Resumo Introdução: O papel da tomografia computorizada torácica de ultrabaixa dose (ULDCT) em contexto de urgência é desconhecido. Objetivo: Este estudo avaliou a viabilidade da ULDCT em adultos na urgência com suspeita de pneumonia. Foram medidos a dose de radiação, o tempo entre a solicitação e os relatórios do exame, prescrições de antimicrobianos também foram correlacionadas. Métodos: Este estudo avaliou a viabilidade da ULDCT em adultos na urgência com suspeita de pneumonia. Foram medidas a dose de radiação, o tempo entre a solicitação e a disponibilização dos relatórios do exame, e também foram correlacionadas as prescrições de antimicrobianos. Entre outubro de 2017 e dezembro de 2018, incluímos prospectivamente adultos com suspeita de pneumonia adquirida na comunidade, sem diagnóstico definitivo por avaliação clínica. Os critérios de exclusão abrangeram condições clínicas que pudessem afetar a interpretação da imagem. Resultados: Foram incluídos 131 pacientes elegíveis (idade média de 43,5 anos; 57% homens) encaminhados para ULDCT. Dezoito (13,7%) foram excluídos. A dose média de radiação da ULDCT foi de 0,14 mSv. O intervalo de tempo entre a solicitação médica e o resultado da ULDCT foi de 43 minutos. Conclusão: A ULDCT mostrou-se viável para pacientes adultos com suspeita de pneumonia em contexto de urgência. Os relatórios foram disponibilizados num intervalo razoável e os resultados negativos foram associados a uma baixa taxa de prescrição de antibióticos.
Abstract Background: The role of ultra-low-dose chest CT (ULDCT) in the emergency department is unknown. Purpose: This study sought to examine feasibility of ULDCT in adults presenting to the emergency department for whom pneumonia was suspected. Radiation dose and time interval between medical request and ULDCT results were measured, antibiotic prescriptions were correlated. Methods: Between October 2017 and December 2018, we prospectively enrolled consecutive healthy adult patients in a single-center for whom community-acquired pneumonia was suspected and for whom no definitive diagnosis was possible by clinical judgment. Exclusion criteria comprised all chronic conditions that could impact severity and interpretation of image. Results: 131 eligible patients (mean age 43.5 years; 57% men) were enrolled to ULDCT. Eighteen (13.7%) patients were excluded. Average CT radiation dose was 0.14 mSv. Time interval between medical request and ultra-low-dose chest CT result was 43 minutes. Only 15.9% of patients with negative CT received antibiotics. Conclusion: Ultra-low-dose chest CT was feasible for almost all patients in a group of healthy adults presenting to the emergency department with suspected community-acquired pneumonia. Results were made available in a reasonable interval, and negative ones were associated with a low antibiotic prescription rate.

ABSTRACT Single nucleotide polymorphisms are variations of a single nucleotide base pair and can be associated to phenotypic characteristics. This study aimed to determine the association of CAPN1 and CAST gene polymorphisms with the tenderness of Creole cattle meat from the Amazonas region, Peru. The texture profile (adhesiveness, cohesiveness, Warner-Bratzler shear force, elasticity, gumminess, and chewiness) of 100 animals was determined in 100 g of Longissimus dorsi et lumborum muscle. Allelic frequencies, genotypic frequencies, and Hardy-Weinberg equilibrium (HWE) of calpain (CAPN-316, CAPN-530) and calpastatin (CAST-2959) gene polymorphisms were studied. Allelic and genotypic frequencies were calculated, as well as the Hardy-Weinberg equilibrium with the Chi-square test. The texture profile of each group of samples corresponding to a polymorphism was compared with the Duncan’s test and the t-test for independent samples (p < 0.05). Genotypic frequencies were 78 % GG and 22 % CC for CAPN-316; 68 % GG, 5 % GA, and 27 % AA for CAPN-530; and 74 % AA, 18 % AG, and 8 % GG for CAST-2959. The CAPN-316, CAPN-530, and CAST-2959 polymorphisms were not in Hardy-Weinberg equilibrium. The CC genotype of CAPN-316 marker influences meat tenderness on day 21 of meat aging. ABSTRACT In 2000, Cleide Costa published a paper presenting the state of knowledge of the Neotropical Coleopte ra, with a focus on the Brazilian fauna. Twenty-four years later, thanks to the development of the Coleoptera section of the Taxonomic Catalog of the Brazilian Fauna (CTFB - Catálogo Taxonômico da Fauna do Brasil) through the collaboration of 