Validation of entrustable professional activities for use in neonatal care residency programs

Abstract Objective Define and develop a set of entrustable professional activities (EPAs) to link clinical training and assessment of the hospital components of neonatal care in neonatology medical residency programs. Methods An exploratory study was conducted in two phases using a modified Delphi approach. In the first phase, a committee of five neonatology residency program coordinators drafted an initial set of EPAs based on the national matrix of competencies and on EPAs defined by international organizations. In the second phase, a group of neonatal care physicians and medical residents rated the indispensability and clarity of the EPAs and provided comments and suggestions. Results Seven EPAs were drafted by the coordinators´ committee (n = 5) and used in the content validation process with a group (n = 37) of neonatal care physicians and medical residents. In the first Delphi round, all EPAs reached a content validity index (CVI) above 0.8. The coordinators´ committee analyzed comments and suggestions and revised the EPAs. A second Delphi round with the revised EPAs was conducted to validate and all items maintained a CVI above 0.8 for indispensability and clarity. Conclusion Seven entrustable professional activities were developed to assess residents in the hospital components of neonatal care medicine. These EPAs might contribute to implementing competency-based neonatology medical residency programs grounded in core professional activities. Resumo O aumento da modificação dos habitats e da perda de espécies demanda esforços consistentes para descrever e compreender os padrões de biodiversidade. O programa BIOTA/FAPESP foi criado nesse contexto e é uma iniciativa de sucesso para promover estudos em biodiversidade e conservação no Brasil. O BIOTA/Araçá é um projeto interdisciplinar que promoveu uma avaliação detalhada da biodiversidade da Baía do Araçá, um ecossistema costeiro localizado ao Norte do estado de São Paulo, Sudeste do Brasil. A baía engloba múltiplos habitats, tais como praias, manguezais, costões rochosos, e uma planície de maré, e também fornece importantes serviços ecossistêmicos. Infelizmente, a baía está sujeita à conflitos sócio-ambientais complexos que contrastam demandas econômicas, sociais e ambientais (i.e. a expansão das atividades do porto vizinho vs. a pesca artesanal de pequena escala e a proteção da biodiversidade). O presente estudo apresenta um levantamento das espécies bentônicas que ocorrem nos diferentes habitats da Baía do Araçá, incluindo dados obtidos durante o projeto BIOTA/Araçá e de investigações realizadas anteriormente na área. As espécies bentônicas desempenham um papel importante no ambiente marinho, e estudar a diversidade desses organismos que vivem associados ao fundo é indispensável para compreender o funcionamento do meio ambiente. A macrofauna, meiofauna, e microorganismos associados aos fundos consolidado e inconsolidado foram listados, e informações adicionais foram fornecidas para cada espécie, tais como a distribuição geográfica e nos habitats. O checklist inclui 826 espécies, quase 70% registradas durante o projeto BIOTA/Araçá. Os taxa mais especiosos foram os anelídeos (225 spp.), moluscos (194 spp.), e crustáceos (177 spp.). Entre as espécies bentônicas listadas, sete são endêmicas da Baía do Araçá, 14 são consideradas ameaçadas de extinção, e sete são exploradas economicamente. A baía é a localidade tipo de vários taxa, e 11 novas espécies bentônicas foram descritas com base em espécimes amostrados durante o projeto. Abstract Increasing habitat modification and species loss demand consistent efforts to describe and understand biodiversity patterns. The BIOTA/FAPESP Program was created in this context and it has been a successful initiative to promote studies on biodiversity and conservation in Brazil. The BIOTA/Araçá is an interdisciplinary project that provided a detailed evaluation of the biodiversity of Araçá Bay, a coastal seascape located on the North coast of the state of São Paulo, Southeast Brazil. The bay encompasses multiple habitats, such as beaches, mangroves, rocky shores, and a tidal flat, and provides important ecosystem services. Unfortunately, the bay is the subject of complex social-environmental conflicts that oppose economic, social, and environmental demands (i.e., the expansion of neighboring harbor activities vs. small-scale artisanal fisheries and protection of biodiversity). The present study presents a survey of the benthic species occurring in the different habitats of Araçá