ABSTRACT Purpose analyze the effects of hospitalization in the Kangaroo Neonatal Intermediate Care Unit (UCINCa), the second stage of the Kangaroo Care (KC), on the development of oral feeding skills in preterm neonates. Methods an analytical observational study of the prospective longitudinal type, carried out in a public hospital in Southern Brazil, where infants were accompanied until hospital discharge. The sample consisted of 20 preterm neonates hospitalized at the UCINCa and 26 preterm neonates at the Conventional Neonatal Intermediate Care Unit (UCINCo), that were periodically evaluated through the levels of oral skill, in a bottle, according to the criteria of proficiency and milk transfer rate. The outcomes considered were a progression of the oral skill level, days of transition to obtain the full oral route, and days of hospital stay. Results the duration of transition to exclusive oral feeding was shorter for preterm neonates at the UCINCa (4.5 vs. 10 days) relative to those at the UCINCo (p = 0.041). By the third assessment, all preterm neonates at the UCINCa had reached level 4, while participants at the UCINCo only achieved this level of performance on the fifth assessment. The average number of days of hospitalization was four days shorter in UCINCa participants (p=0.098). Conclusion the admission to the UCINCa had been a further acceleration in the maturation of oral skills, which allowed for a faster transition to exclusive oral feeding as compared to neonates admitted in UCINCo.

RESUMO Objetivo verificar os efeitos da internação em Unidade de Cuidado Intermediário Neonatal Canguru (UCINCa), segunda etapa do Método Canguru, no desenvolvimento das habilidades de alimentação oral em recém nascidos pré-termo (RNPT). Método estudo observacional analítico do tipo longitudinal prospectivo, realizado em um hospital público no Sul do Brasil, com acompanhamento até a alta hospitalar. A amostra foi constituída por 20 RNPT internados na UCINCa e 26 RNPT na Unidade de Cuidado Intermediário Neonatal Convencional (UCINCo), que foram avaliados periodicamente por meio dos níveis de habilidade oral, em mamadeira, conforme os critérios de proficiência e taxa de transferência de leite. Os desfechos considerados foram progressão do nível de habilidade oral, dias de transição para obtenção da via oral plena, dias de internação hospitalar. Resultados o tempo de transição para obtenção da via oral exclusiva foi menor nos RNPT da UCINCa (4,5 dias versus 10 dias) quando comparados aos da UCINCo (p=0,041). Na terceira avaliação de habilidade oral, a totalidade de RNPT da UCINCa atingiu o nível 4, já os participantes da UCINCo somente alcançaram esse nível na quinta avaliação. A mediana dos dias de internação foi de quatro dias menor nos participantes da UCINCa (p=0,098). Conclusão a internação na UCINCa favoreceu uma aceleração no processo de maturação das habilidades orais, influenciando positivamente na transição alimentar oral do RNPT, na comparação com os RNPT internados na UCINCo.

RESUMO Objetivo Verificar a influência de um estímulo gustativo na pressão de sucção, durante a sucção não nutritiva (SNN), em recém-nascidos a termo, saudáveis e com peso adequado à idade gestacional. Método Estudo quase experimental do tipo ensaio clínico não randomizado com uma amostra de conveniência de 60 recém-nascidos (RN), 30 alocados no grupo estudo (GE) e 30 no grupo controle (GC). Os RN foram avaliados quanto à pressão de sucção, durante a SNN em chupeta. Para o GE foi adicionado estímulo gustativo à chupeta, umedecida com o colostro. O GC não recebeu nenhum estímulo, além da própria chupeta. As pressões média, mínima e máxima foram medidas com o equipamento S-Flex®. Resultados O GE apresentou pressão média e máxima de sucção significativamente maiores do que o GC. Ainda, houve diferença estatisticamente significativa, entre os grupos, para a 2ª medida da pressão média de sucção. Conclusão Os resultados demostraram que os RN do GE apresentaram pressões de sucção, média e máxima, significativamente maiores, quando comparados ao GC. A utilização de um estímulo gustativo associado à SNN modificou a pressão de sucção e parece potencializar as habilidades orais.

Abstract Purpose To verify the influence of a taste stimulus on the suction pressure, during the non-nutritive sucking (SNN), in newborns, healthy and with weight appropriate to the gestational age. Methods Quasi-experimental study of the non-randomized clinical trial type with a convenience sample of 60 newborns (NB), 30 allocated in the study group (EG) and 30 in the control group (CG). The NB were evaluated for sucking pressure during the SNN in a pacifier. For the EG, a gustatory stimulus was added to the pacifier, moistened with colostrum. The CG did not receive any stimulus, other than the pacifier itself. The average, minimum and maximum pressures were measured with the equipment S-Flex®. Results The SG presented mean and maximum sucking pressure significantly higher than the CG. In addition, there was a statistically significant difference between the groups for the second measurement of mean sucking pressure. Conclusion The results showed that the NB of the SG presented sucking pressures, average and maximum, significantly higher, when compared to the CG. The use of a taste stimulus associated with SNN modified the sucking pressure and seems to enhance oral skills.

