Introdução : A obesidade tem sido associada ao aparecimento ou piora de doenças autoimunes.
Objetivo : Revisar a presença de autoanticorpos em obesos submetidos à cirurgia bariátrica e verificar a possível alteração deste perfil após o procedimento cirúrgico.
Método : Revisão narrativa da literatura efetuada no PubMed e Scielo. A busca utilizou as palavras-chave: “ obesidade, autoimunidade, fator antinuclear, cirurgia bariátrica” . Os critérios de inclusão compreenderam artigos de revisão, estudos experimentais, pesquisas pré-clínicas e clínicas, escritos em português e inglês e disponíveis em texto completo.
Resultado : Foram incluídos 63 artigos.
Conclusão : Existe alta prevalência de FAN positiva na população obesa e candidata à cirurgia bariátrica que diminui consideravelmente no pós-operatório. O mesmo não pode ser verificado com o fator reumatóide, anticorpos do perfil ENA e anti-ds DNA, anticorpos anticardiolipina IgG e IgM, e anticorpos anti-endomísio. Nenhuma variável clínica ou antropométrica pôde estar ligada à presença de autoanticorpos, salvo níveis de vitamina D que foram significativamente menores nos indivíduos no pré-operatório e FAN positivos.

ABSTRACT Introduction Instruments to manage adverse effects of endocrine therapy with Aromatase inhibitors (AI) may improve adherence and persistence to treatment and Health-Related Quality of Life (HRQL). The 31-item Cervantes Scale (CS-31) is an HRQL questionnaire with particularities of the perimenopausal and postmenopausal period that could be an appropriate instrument to assess HRQL in Breast Cancer (BC) survivors. Objective This study aimed to perform additional validation of the CS-31 for BC survivors undergoing adjuvant endocrine therapy. Methods This prospective study was performed at three time points named T0, T1, and T2: initial, intermediate, and final follow-up period, respectively, totaling 24 months of follow-up. At each time point, the participants completed the CS-31, Functional Assessment of Chronic Illness Therapy - Fatigue (FACIT-F), and Hospital Anxiety and Depression Scale (HADS). The internal consistency, construct validity, responsiveness analyses, and known-group validity of CS-31 were evaluated. Results This study included 89 postmenopausal women diagnosed with hormone receptor-positive early BC in adjuvant endocrine therapy with AI. The internal consistency was good (Cronbach's alpha = 0.89). Construct validity received a positive rating, with 100% of results consistent with prior hypotheses. A prospective improvement in HRQL was identified for the CS-31 Global Score and FACIT-F Total Score and for most of their domains. Furthermore, women with anxiety and depression by HADS presented worse HRQL by CS-31. Conclusion The authors identified that the CS-31 seems to be appropriate for use in oncology medical routine and may help to monitor adverse effects and HRQL of BC survivors during adjuvant endocrine therapy. The structure of food webs is a fundamental attribute of ecosystems, and their characterization provides an intrinsic knowledge of the trophic interactions among organisms and of nutrient and energy transfer within ecosystems. Over the last few decades, several chemical and biogeochemical approaches have been proposed to explore different aspects of food webs. In this sense, this study reviews the application of stable isotopes and mercury as main auxiliary tools to the characterization and modeling of aquatic food webs, including analytical and modeling advances, strengths and limitations. The metanalysis performed showed that the most used tools for trophic ecology studies are stable isotopes, and that they can provide better results when combined with mercury and specific chemical molecules such as amino and fatty acids. In addition, the statistical methods applied in the interpretation of results, such as isotope mixing models, have witnessed significant advances in the last two decades. All approaches have premises and limitations when applied to aquatic ecosystems, which must be well understood prior to results interpretation. The use of multiple tracers in trophic studies provides complementary information and in many cases is an appropriate alternative to overcome some limitations, allowing to expand the knowledge of the food webs. ecosystems decades sense acids addition models

