Resumen: Objetivo: Identificar las plantas medicinales que presentan interacciones farmacológicas con los fármacos antivirales. Materiales y métodos: Se realizó una revisión bibliográfica mediante la recolección de artículos en las bases de datos PubMed, Scielo, Google académico. Se recuperó información de cada una de las plantas estudiadas hasta mayo de 2018. Se confeccionó una ficha instructiva a partir de la información obtenida, se tuvo en cuenta su utilidad y actualidad. Resultados: El 57.9% de la información fue recuperada de Google académico. El 47.9% del total de estudios revisados se referían a estudios clínicos y el 27% fueron investigaciones realizadas en Cuba. Allium sativum L. (19.7%), Hypericum perforatum (7.8%) y Panax quinquefolius (7.6%) fueron las plantas que presentaron mayor número de estudios concernientes a interacciones con antivirales. Las acciones sobre el citocromo P450 y la glicoproteína-P fueron las principales responsables de la ocurrencia de interacciones entre las plantas medicinales y los antivirales. La curcumina metabolito secundario de la Curcuma longa L. mostró potencial actividad antiviral ante virus de inmunodeficiencia humana tipo 1 sin obtenerse resultados concluyentes. Conclusiones: Podemos concluir que a pesar de que las interacciones farmacológicas entre antivirales y plantas medicinales son escasas, cuando se presentan lo hacen, en su mayoría, en forma de alteraciones farmacocinéticas. Los antirretrovirales fueron los fármacos de este grupo más involucrados en interacciones con plantas de uso común como el ajo. Los resultados encontrados fueron contradictorios en ocasiones y no todos estaban basados en evidencias clínicas.

Abstract: Objective: To identify medicinal plants that show pharmacological interactions with antiviral drugs. Materials and methods: A literature review carried out through the collection of articles in the PubMed, Scielo, Google academic databases. Information retrieved from each of the plants studied up to May 2018. An information sheet was prepared based on the information obtained and taking into account its usefulness and topicality. Results: 57.9% of the information was retrieved from academic Google. 47.9% of the total studies reviewed referred to clinical studies and 27% were investigations carried out in Cuba. Allium sativum L. (19.7%), Hypericum perforatum (7.8%) and Panax quinquefolius (7.6%) were the plants that presented the highest number of studies concerning interactions with antivirals. Actions on cytochrome P450 and P-glycoprotein were mainly responsible for the occurrence of interactions between medicinal plants and antivirals. The secondary metabolite curcumin of Curcuma longa L. showed potential antiviral activity against human immunodeficiency virus type 1 without obtaining conclusive results. Conclusions: We can conclude that although the interactions between antivirals and medicinal plants are rather rare when they occur, they do so mostly in the form of pharmacokinetic alterations. Antiretroviral drugs are the drugs of this group most involved in interactions with commonly used plants such as garlic. The results found are not all base on clinical evidence and sometime they were contradictories.

ABSTRACT Introduction: There are currently more than 150 medications related to the appearance of rhabdomyolysis and transient acute renal failure. Streptokinase can be one of them. Objective: Presenting a case the administration of streptokinase as a possible cause of acute, transient renal failure. Case report: A 38-year-old patient with a previous health history and 81 mg creatinine per liter days before admission, suffers acute myocardial infarction and after streptokinase thrombolysis he suffers severe muscle pain, nausea, vomiting, severe low back pain, creatinine ascends progressively and progressive oligoanuria that evolves until two weeks when it begins to return. At 21 days with 116 mg creatinine per liter, coronary angiography was performed with normal coronaries. Comments: The causes of transient acute renal failure may be rhabdomyolysis associated with abnormalities of the metabolism of ATP and immune disorders, caused by the administration of streptokinase. His coronary angiography was completely normal.

