Resumo: Introdução: Os cuidados paliativos (CP) possibilitam a interação da equipe multidisciplinar e proporcionam qualidade de vida e alívio da dor, ao considerarem o biopsicossocial e a espiritualidade do indivíduo, de modo a não adiar ou acelerar a morte. Objetivo: Este estudo teve como objetivo identificar o nível de compreensão dos acadêmicos de Medicina e médicos sobre os CP. Método: A primeira amostra foi constituída de 73 acadêmicos distribuídos entre os 12 períodos do curso de Medicina. A segunda população amostral englobou três médicos sem a prática dos CP. A coleta de dados fundamentou-se na aplicação da versão portuguesa do questionário Bonn Palliative Care Knowledge Test (BPW) do Porto. O arquivo é composto por duas seções: a primeira contém 23 questões, e a segunda, 15, incluindo quatro opções de resposta (correta, razoavelmente correta, pouco correta e incorreta). Como suporte para a aplicação virtual, utilizou-se a plataforma do Google Forms. Os participantes foram informados, por meio de mensagem de texto e também de forma presencial, no caso dos profissionais, sobre do motivo da pesquisa e as questões éticas. Resultado: A compreensão dos CP foi variável, principalmente entre os estudantes, já que alguns tópicos apresentaram maior divergência entre as respostas do que outros. Os acadêmicos (84,9%) e médicos (66,6%) demonstraram ter conhecimento sobre a possibilidade de combinar os CP com tratamentos curativos. Somente 9,1% dos acadêmicos marcaram como inadequada a gestão da dor com opioide transdérmico. Quanto aos médicos, não houve nenhuma menção a isso. Ademais, 95,9% dos estudantes e 100% dos profissionais reconheceram que é possível aprender habilidades interpessoais, como a comunicação. Com relação à identificação e discussão dos problemas reais no ambiente social da pessoa em CP, 100% dos médicos e apenas 50,7% dos acadêmicos afirmaram que são capazes de tal ação. Conclusão: É necessária a capacitação teórica e prática desses indivíduos por meio da inclusão da disciplina de CP de modo individualizado, desde os primeiros períodos do curso de Medicina e também em outros cursos da saúde, visto que a equipe multidisciplinar é essencial para a concretização desse cuidado holístico.

Abstract: Introduction: Palliative Care (PC) enables interaction of the multidisciplinary team and provides quality of life and pain relief, considering the biopsychosocial and spirituality of the individual, so as not to postpone or accelerate death. Objective: Identify the level of knowledge of medical students and physicians about PC. Methods: The first sample consisted of 73 academics distributed among the twelve periods of the medical course. The second sample population includes 3 doctors, none of whom practice PC. Data collection was based on the application of the Portuguese version of the BPW (Bonn Palliative Care Knowledge Test) questionnaire from Porto. The file is composed of two sections, the first has 23 questions and the second 15, including 4 answer options (correct, reasonably correct, somewhat correct and incorrect). As support for the virtual application, the Google Forms platform was used. Participants were informed, via text message, and in person in the case of professionals, of the reason for the research and about ethical issues. Results: Compression regarding PC is variable, especially among students, with some topics showing greater divergence between responses than others. Academics (84.9%) and physicians (66.6%) demonstrated knowledge about the possibility of combining PC with curative treatments. Only 9.1% of academics and 0% of physicians rated pain management with transdermal opioids as inadequate. Furthermore, 95.9% of students and 100% of professionals recognize that interpersonal skills, such as communication, are something that can be learned. Regarding the identification and discussion of real problems in the social environment of the person in PC, 100% of physicians and only 50.7% of academics said they were capable of such action. Conclusion: Theoretical and practical training of these individuals is necessary, through the inclusion of the PC discipline in an individualized way, from the first periods of the medical course and in other health courses, since the multidisciplinary team is essential for the execution of this holistic care.

Resumo Introdução: As quedas são consideradas um problema de saúde pública, portanto torna-se relevante identificar se as alterações que a postura sofre no decorrer dos anos contribui para o risco de quedas em idosos. Objetivo: Analisar se as alterações posturais favorecem as quedas e/ou desequilíbrio postural, em idosos saudáveis da comunidade. Métodos: A busca ocorreu no mês de abril de 2016, de forma independente, por dois revisores, nas bases de dados PubMed, Web of Science, SPORTDiscus, e CINAHL, com delimitação de publicação dos últimos 10 anos, em língua inglesa, com as palavras chaves “posture” ou (“kyphosis”,“lumbar lordose”,“flexed posture”,“spinal curvature”,“spinal sagital contour”) em combinação com “elderly” e “fall”. A qualidade dos estudos foi avaliada pelas diretrizes do STROBE (Strengthening the Reporting of Observational Studies in Epidemiology) para estudos observacionais. Resultados: Foram encontrados 1734 artigos, sendo considerados elegíveis, os estudos observacionais com avaliação postural, equilíbrio e/ou quedas em idosos, em um total de 17 artigos analisados. A confiabilidade e reprodutibilidade dos instrumentos não foram informados em cinco estudos, e em dois apresentaram descriçao questionável. Quatorze artigos analisaram alterações posturais a nível de tronco e 03 artigos avaliaram tornozelos e pés. A maioria dos estudos encontraram associaçao entre as alterações posturais com a perda do equilibrio e aumento das quedas em idosos. Conclusão: Hipercifose torácica, retificação da lordose lombar, diminuição do arco plantar de idosos que vivem na comunidade, parecem contribuir para maior instabilidade postural, e consequentemente aumentar o risco de quedas na população idosa que vive na comunidade.

