RESUMO A 11a Classificação Estatística Internacional de Doenças e Problemas Relacionados à Saúde representa um avanço no enfoque do conhecimento e em novas abordagens das doenças. A Classificação Estatística Internacional de Doenças e Problemas Relacionados à Saúde é utilizada para diferentes finalidades práticas, possibilitando avaliação do avanço da agenda de saúde global, alocação de recursos, segurança do paciente, qualificação da assistência à saúde e reembolso de seguros de saúde. É inteiramente digital, com recursos tecnológicos que permitem sua atualização periódica. No início de 2022, a 11a Classificação Estatística Internacional de Doenças e Problemas Relacionados à Saúde entrou em vigência oficial, tendo sido disponibilizada em vários de seus idiomas oficiais, como o árabe, chinês, espanhol, francês e inglês. Apresenta-se aqui o processo de tradução para a língua portuguesa em uso no Brasil, coordenado pela Universidade Federal de Minas Gerais, com apoio do Ministério da Saúde do Brasil e da Organização Pan-Americana da Saúde/Organização Mundial da Saúde. O trabalho foi realizado em três etapas entre agosto de 2021 e dezembro de 2022 por tradutores com diferentes formações: médicos especialistas (49), fisioterapeuta (1), farmacologista (1) e odontologista (1). Com este artigo metodológico, almeja-se ampliar a discussão de perspectivas para implementação da 11a Classificação Estatística Internacional de Doenças e Problemas Relacionados à Saúde no Brasil e construir uma oportunidade para sua adaptação e uso por outros países de língua oficial portuguesa.

ABSTRACT The 11th International Statistical Classification of Diseases and Related Health Problems (ICD-11) represents an advance in the focus on knowledge and new disease approaches. The ICD is used for different practical purposes, enabling assessment of progress in the global health agenda, resource allocation, patient safety, health care qualification, and health insurance reimbursement. It is entirely digital, with technological resources that allow periodic updating. In early 2022, ICD-11 entered into official force, having been made available in several official ICD languages such as Arabic, Chinese, Spanish, French, and English. The translation process into Brazilian Portuguese, coordinated by the Federal University of Minas Gerais (UFMG), with support from the Brazilian Ministry of Health (MS) and PAHO/WHO, is presented here. The work was carried out in three stages between August 2021 and December 2022 by translators with different backgrounds: medical specialists (49), physiotherapists (1), pharmacologists (1), and dentists (1). This methodological article aims to broaden the discussion of perspectives on implementing the ICD-11 in Brazil and build an opportunity for its adaptation and use by other Portuguese-speaking countries.

Resumo Os aplicativos de saúde para dispositivos móveis se revelam como uma ferramenta importante para a educação em saúde para apoiar o tratamento de diferentes doenças, como o diabetes mellitus (DM). Nesse sentido, esta pesquisa analisa as funcionalidades dos aplicativos existentes na língua portuguesa destinados a auxiliar pessoas com DM. Foi realizada uma busca sistemática para identificação dos aplicativos por meio dos indexadores “Controle de diabetes”, “Diabetes”, “Glicose” e “Insulina” nos sistemas operacionais Android e iOS. Foram encontrados 576 aplicativos, porém, apenas 63 (10,9%) atenderam aos critérios de inclusão. De acordo com sua funcionalidade, os aplicativos apresentaram as seguintes possibilidades de uso: 13 educativos; 12 monitoram atividade física; 18 contêm diário alimentar; 28 contam carboidratos; 34 monitoram a insulina; 49 monitoram a glicemia; 33 apresentam gráficos; 9 alertam sobre a presença de hipo ou hiperglicemia; 17 lembram coleta de glicemia; e 5 apresentaram receitas culinárias. As funcionalidades que auxiliam na mudança de comportamento e individualizam estratégias de tratamento ainda são incipientes. Sugere-se o desenvolvimento de ferramentas auxiliares para interação dos aplicativos com seus usuários e a comprovação da sua eficácia.

