A gravidez na adolescência é um problema significativo de saúde pública no Brasil, contribuindo para altas taxas de morbimortalidade entre mulheres jovens. Durante a gestação, muitas adolescentes abandonam a escola e o trabalho, o que resulta em menor qualificação e maior vulnerabilidade ao mercado de trabalho informal. Além disso, as adolescentes grávidas enfrentam riscos elevados de complicações gestacionais e desafios emocionais, como ansiedade e depressão. A pandemia exacerbou essas vulnerabilidades, especialmente em relação ao abuso doméstico, devido ao isolamento social. Este estudo justifica-se pela necessidade de compreender o perfil demográfico e as características das parturientes adolescentes no Ceará entre 2020 e 2022, oferecendo subsídios para políticas públicas que visem melhorar a qualidade de vida dessas jovens e de seus filhos. Todos os dados foram analisados utilizando R. Durante o período analisado, foram registrados 331.924 nascimentos vivos no Ceará, dos quais 21.667 (6,53%) eram de mães com idade igual ou inferior a 17 anos. A idade média das mães adolescentes foi de 15,93 ± 1,11 anos, enquanto a média para as mães adultas foi de 28,00 ± 6,21 anos, evidenciando uma diferença estatisticamente significativa (Teste-t de Student, P < 0,05). A análise também revelou uma diferença significativa entre a idade dos pais de crianças nascidas de mães adolescentes (22,25 ± 5,72 anos) em comparação à idade dos pais de crianças nascidas de mães adultas (32,45 ± 7,81 anos) (Figura 3, Teste-T, P < 0,05). No que se refere à etnia, adolescentes indígenas representaram 9,95% dos casos de gravidez, seguidas pelas adolescentes pardas (7,26%) e pretas (5,9%). A menor proporção foi observada entre adolescentes brancas (4,43%). Não houve diferença estatisticamente significativa entre os grupos indígenas, pardas, pretas e a população amarela. Quanto ao estado civil, 71% das adolescentes se declararam solteiras. A análise educacional demonstrou uma tendência significativa de gravidez entre adolescentes com Ensino Fundamental II incompleto, completo, ou Ensino Médio incompleto, indicando que essas jovens ainda estavam em fase de conclusão desses ciclos escolares. Não foi observada uma relação significativa entre a redução da idade materna e a diminuição do escore APGAR em recém-nascidos (APGAR = 0,014 × “IDADE DA MÃE” + 8,5; P < 0,05, R² = 0,0004). Além disso, não houve diferença significativa na proporção de anomalias congênitas entre os nascimentos de mães adolescentes e adultas.
El embarazo en la adolescencia es un problema significativo de salud pública en Brasil, que contribuye a altas tasas de morbimortalidad entre las mujeres jóvenes. Durante el embarazo, muchas adolescentes abandonan la escuela y el trabajo, lo que resulta en menor calificación y mayor vulnerabilidad en el mercado laboral informal. Además, las adolescentes embarazadas enfrentan mayores riesgos de complicaciones gestacionales y desafíos emocionales, como ansiedad y depresión. La pandemia exacerbó estas vulnerabilidades, especialmente en relación con el abuso doméstico, debido al aislamiento social. El análisis también reveló una diferencia significativa entre la edad de los padres de niños nacidos de madres adolescentes (22,25 ± 5,72 años) en comparación con la edad de los padres de niños nacidos de madres adultas (32,45 ± 7,81 años) (Prueba de Wilcoxon, P < 0,05). En cuanto a la etnia, las adolescentes indígenas representaron el 9,95% de los casos de embarazo, seguidas por las adolescentes mestizas (7,26%) y negras (5,9%). La menor proporción se observó entre las adolescentes blancas (4,43%). No hubo diferencia estadísticamente significativa entre los grupos indígenas, mestizas, negras y la población amarilla. En cuanto al estado civil, el 71% de las adolescentes se declararon solteras. El análisis educativo demostró una tendencia significativa de embarazo entre adolescentes con secundaria incompleta o completa, o educación media incompleta, lo que indica que estas jóvenes aún estaban en proceso de completar estos niveles educativos. No se observó una relación significativa entre la disminución de la edad materna y la reducción de la puntuación APGAR en los recién nacidos (APGAR = 0,014 × "EDAD DE LA MADRE" + 8,5; P < 0,05, R² = 0,0004). Además, no hubo diferencia significativa en la proporción de anomalías congénitas entre los nacimientos de madres adolescentes y adultas.
