RESUMO Objetivo: avaliar os resultados dos nossos pacientes que foram submetidos a transplante hepático por lesão iatrogênica do ducto biliar. Métodos: todos os pacientes que foram submetidos a transplante hepático para tratamento de complicações da lesão do ducto biliar foram incluídos no estudo. Os prontuários e protocolos de estudo desses pacientes foram analisados retrospectivamente para determinar características demográficas e clínicas, tratamento e desfecho dos pacientes. Resultados: de um total de 846 transplantes hepáticos realizados, 12 (1,4%) foram por lesão iatrogênica de via biliar: 10 (83,3%) ocorreram durante colecistectomia, 1 (8,3%) após quimioembolização e 1 (8,3%) durante laparotomia para controle de sangramento abdominal. A colecistectomia foi realizada por via aberta em 8 pacientes e por via laparoscópica em dois. Haviam 8 mulheres (66,7%) e 4 homens (33,3%), com média de idade de 50,6 ± 13,1 anos (variação de 23 a 70 anos). Todos os transplantes foram realizados com fígados de doadores cadavéricos. O tempo operatório médio foi de 565,2 ± 106,2 minutos (variação de 400-782 minutos). A reconstrução biliar foi realizada com hepaticojejunostomia em Y de Roux em 11 pacientes e coledococoledocostomia em um. Sete pacientes morreram (58,3%) e cinco (41,7%) estavam vivos durante um seguimento médio de 100 meses (variação de 18 a 118 meses). Conclusão: o transplante hepático em pacientes com lesão iatrogênica das vias biliares é um procedimento complexo com elevada morbimortalidade.

ABSTRACT Objective: to assess the outcomes of our patients who were subjected to LT for iatrogenic bile duct injury. Methods: all patients who underwent LT for treatment of complications of biliary duct injury were included in the study. Medical records and study protocols of these patients were retrospectively analyzed to determine demographic and clinical characteristics, treatment, and outcome of the patients. Results: of a total of 846 liver transplants performed, 12 (1.4%) were due to iatrogenic bile duct injury: 10 (83.3%) occurred during cholecystectomy, 1 (8.3%) following chemoembolization, and 1 (8.3%) during laparotomy to control abdominal bleeding. Cholecystectomy was performed by open access in 8 patients and by laparoscopic access in two . There were 8 female (66.7%) and 4 male (33.3%) with a mean age of 50.6 ± 13.1 years (range 23 to 70 years). All transplants were performed with livers from cadaveric donors. The mean operative time was 558.2 ± 105.2 minutes (range, 400-782 minutes). Biliary reconstruction was performed with Roux-en-Y hepaticojejunostomy in 11 patients and choledochocholedochostomy in one. Seven patients died (58.3%) and five (41.7%) were alive during a mean followed up of 100 months (range 18 to 118 months). Conclusion: liver transplantation in patients with iatrogenic bile duct injury is a complex procedure with elevated morbimortality.

Contexto O hipogonadismo é uma situação clínica comum em pacientes do sexo masculino com cirrose hepática. Objetivos O objetivo do presente estudo foi avaliar os efeitos do transplante hepático ortotópico nos níveis de testosterona, testosterona livre e globulina de ligação do hormônio sexual na doença hepática avançada em homens e, determinar a relação dessas mudanças com o Model for End-stage Liver Disease (MELD). Métodos Em um estudo prospectivo, os níveis séricos de testosterona, testosterona livre e globulina de ligação do hormônio sexual de 30 pacientes adultos do sexo masculino com doença hepática em estágio final foram medidos 2 a 4 horas antes e 6 meses após transplante hepático ortotópico. Resultados Os níveis de testosterona total aumentaram após transplante hepático ortotópico e o número de pacientes com níveis normais de testosterona aumentou de 18 para 24. O nível médio de testosterona livre no grupo de pré-transplante foi de 7,8 pg/mL, aumentou para 11,5 pg/mL (P = 0,10) e globulina de ligação do hormônio sexual diminuiu após transplante hepático ortotópico retornando aos níveis normais no grupo com MELD ≤18 - grupo (A) (P<0,05). Conclusões As mudanças nos níveis séricos de testosterona, testosterona livre e globulina de ligação do hormônio sexual são mais pronunciadas em homens com cirrose e MELD ≤18. Os níveis séricos de testosterona e testosterona livre aumentaram e de globulina de ligação do hormônio sexual reduziram após transplante hepático ortotópico. Os níveis de testosterona livre aumentaram em pacientes com cirrose hepática avançada após transplante hepático ortotópico enquanto globulina de ligação do hormônio sexual diminuiu após transplante hepático ortotópico e estas alterações foram mais intensas em pacientes com MELD ≤18.

