Objective: An excess of mortality was detected in Spain in February and March 2012 by the Spanish daily mortality surveillance system and the "European monitoring of excess mortality for public health action" program. The objective of this article was to determine whether this excess could be attributed to influenza in this period. Methods: Excess mortality from all causes from 2006 to 2012 were studied using time series in the Spanish daily mortality surveillance system, and Poisson regression in the European mortality surveillance system, as well as the FluMOMO model, which estimates the mortality attributable to influenza. Excess mortality due to influenza and pneumonia attributable to influenza were studied by a modification of the Serfling model. To detect the periods of excess, we compared observed and expected mortality. Results: In February and March 2012, both the Spanish daily mortality surveillance system and the European mortality surveillance system detected a mortality excess of 8,110 and 10,872 deaths (mortality ratio (MR): 1.22 (95% CI:1.21-1.23) and 1.32 (95% CI: 1.29-1.31), respectively). In the 2011-12 season, the FluMOMO model identified the maximum percentage (97%) of deaths attributable to influenza in people older than 64 years with respect to the mortality total associated with influenza (13,822 deaths). The rate of excess mortality due to influenza and pneumonia and respiratory causes in people older than 64 years, obtained by the Serfling model, also reached a peak in the 2011-2012 season: 18.07 and 77.20, deaths per 100,000 inhabitants, respectively. Conclusion: A significant increase in mortality in elderly people in Spain was detected by the Spanish daily mortality surveillance system and by the European mortality surveillance system in the winter of 2012, coinciding with a late influenza season, with a predominance of the A(H3N2) virus, and a cold wave in Spain. This study suggests that influenza could have been one of the main factors contributing to the mortality excess observed in the winter of 2012 in Spain.

Objetivo: El sistema de monitorización de la mortalidad diaria en España y el programa European monitoring of excess mortality for public health action detectaron un exceso de mortalidad en España en febrero y marzo de 2012. El objetivo de este trabajo es estudiar el papel que puede atribuirse a la gripe como factor de sobremortalidad en ese periodo. Métodos: Se estudiaron los excesos de mortalidad por todas las causas en el periodo 2006-2012 utilizando series temporales en el sistema de vigilancia de la mortalidad diaria español y mediante regresión de Poisson en el sistema de vigilancia de la mortalidad europeo y en un modelo que estima la mortalidad atribuible a la gripe. Resultados: En febrero y marzo de 2012, en los sistemas de monitorización de mortalidad español y europeo se detectó un exceso de mortalidad de 8110 y 10.872 defunciones (razón de mortalidad: 1,22, intervalo de confianza del 95% [IC95%]: 1,21-1,23, y 1,32, IC95%: 1,29-1,31, respectivamente). El modelo que estima la mortalidad atribuible a la gripe identificó en la temporada 2011-2012 el máximo porcentaje (97%) de defunciones atribuibles a la gripe en mayores de 64 años con respecto al total de la mortalidad asociada a la gripe (13.822 defunciones). La tasa de excesos de defunciones por gripe y neumonía y causas respiratorias en mayores de 64 años, obtenida con el modelo Serfling, fue máxima en la temporada 2011-2012: 18,07 y 77,20 defunciones por 100.000 habitantes, respectivamente. Conclusiones: Uno de los principales incrementos significativos de la mortalidad acontecidos en España en los inviernos de los últimos años, en mayores de 64 años, fue detectado por los sistemas de monitorización de mortalidad español y europeo en la temporada 2011-2012, coincidiendo en el tiempo con una tardía temporada gripal, con predominio de virus A(H3N2), y una ola de bajas temperaturas. Este estudio muestra que la gripe pudo ser uno de los principales factores contribuyentes al exceso de mortalidad observado en el invierno de 2012 en España.

