Abstract Introduction: non-alcoholic fatty liver disease (NAFLD) is a disease in which there is excessive fat deposition in hepatocytes due to hepatoprotective factors. Objectives: to assess the association of the triglyceride-glucose index with the occurrence of non-alcoholic fatty liver disease and mortality in elderly inpatients. To identify the TyG index as a predictive factor of NAFLD. Methods: this prospective observational study included elderly inpatients admitted to the Department of Endocrinology at the Linyi Geriatrics Hospital, Affiliated to Shandong Medical College, between August 2020 and April 2021. The TyG index was calculated according to an established formula: TyG = Ln [triglycerides (TG) (mg/dl) × Fasting plasma glucose (FPG) (mg/dl) / 2]. Result: a total of 264 patients were enrolled, with 52 (19.7 %) cases occurred NAFLD. Multivariate logistic regression analysis showed that TyG (OR = 3.889; 95 % CI: 1.134-11.420; p = 0.014) and ALT (OR = 1.064; 95 % CI: 1.012-1.118; p = 0.015) were independently associated with the occurrence of NAFLD. Furthermore, receiver operating characteristic (ROC) curve analysis showed that the area under the curve (AUC) of TyG was 0.727, with sensitivity = 80.4 % and specificity = 57.8 % at cut off = 8.71. A Cox proportional hazards regression model showed that, after adjusting for age, sex, smoking, drinking, hypertension, and type 2 diabetes TyG > 8.71 (HR = 3.191; 95 % CI: 1.347 to 7.560; p < 0.001) was an independent risk factor for mortality in the elderly. Conclusions: the TyG index can predict non-alcoholic fatty liver disease and mortality in elderly Chinese inpatients.

Resumen Introducción: la enfermedad del hígado graso no alcohólico (EHGNA) es una enfermedad en la que una cantidad excesiva de grasa se acumula en los hepatocitos debido a factores hepatoprotectores. Objetivos: evaluar la asociación entre el índice glucemia-triglicéridos con la presencia de hígado graso no alcohólico (EHGNA) y mortalidad en pacientes hospitalizados de avanzada edad. Identificar el índice TyG como factor predictivo de la EHGNA. Métodos: este estudio observacional y prospectivo incluyó a pacientes de avanzada edad hospitalizados en el Departamento de Endocrinología del Hospital de Geriatría de Linyi, Afiliado al Colegio Médico de Shandong, entre agosto de 2020 y abril de 2021. El índice TyG se calculó según una fórmula establecida: TyG = Ln [triglicéridos (TG) (mg/dl) × Glucosa en sangre en ayunas (FPG) (mg/dl) / 2]. Resultado: se incluyeron 264 pacientes, con 52 (19,7 %) casos de EHGNA. El análisis de regresión logística multivariante mostró que el TyG (razón de momios (OR) = 3,889; intervalo de confianza del 95 %: 1,134-11,420; p = 0,014) y ALT (razón de momios (OR) = 1,064; intervalo de confianza del 95 %: 1,012-1,118; p = 0,015) se asociaban de forma independiente con la aparición de EHGNA. Además, el análisis de la curva de características operativas del receptor (ROC) mostró que el área bajo la curva (AUC) de TyG era de 0,727, con una sensibilidad del 80,4 % y una especificidad del 57,8 % en el punto de corte de 8,71. El modelo de regresión de riesgos proporcionales de Cox mostró que, tras ajustar por edad, sexo, tabaquismo, consumo de alcohol, hipertensión y diabetes tipo 2, un TyG > 8,71 (HR = 3,191; intervalo de confianza del 95 %: 1,347 a 7,560; p < 0,001) era un factor de riesgo independiente de mortalidad para los paciente de edad avanzada. Conclusiones: el índice TyG puede predecir la enfermedad del hígado graso no alcohólico y la mortalidad en pacientes chinos hospitalizados de avanzada edad.