Abstract Health applications for mobile devices are an important tool for a health education to support the treatment of different diseases, such as diabetes mellitus (DM). In this sense, this research analyzes the functionalities of existing applications in Portuguese aimed at helping people with DM. A systematic search was performed to identify the applications by the descriptors "Diabetes control,” “Diabetes,” “Glucose,” and “Insulin” in Android and iOS operating systems. A total of 576 applications were found, but only 63 (10.9%) met the inclusion criteria. According to their functionality, the apps presented the following possibilities of use: 13 are educational; 12 monitor physical activity; 18 contain a food diary; 28 count carbohydrates; 34 monitor insulin; 49 monitor blood glucose; 33 present graphs; 9 warn about the presence of hypo or hyperglycemia; 17 recall blood glucose collection; and 5 present culinary recipes. Functionalities that assist in behavior change and that individualize treatment strategies are still incipient. We suggest the development of auxiliary tools for interaction of applications with their users and the proof of their effectiveness.

Abstract Scientific evidence about genetic and molecular changes in oral squamous cell carcinoma (OSCC) among smokers and non-smokers is inconclusive. This systematic review and meta-analysis assessed the effects of tobacco on the DNA of individuals with OSCC based on protein mutations. Electronic searches were conducted on PubMed, Ovid, Web of Science, and Scopus to identify observational studies published up to January/2022. The Joanna Briggs Institute tool was used for the critical appraisal of studies. The certainty of the evidence was evaluated. Twenty-three studies assessing 4,060 individuals (2,967 smokers vs. 1,093 non-smokers) were included in this review. Fifteen groups of proteins/genes were investigated. Analysis of the quality of articles revealed low risk of bias in most studies. The certainty of the evidence was very low. The meta-analysis confirmed no significant difference between smokers and non-smokers with respect to damage to GSTM1 (OR: 0.60; 95%CI: 0.30–1.18), GSTT1 (OR: 1.18; 95%CI:0.49–2.83), hydrolase proteins (Ku70 and Ku80) (OR: 0.74; 95%CI: 0.18–3.05), and transferase proteins (GSTM1, GSTT1, GSTM3) (OR: 0.74; 95%CI: 0.47–1.18). Most of the studies included showed that smokers are more likely to exhibit genetic instability. However, the meta-analysis revealed that smokers do not necessarily have more genetic alterations in the DNA than non-smokers.

Abstract The objective of this work was to verify if there is variability in the dry matter accumulation rate of grains of the carioca-type common bean, as well as to identify the success of selection for this trait and the best time for harvest. Sixteen lines with carioca-type grains were evaluated in three sowing seasons through samples of five plants obtained at regular intervals. A quadratic equation was used to estimate the number of days to obtain the maximum dry matter in the grains. A low variability was observed among the lines, discouraging selection for this trait. The ideal moment for harvesting is when 100% of the grains present the typical stripes of carioca common bean, which may vary among and within pods of the same plant. If the harvest is carried out before complete physiological maturation is reached, the dry matter in the grains decreases, on average, 2.75% per day.

Resumo O objetivo deste trabalho foi verificar se há variabilidade na taxa de acúmulo de matéria seca nos grãos de feijão comum do tipo carioca, bem como identificar o sucesso na seleção para este caráter e a melhor época para colheita. Dezesseis linhagens com grãos do tipo carioca foram avaliadas em três épocas de semeadura, por meio de amostras de cinco plantas, obtidas em intervalos regulares. Utilizou-se equação quadrática para estimar o número de dias para obtenção do máximo de matéria seca nos grãos. Observou-se baixa variabilidade entre as linhagens, o que desestimula a seleção para esse caráter. O momento ideal para a colheita é quando 100% dos grãos apresentam as listras típicas do feijão carioca, o que pode variar entre e dentro das vagens da mesma planta. Se a colheita for realizada antes da maturação fisiológica completa ser atingida, a matéria seca nos grãos é reduzida, em média, 2,75% por dia.