Abstract Background: Arboviral diseases are a group of infectious diseases caused by viruses transmitted by arthropods, mainly mosquitoes. These diseases, such as those caused by the dengue (DENV), Zika (ZIKV), chikungunya (CHIKV), and yellow fever (YFV) viruses, have a significant impact worldwide. In this context, entomological surveillance plays a crucial role in the control and prevention of arboviruses by providing essential information on the presence, distribution, and activity of vector mosquitoes. Based on entomological surveillance, transovarian transmission provides information regarding the maintenance and dissemination of arboviruses. The objective of this study was to detect these arboviruses in Goiânia, Goiás, and analyze the occurrence of transovarian transmission. Methods: Aedes aegypti eggs were collected from different regions of Goiânia and cultivated under controlled laboratory conditions until the emergence of adult mosquitoes. Adult females were grouped into pools containing their heads and thoraxes. These pools were subsequently evaluated using reverse-transcription quantitative polymerase chain reaction (RT-qPCR) assay. Results: A total of 157 pools (N=1570) were analyzed, with two pools testing positive for CHIKV and one pool testing positive for ZIKV, indicating that the offspring resulting from transovarian transmission are potentially infectious. Conclusions: In summary, the demonstration of the vertical transmission mechanisms of CHIKV and ZIKV in A. aegypti serves as an alert to health authorities, as these diseases are still underreported, and their primary urban vector has likely acquired this capacity, contributing to the dissemination of these infections. ABSTRACT Asbestos was largely used in Brazil. It is a mineral that induces pleural and pulmonary fibrosis, and it is a potent carcinogen. Our objective was to develop recommendations for the performance of adequate imaging tests for screening asbestos-related diseases. We searched peer-reviewed publications, national and international technical documents, and specialists’ opinions on the theme. Based on that, the major recommendations are: Individuals exposed to asbestos at the workplace for ≥ 1 year or those with a history of environmental exposure for at least 5 years, all of those with a latency period > 20 years from the date of initial exposure, should initially undego HRCT of the chest for investigation. Individuals with pleural disease and/or asbestosis should be considered for regular lung cancer monitoring. Risk calculators should be adopted for lung cancer screening, with a risk estimate of 1.5%. Abstract Objective To analyze marital outcomes, divorce or separation, and its association with demographic, socioeconomic, and clinicopathological factors among breast cancer (BC) survivors after 2-years of diagnosis. Methods We performed a retrospective analysis of marital status at baseline and at years 1 and 2 of follow-up of women aged ≥ 18 years diagnosed with invasive BC participating in the AMAZONA III (GBECAM0115) study. The BC diagnosis occurred between January 2016 and March 2018 at 23 institutions in Brazil. Results Of the 2974 women enrolled in AMAZONA III, 599 were married or living under common law at baseline. Divorce or separation occurred in 35 (5.8%) patients at 2 years of follow-up. In the multivariate analysis, public health insurance coverage was associated with a higher risk of marital status change (8.25% vs. 2.79%, RR 3.09, 95% CI 1.39 - 7.03, p = 0.007). Women who underwent mastectomy, adenomastectomy or skin-sparing mastectomy were associated with a higher risk of divorce or separation (8.1% vs. 4.49%, RR 1.97, 95 CI 1.04 – 3.72, p = 0.0366) than those who underwent breast-conserving surgery. Conclusion Women covered by the public health system and those who underwent mastectomy, adenomastectomy or skin-sparing mastectomy were associated with a higher risk of divorce or separation. This evidence further supports the idea that long-term marital stability is associated with a complex interplay between socioeconomic conditions and stressors, such as BC diagnosis and treatment. ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. Resumo Introdução: É crucial integrar abordagens avaliativas realistas para habilidades fisioterapêuticas cardiovasculares no âmbito profissional. O Objective Structured Clinical Examination (OSCE) é notoriamente confiável e válido, entretanto, compreender a percepção do aluno após essa experiência é importante para a readequação dessa avaliação. Objetivo: Avaliar os pontos fortes e as limitações da aplicação do OSCE em discentes do curso de graduação em fisioterapia durante estágio supervisionado de fisioterapia cardiovascular. Métodos: Trata-se de um estudo descritivo, qualitativo, observacional e transversal. Os discentes foram submetidos ao OSCE e, após a finalização, aplicou-se