This study evaluated the effects of perioperative nutrition management by a multidisciplinary team on nutrition and postoperative complications of patients with esophageal cancer. A total of 239 patients with esophageal cancer who underwent esophagectomy and gastric conduit reconstruction for esophageal or esophagogastric junction cancer between February 2019 and February 2020 were included in the study. They were divided into the experimental group (120 patients) and the control group (119 patients) using the random number table method. Control group patients received routine diet management and experimental group patients received perioperative nutrition management by a multidisciplinary team. The differences of nutriture and postoperative complications between the two groups were compared. At 3 and 7 days after surgery, the experimental group patients had higher total protein and albumin levels (P<0.05), shorter postoperative anal exhaust time (P<0.05), lower incidence of postoperative gastrointestinal adverse reactions, pneumonia, anastomotic fistula, hypoproteinemia (P<0.05), and lower hospitalization costs (P<0.05) than the control group. Nutrition management by a multidisciplinary team effectively improved the nutriture of patients, promoted the rapid recovery of postoperative gastrointestinal function, reduced postoperative complications, and reduced hospitalization costs.

ABSTRACT This paper discusses the design of a safflower picking robot control system and focuses on a navigation control subsystem based on multisensor fusion. A navigation subsystem, an identification and positioning subsystem, a picking subsystem, and a levelling subsystem are designed. The hardware and software of the navigation subsystem are designed in detail, and a multisensor fusion positioning method based on extended Kalman fusion technology is proposed. The accuracy and stability levels of different combined navigation methods are compared. To test the effectiveness and accuracy of the proposed method, an outdoor test is carried out. The test results show that the outdoor fusion positioning accuracy of the robot is less than 8 cm, and when the satellite signal is lost, the navigation control subsystem can still provide high positioning accuracy. The final positioning result obtained using the integrated positioning method of the wheel odometer + IMU + DGNSS is approximately 52% higher than that of the odometer, approximately 29% higher than that of the wheel odometer + IMU, and approximately 11% higher than that of the IMU + DGNSS.

Abstract Voriconazole increases tacrolimus blood concentration significantly when coadministrated. The recommendation of reducing tacrolimus to 1/3 in voriconazole package insert seems not to be satisfactory in clinical practice. In vitro studies demonstrated that the magnitude of inhibition depends on the concentration of voriconazole, while voriconazole exposure is determined by the genotype status of CYP2C19. CYP2C19 gene polymorphism challenges the management of drug-drug interactions(DDIs) between voriconazole and tacrolimus. This work aimed to predict the impact of CYP2C19 polymorphism on the DDIs by using physiologically based pharmacokinetics (PBPK) models. The precision of the developed voriconazole and tacrolimus models was reasonable by evaluating the pharmacokinetic parameters fold error, such as AUC0-24, Cmax and tmax. Voriconazole increased tacrolimus concentration immediately in all population. The simulated duration of DDIs disappearance after voriconazole withdrawal were 146h, 90h and 66h in poor metabolizers (PMs), intermediate metabolizers (IMs) and extensive metabolizers(EMs), respectively. The developed and optimized PBPK models in this study can be applied to assit the dose adjustment for tacrolimus with and without voriconazole.