RESUMO Objetivo Verificar a influência do Método Canguru sobre as taxas de aleitamento materno em recém-nascidos pré-termo, tanto na alta hospitalar, como ao longo do primeiro ano de vida, assim como sua relação com o momento de introdução da alimentação complementar e a qualidade da dieta. Métodos Estudo longitudinal do tipo observacional, prospectivo e analítico em um hospital público no Sul do Brasil. Participaram do estudo 46 recém-nascidos pré-termo, que foram internados em Unidade de Cuidado Intermediário Neonatal Canguru ou Unidade de Cuidado Intermediário Neonatal Convencional. Resultados A frequência de aleitamento materno exclusivo na alta hospitalar foi mais elevada nos recém-nascidos pré-termo da Unidade de Cuidado Intermediário Neonatal Canguru (p<0,001), sendo certo que, aos quatro meses de idade corrigida, 35% deles continuavam em aleitamento materno exclusivo (p=0,029), em comparação com os bebês da Unidade de Cuidado Intermediário Neonatal Convencional. Já aos seis e 12 meses, não foram encontradas diferenças nas taxas de aleitamento materno entre os grupos participantes. Aos quatro meses de idade corrigida, aproximadamente 45% dos lactentes de ambos os grupos já haviam iniciado a alimentação complementar. Aos 12 meses de idade corrigida, o consumo de alimentos ultraprocessados foi de 38,9% nos lactentes da Unidade de Cuidado Intermediário Neonatal Canguru e de 70% nos da Unidade de Cuidado Intermediário Neonatal Convencional (p=0,054), não sendo encontradas diferenças significativas. Conclusão O Método Canguru favoreceu o aleitamento materno exclusivo na alta hospitalar e aos quatro meses de idade corrigida. Já para a introdução precoce da alimentação complementar, o Método Canguru não se mostrou protetor, sendo encontrado um elevado percentual de consumo de alimentos processados pelos lactentes.

ABSTRACT Objective To assess the influence of Kangaroo Mother Care on breastfeeding rates in preterm neonates, both at hospital discharge and throughout the first year of life, as well as its relation with the time of introduction of early complementary feeding and diet quality. Methods Observational, prospective and analytical longitudinal study conducted in a public hospital in southern Brazil. The study included 46 preterm neonates, who were admitted to a Kangaroo Neonatal Intermediate Care Unit or Conventional Neonatal Intermediate Care Unit. Results The frequency of exclusive breastfeeding at hospital discharge was higher in preterm neonates of Kangaroo Neonatal Intermediate Care Unit (p<0.001), and at four months of corrected age, 35% of them continued on exclusive breastfeeding (p=0.029), as compared to infants in Conventional Neonatal Intermediate Care Unit. However, at six and 12 months, no difference was found in breastfeeding rates between the participating groups. At four months of corrected age, approximately 45% of the breastfeeding infants in both groups were already on complementary feeding. At 12 months of corrected age, consumption of ultra-processed foods was 38.9% in infants from the Kangaroo Neonatal Intermediate Care Unit and 70% in infants from the Conventional Neonatal Intermediate Care Unit (p=0.054), and no significant differences were found. Conclusion The Kangaroo Mother Care fostered exclusive breastfeeding at hospital discharge and at 4 months of corrected age. On the other hand, with regard to the early introduction of complementary feeding, Kangaroo Mother Care was not protective, and a high rate of consumption of processed foods by infants was found.

Resumo Introdução Diferentes fatores afetam o desenvolvimento motor no primeiro ano de vida, tendo sido pouco estudada a interferência da variação sazonal. Objetivo Investigar a influência da sazonalidade e de fatores de risco e proteção para o desenvolvimento motor de lactentes nascidos a termo, aos 7 e 10 meses de idade. Método Foram incluídos no estudo lactentes a termo (N=174) que vivenciaram os marcos de desenvolvimento referentes ao 2º e 3º trimestres no período de verão ou de inverno. Prontuários, entrevistas, e a Alberta Infant Motor Scale foram utilizados. Resultados A aquisição do sentar-se e engatinhar foi mais tardia nos grupos pós-inverno (p < 0,001). Quanto aos grupos de risco e sem risco para o desenvolvimento motor, observou-se, aos 7 meses, menor prevalência de planejamento da gravidez (p=0,015), menor tempo de aleitamento materno exclusivo (p=0,004) e de amamentação (p=0,012) no grupo com risco. Aos 10 meses, os lactentes do grupo de risco se caracterizaram pela menor idade gestacional (p=0,040), por serem filhos de mães mais velhas (p=0,020), maior número de irmãos (p=0,002), níveis mais elevados de pobreza (p=0,002) e maiores restrições de movimento (p=0,000). A regressão logística evidenciou que, aos 7 meses, o aleitamento materno foi a variável associada ao desenvolvimento motor, enquanto aos 10 meses, o número de filhos, a pobreza e a restrição de movimento do lactente explicaram a variação no desenvolvimento motor. Conclusão O clima frio não se mostrou um determinante isolado para o risco de atraso motor, variáveis ambientais foram mais influentes no modelo.

