Acetic acid bacteria (AAB) are versatile organisms that catalyze the conversion of a wide range of carbon sources into biomolecules of great industrial interest. In this study, we exploited the ability of Gluconobacter spp. to synthesize levans. Among the isolated strains, Gluconobacter cerinus UELBM11 produced approximately 14.0 g L−1 of levan under non-optimized conditions. Gas chromatography-mass spectrometry (GC-MS), Fourier transform infrared spectroscopy (FTIR), and nuclear magnetic resonance (NMR) analyses confirmed that levan obtained from G. cerinus UELBM11 consisted of a β-(2→6)-D-fructose backbone with some β-(2→1) ramifications. The average molecular weight (Mw) of the purified levan was 8.78 × 105 Da. Thermogravimetric/differential thermogravimetric (TGA/DTG) analysis indicated high thermal stability, with the maximum decomposition rate observed at 227.44 °C. Scanning electron microscopy (SEM) revealed a microporous morphology, and the antioxidant activity assays demonstrated that levan had a high scavenging capacity of 2,2’-azino-bis (3-ethylbenzothiazoline-6-sulfonic acid) (ABTS) and hydroxyl radicals. Therefore, it has been demonstrated the levan produced by G. cerinus UELBM11 is a promising natural antioxidant and, owing to its microporosity and excellent thermal properties and stability, is a potential candidate as an additive in cosmetics, pharmaceuticals, and food products. Resumo As síndromes poliglandulares autoimunes (PAS) são um grupo complexo e heterogéneo de doenças caracterizado pela combinação de duas ou mais disfunções endócrinas autoimunes. É apresentado o caso de uma adolescente de 16 anos de idade com história de astenia, prostração e perda de peso, que foi diagnosticada com hipotiroidismo e anemia ferropénica. Duas semanas após ter iniciado tratamento com levotiroxina e ferro, desenvolveu insuficiência suprarrenal com náuseas e vómitos. Os níveis de cortisol baixo e hormona adrenocorticotrópica elevada e presença de anticorpos anti-suprarrenal e antiperoxidase tiroideia positivos confirmaram o diagnóstico de síndrome poliglandular autoimune tipo 2 (PAS-2). O tratamento com hidrocortisona, fludrocortisona e levotiroxina levou à remissão dos sintomas A PAS-2 é uma condição rara, sendo necessária uma elevada suspeita clínica para a mesma, dado que a maioria dos sintomas são inespecíficos. O diagnóstico precoce pode prevenir complicações potencialmente fatais.
Abstract Autoimmune polyglandular syndromes (PAS) are a complex and heterogeneous group of diseases characterized by the combination of two or more endocrine or non-endocrine organ dysfunctions. Herein is presented the case of a 16-year-old female with a history of asthenia, prostration, and weight loss who was diagnosed with hypothyroidism and iron deficiency anemia. Two weeks after starting therapy with levothyroxine and iron, she developed an adrenal crisis with nausea and vomiting. Low cortisol, elevated adrenocorticotropic hormone, and positive antiadrenal and antithyroid peroxidase antibodies confirmed the diagnosis of autoimmune polyglandular syndrome type 2 (PAS-2). Symptoms resolved with hydrocortisone, fludrocortisone, and levothyroxine. PAS-2 is a rare but potentially serious disorder. A high clinical suspicion is required, as most symptoms are nonspecific. Early diagnosis may prevent life-threatening complications.

