ABSTRACT The study addresses decision-making under conditions of uncertainty regarding the development of public policies and sustainable development strategies in an urban region in Brazil. Based on the case study, a decision support framework for planning and prioritizing urban sustainable development strategies (SDS) under uncertainty conditions is developed. In the framework social, economic and environmental factors have been considered using a fuzzy multi-criteria comprehensive evaluation approach in order to analyze and select SDSs. Critical factors and sub-factors for SDS are identified with the active participation of a committee of experts and representatives from different sectors of civil society. As a result, a set of urban sustainable development strategies are prioritized using the Fuzzy VIKOR Method that will help local authorities to improve their decisions. ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. Abstract Psychoactive drug use is related to the deterioration of neuropsychological functions, with attention, memory and executive functions being the most affected. Clinical functionality is also compromised, resulting in lower participation and treatment implication, enhancing relapse and / or dropout rate of treatment programs. A battery of neuropsychological and clinical tests was applied to a sample of 31 users of assistive care devices for addictive behaviors and addictions of ARS Norte, I.P. The aim of this study is to describe cognitive and executive neuropsychological changes, as perceived symptoms in a sample of users with substance related disorders. The results reflect a neuropsychological performance indicative of reduced psychomotor, reader and visuospatial processing speed, deficits in sustained, selective and alternating attention subprocesses, and difficulties in executive decision making, inhibition and change and clinical manifestations of obsessions / compulsions, phobic anxiety, psychoticism and paranoid ideation.
Resumo O consumo de drogas psicoactivas está relacionado com a deterioração das funções neuropsicológicas, destacando-se os domínios da atenção, memória e funções executivas como os mais afectados. A funcionalidade a nível clínico está também comprometida, traduzida na menor participação e implicação no tratamento, potenciando as recaídas e/ou a taxa de abandono dos programas de tratamento. Aplicou-se uma bateria de provas neuropsicológicas e clínicas, a uma amostra de 31 de utentes em dispositivos assistenciais de tratamento para os comportamentos aditivos e dependências da ARS Norte, I.P. O objetivo deste estudo é descrever as alterações neuropsicológicas, a nível cognitivo e executivo, assim como a sintomatologia percebida. Os resultados refletem um desempenho neuropsicológico indicativo de uma velocidade reduzida de processamento psicomotor, leitor e visuoespacial, déficits nos subprocessos de atenção sustentada, seletiva e alternada e dificuldades nos componentes executivos de tomada de decisão, inibição e mudança, e manifestações clínicas de obsessões/compulsões, ansiedade fóbica, psicoticismo e ideação paranóide.

Abstract: Introduction: Acute myocardial infarction has a high prevalence and possibility of death if timely care is not given. It is possible that treatment could be delayed during a health crisis such as the COVID-19 pandemic. Objective: The objective of this study was to evaluate the impact of the COVID-19 pandemic on the function of the infarction code in the High Specialty Medical Unit of our institution. The medical indicators of acute coronary ischemic syndrome with ST-segment elevation were compared at two times: before the COVID-19 pandemic and during it. Material and methods: An ambispective, analytical, comparative cohort study was conducted, comparing the periods of February-September 2019 and February-September 2020, in the High Specialty Medical Unit, Centro Médico del Bajío number 1, Instituto Mexicano del Seguro Social, Leon, Guanajuato, Mexico. All patients older than 18 years with clinical, electrocardiographic, and enzymatic data of acute coronary ischemic syndrome, accepted in the medical network as infarction code registered in the National Registry of Acute Coronary Ischemic Syndromes (RENASCA) electronic case report form were included in the study. Results: The independent samples t-test was used to determine the impact of the infarction code initiative based on the reperfusion therapies performed, the door-to-needle, the door-to-balloon time, and the ischemia time. There was a significant difference in the reperfusion strategy, with greater thrombolysis during the COVID-19 pandemic (57.4% versus 72.6%, odds ratio [OR] 1.97, 95% confidence interval [CI] 0.49-0.82, p < 0.001), as well as reperfusion criteria (21.03% versus 35.37%, OR 2.05, 95% CI 0.53-0.80, p < 0.0001). Mortality was not different between the time periods. Conclusions: The COVID-19 pandemic has not impacted operation of the infarction code in the High Specialty Medical Unit Centro Médico del Bajío number 1 because the medical indicators of acute coronary ischemic syndrome with ST-segment elevation did not change.