Abstract Introduction: Since falls are considered to be a public health problem, it is important to identify whether postural changes over time contribute to the risk of falls in older adults. Objective: To investigate whether postural changes increase fall risk and/or postural imbalance in healthy, community-dwelling older adults. Methods: In April 2016, two reviewers independently searched the PubMed, Web of Science, SPORTDiscus, and CINAHL databases for studies in English published in the previous 10 years, using the following combined keywords: “posture” or (“kyphosis”,“lumbar lordosis”,“flexed posture”,“spinal curvature”,“spinal sagittal contour”) AND “elderly” AND “fall”. Study quality was assessed according to the STROBE (Strengthening the Reporting of Observational Studies in Epidemiology) guidelines for observational studies. Results: The search retrieved 1,734 articles. Only observational studies that assessed posture, balance, and/or falls in older adults were considered eligible for review. The final sample included 17 articles: reliability and reproducibility of the instruments were not reported in five studies, while two studies offered a questionable description of the instruments used. Fourteen articles analyzed postural changes at the trunk level and three articles assessed them at the ankles and feet. Most studies found a positive association between postural changes and an increased risk for loss of balance and falls. Conclusion: Thoracic hyperkyphosis, loss of lumbar lordosis, and decreased plantar arch seem to contribute to greater postural instability, and thus to a higher risk of falls in community-living older adults.

Objetivo: Analisar a percepção dos pais a respeito do tabagismo passivo na saúde de seus filhos. Métodos: Estudo qualiquantitativo de caráter etnográfico. Buscou-se o ponto de vista e o conhecimento dos pais fumantes ativos quanto à poluição tabagística ambiental e ao tabagismo passivo. Foram incluídos mães e pais fumantes ativos que conviviam diariamente com seus filhos em sete escolas públicas da cidade de Anápolis (GO) no primeiro semestre de 2014. Os pais foram entrevistados em uma sala reservada nas escolas. Procedeu-se à análise descritiva e qualitativa por meio da etnografia. Resultados: A amostra foi de 58 pais, o tempo médio de tabagismo de 15,3 anos e a quantidade média de cigarros fumados por dia de 20,1. Grande parte (59%) dos pais não sabia o que era poluição tabagística ambiental e 60% disseram saber o que era um fumante passivo. Contudo, quando perguntados a respeito de considerarem seus filhos fumantes passivos, 52% não os consideravam. Observou-se que alguns pais têm conhecimento sobre a influência do tabagismo passivo na saúde de seus filhos. Contudo, a maioria (52%) deles acredita que seus filhos podem não sofrer prejuízo respiratório ou não sabem quais prejuízo são esses. Conclusões: As crianças analisadas ficavam expostas à poluição tabagística ambiental no domicilio, o que ficou evidente por meio dos dados, do tempo de tabagismo e da média de cigarros fumados por dia. Entretanto, percebeu-se carência no conhecimento dos pais a respeito da poluição tabagística ambiental, do tabagismo passivo e dos males que o cigarro pode causar na saúde dos filhos.

Objective: To analyze the perception of parents about secondhand smoking in their children's health. Methods: Ethnographic qualitative and quantitative study. We sought the point of view and understanding of the parents who were active smokers in relation to environmental tobacco smoke (ETS) and secondhand smoking. Mothers and fathers who are active smokers and that live with their children from seven different public schools in the city of Anápolis, Midwest Brazil, were interviewed in the first semester of in a reserved room in the schools. A descriptive and qualitative analysis was carried out through the ethnography. Results: 58 parents with an average time of smoking of 15.3 years and an average quantity of cigarettes smoked per day of 2 were interviewed. Among them, 59% did not know what ETS was, and 60% stated knowing what a secondhand smoker was. However, when questioned about their children as secondhand smokers, 52% did not consider them to be. Some parents knew some of the effects of secondhand smoking in the health of their children. However, the majority (52%) of them did not believe that their children would suffer any respiratory impairment or did not know about these impairments. Conclusions: Children were exposed to environmental tobacco pollution in their residence if one considers parental duration of smoking and average of cigarettes smoked per day. There was a lack of knowledge of the parents about ETS, secondhand smoking and the evils that cigarettes could cause in the health of their children.
