obtidas. Resultados: Na porção transiente observaram-se 100% de ocorrência de resposta para todas as ondas analisáveis (Onda I, Onda III, Onda V e Onda A) e esse conjunto de ondas apresentou latência inferior a 10 ms. Na porção sustentada a frequência de ocorrência da Onda B foi de 53,12%; da Onda C 75%; da Onda D 90,62%; da Onda E 96,87%; da Onda F 87,5% e da Onda O 87,5% e a latência observada dessas ondas variou de 11,51 ms a 52,16 ms. Observou-se maior similaridade nas latências das respostas e maior variação da amplitude no grupo estudado. Conclusões: Embora a morfologia das ondas obtidas para os potenciais evocados de tronco encefálico para o estímulo de fala em neonatos seja bastante semelhante às dos adultos, observou-se maior latência e maior variação da amplitude das ondas analisáveis.

Abstract Introduction: Brainstem auditory evoked potentials in response to complex sounds, such as speech sounds, investigate the neural representation of these sounds at subcortical levels, and faithfully reflect the stimulus characteristics. However, there are few studies that utilize this type of stimulus; for it to be used in clinical practice it is necessary to establish standards of normality through studies performed in different populations. Objective: To analyze the latencies and amplitudes of the waves obtained from the tracings of brainstem auditory evoked potentials using speech stimuli in Brazilian neonates with normal hearing and without auditory risk factors. Methods: 21 neonates with a mean age of 9 days without risk of hearing loss and with normal results at the neonatal hearing screening were evaluated according to the Joint Committee on Infant Hearing protocols. Auditory evoked potentials were performed with speech stimuli (/da/ syllable) at the intensity of 80 dBNA and the latency and amplitude of the waves obtained were analyzed. Results: In the transient portion, we observed a 100% response rate for all analyzable waves (Wave I, Wave III, Wave V and Wave A), and these waves exhibited a latency <10 ms. In the sustained portion, Wave B was identified in 53.12% of subjects; Wave C in 75%; Wave D in 90.62%; Wave E in 96.87%; Wave F in 87.5% and Wave O was identified in 87.5% of subjects. The observed latency of these waves ranged from 11.51 ms to 52.16 ms. Greater similarity was observed for the response latencies, as well as greater amplitude variation in the studied group. Conclusions: Although the wave morphology obtained for brainstem evoked potentials with speech stimulation in neonates is quite similar to that of adults, a longer latency and greater variation in amplitude were observed in the waves analyzed.

RESUMO Objetivo Verificar o efeito da cognição no benefício obtido com o uso de próteses auditivas e na qualidade de vida de idosos com perda auditiva. Método Foram avaliados 17 idosos com perda auditiva neurossensorial de grau moderado bilateral. Os idosos foram distribuídos em dois grupos segundo os resultados da triagem cognitiva 10-CS: G1- sete idosos sem alteração e G2- dez idosos com resultado sugestivo de alteração cognitiva. Protocolo de estudo: questionário de avaliação de restrição de participação - HHIE, escala de depressão geriátrica – EDG, questionário de qualidade de vida – SF36 e avaliação do esforço de escuta por meio da escala visual analógica. A seguir, os idosos receberam as próteses auditivas. Após três meses de uso efetivo da amplificação, o protocolo foi reaplicado com a inclusão do Questionário Internacional – QI-AASI. Resultados Caracterizou-se a amostra quanto à idade, escolaridade, Critério de Classificação Econômica Brasil - ABEP, grau da perda, reconhecimento de fala e índice de Inteligibilidade de fala (SII) da orelha com melhor audibilidade. O estudo estatístico revelou diferença somente com relação à idade. O estudo comparativo antes e após intervenção revelou melhora significante nos escores do 10-CS no grupo G2, na restrição de participação, esforço de escuta e alguns domínios do questionário de qualidade de vida. Não houve diferença no QI-AASI entre grupos segundo Fator 1, Fator 2 e Escore Total. Conclusão Houve melhora da qualidade de vida após três meses de uso de amplificação. Não houve efeito da cognição no benefício obtido com o uso de próteses auditivas.