RESUMO O presente artigo tem como objetivo analisar as implicações dos rankings acadêmicos, como instrumentos de avaliação externa, na estrutura de governança das universidades católicas brasileiras. Para o desenvolvimento do estudo foi realizada pesquisa exploratória por meio de análises documentais e bibliográficas, tomando como referência produção científica obtida em bases indexadas, legislação federal, constituições apostólicas, direito canônico, estatutos, regimentos e Planos de Desenvolvimento Institucional (PDI’s) dessas instituições. Diante dos inúmeros rankings existentes, evidencia-se em grande parte das universidades católicas, o uso desse ferramental no processo de planejamento estratégico, por meio de seus respectivos desempenhos para o estabelecimento de metas e objetivos.
ABSTRACT This article aims to analyze the implications of academic rankings, as external assessment tools, in the governance structure of Brazilian Catholic universities. For the development of this study, exploratory research was carried out through documentary and bibliographic analyses, taking as reference scientific production found in indexed bases, federal legislation, apostolic constitutions, canon law, statutes, regulations and Institutional Development Plans (IDPs) of these institutions. In light of the numerous existing rankings, their use is evident in the strategic planning process of most Catholic universities, through their respective performances in establishing goals and objectives.

Abstract Background: Cardiovascular diseases are the leading cause of mortality worldwide and Mexico is no exception. The epidemiological data obtained in 1990 showed that cardiovascular diseases represented 19.8% of all causes of death in our country. This figure increased significantly to 25.5% for 2015. Some national surveys suggest that more than 60% of the adult population has at least one risk factor for cardiovascular disease (obesity or overweight, hypertension, smoking, diabetes, dyslipidemias). On the other hand, data from the Pan American Health Organization have linked the process of atherosclerosis as the first cause of premature death, significantly reducing life expectancy, which has enormous social repercussions. Objective: This document constitutes the Clinical Practice Guide (CPG) prepared at the initiative of the Mexican Society of Cardiology in collaboration with the Mexican Society of Nutrition and Endocrinology, AC, National Association of Cardiologists of Mexico, AC, Mexican Association for the Prevention of Atherosclerosis and its Complications, AC, National Normative Committee of General Medicine, AC, National College of Geriatric Medicine, AC, College of Internal Medicine of Mexico, AC, Mexican Society of Angiology and Vascular and Endovenous Surgery, AC, Mexican Institute of Research Nephrological, AC and the Mexican Academy of Neurology, A.C.; with the methodological support of the Ibero-American Agency for the Development and Evaluation of Health Technologies, in order to establish recommendations based on the best available evidence and agreed upon by an interdisciplinary group of experts. The objective of this document is to provide evidence-based recommendations to help decision makers in the diagnosis and treatment of dyslipidemias in our country. Material and methods: This document complies with international quality standards, such as those described by the Institute of Medicine of the USA, the Institute of Clinical Excellence of Great Britain, the Scottish Intercollegiate Guideline Network and the Guidelines International Network. A multidisciplinary group of clinical experts and methodologists with experience in systematic reviews of the literature and the development of clinical practice guidelines was formed. A scope document was agreed upon, relevant clinical questions were established, the best available evidence critically evaluated in systematic literature reviews was exhaustively identified, and clinical recommendations were developed. The modified Delphi Panel methodology was used to achieve an adequate level of consensus in each of the recommendations contained in this CPG. Results: 23 clinical questions were agreed upon which gave rise to their respective clinical recommendations. Conclusions: We consider that this document contributes to better clinical decision-making and becomes a point of reference for clinicians and patients in the management of dyslipidemias and this contributes to reducing the morbidity and mortality derived from atherosclerotic cardiovascular events in our country.