Adolescent pregnancy is a significant public health issue in Brazil, contributing to high morbidity and mortality rates among young women. During pregnancy, many adolescents drop out of school and work, leading to lower qualifications and increased vulnerability to the informal labor market. Moreover, pregnant adolescents face higher risks of gestational complications and emotional challenges, such as anxiety and depression. The pandemic exacerbated these vulnerabilities, particularly concerning domestic abuse, due to social isolation. This study is justified by the need to understand the demographic profile and characteristics of adolescent mothers in Ceará between 2020 and 2022, providing support for public policies aimed at improving the quality of life for these young women and their children. All data were analyzed using R. During the analyzed period, 331,924 live births were recorded in Ceará, of which 21,667 (6.53%) were from mothers aged 17 years or younger. The average age of adolescent mothers was 15.93 ± 1.11 years, while the average for adult mothers was 28.00 ± 6.21 years, showing a statistically significant difference (Student's t-test, P < 0.05). The analysis also revealed a significant difference between the age of fathers of children born to adolescent mothers (22.25 ± 5.72 years) compared to the age of fathers of children born to adult mothers (32.45 ± 7.81 years) (Wicoxon test, P < 0.05). Regarding ethnicity, indigenous adolescents accounted for 9.95% of pregnancy cases, followed by brown (7.26%) and black (5.9%) adolescents. The lowest proportion was observed among white adolescents (4.43%). There was no statistically significant difference among indigenous, mixed-race, black, and yellow populations. Concerning marital status, 71% of adolescents declared themselves single. Educational analysis showed a significant trend of pregnancy among adolescents with incomplete or complete middle school or incomplete high school education, indicating that these young women were still in the process of completing these educational levels. There was no significant relationship between decreasing maternal age and a reduction in newborns' APGAR scores (APGAR = 0.014 × "MOTHER'S AGE" + 8.5; P < 0.05, R² = 0.0004). ABSTRACT Introduction Chemotherapy and radiation therapy are considered standard treatments for stage II seminoma patients; however, these therapies are associated with long-term toxicities. Recently, retroperitoneal lymph node dissection has emerged as an alternative strategy, and the first three phase II trials were published in 2023 with promising results. The present study conducted a systematic review and meta-analysis to evaluate this surgery as an alternative treatment for stage IIA/B seminoma patients. Purpose Seminomas are the most common testicular tumors, often affecting young adult males. Standard treatments for stage II seminomas include chemotherapy and radiation therapy, but these therapies are associated with long-term toxicities. Thus, identifying alternative strategies is paramount. Herein, we conducted a systematic review and meta-analysis to appraise the efficacy and safety of retroperitoneal lymph node dissection (RPLND) for treating this condition. Methods We systematically searched the PubMed, Embase, and Cochrane databases for studies evaluating RPLND as a primary treatment for stage II A/B seminomas. Using a random-effects model, single proportion and means and pooled 2-year recurrence-free survival rates with hazard rates and 95% CI were calculated. Results Seven studies were included, comprising 331 males with stage II seminomas. In the pooled analysis, the recurrence rate was 17.69% (95% CI 12.31–24.75), and the 2-year RFS rate was 81% (95% CI 0.77–0.86). The complication rate was 9.16% (95% CI 6.16–13.42), the Clavien–Dindo > 2 complication rate was 8.83% (95% CI 5.76–13.31), and the retrograde ejaculation rate was 7.01% (95% CI 3.54–13.40). The median operative time was 174.68 min (95% CI 122.17–249.76 min), median blood loss was 105.91 mL (95% CI 46.89–239.22 mL), and patients with no evidence of lymph node involvement ranged from 0–16%. Conclusions Primary RPLNDs for treating stage IIA/B seminomas have favorable RFS rates, with low complication and recurrence rates. ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. Abstract Background A cost of illness (COI) study aims to evaluate the socioeconomic burden that an illness imposes on society as a whole. This study aimed to describe the resources used, patterns of care, direct cost, and loss of productivity due to systemic lupus erythematosus (SLE) in Brazil. Methods This 12-month, cross-sectional, COI study of patients with SLE (ACR 1997 Classification Criteria) collected data using patient interviews (questionnaires) and medical records, covering: SLE profile, resources used, morbidities, quality of life (12-Item Short Form Survey, SF-12), and loss of productivity. Patients were excluded if they were retired or on sick leave for another illness. Direct resources included health-related (consultations, tests, medications, hospitalization) or non-health-related (transportation, home adaptation, expenditure on caregivers) hospital resources. Costs were calculated using the unit value of each resource and the quantity consumed. A gamma regression model explored cost predictors for patients with SLE. Results Overall, 300 patients with SLE were included (92.3% female, mean [standard deviation (SD)] disease duration 11.8 [7.9] years), of which 100 patients (33.3%) were on SLE-related sick leave and 46 patients (15.3%) had stopped schooling. Mean (SD) travel time from home to a care facility was 4.4 (12.6) hours. Antimalarials were the most commonly used drugs (222 [74.0%]). A negative correlation was observed between SF-12 physical component and SLE Disease Activity Index (- 0.117, p = 0.042), Systemic Lupus International CollaboratingClinics/AmericanCollegeofRheumatology Damage Index (- 0.115, p = 0.046), medications/day for multiple co-morbidities (- 0.272, p < 0.001), SLE-specific drugs/day (- 0.113, p = 0.051), and lost productivity (- 0.570, p < 0.001). For the mental component, a negative correlation was observed with medications/day for multiple co-morbidities (- 0.272, p < 0.001), SLE-specific medications/day (- 0.113, p = 0.051), and missed appointments (- 0.232, p < 0.001). Mean total SLE cost was US$3,123.53/patient/year (median [interquartile range (IQR)] US$1,618.51 [$678.66, $4,601.29]). Main expenditure was medication, with a median (IQR) cost of US$910.62 ($460, $4,033.51). Mycophenolate increased costs by 3.664 times (p < 0.001), and inflammatory monitoring (erythrocyte sedimentation rate or C-reactive protein) reduced expenditure by 0.381 times (p < 0.001). Conclusion These results allowed access to care patterns, the median cost for patients with SLE in Brazil, and the differences across regions driven by biological, social, and behavioral factors. The cost of SLE provides an updated setting to support the decision-making process across the country. RESUMO O envelhecimento populacional traz consigo inúmeros desafios, especialmente no que diz respeito à saúde cognitiva. Nesse contexto, fatores socioeconômicos como a educação têm recebido atenção especial em razão do seu papel na saúde cerebral. Objetivo: Este estudo teve como objetivo descrever o desempenho das funções executivas, variáveis de saúde mental e qualidade de vida entre idosos saudáveis em relação ao seu nível de educação. Métodos: Foi realizado um estudo descritivo e transversal com participantes de 60 anos ou mais. O protocolo de avaliação incluiu o Exame Cognitivo de Addenbrooke e Teste de Fluência Verbal (FAS), Testes de Trilhas A e B, Escala de Controle, Autonomia, Autorrealização e Prazer (CASP-19), bem como a Escala de Depressão, Ansiedade e Estresse. Resultados: Foram encontradas diferenças significativas no desempenho das funções executivas entre idosos com níveis mais elevados de educação. No entanto, variáveis de saúde mental e qualidade de vida estavam relacionadas apenas à idade dos participantes. ABSTRACT Population aging brings about numerous challenges, particularly concerning cognitive health. In this context, socioeconomic factors such as education have received special attention due to their role in brain health. Objective: The aim of this study was to describe the performance of executive functions, mental health variables, and quality of life among healthy older adults in relation to their level of education. Methods: A descriptive, cross-sectional study was conducted with participants aged 60 or older. ABSTRACT The SISBIOTA-BRASIL was a three-year multimillion-dollar research