RESUMEN Objetivos. Determinar la asociación entre el tiempo de enfermedad previo a broncoscopia y el tiempo de alta posbroncoscopia en pacientes pediátricos con aspiración de cuerpo extraño. Materiales y métodos. Estudio de cohorte retrospectivo. El estudio y la revisión de historias clínicas se realizó en el Hospital de Emergencias Pediátricas de Lima, Perú. Se revisaron 324 historias clínicas, de las cuales, 183 historias fueron seleccionadas por contar con diagnóstico de cuerpo extraño en vías aéreas y estar adecuadamente llenadas. Para el análisis bivariado se utilizó la prueba exacta de Fisher y U de Mann Whitney, mientras que para el cálculo del riesgo relativo (RR) y su respectivo intervalo de confianza (IC) al 95% se utilizó regresión de Poisson. Resultados. Se incluyeron 183 pacientes, de los cuales, el 65,6% fueron hombres con una media de 2,4 años. La localización más frecuente fue el árbol bronquial derecho y cuerpo extraño de material orgánico. La mayoría (72,7%) de pacientes tuvieron un alta temprana antes de las 24 horas. Se encontró una asociación entre el tiempo de enfermedad previo a la broncoscopia y el tiempo alta posbroncoscopia (RR: 2,94, IC 95%: 1,72-5,01). Conclusiones. Existe una asociación estadísticamente significativa entre el tiempo de enfermedad previo a la broncoscopia y el tiempo de estancia hospitalaria posextracción del cuerpo extraño al ajustar por edad, sexo tipo de cuerpo extraño y maniobra de boca como medida de rescate. Este hallazgo es relevante debido a que pone en evidencia la importancia de una atención temprana, diagnóstico oportuno y manejo precoz en el paciente pediátrico.

ABSTRACT Objective. To determine the association between pre-bronchoscopy time of illness and post-bronchoscopy discharge time in pediatric patients with foreign body aspiration. Materials and methods. Retrospective cohort study. Medical records were studied and reviewed at the Pediatric Emergency Hospital of Lima, Peru. We reviewed 324 medical records, and selected 183 because they had the diagnosis of foreign body aspiration and complete data. Fisher’s exact test and Mann Whitney U test were used for the bivariate analysis, while Poisson regression was used to calculate the Relative Risk (RR) and its 95% confidence interval (CI). Results. We included 183 patients, of whom 65.6% were men with a mean age of 2.4 years. The most frequent location of the foreign body was the right bronchial tree and most of them were made of organic material. The majority (72.7%) of patients were discharged before 24 hours. We found an association between the time of illness prior to bronchoscopy and post-bronchoscopy discharge time (RR: 2.94, 95%CI: 1.72 - 5.01). Conclusions. The time of illness prior to bronchoscopy and the length of hospital stay after removal of the foreign body were significantly associated when adjusted for age, sex, type of foreign body and mouth sweep maneuver as a rescue measure. Our findings are relevant because they highlight the importance of early care, timely diagnosis and early management of pediatric patients.

ABSTRAC Objective: To determine the association between glycemia values and mortality in the Pediatric Intensive Care Unit. The study: Retrospective cohort study in pediatric patients between 1 month and 18 years; hospitalized in the PICU for more than 48 hours. Findings: 184 patients were included. The median age was 33.5 months; mortality was 11.54%. The adjusted analysis showed that maximum glucose in 24 hours and PIM2 scale value could be presented as risk factors for mortality, unlike sex, age, and Glasgow scale value, which were presented as protective factors. Conclusion: In the series evaluated, an association was found between the value of maximum glucose in the first 24 hours and mortality in the PICU. Prospective studies are needed to assess this association.