100 coleopterists from all over the globe, we can build on Costa’s work and present an updated overview of the state of knowledge of the beetles from Brazil. There are currently 35,699 species in 4,958 genera and 116 families known to occur in the country, including representatives of all extant suborders and superfamilies. Our data show that the Brazilian beetle fauna is the richest on the planet, concentrating 9% of the world species diversity, with some estimates accounting to up to 15% of the global total. The most diverse family in numbers of genera is Cerambycidae (1,056 genera), while in number of species it is Chrysomelidae (6,079 species). Conotrachelus Dejean, 1835 (Curculionidae) is the most species-rich genus, with 570 species. The French entomologist Maurice Pic is the author who has contributed the most to the naming of species recorded from Brazil, with 1,794 valid names in 36 families, whereas the Brazilians Ubirajara R. Martins and Maria Helena M. Galileo are the only ones among the top-ten authors to have named species in the 21st century. ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. Resumo Os mamíferos são organismos carismáticos que desempenham um papel fundamental na função ecológica e nos serviços ecossistêmicos, como polinização, dispersão de sementes, ciclagem de nutrientes e controle de pragas. O Estado de São Paulo representa apenas 3% do território brasileiro, mas detém 33% da diversidade de mamíferos. A maior parte de seu território é dominado pela agricultura, pastagens e áreas urbanas que afetam diretamente a diversidade e a persistência dos mamíferos na paisagem. Além disso, São Paulo possui o maior porto da América Latina e o maior reservatório de petróleo costeiro do Brasil, com 600 km de extensão de litoral com diversas espécies de mamíferos marinhos. Essas infraestruturas afetam a diversidade, distribuição, ecologia e o futuro dos mamíferos no estado. Aqui, respondemos cinco perguntas principais: 1) Qual é a diversidade de mamíferos silvestres no Estado de São Paulo? 2) Onde eles ocorrem? 3) Qual é o seu impacto positivo e negativo no bem-estar humano? 4) Como os mamíferos persistem em paisagens modificadas pelo homem? 5) Qual é o futuro dos mamíferos no estado? O estado de São Paulo possui 255 espécies de mamíferos nativos, com quatro espécies endêmicas, duas delas globalmente ameaçadas de extinção. Pelo menos seis espécies (dois marsupiais, ariranha, veado-campeiro, veado-cambuta e tatu-canastra) foram extirpadas do estado devido à caça e perda de habitat. O intenso uso humano da terra no estado forçou muitas espécies de mamíferos a mudar sua dieta para lidar com a intensa fragmentação e agricultura. A monocultura em larga escala facilitou a invasão de espécies exóticas, como porcos selvagens (javaporco) e a lebre europeia. Várias espécies de áreas abertas estão expandindo suas áreas de distribuição (lobo-guará, veado-catingueiro) sobre áreas desmatadas e provavelmente refletem mudanças em direção a um clima mais seco. Como o estado possui o maior sistema rodoviário do Brasil, cerca de 40 mil mamíferos de 33 espécies são mortos por ano em colisões, causando um prejuízo econômico de 12 milhões de dólares/ano. A diversidade de mamíferos está concentrada nos maiores remanescentes florestais da Serra do Mar e no interior do Estado, principalmente nas regiões de Ribeirão Preto e Jundiaí. As lacunas amostrais estão concentradas em todo o interior do estado, principalmente na região noroeste. Os mamíferos silvestres desempenham um papel fundamental em muitos serviços ecossistêmicos, mas também podem ser uma preocupação em trazer novas doenças emergentes para as populações humanas. Embora a taxonomia de mamíferos pareça ser bem conhecida, mostramos que novas espécies estão sendo continuamente descobertas no estado. Portanto, pesquisas usando tecnologias tradicionais e novas (eDNA, iDNA, drones), monitoramento populacional de longo prazo, a investigação da interface do conflito homem-vida selvagem e a compreensão do papel único no ecossistema desempenhado pelos mamíferos são um caminho futuro para promover uma paisagem verde sustentável aliada ao bem-estar humano no estado. O plantio de corredores florestais ou de cerrado, principalmente junto aos principais sistemas fluviais, no planalto, o controle da caça ilegal nas áreas costeiras, o manejo dos regimes de fogo no Cerrado e a mitigação dos atropelamentos devem ser uma prioridade para proteger essa notável diversidade de mamíferos.