ABSTRACT Purpose To verify the effects of cognition on the benefit obtained with the use of hearing aids and on the quality of life of elderly people with hearing loss. Methods 17 elderly people with moderate sensorineural hearing loss (SNHL) bilateral were evaluated. The elderly people were divided into two groups according to the results of 10-point cognitive screening 10-CS: G1- seven elderly people without change and G2-10 elderly people with a suggestive result of cognitive alteration. Study protocol: self-evaluation questionnaire – (Hearing Handicap Inventory for the Elderly (HHIE)), geriatric depression scale (GDS), quality of life questionnaire – (Short-Form 36 (SF36)) and evaluation of the listening effort through the Visual Analogue Scale (VAS). After the evaluation, the elderly people received hearing aids. After three months of effective amplification, the protocol was reapplied with the inclusion of the International Questionnaire for the Evaluation of Hearing Aids – (IOI-HA). Results The sample was characterized according to age, schooling, Critério de Classificação Econômico Brasil Associação Brasileira das Empresas de Pesquisa (ABEP), degree of loss, Índice Percentual de Reconhecimento da Fala (IPRF) and Speech Intelligibility Index (SII) of the ear with better audibility. The statistical study revealed that there was significant difference only in relation to age. The comparative study before and after the intervention revealed a significant improvement in the 10-CS scores in the group G2, in the participation restriction - HHIE, in the listening effort and in some areas of the SF36. There was no difference in QI-AASI between groups second the factor 1, factor 2 and Total Score. Conclusion There was improvement of the quality of life after three months of amplification use. There was no effect of cognition on the benefit obtained with the use of hearing aids.

RESUMO Objetivo Comparar o Índice de Inteligibilidade de Fala e o Índice Percentual de Reconhecimento de Fala (IPRF) obtidos pré e pós-ajustes de acordo com valores prescritos, investigar correlações entre o Índice de Inteligibilidade de Fala pré e pós-ajustes e investigar correlações entre o IPRF pré e pós-ajustes em usuários de próteses auditivas intra-aurais. Métodos Participaram 20 adultos de 18 a 59 anos, com perda auditiva neurossensorial bilateral de graus moderado e severo. Foram comparados o Índice de Inteligibilidade de Fala e o IPRF obtidos com próteses auditivas, em dois momentos: com os ajustes até então utilizados (momento pré) e após a regulagem de acordo com os valores prescritos em verificação, com emprego do método NAL-NL1 (momento pós). Os dados foram analisados por meio de estatística descritiva e de testes não paramétricos, com nível de significância de 0,05. Resultados Verificou-se correlação negativa entre o IPRF pré-ajuste e seu Delta pós-pré-ajuste, assim como entre o Índice de Inteligibilidade de Fala pré-ajuste e seu Delta pós-pré-ajuste. Houve correlação positiva entre o IPRF e o Índice de Inteligibilidade de Fala pós-ajuste. Conclusão Quanto menores o IPRF e o Índice de Inteligibilidade de Fala pré-ajuste, maiores suas diferenças, comparando os momentos pré e pós-regulagem. Quanto maior o acesso aos sons de fala, promovido pela regulagem ideal das próteses auditivas, maior o IPRF.

ABSTRACT Purpose To compare the Speech Intelligibility Index (SII) and the Word Recognition Score (WRS) in quiet obtained pre and post adjustments according to prescribed values, to investigate correlations between the Speech Intelligibility Index before and after adjustments, and to investigate correlations between pre- and post-adjustment SII in intra-aural hearing aids users. Methods 20 adults participated, aged 18 to 59 years, with moderate or severe bilateral sensorineural hearing loss. We compared the Speech Intelligibility Index and the WRS obtained with hearing aids in two moments: with the adjustments previously used (pre-adjustment moment) and after modification according to the values prescribed in verification using the NAL-NL1 method (post-adjustment moment). The data were analyzed through descriptive statistics and nonparametric tests, with a significance level of 0.05. Results There was a negative correlation between the pre-adjustment WRS and its post-pre adjustment difference, as well as between the pre-adjustment Speech Intelligibility Index and its post-pre adjustment Delta. There was a positive correlation between post-adjustment PISR and the SII. Conclusion The lower the pre-adjustment WRS and Speech Intelligibility Index, the greater their differences comparing the pre- and post-adjustment moments. The greater the access to speech sounds, promoted by the ideal regulation of the hearing aids, the higher the WRS.