Abstract Introduction: The negotiation process between parents, children, and nurses during hospitalization must be structured for a successful interaction. However, there is limited scientific evidence on parents' perspectives. Objective: Understand the negotiation process that takes place between nurses and parents in the Paediatric Ward. Methods: A qualitative descriptive study was conducted with 11 parents using semi-structured interviews. The data was analysed using content analysis techniques by Bardin (2016). Results: Five domains emerged from the discourse and perspectives of parents regarding care negotiation. The structuring conditions and knowledge of the child and family serve as foundations to support the negotiation process. During the negotiation itself, two roles converge: the parental role, involving the assumption and adaptation to the parental role, and the role of the nurses. Some behaviours promote negotiation, aimed at making the process more natural. However, some behaviours on the part of the nurses could hinder negotiation. Post-discharge support is acknowledged as an essential extra support once the child returns home. Conclusion: The findings provide clarification of the roles of the different participants in care negotiation, with potential contributions to the implementation of actions that improve the negotiating process in a more sustained and intentional manner.
Resumen Introducción: La negociación de los cuidados es un aspecto clave para el éxito de la interacción entre padres, hijos y personal de enfermería durante la hospitalización. Sin embargo, hay poca evidencia científica sobre las perspectivas de los padres en el proceso de negociación. Objetivo: Comprender la negociación de los cuidados entre enfermeras y padres en el servicio de Pediatría Médica. Métodos: Estudio de enfoque cualitativo de tipo descriptivo, que contó con la participación de 11 padres. La recolección de datos se llevó a cabo a través de entrevistas semiestructuradas y los datos fueron analizados mediante la técnica de análisis de contenido según Bardin (2016). Resultados: De los discursos y perspectivas de los padres sobre la negociación de cuidados, surgieron cinco dominios. Las condiciones estructurantes y el conocimiento del niño y la familia se presentan como pilares para sustentar el proceso negociador. En la negociación propiamente dicha, convergen dos roles: el parental, con la asunción y adaptación a la condición del rol parental, y el rol de los enfermeros. Hay comportamientos que favorecen la negociación, que tienden hacia la naturalidad del proceso de negociar; sin embargo, también existen comportamientos por parte de los enfermeros que perjudican la negociación. El apoyo después del alta es reconocido como un soporte importante después del regreso del niño a casa. Conclusión: Los hallazgos permiten aclarar los roles de los diferentes intervinientes en la negociación de cuidados, con el potencial de contribuir a la implementación de acciones favorecedoras del proceso negociador de manera más sostenida e intencional.

Contudo, constata-se uma escassa evidência científica sobre as perspetivas de pais relativamente ao processo negocial. Objetivo: Compreender a negociação de cuidados desenvolvida entre enfermeiros e pais no serviço de Pediatria Médica. Métodos: Estudo de abordagem qualitativa de tipo descritivo, que contou com a participação de 11 pais. A recolha de dados realizou-se através de entrevistas semiestruturadas e os dados analisados com recurso à técnica de análise de conteúdo segundo Bardin (2016). Resultados: Dos discursos e perspetivas dos pais sobre a negociação de cuidados, emergiram cinco domínios. As condições estruturantes, e o conhecimento da criança e família, são alicerces para sustentar o processo negocial. Na negociação propriamente dita, confluem dois papéis, o parental, com a assunção e a adaptação à condição do papel parental, e o papel dos enfermeiros. Há comportamentos promotores da negociação, que tendem para a naturalidade do processo de negociar, não obstante, existirem também comportamentos por parte dos enfermeiros que inibem a negociação. O apoio após a alta é reconhecido como um importante suporte após o regresso da criança a casa. Conclusão: Os achados permitem uma clarificação dos papéis dos diferentes intervenientes na negociação de cuidados, com potencial para contribuir para a implementação de ações favorecedoras do processo negocial de uma forma mais sustentada e intencional.