ABSTRACT Introduction Different factors affect motor development in the first year of life; the interference of seasonal variation lacks further investigation. Objective To investigate the influence of seasonality and the protective and risk factors on the motor development of full-term infants, at 7 months and 10 months of age. Method This study included full-term infants (N=174) who experienced developmental milestones to the 2nd and 3td quarters in summer or winter. Medical records, interviews, and the Alberta Infant Motor Scale were used. Results The acquisition of setting and crawling was later in the post-winter groups (p<0,001). Regarding groups at risk and without risk of motor delays, a low prevalence of pregnancy planning (p=0.015), a short duration of breastfeeding (p=0.004), and breastfeeding time (p=0.012) was found in the risk group at 7 months. At 10 months, children in the risk group had shorter gestational age (p=0.040), were children of older mothers (p=0.020), had more siblings (p=0.002), higher levels of poverty (p=0.002), and more restrictions of movement (p=0.000). Logistic regression showed that, at 7 months of age, breastfeeding was the variable associated with motor development, while at 10 months, the number of children, poverty, and infant movement restriction were significantly explained the variation in motor development. Conclusion The cold climate was not an isolated determinant for the risk of delayed motor development; environmental variables were more relevant in the model.

Resumo Objetivo Aproximadamente 85% dos partos vaginais cursam ou com lacerações perineais e/ou com episiotomia. Este estudo objetivou determinar a incidência de lacerações e episiotomias das parturientes de 2018 de uma maternidade pública de risco habitual, no sul do Brasil, bem como determinar os fatores de risco e proteção para tais eventos. Métodos Estudo transversal retrospectivo, no qual os dados foram obtidos dos prontuários e analisados no programa Stata. Realizaram-se regressões logísticas uni e multivariada. Foram considerados como significantes valores de p<0,05. Resultados Em 2018, aconteceram 525 partos vaginais, sendo 27,8% assistidos por médicos obstetras, 70,7%, por enfermeiros obstetras, e 1,5% evoluíram sem assistência. Ao todo, 55,2% das parturientes apresentaram algum grau de laceração. O profissional que assistiu ao parto foi uma variável que demonstrou significância: um maior número de lacerações de primeiro e segundo graus, bem como casos de maior gravidade, ocorreram em partos assistidos por enfermeiros (razão de probabilidades [RP]: 2,95; intervalo de confiança de 95% [IC 95%]: 1,74 a 5,03). Posições ao nascimento que não permitiam técnicas de proteção perineal (período expulsivo na técnica “sem mãos” [hands off, em inglês]), quando analisadas isoladamente, determinaram o risco; contudo, no modelo final de regressão, essa relação não se confirmou. Apesar de relatada na literatura, não houve associação entre a ocorrência de laceração e a idade, a cor da pele, ou o peso de nascimento. Em 24% dos partos, uma episiotomia foi realizada, tendo os médicos executado 63,5% delas. Conclusão Partos assistidos por enfermeiros resultaram em um maior risco de lacerações perineais, de variados graus. Por sua vez, os assistidos por médicos apresentaram maior ocorrência de episiotomia.

Abstract Objective In around 85% of vaginal births, the parturients undergo perineal lacerations and/or episiotomy. The present study aimed to determine the incidence of lacerations and episiotomies among parturients in 2018 in a habitual-risk public maternity hospital in southern Brazil, and to determine the risk and protective factors for such events. Methodology A retrospective cross-sectional study. Data were obtained from medical records and analyzed using the Stata software. Univariate and multivariate logistic regressions were performed. Values of p<0.05 were considered significant. Results In 2018, there were 525 vaginal births, 27.8% of which were attended by obstetricians, 70.7% by obstetric nurses, and 1.5% evolved without assistance. Overall, 55.2% of the parturients had some degree of laceration. The professional who attended the birth was a significant variable: a greater number of first- and second-degree lacerations, as well as more severe cases, occurred in births attended by nurses (odds ratio [OR]: 2,95; 95% confidence interval [95%CI]: 1,74 to 5,03). Positions at birth that did not enable perineal protection techniques (expulsive period with the “hands-off” method), when analyzed in isolation, determined the risk; however, in the final regression model, this relationship was not confirmed. Although reported in the literature, there were no associations between the occurrence of laceration and age, skin color, or birth weight. In 24% of the births, episiotomy was performed, and doctors performed 63.5% of them. Conclusion Births attended by nurses resulted in an increased risk of perineal lacerations, of varying degrees. In turn, those assisted by physicians had a higher occurrence of episiotomy.