RESUMO Introdução: A deficiência auditiva pode exacerbar mudanças cognitivas decorrentes do envelhecimento. Objetivo: Estudar a restrição de participação em atividades diárias e processos cognitivos em idosos, novos usuários de próteses auditivas. Métodos: Foram avaliados 50 idosos com perda auditiva neurossensorial de grau leve a moderadamente severo, novos usuários de amplificação sonora, distribuídos em três grupos, segundo o grau da perda auditiva. Foram aplicados o Hearing Handicap Inventory For Elderly e o Mini Exame do Estado Mental, antes e após 12 a 16 semanas de uso das próteses auditivas. O tratamento estatístico foi realizado por meio da análise de variância e comparações múltiplas de Bonferroni, com nível de significância de 0,05. Resultados: Após a estimulação acústica por meio do uso de próteses auditivas por 12 a 16 semanas, houve redução da restrição de participação em atividades diárias, tanto na subescala emocional, quanto na social/situacional do Hearing Handicap Inventory For Elderly. Idosos com perda de grau moderadamente severo apresentaram maiores restrições de participação na subescala social e no escore total do Hearing Handicap Inventory For Elderly. O Mini Exame do Estado Mental revelou maiores escores total e dos domínios orientação, memória imediata, atenção e cálculo, evocação e linguagem, após o uso das próteses auditivas. Quanto ao gênero, idosos do gênero feminino apresentaram média dos escores menores em atenção e cálculo. Conclusão: Os idosos com deficiência auditiva apresentaram redução da autopercepção das restrições de participação e melhora dos processos cognitivos de orientação, memória imediata, atenção e cálculo, evocação e linguagem, com a estimulação acústica.

ABSTRACT Introduction: Hearing impairment may accentuate cognitive decline caused by ageing. Purpose: To study restriction of participation in daily activities and cognitive processes in new elderly hearing aids users. Methods: Fifty elderly individuals, all new users of amplification, with mild to moderately severe post-lingual symmetrical sensorineural hearing loss were evaluated. They were then divided into three groups according to the degree of hearing loss. The Hearing Handicap Inventory for the Elderly questionnaire and the Mini Mental State Examination were applied pre and post-fitting of the hearing aids (after 12 to 16 weeks). The analysis of variance and Bonferroni multiple comparisons with significance level of 0.05 were used as statistical analyses. Results: After acoustical stimulation through the use of hearing aids for 12 to 16 weeks, there was a reduction in the restriction of participation in daily activities both in the emotional and social/situational scales of the Hearing Handicap Inventory for the Elderly. Elderly individuals with moderately severe loss reported higher hearing handicap at the social subscale and the global score. The Mini Mental State Examination analysis revealed higher scores for the Orientation to Time and Place, Repetition/Registration, Attention and Calculation, Recall, and Language domains at the post-fitting evaluation. Regarding gender, females presented lower mean scores in Attention and Calculation. Conclusion: Elderly individuals were benefited by the use of hearing aids, which reduced self-perception of hearing handicap and improved cognition aspects of Orientation to Time and Place, Repetition/Registration, Attention and Calculation, Recall, and Language.

RESUMO Objetivo Investigar os efeitos dos processos cognitivos e do Índice de Inteligibilidade de Fala no reconhecimento de fala no ruído em idosos, com e sem alteração cognitiva, usuários de próteses auditivas. Métodos 34 idosos, de 64 a 87 anos, com perda auditiva neurossensorial simétrica de grau moderado, usuários de próteses auditivas, foram distribuídos em grupos de idosos sem (GA; n=21) e com (GB; n=13) evidências de alteração cognitiva. A fim de garantir que o ajuste das próteses auditivas estivesse adequado, realizou-se o mapeamento visível de fala amplificada e foram obtidos os indices de ínteligibilidade de fala. Os idosos foram submetidos a uma triagem cognitiva (10-CS) e ao teste Lista de Sentenças em Português. A avaliação constou da pesquisa do limiar de reconhecimento de sentenças no ruído. Esta pesquisa foi realizada em campo livre, na condição sem e com próteses auditivas. Para análise estatística, foram utilizados os testes de Qui-Quadrado e Mann-Whitney. O nível de significância adotado foi de 0,05. Resultados Não houve diferença significativa entre os índices de inteligibilidade de fala obtidos em ambos os grupos, tanto na condição com próteses auditivas, como na condição sem as próteses. Verificou-se que os idosos com e sem alteração cognitiva apresentaram o mesmo acesso aos sons da fala (SII), nas duas condições. Observou-se que os idosos sem alteração cognitiva apresentaram menor relação sinal/ruído média, para o reconhecimento de 50% das sentenças na presença de ruído tanto na condição sem próteses auditivas como na condição com próteses, do que aqueles com alteração cognitiva. Conclusão Idosos com melhor cognição apresentaram melhor reconhecimento de fala em condições de escuta difícil.