Context Hypogonadism is a common clinical situation in male patients with liver cirrhosis. Objectives The aim of the present study was to evaluate the effects of orthotopic liver transplantation on testosterone, free testosterone and sex hormone-binding globulin in male with advanced liver disease and also to determine the relationship of these changes with Model for End-stage Liver Disease (MELD) score. Methods In a prospective study, serum levels of testosterone, free testosterone and sex hormone-binding globulin of 30 male adult patients with end-stage liver disease were measured 2 to 4 hours before and 6 months after orthotopic liver transplantation. Results Total testosterone levels increased after orthotopic liver transplantation and the number of patients with normal testosterone levels increased from 18 to 24. Free testosterone mean level in the pre-transplant group was 7.8 pg/mL and increased to 11.5 pg/mL (P = 0.10) and sex hormone-binding globulin level decreased after orthotopic liver transplantation returning to normal levels in MELD ≤18 - group (A) (P<0.05). Conclusions Serum level changes of testosterone, free testosterone and sex hormone-binding globulin are more pronounced in cirrhotic males with MELD ≤18. Serum levels of testosterone and free testosterone increase and serum levels of sex hormone-binding globulin decrease after orthotopic liver transplantation.

RACIONAL: Variações anatômicas das vias biliares são freqüentes e aumentam as complicações dos transplantes hepáticos. OBJETIVO: Avaliar a anatomia das vias biliares nos doadores e receptores do transplante hepático inter vivos realizados no Hospital de Clínicas da Universidade Federal do Paraná e do Hospital Nossa Senhora das Graças de Curitiba, Curitiba, PR. CASUÍSTICA E MÉTODOS: A avaliação foi retrospectiva de março de 1998 até setembro de 2002 (23 transplantes), quando os dados passaram a ser captados de forma prospectiva (17 transplantes), até agosto de 2003. Foram obtidos dados de 80 pacientes consecutivos (40 doadores e 40 receptores), submetidos a transplante hepático inter vivos, sendo 32 transplantes com receptor adulto e 8 transplantes pediátricos (receptor com idade inferior a 15 anos). Entre os 80 pacientes incluídos no estudo, 51 eram do sexo masculino (27 receptores e 24 doadores) e 29 do feminino (13 receptores e 16 doadores). A idade média dos doadores foi de 32,6 anos e a dos receptores de 36,3 anos. RESULTADOS: Nas cirurgias de captação do lobo hepático direito, a via biliar direita foi única em 25 entre 29 casos (86,2%), dupla em dois (6,9%) e tripla em dois (6,9%). Nos oito casos em que o setor lateral esquerdo ou o lobo hepático esquerdo foram captados, a via biliar foi única. Entre os 33 pacientes submetidos a colangiografia por ressonância magnética, variações foram verificadas em três pacientes (9,1%), sendo a mais comum o ducto biliar posterior direito tributário do ducto hepático esquerdo (6,06%; n = 2). CONCLUSÃO: A prevalência de anomalias biliares nos transplantes inter vivos de fígado é elevada e algumas não são diagnosticadas com os exames de imagem pré-operatórios.

