Abstract This article aims to discuss the expectations of Homosexual Men, Bisexual Men and a Transgender Woman, who use or want to use an oral pre-exposure prophylaxis (PrEP) for the human immunodeficiency virus (HIV) about PrEP modalities. Sixteen PrEP users, who are followed up in the BCN Checkpoint, were interviewed,. The interviews were audio-recorded, subjected to thematic categorical analysis within the theoretical framework from the praxiographic perspective. They are all adapted to the use of daily oral and event-based PrEP. In relation to the new PrEP modalities (monthly pill; intramuscular injection every two months; subcutaneous injection every six months), they are all very receptive to these possibilities, but they lack information on the specificities of each and specific assessment of their needs. Comments about the use of oral PrEP are positive, and expectations regarding the new PrEP modalities are visibly high. However, the most important thing for the interviewees is the guarantee that they will have follow-up appointments to continue taking care of their affective-sexual health, which is not dependent on the type of PrEP modalities.
Resumen Este artículo tiene como objetivo discutir las concepciones de los Hombres Gay, Hombres Bisexuales y una Mujer Transgénero que usan o quieren usar profilaxis previa a la exposición por el virus de la inmunodeficiencia humana oral (PrEP) sobre nuevas vías de administración. Fueron entrevistados 17 usuarios del BCN Checkpoint. Las entrevistas fueron grabadas en audio, sometidas a análisis categorial temático teniendo en cuenta la perspectiva praxeográfica. Todos están adaptados al uso de la PrEP diaria y a demanda. En relación con las nuevas vías de administración (PrEP inyección intramuscular cada dos meses; pastilla mensual; inyección subcutánea cada seis meses) todos son muy receptivos a esas posibilidades, pero les falta información sobre las especificidades de cada una de ellas y una evaluación específica de sus necesidades. Tanto la satisfacción con el uso de PrEP oral, como las expectativas sobre las nuevas vías de administración son positivas. Sin embargo, lo más importante para los/a entrevistados/a es la garantía de que tendrán seguimiento para continuar cuidando de la salud afectivo-sexual, lo que no depende del tipo de vía de administración.

Resumen Este artículo tiene como objetivo discutir las concepciones de los Hombres Gay, Hombres Bisexuales y una Mujer Transgénero que usan o quieren usar profilaxis previa a la exposición por el virus de la inmunodeficiencia humana oral (PrEP) sobre nuevas vías de administración. ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. Abstract Biomphalaria straminea is one of the three snails that act as the intermediate hosts of Schistosoma mansoni and is responsible for maintaining high rates of schistosomiasis in some areas of northeastern Brazil. The principal morphological characteristic of B. straminea is the marked vaginal wrinkling, although it is also present in congeneric species, a group known as the B. straminea complex. Based on the morphological and molecular analyses, we investigated the intraspecific variation of B. straminea. The specimens were obtained from 10 sites in Brazil, and the shells were analyzed, as well as 16 morphological structures of the reproductive system. The COI and ITS2 sequences were used for phylogenetic analysis, genetic divergence, and haplotype network (COI). We observed a large intraspecific variation in the morphological structures examined. The genetic divergence also demonstrated significant intraspecific variability in B. straminea: 0–3% in ITS2 sequences, and 0–6% in COI sequences. Analysis of the distribution of COI haplotypes recovered 16 haplotypes and haplotype diversity of 0.9088. These results indicate phenotypic variability that is not constrained to a locality or strictly controlled genetically by B. straminea, which may have driven a misidentified of these species. Abstract The aim of this study was to develop and achieve consensus on a cariology teaching framework for dental schools in Latin American Spanish-speaking countries. The Delphi process, with a ≥8 0% pre-defined participants’ agreement, included