program of the Brazilian government to document plants and animals in endangered/understudied areas and biomes in Brazil. Distributional patterns and the historical events that generated them are extensively unknown regarding Brazilian fauna and flora. This deficiency hinders the development of conservation policies and the understanding of evolutionary processes. Conservation decisions depend on precise knowledge of the taxonomy and geographic distribution of species. Given such a premise, we proposed to research the diversity of Diptera of the Brazilian western arc of Amazon, Cerrado, and Pantanal in the states of Mato Grosso, Mato Grosso do Sul, and Rondônia. Three important biomes of the South American continent characterize these Brazilian states: Amazon forest, Cerrado (Brazilian Savannah), and Pantanal. Besides their ecological relevance, these biomes historically lack intensive entomological surveys. Therefore, they are much underrepresented in the Brazilian natural history collections and in the scientific literature, which is further aggravated by the fact that these areas are being exponentially and rapidly converted to commercial lands. Our project involved over 90 collaborators from 24 different Brazilian institutions and one from Colombia among researchers, postdocs, graduate and undergraduate students, and technicians. We processed and analyzed nearly 300,000 specimens from ~60 families of Diptera collected with a large variety of methods in the sampled areas. Here, we provide a detailed overview of the genera and species diversity of 41 families treated. Our results point to a total of 2,130 species and 514 genera compiled and identified for the three states altogether, with an increase of 41% and 29% in the numbers of species and genera known for the three states combined, respectively. Overall, the 10 most species-rich families were Tachinidae, Cecidomyiidae, Tabanidae, Psychodidae, Sarcophagidae, Stratiomyidae, Bombyliidae, Syrphidae, Tephritidae, and Asilidae. The 10 most diverse in the number of genera were Tachinidae, Stratiomyidae, Asilidae, Mycetophilidae, Syrphidae, Tabanidae, Muscidae, Dolichopodidae, Sarcophagidae, and Chloropidae. So far, 111 scientific papers were published regarding taxonomic, phylogenetic, and biogeographical aspects of the studied families, with the description of 101 new species and three new genera. We expect that additional publications will result from this investigation because several specimens are now curated and being researched by specialists. RESUMO OBJETIVO Identificar características relacionadas ao estilo de vida, sociodemográficas e saúde mental de pessoas com sintomas de insônia e pessoas sem insônia durante a pandemia. MÉTODOS A partir de dados coletados por amostragem em bola de neve, por meio de um questionário online foi realizado um estudo caso-controle. Durante o período de novembro de 2020 a abril de 2021, 6.360 pessoas com idade média de 43,5 anos (DP = 14,3) participaram da pesquisa. No presente estudo, foram considerados 158 casos de transtorno de insônia e 476 controles (três controles por caso) selecionados aleatoriamente dentre os participantes sem problemas de sono. RESULTADOS Os resultados da análise comparativa entre casos e controles mostraram que dormir menos de seis horas diárias (OR = 3,89; IC95% 2,50–6,05), sentir tristeza frequentemente (OR = 2,95; IC95% 1,69–5,17), residir em metrópoles (OR = 1,71; IC95% 1,04–2,84), estar com 40 anos ou mais (OR = 1,93; IC95% 1,22–3,06) e a interação entre ocupação e escolaridade mais precária (OR = 2,12; IC95% 1,22–3,69) foram fatores preditores para sintomas de transtorno de insônia durante a pandemia. ABSTRACT OBJECTIVE To identify lifestyle-related, sociodemographic, and mental health characteristics of people with insomnia symptoms and people without insomnia during the pandemic. METHODS A case-control study was conducted with data collected by snowball sampling using an online questionnaire. From November 2020 to April 2021, 6,360 people with a mean age of 43.5 years (SD = 14.3) participated in the survey. For this study, we considered 158 cases of insomnia disorder and 476 controls (three controls per case) randomly selected from the participants without sleep problems. RESULTS The results of the comparative analysis between cases and controls showed that sleeping