Inventario florístico en un bosque amenazado por la expansión agrícola en la reserva del Centro Turístico Los Chocuacos, Costa Rica

Resumen (Introducción): La expansión de monocultivos de palma africana (Elaeis guineensis) y piña (Ananas comosus), en Costa Rica, ha degradado en forma progresiva el entorno natural del país, particularmente la región del distrito de Potrero Grande de Buenos Aires. (Objetivo): Se buscó evidenciar la importancia de la conservación en la reserva del Centro Turístico Los Chocuacos (CTLC), que conserva un mosaico de bosque, entre secundario y primario, de aproximadamente 30 años y la cual, está amenazada por el crecimiento de monocultivos. (Metodología): En un área de 25 ha, se realizó un inventario florístico mediante recorridos aleatorios en el borde del bosque, los senderos, las áreas sin senderos, las quebradas y nacientes, donde se recolectaron especímenes botánicos en estado reproductivo, que fueron herborizados y depositados en el Herbario Luis A. Fournier Origgi de la Universidad de Costa Rica. (Resultados): Se obtuvo una riqueza florística de 75 familias, 219 géneros y 266 especies. Se encontró un nuevo registro para el país del arbusto Eugenia veraguensis y una especie del género Aristolochia que en la actualidad está siendo descrita como una nueva especie para la ciencia. La forma de vida más representativa es árbol, con hábitat, principalmente presente terrestre. Las familias Fabaceae, Rubiaceae y Malvaceae fueron las que poseen mayor riqueza de especies. (Conclusiones): Este estudio evidencia la importancia de los inventarios florísticos para aumentar el conocimiento de especies presentes en zonas poco estudiadas del trópico que están siendo afectadas por el desordenado cambio en el uso de la tierra.
Abstract (Introduction): The expansion of African palm (Elaeis guineensis) and pineapple (Ananas comosus) monocultures in Costa Rica has progressively degraded the natural environment of the country, standing out the region of the Potrero Grande district of Buenos Aires. (Objective): This study aims to show the importance of conservation in the Centro Turistico Los Chocuacos (CTLC) reserve, which is a threatened area, and that conserves a forest, between secondary and primary, of approximately 30 years. (Methodology): In an area of 61.77 acres (25 hectares), a floristic inventory was performed by random walks along the forest edge, trails, river, waters springs, and areas not crossed by trails. The botanical specimens were collected in a reproductive state, botanized and deposited in the Herbarium Luis A. Fournier Origgi of the University of Costa Rica. (Results): The CTLC reserve has a floristic wealth of 75 families, 219 genera, and 266 species. The results yield important data, such as a new record for Costa Rica, Eugenia veraguensis, and a species of the genus Aristolochia, which is currently being described as a new species for science. The most representative life form is the tree, and the habit is mostly terrestrial. The Fabaceae, Rubiaceae, and Malvaceae families were the most representative in the study area. (Conclusions): The study highlights the importance of floristic inventories to increase knowledge of species present in poorly studied regions of the tropics that are also being affected by the disorderly land-use change.