ABSTRACT Purpose Investigate the effects of cognitive processes and Speech Intelligibility Index (SII) in sentence recognition in noise among elderly users of hearing aids with and without cognitive disorders. Methods Study participants were 34 older adults, aged 64-87 years, with bilateral moderate sensorineural hearing loss, users of hearing aids for over a year. The individuals were divided into two groups: GA - without cognitive impairment (n=21) and GB - with cognitive impairment (n=13). In order to verify the adequate amplification of the hearing aids, participants of both groups underwent Visible Speech Mapping and had their Speech Intelligibility Indexes obtained. Subsequently, they were submitted to cognitive screening (10-CS) and the Brazilian Portuguese Sentence List Test. Evaluation consisted of a search for speech perception thresholds in noise (S/N ratio). This search was performed in free-field conditions first without and then with the hearing aids. Data were statistically analyzed at a significance level of 5% using the Chi-squared and Mann-Whitney tests. Results No statistically significant differences were observed between the SII results obtained in both groups either with or without hearing aids. These results demonstrate that the older individuals in the GA and GB present the same access to speech sounds with and without hearing aids. Individuals in the GA presented lower S/N ratio under both conditions. Conclusion Older individuals with better cognition present greater speech recognition in the presence of competitive noise.

RESUMO Objetivo Avaliar a influência das variáveis idade, escolaridade e gênero na ocorrência de pausas após o tempo padrão no teste de identificação de sentenças dicóticas. Método A amostra foi constituída por 200 indivíduos destros distribuídos em quatro grupos segundo a faixa etária: grupo I - 13 a 19 anos de idade, grupo II - 20 a 29 anos de idade, grupo III - 30 a 39 anos de idade e o grupo IV - 40 a 49 anos de idade. Cada grupo continha 50 sujeitos (25 homens e 25 mulheres) pareados por escolaridade. Foram adotados os seguintes critérios de elegibilidade: língua materna português brasileiro, normouvinte, leitura fluente independentemente do grau de escolaridade. O teste de identificação de sentenças dicóticas foi aplicado nas etapas de integração binaural e escuta direcionada e a necessidade de pausas no teste após o tempo padrão foi anotada. Foram realizadas estatísticas descritivas e inferenciais. Resultados Para as etapas de integração binaural, houve associação positiva entre idade e ocorrência de pausas. A variável escolaridade apresentou associação negativa com a ocorrência de pausas em todas as etapas do teste. A variável gênero não apresentou nenhuma associação com a ocorrência de pausa em nenhuma das etapas do teste. Conclusão Com o aumento da idade, há um aumento na incidência de pausas na etapa de integração binaural do teste. Quanto mais anos de estudo menor a chance de o indivíduo necessitar de pausas para realizar o teste em todas as etapas de apresentação. A variável gênero não influenciou na ocorrência de pausas.

ABSTRACT Purpose To evaluate the influence of variables age, educational status and sex in the occurrence of pauses after the standard time in the dichotic sentence identification test. Methods This investigation included 200 right-handed subjects divided into four groups according to age: group I - from 13 to 19 years old, group II - from 20 to 29 years old, group III - from 30 to 39 years old and group IV - from 40 to 49 years old. Each group contained 50 subjects (25 men and 25 women) matched by educational level. The following eligibility criteria was adopted: Brazilian Portuguese mother language, listeners, and fluent readers independent of the educational level. It was applied the dichotic sentence identification test in the steps of binaural integration, directed listening, and it was noted the need for pauses in the test after the standard time. The descriptive and inferential statistics were performed. Results For the binaural integration stages there was a positive association between age and pause occurrence. The educational variable presented a negative association with the occurrence of pauses in all stages of the DSI test. The gender variable showed no association with the occurrence of pause in any of the test steps. Conclusion With increasing age, there is an increase in the incidence of pauses in the binaural integration stages of the test. The more years of study the less chance that the individual will need breaks to perform the test at all stages of presentation. The variable gender did not influence the occurrence of pauses.