less than six hours daily (OR = 3.89; 95%CI 2.50–6.05), feeling sadness frequently (OR = 2.95; 95%CI 1.69–5.17), residing in metropolitan areas (OR = 1.71; 95%CI 1.04–2.84), being 40 years or older (OR = 1.93; 95%CI 1.22–3.06), and the interaction between occupation and poorer education (OR = 2.12; 95%CI 1.22–3.69) were predictors for symptoms of insomnia disorder during the pandemic. Abstract The Santos Basin (SB) is the main petroliferous basin in the Brazilian continental margin and one of the most studied marine areas in Brazil. However, historical data suggest that new efforts should be carried out to acquire quantitative biological data, especially in the deep sea, to establish the baseline of essential ocean variables in different ecosystems for future monitoring programs. The Brazilian energy company Petrobras planned and executed 24 oceanographic cruises over a period of 2 years to assess the benthic (SANSED cruise) and pelagic (SANAGU cruise) systems of the SB (356 days at sea in 2019 and 2021/2022). These efforts were part of the Santos Project, which comprised a comprehensive environmental study aimed at investigating benthic and pelagic variables to characterize ecology, biogeochemistry, thermohaline properties of water masses, and ocean circulation patterns, geomorphology, and sedimentology, as well as organic and inorganic chemistry. Here we present the detailed sampling designs and the field methods employed on board, during the SB scientific cruises. All sampling protocols were based on standardized approaches. For the benthos analyses, triplicate sediment samples were performed using a GOMEX-type box corer (0.25 m²) or a large modified Van Veen grab (0.75 m²) at 100 stations ranging from 25 to 2400 m depth. At each station, 25 geochemical and physico-chemical parameters were analyzed in addition to micro-, meio-, and macrofauna and living foraminifera samples. For the pelagic system, 60 stations were selected to investigate the plankton community, ranging in size from pico- to macroplankton, through vertical, horizontal, and oblique net hauls (20, 200, and 500 μm mesh size), as well as 25 biogeochemical parameters collected with an aid of a CTD-rosette sampler. Part of this scientific information also serves the Regional Environmental Characterization Project (PCR-BS) in support of Petrobras’ Santos Basin drilling licensing process led by the Brazilian Environmental Agency – IBAMA. This project contributes to the sustainable development of the SB, in line with the guidelines of the United Nations Decade of Ocean Science for Sustainable Development. ABSTRACT The effects of diets containing oilseeds were measured to evaluate the productive and economic parameters in the finishing of young, feedlot Nellore bulls. Twenty-four young Nellore bulls were used, with an initial body weight (BW) of 311.46±0.37 kg and 24 months of age, distributed into individual stalls ( 4 × 20 m) in a completely randomized design, totaling four treatments with six repetitions per treatment. Four diets (control, based on corn and soybean meal, and three diets containing cottonseed, soybean, and sunflower) were evaluated. Feed and orts were measured daily to calculate intake and costs. The dry matter intake of the control group was higher than soybean (10.64 kg/day), cotton (9.88 kg/day), and sunflower (9.30 kg/day) treatments, respectively. The cottonseed treatment showed the highest average neutral detergent fiber intake. There was a dietary effect of diets on average daily gain, total weight gain, and final weight. The soybean treatment showed the highest performance, total gain (232.55 kg), and final weight (544.38 kg). Oilseed intake can modify the fatty acids profile in the meat, decreasing its saturated fatty acid content. Whole soybean seed favors performance, improves feed efficiency, fatty acid profile, and fat distribution in the carcass, and can reduce production costs. CONCLUSÕES Além da confirmação da hipótese de que problemas de saúde mental estão associados a sintomas de insônia, os resultados apontam para a insônia como um desfecho importante para estudos sobre efeitos do desemprego, vulnerabilidade e baixa escolaridade da população, sobretudo nas grandes metrópoles, ressaltando que os efeitos da crise sobre a saúde e a economia são distribuídos de forma extremamente desiguais.