um questionário para avaliação da percepção contendo sete itens: A) organização geral do exame; B) casos clínicos e instruções para o exame; C) adequação das tarefas exigidas; D) qualidade das explicações pós-exame; E) complexidade do conteúdo envolvido, em que o discente poderia atribuir uma nota de 1 a 5 (1 = insuficiente; 2 = regular; 3 = bom; 4 = ótimo; 5 = excelente); F) dificuldade com o gerenciamento do tempo; e G) estresse emocional. Os itens F e G tinham as notas: 1 = muito baixo; 2 = baixo; 3 = médio; 4 = alto; e 5 = muito alto. Resultados: Cinquenta e um alunos responderam ao questionário. Para o itens A, B, C, D e E, a resposta mais prevalente foi excelente (72,5%, 43,1%, 52,9%, 88,2% e 54,9% respectivamente). Em relação ao item F, 54,9% dos alunos relataram dificuldade média a muito alta em gerenciar o tempo. Em relação ao item G, 94,2% relataram nível muito alto de estresse emocional. Abstract Introduction: It is crucial to integrate realistic assessment approaches for cardiovascular physiotherapeutic skills in the professional context. The Objective Structured Clinical Examination (OSCE) is notably reliable and valid. However, understanding the student's perception after this experience is important for readjusting the assessment. Objective: To evaluate the strengths and limitations of applying the OSCE to undergraduate students during the Supervised Cardiovascular Physiotherapy Internship. Methods: Descriptive, qualitative, observational, and cross-sectional study. The students were submitted to the OSCE. After completion, a questionnaire was administered to assess their perception, containing seven items: A) General exam organization; B) Clinical vignettes and instructions; C) Adequacy of the tasks required; D) Quality of post-examination explanations; E) Complexity of the content involved, in which the student could assign a grade from 1 to 5 (1 = insufficient; 2 = fair; 3 = good; 4 = great; 5 = excellent); F) Difficulty with time management; and G) Emotional stress. Items F and G were rated: 1 = very low; 2 = low; 3 = moderate; 4 = high; and 5 = very high. Results: Fifty-one students responded to the questionnaire. For items A, B, C, D, and E, the most prevalent response was excellent (72.5%, 43.1%, 52.9%, 88.2%, and 54.9% respectively). Regarding item F, 54.9% of students reported moderate difficulty managing their time. Regarding item G, 94.2% reported a very high level of emotional stress. Conclusion: Application of the OSCE provided experiences of clinical cases with adequate complexity, in an organized manner and with an enriching final evaluation. ABSTRACT Background: Healthcare systems are currently ill-equipped to diagnose arboviruses rapidly and efficiently or to differentiate between various viruses. Methods: Utilizing molecular techniques, this study examined arbovirus infections in 459 patients from a public health unit in Goiânia-Goiás, Brazil, a region where arbovirus infection poses a significant public health challenge. Results: Nearly 60% of the analyzed samples tested positive for at least one arbovirus, and over 10% of the patients were co-infected with more than one virus. ABSTRACT Background: This study aimed to evaluate the effectiveness and safety of direct-acting antivirals (DAAs) for hepatitis C treatment by measuring sustained virologic response (SVR) and serious adverse events to help design effective interventions for reducing disease prevalence. Methods: This was a retrospective, observational, real-life study of patients with chronic hepatitis C receiving DAA treatment in the state of Ceará, Brazil. Data were collected in REDCap and analyzed using R® software by the Student's t, chi-square, and Fisher’s exact tests, with a significance level of 5%. Results: In this study, 1075 patients who were diagnosed with hepatitis C infection between October 2015 and October 2023 were included. The mean age of the participants was 56.6 ± 11 years and 60.2% were men. The sample included 51 HIV-infected patients (6.6%), 166 (15,4%) liver transplant recipients, 34 (3,1%) kidney transplant recipients, and 446 patients with cirrhosis (41.4%). The overall SVR rate was 96.4%. The sofosbuvir/daclatasvir/ribavirin regimen used in 354 (32.9%) patients achieved an SVR of 96%. The cure rate was 96.5%, with a lower SVR in patients with cirrhosis (93.4%) than in those with less severe fibrosis (97.9%) (p=0.0015). Serious adverse events associated with ribavirin use occurred in 3.5% of patients. Resumo Antecedentes A migrânea é uma cefaleia primária que afeta 14,4% da população global. Os mecanismos fisiopatológicos da