Disseminated histoplasmosis is an endemic systemic mycosis, caused by the dimorphic fungus Histoplasma capsulatum. The clinical manifestations depend on the inhaled fungal load, the immunological state of the patient and the virulence of the infecting fungal strain. The disseminated variant is most frequent in immunocompromised patients, with 90% of cases in patients with positive serology for HIV, In this series, 37 patients with diagnosis of HIV and disseminated histoplasmosis were evaluated. The 43.2% of the patients did not know their serology prior to the consultation due to histoplasmosis. The main clinical causes were impregnation syndrome (67.6%), fever (64.9%), respiratory symptoms (62.2%) and mucocutaneous manifestations (67.6%), among others. The 94.6% of the patients presented anemia, 75.7% had bicytopenia and 32.4% pancytopenia. The sensitivity of the blood cultures was 70.4%. The 86.5% of the patients did not have another opportunistic infection concomitant at the time of diagnosis. In 83.8% of patients, disseminated histoplasmosis was the first AIDS disease. A mortality of 14.7% was recorded.

Resumen La histoplasmosis diseminada es una micosis sistémica endémica, causada por el hongo dimórfico Histoplasma capsulatum. Las manifestaciones clínicas dependen de la carga fúngica inhalada, del estado inmunológico del paciente y de la virulencia de la cepa infectante. La variante diseminada es más frecuente en inmunocomprometidos, registrándose 90% de los casos en pacientes con serología positiva para VIH. En esta serie se evaluaron 37 pacientes con diagnóstico de infección por VIH e histoplasmosis diseminada. El 43,2% de los pacientes no sabía su serología en forma previa a la consulta por histoplasmosis. Las principales manifestaciones clínicas fueron síndrome de impregnación (67,6%), fiebre (64,9%), síntomas respiratorios (62,2%) y manifestaciones muco-cutáneas (67,6%), entre otras. El 94,6% de los pacientes presentó anemia, 75,7% tenía bicitopenia y 32,4% pancitopenia. La sensibilidad de los hemocultivos fue de 70,4%. El 86,5% no tuvo otra infección oportunista concomitante al momento del diagnóstico. En 83,8% de los pacientes la histoplasmosis diseminada fue la primera enfermedad marcadora de SIDA. Se registró una mortalidad de 14,7%.

Resumo Introdução: A perda auditiva pode influenciar negativamente o desempenho comunicativo e estes indivíduos devem ser avaliados com material adequado e em situações de escuta próximas às observadas no cotidiano. Objetivo: Analisar e comparar o desempenho de indivíduos com perda auditiva neurossensorial de grau leve a moderado em testes de reconhecimento de fala apresentados no silêncio e no ruído segundo as variáveis orelha e tipos de apresentação do estímulo. Método: Participaram do estudo 19 indivíduos destros com perda auditiva neurossensorial bilateral simétrica de grau leve a moderado, submetidos ao teste de reconhecimento de fala com palavras em diferentes modalidades e ao teste de fala com ruído branco com figuras. Resultados: Não houve diferença significante entre as orelhas direita e esquerda para nenhum dos testes realizados. A média de acertos no teste de reconhecimento de fala com figuras, viva voz e monossílabos gravados foi 97,1%; 85,9% e 76,1%, respectivamente, e 72,6% de acertos no teste com ruído. Conclusões: O melhor desempenho no Índice Percentual de Reconhecimento de Fala foi obtido utilizando como estímulos monossílabos representados por figuras apresentados no silêncio, sem diferenças significantes entre as orelhas direita e esquerda. Com a introdução do ruído competitivo, houve descréscimo no desempenho dos indivíduos.

ABSTRACT INTRODUCTION: Hearing loss can negatively influence the communication performance of individuals, who should be evaluated with suitable material and in situations of listening close to those found in everyday life. OBJECTIVE: To analyze and compare the performance of patients with mild-to-moderate sensorineural hearing loss in speech recognition tests carried out in silence and with noise, according to the variables ear (right and left) and type of stimulus presentation. METHODS: The study included 19 right-handed individuals with mild-to-moderate symmetrical bilateral sensorineural hearing loss, submitted to the speech recognition test with words in different modalities and speech test with white noise and pictures. RESULTS: There was no significant difference between right and left ears in any of the tests. The mean number of correct responses in the speech recognition test with pictures, live voice, and recorded monosyllables was 97.1%, 85.9%, and 76.1%, respectively, whereas after the introduction of noise, the performance decreased to 72.6% accuracy. CONCLUSIONS: The best performances in the Speech Recognition Percentage Index were obtained using monosyllabic stimuli, represented by pictures presented in silence, with no significant differences between the right and left ears. After the introduction of competitive noise, there was a decrease in individuals' performance.