three phases and a Coordinating Group. During the Preparation phase three panels of experts were selected and invited to participate: a) Regional academic/professional Dental Associations (Associations-Panel): n = 12; b) Regional Dental Schools (Dental-Schools-Panel): existing dental schools (n = 263) from the 19 Spanish-speaking regional countries; c) International academic/professional associations Peer Experts (Peer-Panel): n = 4. Based on consensus documents from Europe, Colombia, the Caribbean, USA, Chile and Spain, and updated scientific evidence, the Coordinating Group developed a baseline framework proposal of domains, main competencies (MC) and specific competencies (SC). The Consultation-Agreement and Consensus phases included three rounds of questionnaires with a step-wise sharing of the MC updated version of the consensus framework with the Dental-Schools-Panel and including SC with the Associations-Panel. Diverse communication strategies were used ( e.g ., independent google-form questionnaires and workshops). Consensus was reached after an on-site Associations-Panel workshop and secret voting, followed by an online meeting with the Peers-Panel. A total of 127 academic/professional institutions participated (Associations-Panel: 11, 91.6%; Dental-Schools-Panel: 112, 42.6%, all countries; Peers-Panel: 4, 100%). The baseline Cariology teaching framework of 5 domains, 10 MC and 92 SC underwent modifications after agreements for a final consensus framework consisting of 5 domains, 10 MC and 85 SC. Resumo A Coleção Helmintológica do Instituto Oswaldo Cruz é a maior da América Latina e está entre as maiores coleções de referência mundial, com cerca de 40.000 lotes e, aproximadamente, um milhão de espécimes. Seu acervo reúne helmintos parasitos de animais vertebrados e invertebrados da fauna brasileira e de outros países. Seus exemplares são holótipos, parátipos e espécimes representativos de parasitos, pertencentes aos filos Platyhelminthes, Acanthocephala, Nematoda e, ainda alguns espécimes não-helmintos, pertencentes aos filos Annelida e Arthropoda. Parte das amostras preservadas como material líquido foram encontradas dessecadas. Esta condição torna as amostras inviáveis para análise morfológica para propósitos taxonômicos. Portanto, o objetivo deste trabalho foi testar técnicas usadas na reidratação do tegumento dos espécimes que se encontram dessecados e apresentar seus protocolos. Foram analisados 528 lotes, cujos espécimes encontravam-se sem conservantes ou já dessecados: 96 trematodeos digenéticos, 45 cestoides, 22 acantocéfalos, 357 nematoides, quatro hirudíneos e quatro crustáceos pentastomídeos. Abstract The Helminthological Collection of the Oswaldo Cruz Institute is the biggest in Latin America and it is among the largest collections at worldwide reference level, with around 40,000 sets of specimens and approximately one million individual specimens. It contains helminths parasites of vertebrate and invertebrate animals that form part of the fauna of Brazil and other countries. The aim of this study was to test techniques used for rehydration of the tegument of specimens that had been found to have dried out and present protocols for such techniques. Resumo O artigo tem como objetivo compreender como os municípios paulistas organizaram o enfrentamento da pandemia de COVID-19, destacando o papel da Atenção Primária à Saúde (APS) como elemento analisador do modelo de atenção. Estudo quantitativo descritivo a partir da realização de inquérito com uma amostra probabilística de 253 municípios do estado de São Paulo no qual foram entrevistados gestores municipais por meio de questionário. A descrição das frequências absolutas (n) e relativas (%) foi feita após ponderação segundo os três estratos de portes populacionais. Os resultados indicam que o elemento de porte populacional constitui importante componente analítico. A organização durante a pandemia priorizou, na maioria dos municípios, reajuste de fluxo e cuidados clínicos. As ações continuadas de pré-natal e puericultura também eram de caráter biomédico, com consultas. Em relação ao fomento de respostas de cuidado ampliado e territorial, os municípios de menor porte, com centralidade