RESUMEN Objetivo: Determinar la asociación entre los valores de glicemia y mortalidad en la Unidad de Cuidados Intensivos Pediátricos (UCIP) del Hospital de Emergencias Pediátricas. El estudio: Estudio de cohorte retrospectivo, en pacientes pediátricos entre 1 mes y 18 años; hospitalizados en la UCIP del Hospital de Emergencias Pediátricas por más de 48 horas, durante el año 2016. Hallazgos: Se incluyeron 184 pacientes. La mediana de edad fue de 33.5 meses, la mortalidad fue de 11.54%. El análisis ajustado demostró que glucosa máxima en 24 horas y valor en la escala de Pediatric Index of Mortality 2 (PIM2) podrían presentarse como factores de riesgo para mortalidad, a diferencia de sexo, edad y valor de escala de Glasgow que se presentaron como factores protectores. Conclusión: En la serie evaluada se encontró asociación entre el valor de glucosa máxima en las primeras 24 horas y mortalidad en la UCIP. Es preciso realizar estudios prospectivos para evaluar dicha asociación

RESUMEN La pandemia de la COVID-19 cambió el patrón de asistencia de los pacientes a los centros de salud. Se describen los cambios comparativos en la asistencia de pacientes pediátricos que acudieron al servicio de Emergencia del Hospital de emergencias Pediátricas de Lima, Perú, entre las semanas epidemiológicas 10 a 48 del 2019 y 2020. Se analizaron datos sociodemográficos, condición del paciente, tipo de seguro y diagnóstico agrupado según código CIE-10. Se encontró una reducción del 48,2% en el número de atenciones en el 2020 con respecto al 2019 y un aumento de cinco veces el número de atenciones procedentes de localidades diferentes de Lima Metropolitana. Asimismo, se encontró un aumento del 27% en la probabilidad del diagnóstico de «enfermedades infecciosas y parasitarias» y una disminución del 61% en la probabilidad de diagnóstico de «enfermedades del sistema respiratorio».

Sociodemographic data, patient condition, type of insurance and diagnosis grouped according to ICD-10 code were analyzed. We found a 48.2% reduction in the number of visits in 2020 when compared to 2019, and a five-fold increase in the number of visits from localities other than Metropolitan Lima. Likewise, the probability of diagnosis of “infectious and parasitic diseases” increased by 27% and the probability of diagnosis of “diseases of the respiratory system” decreased by 61%.

Resumen: Objetivo: Evaluar la asociación entre la sobrecarga de fluido (SF) y mortalidad en pacientes hospitalizados en la Unidad de Cuidados Intensivos Pediátricos (UCIP). Pacientes y Método: Se realizó un estudio de tipo cohortes histórica. Se incluyeron pacientes pediátricos mayores de un mes y menores de 18 años que se encontraban hospitalizados en la UCIP por más de 48 horas durante el año 2016. Se registraron datos demográficos y clínicos. La SF se calculó como [Fluido ingresado (L) - Fluido eliminado (L) /peso al ingreso (Kg)] x 100. Para análisis multivariado, se usó el mo delo de regresión de Poisson. Resultados: Se incluyeron 171 pacientes. La mediana de edad fue de 31 meses (RIQ 8;84) y la mortalidad del 8,18%. La SF en la población sobreviviente fue de 7% y 11,5% en la población fallecida (p < 0,05). El análisis ajustado asoció la sobrecarga de fluidos con un mayor riesgo de mortalidad, con un Riesgo Relativo de 1,32 (1,24 -1,40), mientras que edad y valor de la escala de Glasgow se asociaron con menor riesgo. Conclusión: La sobrecarga de fluidos es un factor de riesgo independiente para mortalidad en la población de la Unidad de Cuidados Intensivos Pediátrica.

Abstract: Objective: To assess the association between fluid overload (FO) and other risk factors in the mortality of patients admitted to the Pediatric Intensive Care Unit (PICU). Patients and Method: A historical cohort study was conducted. Pediatric patients older than one month and younger than 18 years who were hospitalized in the PICU for more than 48 hours during 2016 were included. Demographic and clinical data were recorded. FO was calculated as [Sum of daily (fluid in - fluid out)/weight at ICU admission] x 100. Poisson regression analysis was performed to determine factors associated with mortality. Results: 171 patients were included. The median age was 31 months (RIQ 8; 84). Mortality was 8.18%. FO in the surviving population was 7% and 11.5% in the deceased patients (p < 0.05). The adjusted analysis identified FO as a major risk factor for mortality with a Relative Risk 1.32 (1.24 - 1.40); age and Glasgow Coma Scale were protective factors. Conclusion: Fluid overload is an independent risk factor for mortality in the analyzed PICU cohort.