Abstract Objectives: The incidence of cerebellar Glioblastoma Multiforme (cGBM) is rare. Database like TCGA have not distinguish cGBM from GBM, our knowledge on cGBM gene expression characteristics is limited. The expression status of Oligodendrocyte Lineage Transcription factor 2 (OLIG2) and its clinical significance in cGBM is still unclear. Methods: The clinical data and tissue specimens of 73 cGBM patients were retrospectively studied. The association between OLIG2 expression level and the demographic characteristics of cGBM patients was identified by the Chi-Square test. The survival curves were drawn by Kaplan-Meier analysis. The independent prognostic factors was calculated according to Cox regression analysis. Results: The OLIG2 high expression was observed in about 57.5% (42/73) of the cGBM patients. Patients with high OLIG2 expression levels had a higher alive ratio at the end of follow-up (alive ratio: 70.6% vs. 29.4%, p = 0.04). The median survival time was 21 months and 13 months for high and low expression of OLIG2 (p < 0 .05). Univariate analysis and Multivariate analysis indicated that EOR (HR = 3.89, 95% CI 1.23−12.26, p = 0.02), low OLIG2 expression (HR = 5.26, 95% CI 1.13−24.59, p = 0.04), and without adjuvant therapy (HR = 4.95, 95% CI 1.22−20.00, p = 0.03) were independent risk factors for the OS of cGBM patients. Conclusion: High expression level of OLIG2 could be used as an independent favorable prognosis indicator in cGBM patients and be recognized as a characteristic biomarker of cGBM.

ABSTRACT Introduction: The sports psychology of athletes and public service policies in various places have been adjusted under the influence of the epidemic of COVID-19. However, team sports also need adjustments, and the paucity of evidence prevents safe decision-making. Objective: Research the current model to outline optimizations to team sports public service during the epidemic of COVID-19. Methods: The mechanism of athletes’ satisfaction was investigated, mainly involving organizing activities in sports venues, including geographical distribution, technical guidance, health services, etc. Results: According to the survey, the first concern was “organization of activities,” with a score of 3.783; followed by “number of places,” with a score of 3.252; and “health service,” with a score of 3.142. In the athletes’ satisfaction score relative to supply and demand, the first concern highlighted was “distribution of seats”, with a score of 3.682; followed by “number of seats”, with a score of 3.484; and “organization of activities”, with a score of 3.112. Conclusion: In optimizing the public team sports service model, the actual needs of the facilities should be fully considered to achieve scientific optimization of supply and demand. Abstract Objectives The pathological mechanisms of patients with Renal Cell Carcinoma (RCC) remain defined. This study aimed to evaluate relationships between the landscape of gene mutations and their clinical significance in RCC patients. Methods Tissue and peripheral blood samples of 42 patients with RCC were collected and performed for the Next Generation Sequencing (NGS) with Geneseeq PrimeTM 425-gene panel probes. Their landscapes of gene mutation were analyzed. We also carried out an evaluation of Tumor-Node-Metastasis (TNM) staging, RENAL nephelometry score, surgery, and targeted drug treatment of patients. Then we compared the correlations of landscape in gene mutations and the prognosis. Results The most common gene alternations, including BAP1, PBRM1, SETD2, CSF1R, NPM1, EGFR, POLE, RB1, and VHL genes, were identified in tissue and blood samples of 75% of patients. EGFR, POLE, and RB1 gene mutations frequently occurred in relapsed and metastatic patients. BAP1, CCND2, KRAS, PTPN11, ERBB2/3, JAK2, and POLE were presented in the patients with > 9 RENAL nephelometry score. Univariable analysis indicated that SETD2, BAP1, and PBRM1 genes were key factors for Disease-Free Survival (DFS). Multivariable analysis confirmed that mutated SETD1, NPM1, and CSF1R were critical factors for the Progression Free Survival (PFS) of RCC patients with target therapy. Conclusions Wild-type PBRM1 and mutated BAP1 in patients with RCC were strongly associated with the outcomes of the patient. The PFS of the patients with SETD2, NPM1, and CSF1R mutations were significantly shorter than those patients without variants.