migrânea envolvem a interação entre a ativação hipotalâmica, depressão cortical alastrante, estimulação trigeminal e componentes inflamatórios com inflamação neurogênica ou neuroinflamação. Objetivo Avaliar a frequência da variante genética IL1A -899C > T (rs1800587) em pacientes com migrânea e controles saudáveis, bem como sua associação com as manifestações clínicas da migrânea. Métodos Realizamos um estudo do tipo caso-controle com 92 pacientes com migrânea e 88 indivíduos saudáveis pareados por idade, sexo, índice de massa corporal (IMC) e etnia. Dados demográficos, antropométricos e clínicos foram obtidos. A variante IL1A -889C > T (rs1800587) foi identificada por meio de reação em cadeia da polimerase em tempo real. Resultados O estudo foi composto predominantemente por mulheres e indivíduos caucasianos, sem diferenças significativas em termos de idade, sexo, etnia e IMC entre os grupos de migrânea e controle. No grupo de migrânea, 57,6% apresentavam migrânea episódica e 45,7% tinham aura. Encontrou-se uma associação entre os pacientes portadores do genótipo CT e a presença de aura (CT: 57,7%; TT: 27,5%; p = 0,027), e os portadores dos genótipos CT e TT apresentaram taxas mais altas de osmofobia quando comparados com os portadores do genótipo CC (p = 0,003). A variante IL1A -889C > T não esteve associada à suscetibilidade à migrânea, a sua forma crônica ou episódica, bem como a outros sintomas relacionados à migrânea. Abstract Background Migraine belongs to the group of primary headaches, affecting 14.4% of the global population. The pathophysiological mechanisms of migraine involve the interplay between hypothalamic activation, cortical spreading depression, trigeminal stimulation, and inflammatory components with neurogenic inflammation or neuroinflammation. Objective To assess the frequency of the IL1A -899C > T (rs1800587) genetic variant in patients with migraine and healthy controls, as well as its association with the clinical manifestations of migraine. Methods We conducted a case-control study involving 92 migraine patients and 88 healthy controls matched for age, sex, body mass index (BMI), and ethnicity. Demographic, anthropometric, and clinical data were obtained. The IL1A -889C > T (rs1800587) variant was identified using real-time polymerase chain reaction. Results The study comprised predominantly women and Caucasian individuals, with no significant differences in terms of age, sex, ethnicity, or BMI observed between the migraine and control groups. Within the migraine group, 57.6% had episodic migraines, and 45.7% experienced aura. The patients carrying the CT genotype showed stronger associations with the presence of aura (CT: 57.7%; TT: 27.5%; p = 0.027), and those carrying the CT and TT genotypes showed higher osmophobia rates when compared with the CC genotype (p = 0.003). The IL1A -889C > T genetic variant was not associated with migraine susceptibility, be it chronic or episodic, nor to other symptoms associated with migraine. Large hiatal hernias (LHH) besides being more prevalent in the elderly, have different clinical presentation: fewer reflux, more mechanical symptoms and a greater possibility of acute, life-threatening complications such as gastric volvulus, ischemia and visceral mediastinal perforation. Thus, surgical indications are distinct from gastroesophageal reflux disease (GERD-related), sliding hiatal hernias. Heartburn tends to be less intense, while symptoms of chest pain, cough, discomfort, and tiredness are reported more frequently. Complaints of vomiting and dysphagia may suggest the presence of associated gastric volvulus. Signs of iron deficiency and anemia are found. Surgical indication is still controversial and was previously based on high mortality reported in emergency surgeries for gastric volvulus. Postoperative mortality is especially related to three factors: body mass index (BMI above 35), age over 70 years and presence of comorbidity.
Minimally invasive elective surgery should be offered to symptomatic individuals with good or reasonable performance status, regardless of age group. In asymptomatic and oligosymptomatic patients, besides obviously identifying the patient's desire, case-by-case analysis of surgical risk factors such as age, obesity and comorbidities, should be taken under consideration. One should also pay attention to situations with greater technical difficulty and risks of acute migration due to increased abdominal pressure (abdominoplasty, manual workers, spastic diseases). Technical alternatives such as partial fundoplication and anterior gastropexy can be considered.