Abstract Difficult airway management in pediatrics during anesthesia represents a major challenge, requiring a careful approach, advanced technical expertise, and accurate protocols. The task force of the Brazilian Society of Anesthesiology (SBA) presents a report containing updated recommendations for the management of difficult airways in children and neonates. These recommendations have been developed based on the consensus of a panel of experts, with the objective of offering strategies to overcome challenges during airway management in pediatric patients. Grounded in evidence published in international guidelines and expert opinions, the report highlights crucial steps for the appropriate management of difficult airways in pediatrics, encompassing assessment, preparation, positioning, pre-oxygenation, minimizing trauma, and, paramountly, the maintenance of arterial oxygenation. The report also delves into additional strategies involving the use of advanced tools, such as video laryngoscopy, flexible intubating bronchoscopy, and supraglottic devices. Emphasis is placed on the simplicity of implementing the outlined recommendations, with a focus on the significance of continuous education, training through realistic simulations, and familiarity with the latest available technologies. Abstract Antimicrobial photodynamic therapy (aPDT) is an adjuvant treatment to scaling and root planing (SRP) which improves periodontal health. It may be beneficial to patients with systemic diseases, such as type 1 diabetes mellitus. Objective This randomized clinical trial evaluated the adjunctive effect of aPDT on the periodontal treatment of patients with type 1 diabetes (T1D). Methodology 38 patients were included in the study and divided into four groups: DSRP – T1D patients treated with SRP; CSRP – normoglycemic patients treated with SRP; DPDT – T1D patients treated with SRP + aPDT (methylene blue and red laser); CPDT – normoglycemic patients treated with SRP + aPDT. , Periodontal clinical parameters and inflammatory cytokines in crevicular fluid were recorded at baseline and then after 1, 3 and 6 months. The clinical endpoint for treatment was evaluated after 6 months. Results Adjuvant aPDT treatment resulted in reduction of probing depth after 3 months (0.38 mm - p<0.05) on T1D patients and in control group after 6 months (0.66 mm - p<0.05). Reduction of clinical attachment levels was similar for both treatments in control patients (p>0.05). There was a significant reduction of TNF-α in crevicular fluid in both groups treated with aPDT (p<0.05). The T1D (65%) and normoglycemic (72%) groups achieved the clinical endpoint after both treatments (p>0.05). Conclusions Adjuvant aPDT provided additional benefits in improving periodontal clinical parameters and reducing inflammatory cytokines in both T1D and normoglycemic patients. ABSTRACT Waste pickers constitute a marginalized demographic engaged in the collection of refuse, facing considerable occupational hazards that heighten their susceptibility to contract infectious diseases. Moreover, waste pickers contend with societal stigmatization and encounter barriers to accessing healthcare services. To explore the viral profile of waste pickers potentially linked to their occupational environment, we conducted a metagenomic analysis on 120 plasma specimens sampled from individuals employed at the Cidade Estrutural dumpsite in Brasilia city, Brazil. In total, 60 blood donors served as a comparative control group. Specimens were pooled and subjected to Illumina NextSeq 2000 sequencing. Viral abundance among waste pickers revealed the presence of significant pathogens, including HIV, HCV, and Chikungunya, which were not detected in the control group. Additionally, elevated levels of anelloviruses and Human pegivirus-1 were noted, with a comparable incidence in the control group. These findings underscore the utility of metagenomics in identifying clinically relevant viral agents within underserved populations. Abstract Objectives To present the Brazilian Psychiatric Association’s Consensus on the Management of Acute Intoxication. Methods A group of experts selected by the Brazilian Psychiatric Association searched for articles on the MEDLINE (by PubMed) and Cochrane databases, limited to human studies and acute intoxication. Working groups reviewed these materials for appropriateness to the topic and the quality of the work. A survey was conducted using the Delphi