ABSTRACT Purpose: Chronic inflammation in the liver is a key trigger for liver injury and fibrosis in various liver diseases. Given the anti-inflammatory and antioxidant effects of Saffron, this study aimed to investigate the pharmacological effects of Saffron on hepatic inflammation and fibrosis. Methods: The mice model of hepatic fibrosis was constructed using CCl4, and Saffron was administered at low (10 mg/kg) and high (20 mg/kg) doses by gavage. Then, the changes in liver function, liver inflammation and fibrosis markers were evaluated. The effects and mechanisms of Saffron on hepatic stellate cells were further investigated in in-vitro experiments. Results: Saffron improved liver function, reduced liver inflammation and attenuated liver fibrosis in a dose-dependent manner in hepatic fibrosis mice. Furthermore, Western blotting showed that Saffron significantly inhibited JAK/STAT3 phosphorylation in fibrotic livers. Conclusions: Saffron can attenuate liver fibrosis by inhibiting the JAK/STAT3 pathway and the activation of hepatic stellate cell, providing a theoretical basis for the development of new anti-fibrotic drugs.

Abstract Background Dexmedetomidine (DEX), a specific α2-adrenergic receptor agonist, is protective against myocardial ischemia/reperfusion injury (MIRI). However, the association between DEX preconditioning-induced cardioprotection and mitophagy suppression remains unclear. Objective Hence, we aimed to investigate whether DEX preconditioning alleviates MIRI by suppressing mitophagy via α2-adrenergic receptor activation. Method Sixty isolated rat hearts were treated with or without DEX before inducing ischemia and reperfusion; an α2-adrenergic receptor antagonist, yohimbine (YOH), was also administered before ischemia, alone or with DEX. The heart rate (HR), left ventricular diastolic pressure (LVDP), left ventricular end-diastolic pressure (LVEDP), maximal and minimal rate of left ventricular pressure development (±dp/dtmax), and myocardial infarction size were measured. The mitochondrial ultrastructure and autophagosomes were assessed using transmission electron microscopy. Mitochondrial membrane potential and reactive oxygen species (ROS) levels were measured using JC-1 and dichloride hydrofluorescein diacetate assays, respectively. The expression levels of the mitophagy-associated proteins Beclin1, LC3II/I ratio, p62, PINK1, and Parkin were detected by western blotting. Results Compared with the control group, in the ischemia/reperfusion group, the HR, LVDP, and ±dp/dtmax were remarkably decreased (p< 0.05), whereas LVEDP and infarct sizes were significantly increased (p< 0.05). DEX preconditioning significantly improved cardiac dysfunction reduced myocardial infarction size, maintained mitochondrial structural integrity, increased mitochondrial membrane potential, inhibited autophagosomes formation, and decreased ROS production and Beclin1, LC3II/I ratio, PINK1, Parkin, and p62 expression(p< 0.05). When DEX and YOH were combined, YOH canceled the effect of DEX, whereas the use of YOH alone had no effect. Conclusion Therefore, DEX preconditioning was cardioprotective against MIRI in rats by suppressing mitophagy via α2-adrenergic receptor activation.