Abstract The objective of this work was to evaluate the economic and environmental benefits of a 158 megawatts (MW) steam generator that operates with fuel oil as fuel, derived from making operational adjustments to the combustion system and the design of its atomizers. To achieve the objective, various different types of atomizers were designed and built, which were cold tested in a real-scale atomization bank. The atomizer design INEEL of better performance, it was compared in operation against the original atomizer. The most important benefits of the INEEL design atomizer were a 53 % reduction in particle matter (PST) emission, a reduction of 93 % in carbon monoxide (CO) and a fuel saving of 897 kilograms per hour (kg/h), which will stop emitting 651 tons per year (tpa) of CO, 19 tpa of nitrogen oxides (NOX), 1,091 tpa of PST, 583 tpa of sulphur dioxide (SO2) and 23,614 tpa of carbon dioxide (CO2), also reaching a monetary saving of 170,352 pesos per day. The results show that the application of scientific studies on combustion systems is a viable alternative to improve the financial efficiency and environmental performance of an industrial complex.
Resumen El objetivo de este trabajo fue evaluar los beneficios económicos y ambientales de un generador de vapor de 158 megawatts (MW) que opera con combustóleo como combustible, derivados de realizar ajustes operativos al sistema de combustión y al diseño de sus atomizadores. Para lograr el objetivo, se diseñaron y construyeron diversos tipos de atomizadores, mismos que fueron probados en frío en un banco de atomización a escala real. El atomizador diseño INEEL de mejor desempeño fue comparado en operación contra el atomizador original. Los beneficios más importantes del atomizador diseño INEEL fueron una disminución en la emisión de partículas (PST) de 53 %, la reducción de 93 % del monóxido de carbono (CO) y un ahorro de combustible de 897 kilogramos por hora (kg/h), con lo cual se dejaron de emitir 651 toneladas por año (tpa) de CO, 19 tpa de óxidos de nitrógeno (NOX), 1,091 tpa de PST, 583 tpa de bióxido de azufre (SO2) y 23,614 tpa de bióxido de carbono (CO2), alcanzando además un ahorro monetario de 170,352 pesos por día. Los resultados muestran que la aplicación de estudios científicos sobre los sistemas de combustión, es una alternativa viable para mejorar la eficiencia financiera y el desempeño ambiental de un complejo industrial.

Correlation of hepatic fibrosis assessed by Metavir score and digital morphometry in a murine model

Background: Animal models for hepatic fibrosis are currently used for clinical studies, and research works aimed to the progression and regression of the disease. In these, fibrosis is evaluated using histopathological assessment with high variability inter- and intra-observers. Digital morphometry in the space color LAB could reduce this variability; however, its correlation degree with the histopathological assessment is unknown. Objective: The objective of the study was to analyze the correlation between digital morphometry and the histopathological assessment of hepatic fibrosis in images of biopsies of a murine model. Materials and methods: We performed a descriptive, experimental, longitudinal, prospective, analytical, and correlational study. Fibrosis was induced by intraperitoneal injection of carbon tetrachloride in 16 Wistar male rats. We did a postmortem hepatectomy and took histological specimens stained by Masson trichrome, obtaining ten randomly selected images at ×10 in each liver. We quantified the fibrosis percentage (FP) by digital morphometry and compared it to the Metavir score given by three independent evaluators. FP was analyzed by a ANOVA test and Spearman Rho test. Results: The evaluated images had 70% concordance of the three evaluators. The analysis of the FP revealed fibrosis evaluation groups with statistical differences. The FP had a very high correlation with the histopathological diagnosis (p = 0.0001, r = 0-9). The ranges obtained from FP for each phase of fibrosis were: 0.4% for F0, 0.44-1.14% for F1, 1.52-2.74% for F2, 3.1-3.8% for F3y, and 5.12-6.12% for F4. Conclusions: The morphometric quantification had a high correlation with the histopathological evaluation of images of histological sections of liver biopsies of the murine model of fibrosis. The quantification of the FP by digital morphometry will be useful to increase the accuracy in the evaluation of liver fibrosis.