Resumo Objetivo: Verificar o efeito do uso de próteses auditivas na autopercepção do zumbido e da perda auditiva em idosos. Método: Foram avaliados 24 idosos com perda auditiva neurossensorial de grau moderado e idades entre 60 e 70 anos divididos em dois grupos conforme a presença ou não de zumbido. Todos os voluntários foram adaptados binauralmente com próteses auditivas micro canais de mesmo fabricante e modelo e submetidos a testes, escalas e questionários pertinentes ao tema. As avaliações ocorreram pré, e após um e três meses de uso efetivo das próteses auditivas. Resultados: A estimulação acústica por meio do uso efetivo de próteses auditivas propiciou redução na autopercepção da intensidade sonora do zumbido (principalmente nas avaliações com as próteses) e no desconforto com este sintoma e com a perda auditiva. Além disso, todos os participantes mostraram-se satisfeitos com o uso dos dispositivos de amplificação. Conclusão: O uso contínuo das próteses auditivas é benéfico ao tratamento do zumbido e da perda auditiva, além de implicar na satisfação dos usuários.

ABSTRACT OBJECTIVE: To determine the effect of the use of hearing aids in self-perception of tinnitus and hearing loss in the elderly. METHODS: A total of 24 elderly patients between 60 and 70 years of age with moderate-grade sensorineural hearing loss were evaluated and divided into two groups according to the presence or absence of tinnitus. All volunteers were fitted with binaural micro-channel hearing aids of the same brand and model and submitted to tests, scales, and questionnaires relevant to this topic. The evaluations were performed before and after one and three months of effective use of these hearing aids. RESULTS: Acoustic stimulation through the effective use of hearing aids caused a reduction in the perception of tinnitus sound intensity (especially in evaluations with the prosthesis on) and in nuisance associated with this symptom and with hearing loss. In addition, all participants were satisfied with the use of hearing aids. CONCLUSION: The continuous use of hearing aids is beneficial for the treatment of tinnitus and hearing loss, bringing satisfaction to users.

RESUMO Objetivo Desenvolver um teste para avaliar a compressão de frequências por meio da identificação de fonemas fricativos e aplicá-lo. Métodos Organizou-se um material de fala composto por 24 palavras monossilábicas, contendo os fonemas /s, z, f, v, ∫, 3/. As palavras foram gravadas por oito sujeitos, totalizando uma amostra de 192 gravações. Aplicaram-se filtros passa-baixas nas palavras com frequências de cortes em 1,5 kHz; 2 kHz e 3 kHz, a fim de impedir a detecção sonora nas faixas de frequências filtradas, simulando perdas auditivas em frequências altas. Aplicou-se o teste em dez indivíduos normo-ouvintes, que deveriam ouvir a palavra e identificá-la na tela de um computador. Foram oferecidas 12 opções de resposta, fixando a vogal e variando o fonema fricativo inicial e a presença da fricativa final /s/. O teste foi composto por 384 repetições de palavras, sendo 192 com compressão de frequências. As respostas foram computadas e comparadas por meio de testes estatísticos. Resultados A identificação dos fonemas foi favorecida pelo aumento da frequência de corte, exceto em 3 kHz. Houve melhora na identificação dos fonemas fricativos /∫, 3/ com o uso da compressão de frequências, porém, para os demais fonemas avaliados (/f, v/ e /s, z/) não houve diferenças. Conclusão O teste de identificação de fonemas fricativos se mostrou eficiente e confiável. Para as frequências de corte em 1,5 kHz e 2 kHz, a compressão de frequências foi eficaz para a identificação dos fonemas fricativos /∫, 3/, porém, a identificação do /s/ final mostrou-se comprometida pelo algoritmo.