RESUMO Objetivo determinar a confiabilidade do equipamento S-FLEX® para verificar pressão de sucção não nutritiva em recém-nascidos a termo. Métodos estudo transversal e quantitativo, com 30 recém-nascidos a termo, saudáveis, entre o primeiro e terceiro dia de vida, internados em um hospital terciário. Foram realizadas duas avaliações em cada recém-nascido, por dois examinadores distintos, com intervalo de quatro horas. Foram realizadas três determinações sucessivas da pressão máxima e a média de sucção não nutritiva, por meio do equipamento S-FLEX®. Para determinar a confiabilidade, avaliou-se a repetibilidade e a reprodutibilidade das medidas. Análise de variância e coeficiente de correlação intraclasse foram utilizados para análise dos resultados. Resultados a pressão máxima de sucção, na 1ª avaliação, foi de 10,44 (± 2,25), 10,12 (± 2,25) e 9,73 (± 2,61) mmHg, (p = 0,120), respectivamente para a 1ª, 2ª e 3ª medidas. Na 2ª avaliação, foi de 10,10 (± 2,35), 9,98 (± 2,02) e 10,37 (± 2,29), (p = 0,404). A pressão média de sucção na 1ª avaliação foi de 3,64 (± 1,04), 3,59 (± 1,04) e 3,56 (± 1,03), (p = 0,852) e, na 2ª avaliação, foi 3,59 (± 1,04), 3,51 (± 0,92) e 3,63 (± 1,04), (p = 0,721). Para a pressão máxima de sucção, o coeficiente de correlação foi de 0,708 (p = 0,001); para a pressão média foi de 0,828 (p<0,001), evidenciando boa e excelente correlação, respectivamente. Conclusão o S-FLEX® mostrou-se um equipamento confiável para determinar a pressão de sucção não nutritiva em recém-nascidos a termo, saudáveis.

ABSTRACT Purpose to establish the reliability of the S-FLEX® to measure non-nutritive sucking pressure in full-term newborns. Methods a cross-sectional and quantitative study was conducted in a tertiary hospital, on 30 one- to three-days-old infants born healthy and at term. Each newborn was evaluated twice, each time by a different examiner, at an interval of 4 hours. In each assessment, maximum and mean non-nutritive sucking pressure were measured three times consecutively. The reliability of the instrument was investigated by calculating the repeatability and reproducibility of these measurements. Repeatability was determined by analyzing the consecutive measures taken by each examiner using repeated-measures analysis of variance (ANOVA). Results the maximum sucking pressure was found to be 10.44 (± 2.25), 10.12 (± 2.25) and 9.73 (± 2.61) mmHg, (p = 0.120), in the first, second and third consecutive measurements, respectively. In the second evaluation, the values were 10.10 (± 2.35), 9.98 (± 2.02) and 10.37 (± 2.29) mmHg, (p = 0.404). The measurements of mean sucking pressure taken by examiner A in the first evaluation were 3.64 (± 1.04), 3.59 (± 1.04) and 3.56 (± 1.03) mmHg, (p = 0.852), respectively, and, in the second evaluation, 3.59 (± 1.04), 3.51 (± 0.92) and 3.63 (± 1.04) mmHg, (p = 0.721). The correlation coefficients between measurements of maximum and mean sucking pressure were 0.708 (p = 0.001) and 0.828 (p<0.001), which are indicative of good and excellent correlations, respectively. Conclusion The S-FLEX® can provide reliable measurements of maximum and mean non-nutritive sucking pressure in healthy full-term newborns.

RESUMO Objetivo: investigar o Schedule Oral Motor Assessment (SOMA) para utilização com recém-nascidos pré-termo, e comparar seus resultados com o Preterm Oral Feeding Readiness Scale (POFRAS) na liberação da alimentação por via oral. Métodos: trabalho de caráter transversal e quantitativo. Estudo composto por uma amostra de 45 recém-nascidos pré-termo, saudáveis e clinicamente estáveis, avaliados no momento da liberação da alimentação por via oral. Neste momento foi realizada a avaliação do POFRAS e do SOMA. Para a análise dos dados foi utilizado o software Stata 10.0. Resultados: todos 10 recém-nascidos pré-termo que apresentaram prontidão para alimentação por via oral tinham função motora oral normal, e, os 16 que apresentaram disfunção motora oral, esses não apresentavam prontidão para a mamada (p<0,05). O tempo de transição de alimentação por via oral foi de 13,5 (±8,1) dias nos recém-nascidos pré-termo, com melhores resultados nas duas avaliações, e de 17,7 (±10,9) dias aos que não apresentaram prontidão para a alimentação oral e tiveram disfunção motora oral no momento da liberação da via oral, resultando em um dado com relevância clínica mesmo não apresentando significância (p>0,05). Conclusão: estes resultados sugerem que o SOMA pode ser um método complementar para avaliação da função oral-motora no momento da liberação da via oral de recém-nascido pré-termo.