ABSTRACT OBJECTIVE To identify lifestyle-related, sociodemographic, and mental health characteristics of people with insomnia symptoms and people without insomnia during the pandemic. METHODS A case-control study was conducted with data collected by snowball sampling using an online questionnaire. From November 2020 to April 2021, 6,360 people with a mean age of 43.5 years (SD = 14.3) participated in the survey. For this study, we considered 158 cases of insomnia disorder and 476 controls (three controls per case) randomly selected from the participants without sleep problems. RESULTS The results of the comparative analysis between cases and controls showed that sleeping less than six hours daily (OR = 3.89; 95%CI 2.50–6.05), feeling sadness frequently (OR = 2.95; 95%CI 1.69–5.17), residing in metropolitan areas (OR = 1.71; 95%CI 1.04–2.84), being 40 years or older (OR = 1.93; 95%CI 1.22–3.06), and the interaction between occupation and poorer education (OR = 2.12; 95%CI 1.22–3.69) were predictors for symptoms of insomnia disorder during the pandemic. CONCLUSIONS In addition to confirming the hypothesis that mental health problems are associated with insomnia symptoms, the results point to insomnia as an important outcome for studies on the effects of unemployment, vulnerability and low education of the population, especially in large cities, highlighting that the effects of the crisis on health and the economy are extremely unequally distributed.

ABSTRACT Introduction: Existing literature presents varying perspectives on the impact of COVID-19 on liver transplant recipients. However, no research has specifically investigated the role of gender differences in the manifestation of COVID-19 among liver transplant recipients. This study aims to examine the effects of COVID-19 on liver transplant recipients, with a focus on gender differences in disease presentation and progression. Methods: Conducted as a multicenter historical cohort study, this research collected patient records through an online questionnaire. Assessing COVID-related mortality was the main objective. Additionally, demographic, clinical, and laboratory data pertaining to disease presentation and progression were collected. Results: The study included a total of 283 patients, of whom 76 were female and 206 were male. The median follow-up period for males was 99 days (IQR 38-283), while for females, it was 126 days (IQR 44-291). A higher prevalence of cardiovascular disease was observed in males (p=0.002). Females frequently experienced a loss of smell (p=0.021), whereas males commonly exhibited fever (p=0.031). Levels of ALT and gamma-glutamyl transferase were significantly elevated in males (p=0.008 and 0.004, respectively). Although there was a trend towards increased mortality in males, it did not reach statistical significance. Conclusion: This study is the first attempt to investigate gender differences in COVID-19 among liver transplant recipients. Our findings highlight the need for a comprehensive and personalised approach to treating this patient population and underscore the importance of further elucidating the disease presentation in these individuals. RESUMO Introdução: A literatura existente apresenta perspectivas variadas sobre o impacto da COVID-19 em receptores de transplante de fígado. No entanto, nenhuma pesquisa investigou especificamente o papel das diferenças de gênero na manifestação da COVID-19 entre os receptores de transplante de fígado. Este estudo pretende examinar os efeitos da COVID-19 em receptores de transplante de fígado, com foco nas diferenças de gênero na apresentação e progressão da doença. Métodos: Conduzida como um estudo de coorte histórico multicêntrico, esta pesquisa coletou registros de pacientes por meio de um questionário on-line. O principal objetivo foi avaliar a mortalidade relacionada à COVID. Além disso, foram coletados dados demográficos, clínicos e laboratoriais relativos à apresentação e progressão da doença. Resultados: O estudo incluiu um total de 283 pacientes, sendo 76 do sexo feminino e 206 do sexo masculino. O período médio de acompanhamento para os homens foi de 99 dias (IQR 38-283), enquanto para as mulheres foi de 126 dias (IQR 44-291). Foi observada uma maior prevalência de doença cardiovascular nos homens (p=0,002). As mulheres frequentemente apresentavam perda de olfato (p=0,021), enquanto os homens geralmente apresentavam febre (p=0,031). Os níveis de ALT e gama-glutamil transferase foram significativamente elevados nos homens (p=0,008 e 0,004, respectivamente). Embora tenha havido uma tendência de aumento da mortalidade nos homens, ela não alcançou significância estatística. This guideline aimed to update the concepts and formulate the standards of conduct and scientific evidence that support them, regarding the diagnosis and treatment of the Cardiomyopathy of Chagas disease, with special emphasis on the rationality base that supported it.nbsp;
Chagas disease in the 21st century maintains an epidemiological pattern of endemicity in 21 Latin American countries. Researchers and managers from endemic and non-endemic countries point to the need to adopt comprehensive public health policies to effectively control the interhuman transmission of T. cruzi infection, and to obtain an optimized level of care for already infected individuals, focusing on diagnostic and therapeutic opportunistic opportunities.