na APS, tiveram um desempenho superior. Já os municípios de grande porte fragmentaram o cuidado e a vacinação. As ações intersetoriais, de cuidado comunitário, e de utilização da perspectiva territorial, mostraram-se ainda retraídas e persistem dificuldades da APS. Abstract This article aims to understand how the cities of São Paulo state organized the coping with the COVID-19 pandemic, highlighting the role of Primary Health Care (PHC) as an analyzing element of the healthcare model. This descriptive quantitative study was grounded on a survey with a probabilistic sample of 253 municipalities in the state of São Paulo in which municipal managers were interviewed through a questionnaire. Absolute (n) and relative (%) frequencies were described after weighting according to the three population strata. The results indicate that the population size is an essential analytical component. During the pandemic, the organization prioritized flow readjustment and clinical care in most municipalities. Prenatal care and childcare continuing actions consisted of biomedical actions with appointments. Regarding the promotion of expanded healthcare responses, the smaller municipalities, which are structured based on the PHC, performed better. Abstract Infectious mononucleosis (MI) is a generally benign and self-limited condition in childhood and adolescence due to the primary EBV infection characterized by the triad of pharyngitis, fever, and lymphadenopathies. The risk of complications increases with age and immunosuppression. The most frequent fatal complications are those associated with splenic rupture, neurological alterations, and airway obstruction due to increased tonsillar size. Cervical abscesses associated with MI are rare, being mostly peritonsillar and intra-tonsil. We present two surgical cases of big deep cervical abscesses with retropharyngeal and parapharyngeal involvement in healthy very young adolescents (14 and 15 years old), without any type of immunosuppression or risk factors, one of them associated with a clinically relevant spontaneous tonsillar bleeding, which had not been described in the literature associated with MI in such young patient.
Resumen La mononucleosis infecciosa (MI) es un cuadro clínico generalmente benigno y autolimitado en la infancia y adolescencia debido a la primoinfección del virus de Epstein-Barr caracterizado por la triada de faringitis, fiebre y adenopatías. El riesgo de complicaciones aumenta con la edad y la inmunosupresión, siendo las complicaciones letales más frecuentes las asociadas a rotura esplénica, alteraciones neurológicas y obstrucción de la vía aérea por el aumento del tamaño amigdalar. Los abscesos cervicales asociados a MI son poco frecuentes, siendo mayoritariamente periamigdalinos e intraamigdalares. Presentamos dos casos quirúrgicos de abscesos cervicales profundos de gran tamaño con afectación retrofaríngea y parafaríngea en adolescentes sanos de corta edad (14 y 15 años), sin ningún tipo de inmunosupresión o factores de riesgo, uno de ellos asociado además, a una relevante hemorragia amigdalar espontanea, condición no descrita previamente en la literatura en relación a MI en un paciente tan joven.

Abstract The lockdown was the measure taken by the Peruvian state as a response to the pandemic caused by COVID-19. This imposition meant a delay for almost all sectors, including the academic sector. This study analyzes the repercussions of the lockdown through scientific work in Peru in 2020, focusing on subjects of biodiversity and conservation. We conducted a during July and August of 2020, which was answered by 111 researchers. Results of the survey show that the most affected research area was field research due to the restrictions; and that the most affected group were women, which presented the highest percentages of dissatisfaction, both in their work and emotional state. This research reveals the reality of a country with a historically low investment in research, and now the work of many researchers is restricted due to the current economic and employment situation due to the pandemic. Its replication in other Latin American countries could be of help for future policies in favor of academic research.