ABSTRACT Objective: this study aimed at investigating the Schedule Oral Motor Assessment (SOMA) tool to be used with preterm infants and to compare its results with the Preterm Oral Feeding Readiness Assessment Scale (POFRAS) to start oral feeding. Methods: a cross-sectional and quantitative study, consisting in a sample of 45 healthy and clinically stable preterm infants, assessed at their first oral feeding with two tools: the Schedule Oral Motor Assessment and Preterm Oral Feeding Readiness Assessment Scale. Stata 10.0 software was used for data analysis. Results: 10 preterm infants with readiness for oral feeding showed normal oral motor function, and 16, presented with oral motor dysfunction, did not show readiness for feeding (p <0.05). The time of transition for full oral feeding was 13.5 (± 8.1) days for preterm infants with better results in both assessment tools, and 17.7 (± 10.9) days for those who did not show readiness for oral feeding and had oral motor dysfunction to initiate oral feeding, resulting in a given clinical relevance, even showing no significance (p> 0.05). Conclusion: these results suggest that the Schedule Oral Motor Assessment can be an adjunctive method for evaluation of the oral motor function at the first oral feeding in preterm infants.

RESUMO Objetivo Correlacionar a saturação periférica de oxigênio com a idade gestacional e com o nível de habilidade de alimentação oral, na introdução da alimentação oral, de recém-nascidos pré-termo. Método Estudo transversal e quantitativo no qual participaram 169 recém-nascidos pré-termo, clinicamente estáveis. A verificação da saturação de oxigênio foi realizada antes e após a introdução da primeira alimentação por via oral. Os recém-nascidos pré-termo foram estratificados em três grupos baseados na idade gestacional ao nascimento: 26-29, 30-33 e 34-36 semanas. A habilidade para alimentação oral foi classificada em quatro níveis com base na habilidade oral e resistência apresentada pelo recém-nascido pré-termo. Resultados Não houve diferença na saturação periférica de oxigênio entre os estratos da idade gestacional e entre os níveis de habilidade oral. Foram observadas diferenças nos grupos com idade gestacional de 30-33 semanas (p=0,04) e 34-36 semanas (p=0,02) e no nível I de habilidade de alimentação oral (p=0,04) quando comparada com a saturação inicial e final. Correlações significativas (p<0,001) foram encontradas entre a idade gestacional e níveis de habilidade oral (r=0,38); no Grupo A, entre a idade gestacional e a saturação inicial (r=0,83); e, no Grupo B, entre o nível de habilidade oral com a saturação inicial, e com a idade gestacional. Conclusão A saturação de oxigênio apresentou correlação quando comparada com a idade gestacional e com o nível de habilidade oral.

ABSTRACT Purpose To correlate the peripheral oxygen saturation with gestational age and the level of oral feeding skills in the introduction of oral feeding in preterm infants. Methods This is a cross-sectional, quantitative study whose sample was composed of 169 clinically stable preterm infants. Peripheral oxygen saturation was assessed before and after introduction of oral feeding. The preterm infants were stratified into three groups based on their gestational age at birth: 26-29, 30-33, and 34-36 weeks. The preterm infants were classified into four levels according to their oral feeding skill and resistance. Results No differences in oxygen saturation were observed between the strata of gestational age and between the levels of oral feeding skill. Differences were observed in the groups of preterm infants aged 30-33 weeks (p=0.04) and 34-36 weeks (p=0.02) and on the level I of oral feeding skills (p=0.04) when oxygen saturation was compared at pre- and post-first oral feeding. Significant correlations (p<0.001) were found between gestational age and the levels of oral feeding skills (r=0.38); in Group A, between gestational age and oxygen saturation before the first oral feeding (r=0.83); in Group B, between the level of oral feeding skill and oxygen saturation before the first oral feeding (r=0.26) and between level of oral feeding skill and gestational age (r=0.26). Conclusion Correlation was found for peripheral oxygen saturation when compared with gestational age and with the level of oral feeding skills.

RESUMO Objetivo Correlacionar os parâmetros cardiorrespiratórios com a idade gestacional e com o nível de habilidade de alimentação oral na introdução da alimentação oral de recém-nascidos pré-termo. Método Participaram desta pesquisa 37 recém-nascidos pré-termo, clinicamente estáveis. A verificação das frequências cardíaca e respiratória foi realizada antes e após a introdução da primeira alimentação por via oral. Os recém-nascidos foram distribuídos em três estratos conforme a idade gestacional do nascimento. A habilidade oral foi classificada em quatro níveis: I - baixa habilidade oral e baixa resistência para alimentação; II - baixa habilidade oral e alta resistência para alimentação; III - alta habilidade oral e baixa resistência para alimentação; IV - alta habilidade oral e alta resistência para alimentação. Resultados Não houve diferença na frequência cardíaca e respiratória entre os estratos da idade gestacional e entre os níveis de habilidade oral. Ao comparar a frequência cardíaca e respiratória inicial e final dentro de cada nível de habilidade oral e estrato da idade gestacional, observou-se diferença entre a frequência cardíaca nos estratos das idades gestacionais entre 30 e 33 semanas e entre 34 e 36 semanas, bem como nos níveis de habilidade oral I, II e IV; com relação à comparação entre a frequência respiratória inicial e final, foi encontrada diferença no nível I de habilidade de alimentação oral. Conclusão As frequências cardíaca e respiratória sofreram alterações quando comparadas com dados pré e pós-prandial na primeira alimentação por via oral, assim como quando comparadas sobre aspectos da idade gestacional e nível de habilidade oral.