Abstract: OBJECTIVE: Determine the risk of requiring hospitalization in the neonatal intensive care unit of the children of patients with preeclampsia, according to their type and time of onset. MATERIALS AND METHODS: Retrospective cohort study executed at Hospital Nacional Docente Madre Niño San Bartolomé, which included pregnant women who met the diagnosis of preeclampsia. The risk ratio of admission to the Neonatal Intensive Care Unit from pregnant women with early onset preeclampsia and late onset preeclampsia was evaluated through Poisson regression with robust variance and adjusted by the confounding variables. RESULTS: This study included 369 women who met the diagnosis of preeclampsia. The percentage of admission to the Neonatal Intensive Care Unit was 7,0% and the incidence of preeclampsia during the years 2015 and 2016 was 3,04%. In the bivariate analysis, the variables type of preeclampsia (early onset and late onset), sepsis and neonatal seizures, birth weight, weight for the gestational age, Apgar score and perinatal mortality were statistically significant (p <0.05) for admission to Neonatal Intensive Care Unit. In the adjusted multivariate analysis, the children of pregnant women with EOP had a higher risk of being admitted to Neonatal Intensive Care Unit than those of pregnant women with late onset preeclampsia (aRR = 19,51; 95% CI 9,74- 39,11; p<0,001). CONCLUSIONS: The neonates of pregnant women with early onset preeclampsia have a higher risk of being admitted to the Neonatal Intensive Care Unit compared to the neonates of pregnant women with LOP.

Resumen: OBJETIVO: Determinar el riesgo de requerir internamiento en la unidad de cuidados intensivos neonatales de los hijos de pacientes con preeclampsia, según su tipo y tiempo de inicio. MATERIALES Y MÉTODOS: Estudio cohorte, retrospectivo, efectuado en el Hospital Nacional Docente Madre Niño San Bartolomé entre 2015 y 2016. Se incluyeron las embarazadas que cumplieron con el diagnóstico de preeclampsia. El riesgo relativo de admisión a la unidad de cuidados intensivos neonatales de los hijos de pacientes con preeclampsia de inicio temprano y tardío se determinó mediante la regresión de Poisson, con varianza robusta y modelo ajustado. RESULTADOS: Se incluyeron 369 pacientes que cumplieron el diagnóstico de preeclampsia. El porcentaje de ingreso a la unidad de cuidados intensivos neonatales fue de 7.0% y la incidencia de preeclampsia (2015-2016) 3.04%. En el análisis bivariado resultaron estadísticamente significativos para el ingreso a la unidad de cuidados intensivos neonatales (p < 0.05): el tipo de preeclampsia (de inicio temprano o tardío), sepsis y convulsiones neonatales, peso al nacer, peso para la edad gestacional, puntaje de Apgar y mortalidad perinatal. En el análisis multivariado ajustado, los hijos de las pacientes con preeclampsia de inicio temprano tuvieron mayor riesgo de ser internados en la unidad de cuidados intensivos neonatales que los hijos de las embarazadas con preeclampsia de inicio tardío (RRa = 19.51; IC95%: 9.74-39.11; p < 0.001). CONCLUSIONES: Los hijos de las pacientes con preeclampsia de inicio temprano tienen mayor riesgo de requerir ser ingresados a la unidad de cuidados intensivos neonatales vs los hijos de las embarazadas con preeclampsia de inicio tardío.

ABSTRACT CONTEXT: The seeds from Lupinus mutabilis Sweet, also called “chocho”, are an important part of the diet in several countries in South America. Prior to consumption, processing is required to remove toxic alkaloids. These alkaloids are known to have pharmacological properties as antiarrhythmics, antimuscarinics and hypoglycemics. CASE REPORT: We report a case in which a one-year-old male initially presented with altered mental status and respiratory distress and subsequently developed symptoms of anticholinergic toxicity, after ingesting a large amount of chocho seeds. CONCLUSION: In spite of going through a difficult clinical condition, the subject evolved favorably through receiving supportive treatment. The seeds from Lupinus mutabilis provide nutritional benefits when consumed, but people need to know their risks when these seeds are consumed without proper preparation.

