Abstract Introduction: We investigated the clinical course and outcomes of patients submitted to cardiovascular surgery in Brazil and who had developed symptoms/signs of coronavirus disease 2019 (COVID-19) in the perioperative period. Methods: A retrospective multicenter study including 104 patients who were allocated in three groups according to time of positive real time reverse transcriptase-polymerase chain reaction (RT-PCR) for the SARS-CoV-2 (severe acute respiratory syndrome coronavirus 2): group 1, patients who underwent cardiac surgery > 10 days after positive RT-PCR; group 2, patients with a positive RT-PCR within 10 days before or after surgery; group 3, patients who presented positive RT-PCR > 10 days after surgery. The primary outcome was mortality and secondary outcomes were postoperative complications, intensive care unit (ICU) length of stay, and postoperative days of hospitalization. Results: The three groups were similar with respect to age, the European System of Cardiac Operative Risk Evaluation score, and comorbidities, except hypertension. Postoperative complications and death were significantly higher in groups 2 and 3 than in group 1, and no significant difference between groups 2 and 3 was seen. Group 2 showed a high prevalence of surgery performed as an urgent procedure. Although no significant differences were observed in ICU length of stay, total postoperative hospitalization time was significantly higher in group 3 than in groups 1 and 2. Conclusion: COVID-19 affecting the postoperative period of patients who underwent cardiovascular surgery is associated with a higher rate of morbidity and mortality. Delaying procedures in RT-PCR-positive patients may help reduce risks of perioperative complications and death.

Resumo A incorporação do uso da fitoterapia na atenção primária à saúde é um ganho importante para o Sistema Único de Saúde, além da redução do custo no tratamento do paciente, resgata o conhecimento popular e promove o uso racional das plantas medicinas. Determinar o perfil diagnóstico situacional dos profissionais de unidades de saúde da família sobre fitoterapia. Foi realizado um estudo observacional quantitativo, no qual foram aplicados formulários para a equipe das unidades de saúde da família do município de Marechal Deodoro da Fonseca/Alagoas. Apenas 23% dos profissionais entrevistados afirmaram ter tido contato com informações científicas sobre fitoterapia durante a formação profissional. Todos aprovam a implantação da prática oficial da fitoterapia e prescrevem rotineiramente fitoterápicos e plantas medicinais, entretanto desconhecem os possíveis riscos de interação medicamentosa com o uso das plantas medicinas. Os profissionais de saúde apoiam a prática oficial da fitoterapia na saúde pública e demonstram interesse em participar de cursos/treinamentos sobre a temática para melhor atender a população.
Abstract The incorporation of the use of phytotherapy in the primary health care is an important gain for the Unified Health System, besides reducing the cost in the treatment of the patient, rescues the popular knowledge and promotes the rational use of the medicinal plants. To determine the situational diagnostic profile of professionals of family health units on phytotherapy. A quantitative observational study was carried out, in which forms were applied to the team of the family health units of the municipality of Marechal Deodoro da Fonseca/Alagoas. Only 23% of the interviewed professionals reported having had contact with scientific information on phytotherapy during professional training. All of them approve the implantation of the official practice of phytotherapy and routinely prescribe phytotherapics and medicinal plants, but they do not know the possible risks of drug interaction with the use of medicinal plants. Health professionals Health professionals support the official practice of phytotherapy in public health and show interest in participating in courses / training on the subject to better serve the population.

OBJECTIVES: To prospectively evaluate demographic, anthropometric and health-related quality of life (HRQoL) in pediatric patients with laboratory-confirmed coronavirus disease 2019 (COVID-19) METHODS: This was a longitudinal observational study of surviving pediatric post-COVID-19 patients (n=53) and pediatric subjects without laboratory-confirmed COVID-19 included as controls (n=52) was performed. RESULTS: The median duration between COVID-19 diagnosis (n=53) and follow-up was 4.4 months (0.8-10.7). Twenty-three of 53 (43%) patients reported at least one persistent symptom at the longitudinal follow-up visit and 12/53 (23%) had long COVID-19, with at least one symptom lasting for >12 weeks. The most frequently reported symptoms at the longitudinal follow-up visit were headache (19%), severe recurrent headache (9%), tiredness (9%), dyspnea (8%), and concentration difficulty (4%). At the longitudinal follow-up visit, the frequencies of anemia (11% versus 0%, p=0.030), lymphopenia (42% versus 18%, p=0.020), C-reactive protein level of >30 mg/L (35% versus 0%, p=0.0001), and D-dimer level of >1000 ng/mL (43% versus 6%, p=0.0004) significantly reduced compared with baseline values. Chest X-ray abnormalities (11% versus 2%, p=0.178) and cardiac alterations on echocardiogram (33% versus 22%, p=0.462) were similar at both visits. Comparison of characteristic data between patients with COVID-19 at the longitudinal follow-up visit and controls showed similar age (p=0.962), proportion of male sex (p=0.907), ethnicity (p=0.566), family minimum monthly wage (p=0.664), body mass index (p=0.601), and pediatric pre-existing chronic conditions (p=1.000). The Pediatric Quality of Live Inventory 4.0 scores, median physical score (69 [0-100] versus 81 [34-100], p=0.012), and school score (60 [15-100] versus 70 [15-95], p=0.028) were significantly lower in pediatric patients with COVID-19 at the longitudinal follow-up visit than in controls. CONCLUSIONS: Pediatric patients with COVID-19 showed a longitudinal impact on HRQoL parameters, particularly in physical/school domains, reinforcing the need for a prospective multidisciplinary approach for these patients. These data highlight the importance of closer monitoring of children and adolescents by the clinical team after COVID-19.