ABSTRACT Purpose Develop a test to evaluate frequency compression by identifying and applying fricative phonemes. Methods A speech material was composed of 24 monosyllabic words, containing the phonemes /s, z, f, v, ∫, 3/. Eight subjects recorded words, for a total sample of 192 recordings. Low-pass filters were applied to words with cutoff points values of 1,5 kHz; 2 kHz e 3 kHz, in order to prevent sound detection in the filtered frequency bands, simulating hearing loss at high frequencies. The test was applied to ten normal hearing individuals, who had to listen to the word and identify it on a computer screen. Twelve response options were offered, fixing the vowel and varying the initial fricative phoneme and the presence of the final fricative /s/. The test consisted of 384 word repetitions, 192 with frequency compression. Responses were computed and compared using statistical tests. Results The identification of the phonemes was favored by the increase of the cutoff frequency, except in 3 kHz. There was improvement in the identification of fricative phonemes /∫, 3/ with the use of frequency compression, however, for the other assessed phonemes (/f, v/ e /s, z/) no differences were found. Conclusion The identification test of fricative phonemes was efficient and reliable. For the cutoff frequencies in 1,5 kHz e 2 kHz, the frequency compression was effective for the identification of the fricative phonemes /∫, 3/, however the identification of /s/ at the end was affected by the algorithm.

RESUMO Objetivo Avaliar a percepção de fala de adultos usuários de próteses auditivas, antes e depois do ajuste fino realizado com base nos resultados obtidos no Mapeamento de Fala com Estímulo em Português (MFP). Métodos Participaram desta pesquisa 20 adultos, com idades entre 18 e 60 anos, com perda auditiva neurossensorial bilateral simétrica de grau moderado a severo, usuários de próteses auditivas digitais, com no mínimo, seis meses de experiência e programadas de acordo com a regra prescritiva National Acoustics Labs, Non-Linear, version 1 (NAL-NL1).Para avaliação da percepção de fala, foi aplicado o Teste Listas de Sentenças em Português (LSP) em dois momentos distintos: T1, antes do ajuste fino realizado com base no MFPe T2, após ajuste fino realizado com base no MFP. O teste Lista de Sentenças em Português (LSP) possibilitou a pesquisa do Limiar de Reconhecimento de Sentenças no Silêncio (LRSS) e do Limiar de Reconhecimento de Sentenças no Ruído (LRSR), bem como a Relação Sinal/Ruído (S/R) necessária para compreensão das sentenças do teste. Resultados Verificou-se melhora significativa dos LRSS e LRSR no momento T2, ou seja, após o ajuste fino para alcance dos alvos prescritos. Em ambas as medidas, houve redução dos valores: o LRSS médio caiu de 39,60 dB para 34,41 dB e a relação sinal/ruído, de 5,82 dB para 3,34 dB. Conclusão Foi possível estabelecer uma relação nítida entre o alcance dos alvos prescritos e a melhora na percepção de fala. Usuários de próteses auditivas apresentaram melhor desempenho em testes verbais após o ajuste das características eletroacústicas de suas próteses auditivas, por meio do procedimento de mapeamento de fala.

ABSTRACT Purpose To evaluate the speech perception of adult hearing aid users pre and post fine tuning based on Speech Mapping in Brazilian Portuguese (SMBP). Methods Twenty adults aged 18 to 60 years old presenting moderate to severe bilateral symmetrical sensorineural hearing loss and wearing digital hearing aids programmed according to the prescriptive formula National Acoustics Labs, Non-Linear, version 1 (NAL- NL1) for at least six months participated in this study. The test Lists of Sentences in Portuguese (LSP) has been used to evaluate speech perception. The test was applied in two different moments: T1 - before hearing aid fine tuning based on the SMBP and T2 – after fine tuning based on SMBP. The LSP test allowed the Sentence Recognition in Silence (SRTS) and in Noise (SRTN) Threshold calculation and the minimal signal to noise ratio (SNR) required to understand the sentences. Results There was significant improvement of SRTS and SRTN in moment T2, or after hearing aid fine-tuning, to reach the prescribed targets. In both tests decreased values could be observed; the average SRTS fell from 39.60 dB to 34.41 dB and the signal to noise ratio (SNR) fell from 5.82 dB to 3.34 dB. Conclusion It was possible to establish a clear link between achieving the prescribed targets and the improvement in speech perception. Hearing aid users performed better on speech perception tests after adjustments of the electroacoustic characteristics of their hearing aids based on the Speech Mapping procedure.