We emphasize the importance of performing surgical procedures in cases of LHH in high-volume centers, with experienced surgeons.
As grandes hérnias de hiato (HHG), além de serem mais prevalentes em idosos, têm apresentação clínica diferente: menos refluxo, mais sintomas mecânicos e maior possibilidade de complicações agudas e potencialmente fatais, como vólvulo gástrico, isquemia e perfuração mediastinal visceral. Assim, as indicações cirúrgicas são distintas das hérnias de hiato por deslizamento, relacionadas à doença do refluxo gastroesofágico (DRGE). A azia tende a ser menos intensa, enquanto os sintomas de dor no peito, tosse, desconforto e cansaço são relatados com maior frequência. Queixas de vômitos e disfagia podem sugerir a presença de volvo gástrico associado. São encontrados sinais de deficiência de ferro e anemia. A indicação cirúrgica ainda é controversa e foi anteriormente baseada na alta mortalidade relatada em cirurgias de emergência para volvo gástrico. A mortalidade pós-operatória está especialmente relacionada a três fatores: índice de massa corporal (IMC acima de 35), idade superior a 70 anos e presença de comorbidades.
A cirurgia eletiva minimamente invasiva deve ser oferecida a indivíduos sintomáticos, com desempenho bom ou razoável, independentemente da faixa etária. Em pacientes assintomáticos e oligossintomáticos, além de obviamente identificar o desejo do paciente, deve-se levar em consideração a análise caso a caso dos fatores de risco cirúrgico, como idade, obesidade e comorbidades. Deve-se atentar também para situações de maior dificuldade técnica e riscos de migração aguda por aumento da pressão abdominal (abdominoplastia, trabalhos manuais, doenças espásticas). Alternativas técnicas como fundoplicatura parcial e gastropexia anterior podem ser consideradas.
Ressaltamos a importância da realização de procedimentos cirúrgicos nos casos de GHH em centros de grande volume, com cirurgiões experientes.

RESUMO O objetivo do estudo foi identificar as barreiras encontradas pelos profissionais de educação física no atendimento a pessoas com deficiência em meio à pandemia do Covid-19. A pesquisa foi descritiva transversal e de natureza exploratória. O instrumento utilizado para a realização deste estudo apoiou-se em um questionário de criação própria dos pesquisadores, sendo validado por meio do método face validty que constitui de três profissionais doutores da área da Educação Física que formularam parecer sobre o questionário. Os critérios de inclusão foram ser profissional de Educação Física, atuante com pessoas com deficiência por meio da prática esportiva. Já para critério de exclusão foram profissionais não graduados em educação física e sem registro profissional. A amostra do presente estudo foi constituída por 64 profissionais de Educação Física que oferecem aulas para pessoas com deficiência em todo território Brasileiro. Apresentaram uma significativa prevalência para variação de modelo de aulas, dificuldades no emprego da execução das atividades e o desinteresse dos alunos em realizar os exercícios de forma virtual. Conclui-se que as barreiras encontradas pelos profissionais de educação física que atuam diretamente com Pessoas com Deficiência foram as adaptações curriculares, metodológicas, as fragilidades do uso da ferramenta tecnológica e os fatores de ensino aprendizagem.
ABSTRACT The study aimed to identify the barriers encountered by physical education professionals in serving people with disabilities amid the Covid-19 pandemic. This study consisted of cross-sectional descriptive research of an exploratory nature. The instrument used to carry out this study was based on a questionnaire created by the researchers and validated through the face validity method, which consists of three professionals with a PhD in the field of physical education who formulated an opinion on the questionnaire. The inclusion criteria was to be a physical education professional working with people with disabilities through sports, while the exclusion criteria were professionals without a degree in physical education and professional registration. The present study sample consisted of 64 physical education professionals who offer classes for people with disabilities throughout Brazil. They presented a significant prevalence for the variation of the class model, difficulties in the use of the execution of the activities and the disinterest of the students in carrying out the exercises virtually. It is concluded that the barriers encountered by physical education professionals who work directly with People with Disabilities were the curricular and methodological adaptations, the weaknesses in using the technological tool and the teaching-learning factors.