Resumo Fundamento A dexmedetomidina (DEX), um agonista específico do receptor α2-adrenérgico, é protetora contra lesão de isquemia/reperfusão miocárdica (I/R). No entanto, a associação entre a cardioproteção induzida pelo pré-condicionamento DEX e a supressão da mitofagia permanece pouco clara. Objetivo Portanto, nosso objetivo foi investigar se o pré-condicionamento com DEX alivia a I/R, suprimindo a mitofagia via ativação do receptor α2-adrenérgico. Método Sessenta corações de ratos isolados foram tratados com ou sem DEX antes de induzir isquemia e reperfusão; um antagonista do receptor α2-adrenérgico, a ioimbina (YOH), também foi administrado antes da isquemia, isoladamente ou com DEX. A frequência cardíaca (FC), pressão diastólica do ventrículo esquerdo (PDVE), pressão diastólica final do ventrículo esquerdo (PDFVE), taxa máxima e mínima de desenvolvimento da pressão ventricular esquerda (±dp/dtmax) e tamanho do infarto do miocárdio foram medidos. A ultraestrutura mitocondrial e as autofagossomas foram avaliadas por microscopia eletrônica de transmissão. O potencial de membrana mitocondrial e os níveis de espécies reativas de oxigênio (ROS) foram medidos usando os ensaios JC-1 e diacetato de diclorodi hidrofluoresceína, respectivamente. Abstract Objective This study aimed to investigate the mid-pregnancy blood glucose levels of women with singleton or twin pregnancies. Method The relationship between blood glucose levels and Gestational Diabetes Mellitus (GDM) was studied in women with different pre-pregnancy Body Mass Index (BMI), and the effect of GDM on twin pregnancy outcomes was analyzed. Women with twin (n= 1,985) and singleton (n= 1,985) pregnancies were categorized into underweight (BMI < 18.5 kg/m2, n= 597), normal weight (BMI: 18.5-23.9 kg/m2, n= 2,575), and overweight/obese (BMI ≥ 24 kg/m2, n= 798) groups. Results The incidence of GDM was 21.01% in women with twin pregnancies. Among the women with GDM in twin pregnancies, 38.37% had at least two abnormal blood glucose levels. The incidence of these parameters increased with preconception BMI, and the incidence of twin pregnancies was higher than that of singleton pregnancies (p < 0.001). In the normal weight and overweight/obese group, the oral glucose tolerance test glucose level and incidence of GDM were higher in women with twin than singleton pregnancies (p < 0.05). For twin pregnancies, the prevalence of selective fetal growth restriction was higher and anemia was lower in the GDM group than in the non-GDM group (all p < 0.05). Conclusion Therefore, a greater emphasis should be placed on BMI before conception, and well-controlled GDM does not increase adverse pregnancy outcomes for twin pregnancies.

Abstract Objects: Radiotherapy (RT) serves as the most effective treatment for Nasopharyngeal Carcinoma (NPC) and can cause carotid stenosis. The aim of this study is to assess the impact of RT on carotid stenosis in NPC patients, as well as to explore the risk factors for significant carotid stenosis. Methods: Studies reporting the carotid stenosis in NPC patients who underwent RT were found on PubMed, Embase and Web of Science. Outcomes of our interest included incidence of overall/significant stenosis, Common Carotid Artery (CCA) stenosis, External Carotid Artery (ECA) stenosis, Internal Carotid Artery (ICA) stenosis, and risk factors for significant carotid stenosis. Results: Sixteen studies met the inclusion criteria and were included in this meta-analysis. Pooled estimate showed that RT was associated with a significantly higher incidence of overall stenosis (Risk Ratio [RR = 3.53], 95% CI: 2.32–5.37; p < 0.001) and significant stenosis (RR = 7.06, 95% CI: 3.61–13.79; p < 0.001) as compared with controls. Moreover, patients treated with RT had a significantly higher risk of stenosis in CCA (RR = 6.87, 95% CI: 4.08–11.58; p < 0.001), ICA (RR = 3.43, 95% CI: 1.35–8.73; p= 0.010), ECA (RR = 9.37, 95% CI: 2.06–42.68; p = 0.004), and ECA/ICA (RR = 2.18, 95% CI: 1.52–3.13; p < 0.001). Meta-analysis indicated that age (RR = 1.46, 95% CI: 1.05–2.04; p = 0.024), smoking habit (RR = 1.20, 95% CI: 1.02–2.78; p = 0.045) and time interval from radiotherapy (RR = 1.56, 95% CI: 1.07–2.28; p = 0.02) were independent predictors of significant carotid stenosis. Conclusion: Our results suggested that RT increased the risk of carotid stenosis in patients with NPC. Prevention and control measurements should be made for older NPC patients with longer interval from RT, especially those with smoking habit. Level of evidence: 3.