BACKGROUND: Anatomic variations of the biliary tree are frequent and increase complications after liver transplantation. AIM: To describe the anatomy of the bile ducts of donors and recipients of living related liver transplantation. METHODS: From March 1998 to September 2002, the study was retrospective (23 transplantations). From October 2002 to August 2003, the study was prospective (17 transplantations). We studied the hepatic anatomy of 80 consecutive patients (40 donors and 40 recipients) of the living-related liver transplantation program of the "Hospital de Clínicas da Universidade Federal do Paraná" and the "Hospital Nossa Senhora das Graças", Curitiba, PR, Brazil; 51 were male (27 recipients 24 donors) and 29 female (13 recipients and 16 donors). The median age among the donors was 32.6 years and among the recipients was 36.3 years. Thirty-two recipients were adults and 8 recipients were under 15 years old. The bile duct anatomy was studied by magnetic resonance cholangiography in 33 patients, and anomalies were seen in 3 of them (9.1%). RESULTS: The most prevalent variation of bile ducts was the fusion of the right posterior duct with the left duct (6.06%; n=2). In the 40 harvesting operations, the right bile duct was single in 25 patients (87.5%), among the 32 right-lobe donors, double in 2 (6.25%) and triple in 2 (6.25%). All of the eight left livers procured had single bile ducts. Among the 40 recipients, the common bile duct was bifurcated in 2 of them (5%). CONCLUSION: The prevalence of biliary anomalies is high in patients subjected to living liver transplantation and some anomalies are not diagnosed with preoperative imaging exams.

MÉTODO: A avaliação foi retrospectiva de março de 1998 até setembro de 2002 (23 transplantes), quando os dados passaram a ser captados de forma prospectiva (17 transplantes), até agosto de 2003. Foram obtidos dados de 80 pacientes consecutivos (40 doadores e 40 receptores), submetidos a transplante hepático intervivos, sendo 32 transplantes com receptor adulto e 8 transplantes pediátricos (receptor com idade inferior a 15 anos). Entre os 80 pacientes incluídos no estudo, 51 eram do sexo masculino (27 receptores e 24 doadores) e 29 eram do sexo feminino (13 receptores e 16 doadores). A idade média dos doadores foi de 32,6 anos e a dos receptores de 36,3 anos. RESULTADOS: No estudo da anatomia da artéria hepática, realizaram-se arteriografias em 43 pacientes, e variações anatômicas foram encontradas em 18 casos (41,86%), sendo a mais comum a artéria hepática direita ramo da artéria mesentérica superior (12,5%; n=5); no estudo da anatomia arterial realizado nas cirurgias, foram verificadas variações em 16 casos (20%) entre os 80 casos estudados, sendo a mais comum a artéria hepática direita como ramo da artéria mesentérica superior (7,5%; n=6). CONCLUSÃO: Conclui-se que a prevalência de variações na anatomia da artéria hepática é elevada.

BACKGROUND: Detailed knowledge of hepatic artery anatomy is essential for the success of living related liver transplantation. The purpose of this study is to describe the anatomy of the hepatic artery of donors and recipients of living related liver transplantation. METHODS: From March 1998 to September 2002, the study was retrospective (23 transplantations). From October 2002 to August 2003, the study was prospective (17 transplantations). We studied the hepatic artery anatomy of 80 consecutive patients (40 donors and 40 recipients) at the Liver Transplantation Unit of the Hospital de Clínicas of the Universidade Federal do Paraná and of the Hospital Nossa Senhora das Graças; 51 were male (27 recipients and 24 donors) and 29 female (13 recipients and 16 donors). The median age of the donors was 32.6 years and of the recipients was 36.3 years. Thirty-two recipients were adults and 8 were under than 15 years of age. RESULTS: Forty-three patients underwent abdominal arteriography and variations of the hepatic artery anatomy were found in 18 of them (41.86%). The most prevalent anomaly was the right hepatic artery arising from the superior mesenteric artery (12.5%; n=5). The anatomy of the hepatic artery was also evaluated at operation and variations were observed in 16 of the 80 patients (20%). The most common variation observed at operation was the right hepatic artery arising from the superior mesenteric artery (7.5%; n=6). CONCLUSION: It is concluded that the prevalence of hepatic arterial variations is high.