Abstract Mammals are charismatic organisms that play a fundamental role in ecological functions and ecosystem services, such as pollination, seed dispersal, nutrient cycling, and pest control. The state of São Paulo represents only 3% of the Brazilian territory but holds 33% of its mammalian diversity. Most of its territory is dominated by agriculture, pastures, and urban areas which directly affect the diversity and persistence of mammals in the landscape. In addition, São Paulo has the largest port in Latin America and the largest offshore oil reservoir in Brazil, with a 600 km stretch of coastline with several marine mammal species. These human-made infrastructures affect the diversity, distribution, ecology, and the future of mammals in the state. Here, we answer five main questions: 1) What is the diversity of wild mammals in São Paulo state? 2) Where are they? 3) What is their positive and negative impact on human well-being? 4) How do mammals thrive in human-modified landscapes? 5) What is the future of mammals in the state? The state of São Paulo holds 255 species of native mammals, with four endemic species, two of them globally endangered. At least six species (two marsupials, Giant otter, Pampas deer, Brazilian dwarf brocket deer, and Giant armadillo) were extirpated from the state due to hunting and habitat loss. The intense human land use in the state forced many mammalian species to change their diet to cope with the intense fragmentation and agriculture. Large-scale monoculture has facilitated the invasion of exotic species such as wild boars (javali) and the European hare. Several “savanna-dwelling” species are expanding their ranges (Maned wolf, Brocket deer) over deforested areas and probably reflect changes towards a drier climate. Because the state has the largest road system, about 40,000 mammals from 33 species are killed per year in collisions causing an economic loss of 12 million dollars/year. The diversity of mammals is concentrated in the largest forest remnants of Serra do Mar and in the interior of the State, mainly in the regions of Ribeirão Preto and Jundiaí. Sampling gaps are concentrated throughout the interior of the state, particularly in the northwest region. Wild mammals play a fundamental role in many ecosystem services, but they can also be a concern in bringing new emergent diseases to humans. Although the taxonomy of mammals seems to be well known, we show that new species are continuously being discovered in the state. Therefore, continuous surveys using traditional and new technologies (eDNA, iDNA, drones), long-term population monitoring, investigation of the interface of human-wildlife conflict, and understanding of the unique ecosystem role played by mammals are future avenues for promoting sustainable green landscapes allied to human well-being in the state. The planting of forest or savanna corridors, particularly along with major river systems, in the plateau, controlling illegal hunting in the coastal areas, managing fire regimes in the Cerrado, and mitigating roadkill must be prioritized to protect this outstanding mammal diversity.

RESUMO O uso de modelos para estimar a produção florestal é uma importante ferramenta em áreas plantadas. Embora esse assunto tenha sido estudado em todo o mundo, ainda falta conhecimento a respeito da medição de volume para locais específicos, como os do Nordeste do Brasil. Desta forma, objetivou-se com este estudo avaliar o potencial de predição de redes neurais artificiais e regressão para a estimativa do volume de madeira em povoamentos homogêneos de Anadantera macrocarpa, Genipa americana e Mimosa caesalpiniflolia. Os métodos de regressão e de redes neurais artificiais (RNAs) mostraram-se aplicáveis para a estimativa do volume individual dos povoamentos em diferentes espaçamentos, aos sete anos de idade. O modelo de regressão de Spurr apresentou melhores resultados estatísticos e dispersão dos erros não tendenciosos para as espécies Anadantera macrocarpa e Genipa americana. Já o modelo de Shumacher-Hall foi mais preciso para a estimativa do volume da espécie Mimosa caesalpinifolia. As RNAs, com dois neurônios na camada intermediária, proporcionaram melhores ajustes para as três espécies, portanto, são recomendadas para estimar os volumes individuais das espécies avaliadas, por mostrar maior precisão, em relação à regressão, na estimativa do volume das espécies nativas avaliadas.