ABSTRACT Objective: To determine the risk factors for extubation failure in the intensive care unit. Methods: The present case-control study was conducted in an intensive care unit. Failed extubations were used as cases, while successful extubations were used as controls. Extubation failure was defined as reintubation being required within the first 48 hours of extubation. Results: Out of a total of 956 patients who were admitted to the intensive care unit, 826 were subjected to mechanical ventilation (86%). There were 30 failed extubations and 120 successful extubations. The proportion of failed extubations was 5.32%. The risk factors found for failed extubations were a prolonged length of mechanical ventilation of greater than 7 days (OR = 3.84, 95%CI = 1.01 - 14.56, p = 0.04), time in the intensive care unit (OR = 1.04, 95%CI = 1.00 - 1.09, p = 0.03) and the use of sedatives for longer than 5 days (OR = 4.81, 95%CI = 1.28 - 18.02; p = 0.02). Conclusion: Pediatric patients on mechanical ventilation were at greater risk of failed extubation if they spent more time in the intensive care unit and if they were subjected to prolonged mechanical ventilation (longer than 7 days) or greater amounts of sedative use.

RESUMEN Objetivo: Determinar los factores de riesgo para fracaso en la extubación en la unidad de cuidados intensivos. Métodos: El presente estudio de casos y controles se llevó a cabo en la unidad de cuidados intensivos. Se tomó como casos a las extubaciones fallidas y como controles a las extubaciones exitosas. El fracaso de la extubación se definió como la reintubación dentro de las primeras 48 horas. Resultados: De un total de 956 pacientes que fueron admitidos en la unidad de cuidados intensivos, 826 fueron sometidos a ventilación mecánica (86%). Se presentaron 30 extubaciones fallidas y 120 extubaciones exitosas. La proporción de extubaciones fallidas fue de 5,32%. Los factores de riesgo encontrados para extubaciones fallidas fueron la estancia prolongada de ventilación mecánica mayor a 7 días (OR = 3,84; IC95% = 1,01 - 14,56; p = 0,04), el tiempo en unidad de cuidados intensivos (OR = 1,04; IC95% = 1,00 - 1,09; p = 0,03) y el uso de sedantes mayor a 5 días (OR = 4,81; IC95% = 1,28 - 18,02; p = 0,02). Conclusión: Los pacientes pediátricos en ventilación mecánica tienen más riesgo de presentar extubaciones fallidas si permanecen mayor tiempo en unidad de cuidados intensivos, si están sometidos a tiempo prolongado de ventilación mecánica mayor de 7 días y al uso de sedantes.

ABSTRACT Objectives. To assess the health related quality of life in a sample of elderly population in a rural and urban Peruvian area. Materials and methods. A cross-sectional study was performed in elderly population from four rural districts and one urban from the period October 2014 to January 2016. The association between the area of residence and five sociodemographic variables was assessed with the chi square test. Using the Wilcoxon rank sum test and assessing size effect, WHOQoL-OLD and WHOQoL-BREF domain scores and global score of the former obtained in the survey were measured and compared. Then, simple and multiple regressions were performed to adjust values to the sociodemographic differences. Results. A total of 447 elderly people were surveyed with a mean age of 69 (DE=6,46), 207 from the rural area, most of whom worked and had less years of education.. The elderly population from the rural area had a higher quality of life in the domains "Physical", "Psychological" and "Environment" from WHOQoL-BREF and in "Sensory abilities", "Autonomy", "Past, Present and Future Activities", "Social participation" and the total score from WHOQoL-OLD, while those from the urban area only had a higher quality of life in "Intimacy". Conclusions. The area of residency exerts a differential effect on the distinct domains of health related quality of life in the evaluated elderly population

Estudio transversal en población adulta mayor de cuatro distritos rurales y uno urbano realizado entre octubre del año 2014 a enero de 2016. Se evaluó la asociación entre el área de residencia y cinco variables sociodemográficas mediante la prueba de chi cuadrado. Utilizando la prueba de suma de rangos de Wilcoxon y evaluando magnitud de efectos se midieron y compararon los puntajes por dimensiones obtenidos en los instrumentos WHOQOL-BREF y WHOQOL-OLD y el puntaje total de este último. Se realizó regresión lineal simple y múltiple para hallar los valores ajustados a las diferencias sociodemográficas. Resultados. Se encuestaron a 447 adultos mayores con una media de edad de 69 años (+ 6,46 años), 207 del área rural, los cuales, en su mayoría, trabajaban y tenían menos años de educación. La población adulta mayor del área rural tuvo mayor calidad de vida en las dimensiones "Física", "Psicológico" y "Medioambiente" del WHOQoL-BREF y en "Habilidades sensoriales", "Autonomía", "Actividades del pasado, presente y futuro", "Participación social" y en el puntaje total del WHOQoL-OLD; mientras que aquellos del área urbana sólo mostraron mayor calidad de vida en la dimensión "Intimidad". Conclusiones. El área de residencia ejerce un efecto diferencial en las distintas dimensiones de la calidad de vida relacionada a salud de la población adulta mayor estudiada.

