RESUMO A dooença de Alzheimer (DA) é uma demência que acomete uma grande parcela da população idosa e caracteriza-se pela presença de emaranhados neurofibrilares e placas senis. O traumatismo cranioencefálico (TCE) é uma lesão não degenerativa causada por uma força mecânica externa. Uma das principais causas de TCE é a lesão axonal difusa (LAD), causada por mecanismos de aceleração-desaceleração. Objetivo: Entender as diferenças dos mecanismos funcionais entre os grupos - DA e LAD do ponto de vista eletroencefalográfico. Métodos: Participaram deste estudo 56 indivíduos adultos. Destes, 20 com DA, 19 com LAD e 17 adultos saudáveis submetidos ao EEG de alta resolução com 128 canais. As fontes corticais dos ritmos do EEG foram estimadas pela análise por tomografia eletromagnética exata de baixa resolução (eLORETA). Resultados: A análise por eLORETA mostrou que, em comparação ao grupo controle (CTL), o grupo DA apresentou aumento da atividade teta nos lobos parietal e frontal e diminuição da atividade alfa 2 nos lobos parietal, frontal, límbico e occipital. Em comparação ao grupo CTL, o grupo LAD apresentou aumento da atividade teta nas áreas límbica, occipital sub-lobar e temporal. Conclusão: Os resultados sugerem que os indivíduos com DA e com LAD apresentam comprometimento da atividade elétrica em áreas importantes para a memória e aprendizagem.

ABSTRACT Alzheimer's disease (AD) is a dementia that affects a large contingent of the elderly population characterized by the presence of neurofibrillary tangles and senile plaques. Traumatic brain injury (TBI) is a non-degenerative injury caused by an external mechanical force. One of the main causes of TBI is diffuse axonal injury (DAI), promoted by acceleration-deceleration mechanisms. Objective: To understand the electroencephalographic differences in functional mechanisms between AD and DAI groups. Methods: The study included 20 subjects with AD, 19 with DAI and 17 healthy adults submitted to high resolution EEG with 128 channels. Cortical sources of EEG rhythms were estimated by exact low-resolution electromagnetic tomography (eLORETA) analysis. Results: The eLORETA analysis showed that, in comparison to the control (CTL) group, the AD group had increased theta activity in the parietal and frontal lobes and decreased alpha 2 activity in the parietal, frontal, limbic and occipital lobes. In comparison to the CTL group, the DAI group had increased theta activity in the limbic, occipital sublobar and temporal areas. Conclusion: The results suggest that individuals with AD and DAI have impairment of electrical activity in areas important for memory and learning.

Introdução: São escassos estudos dos custos dos insumos consumidos em hemodiálise e, dentre estes gastos, os compostos que compõem o dialisato estão entre os valores considerados como representativos nessa terapia. Contudo, não foram encontrados estudos que orientem sobre o comportamento de custos dessas soluções. Objetivo: O objetivo do artigo é avaliar se há desperdício no consumo de soluções alcalinas em hemodiálise ambulatorial e, consequentemente, a possibilidade de redução no custo a partir da simulação de padronização no processo de estabelecimento do fluxo do dialisato nos períodos entre turnos em sessões de hemodiálise ambulatorial. Métodos: Partindo de um estudo observacional analítico, foi realizada uma simulação de 20 cenários, sendo 10 estabelecidos pela padronização dos processos de controle no fluxo do dialisato nos intervalos das sessões. A combinação dos dados foi realizada tomando por base os preços de três fornecedores de soluções alcalinas líquidas ou em pó. Resultados: Observou-se, dentre os cenários com processos padronizados, uma variação entre 7,7% e 33,3% de economia no custo da solução alcalina (em pó ou líquida), pela redução do desperdício. Conclusão: É possível refrear o desperdício no uso de soluções alcalinas, tanto em pó quanto líquidas e, consequentemente, seus custos, a partir da padronização na redução do fluxo de dialisato durante os intervalos verificados entre os turnos na hemodiálise ambulatorial. Todavia, estes resultados estão condicionados ao comprometimento de profissionais de saúde, principalmente no que tange ao exercício da supervisão e controle das atividades no desdobramento da função qualidade.

Introduction: There are few studies about costs of inputs used in hemodialysis and among these expenditures, the compounds that make up the dialysate are one of the values considered as representative of this therapy. However, there aren’t costs studies that guiding solutions. Objective: The objective of this article is discuss whether there is wasteful of alkaline solutions in ambulatory hemodialysis and hence the possibility of reduction in cost from the standardization process simulation of establishment of dialysate flow in periods between shifts in hemodialysis outpatients. Methods: Starting from an observational analytic, a simulation was performed twenty case scenarios, which ten cases established by standardizing processes control on the dialysate flow in recession. The combination of data was performed using as a basis the prices of three suppliers of alkali liquid or powder. Results: It was observed among the scenarios with standardized processes, ranging between 7.7% and 33.3% savings in the alkaline solution cost (powder or liquid), by reducing waste. Conclusion: It is possible to restrain the wasteful use of alkaline solutions, both powder and liquid. Consequently, its cost from the patterning on reducing the flow of dialysate during the intervals between shifts observed in the outpatient hemodialysis. However, these results are conditional upon the commitment of health professionals, mainly to supervision exercise and control of activities in quality function deployment.