OBJETIVO: Avaliar a qualidade de vida sexual masculina antes e depois do transplante hepático. MÉTODOS: Foi enviado questionário de avaliação para 56 doentes masculinos, que tinham idade superior a 18 anos e que sobreviveram mais de seis meses após o transplante hepático. O questionário continha 15 perguntas com cinco ou seis alternativas de resposta para mensurar a função sexual masculina segmentada por cinco componentes: função erétil, função orgástica, desejo sexual, satisfação com a relação sexual e satisfação com a vida sexual como um todo. As respostas geraram valores numéricos, os quais foram agrupados dentro de cada domínio e comparados antes e depois do transplante hepático. RESULTADOS: Vinte e cinco doentes responderam completamente o questionário. Todas os cinco componentes da função sexual melhoraram após o transplante hepático, sendo que o escore da função erétil aumentou de 21,12±8,07 para 26,52±5,22 (p=0,004), da função orgástica de 7,28±3,05 para 9,36±1,47 (p=0,008), da função desejo sexual de 6,64±2,58 para 8,68±1,35 (p=0,005), da satisfação com relação sexual de 9,16±3,83 para 12,52±2,65 (p<0,0001) e da satisfação com a vida sexual de 7,12±2,64 para 9,24±1,65 (p=0,002). CONCLUSÕES: A disfunção sexual é comum em homens com hepatopatia crônica grave e o transplante hepático melhora todos os componentes da função sexual: função erétil, orgasmo, desejo sexual, satisfação na relação sexual e satisfação pessoal.

BACKGROUND: Sexual dysfunction is very common in liver transplantation candidates. Our objective is to determine the sexual life quality of males before and after liver transplantation. METHODS: Questionnaire was sent to 56 males over 18 years of age with at least six-month survival after orthotopic liver transplantation. The self-administered questionnaire contained 15 questions with 5 or 6 alternatives to determine the male sexual function which may be divided into 5 domains: 1) erectile function; 2) orgasmic function; 3) sexual desire; 4) intercourse satisfaction; and 5) overall satisfaction with sexual life. Each answer received a score. Domains scores were computed by summing the scores for individual answers and they were compared before and after the liver transplantation. RESULTS: Twenty-five patients answered the questionnaire completely. All 5 sexual function domains improved after liver transplantation. The score of the erectile function increased from 21.12± 8.07 to 26.52± 5.22 (p=0.004), of the orgasmic function from 7.28± 3.05 to 9.36± 1.47 (p=0.008), of the sexual desire from 6.64± 2.58 to 8.68± 1.35 (p=0.005), of intercourse satisfaction from 9.16± 3.83 to 12.52± 2.65 (p<0.0001) and of overall satisfaction from 7.12± 2.64 to 9.24± 1.65 (p=0.002). CONCLUSIONS: Sexual dysfunction is common in males with severe chronic liver disease and liver transplantation improves all sexual function domains: erectile function, orgasmic function, sexual desire, intercourse satisfaction and overall satisfaction.

OBJETIVO: O presente estudo tem por objetivo avaliar a anatomia arterial hepática em doadores e receptores de 150 transplantes hepáticos. MÉTODOS: 246 pacientes foram analisados, 129 doadores e 117 receptores de fígado. RESULTADOS: A anatomia arterial hepática era normal em 189 (76,82%) pacientes. Alterações anatômicas foram encontradas nos demais 57 (23,18%), sendo as principais: artéria hepática direita ramo da artéria mesentérica superior, artéria hepática esquerda ramo da artéria gástrica esquerda, artéria hepática direita ramo da artéria mesentérica superior associada à artéria hepática esquerda ramo da artéria gástrica esquerda e artéria hepática comum ramo da artéria mesentérica superior. Algumas anomalias raras foram visualizadas. CONCLUSÕES: Os achados deste estudo demonstram a variabilidade da anatomia do sistema arterial hepático e alertam para a necessidade de cautela nas dissecções cirúrgicas, principalmente nas captações de enxerto dos transplantes de fígado, para se evitar comprometimento do suprimento sangüíneo hepático.