An influenza A H1N1 infection was confirmed via polymerase chain reaction analysis of a nasopharyngeal swab. The patient, who was not treated with oseltamavir, has responded favorably to supportive measures.

Objectives. To determine whether there are demographic, clinical and radiological differences among patients with pulmonary tuberculosis (TB) and patients with TB and type 2 diabetes mellitus (DM2 + TB). Materials and methods. Observational retrospective cohort study. We compared the clinical characteristics of patients according to sex, age, time to sputum conversion to negative, presence of cavitation and the cure rate, duration of treatment and the proportion of change of treatment regimen, in patients with and without DM2 served by the Tuberculosis Control Program from 2010 to 2012 in the Rebagliati Healthcare Network of Lima, Peru. Results. 31 patients with TB+DM2 and 144 patients with TB were included. Differences (p<0.05) in the diagnostic method, the average of symptoms and the resistance pattern of TB among patients with and without DM2 were found. The presence of cavitation was more frequent in patients with TB + DM2. Having TB + DM2 delayed the time to sputum smear conversion to negative (RRa 4.16, 95% CI: 1.1-1.6) in the adjusted Cox regression analysis. Conclusions. There are differences in demographic, clinical and radiological characteristics in TB patients with and without DM2.The time to sputum conversion to negative is greater in patients with DM2

Objetivos. Conocer si existen diferencias en las características demográficas, clínicas y radiológicas entre los pacientes con tuberculosis pulmonar (TB) y pacientes con TB y diabetes mellitus tipo 2 (TB+DM2). Materiales y métodos. Estudio observacional de cohorte retrospectiva. Se compararon las características clínicas de los pacientes según el sexo, la edad, el tiempo de negativización del esputo, la presencia de cavitación así como la tasa de curación, duración del tratamiento y la proporción del cambio en el esquema del tratamiento antituberculoso en pacientes con y sin DM2 atendidos por el Programa de Control de Tuberculosis entre 2010 y 2012 en la Red Asistencial Rebagliati de Lima, Perú. Resultados. Se incluyeron 31 pacientes con TB+DM2 y 144 pacientes con TB. Objectives. To determine whether there are demographic, clinical and radiological differences among patients with pulmonary tuberculosis (TB) and patients with TB and type 2 diabetes mellitus (DM2 + TB). Materials and methods. Observational retrospective cohort study. We compared the clinical characteristics of patients according to sex, age, time to sputum conversion to negative, presence of cavitation and the cure rate, duration of treatment and the proportion of change of treatment regimen, in patients with and without DM2 served by the Tuberculosis Control Program from 2010 to 2012 in the Rebagliati Healthcare Network of Lima, Peru. Results. 31 patients with TB+DM2 and 144 patients with TB were included. Differences (p<0.05) in the diagnostic method, the average of symptoms and the resistance pattern of TB among patients with and without DM2 were found. The presence of cavitation was more frequent in patients with TB + DM2. Having TB + DM2 delayed the time to sputum smear conversion to negative (RRa 4.16, 95% CI: 1.1-1.6) in the adjusted Cox regression analysis. Conclusions. There are differences in demographic, clinical and radiological characteristics in TB patients with and without DM2.The time to sputum conversion to negative is greater in patients with DM2

Objetivos. Conocer si existen diferencias en las características demográficas, clínicas y radiológicas entre los pacientes con tuberculosis pulmonar (TB) y pacientes con TB y diabetes mellitus tipo 2 (TB+DM2). Materiales y métodos. Estudio observacional de cohorte retrospectiva. Se compararon las características clínicas de los pacientes según el sexo, la edad, el tiempo de negativización del esputo, la presencia de cavitación así como la tasa de curación, duración del tratamiento y la proporción del cambio en el esquema del tratamiento antituberculoso en pacientes con y sin DM2 atendidos por el Programa de Control de Tuberculosis entre 2010 y 2012 en la Red Asistencial Rebagliati de Lima, Perú.