Background: Cardiovascular disease is the leading cause of death worldwide, but its spatial distribution is not homogeneous. The objective of this study is to analyze the spatial pattern of mortality from these diseases for men and women, in the populated urban area (AUP) of the municipality of Madrid, and to identify spatial aggregations. Methods: An ecological study was carried out by census tract, for men and women in 2010. Standardized Mortality Ratio (SMR), Relative Risk Smoothing (RRS) and Posterior Probability (PP) were calculated to consider the spatial pattern of the disease. To identify spatial clusters the Moran index (Moran I) and the Local Index of Spatial Autocorrelation (LISA) were used. The results were mapped. Results: SMR higher than 1.1 was observed mainly in central areas among men and in peripheral areas among women. The PP that RRS was higher than 1 surpassed 0.8 in the center and in the periphery, in both men and women. Moran's I was 0.04 for men and 0.03 for women (p <0.05 in both cases). Conclusions: Sex differences were observed in the spatial distribution of mortality cases. RME RRS and PP maps showed a heterogeneous pattern in men, whereas in women a clearer pattern was detected, with a relatively higher risk in peripheral areas of the AUP. The LISA method showed similar patterns to those previously observed.

Fundamentos: Las enfermedades cardiovasculares son la principal causa de muerte en el mundo, aunque su distribución espacial no es homogénea. El objetivo del estudio fue analizar el patrón espacial de la mortalidad por enfermedades cardiovasculares en el área urbana poblada (AUP) del municipio de Madrid así como identificar agregaciones espaciales. Métodos: Se realizó un estudio ecológico, por sección censal, para hombres y mujeres durante 2010. Se calculó la Razón de Mortalidad Estandarizada (RME), Riesgo Relativo Suavizado (RRS) y Probabilidad Posterior (PP) de que el RRS fuera mayor que 1. Para identificar clusters espaciales se utilizó el índice de Moran (I Moran) y el Índice Local de Autocorrelación Espacial (LISA). Los resultados fueron representados cartográficamente. Resultados: En el caso de los hombres se observó una RME mayor de 1,1 especialmente en áreas centrales y en en el grupo de las mujeres ocurrió en la periferia. LA PP de que el RRS fuera mayor que 1 superó el 0,8 en el centro para los hombres y en la periferia en mujeres. El I Moran fue de 0,04 para hombres y de 0,03 para mujeres (p <0,05 en ambos casos). Conclusiones: En el patrón espacial de la mortalidad por enfermedades cardiovasculares en Madrid, se observaron diferencias por sexo. Los mapas de RME, RRS y PP mostraron un patrón más heterogéneo en los hombres mientras que en las mujeres se detectó uno más definido, con un riesgo relativamente mayor en zonas periféricas del AUP. El método LISA mostró agrupaciones espaciales similares a los patrones anteriormente observados.

Objectives: To analyze the spatial pattern of legionellosis in Spain for men and women during the period 2003-2007 and to identify spatial clustering of risk. Methods: We identified the spatial pattern of the distribution of legionellosis rates based on calculation of rates by municipality through the direct method. Smoothing of these rates was performed by the Empirical Bayes method for studying the spatial pattern of disease for both sexes. We used Moran´s index to analyze spatial autocorrelation rates globally. To calculate local rates, the Local Moran's Index [known as local indicators of spatial association (LISA)], was used to analyze the clusters of municipalities with the highest risk. Results: After smoothing the risk, the highest rates (over 50 per 100,000 inhabitants) were grouped in the eastern Mediterranean coastal areas and the north of the mainland, as well as in the Mediterranean islands. Moran's index smoothed rates were 0.15 for men and 0.23 for women. The spatial clusters of statistically significant higher rates calculated by the LISA index were distributed in the north and east for both sexes. Conclusions: These methods of spatial analysis allow patterns of disease distribution to be identified. All the methods used yielded similar results. These techniques are a complementary tool for epidemiological surveillance of infectious diseases.