Bay, including data obtained during the BIOTA/Araçá project and previous assessments of the area. The benthic species play an important role in marine environments and studying the diversity of these organisms that live associated with the bottom is indispensable for comprehending the environment’s functioning. The macrofauna, meiofauna, and microorganisms associated with soft and hard bottom were listed, and additional information, such as the habitat and geographical distribution, were provided for each species. The checklist includes 826 species, almost 70% recorded during the BIOTA/Araçá project. The most speciose taxa were the annelids (225 spp.), mollusks (194 spp.), and crustaceans (177 spp.). Seven benthic species are endemic to Araçá Bay, 14 are considered threatened, and seven are economically exploited. Furthermore, the bay is the type locality of many taxa, and 11 new benthic species were described based on specimens sampled during the project. ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. Tucumã (Astrocaryum aculeatum) pulp presents a large number of secondary metabolites that can mediate the synthesis of metallic nanoparticles through a greener and more sustainable synthetic route than traditional ones. This study investigated the production of silver nanoparticles (AgNPs) mediated by aqueous extracts of tucumã pulp assessing different parameters, such as temperature, concentration of extract, and pH values of the reaction media. The alkaline reaction media produced the most intense and well-defined bands around 400 nm in the electronic spectra, indicating the formation of AgNPs in the three concentrations of tucumã pulp evaluated. The increase in the temperature of the reaction media also enhanced the surface plasmon resonance band of AgNPs but it increased the polydispersivity of produced nanoparticles according to the dynamic light scattering (DLS) and transmission electron microscopy (TEM) data. Zeta potential results indicated moderate stability of the AgNPs (close to -30 mV). Antimicrobial assessment of the colloidal metal particles indicated microbicide activity against the Gram-negative Escherichia coli bacterium. High water consumption during irrigation is one of the main problems in agriculture. The preparation of a material that prolongs the water retention capacity of the soil is important. This article aimed to prepare superabsorbent hydrogels by the complexation of chitosan (CHI), with high degree of deacetylation (DD), and carboxymethylcellulose (CMC), with high degree of substitution (SD). Different parameters (concentration and polymeric solutions addition order, polysaccharides’ ratio, pH) were studied. Morphology, thermal stability, chemical composition, mechanical strength, and swelling kinetics of hydrogels in water were evaluated. A hydrogel formation mechanism was discussed and proposed. Results showed that most samples prepared at acid pH disintegrated during swelling test. The morphology of the hydrogels varied with the pH. It was obtained superabsorbent hydrogels (swelling > 400%) based on CMC (SD = 2.23) and CHI (DD = 92%), at basic pH, with good compression strength, and applicable as soil conditioner. Abstract Fibrinolytic proteases are a promising alternative in the pharmaceutical industry, they are used in the treatment of cardiovascular diseases, especially thrombosis. Microorganisms are the most interesting source of fibrinolytic proteases. The aim of this study was the production of fibrinolytic protease from Streptomyces parvulus DPUA 1573, the recovery of the protease by aqueous two-phase system and partial biochemical characterization of the enzyme. The aqueous two-phase system was performed according to a 24-full factorial design using polyethylene glycol molar mass, polyethylene glycol concentration, citrate concentration and pH as independent variables. It was analyzed the effect of different ions, surfactants, inhibitors, pH and temperature on enzyme activity. The best conditions for purifying the enzyme were 17.5% polyethylene glycol 8,000, 15% Phosphate and pH 8.0, it was obtained a partition coefficient of 7.33, a yield of 57.49% and a purification factor of 2.10-fold. There was an increase in enzyme activity in the presence of Fe2+ and a decrease in the presence of $\beta$-Mercaptoethanol, phenylmethylsulfonyl fluoride and Iodoacetic acid. The optimum pH was 7.0 and the optimum temperature was 40 ºC. The purified protease exhibited a molecular mass of 41 kDa. The fibrinolytic protease from Streptomyces parvulus proved to be a viable option for the development of a possible drug with fibrinolytic action.