OBJECTIVE: To assess the possible factors that influence sleep quality in adolescents with and without chronic immunosuppressive conditions quarantined during the coronavirus disease 2019 (COVID-19) pandemic. METHODS: This cross-sectional study included 305 adolescents with chronic immunocompromised conditions and 82 healthy adolescents. Online surveys were completed, which included questions on socio-demographic data and self-rated healthcare routine during COVID-19 quarantine and the following validated questionnaires: the Pittsburgh Sleep Quality Index (PSQI), Pediatric Quality of Life Inventory 4.0 (PedsQL4.0), and Pediatric Outcome Data Collection Instrument (PODCI). RESULTS: The median current age [14 (10-18) vs. 15 (10-18) years, p=0.847] and frequency of female sex (62% vs. 58%, p=0.571) were similar in adolescents with chronic conditions compared with healthy adolescents. The frequency of poor sleep quality was similar in both groups (38% vs. 48%, p=0.118). Logistic regression analysis, including both healthy adolescents and adolescents with chronic conditions (n=387), demonstrated that self-reported increase in screen time (odds ratio [OR] 3.0; 95% confidence interval [CI] 1.3-6.8; p=0.008) and intrafamilial violence report (OR 2.1; 95% CI 1.2-3.5; p=0.008) were independently associated with poor sleep quality in these adolescents. However, the PODCI global function score was associated with a lower OR for poor sleep quality (OR 0.97; 95% CI 0.94-0.99; p=0.001). Further logistic regression, including only adolescents with chronic conditions (n=305), demonstrated that self-reported increase in screen time (OR 3.1; 95% CI 1.4-6.8; p=0.006) and intrafamilial violence report (OR 2.0; 95% CI 1.2-3.4; p=0.011) remained independently associated with poor quality of sleep, whereas a lower PODCI global function score was associated with a lower OR for sleep quality (OR 0.96; 95% CI 0.94-0.98; p<0.001). CONCLUSION: Self-reported increases in screen time and intrafamilial violence report impacted sleep quality in both healthy adolescents and those with chronic conditions. Decreased health-related quality of life was observed in adolescents with poor sleep quality.

Resumo Este trabalho teve como objetivo conhecer perfil diagnóstico de usuários do SUS com relação à fitoterapia. Trata-se de um estudo observacional transversal, com coleta de dados realizada no Município de Marechal Deodoro, por meio de um questionário semi-estruturado com perguntas relativas a dados socioeconômicos, utilização e conhecimentos sobre plantas medicinais e fitoterápicos e satisfação com o uso de plantas medicinais (PM) e medicamentos fitoterápicos (MF). Entre os usuários abordados, 87,08% utilizam plantas medicinais frequentemente, principalmente a erva cidreira (Melissa officinalis L.). Também foi observado que as plantas são geralmente usadas como calmante (16,05%) e na forma de chá (68,35%). 82,51% dos entrevistados afirmam ter adquirido o conhecimento sobre fitoterapia através de algum familiar e 85,56% fazem uso das PM e MF sem ter recebido algum tipo de informativo sobre o seu uso. 61% das plantas descritas pelos usuários teve seu uso e terapêutica coincidente com o citado em literatura específica. Com relação a satisfação através do tratamento com PM e MF observou-se que 56,80% estão satisfeitos. O uso de plantas medicinais por parte dos usuários é bastante frequente, principalmente pelo conhecimento acumulado ao longo de gerações. Portanto, se faz necessária maior atenção a esse aspecto de utilização, visando evitar possíveis reações adversas advindas do uso incorreto.
Abstract This study aimed to assess diagnosis of Unique Health System (SUS) users profile with regard to herbal medicine. This is a cross-sectional study with observational data collection conducted in the city of Marechal Deodoro, through a semi-structured questionnaire with questions related to socioeconomic data, use, and knowledge of medicinal plants and herbal medicines and satisfaction with the use of medicinal herbs (MH) and interactions of phytotherapeutic drugs (PD). Among the topics, 86.89% of users used herbal medicines often, especially lemon balm (Melissa officinalis L.). It was also observed that the plants are generally used as a sedative (21.74%) and as tea (87.84%). 82.32% of respondents claim to have acquired the knowledge of herbal medicine through a family member and 85.36% make use of medicinal plants without having received any information on the use of MH and PD. 61% of the plants described that coincided with the form of use and therapeutic properties found in the literature. With regard to satisfaction by treatment with MH and PD, it was found that 56.67% are satisfied. Therefore, it is necessary to pay attention to this aspect of usage, in order to avoid possible drug interactions or adverse reactions resulting from the incorrect use.