Resumen La cuarentena fue la medida tomada por el estado peruano para enfrentar a la pandemia causada por el COVID- 19. Esta significó un retraso para casi todos los sectores, incluyendo el sector académico. Este estudio analiza las consecuencias de esta medida en el trabajo de los investigadores científicos del Perú durante el 2020, enfocándose en aquellos que trabajan en biodiversidad y conservación. Se realizó una encuesta durante los meses de julio y agosto de 2020 que fue respondida por 111 investigadores. Los resultados de la encuesta muestran que el área más afectada fue la investigación en campo por las evidentes restricciones, y el grupo más afectado fue el de las investigadoras quienes presentaron los porcentajes más altos de insatisfacción, tanto en su vida laboral como en su estado emocional. Esta investigación da a conocer la realidad del Perú, con una inversión históricamente baja en investigación y donde, con la situación económica y laboral provocada por la pandemia, el trabajo de muchos investigadores se vio restringido. Consideramos que la réplica de esta investigación en otros países de Latinoamérica podría ser de ayuda para futuras políticas a favor del trabajo científico en los países y regionalmente.

RESUMO O trabalho teve como objetivo analisar as possíveis vias de introdução de Fusarium spp. na Madeira entre 1990 e 2018, para elucidar os fatores que favoreceram a sua introdução e fixação na ilha. Realizaram-se análises fitossanitárias em culturas e plantas importadas para a Madeira. Os isolados de Fusarium spp. foram obtidos de órgãos subterrâneos e aéreos das plantas e cultivados em meios diferenciais para identificação das espécies. A colônia de fungos e a morfologia dos esporos foram observadas ao microscópio e identificados com o auxílio de bibliografia e chaves dicotómicas. A análise fitossanitária revelou que a incidência de Fusarium spp. em plantas ou culturas apresentou uma tendência crescente no número de fungos, principalmente entre 1990 e 2001. A contaminação fúngica mais elevada ocorreu nos municípios localizados no litoral sul da ilha. A maioria das amostras contaminadas proveio de Portugal (96,9%) e 0,1% a 1,6% provieram do exterior, sobretudo da Holanda e França. De 3.246 amostras infectadas com Fusarium spp., 1212 eram hortícolas (37,3%), ornamentais (36,9%), tropicais e subtropicais (16,9%) e temperadas (9,9%). Fusarium Wilt (doença do Panamá) causada por F. oxysporum f. sp. cubense ocorreu em vários produtos importados para a Madeira. Outros como Fusarium oxysporum spp. foram detectados em várias plantas e produtos agrícolas, juntamente com Fusarium sp. e Fusarium equiseti. As condições geoclimáticas da Madeira podem ser adequadas para o desenvolvimento e dispersão destes fungos. O comércio de cultivares e plantas de Portugal e do estrangeiro tem sido uma importante via de introdução do Fusarium na Madeira.
ABSTRACT The present study aimed to analyze the possible routes of Fusarium spp. introduction in Madeira between 1990 and 2018 and to elucidate the factors that favored the introduction and settling of this fungus in the Island. Phytosanitary surveys were carried out in several crops and plants imported to Madeira. The isolates of Fusarium spp. were obtained from underground and aerial organs of the plants and cultured on differential media for species identification. Fungal colony and spore morphology were observed under a microscope and identified based on bibliography and dichotomous keys. The phytosanitary analysis for the incidence of Fusarium spp. in plants or crops indicated an increasing trend in the number of fungi, especially between 1990 and 2001. The greatest fungal contamination occurred in the municipalities located in the southern coast of the Island. Most of the contaminated samples were from Portugal (96.9%), while 0.1% to 1.6% samples derived from foreign countries, especially the Netherlands and France. Of 3,246 samples infected with Fusarium spp., 1,212 were from horticultural (37.3%), ornamental (36.9%), tropical and subtropical (16.9%), and temperate plants (9.9%). Fusarium wilt (Panama Disease) caused by F. oxysporum f. sp. cubense occurred in several products imported to Madeira. Other species like Fusarium oxysporum were also detected in several plants and agricultural products, along with Fusarium sp. and Fusarium equiseti. The geoclimatic conditions of Madeira can be suitable for the development and dispersion of these fungi. The trade of cultivars and plants from Portugal and from foreign countries has been an important route of Fusarium introduction in Madeira.