Abstract The Program for Biodiversity Research (PPBio) is an innovative program designed to integrate all biodiversity research stakeholders. Operating since 2004, it has installed long-term ecological research sites throughout Brazil and its logic has been applied in some other southern-hemisphere countries. The program supports all aspects of research necessary to understand biodiversity and the processes that affect it. There are presently 161 sampling sites (see some of them at Supplementary Appendix), most of which use a standardized methodology that allows comparisons across biomes and through time. To date, there are about 1200 publications associated with PPBio that cover topics ranging from natural history to genetics and species distributions. Most of the field data and metadata are available through PPBio web sites or DataONE. Metadata is available for researchers that intend to explore the different faces of Brazilian biodiversity spatio-temporal variation, as well as for managers intending to improve conservation strategies. The Program also fostered, directly and indirectly, local technical capacity building, and supported the training of hundreds of undergraduate and graduate students. The main challenge is maintaining the long-term funding necessary to understand biodiversity patterns and processes under pressure from global environmental changes.

Abstract A total of 40 lambs were divided into four different treatments according to the inclusion level of the macadamia nut cake: C-control (0%), M1 (6.5%), M2 (12%) and M3 (20%). Feed was provided twice a day; animal weighing along with body condition scoring occurred within a 14-day interval. The lambs were slaughtered at the end of the performance test. Analysis of variance was performed through the Mixed procedure of the SAS, as well as linear and quadratic regression analysis. The groups presented differences between the treatments (P <0.05) for dry matter intake (DMI), ethereal extract intake (EEI), consumption in relation to live weight percentage and feed conversion ratio (FCR). The lowest mean DMI was obtained by the animals that received the M2 and differed from the C treatment, whose average was the highest. The EEI was highest for the M3 group and the FCR was also better for this group. There was a linear effect for EEI and FCR, and quadratic for crude protein intake. There was no effect for carcass characteristics, and only initial pH had a decreasing linear effect. The macadamia nut cake was effective in promoting the performance of the animals, since there was an improvement in feed conversion.

ABSTRACT Objective: Our aim is to establish genetic diagnosis of congenital generalized lipodystrophy (CGL) using targeted massively parallel sequencing (MPS), also known as next-generation sequencing (NGS). Subjects and methods: Nine unrelated individuals with a clinical diagnosis of CGL were recruited. We used a customized panel to capture genes related to genetic lipodystrophies. DNA libraries were generated, sequenced using the Illumina MiSeq, and bioinformatics analysis was performed. Results: An accurate genetic diagnosis was stated for all nine patients. Four had pathogenic variants in AGPAT2 and three in BSCL2. Three large homozygous deletions in AGPAT2 were identified by copy-number variant analysis. Conclusions: Although we have found allelic variants in only 2 genes related to CGL, the panel was able to identify different variants including deletions that would have been missed by Sanger sequencing. We believe that MPS is a valuable tool for the genetic diagnosis of multi-genes related diseases, including CGL.

Abstract The aim of this study was to evaluate the influence of two sub-periods of the calving season and two cow maturity stages on the efficiency of beef cows and their calves. A total of 159 cow-calf pairs were divided by calving time (early or late) within the calving season and maturity stage (young or adult). Calves were weaned at 42 or 63 days after birth and evaluated until 210 days of age. Cows and calves had their development examined based on their weight and body condition score at calving, at weaning, and at 210 days. Reproductive performance was evaluated on the basis of time to become pregnant again. Milk yield was assessed by the direct method on three occasions spaced 21 days apart. Adult cows were heavier than young cows, at calving (398.5 vs 327.5 kg, respectively), weaning (397.3 vs 324.1 kg, respectively) at the end breeding season (424.1 vs 342.1 kg, respectively). Reproductive performance was influenced by calving time. Adult cows had higher pregnancy rates (83.75 and 69.17%, for early and for late calving, respectively) than young cows (57.03 and 35.01% for early and for late-calving, respectively). Calves from early-calving young cows weighed 158.8 kg at 210 days of age vs. 123.7 kg for those born from late-calving adult cows. However, late-calving cows produced 10.7% more milk than those that calved early in the season (227.0 vs 205.0 liters, respectively). Early calving associated with non-requirement of growth determine higher productivity efficiency in beef cows.

ABSTRACT Leishmania infantum is an etiologic agent of visceral leishmaniasis. This disease is a neglected disease that can be fatal if not treated and additionally, the few therapeutic option present several drawbacks, including difficult route of administration and toxicity, which turn the search for new therapeutic alternatives necessary. Herein, we evaluated the leishmanicidal in vitro activity of the solanum extract from Solanum lycocarpum A. St.-Hil., Solanaceae, and the isolated alkaloids solasodine, solamargine and solasonine against promastigotes and intracellular amastigotes of L. infantum. Solasodine (IC50-pro = 4.7 µg/ml; IC50-ama = 10.8 µg/ml) and solamargine (IC50-pro = 8.1 µg/ml; IC50-ama = 3.0 µg/ml) exhibited interesting leishmanicidal ativity. Solasonine was approximately four-times (Selective Index 3.7) more selective to the parasite than to the host cells. This data suggest that solasonine might be considered as a potential drug candidate for leishmaniasis treatment.