Pathogenic and pathophysiological mechanisms of the Cardiomyopathy of Chagas disease were revisited after in-depth updating and the notion that necrosis and fibrosis are stimulated by tissue parasitic persistence and adverse immune reaction, as fundamental mechanisms, assisted by autonomic and microvascular disorders, was well established. Some of them have recently formed potential targets of therapies.nbsp;
The natural history of the acute and chronic phases was reviewed, with enhancement for oral transmission, indeterminate form and chronic syndromes. Recent meta-analyses of observational studies have estimated the risk of evolution from acute and indeterminate forms and mortality after chronic cardiomyopathy. Therapeutic approaches applicable to individuals with Indeterminate form of Chagas disease were specifically addressed. All methods to detect structural and/or functional alterations with various cardiac imaging techniques were also reviewed, with recommendations for use in various clinical scenarios. Mortality risk stratification based on the Rassi score, with recent studies of its application, was complemented by methods that detect myocardial fibrosis.nbsp;
The current methodology for etiological diagnosis and the consequent implications of trypanonomic treatment deserved a comprehensive and in-depth approach. Also the treatment of patients at risk or with heart failure, arrhythmias and thromboembolic events, based on pharmacological and complementary resources, received special attention. Additional chapters supported the conducts applicable to several special contexts, including t. cruzi/HIV co-infection, risk during surgeries, in pregnant women, in the reactivation of infection after heart transplantation, and others.nbsp; nbsp;nbsp;
Finally, two chapters of great social significance, addressing the structuring of specialized services to care for individuals with the Cardiomyopathy of Chagas disease, and reviewing the concepts of severe heart disease and its medical-labor implications completed this guideline.
Esta diretriz teve como objetivo principal atualizar os conceitos e formular as normas de conduta e evidências científicas que as suportam, quanto ao diagnóstico e tratamento da CDC, com especial ênfase na base de racionalidade que a embasou.
A DC no século XXI mantém padrão epidemiológico de endemicidade em 21 países da América Latina. Investigadores e gestores de países endêmicos e não endêmicos indigitam a necessidade de se adotarem políticas abrangentes, de saúde pública, para controle eficaz da transmissão inter-humanos da infecção pelo T. cruzi, e obter-se nível otimizado de atendimento aos indivíduos já infectados, com foco em oportunização diagnóstica e terapêutica.
Mecanismos patogênicos e fisiopatológicos da CDC foram revisitados após atualização aprofundada e ficou bem consolidada a noção de que necrose e fibrose sejam estimuladas pela persistência parasitária tissular e reação imune adversa, como mecanismos fundamentais, coadjuvados por distúrbios autonômicos e microvasculares. Alguns deles recentemente constituíram alvos potenciais de terapêuticas.
A história natural das fases aguda e crônica foi revista, com realce para a transmissão oral, a forma indeterminada e as síndromes crônicas. Metanálises recentes de estudos observacionais estimaram o risco de evolução a partir das formas aguda e indeterminada e de mortalidade após instalação da cardiomiopatia crônica. Condutas terapêuticas aplicáveis aos indivíduos com a FIDC foram abordadas especificamente. Todos os métodos para detectar alterações estruturais e/ou funcionais com variadas técnicas de imageamento cardíaco também foram revisados, com recomendações de uso nos vários cenários clínicos. Estratificação de risco de mortalidade fundamentada no escore de Rassi, com estudos recentes de sua aplicação, foi complementada por métodos que detectam fibrose miocárdica.