RESUMEN Objetivos. Determinar los cambios en las características clínicas y desenlaces intrahospitalarios de los pacientes hospitalizados por COVID-19 en un hospital privado de Caracas durante dos años de pandemia. Materiales y métodos. Estudio retrospectivo, observacional, de pacientes hospitalizados por COVID-19. Se investigó la correspondencia entre las olas de ingresos hospitalarios con las variantes circulantes del SARS-CoV-2 en la población general del Distrito Capital y estado Miranda. Resultados. Se incluyeron 1025 pacientes (569 hombres y 456 mujeres), con edad promedio de 62,9 DE: 16,2 años. Cuatro olas de ingresos hospitalarios fueron identificadas: primera (marzo-noviembre 2020) 150/1025 (14,6%) casos; segunda (diciembre-2020 a mayo-2021) 415/1025 (40,5%) casos; tercera (junio-diciembre 2021) 344/1025 (33,6%) casos; cuarta (enero-febrero 2022) 116/1025 (11,3%) casos. La edad promedio fue mayor en la cuarta ola (primera 64,0±15,7, segunda 61,4±15,8, tercera 62,1±16,5, y cuarta ola 68,5±16,4), mientras que la proporción de pacientes masculinos (primera 66,7%, segunda 58,8%, tercera 50,3%, y cuarta 44,8%), los pacientes con enfermedad grave-crítica (primera 65,3%, segunda 57%, tercera 51,7% y cuarta 44,8%), la estadía intrahospitalaria (primera 9,1±6,0, segunda 9,0±7,3, tercera 8,8±7,7, y cuarta 6,9±5,0 días), los ingresos a la UCI (primera 23,3%, segunda 15,7%, tercera 14,0%, y cuarta 11,2%; p=0,027) y la mortalidad (primera 21.8%, segunda 10,7%, tercera 9,1%, y cuarta 7,1%; p<0,001) disminuyeron progresivamente con el tiempo. Conclusiones. Los resultados muestran menor frecuencia de casos severos y mejoría de los desenlaces intrahospitalarios en dos años de pandemia. Es probable que los cambios en las variantes circulantes, las mejoras del manejo de la enfermedad y la vacunación hayan influido sobre estos resultados.

The mean age was higher in the fourth wave (first: 64.0±15.7, second: 61.4±15.8, third: 62.1±16.5, and fourth wave: 68.5±16.4), while the proportion of male patients (first: 66.7%, second: 58.8%, third: 50.3%, and fourth wave: 44.8%), patients with severe-critical illness (first: 65.3%, second: 57%, third: 51.7%, and fourth wave: 44.8%), in-hospital stay (first: 9.1±6.0, second: 9.0±7.3, third: 8.8±7.7, and fourth wave: 6.9±5.0 days), ICU admissions (first: 23.3%, second: 15.7%, third: 14.0%, and fourth wave: 11.2%; p=0.027) and mortality (first: 21. 8%, second: 10.7%, third: 9.1%, and fourth wave: 7.1%; p<0.001) progressively decreased over time. Conclusions. The results show lower frequency of severe cases and improvement of in-hospital outcomes in two years of the pandemic. Changes in circulating variants, improvements in disease management and vaccination are likely to have influenced these results.

Abstract Although subirrigation irrigation is a well indicated technique for the production of ornamental flowers, due to its high efficiency in the use of water and fertilizers, and to the absence of surface runoff and leaching, there are few studies on its management. Within this context, the objective of this study was to evaluate the capillarity (subirrigation irrigation) technique on root growth and yield of capuchin flowers. The experiment was two-level factorial (4x4) in the completely randomized design. The first factor was water blade height (13, 12, 10, and 8 cm) and the second factor was time of subirrigation (T1: increase of 2 minutes of vessel capacity time - CV; T2: time to reach CV; T3: reduction of 2 minutes of CV and T4 time: reduction of 4 minutes of CV time). Flower yield was calculated weekly from 56 days after transplant (DAT). The experiment was closed at 180 DAT, for the destruction of plants and thus analysis of root growth. The average daily water consumption was 0.77 mm.day-1. The highest flower yield and root growth occurred at leaf height of 13 cm and subirrigation time of 3 minutes. Irrigation is a viable technique in capuchin cultivation, where flower yield and root growth can be regulated through water levels and irrigation times.

Resumo Embora a subirrigação seja uma técnica bastante indicada na produção de flores ornamentais, por apresentar elevada eficiência no uso de água e fertilizantes, devido à ausência de escoamento superficial e lixiviação, poucos são os estudos sobre seu manejo. Dentro deste contexto objetivou-se avaliar a técnica de irrigação por capilaridade (subirrigação) no crescimento radicular e na produção de flores da capuchinha. O experimento foi bifatorial (4x4) no delineamento inteiramente casualizado. Sendo o primeiro fator altura de lâmina de água (13, 12, 10 e 8 cm) e o segundo fator tempo de subirrigação (T1: acréscimo de 2 minutos do tempo de capacidade de vaso - CV; T2: tempo para atingir a CV; T3: redução de 2 minutos do tempo de CV e T4: redução de 4 minutos do tempo de CV). A produção de flores foi computada semanalmente a partir dos 56 dias após o transplante (DAT). O experimento foi encerrado aos 180 DAT, para a destruição das plantas e assim análise do crescimento radicular. O consumo médio diário de água foi de 0,77 mm.dia-1 . A produção de flores mais elevada e o maior crescimento de raízes ocorreram na altura de lâmina de 13 cm e tempo de subirrigação de 3 minutos. A subirrigação é uma técnica viável no cultivo da capuchinha, onde a produção de flores e crescimento radicular, podem ser regulados através dos níveis de lâmina de água e tempos de subirrigação.

