Impacto do tratamento medicamentoso na voz, fala e deglutição de pacientes com esclerose lateral amiotrófica: revisão sistemática

RESUMO Objetivos Revisar sistematicamente a literatura sobre o impacto do tratamento medicamentoso nas funções de voz, fala e deglutição de indivíduos adultos com esclerose lateral amiotrófica esporádica, mensuradas por meio de escalas e seus respectivos escores, em relação ao grupo placebo. Estratégia de pesquisa A busca foi realizada com base na estratégia PICO (problema/população/paciente; intervenção; comparação/controle; desfecho/outcome). As palavras-chave foram selecionadas a partir de consulta aos Descritores em Ciências da Saúde (DeCS) e ao Medical Subject Headings (MeSH). Dois pesquisadores independentes fizeram busca na American Speech-Language-Hearing Association (ASHA), Cochrane, LILACS, PubMed, Scopus e Web of Science, em inglês, espanhol e português. Critérios de seleção Foram incluídos ensaios clínicos randomizados, realizados em adultos, e excluídos artigos cujos desfechos estavam relacionados à autoavaliação e à qualidade de vida, teses, dissertações, apenas resumos disponíveis, estudos de caso, estudos experimentais, capítulos de livro, enciclopédias e comunicações breves. Os estudos foram avaliados por meio das ferramentas Robins II (Risk Of Bias In Non-randomized Studies II) e GRADE (Grading of Recommendations Assessment, Development and Evaluation). Resultados dos 9824 artigos encontrados, 5 realizaram a intervenção medicamentosa e foram selecionados para análise. Observou-se ausência de estudos voltados para reabilitação das funções bulbares. A qualidade de evidência gerada variou de alto a baixo risco e o nível de evidência, de baixo a muito baixo. Conclusão a maioria dos estudos demonstra que o tratamento medicamentoso atrasa a degeneração das funções bulbares, com relação ao placebo, embora tal achado não tenha sido observado nos escores de escalas que mensuram tais funções. Os estudos apresentam risco de viés de seleção e muito baixa/baixa qualidade metodológica, limitando a confiança nos achados.
ABSTRACT Purpose To carry out a systematic review of the literature on the impact of drug treatment on the voice, speech, and swallowing functions of adult individuals with sporadic ALS, measured through scales and their respective scores, concerning the placebo group. Research strategy The search strategy was created based on the PICO strategy. The keywords were selected from a consultation with the health sciences descriptors – DECS and the medical subject headings – MeSH. Two independent researchers searched ASHA, Cochrane, Lilacs, Pubmed, Scopus and Web of Science, in English, Spanish and Portuguese. Selection criteria Randomized clinical trials, carried out on adults, were included, and articles with outcomes related to selfassessment and quality of life, theses, dissertations, abstracts only , case studies, experimental studies, book chapters, encyclopedia and brief communication were excluded. The studies were evaluated using the Robins II and Grade tool. Results Of the 9824 articles found, 5 were selected for analysis and underwent drug intervention. It is noticed the absence of studies aimed at the rehabilitation of bulb functions. The quality of evidence generated varied from high to low risk and the level of evidence low and very low. Conclusion Most studies show a delay in the degeneration of bulbar functions in relation to placebo, although this finding has not been observed in the scores of scales that measure such functions. Studies are at risk of selection bias and very low/low methodological quality makes the findings questionable.

ABSTRACT. The physicochemical characteristics of the meat from lambs fed diets containing whole or disintegrated cottonseed, associated or not with calcium lignosulfonate (LignoCaSO3), were evaluated. Thirty non-castrated Dorper x Santa Inês lambs, with an average of 24.9 ± 3.6 kg and four months of age were confined for 60 days in collective stalls and distributed in a completely randomized design with six replications. After slaughter, by means of contrasts, the averages of the parameters of the semimembranous and semitendinosus muscles were analyzed. The cottonseed increased cooking loss and ash, and reduced muscle weight, water holding capacity and red intensity. The disintegration of the cottonseed reduced the shear force in diets without LignoCaSO3, increased the protein and the loss by cooking and reduced the pH in the diets with the additive. The luminosity values increased with the disintegration of the cottonseed in diets with and without LignoCaSO3. The addition of LignoCaSO3 increased the weight of the muscle, protein, ash, pH, shear strength and the intensity of red. Moisture, lipids and yellow intensity were not influenced by the diets. Even changing the physical-chemical characteristics, the cottonseed with or without LignoCaSO3 does not change the quality of the meat.