Trends in public sector radiological usage in the Western Cape Province, South Africa: 2009-2019

BACKGROUND: Although global use of medical imaging has increased significantly, little is known about utilisation trends in low- and middle-income countries (LMICs). OBJECTIVES: To evaluate changes over a decade in public sector diagnostic imaging utilisation at provincial level in a middle-income country. METHOD: A retrospective analysis of medical imaging utilisation in the Western Cape Province of South Africa in 2009 and 2019. Use of conventional radiography, ultrasonography (US), fluoroscopy, CT, MRI, digital subtraction angiography (DSA) and whole-body digital radiography was assessed by total studies and studies/10³ people, for the whole province, the rural and metropolitan areas. Mammography utilisation was calculated for every 10³ females aged 40-70 years. RESULTS: The provincial population and total imaging investigations increased by 25% and 32%, respectively, whilst studies/10³ people increased by 5.5% (256 vs 270/10³), with marked variation by modality. Provincial US, CT and MRI utilisation/10³ people increased by 111% (20 vs 43/10³), 78% (10 vs 18/10³) and 32% (1.9 vs 2.5/10³) respectively, whilst use of fluoroscopy (3.6 vs 3.7/10³) and mammography (14.2 vs 15.9/10³ women aged 40-70 years) was steady and plain radiography decreased by 20% (216 vs 196/10³). For CT, mammography and fluoroscopy, percentage utilisation increases/10³ people were higher in the rural than metropolitan areas. CONCLUSION: Population growth is the main driver of overall imaging utilisation in our setting. The relatively constant imaging workload per 1000 people, albeit with increasing ultrasound, CT and MR utilisation, and decreasing use of plain radiography, reflects improved provincial imaging infrastructure, and appropriate use of available resources.

Abstract The Japanese invasive jumping snail Ovachlamys fulgens is a pest of ornamental plants and an intermediate host of a nematode that causes eosinophilic meningitis. We expand its distribution to eight municipalities from Rio de Janeiro State, and one locality from the Paraná State, and generated for the first time partial sequences of the cytochrome c oxidase subunit I (COI) gene for Brazilian populations. External morphology, reproductive system, shell, radula, and jaw were also analyzed and described. Twenty-one lots were collected from Rio de Janeiro, Niterói, Magé, Miguel Pereira, Petrópolis, Teresópolis, Nova Friburgo, Bom Jardim and Paraty, in Rio de Janeiro State, and from Foz do Iguaçu, Paraná State. External morphology, shell and reproductive system were typical of O. fulgens, with some peculiarities found in the shell and radula. A single haplotype was found, which was 100% similar to sequences of COI available in GenBank for specimens from Japan and Argentina. The species seems to be adapted to many habitats and be rapidly expanding its distribution in Southeastern and Southern Brazil, and other South America countries. We highlight the importance of monitoring O. fulgens, considering its potential to compete with native mollusks, attack several plants, and be a transmitter of diseases.

Infection by Angiostrongylus cantonensis in both humans and the snail Achatina (Lissachatina) fulica in the city of Macapá, in the Amazon Region of Brazil

In January and February 2019, a malacological survey was conducted in the area surrounding the residence of a 12-year-old child that had contracted cerebral angiostrongyliasis in the municipality of Macapá, capital of the Amapá State, northern Brazil. The serological examination was positive for Angiostrongylus cantonensis infection, the principal etiological agent of this parasitosis. A sample of 54 molluscs was artificially and individually digested for parasitological analysis, containing 38 specimens of Achatina fulica, nine specimens of Bulimulus tenuissimus and seven specimens of Sarasinula linguaeformis. A. fulica was the most abundant mollusc, and the only species infected with A. cantonensis, as well as presenting co-infections with other nematodes. This is the first report of cerebral angiostrongyliasis in the Amazon Region, and the first record of A. fulica infected with A. cantonensis in Amapá. These findings highlight the potential risks of human angiostrongyliasis, and the need to implement public health measures to control the spread of the disease.