ABSTRACT Objective. to develop a microfluidic system (lab-on-a-chip) for detecting circulating breast cancer tumor cells. Materials and methods . the device was designed using 3D technology, and it was manufactures using soft photolithography and a laser cutting machine. The system performance and its magnetic settings were assessed using Jurkat cells and breast cancer cells that show different expression of CD45 and EpCAM surface markers. Antibodies against these markers were bound to magnetic pellets. Additionally, iron nanoparticles were used for assessing their entrapment. Results . nanoparticles were significantly trapped in the area set by magnetic field modeling. Tumor cells labeled with magnetic antibodies became trapped. Conclusions . we were able to manufacture a lab-on-a-chip system that is capable to trap circulating breast cancer tumor cells, which may become an excellent tool for diagnosis and follow-up for this condition.
RESUMEN Objetivo: desarrollar un sistema microfluídico (lab-on-a-chip) para la detección de células tumorales circulantes de cáncer de mama (CTCs). Materiales y métodos: se diseñó el dispositivo en 3D y se fabricó usando fotolitografía suave y una cortadora láser. Se evaluó el funcionamiento del sistema y del arreglo magnético usando células Jurkat y células de cáncer de mama que poseen diferente expresión de los marcadores superficiales CD45 y EpCAM. Los anticuerpos contra los marcadores fueron unidos a perlas magnéticas. Adicionalmente se usaron nanopartículas de hierro para evaluar su atrapamiento. Resultados: las nanopartículas lograron atraparse de manera significativa en el área propuesta por el modelamiento de campos magnéticos. Las células tumorales marcadas con los anticuerpos magnéticos quedaron atrapadas. Conclusiones: se logró fabricar un lab-on-a-chip capaz de atrapar CTCs generando una excelente herramienta de diagnóstico y de análisis de la progresión de la enfermedad.

Abstract The recommendations in which the Mexican Society of Cardiology (SMC) in conjunction with the National Association of Cardiologists of Mexico (ANCAM) as well as different Mexican medical associations linked to cardiology are presented, after a comprehensive and consensual review and analysis of the topics related to cardiovascular diseases in the COVID-19 pandemic. Scientific positions are analyzed and responsible recommendations on general measures are given to patients, with personal care, healthy eating, regular physical activity, actions in case of cardio-respiratory arrest, protection of the patient and health personnel as well as precise indications in the use of non-invasive cardiovascular imaging, prescription of medications, care in specific topics such as systemic arterial hypertension, heart failure, arrhythmias and acute coronary syndromes, in addition to emphasizing electrophysiology, interventionism, cardiac surgery and in cardiac rehabilitation. The main interest is to provide the medical community with a general orientation on what to do in daily practice and patients with cardiovascular diseases in the setting of this unprecedented epidemiological crisis of COVID-19.
Resumen Se presentan las recomendaciones en las cuales la Sociedad Mexicana de Cardiología (SMC) en conjunto con la Asociación Nacional de Cardiólogos de México (ANCAM), así como diferentes asociaciones médicas mexicanas vinculadas con la cardiología, después de una revisión y análisis exhaustivo y consensuado sobre los tópicos relacionados con las enfermedades cardiovasculares en la pandemia de COVID-19, se analizan posturas científicas y se dan recomendaciones responsables sobre medidas generales a los pacientes, con cuidados personales, alimentación saludable, actividad física regular, acciones en caso de paro cardiorrespiratorio, la protección del paciente y del personal de salud así como las indicaciones precisas en el uso de la imagen cardiovascular no invasiva, la prescripción de medicamentos, cuidados en tópicos específicos como en la hipertensión arterial sistémica, insuficiencia cardiaca, arritmias y síndromes coronarios agudos, además de hacer énfasis en los procedimientos de electrofisiología, intervencionismo, cirugía cardiaca y en la rehabilitación cardiaca. El interés principal es brindar a la comunidad médica una orientación general sobre el quehacer en la práctica cotidiana y pacientes con enfermedades cardiovasculares en el escenario esta crisis epidemiológica sin precedentes de COVID-19.