Se realizó el suavizado de estas tasas por el método Empirical Bayes para estudiar el patrón espacial de la enfermedad, para ambos sexos. Se utilizó el índice de correlación espacial de Moran para analizar la autocorrelación global de las tasas. Localmente se utilizó el índice local de Moran (LISA) para analizar los agrupamientos (clusters) de municipios con mayor riesgo. Resultados: Una vez suavizado el riesgo, las mayores tasas (más de 50 por 100.000 habitantes) se agrupan en las zonas costeras del Mediterráneo oriental y en el norte de la Península, así como en los territorios insulares del Mediterráneo. El índice de Moran de las tasas suavizadas es 0,15 para los hombres y 0,23 para las mujeres. Las agrupaciones espaciales de las tasas más altas estadísticamente significativas calculadas mediante el LISA se distribuyen en el eje norte-levante para ambos sexos. Conclusiones: Estos métodos de análisis espacial permiten identificar los patrones de distribución de la enfermedad. Los métodos empleados presentan resultados similares. Estas técnicas son una herramienta complementaria para la vigilancia epidemiológica de las enfermedades infecciosas.

In Spain, tuberculosis is a statutorily notifiable disease requiring individualised reporting. During the last few years rates of respiratory tuberculosis show a steady decline. This study sought to assess respiratory tuberculosis morbidity and mortality in association to socio-economic and epidemiological covariates and estimate its spatial distribution across the country, using geo-statistical methods. Methods: Respiratory tuberculosis incidence rates were standardised by age and sex with the data of the National Epidemiological Surveillance Network (RENAVE, Red Nacional de Vigilancia Epidemiológica) for 2006. The following socio-economic variables were included in the study: socio-economic status, educational level, overcrowding rate, population density, standardised immigration rate by sex, unemployment rate and average spending per person in euros. The epidemiological variables included were, such as, AIDS rate and the influenza incidence rate. To assess the association of covariables a multivariate analysis was performed using a Generalised Linear Model assuming Poisson distribution. The goestatistical method co-kriging was adjusted with the significant variables to built the spatial distribution of risk. Results: The statistically significant covariates were overcrowding rate, standardised immigration rate by sex, educational level, unemployment rate, average spending per person in euros, AIDS rate and the influenza incidence rate. The geostatistical method shows spatial variability of the risk with higher risks in the northwest and southeast of the peninsula. Conclusion: Results prove that the co-kriging method is a useful tool to show the spatial distribution of risk. Alternatively, tuberculosis is associated with both social and epidemiological covariates.

Las tasas de TB respiratoria experimentan un descenso constante en los últimos años. El objetivo es valorar la asociación entre la morbilidad por TB respiratoria y variables socioeconómicas y epidemiológicas así como su distribución espacial mediante métodos geoestadísticos. Método: Las tasas de incidencia se estandarizaron por edad y sexo con datos de la Red Nacional de Vigilancia (2006). Las variables socioeconómicas incluidas son: condición socioeconómica, nivel de estudios, tasa de hacinamiento, densidad de población, tasa de inmigración estandarizada por sexo, tasa de analfabetismo, tasa de paro, gasto medio en euros por persona. Las variables epidemiológicas incluidas han sido la tasa de SIDA y la tasa de incidencia de gripe. Se realizó un análisis multivariable mediante un Modelo Lineal Generalizado poisson. Se aplicó la técnica geoestadística cokringing ajustada por las variables estadísticamente significativas para ver la distribución espacial de riesgo. Resultados: Las variables estadísticamente significativas son la tasa de hacinamiento, tasa de inmigración, tasa de analfabetismo, tasa de paro, gasto medio euros por persona, tasa de gripe y tasa de sida. La técnica geoestadística muestra una variabilidad espacial del riesgo y una concentración del riesgo en el noroeste y sureste de la península. Conclusiones: Los resultados permiten afirmar que el método Cokriging es una herramienta útil para representar la distribución espacial del riesgo.