Abstract INTRODUCTION: Excess Mortality by all causes considers deaths directly related to COVID-19 and those attributed to conditions caused by the pandemic. When stratified by social dimensions, such as race/color, it allows for the evaluation of more vulnerable populations. The study estimated the excess mortality by natural causes, separating the white and black populations in 2020. METHODS Public civil registration data on deaths observed in 2020, corrected for under registration, were used. The expected number of deaths was estimated based on the mortality rates observed in 2019, applied to the estimated population in 2020. The difference between the values expected and observed and the proportion of excess was considered the excess mortality. RESULTS: The present study found an excess of 270,321 deaths (22.2% above the expected) in 2020. Every state of Brazil reported deaths above the corresponding expected figure. The excess was higher for men (25.2%) than for women (19.0%). Blacks showed an excess of 27.8%, as compared to whites at 17.6%. In both sexes and all age groups, excess was higher in the black population, especially in the South, Southeast, and Midwest regions. São Paulo, the largest in population number, had twice as much excess death in the black population (25.1%) than in the white population (11.5%). CONCLUSIONS: The present study showed racial disparities in excess mortality during the COVID-19 pandemic in Brazil. The higher excess found for the black suggests an intrinsic relationship with the socioeconomic situation, further exposing the Brazilian reality, in which social and structural inequality is evident.

Resumo: O objetivo deste estudo foi comparar os resultados obtidos para a coordenação do cuidado a partir do Programa Nacional de Melhoria do Acesso e da Qualidade da Atenção Básica (PMAQ-AB), com os parâmetros adotados pelo Atlas de Medidas de Coordenação do Cuidado e pelo Observatório Europeu de Políticas e Sistemas de Saúde. Foi realizado estudo transversal, com base no banco de dados do 3º ciclo do PMAQ-AB. Foram criadas três tipologias de coordenação do cuidado: PMAQ-AB, Atlas e Observatório. O teste qui-quadrado foi aplicado para comparar as proporções; os testes de Kruskal-Wallis e de Nemenyi para verificar e identificar eventuais diferenças entre as tipologias. O nível de significância foi de 5%. Foram avaliadas 35.350 equipes que realizaram alguma atividade de coordenação do cuidado. Observou-se diferença significativa (p < 0,001), entre os níveis de coordenação, com maior percentual entre o nível alto e médio nos três instrumentos, PMAQ-AB (56,07% e 38,35%), Atlas (52,63% e 40,66%) e o Observatório (44,82% e 43,98%). Na comparação dos indicadores, houve diferença significativa (p < 0,001) entre as tipologias. Para o Brasil, na tipologia PMAQ-AB, todos os estratos exibiram maior percentual entre o nível alto e médio; no Atlas, o estrato 1 destacou-se no nível médio (43,81%) e, no Observatório, predominou o nível alto. Na comparação dos indicadores por estratos, pelo menos um estrato diferiu dos demais (p < 0,001). O 6 se distinguiu dos demais (p < 0,001), e o 1 diferiu de todos (p < 0,001), exceto do 2 (p > 0,05). Os níveis de coordenação do cuidado diferenciaram-se entre os instrumentos utilizados. Altos e médios níveis foram identificados, demonstrando a necessidade de estudos adicionais.