RESUMO: A produção de madeira de Tectona grandis (teca) em sistemas integrados com pecuária ou agricultura demonstra alta perspectiva de retorno financeiro. Entretanto, estudos sobre o desenvolvimento da teca ainda são escassos, principalmente na região norte de Mato Grosso. O objetivo do trabalho foi avaliar o crescimento das variáveis dendrométricos de um povoamento clonal de teca, em um sistema silvipastoril (iPF), ao longo de 53 meses, no município de Alta Floresta-MT, Brasil. Para isso, foram amostradas três parcelas, totalizando 360 indivíduos, sendo que em cada indivíduo foi mensurado o diâmetro a 1,3 m do solo e altura total. Em seguida realizou-se o cálculo do volume. Para descrever o crescimento da variável diâmetro, altura total e volume foi realizado o ajuste através de cinco modelos matemáticos que expressam o crescimento ao longo do tempo. Posteriormente, foram calculados o incremento corrente anual (ICA) e incremento médio anual (IMA). O modelo de Hoerl apresentou melhores resultados de R²aj, obteve menores valores para o erro padrão de estimativa (Syx) e para o CV%, com isso, foi selecionado para a elaboração das curvas de crescimento. O povoamento clonal de teca no iPF apresentou incremento em DAP, Altura e volume foram superiores em relação a outros estudos científicos com teca, indicando sua viabilidade em sistemas integrados com pastagens na região.
ABSTRACT: Production of Tectona grandis (teak) in integrated systems with livestock or agriculture demonstrates high potential of financial return. However, studies on the development of teak are still scarce, especially in the northern region state of Mato Grosso. In this study we sought to evaluate dendometric variables of a clonal population of teak in a forest-livestock integration system (LFIS), during a period of 53 months in the city of Alta Floresta, Mato Grosso, Brazil. For this purpose, three installments were samples, a total of 360 individuals, and for each the diameter was measured at 1.3 meters from the base so as to calculate the total volume, the current annual increment (CAI) and mean annual increment (MAI), and five adjusted regression models. The Hoerl model provided the highest adjusted coefficient of determination (R2aj), lowest standard error of estimate (Syx), coefficient of variation (CV %), and from this the growth curves were developed. Clonal stands of teakin the forest-livestock system presented increases in DBH, height and volume were superior in relation to other scientific studies with teak, indicating their viability in integrated systems with pastures in the region.

Abstract The first Mexican results of the concentration, morphology and chemical composition of Total Suspended Particles (TSP) and particles smaller than 10 microns (PM10) from three Internal Combustion Engines (ICE) of 42 MW, employees to generate electricity, which were obtained by an experimental campaign on November, 2015 and follows the sampling procedures described in the EPA methods 5 and 201A. The ICE’s used two kind of fuels: in the ICE-2 and ICE-3, it was a Mexican heavy oil with 4% by weight of sulfur, 0.07% ash and a specific gravity greater than 1; while in ICE-1, it was used a mixture with 85% of Mexican Heavy oil and 15% of diesel. The TSP concentration in ICE-1 was of 178 mg/Nm3 in which 62% were PM10; for the case of the ICE-2 and ICE-3, the TSP were of 190 and 151 mg/Nm3, respectively, and in both cases the PM10 were the 80% of the total TSP. The morphology of the TSP and PM10 in all ICE’s, were of solid and spherical particles, with the majority of the sizes under 10µm. When this morphology is compared with the TSP of a conventional furnace, the amorphous, porosity and the wide variety of sizes are the main differences. As principal components in the particles of the ICE´s were identified the O, C, V and S.
Resumen En este estudio se presentan los primeros resultados en México de la concentración, morfología, y composición química de partículas suspendidas totales (PST) y partículas con diámetros aerodinámicos menores de 10 micras (PM10) emitidas por tres motores de combustión interna (MCI) de 42 megawatts (MW), empleados para generar energía eléctrica. La campaña se realizó en noviembre de 2015 y el muestreo se efectuó siguiendo los métodos 5 y 201A EPA. El combustible utilizado en los MCI-2 y MCI-3, fue un combustóleo mexicano con 4% de azufre, y gravedad específica mayor que 1. El MCI-1 utilizó una mezcla con 85% de combustóleo y 15% diésel. La concentración de PST en el MCI-1 fue de 178 mg/Nm3, 190 mg/Nm3 en el MCI-2 y 151 mg/Nm3 en el MCI-3. El porcentaje en peso de las PM10 en relación con las PST fue de 62% en el MCI-1 y de 80% en los MCI-2 y MCI-3. La morfología de las PST y las PM10 en cada MCI se comparó contra la de las partículas de un horno experimental de 0.35 MW. Las partículas emitidas por los MCI fueron sólidas, esféricas, con tamaños menores de 10 micras, difiriendo de las PST del horno piloto, las cuales son amorfas, porosas y de diversos tamaños. La composición química de las partículas de los tres MCI tuvo como componentes principales al O, C, V y S.