A metodologia atual para diagnóstico etiológico e as consequentes implicações do tratamento tripanossomicida mereceram enfoque abrangente e aprofundado. Também o tratamento de pacientes em risco ou com insuficiência cardíaca, arritmias e eventos tromboembólicos, baseado em recursos farmacológicos e complementares, recebeu especial atenção. Capítulos suplementares subsidiaram as condutas aplicáveis a diversos contextos especiais, entre eles o da co-infecção por T. cruzi/HIV, risco durante cirurgias, em grávidas, na reativação da infecção após transplante cardíacos, e outros.nbsp;nbsp;nbsp;
Por fim, dois capítulos de grande significado social, abordando a estruturação de serviços especializados para atendimento aos indivíduos com a CDC, e revisando os conceitos de cardiopatia grave e suas implicações médico-trabalhistas completaram esta diretriz.nbsp;

Mechanical performance of original; yellowish and blueish ProFile instruments: isolating heat-treatment as a variable

Resumo O presente estudo teve como objetivo realizar dois tratamentos térmicos diferentes em instrumentos de NiTi austenítico ProFile e comparar o desempenho mecânico de instrumentos originais e tratados termicamente. O tratamento térmico dos instrumentos ProFile (tamanho de ponta 25 e conicidade 0.06) foi realizado em um forno em atmosfera de argônio usando uma taxa de aquecimento de 10°C/min. Após atingir as temperaturas programadas de 450 ºC ou 500 ºC o sistema permaneceu em temperatura constante por 10 minutos; seguido de resfriamento em água à temperatura ambiente. Em seguida; os três grupos de instrumentos (n=30 por grupo) foram comparados quanto à fadiga cíclica (n=10 por grupo); flexão (n=10 por grupo) e resistência à flambagem (n=10 por grupo). Após testes de fadiga cíclica; um microscópio eletrônico de varredura foi utilizado para analisar as superfícies de fratura e observar o modo de fratura. A análise estatística foi realizada por meio dos testes de One-way ANOVA e teste de Student-Newman-Keuls; com erro de tipo alfa fixado em 0.05. Coloração amarelada e azulada foi observada nos instrumentos ProFile após tratamentos térmicos a 450 ºC ou 500 ºC; respectivamente. Os instrumentos ProFile originais apresentaram menor fadiga cíclica e maior resistência à flexão e flambagem (P<0.05). Em contraste; os instrumentos ProFile azulados (tratados termicamente a 500°C) apresentaram maior fadiga cíclica e menor resistência à flexão e flambagem (P>0.05). Pode-se concluir que os diferentes tratamentos térmicos realizados nos instrumentos ProFile aumentaram sua resistência à fadiga cíclica e melhoraram a flexibilidade e resistência à flambagem.
Abstract The present study aimed to perform two different heat-treatments in an austenitic NiTi ProFile instrument and to compare the mechanical performance of original and heat-treated instruments. Heat treatment of ProFile (tip size 25 and 0.06 taper) instruments were carried out in a furnace in argon atmosphere using a heating rate of 10° C/min. After reaching the programmed temperatures of 450 ºC or 500 ºC the system remained at a constant temperature for 10 minutes; followed by cooling in water at room temperature. Afterwards; the three groups (n=30 per group) of instruments were compared regarding their cyclic fatigue (n=10 per group); bending (n=10 per group); and buckling resistance (n=10 per group). After cyclic fatigue tests; a scanning electron microscope was used to analyze the fracture surfaces and observe the fracture mode. Statistical analysis was performed using One-way ANOVA and Student-Newman-Keuls test; with an alpha type error set at 0.05. Yellowish and blueish coloration was observed in the ProFile instruments after 450 ºC or 500 ºC heat treatments; respectively. Conventional ProFile instruments showed the lower cyclic fatigue; and the higher bending and buckling resistance (P<0.05). In contrast; yellowish ProFile instruments (heat treated at 500° C) showed the higher cyclic fatigue; and the lower bending and buckling resistance (P>0.05). It can be concluded that the different heat treatments performed on ProFile instruments increased its cyclic fatigue resistance and improved the flexibility and buckling resistance.