ABSTRACT Modeling is an important tool to estimate forest production in planted areas. Although this issue has been studied worldwide, knowledge regarding volume measurement in specific locations such as Northeast Brazil is still scarce. The present study aimed to evaluated the effectiveness of artificial neural networks (ANNs) and regression analysis in estimating the timber volume of homogeneous stands of Anadantera macrocarpa, Genipa americana, and Mimosa casalpinifolia, in order to better predict the growth and production of these species. Both methods were suitable for estimating the individual volume in 7-year-old stands with different spacing. The Spurr regression model showed better statistical results and dispersion of unbiased errors for Anadantera macrocarpa and Genipa americana, whereas the Shumacher-Hall model provided more accurate volume estimates for Mimosa caesalpinifolia. The ANNs calibrated with two neurons in the middle layer exhibited the best fit for all three species. As such, artificial neural networks can be recommended to estimate the individual volumes of the species analyzed in the study area.

ABSTRACT In recent years the immunomodulatory actions of vitamin D, a steroid hormone, have been extensively studied. In 2020, due to the COVID-19 pandemic, the question arose as to 25(OH)D status would be related to susceptibility to SARS-CoV-2 infection, since several studies pointed out a higher prevalence and severity of the disease in populations with low levels of 25(OH)D. Thus, we investigated the 25(OH)D levels in adults “Detected” positive for SARS CoV-2 by RT-PCR (reverse transcriptase polymerase chain reaction) test, and in negative controls, “not Detected”, using the Fleury Group's examination database, in Sao Paulo, Brazil. Of a total of 14.692 people with recent assessments of 25(OH)D and RT-PCR tests for COVID-19, 2.345 were positive and 11.585 were negative for the infection. The groups did not differ in the percentage of men and women, or in the age distribution. There were no differences in the distribution of 25(OH)D between the two groups (p = 0.08); mean 25(OH)D of 28.8 ± 21.4 ng/mL and 29.6 ± 18.1 ng/mL, respectively. In the specific population studied, clinical, environmental, socioeconomic and cultural factors should have greater relevance than 25(OH)D in determining the susceptibility to COVID-19.

ABSTRACT Objective: Choosing Wisely (CW) is an initiative that aims to advance the dialogue between physicians and patients about low-value health interventions. Given that thyroid conditions are frequent in clinical practice, we aimed to develop an evidence-based list of thyroid CW recommendations. Materials and methods: The Thyroid Department of the Brazilian Society of Endocrinology and Metabolism (SBEM) named a Task Force to conduct the initiative. The Task Force work was based on an electronic Delphi approach. The 10 recommendations that received the highest scores by the Task Force were submitted for voting by all SBEM associates. The 5 recommendations that received the highest scores by SBEM associates are presented herein. Results: The Task Force was composed of 14 thyroidologists from 10 tertiary-care, teaching-based Brazilian institutions. The brainstorming/ideation phase resulted in 69 recommendations. After the removal of duplicates and recommendations that did not adhere to the initiative's scope, 35 remained. Then the Task Force voted to attribute a grade (0 [lowest agreement] to 10 [highest agreement]) for each recommendation. The 10 recommendations that received the highest scores by the Task Force were submitted to all SBEM associates. A total of 683 associates voted electronically, attributing a grade (0 to 10) for each recommendation. The 5 recommendations that received the highest scores by the SBEM associates compose our final list. Conclusion: A set of recommendations to avoid unnecessary medical tests, treatments, or procedures for thyroid conditions are offered with a transparent methodology. This initiative aims to foster productive interactions between physicians and patients, stimulating shared decision-making.