Abstract Our hypothesis was that different sources of unsaturated fatty acids from oilseeds (sunflower, soybeans, and cottonseed) could alter milk yield, fatty acid profile, and the amount of unsaturated fatty acids yielded in milk. It aimed to evaluate the effects of lipid sources on yield and composition and fatty acids in milk of primiparous Girolando cows. Five cows in lactation (35 ± 8 days), with 410 ± 7.15 kg BW were randomly distributed in a 5 x 5 Latin square. Five experimental diets, namely a control diet without an additional source of lipid and four diets with different lipid sources: cottonseed, sunflower seed, whole soybean, or soybean oil, as lipid source, to reach 70 g/kg of ethereal extract dry matter basis were used. Cottonseed and soybean oil addition reduced daily yield by 15 and 22%. Oilseed can alter fatty acid profile in milk. Whole soybean provided greater daily milk and fatty acids yields.

ABSTRACT. The objective of this study was to evaluate the effect of use of agroindustrial co-products in the diet of lambs on the performance and nutritional quality of meat fat. Twenty lambs were distributed into four experimental groups: control diet (C) or diets containing cottonseed (CS), sunflower meal (SFM), and castor cake (CC). During the experimental period, data on dry matter consumption (DMC) and total weight gain (TWG) were collected. After slaughter, samples of Logissimus dorsi (LD) muscle were taken for fatty acids and cholesterol analysis. Diets C and CS provided similar characteristics of fat quality and animal performance. However, SFM and CC diets tend to form isolated groups, with different fat and performance characteristics. The SFM diet results in meat with a higher saturated fatty acids and cholesterol content. The CC diet is related to improved performance characteristics and lower cholesterol content. In conclusion, the inclusion of CC in the experimental diets provided, in the proportions used in this experiment, better characteristics of animal performance and meat fat quality.

RESUMO Objetivo Analisar a frequência combinada de queixas relacionadas à deglutição e voz, antes da tireoidectomia. Métodos Foi realizada uma análise retrospectiva de 51 entrevistas de pacientes encaminhados para tireoidectomia parcial ou total. A partir da mediana da quantidade total de cada grupo de sintomas de deglutição e voz, determinou-se a frequência combinada de queixas relacionadas à deglutição e voz. Investigou-se, ainda, como os pacientes classificaram cada uma das duas funções (excelente, muito boa, boa, razoável ou ruim) e se esta classificação se relacionava com a quantidade de sintomas e com a frequência combinada. Para a análise bivariada, foram utilizados os testes Qui-quadrado de Pearson ou exato de Fisher e o teste não paramétrico de Mann-Whitney. O nível de significância foi de 5%. Resultados A frequência combinada de queixas relacionadas à deglutição e voz antes da tireoidectomia foi de 31,4%. Os sintomas mais citados, relacionados à deglutição, foram pigarro após deglutir (39,3%), esforço para deglutir (37,2%) e engasgo (35,3%) e os relacionados à voz foram garganta seca (72,6%), pigarro na garganta (72,5%) e coceira na garganta (47%). A quantidade de sintomas foi significativamente maior entre os que classificaram a deglutição (3,13 ± 2,21) e a voz (5,91 ± 2,81) de forma negativa. Houve associação entre autoavaliação negativa da voz e a frequência combinada de queixas relacionadas à deglutição e voz (p=0,003). Conclusão A combinação de queixas relacionadas à deglutição e voz antes da tireoidectomia ocorre em um terço dos pacientes e está associada à autoavaliação negativa da voz.
ABSTRACT Purpose To analyze the combined frequency of patient-reported swallowing and voice complaints before thyroidectomy. Methods This is a retrospective analysis of 51 interviews of patients referred for partial or total thyroidectomy. The combined frequency of patient-reported swallowing and voice complaints was determined from the median of the total number of related symptoms in each group. We also investigated how patients rated each function (excellent, very good, good, fair, poor) and whether this rating was related to the number of symptoms and the combined frequency. For bivariate analysis, Pearson’s chi-square test or Fisher’s exact test and the nonparametric Mann-Whitney test were used. The level of significance was 5%. Results The combined frequency of patient-reported swallowing and voice complaints before thyroidectomy was 31.4%. The most commonly swallowing-related symptoms were throat clearing after swallowing (39.3%), swallowing effort (37.2%), and choking (35.3%). The most commonly voice-related symptoms were dry throat (72.6%), throat clearing (72.5%), and itchy throat (47%). The total number of symptoms was significantly higher among those who rated swallowing (3.13 ± 2.21) and voice (5.91 ± 2.81) negatively. Negative self-assessment of the voice was associated with the combined frequency of patient-reported swallowing and voice complaints (p=0.003). Conclusion The combined frequency of patient-reported swallowing and voice complaints before thyroidectomy occurs in one third of the patients and is associated with negative self-assessment of the voice.