Potencial de plantas acuáticas para la remoción de coliformes totales y Escherichia coli en aguas servidas

Abstract: In Ecuador, most communities rely on non-treated surface water as their main drinkable water source. In most cases, the water source is contaminated with fecal coliforms and Escherichia coli. This research is intended to find aquatic plants with the ability to remove the overall coliforms and E. coli from contaminated water. Aquatic plants were taken in Rio Guayas, Recinto Aguas Frias and Estero Peñafiel, and the following species were selected to carry out the experiment: Azolla caroliniana Willd, Eichhornia crassipes (Mart.) Solms, Pistia stratiotes L., Salvinia auriculata Aubl. and Lemna minor L. specimens of these plants were grown in vitro to verify their capacity and ability to remove E. coli and the coliforms using a bioassay method. Physico-chemical parameters were also tested, such as: temperature, PH levels and total dissolved solids. This experiment was developed in water and using a method inoculating a referential E. coli ATCC25922 strain. After 7 days the remaining bacterial percentage was determined, and it was noted a capacity of 99% of E. coli removal for A. caroliniana, E. crassipes and L. minor, and 100% for P. stratiotes and S. auriculata (p = 0.027). Once the potential of bacterial removal of these plants were proved, test were executed in wastewater, resulting in a 100% removal of coliforms and E. coli for S. auriculata and A. caroliniana (p = 0.003). After the results obtained in this experiment, it is recommended to use S. auriculata and A. caroliniana as a strategy for treatment and removal of bacteria from contaminated water.
Resumen: En el Ecuador, muchas comunidades dependen del agua superficial no tratada como la principal fuente de agua potable y estas se encuentran contaminadas con coliformes fecales y Escherichia coli. El objetivo de esta investigación fue encontrar plantas acuáticas con potencial de remover E. coli y coliformes totales de agua contaminada. Se realizaron muestreos en: el Río Guayas, el Recinto Aguas Frías y el Estero Peñafiel donde se seleccionaron las especies: Azolla caroliniana Willd, Eichhornia crassipes (Mart.) Solms, Pistia stratiotes L., Salvinia auriculata Aubl. y Lemna minor L. Las plantas se reprodujeron in vitro y se realizaron bioensayos para verificar su capacidad de remover E. coli y coliformes. También se determinaron los parámetros fitoquímicos: temperatura, pH y sólidos totales disueltos. Los ensayos se realizaron por triplicado en agua con fertilizante y se inoculó una cepa de referencia E. coli ATCC25922. Al transcurrir siete días se determinó la carga bacteriana remanente y se obtuvo un porcentaje de remoción de E. coli en comparación al control del 99% para A. caroliniana, E. crassipes y Lemna sp., mientras que para P. stratiotes y S. auriculata fue del 100% (p = 0.027). Una vez evidenciado el potencial de estas especies para remover las bacterias, se realizaron ensayos con aguas servidas en los cuales S. auriculata y A. caroliniana lograron el 100% de remoción de las coliformes y E. coli (p = 0.003). Se sugiere el uso de S. auriculata y A. caroliniana para el tratamiento de las aguas.