method to produce a table of agreed recommendations presented at the end of the systematic review. Three survey rounds were held to reach consensus. Results Support for intoxication should start with Initial Management: Resuscitation/Life Support/Differential Diagnosis. For this, the group proposed the following sequence of assessments: A (airway), B (breathing), C (circulation), D.1 (disability), D.2 (differential diagnosis), D.3 (decontamination), D.4 (drug antidotes), E (enhanced elimination). The group of experts then presented specific interventions for the main drugs of abuse. Conclusions Management of intoxication with drugs of abuse is complex and requires systematic protocols. The group suggests adoption of the A-B-C-D-E technique first, with constant investigation. Then, specific conduct and support until remission of intoxication. The literature is still scarce in evidence on the subject. Therefore, this consensus was necessary. We believe that at present this document can help psychiatric, general, and emergency physicians deal with emergency psychiatric episodes due to acute intoxication. This work could stimulate future studies on the topic. Abstract Background There is a remarkable variability in the frequency of HLA-B27 positivity in patients with spondyloarthritis (SpA), which may be associated with different clinical presentations worldwide. However, there is a lack of data considering ethnicity and sex on the evaluation of the main clinical and prognostic outcomes in mixed-race populations. The aim of this study was to evaluate the frequency of HLA-B27 and its correlation with disease parameters in a large population of patients from the Brazilian Registry of Spondyloarthritis (RBE). Methods The RBE is a multicenter, observational, prospective cohort that enrolled patients with SpA from 46 centers representing all five geographic regions of Brazil. The inclusion criteria were as follow: (1) diagnosis of axSpA by an expert rheumatologist; (2) age ≥18 years; (3) classification according to ASAS axial. The following data were collected via a standardized protocol: demographic data, disease parameters and treatment historical. Results A total of 1096 patients were included, with 73.4% HLA-B27 positivity and a mean age of 44.4 (±13.2) years. Positive HLA-B27 was significantly associated with male sex, earlier age at disease onset and diagnosis, uveitis, and family history of SpA. Conversely, negative HLA-B27 was associated with psoriasis, higher peripheral involvement and disease activity, worse quality of life and mobility. Conclusions Our data showed that HLA-B27 positivity was associated with a classic axSpA pattern quite similar to that of Caucasian axSpA patients around the world. Furthermore, its absence was associated with peripheral manifestations and worse outcomes, suggesting a relevant phenotypic difference in a highly miscegenated population. SUMMARY OBJECTIVE: To compare the Mini-Mental State Examination (MMSE) and Montreal Cognitive Assessment (MoCA) tests for the identification of cognitive deficit (CD) in elderly patients with heart failure (HF). METHODS: This was a cross-sectional study with an observational design involving 43 elderly patients with HF of both sexes, treated by the Unified Health System, who were able to understand and follow the study instructions. A sociodemographic and clinical questionnaire and the MMSE and MoCA neurocognitive tests were applied. RESULTS: The mean age of the patients was 67 years; 67.44% were male; 53.49% were white; 58.14% had 1–4 years of schooling; 58.14% had an income of half to one minimum wage; 55.81% were married; 53.49% had a family history of HF; 90.7% denied smoking; 83.72% denied alcohol intake; 65.12% did not practice physical activity; 83.72% were hypertensive; 30.23% were diabetic; 57.89% had LVEF ≥ 50%; 39.53% have NYHA II; and 88.37% did not have a pacemaker. In the identification of CD, the MMSE test detected it in 25.58% of the patients, while the MoCA test identified it in 23.26% (p=0.043). ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. Resumo Objetivos: avaliar a prevalência e fatores associados à sífilis adquirida em gestantes atendidas na atenção primária à saúde de Montes Claros-MG. Métodos: trata-se de um estudo transversal realizado com gestantes cadastradas nas equipes da Estratégia Saúde da Família de Montes Claros, Minas Gerais, Brasil (2018-2019). Fatores sociodemográficos, comportamentais e obstétricos foram coletados por meio de questionário aplicado por entrevista. Foi avaliado o resultado do exame Venereal Disease Research Laboratory (VDRL). Uma análise de regressão logística binária foi usada para determinar os fatores associados à sífilis na gestação. Resultados: a prevalência de sífilis encontrada foi de 1,7% (IC95%= 1,44-1,96). Gestantes com ensino médio tiveram 75% (OR = 0,25; IC95% = 0,08-0,81) menos chance de ter sífilis, quando comparado a gestantes com ensino fundamental. Gestantes que fizeram uso de drogas ilícitas antes da gestação (OR = 3,47; IC95%= 1,02-11,82), e consumiram bebidas alcoólicas durante a gestação (OR = 16,35; IC95%= 3,81-70,20) apresentaram maior chance de ter a doença. Conclusão: a escolaridade, o consumo de álcool e drogas ilícitas estão associados ao diagnóstico de sífilis em gestantes. Portanto, são necessários programas e intervenções educativas para abordar questões relacionadas à prevenção, diagnóstico, tratamento e os seus fatores de risco, sobretudo, os determinantes sociais e as questões de saúde reprodutiva das mulheres. Abstract Objectives: to evaluate the prevalence and associated factors with acquired syphilis in pregnant women attended at the primary health care in Montes Claros-MG. Methods: a cross-sectional study carried out with pregnant women enrolled in the Estratégia Saúde da Família (Family Health Strategy) teams in Montes Claros, Minas Gerais, Brazil (2018-2019). Sociodemographic, behavioral and obstetric factors were collected through a questionnaire applied by interview. The result of the Venereal Disease Research Laboratory (VDRL) examination was evaluated. Binary logistic regression analysis was used to determine associated factors with syphilis in pregnancy. Results: the prevalence of syphilis found was 1.7% (CI95%= 1.44–1.96). Pregnant women who graduated from high school had 75% (OR = 0.25; CI95%= 0.08–0.81) less chance of having syphilis, when compared to pregnant women with elementary school. Pregnant women who used illicit drugs during pregnancy (OR = 3.47; CI95%= 1.02–11.82) and consumed alcoholic beverages during pregnancy (OR = 16.35; CI95%= 3. 81–70.20) were more likely to have the disease. Conclusion: schooling, consumption of alcohol and illicit drugs are associated with the diagnosis of syphilis in pregnant women. Therefore, educational programs and interventions are needed to address issues related to prevention, diagnosis, treatment and their risk factors, especially social determinants, and women’s reproductive health issues. RESUMO Objetivos: analisar as propriedades psicométricas do instrumento ProQOL-BR em profissionais de enfermagem hospitalares. Métodos: estudo metodológico de validação do ProQOL-BR. Utilizaram-se análise fatorial confirmatória, avaliação da qualidade de ajuste local e ajuste global, teste de hipóteses Pearson e análise de consistência interna alfa de Cronbach. Resultados: participaram 490 profissionais. O modelo apresenta qualidade adequada pelos pesos fatoriais (λ≥ 0,40), qualidade de ajuste global aceitável e razão qui-quadrado e graus de liberdade (2/g.1=2,51) adequada para os parâmetros de CFI (0,923), GFI (0,902), TLI (0,914) e RMSEA (0,042). Na validade, mostrou-se adequado com CC=0,89. A consistência interna obtida pelo alfa de Cronbach padronizado foi de 0,761. A validade de critério mostrou-se favorável com correlações significativas (0,001). Conclusões: o instrumento encontra-se validado em seu conteúdo, critério e confiabilidade. ABSTRACT Objectives: to analyze the psychometric properties of the ProQOL-BR instrument in hospital nursing professionals. Methods: a methodological study to validate the ProQOL-BR. Confirmatory factor analysis, assessment of local and global adjustment quality, Pearson hypothesis testing and Cronbach’s alpha internal consistency analysis were used. Results: a total of 490 professionals participated. The model presents adequate quality due to factor weights (λ≥ 0.40), acceptable overall fit quality and adequate chi-square ratio and degrees of freedom (χ2/g.1=2.51) for the parameters of CFI (0.923), GFI (0.902), TLI (0.914) and RMSEA (0.042). In terms of validity, it was shown to be adequate with CC=0.89. The internal consistency obtained by standardized Cronbach’s alpha was 0.761. Criterion validity was shown to be favorable with significant correlations (0.001). Conclusions: the instrument was validated regarding content, criteria and reliability.
