Resumo O presente caso clínico descreve o tratamento não cirúrgico de uma parestesia do nervo alveolar inferior provocada pela extrusão de medicação intracanalar de hidróxido de cálcio. Uma paciente de 45 anos de idade apresentou-se referindo como queixa principal a dormência e dor na região do lábio inferior esquerdo com 2 meses de evolução. O exame radiográfico revelou a extrusão de hidróxido de cálcio no canal mandibular. Irrigação com soro fisiológico, usando uma agulha 27-G, foi usada para eliminar a maior quantidade possível de hidróxido de cálcio do espaço intracanalar, mas revelou-se ineficaz na eliminação do material dos tecidos circundantes. O tratamento endodôntico foi concluído no espaço de uma semana, e o tratamento não cirúrgico da parestesia foi realizado recorrendo a esteroides e pregabalina por um período de 1 mês. No controlo de 1 ano, a parestesia havia regredido significativamente. Na visita de controlo dos 4 anos, constatou-se a completa resolução da parestesia, e o controlo radiográfico evidenciou a completa reabsorção do hidróxido de cálcio

Abstract The present case report describes the non-surgical treatment of alveolar inferior nerve paresthesia caused by the extrusion of calcium hydroxide as intracanal medicament. A 45-year-old female patient had a chief complaint of numbness and pain in the left lower lip region for 2 months. Radiographic examination revealed calcium hydroxide overflow into the mandibular canal. Irrigation of the root canal system with saline using a 27-gauge needle was done to remove as much calcium hydroxide as possible but proved ineffective in removing material from the outside tissues. Root canal treatment was completed in 1 week, and non-surgical paresthesia treatment with steroids and pregabalin was done for one month. In the 1-year follow-up control, the paresthesia had reduced significantly. In a 4-year follow-up visit, there was a complete resolution of paresthesia, and the radiographic analysis showed that calcium hydroxide was completely resorbed.

Abstract Doctrines of earthquakes and also the latest approaches of earthquake resistant building design in standards need to be revised periodically. While the revisions and updates in the American standards occur over periods of three or five years including limited subjects, in Turkey the same revisions are done once over long periods including the whole subjects of the standards. As examples the standards of 1975, 1998, 2007 and finally 2018 could be given. Especially, in 2018 standard (TBEC-2018) many changes were made over concepts and criteria. The procedure of calculating the earthquake loads in 2018 standards is similar to the one in the American standards of (ASCE-7-16), however for the element design the changes are shown as developments over the one of 2007 earthquake standard (TEC-2007). The changes made by 2018 standard for calculations of earthquake loads and their effects on civil engineering are very important factors of new building design. The earthquake load affecting a building which is the first factor of earthquake resistant building design shows important differences according to the condition changes in the standard. Based on this motivation in this study reinforced concrete frame type buildings of different elevations were researched by using ETABS (structural software for building analysis and design) according to linear equivalent seismic load method. According to the analysis results of the chosen buildings, a comparison forthe base shear force, top displacement and relative story displacement between TEC-2007, TBEC-2018 and ASCE 7-16 standards was carried out. From the analysis results, it is found that for most of the soil classes while the maximum base shear forces in 3 and 5- story buildings are achievedat TEC-2007, the maximum base shear forces in 7 and 9- story buildings are achieved at TBEC-2018. Also, it is predicted that the higher increment in the design forces of buildings with higher elevations is obtained at TBEC-2018 for strong soils, and at TEC-2007 for weak soils. By considering cracked sections at TBEC-2018 the calculations displacement and period was affected as periods in TBEC-2018 were increased by almost 34% respected to TEC-2007. The same increment ratio was determined for ASCE 7-16 as 45%. Also, as a response for the increments in period, the spectral acceleration determined from the elastic spectrum diagram was decreased. At the end of the study, nonlinear performance analysis was also performed and performance points were determined according to the demand spectra of the seismic codes. ASCE's demand displacement values are in any case lower than Turkish codes. TBEC-2018 reveals less displacement demands in high-rise buildings than TEC-2007. The closest results for the three regulations occurred on the softest grounds.According to the results obtained from the static pushover analysis, a ductile behavior occurred in all of the structural systems and plastic hinge mechanism started from the beams firstly.