ABSTRACT Purpose To correlate cardiorespiratory parameters with gestational age and level of oral feeding skills in the first oral feeding in preterm infants. Methods Study participants were 37 clinically stable preterm infants. Cardiorespiratory rate was assessed before and after introduction of oral feeding. The newborns were divided into three strata according to gestational age at birth. Oral skill was classified into four levels: I - low oral skill and low resistance to feeding; II - low oral skill and high resistance to feeding; III - high oral skill and low resistance to feeding; IV - high oral skill and high resistance to feeding. Results No difference was observed in heart and respiratory rate between the strata of gestational age at birth and between the levels of oral skill. Comparison between pre- and post-cardiorespiratory rates within each level of oral skill and stratum of gestational age showed difference between heart rate in the strata of gestational ages of 30 to 33 weeks and of 34 to 36 weeks, as well as between oral skill of levels I, II, and IV. With regard to the comparison between pre- and post- respiratory rates, difference was found in the oral skill of level I. Conclusion Differences were observed between pre- and post-prandial cardiorespiratory rates regarding the first oral feeding, as well as between strata of gestational age at birth and levels of oral feeding skills.

RESUMO Objetivo avaliar a acurácia do Preterm Oral Feeding Readiness Scale - POFRAS para iniciar a alimentação oral de recém-nascidos pré-termo e verificar a concordância entre este instrumento e o instrumento de avaliação do Nível de Habilidade Oral. Métodos Foram avaliados 82 recém-nascidos pré-termo quanto à prontidão para o início da alimentação oral através do POFRAS e da avaliação do Nível de Habilidade Oral, durante a primeira alimentação oral. A acurácia do POFRAS foi estimada em relação à variável proficiência, por meio da Curva ROC (Receiver Operating Characteristic Curve). Para a análise da concordância entre os instrumentos, foi utilizado o coeficiente Kappa. Resultados A acurácia global do POFRAS foi de 71,29%. O ponto de corte 29 foi o que apresentou melhor equilíbrio entre sensibilidade e especificidade. O coeficiente Kappa mostrou fraca concordância entre os instrumentos na identificação dos RN aptos e inaptos a mamar por via oral (k=0.281). Conclusão a acurácia do POFRAS para o início da alimentação oral, estimada por meio da variável proficiência, foi semelhante à obtida com a técnica de translactação. Observou-se fraca concordância entre os instrumentos avaliados. Sugere-se, portanto, que estes instrumentos de avaliação sejam usados de forma complementar na prática clínica, uma vez que ambos apresentam aspectos importantes do comportamento alimentar do prematuro, que ao serem analisados conjuntamente permitirão orientar a conduta necessária para propiciar uma transição alimentar mais breve e eficaz para essa população.

ABSTRACT Purpose To assess the accuracy of the Preterm Oral Feeding Readiness Scale (POFRAS) on the beginning of oral feeding in preterm infants and to verify the concordance between this tool and the Oral Feeding Skill Level. Methods 82 preterm infants were assessed by POFRAS regarding their readiness to initiate oral feeding and by the oral feeding skill level evaluation during the first oral feeding. POFRAS's accuracy was estimated regarding proficiency by a Receiver Operating Characteristics (ROC) curve. The concordance between the tools was obtained by analysis of the Kappa coefficient. Results POFRAS's global accuracy was of 71.29%. The cut-off value of 29 was the one that presented most optimization of the sensitivity based on specificity. The Kappa coefficient has shown a weak concordance between the instruments to identify infants able and unable to oral feeding (k=0.281). Conclusion POFRAS's accuracy to initiate oral feeding considering the proficiency was similar to that obtained with the technique of translactation. We observed a weak concordance between the instruments. We suggest that, in clinical practice, both instruments should be used in a complementary manner, since both present important aspects of the preterm feeding behavior that together will better guide the necessary conduct to provide an effective and quick transition to full oral feeding in this population.

RESUMO Objetivo Verificar se a presença de prontidão para iniciar a alimentação oral, obtida por meio do Preterm Oral Feeding Readiness Scale (POFRAS) se relaciona com o desempenho alimentar na primeira oferta oral de leite, com o tempo de transição da sonda para a via oral plena e o tempo de internação hospitalar. Métodos Estudo longitudinal, que envolveu 65 recém-nascidos pré-termo. Foi realizada a avaliação da prontidão para início da alimentação oral, por meio do POFRAS, constituindo-se dois grupos: com e sem prontidão para iniciar a alimentação por via oral, em dois estratos de idade gestacional ao nascer: de 28 a 33 semanas e de 34 a 36 semanas. Em cada estrato, os grupos foram comparados entre si, quanto ao desempenho alimentar na primeira oferta oral de leite (por meio das variáveis proficiência, taxa de transferência e desempenho alimentar oral) e quanto ao tempo de transição da sonda para a via oral plena e o tempo de internação hospitalar. O teste t-Student ou o teste de Wilcoxon foram utilizados para comparar as variáveis contínuas e o teste exato de Fisher, para as variáveis categóricas. Resultados A prontidão para iniciar a alimentação por via oral se relacionou com melhores resultados de proficiência e taxa de transferência na primeira alimentação, contribuindo para a transição mais rápida da sonda para a via oral plena, nas crianças nascidas com idade gestacional entre 28 a 33 semanas. Conclusão a avaliação por meio do POFRAS permite estabelecer um prognóstico da alimentação oral em recém-nascidos pré-termo, menores de 34 semanas. No entanto, não exclui a necessidade de avaliação da biomecânica da deglutição.