INTRODUÇÃO A concomitância de periodontite crônica (PC) em pacientes com doença renal crônica (DRC) tem sido associada a desfechos adversos. A deficiência de vitamina D (25(OH)D) parece desempenhar papel importante na PC e níveis inadequados de vitamina D têm sido descritos em pacientes com DRC. OBJETIVO: Examinar a relação entre níveis séricos de vitamina D e PC em pacientes com DRC pré-dialítica. MÉTODO: Estudo de caso-controle, definidos, respectivamente, como pacientes com DRC e PC e DRC sem PC. Os dados demográficos, de exame físico e laboratoriais foram obtidos no dia da consulta. A DRC foi definida e estagiada segundo a NKF QDOKI TM. Os níveis séricos de 25(OH) D foram dosados por quimioluminescência quando da avaliação da PC, a qual foi caracterizada segundo os critérios de Academia Americana de Periodontologia (1999). Os resultados de 25(OH)D foram estratificados em deficiência (< 14 ηg/mL), insuficiência (15-29 ηg/mL) e suficiência (> 30 ηg/mL). RESULTADOS: Um total de 29 pacientes foram estudados, 15 no grupo caso e 14 no grupo controle. Os pacientes casos apresentaram mediana de 25(OH) D inferior a dos pacientes controles (22,6 vs. 28,6 ηg/mL; p < 0,01). A frequência de pacientes casos com insuficiência/deficiência de vitamina D foi maior do que entre os pacientes controles (93,3% vs. 57,1%, p < 0,004). Por outro lado, o percentual de pacientes com suficiência de vitamina D foi maior entre os controles se comparados aos integrantes do grupo casos (42,9% vs. 6,7%, p < 0,004). CONCLUSÃO: Em pacientes com DRC, a deficiência de vitamina D se associa com PC.

INTRODUCTION: Concomitance of chronic periodontitis (CP) in patients with chronic kidney disease (CKD) have been associated with adverse outcomes. Vitamin D (25(OH)D) deficiency my play a role in CP and inadequate vitamin D status is common among patients with CKD. OBJECTIVE: To examine the relationship between vitamin 25(OH)D and CP in patients with CKD not yet on dialysis. METHOD: A case-control study was conducted. Cases and controls were defined as patients with CKD with and without CP, respectively. The demographic, clinical and laboratory data were obtained when the patient was attended in the outpatient clinic. CKD was defined and staged according to the NKF QDOKI TM. Serum 25(OH) D levels were measured by chemiluminescence when assessing the CP, which was definined according to the American Academy of Periodontoly (1999). Serum 25(OH)D levels were stratified into deficient (< 14 ηg/mL), insufficient (15-29 ηg/mL) and sufficiency (> 30 ηg/ mL). RESULTS: A total of 15 cases were compared with 14 controls. Cases had lower median 25(OH)D levels than controls (22.6 versus 28.6 ηg/mL, p < 0.01) and were more likely to be categorized as vitamin D insufficiency/deficiency (93,3% versus 57,1%, p < 0,004). On the other hand, the percentage of controls with vitamin D sufficiency was higher then cases (42,9% versus 6,7%, p < 0,004). CONCLUSION: In patients with CKD not yet on dialysis, vitamin D deficiency is associated with CP.

Diversos estudos têm investigado a relação entre a atividade assimétrica do EEG no córtex frontal e mudanças no humor. Adotando tal abordagem, o presente estudo teve como objetivo investigar a associação entre os estados de oscilação na assimetria frontal de alfa e mudanças no estado emocional ou motivacional após 24h de privação de sono. Os resultados mostram que 24h de privação de sono ocasionaram alterações significativas nos valores de assimetria. Ativação cerebral mudou do hemisfério esquerdo, antes da privação de sono, para o hemisfério direito, após a privação de sono, em todos os pares de eletrodos frontais. Além disso, de acordo com a escala relacionada aos efeitos subjetivos do humor após privação de sono, os sujeitos mostraram-se significativamente menos alerta e ativos e mais sonolentos. É possível que as duas variáveis estejam associadas, embora a relação causal seja ainda incerta. Estudos serão ainda necessários para que se possa estabelecer a viabilidade da assimetria pré-frontal e da hipótese de lateralização cerebral na elucidação de mudanças emocionais relacionadas à privação ou falta de sono.

Several studies have investigated the relationship between asymmetrical EEG activity over the frontal cortex and mood. This study aimed at investigating the association between state fluctuations in frontal alpha EEG asymmetry and state changes followed by 24 h of sleep deprivation (SD). Our results show that sleep deprivation caused a significant alteration in the asymmetry values. Activation shifted from the left hemisphere, before SD, to the right hemisphere, after SD, in all frontal electrode pairs. In addition, according to the self-rating scale of SD-related mood effects, subjects became significantly less alerted and active, and sleepier. According to these results, increased right prefrontal activation might be potentially associated with the negative mood states typically seen after sleep deprivation, although the causal relationship is still uncertain. However, more studies will be necessary to establish the viability of EEG asymmetry and the cerebral lateralization hypothesis to explain the SD-related affective changes.