RESUMEN Introducción: En la actualidad existen más de 150 medicamentos relacionados con la aparición de rabdomiólisis e insuficiencia renal aguda transitoria. La estreptoquinasa puede ser uno de ellos. Objetivo: Presentar un caso en el cual la administración de trombólisis con estreptoquinasa pudiera estar relacionado con una insuficiencia renal aguda transitoria. Caso clínico: Paciente de 38 años, con antecedentes de salud anterior y creatinina de 81 mg/L días antes del ingreso, sufrió infarto miocárdico agudo y después de la trombólisis con estreptoquinasa presentó dolores musculares intensos, náuseas, vómitos y lumbalgia intensa. La creatinina ascendió progresivamente; tuvo oligoanuria progresiva que evolucionó hasta las dos semanas y luego se recuperó. A los 21 días, la creatinina estaba en 116 mg/L. En la coronariografía, las coronarias son normales. Comentarios: Las causas de insuficiencia renal aguda transitoria pudieran ser, rabdomiólisis asociada con alteraciones del metabolismo del ATP y trastornos inmunológicos provocados por la administración de estreptoquinasa. La coronariografía resultó normal.

ABSTRACT Introduction: When placing the central venous catheter in patients with chronic renal failure during hemodialysis, prevention is essential to avoid infective endocarditis. Staphylococcus aureus is an aggressive germ, which in immunocompromised patients with pulmonary and systemic cardioembolic phenomena, causes damage to functions of organs and systems. The phenomenon of multi-organ failure is a feared complication. Objective: To present a case of aggressive infective endocarditis in a hemodialysis patient. Clinical case: Female patient, 31 years old, with a diagnosis of infective endocarditis, with timely, adequate and multidisciplinary treatment. After an improvement, he went on to a marked deterioration, died due to multi-organ failure, due to septicemia and multiple cardioembolisms. Comment: The resistance of aggressive germs becomes more frequent. The life of the patient, immunocompromised and manipulated, is at significant risk with multi-organ failure.

RESUMEN Introducción: Al colocar el catéter venoso central en enfermos de insuficiencia renal crónica durante la hemodiálisis, la prevención es imprescindible para evitar la endocarditis infecciosa. El estafilococo dorado es un germen agresivo, que en enfermos inmunodeprimidos con fenómenos cardioembólicos pulmonares y sistémicos, ocasiona daños a funciones de órganos y sistemas. El fenómeno de fallo multiórganos es una complicación temida. Objetivo: Presentar un caso de endocarditis infecciosa agresiva, en un paciente en hemodiálisis. Caso clínico: Paciente femenina, de 31 años de edad, con diagnóstico de endocarditis infecciosa, con tratamiento oportuno, adecuado y multidisciplinario. Después de una mejoría, pasó a un deterioro marcado, falleció por fallo multiórganos, debido a septicemia y cardioembolismos múltiples. Comentarios: La resistencia de los gérmenes agresivos, se hace más frecuente. La vida de la enferma, inmunodeprimida y manipulada, se sitúa en riesgo significativo con fallo multiórganos.

ABSTRACT Introduction: Of the primary cardiac tumors, 75% are benign and more than half correspond to myxomas. They can be asymptomatic or generate cardiac, systemic or embolic manifestations, the latter frequent. Objective: To remark the importance of considering the primary cardiac tumor as a cause of cerebral embolic disease. Clinical case: 53-year-old man with dizziness, headache, general malaise, weakness, joint pain, palpitations, dyspnea and orthopnea. He suffered loss of consciousness and later loss of muscle strength in the left half of the body due to an embolic-type stroke. With the transthoracic echocardiogram, the diagnosis of a primary cardiac tumor was confirmed. After the operation, it was verified in pathological anatomy, that it was a myxoma. Conclusions: Myxoma is a tumor that can produce cardioembolic phenomena and must be taken into account in patients like the one presented.