En Colombia la mayoría de los suelos muestran deficiencia de fósforo disponible, fundamental para el desarrollo de las plantas. Para superar esta dificultad de orden natural y mitigar la demanda de fósforo del suelo, últimamente se adicionan microorganismos solubilizadores de fosfatos (MSF). En esta investigación, se estudiaron los microorganismos Kocuria sp., B. subtilis, S. diversispora y P. ochrochloron y el efecto que tienen sobre la disponibilidad de fósforo, en el desarrollo de plántulas de café, bajo condiciones de invernadero, con 23 tratamientos y 10 repeticiones, en diseño estadístico, completamente al azar. Los experimentos se desarrollaron en un suelo Typic Melanudand con pulpa de café descompuesta y microorganismos (tratamientos 1-11), adición de roca fosfórica (tratamientos 12-22) y un testigo Typic Melanudand, fertilizado con fosfato diamónico (tratamiento 23). Los resultados mostraron que la aplicación de pulpa de café descompuesta con y sin roca fosfórica y microorganismos, favoreció la disponibilidad de fósforo, reflejándose en el crecimiento de las plántulas de café, en el análisis químico del suelo y en las variables de respuesta del fósforo y el área foliar. Se encontró además, una relación inversa entre la concentración de polifenoles y fósforo en suelo y planta.
Most soils in Colombia shown available phosphorus deficiency, which is a fundamental element for the plants development. In order of overcome this natural difficulty and mitigate the demand for phosphorus in the soils, phosphate solubilizing microorganisms (PSM) are added. In this research, were studied the microorganisms Kocuria sp., B. subtilis, S. and P. diversispora ochrochloron and the effect they have on the availability of phosphorus in the development of coffee seedlings under greenhouse conditions, with 23 treatments and 10 replications in statistical design, completely randomized. The experiments were carried out in Typic Melanudand soil with decomposed coffee pulp and presence or absence of microorganisms (treatments 1-11) and adding phosphate rock (12 to 22 treatments). Typic Melanudand was used as a witness in the treatment 23, fertilized with diammonium phosphate. The results showed that the application of decomposed coffee pulp with and without phosphate rock and microorganisms, favored the availability of phosphorus, reflected in the growth of coffee seedlings, in the chemical analysis of the soil and the response variables match and leaf area. There was an inverse relationship between polyphenols and phosphorus in soil and plant.
Na Colômbia a maioria dos solos apresentam deficiência de fósforo, um elemento fundamental para o desenvolvimento das plantas. Para superar esta dificuldade de ordem natural e diminuir a demanda de fósforo do solo, ultimamente são adicionados microrganismos solubilizadores de fosfatos (MSF). Nesta pesquisa foram estudados os microrganismos Kocuria sp., B. subtilis, S. diversispora y P. ochrochloron e o efeito que tem sob a disponibilidade do fósforo, no desenvolvimento de plântulas de café em estufa, com 23 tratamentos e 10 repetições utilizando-se desenho estatístico completamente ao acaso. Os experimentos foram desenvolvidos em soloTypic Melanudand com polpa de café decomposta e microrganismos (1-11 tratamentos), adição de rocha fosfórica (12-22 tratamentos) e Typic Melanudand como testemunha, fertilizado com fosfato diamônico (tratamento 23). Os resultados mostraram que a aplicação de polpa de café decomposta com e sem rocha fosfórica e microrganismos, favoreceu a disponibilidade do fósforo, refletindo-se no crescimento das plântulas de café, na análise química do solo e nas variáveis de resposta do fosforo e na área foliar. Além disso, encontrou-se uma relação inversa entre a concentração de polifenóis e fósforo no solo e na planta.

RESUMO Entre as doenças cuja etiopatogenia está associada à Escherichia coli, destaca-se a doença diarreica aguda. Estudos que visam à caracterização do perfil de suscetibilidade antimicrobiana contribuem para o delineamento de antibioticoterapia empírica eficaz. Neste estudo, foi avaliado o perfil de suscetibilidade a antimicrobianos de 98 amostras de E. coli enterotoxigênica (ETEC) e E. coli enteropatogênica (EPEC) isoladas de crianças com doença diarreica. As frequências de resistência a ampicilina, sulfametoxazol-trimetoprima, amoxicilina-clavulanato e ácido nalidíxico foram elevadas, variando entre 34,7% e 10,2%. Esta pesquisa recomenda o emprego de cefotaxima e ceftriaxona para o tratamento empírico de crianças com quadro de diarreia cuja etiologia sugerida seja ETEC ou EPEC.
ABSTRACT Among the diseases which etiopathogenesis is associated with Escherichia coli, acute diarrhea stands out. Studies on the characterization of the antimicrobial susceptibility profile contribute to the selection of appropriate empirical antimicrobial therapy. In this study, the antimicrobial susceptibility profile of 98 enterotoxigenic E. coli (ETEC) and enteropathogenic E. coli (EPEC) strains isolated from fecal specimens of children with acute diarrhea was evaluated. The resistance rates to ampicillin, sulfamethoxazole/trimethoprim, amoxicillin/clavulanate, and nalidixic acid were high, ranging from 34.7% to 10.2%. The result of this research recommends the use of cefotaxime and ceftriaxone for the empirical treatment of children with acute diarrhea which the etiology suggested is ETEC or EPEC.