ABSTRACT Purpose To verify whether readiness for oral feeding as evaluated by the Preterm Oral Feeding Readiness Scale (POFRAS) is associated with performance on the first oral feeding; length of transition from tube to full oral feeding, and duration of hospitalization. Methods This was a longitudinal study involving 65 preterm infants. Participants were divided into groups according to readiness for oral feeding as determined by the POFRAS, and gestational age (29-33 weeks and 34-36 weeks). Performance on the first oral feeding (proficiency, transfer rate and overall transfer), number of days from introduction to full independent oral feeding, and length of hospital stay were compared between groups. Continuous variables were compared using Student’s t-test or Wilcoxon’s signed-rank test. Categorical variables were compared using Fisher’s exact test. Results Readiness for oral feeding was associated with higher proficiency and transfer rates on the first oral feeding, resulting in a shorter transition from tube to independent oral feeding in preterm infants with 29 to 33 weeks of gestational age at birth. Conclusion The POFRAS contributed to the prognosis of oral feeding outcomes in preterm neonates with less than 34 weeks of gestational age at birth. However, its use does not eliminate the need for an assessment of swallowing mechanisms.

Resumo Objetivo: Compreender a vivência da mãe de recém-nascido pré-termo internado na Unidade de Terapia Intensiva Neonatal diante da manutenção da lactação. Métodos: Estudo de abordagem fenomenológica, de natureza qualitativa, pautado no referencial teórico-filosófico-metodológico de Martin Heidegger. Desenvolvido com sete mães, mediante entrevista fenomenológica, no período de dezembro de 2010 a maio de 2011. Resultados: A mãe se ocupa com a manutenção da lactação, teme pela saúde do filho e pelo leite secar. Mostra-se como ser de relação quando coloca o filho no peito. Ocupa-se com a dupla rotina do lar e da hospitalização. Conclusão: Amamentar o filho no peito, na unidade neonatal, potencializa o vínculo e possibilita que a mãe saia do modo da ocupação mantendo a lactação para estabelecer sua relação com o filho.

Abstract Objective: To comprehend the experiences of mothers of preterm newborns admitted to the Neonatal Intensive Care Unit regarding the maintenance of the lactation. Methods: This is a phenomenological approach study, of qualitative nature, grounded on the theoretical-philosophical-methodological benchmark of Martin Heidegger. It was developed with seven mothers, through phenomenological interviews, in the period from December 2010 to May 2011. Results: Mothers deal with the maintenance of lactation and fear for the health of their babies and for the end of milk reserves. They present themselves as 'the being of the relationship' when she places the child on the breast. She gets involved with the dual routine that includes hospital and home duties. Conclusion: Breastfeeding the infant on the chest in the neonatal unit enhances the bond and enables the mother to exit the mode of occupation with the maintenance of the lactation to establish her relationship with the child.

Resumen Objetivo: Conocer la experiencia de la madre de recién nacidos prematuro ingresados en la Unidad de Cuidados Intensivos Neonatal delante de la manutención de la lactancia. Métodos: Estudio cualitativo fenomenológico de naturaleza cualitativa, con base en el marco teórico-filosófico y metodológico de Martin Heidegger. Desarrollado con siete madres, a través de encuestas fenomenológicas, de diciembre 2010 a mayo 2011. Resultados: La madre se preocupa con la manutención de la lactancia, teme por la salud del niño y por la falta de la leche. Evidencia la relación del niño con su pecho. Tiene una doble rutina el hogar y en el hospital. Conclusión: Amamantar el niño en el pecho en la unidad neonatal fortalece el vínculo y permite que la madre salga del modo de ocupación con la manutención de la lactación para establecer una relación con su hijo.