BACKGROUND: The aim of the present study is to evaluate the hepatic arterial anatomy in donors and recipients of 150 liver transplantations. METHODS: 246 patients were analyzed, 129 donors and 117 liver receptors. RESULTS: Normal arterial anatomy of the liver was seen in 189 (76,82%) patients. Anatomic anomalies were observed in the other 57 (23,18%); the most frequent were: right hepatic artery arising from the superior mesenteric artery, left hepatic artery arising from the left gastric artery, right hepatic artery arising from the superior mesenteric artery associated with left hepatic artery arising from the left gastric artery and , common hepatic artery arising from the superior mesenteric artery. Some rare anomalies were also seen. CONCLUSIONS: The findings of this study show a high anatomic variability of the hepatic arterial system. The surgeon must be careful during liver dissection both of donors and receptors in order to avoid inadvertent damage to the anomalous arteries to the liver.

A reconstrução biliar é um dos pontos vulneráveis do transplante hepático apresentando incidência de complicações biliares, variando de 10 a 35%, nos diversos estudos da literatura. Esse trabalho tem por objetivo apresentar a experiência do nosso serviço em relação à incidência e ao manejo das complicações biliares no transplante de fígado. Foram incluídos no estudo 147 transplantes hepáticos ortotópicos, com idade média de 37,3 anos, correspondendo a 88 procedimentos em pacientes do sexo masculino e 59 do sexo feminino. Complicações biliares ocorreram em 27 transplantes (18,36%) em 25 pacientes (dois retransplantes). A presença de rejeição celular e de complicações vasculares foi identificada como fator de risco para as complicações biliares. A idade, o sexo, a etiologia da cirrose e a técnica utilizada na reconstrução biliar não foram fatores de risco. No total, foram empregados 52 cursos terapêuticos: tratamento cirúrgico em 23 vezes; tratamento endoscópico em 15 vezes; retransplante em sete vezes; drenagem biliar transparieto-hepática em seis vezes e um paciente está em lista de espera para retransplante. Conclui-se deste estudo que as complicações biliares são freqüentes após o transplante hepático e que as vasculares e a rejeição celular são fatores de risco.

Biliary reconstruction is a vulnerable step of liver transplantation, presenting an incidence of biliary complications between 10 to 35% in many studies. Our aim is to present our incidence and treatment of biliary complications, in a total of 147 orthotopic liver transplantations, 88 males and 59 females, with an mean age of 37 years. Biliary complications occurred in 27 transplants (18,36%) performed in 25 patients (two retransplants). Cellular rejection and vascular complications were identified as risk factors for biliary complications. Age, sex, etiology of cirrhosis and biliary reconstruction technique were not risk factors. Fifty-two therapeutic courses were performed: 23 surgical treatments, 15 endoscopic treatments, seven retransplants, six external biliary drainage and one patient is waiting retransplantation. In conclusion, biliary complications are frequent after liver transplantation and vascular complications and cellular rejection are risk factors for them.

Gastric bezoars are impactations offoreign material in lhe stomach. When they are caused by hail; they are named tricho- bezoars. The complications oftrichobezoars are very rare. In this papel; we describe a case of a 16-year-old girl that had a previous history oftricophagia, and had an acute abdominal pain with a pneumoperitoneum in the abdomen radiography. An operation was performed and a gastric perforation was founded associated with a giant trichobezoa7: The trichobezoar was removed by traction through a gastrostomy which was performed in order to remove lhe trichobezoa7: Some fragments of the ulcer were obtained to histological study. The gastrostomy was treated by a gastrorraphy confection. In lhe post- operative period a left subfrenic abscess was revealed and has been drained by laparatomy 15 days after the fisrt operation. After the second surgical procedure the patient had a good evolution, and left the hospital in good health conditions.