Abstract Background: Reclassification of a large thrombus burden is an independent predictor of major adverse cardiac events and no-reflow in patients with ST- segment elevation myocardial infarction (STEMI). Patients with a greater residual thrombus burden have worse microvascular dysfunction and greater myocardial damage. Methods: A retrospective analysis was performed on 833 STEMI patients who underwent primary percutaneous coronary intervention. The final residual thrombus burden was reclassified after the lesion was wired, and a thrombus aspiration or balloon dilatation was performed to restore and stabilise a thrombolysis in myocardial infarction (TIMI) 2-3 flow. Deferred stenting (DEI) was compared with immediate stenting (ISI) group, and the primary outcome was the incidence of no-/slow-reflow (TIMI ≤ 2, or TIMI 3 with myocardial blush grade < 2). Results: Overall, 47 patients (6.8%) had a residual large thrombus burden reclassified. The right coronary artery was the culprit vessel in 34 cases. More patients had coronary ectasia in the DSI group (P=.005). Fewer patients in the DSI had no-/slow-reflow (36% vs. 58%), and the myocardial blush grade 3 was more frequent in the DSI group (P=.005). After repeat coronary angiography in the DSI group, stenting was not performed in 56%, and oral anticoagulation was more frequent in the follow-up (P=.031). Major cardiac adverse events were similar between groups. There was a tendency to better left ventricular function in the DSI group (P=.056). Conclusions: Deferred stenting may be an efficient option in STEMI patients with a residual large thrombus burden reclassified after achieving a stable TIMI 2-3 flow.
Resumen Introducción: Un trombo intracorononario largo reclasificado es un predictor independiente de resultados adversos y no reflujo en el infarto agudo de miocardio con elevación del ST. Pacientes con mayor carga de trombo residual tienen peor disfunción microvascular y mayor daño miocárdico. Métodos: Evaluamos retrospectivamente a 833 pacientes que fueron a angioplastia primaria entre enero del 2011 y junio del 2016. La carga de trombo residual final fue reclasificada tras realizar el cruce de la guía, predilatación con balón o tromboaspiración, para restaurar y estabilizar un flujo TIMI 2-3. Las estrategias de stent diferido vs. stent inmediato fueron comparadas, siendo el objetivo primario la incidencia de no reflujo (TIMI ≤ 2, o TIMI 3 con TMP < 2). Resultados: Cuarenta y siete pacientes (6.8%) presentaron una alta carga trombo residual reclasificado. La coronaria derecha fue la arteria culpable en 34 casos. Hubo mayor frecuencia de ectasia coronaria en el grupo de stent diferido (p = 0.005). Se encontraron menores tasas de no reflujo en el stent diferido (36% vs. 58%), con una mayor frecuencia de un TMP 3 (p = 0.005). Tras la nueva cateterización un 56% quedó libre de stent en el grupo diferido y la anticoagulación oral les fue más frecuentemente indicada (p = 0.031). La tasa de eventos cardiacos adversos mayores fue similar entre los grupos. Hubo una tendencia a una mejor función ventricular izquierda en el grupo diferido (p = 0.056). Conclusiones: El stent diferido puede ser una alternativa eficiente en pacientes con IAM CEST y alta carga de trombo residual reclasificado, después de conseguir un flujo TIMI 2-3 estable.