The sulfur related thermochemical water-splitting cycles are an important class of chemical processes considered for hydrogen production. Recently, the magnesium sulfate thermal decomposition has been reported as a potential unit operation in one of these cycles. Therefore, some interest has been observed in the use of catalysts to lower the activation energy of such reactions. In this context, the present manuscript reports the thermodynamics and kinetics modeling results associated with this reaction system in the presence of a Pd catalyst supported over γ-Al2O3. The presence of such species is responsible for shifting the decomposition temperature to lower values in at least 100 °C. It was observed that the magnesium content is still oriented towards MgO formation. The obtained results indicate that the Pd/Al2O3 catalyst could be a good alternative in reducing the thermal decomposition temperature as its presence was responsible for diminishing the process activation energy from 368.2 to 258.8 kJ.mol−1.

OBJECTIVES: We designed a cohort study to describe characteristics and outcomes of patients with coronavirus disease (COVID-19) admitted to the intensive care unit (ICU) in the largest public hospital in Sao Paulo, Brazil, as Latin America becomes the epicenter of the pandemic. METHODS: This is the protocol for a study being conducted at an academic hospital in Brazil with 300 adult ICU beds dedicated to COVID-19 patients. We will include adult patients admitted to the ICU with suspected or confirmed COVID-19 during the study period. The main outcome is ICU survival at 28 days. Data will be collected prospectively and retrospectively by trained investigators from the hospital’s electronic medical records, using an electronic data capture tool. We will collect data on demographics, comorbidities, severity of disease, and laboratorial test results at admission. Information on the need for advanced life support and ventilator parameters will be collected during ICU stay. Patients will be followed up for 28 days in the ICU and 60 days in the hospital. We will plot Kaplan-Meier curves to estimate ICU and hospital survival and perform survival analysis using the Cox proportional hazards model to identify the main risk factors for mortality. RESULTS: We expect to include a large sample of patients with COVID-19 admitted to the ICU and to be able to provide data on admission characteristics, use of advanced life support, ICU survival at 28 days, and hospital survival at 60 days. CONCLUSIONS: This study will provide epidemiological data about critically ill patients with COVID-19 in Brazil, which could inform health policy and resource allocation in low- and middle-income countries.

Abstract: Here we model and describe the wood volume of Cerrado Sensu Stricto, a highly heterogeneous vegetation type in the Savanna biome, in the state of Minas Gerais, Brazil, integrating forest inventory data with spatial-environmental variables, multivariate regression, and regression kriging. Our study contributes to a better understanding of the factors that affect the spatial distribution of the wood volume of this vegetation type as well as allowing better representation of the spatial heterogeneity of this biome. Wood volume estimates were obtained through regression models using different environmental variables as independent variables. Using the best fitted model, spatial analysis of the residuals was carried out by selecting a semivariogram model for generating an ordinary kriging map, which in turn was used with the fitted regression model in the regression kriging technique. Seasonality of both temperature and precipitation, along with the density of deforestation, explained the variations of wood volume throughout Minas Gerais. The spatial distribution of predicted wood volume of Cerrado Sensu Stricto in Minas Gerais revealed the high variability of this variable (15.32 to 98.38 m3 ha-1) and the decreasing gradient in the southeast-northwest direction.

Abstract Developing a less invasive, practical and cost-effective operative technique for obesity treatment represents a pressing need for our society. In this way, intragastric single port sleeve by endoplication was tested in six pigs during 18 weeks. Celiotomy was performed with animal placed in dorsal decubitus position. Single port gastrostomy was performed and double tobacco pouch sutures were made in fundic region, making a gastric sleeve. At the end, stomach layers and skin were closed in a conventional manner. Means and the standard deviations of surgical time were calculated. The procedure was simple and all animals survived; there were no significant blood loss and no intra and postoperative complications. The procedure was fast (67.4 minutes). The technique has the advantage of not requiring the use of mechanical sutures, making it less costly. The innovation of this procedure was the use of a single port gastrostomy device to perform an intraluminal sleeve. What made this technique less invasive were the use of a single port, nonmanipulation of the stomach intra-abdominally, ease of execution and no need of pneumoperitoneum. The new technique is acceptable and has reproducible viability, had a short procedure time without intra and postoperative complications.