RESUMO A alelopatia contribui com o manejo sustentável de plantas daninhas em áreas de cultivo, devido à capacidade de inibir o desenvolvimento de espécies infestantes. Objetivou-se avaliar o efeito alelopático do extrato de Pachyrhizus erosus na germinação de Euphorbia heterophylla e Bidens pilosa. Utilizou-se delineamento inteiramente casualizado, em fatorial 2 x 2 x 4, com quatro repetições. O primeiro fator correspondeu à parte da planta (parte aérea e raiz tuberosa), o segundo ao método de preparo (água à temperatura ambiente e em ebulição) e o terceiro à concentração dos extratos (0 %, 25 %, 50 % e 100 %). Análises fitoquímicas foram realizadas para identificar a presença de metabólitos secundários. As variáveis porcentagem de germinação, índice de velocidade de germinação e biomassa seca reduziram-se conforme o aumento da concentração dos extratos aquosos, tanto para E. heterophylla quanto para B. pilosa. Houve 18 % e 92 % de redução na germinação de E. heterophylla e B. pilosa, respectivamente, quando utilizado o extrato bruto da parte aérea (ebulição) de P. erosus, com redução, ainda, de 91 % da biomassa seca das plântulas de E. heterophylla. O extrato da raiz tuberosa (ebulição) reduziu em 18 % e 80 % a porcentagem de germinação de E. heterophylla e B. pilosa, respectivamente. A avaliação fitoquímica indicou a presença de flavonoides, cumarinas e saponinas para o extrato da parte aérea e da raiz tuberosa, enquanto taninos foram observados apenas no extrato da parte aérea. Conclui-se que os extratos preparados com P. erosus possuem efeito alelopático nas espécies avaliadas.
ABSTRACT Allelopathy contributes to the sustainable management of weeds in growing areas, due to its ability to inhibit the development of weed species. This study aimed to evaluate the allelopathic effect of Pachyrhizus erosus extract on Euphorbia heterophylla and Bidens pilosa germination. The experiment was carried out in a completely randomized design, in a 2 x 2 x 4 factorial scheme, with four replications. The first factor corresponded to the plant portion (shoot and tuberous root), the second to the extract preparation method (water at room temperature and boiling) and the third to the extract concentrations (0 %, 25 %, 50 % and 100 %). Phytochemical analyses were performed to identify the presence of secondary metabolites. The variables germination percentage, germination rate index and dry biomass decreased with increasing aqueous extract concentrations for both E. heterophylla and B. pilosa. When applied a P. erosus crude shoot extract (boiling), decreases of 18 % and 92 % were observed in the E. heterophylla and B. pilosa germination, respectively, with a decrease of 91 % in the dry biomass of E. heterophylla seedlings. The tuberous root extract (boiling), on the other hand, reduced the germination percentages for E. heterophylla and B. pilosa in 18 % and 80 %, respectively. The phytochemical evaluation indicated the presence of flavonoids, coumarins and saponins in the shoot and tuberous root extracts, while tannins were only observed in the shoot extract. Thus, it can be concluded that P. erosus extracts display an allelopathic effect on the weed species evaluated.

Expression of tumor suppressors miR-195 and let-7a as potential biomarkers of invasive breast cancer

OBJECTIVE: MicroRNAs (miRNAs) are small non-coding RNAs that regulate gene expression at the posttranscriptional level. Some miRNAs, including let-7a and miR-195, have been described as tumor suppressors. However, the roles of these microRNAs in breast cancer progression remain controversial. The aim of this study is to evaluate miR-195 and let-7a expression as potential biomarkers of invasive breast cancer. METHODS: In the present study, 200 individuals were separated into three groups: (i) 72 women constituting the control group who were selected according to rigorous and well-established criteria; (ii) 56 patients with benign breast tumors; and (iii) 72 patients with malignant breast cancers of different clinical stages. The miR-195 and let-7a expression levels in serum were evaluated by real-time PCR. The results were assessed alone and in combination, and the analysis included an estimation of sensitivity and specificity in ROC curves. RESULTS: Compared with the benign and control groups, both microRNAs were downregulated in the malignant breast cancer patient group. Compared with the malignant group, the combination of both biomarkers in the control and benign groups showed good sensitivity and specificity in the serum with AUCs of 0.75 and 0.72, respectively. The biomarker combination for the control group versus the malignant group exhibited a better sensitivity and specificity than for the benign group versus the malignant group. CONCLUSION: These findings support the evidence that the analysis of miR-195 and let-7a can be used as a non-invasive biomarker for breast cancer detection.

DETERMINAÇÃO DE ENXOFRE EM SHAMPOO POR ESPECTROFOTOMETRIA UV-VIS: AVALIAÇÃO DE MÉTODOS DE PREPARO DE AMOSTRAS

Dry ashing, dissolution in aqueous medium or wet digestion methods were evaluated for sample preparation of shampoo for the subsequent indirect determination of sulfur by UV-Vis spectrophotometry. Among the sample preparation methods evaluated, microwave-assisted wet digestion using 7 mol L-1 HNO3 was the most suitable method for this purpose. Accuracy of the proposed method was evaluated by recovery test using standard solution, and the recovery was about 96%. Moreover, for comparison of results, S (as SO42-) was also determined by ion chromatography, after microwave-assisted wet digestion using concentrated HNO3, and no significant difference was observed between the obtained results (according to the t-test for a 95% confidence level, p = 0.288). Relative standard deviations were always lower than 10% and the limit of detection was 0.007% w/v. Finally, the proposed method was applied for analysis of sixteen shampoos used to many types of hair, from different brands produced in Brazil, and the concentration of S ranged from 0.78 to 2.82% w/v.