Mortalidade de bovinos zebuínos por hipotermia em Mato Grosso do Sul

O presente trabalho teve por objetivo descrever a ocorrência periódica de mortalidade de bovinos por hipotermia após inversão térmica no estado de Mato Grosso do Sul. São relatados 16 surtos encaminhados ao Laboratório de Patologia Animal da FAMEZ/UFMS, ocorridos de agosto de 2000 a julho de 2010, em 13 municípios do Estado. O diagnóstico de hipotermia baseou-se na ocorrência de mortes após queda brusca de temperatura acompanhada de chuvas e ventos, simultaneamente em diversas propriedades, nos sinais clínicos observados e na ausência de lesões macro e microscópicas significativas. Em todos os surtos houve uma queda brusca de temperatura, de até 29ºC, em intervalos de um a quatro dias associada a chuva e vento. Na maioria das vezes a morte dos bovinos ocorreu em locais onde havia escassez de pasto e ausência de abrigos naturais ou artificiais. Os bovinos afetados estavam magros, com baixo escore corporal, e, em grande parte dos casos, eram encontrados mortos nos cantos das invernadas e próximos a cercas no dia seguinte à queda brusca da temperatura. Foram afetados bovinos de diferentes idades. Os sinais clínicos se caracterizavam por cegueira, incoordenação, dismetria, fraqueza, decúbito esternal, movimentos de pedalagem, opistótono, tremores musculares, e dificuldade respiratória e morte. Em 30 bovinos necropsiados os principais achados macroscópicos foram edema subcutâneo, cavitário e pulmonar, e, em alguns casos, histologicamente havia eosinofilia neuronal. A hipotermia é uma importante causa de mortalidade quando bovinos com pobre estado nutricional, pouca disponibilidade e qualidade dos pastos e ausência de abrigos naturais, são submetidos a uma condição de mudança climática com queda brusca de temperatura combinada com ventos fortes e chuvas.
This study describes the periodic mortality of cattle due to hypothermia after thermal inversion in Mato Grosso do Sul, Brazil. Sixteen outbreaks, reported to the Laboratory of Animal Pathology FAMEZ/UFMS, occurred from August 2000 to July 2010 in 13 municipalities of the state. The diagnosis of hypothermia was based on the occurrence of deaths after a sudden decrease in temperature accompanied by rain and wind in several farms simultaneously, on clinical signs and on the absence of significant macroscopic and microscopic lesions. In all outbreaks was a sudden fall of temperature up to29°C at intervals of one to four days together with rain and wind. Most deaths occurred in places with scarce pasture and lack of natural or artificial shelters. The affected cattle were lean, with low body score, and in most cases on the morning following sudden fall in temperature, they were found dead in the corners of wintering and near the fences. Cattle of different ages were affected. Clinical signs were characterized by apparent blindness, incoordination, dysmetria, weakness, sternal recumbency, paddling, opisthotonus, muscle tremors, difficulty in breathing and death. In 30 necropsied animals the main macroscopic findings were subcutaneous, cavity and pulmonary edema. Histologically in some cases neuronal eosinophilia was observed. Hypothermia is an important cause of mortality when cattle, not acclimated, with poor nutritional status, low availability and quality of pasture, and lack of natural shelter, are subjected to climate change with sudden fall in temperature combined with high winds and rain.

Essential oil composition and variability in Hyptis fruticosa

The composition of six samples of essential oil (EO) extracted from leaves, flowers and seeds of several plants of Hyptis fruticosa Salzm. ex Benth., Lamiaceae, was investigated by GC/MS and GC/FID. 1,8-Cineole, spathulenol, α-pinene, β-pinene were the major constituents. Ten constituents that have not been previously described in the composition of the oil of H. fruticosa were identified. Hydrocarbons sesquiterpenes represented the main group, followed by hydrocarbons monoterpenes. The results were submitted to Cluster Analysis which allowed three groups of EO to be distinguished with respect to the content of α-pinene/β-pinene, 1,8-cineole and spathulenol. Growth stages of the plants and geographical parameters seem to be important factors determining the variability of the oil. Sesquiterpenes were mainly produced in the seeds.