OBJETIVO: Avaliar a influência da habilidade motora oral do prematuro sobre seu desempenho alimentar oral e crescimento, durante o período de internação neonatal.MÉTODOS: Foram avaliados 51 recém-nascidos (RNs) internados na Unidade de Tratamento Intensivo (UTI) Neonatal de um hospital do Sul do Brasil, entre julho 2012 e março 2013. A avaliação da habilidade de alimentação oral, segundo Lau e Smith, foi realizada após prescrição para início da alimentação por via oral (VO). O desempenho alimentar oral foi avaliado por meio das variáveis dias de transição da sonda para VO plena e tempo de internação. O crescimento foi avaliado por peso, comprimento e perímetro cefálico (PC), utilizando as curvas de Fenton, no nascimento, liberação VO, VO plena e alta hospitalar.RESULTADOS: Ao nascer, 71% dos prematuros eram adequados para idade gestacional, a maioria era do sexo masculino (53%), tendo média de 33,6 (±1,5) semanas de idade gestacional. A idade gestacional nos momentos avaliados não influenciou na habilidade oral do pré-termo, não diferindo entre os níveis. O tempo de transição da sonda para VO plena e o período de internação hospitalar foram menores quanto maior o nível de habilidade oral. Ao nascer, houve uma tendência a baixo peso e baixa habilidade oral. Na liberação da VO, as crianças do nível IV apresentaram maior peso.CONCLUSÃO: O nível de habilidade oral do prematuro interferiu positivamente no tempo de transição alimentar da sonda para VO plena e permanência hospitalar. O crescimento, representado pelo ganho de peso, não sofreu influência do nível de habilidade oral.

OBJECTIVE: To evaluate the influence of oral motor skills of premature infants on their oral feeding performance and growth, during neonatal hospitalization.METHODS: Fifty-one newborns hospitalized in the neonatal intensive care unit of a hospital in Southern Brazil, between July 2012 and March 2013, were evaluated. The evaluation of oral feeding skills, according to Lau and Smith, was applied after prescription for starting oral feeding. The oral feeding performance was analyzed using the following variables: days taken to start independent oral feeding and hospital discharge. Growth was measured by weight, length, and head circumference, using the curves of Fenton, at birth, first and independent oral feeding, and hospital discharge.RESULTS: At birth, 71% preterm infants were proper for gestational age, most of them were males (53%), with average of 33.6 (±1.5) weeks of gestational age. The gestational age in the assessment did not influence the oral feeding performance of the premature infant and did not differ between levels. Time of transition from tube feeding to oral feeding and hospital stay was shorter when the oral skills were higher. At birth, there was a tendency of low weight and low oral feeding performance. Level IV premature infants in the release of oral feeding presented higher weights.CONCLUSION: The level of oral skills of the premature infant interfered positively on time of feeding transition from tube to independent oral feeding and hospital stay. Growth, represented by weight gain, was not affected by the level of oral skill.

Objetivo Verificar a influência do estado comportamental na sucção não nutritiva, na sucção nutritiva e na performance alimentar de recém-nascidos pré-termo, no momento da liberação da via oral. Métodos Participaram 32 recém-nascidos pré-termo, clinicamente estáveis, internados em uma unidade de terapia intensiva neonatal. Foram realizadas, por um fonoaudiólogo, as avaliações do estado comportamental, padrão postural, reflexos orais, características dos órgãos fonoarticulatórios, sucção não nutritiva e nutritiva, número de suções por bloco de sucção, taxa de transferência e desempenho alimentar, após a liberação médica para início da alimentação por via oral. Resultados Não foi possível verificar associação do estado comportamental com a sucção não nutritiva. Na sucção nutritiva, verificou-se influência do estado comportamental sobre o número de sucções nos três primeiros blocos, ou seja, o estado de alerta, ao iniciar a mamada, possibilitou aos recém-nascidos apresentarem maior número de sucções, quando comparados aos recém-nascidos em estado de sono. Isto se refletiu na performance alimentar, por meio da maior taxa de transferência e melhor desempenho, embora não tenha sido observada significância estatística quanto a esses aspectos. Conclusão Os achados mostram que o estado comportamental não influenciou os aspectos de sucção não nutritiva, considerados, entretanto, na sucção nutritiva. O estado de alerta apresentou associação com maior número de sucções por bloco, em relação ao estado de sono.

Purpose To verify the influence of preterm infants’ behavioral states in non-nutritive sucking, nutritive sucking and feeding performance at the moment of oral feeding release. Methods Thirty-two preterm infants participated in the study. They were hospitalized in a Neonatal Intensive Care Unit and presented stable clinical conditions. After medical release for oral feeding, the following aspects of infants were observed by the same speech therapist: behavioral state, postural pattern, oral reflexes, phono-articulatory organs’ characteristics, non-nutritive and nutritive sucking, number of sucks per block, rate of milk transfer and oral feeding performance. Results No relationship was found between the infants’ behavioral states and non-nutritive sucking. In nutritive sucking it was verified the influence of behavioral states on the number of sucks in the three first blocks intervals sucking/pause. This means that at the moment of oral feeding start infants in the alertness state presented a higher number of sucks than those in the sleep state. Consequently an impact on feeding performance was verified, with a higher rate of milk transfer and an increased oral feeding performance. Nonetheless no statistical significance was found in regards to these aspects. Conclusion This research’s findings show that behavioral states did not influence on non-nutritive sucking aspects. But in nutritive sucking, infants in the alertness state presented a higher number of sucks per block than those in the sleep state.