RESUMO Objetivo Investigar a acurácia das medidas acústicas, isoladas e combinadas, na discriminação da intensidade do desvio vocal (GG) e da qualidade vocal predominante (QVP) em pacientes disfônicos. Método Participaram 302 pacientes do gênero feminino, com queixa vocal. A partir da vogal /ɛ/ sustentada, foram extraídas as medidas acústicas de média e desvio padrão (DP) da frequência fundamental (F0), o jitter, o shimmer e o Glottal to noise excitation (GNE) e a média dos três primeiros formantes (F1, F2, F3). A avaliação perceptivo-auditiva do GG e QVP foi realizada por três fonoaudiólogos especialistas em voz. Resultados Isoladamente, apenas o GNE obteve desempenho satisfatório na discriminação do GG e da QVP. Houve uma melhora na classificação do GG e QVP com a combinação das medidas acústicas. A média de F0, F2 e GNE (saudável × desvio leve a moderado), DP de F0, F1 e F3 (leve a moderado × desvio moderado), Jitter e GNE (moderado × desvio intenso) foram as melhores combinações para discriminar o GG. As melhores combinações para discriminação da QVP foram média de F0, Shimmer e GNE (saudável × rugosa), F3 e GNE (saudável × soprosa), média de F0, F3 e GNE (rugosa × tensa), média de F0, F1 e GNE (soprosa × tensa). Conclusão De forma isolada, o GNE mostrou-se o único parâmetro acústico capaz de discriminar o GG e a QVP. Houve um ganho no desempenho da classificação com a combinação das medidas acústicas tradicionais e formânticas, tanto para a discriminação do GG como da QVP.
ABSTRACT Purpose Investigate the accuracy of isolated and combined acoustic measurements in the discrimination of voice deviation intensity (GD) and predominant voice quality (PVQ) in patients with dysphonia. Methods A total of 302 female patients with voice complaints participated in the study. The sustained /ɛ/ vowel was used to extract the following acoustic measures: mean and standard deviation (SD) of fundamental frequency (F0), jitter, shimmer, glottal to noise excitation (GNE) ratio and the mean of the first three formants (F1, F2, and F3). Auditory-perceptual evaluation of GD and PVQ was conducted by three speech-language pathologists who were voice specialists. Results In isolation, only GNE provided satisfactory performance when discriminating between GD and PVQ. Improvement in the classification of GD and PVQ was observed when the acoustic measures were combined. Mean F0, F2, and GNE (healthy × mild-to-moderate deviation), the SDs of F0, F1, and F3 (mild-to-moderate × moderate deviation), and mean jitter and GNE (moderate × intense deviation) were the best combinations for discriminating GD. The best combinations for discriminating PVQ were mean F0, shimmer, and GNE (healthy × rough), F3 and GNE (healthy × breathy), mean F 0, F3, and GNE (rough × tense), and mean F0 , F1, and GNE (breathy × tense). Conclusion In isolation, GNE proved to be the only acoustic parameter capable of discriminating between GG and PVQ. There was a gain in classification performance for discrimination of both GD and PVQ when traditional and formant acoustic measurements were combined.

ABSTRACT The objective of this study was to evaluate the effects of supplementing diet of lambs with agroindustrial co-products on the nutritional quality of meat fat and study the variation of fat quality parameters based on the type of muscle. Twenty lambs were distributed into four experimental groups and fed a control diet or diets containing cottonseed, sunflower meal, and castor cake. After 106 experimental days, the animals were slaughtered. Samples of Biceps femoris, Longissimus dorsi, Semimembranosus, and Triceps brachii muscles were taken for fatty acid and cholesterol analyses. Castor cake and sunflower meal diets produced meats with lower levels of n-6 polyunsaturated fatty acids (PUFA) compared with the control diet. Castor cake diet contributed to tissue reduction of n-3 PUFA. Lower conjugated linoleic acid concentration and polyunsaturated:saturated fatty acid (P:S) ratio and higher saturated fatty acids content and Thrombogenicity Index were observed in Longissimus dorsi meat. Of the co-products studied, although castor cake diet reduced cholesterol levels in Biceps femoris muscle, sunflower meal was identified as the preferred choice for lamb feeding, since it reduced n-6 PUFA without, however, decreasing n-3 PUFA. Longissimus dorsi meat had the lowest fat quality indexes. The more oxidative muscles, such as Biceps femoris, tend to generate meats with higher cholesterol concentrations.