Abstract: This study aimed to compare the results obtained with the coordination of care through the Brazilian National Program for Improvement of Access and Quality of Basic Care (PMAQ-AB), with the parameters adopted by the Care Coordination Measures Atlas and the European Observatory on Health Systems and Policies. A cross-sectional study was performed using the dataset from the third cycle of the PMAQ-AB. Three typologies of coordination of care were created: PMAQ-AB, Atlas, and Observatory. Chi-square test was applied to compare proportions and Kruskal-Wallis and Nemenyi tests to verify and identify potential differences between the typologies. Significance was set at 5%. In all, 35,350 teams were assessed that performed some activity in care coordination. A significant difference was observed (p < 0.001) between levels of coordination, with a higher percentage between the high and medium levels in the three instruments, PMAQ-AB (56.07% and 38.35%), Atlas (52.63% and 40.66%), and Observatory (44.82% and 43.98%). In the comparison of the indicators, there was a significant difference (p < 0.001) between the typologies. For Brazil, in the PMAQ-AB typology, all the strata displayed a higher percentage between the high and medium levels; in the Atlas, stratum 1 stood out in the medium level (43.81%); the high level predominated in the Observatory. In the comparison of the indicators by strata, at least one stratum differed from the others (p < 0.001). Number 6 differed from the others (p < 0.001), and number 1 differed from all of them (p < 0.001) except number 2 (p > 0.05). The levels of coordination of care differed according to the instruments used. High and medium levels were identified, showing the need for additional studies.

Abstract: Tobacco smoking involves a high risk of human malignancies, including oral cancer, because it contains multiple carcinogens that cause genetic instability. Thus, a worse prognosis would be expected for cancer patients who are smokers. The aim of this study was to assess the DNA damage response through the expression of checkpoint kinase 2 (CHK2), H2A histone family member X (H2AX), and P53 among smokers and non-smokers with oral squamous cell carcinoma (OSCC). Associations between immunoexpression of proteins and clinicopathological data and histopathological grading were also analyzed. A total of 35 individuals (18 non-smokers and 17 smokers) with OSCC of the tongue and/or floor of the mouth were included. Immunohistochemistry for H2AX was conducted for the identification of double-strand breaks, CHK2, and P53 to evaluate the expression of this protein in cell cycle regulation. The sample consisted of 22 males and 13 females, with a mean age of 63.9±11.8 years. OSCC of non-smokers were well-differentiated tumors in 50% of the cases, and those of smokers were equally distributed into moderately differentiated and poorly differentiated tumors (35.3% each). Overall, 31 (88.6%) cases were CHK2-positive, 27 (77.1%) were H2AX-positive, and 23 (65.7%) were P53-positive, with no difference between smokers and non-smokers (p > 0.05). No association was found between proteins and clinicopathologic data (p > 0.05). Similarities in CHK2, H2AX, and P53 immunohistochemical staining patterns were observed between smokers and non-smokers, and immunoexpression was not associated with clinicopathological parameters. However, the findings indicated consistent expression of these proteins in OSCC.

O estudo buscou compreender a coordenação do cuidado pela Atenção Primária à Saúde para casos de gestação, câncer de mama e de colo uterino. O atributo foi entendido como a responsabilidade que os profissionais das equipes de Saúde da Família (eSF) têm em coordenar a atenção prestada aos usuários ao longo da rede de atenção, assumindo o acompanhamento continuado e a identificação de necessidades. De modo geral, foram identificadas ações de coordenação do cuidado mais bem estruturadas nas ações internas em nível primário, sobretudo aquelas de atenção às gestantes. Quando o trabalho de coordenação envolvia a rede, o destaque foi para a ausência da contrarreferência e para a informalidade nos processos de continuidade da atenção.

The aim of this study was to understand the coordination of primary health care for pregnant women and breast and cervical cancer patients. The attribute was understood as the responsibility family health care workers have for coordinating care delivery, including continuous follow-up and the identification of patient needs. Overall, the findings show that care coordination was better structured at internal primary care level, particularly pregnancy care. When coordination involved the care network, the findings highlight lack of counter-referral and informal processes for the continuity of care.

El objetivo del estudio fue entender la coordinación del cuidado por parte de la Atención Primaria de la Salud para casos de gestación, cáncer de mama y de cuello uterino. El atributo fue entendido como la responsabilidad que los profesionales de los equipos de salud de la familia tienen para coordinar la atención prestada a los usuarios en la red de atención, asumiendo el acompañamiento continuado y la identificación de necesidades. Por lo general, se identificaron acciones de coordinación del cuidado mejor estructuradas en las acciones internas en el nivel primario, principalmente para aquellas de atención a las gestantes. Cuando el trabajo de coordinación envolvía la red, el destaque fue para la ausencia de la contrarreferencia y para la informalidad en los procesos para la continuidad de la atención.