Abstract Background: exercise can increase the species and quantity of beneficial gut microbiota, enrich the diversity of microflora, and promote the development of symbiotic bacteria, especially in the stage of ontogeny. However, there is little evidence of the short-term voluntary exercise effect on the gut microbiota in developing mice. Material and method: therefore, we used short-term voluntary wheel running model to study the gut microbiota of developing mice (1 month old), and detected the fecal samples by 16S rRNA gene sequencing. Results: the results showed that after 4 weeks of voluntary wheel running, the body weight of the running group was significantly lower than that of the control group. Conclusion: there was a significant separation between the running group and the control group in beta diversity measures. At the family level, the clostridiales flora of the running group was higher than that of the control group. Compared with the control group, the abundance of parabacteroides flora and anaerovorax flora increased significantly, and the abundance of anaerotruncus flora and odoribacter flora decreased significantly in the running group. These results showed that gut microbiota be affected after short-term voluntary wheel running in developing mice.

Sin embargo, hay poca evidencia del efecto del ejercicio voluntario a corto plazo sobre la microbiota intestinal en ratones en desarrollo. Material y método: por lo tanto, utilizamos un modelo de carrera de ruedas voluntario a corto plazo para estudiar la microbiota intestinal de ratones en desarrollo (1 mes de edad) y detectamos las muestras fecales mediante la secuenciación del gen 16S rRNA. Resultados: los resultados mostraron que después de 4 semanas de carrera voluntaria con ruedas, el peso corporal del grupo de carrera fue significativamente más bajo que el del grupo de control. Conclusión: hubo una diferencia significativa entre el grupo de corredores y el grupo de control en las medidas de diversidad beta. A nivel familiar, la flora de clostridiales del grupo de corredores fue mayor que la del grupo de control. En comparación con el grupo de control, la abundancia de flora parabacteroides y flora anaerovorax aumentó significativamente, y la abundancia de flora anaerotruncus y flora odoribacter disminuyó significativamente en el grupo de carrera. Estos resultados mostraron que la microbiota intestinal se ve afectada después de la carrera voluntaria a corto plazo en ratones en desarrollo.

Abstract In this paper, the relationship between Kernel average misorientation (KAM), geometrically necessary dislocation (GND) density and dislocation structures of Nb-bearing austenitic stainless steel under low cycle fatigue (LCF) was studied at 600°C at the total strain amplitude ranged from 0.3% to 1.0%. The results based on EBSD analysis show that the GND density in fatigue specimens gradually increases with the increase of strain amplitude. Under LCF loading, the dislocation structures are mainly planar slip bands (PSBs) and the cell structures. With the increase of strain amplitude, the number of PSBs increases with decrease in width, and the average diameter of cells also decreases. The PSBs originate due to the dynamic strain aging (DSA) effect, and DSA is more significant under high strain amplitude. The average diameter of cell structures has a specific relationship with GND density.

Abstract To investigate the effect of DNMT3A in vascular calcification (VC) induced by high phosphorus. The arterial tissues of 12 patients with end stage renal disease (ESRD) and VC and 12 patients with ESRD without VC were collected. Rat vascular smooth muscle cells (VSMCs) were divided into control group, high phosphorus (P) group, P + DMSO group, p-ERK1/2 inhibitor group, DNMT3A group and DNMT3A + P group and P + shRNA-DNMT3A group. Vascular calcification was evaluated by von kossa staining. Cell calcification was evaluated by alizarin red staining. The calcium content was assessed by calcium determination kit. The levels of DNMT3A, Runx2, LC3 and p-ERK1/2 were significantly up-regulated in CKD patients with VC in comparison with those in CKD patients without VC(p<0.05). Moreover, the levels of SM22α and P62 were notably decreased in CKD patients with VC in comparison with those in CKD patients without VC(p<0.05). Similar changes were observed in VSMCs induced by high phosphorus. Knock down of DNMT3A in VSMCs inhibited phenotypic transformation and induced autophagy, then reduced calcification(p<0.05). Moreover, p-ERK1/2 level was downregulated by knock down of DNMT3A in comparison with the control group(p<0.05). In conclusion, DNMT3A regulated high phosphorus induced vascular medial calcification via ERK1/2 signaling.