RESUMOAs recomendações propostas pela Comissão de Síndrome de Sjögren da Sociedade Brasileira de Reumatologia para tratamento da síndrome de Sjögren foram baseadas em uma revisão sistemática da literatura nas bases de dados Medline (PubMed) e Cochrane até outubro de 2014 e opinião de especialistas na ausência de artigos sobre o assunto. Foram incluídos 131 artigos classificados de acordo com Oxford & Grade. Essas recomendações foram elaboradas com o objetivo de orientar o manejo adequado e facilitar o acesso aos tratamentos para aqueles pacientes com adequada indicação de recebê-los, considerando o contexto socioeconômico brasileiro e os medicamentos disponíveis no país.
ABSTRACTThe recommendations proposed by the Sjögren's Syndrome Committee of the Brazilian Society of Rheumatology for the treatment of Sjögren's syndrome were based on a systematic review of literature in Medline (PubMed) and the Cochrane databases until October 2014 and on expert opinion in the absence of studies on the subject. 131 articles classified according to Oxford & Grade were included. These recommendations were developed in order to guide the management and facilitate the access to treatment for those patients with an appropriate indication, considering the Brazilian socioeconomic context and pharmacological agents available in this country.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Implementation of the "Saúde na Boa" intervention: process evaluation and characteristics of participants and dropouts

Intervenções no ambiente escolar têm sido avaliadas para analisar a qualidade da implementação de suas ações e o perfil dos participantes envolvidos. O objetivo deste estudo foi analisar os resultados da avaliação de processo da intervenção "Saúde na Boa"; e testar diferenças entre estudantes permanecentes e não permanecentes até o final da intervenção. Das vinte escolas selecionadas, dez foram sorteadas para a condição "intervenção" e dez compuseram o grupo "controle", em Recife/PE, e Florianópolis/SC. A avaliação de processo foi realizada nestas escolas, considerando cinco eixos (conhecimento, visibilidade, ambiente, aulas de Educação Física e atividades extraclasse), classificando-se a qualidade operacional da intervenção conforme a mediana do escore em cada um destes fatores (Mediana= 0,0 a 1,0: baixa; 1,1 a 2,0: moderada; 2,1 a 3,0: elevada). As características dos estudantes na linha de base (março de 2006) foram comparadas entre os permanecentes (n= 989) e não permanecentes (n= 1166). Verificou-se qualidade operacional moderada à elevada da intervenção para os itens conhecimento da intervenção (Mediana= 2,0), distribuição de frutas e instalação de bicicletários (Mediana= 1,5), aquisição de materiais (Mediana= 3,0) e distribuição de boletins (Mediana= 2,0). O grupo de não permanecentes apresentou significativamente maior frequência de rapazes (47,8% vs. 40,3%), de jovens que trabalhavam (49,2% vs. 44,2), que consumiam cigarros (21,8% vs. 13,6%) e bebidas alcoólicas (57,4% vs. 49,5%), e dormiam ≥8 h/dia (19,5% vs. 27,3%) do que os permanecentes. Portanto, a intervenção apresentou qualidade operacional moderada, e o perfil do abandono diferiu com relação ao sexo, ocupação e consumo de drogas lícitas.
School-based interventions have been evaluated to examine its implementation quality and to identify the characteristics of successful participants. The objective was to analyze the process evaluation of the "Saúde na Boa" intervention and to test differences between successful participants and dropouts. Twenty schools were randomly selected to receive the intervention (n= 10) or the control (n= 10), in Recife-PE and Florianopolis-SC. The process evaluation was performed in the intervention schools using a checklist of items from five domains (knowledge, visibility, environment, Physical Education classes and extracurricular activities). The operational intervention quality was classified using the median score from each domain (median from 0.0 to 1.0= low, from 1.1 to 2.0= moderate, and from 2.1 to 3.0= high). Student characteristics were assessed at baseline (March 2006) and were compared between the participants who successfully completed the intervention (n= 989) and the dropouts (n= 1,166). We verified moderate to high operational quality of the intervention for knowledge (median= 2.0), fruit distribution and bike rack installation (median= 1.5), acquisition of materials (median= 3.0) and distribution of journals (median= 2.0). Students who dropped out were significantly more frequent in male (47.8% vs. 40.3%), in workers students (49.2% vs. 44.2%), those who smoked (21.8% vs. 13.6%) and consumed alcoholic beverages (57.4% vs. 49.5%), and slept ≥8 h/day (19.5% vs. 27.3%), than participants who did not drop out. Therefore, the "Saúde na Boa" intervention was of moderate operational quality. Participants and dropout students differed according to their gender, work status and use of licit drugs.