ABSTRACT Objectives: Surgical treatment is indicated in patients where medical therapy fails to prove beneficial or in patients who develop complications related with bladder outlet obstruction. In our study, we developed a new surgical technique which can be defined as Transvesical Resection of Prostate (TVRP) without using the urethra. This method was previously described in our articles (1). Materials and Methods: A 62-years-old male patient, using an alpha blocker agent for 5 years, reported increased discomfort with urination. His findings were as follows: PSA: 1.2 ng/dL, prostate volume: 45 cc, digital rectal examination: benign, IPSS: 30, QoL: 5, Qmax: 6, urine volume: 225 cc, post-mictional residue: 65 cc. Eventually the patient was informed and prostate resection decision was made. Results: Suprapubic catheter was removed 1 day after surgery and the patient was discharged. Urethral catheter was removed 4 days after urine output became clear. No complications developed after the operation. At postoperative 1st month, Qmax was 22, urine volume was 260 cc, post-mictional residue was 40 cc, IPSS was 8, QoL was 1, and the pathology was benign prostate tissue. Conclusions: Urethral stricture is one of the most important postoperative complications of TURP. The incidence of urethral stricture is reported between 2.2% and 9.8% in different series (2–5). In this technique which we developed, urethra is not used and prostate is removed through the bladder, similar to open prostatectomies. For this reason, we suggest that it has an advantage over TURP, regarding urethral stricture development.

ABSTRACT Objective: To protect the urethra from instrumentation related urethra injures and stricture, we developed a new surgical technique which can be defined as transvesical resection of prostate without using urethra. Materials and Methods: Our study included 12 consecutive bladder outlet obstruction patients treated with transvesical prostate resection in our clinic between March 2016 and May 2016. Detailed anamnesis, results of physical examination, digital rectal examination, routine lab tests, international prostate symptoms score, transrectal ultrasound, measurement of prostate-specific antigen levels and uroflowmetry was performed in all patients prior to surgery. Results: Hospitalization period following surgery was 1 day. Foley catheter and suprapubic cystostomy catheters were removed in a median period of 3.6 days and 1 day. Median mass of resected adenomas was measured as 21.8 gr. Median maximum flow rate was measured as 6mL/s. Median postvoid residual urine volume was 70.6 cc and median international prostate symptoms score and quality of life scores were 9 and 1.4, respectively. Conclusion: In this study, we would like to show the possible practicality of transvesical resection of prostate technique in this patient group. However, we think that this technique is very useful in special patient groups such as patients with bladder stones, priapism and penile prosthesis.

ABSTRACT Objective To The standard technique for obtaining a histologic diagnosis of prostatic carcinomas is transrectal ultrasound guided prostate biopsy. Acute prostatitis which might develop after prostate biopsy can cause periprostatic inflammation and fibrosis. In this study, we performed a retrospective review of our database to determine whether ABP history might affect the outcome of RP. Materials and Methods 441 RP patients who were operated in our clinic from 2002 to 2014 were included in our study group. All patients’ demographic values, PSA levels, biopsy and radical prostatectomy specimen pathology results and their perioperative/ postoperative complications were evaluated. Results There were 41 patients in patients with acute prostatitis following biopsy and 397 patients that did not develop acute prostatitis. Mean blood loss, transfusion rate and operation period were found to be significantly higher in ABP patients. Hospitalization period and reoperation rates were similar in both groups. However, post-op complications were significantly higher in ABP group. Conclusion Even though it does not affect oncological outcomes, we would like to warn the surgeons for potential complaints during surgery in ABP patients.

Abstract: The aim of this study was to investigate the effect of a placebo, intracanal diode laser application, and low-level laser therapy (LLLT) on the change of the total amount of calcitonin gene-related peptide (CGRP) in the gingival crevicular fluid (GCF) (split-mouth study design). GCF sampling was performed on a contralateral tooth and experimental tooth (root canal-treated tooth) of thirty-nine patients. The patients were divided into three groups (n = 13), as follows: placebo (mock laser application), intracanal laser application, and LLLT. GCF sampling was repeated at the same sites (experimental and control teeth) one week after root canal treatment. The data were analyzed using the Pearson's correlation analysis and the independent-samples t-tests (p=0.05). In the placebo group, the total CGRP level changes in the GCF before and after treatment was significantly higher for experimental teeth than for control teeth (p<0.05). However, there was no significant difference between experimental and control teeth in the intracanal laser application and LLLT groups (p > 0.05). Intracanal laser application and low-level laser therapy have immunomodulation effects linked to the modulation of the total amount of CGRP in the GCF.

ABSTRACT Objective: To evaluate various kinematic movements on postoperative pain using a Reciproc system. Material and Methods: Fifty-six molar teeth were divided into four groups according to kinematics as follows: continuous rotation, 360° CCW – 30° CW, 270° CCW – 30° CW, and 150° CCW – 30° CW. Preoperative and postoperative pain levels using visual analogue scale (VAS), percussion pain, and analgesic intake were recorded for each subject. Postoperative pain levels at 1, 3, 5, and 7 d were evaluated. Data were analyzed statistically using the Kruskal-Walis, Mann-Whitney-U, one-way analysis of variance, and chi-square tests (p=0.05). Results: Continuous rotation resulted in more pain at Day 1 when compared with the reciprocating groups (360° CCW – 30° CW and 270° CCW – 30° C) (p<0.05). Conclusions: Continuous rotation resulted in more postoperative pain at Day 1 than in reciprocating groups, and thereafter no significant pain was found among the groups.