RESUMEN Introducción: De los tumores cardiacos primarios, el 75 % son benignos y más de la mitad corresponde a mixomas. Pueden ser asintomáticos o generar manifestaciones cardíacas, sistémicas o embólicas, estas últimas frecuentes. Objetivo: Divulgar la importancia de tener presente el tumor primario cardiaco como causa de enfermedad embólica cerebral. Caso clínico: Hombre de 53 años de edad, con mareos, cefalea, malestar general, decaimiento, dolores articulares, palpitaciones, disnea y ortopnea. Sufrió pérdida de la conciencia y posteriormente pérdida de la fuerza muscular del hemicuerpo izquierdo debido a un accidente vascular encefálico de tipo embólico. Con el ecocardiograma transtorácico, se confirmó el diagnóstico de un tumor primario cardiaco. Después de la operación, se comprobó en anatomía patológica, que era un mixoma. Conclusiones: El mixoma es un tumor que puede producir fenómenos cardioembólicos y se debe tener presente en pacientes como el que se presenta.

ABSTRACT Introduction: Systemic lupus erythematosus has multiple ways of presentation. Pericardium involvement is the most frequent cardiac manifestation. Cardiac tamponade is very rare, but it presents as a life-threatening clinical hemodynamic syndrome. Objective: To show a very rare form of presentation of systemic lupus erythematosus; cardiac tamponade. Clinical case: 47-year-old white patient with anasarca, dyspnea, palpitations, polypnea, orthopnea, intercostal retraction, throbbing severe chest pain, bad general condition, anxiety, fever, arthralgia, arthritis, dermatological lesions, acrocyanosis, jugular engorgement and severe hypotension. Comments: Cardiac tamponade patient must be assumed by a multidisciplinary team that defines the precise diagnosis and behavior, especially when it comes to the debut of systemic lupus erythematosus, which with its autoimmune phenomena, affects all organs and systems.

RESUMEN Introducción: El lupus eritematoso sistémico tiene múltiples formas de presentarse. La afectación del pericardio es la manifestación cardíaca más frecuente. El taponamiento cardíaco es muy raro, pero se presenta como un síndrome clínico hemodinámico potencialmente mortal. Objetivo: Mostrar una forma muy rara de presentación del lupus eritematoso sistémico; el taponamiento cardíaco. Caso clínico: Paciente blanca de 47 años con anasarca, disnea, palpitaciones, polipnea, ortopnea, tiraje intercostal, dolor torácico intenso, punzante, toma del estado general, ansiedad, fiebre, artralgia, artritis, lesiones dermatológicas, acrocianosis, ingurgitación yugular e hipotensión marcada. Comentarios: El paciente con taponamiento cardíaco debe ser asumido por un equipo multidisciplinario que defina el diagnóstico preciso y la conducta, sobre todo cuando se trata del debut del lupus eritematoso sistémico, que con los fenómenos autoinmunitarios, afecta a todos los órganos y sistemas.

ABSTRACT Introduction: Bad eating habits contribute to the emergence of obesity, which represents a risk factor for the onset of diabetes mellitus. Objective: To determine the association between food habits and the risk of developing diabetes mellitus type 2 in patients with obesity. Methods: An observational, descriptive and cross-sectional study was carried out in the Family Doctor´s office number 6 belonging to ´´José Machado Rodríguez´´ University Policlinic, in Union de Reyes municipality, Matanzas province, during the period from December 2013 to February 2015. The sample was a total of 380 patients diagnosed as obese and it was selected a sample of 186 individuals, according to the selection criteria. It was used the Finish Diabetes Risk Score risk score. It was requested the assisted consent of patients. Results: There was a general predominance of females (51.61 %), which is associated with the most frequent age group of 35 to 54 years (49.46 %). There was a high percentage of obese individuals that add salt to the meals after cooked or while at the table (93.0 %), followed by consumption of animal fat for cooking (52.7 %). There was a low daily consumption of fruits (15.6 %), and vegetables (25.3 %). There was a predominance of patients at high risk of diabetes mellitus type 2 (31.2 %) associated with 29.6 % with bad eating habits within that group. Conclusions: Obesity associated with inadequate feeding patterns prevails in patients considered occupationally active. The highest percentage of individuals with high and very high risk of diabetes presents incorrect dietary habits. There is a percentage association between the bad eating habits and the estimated risk of developing diabetes mellitus type 2 in obese individuals.

RESUMEN Introducción: Los malos hábitos alimentarios contribuyen a la aparición de la obesidad, la cual representa un factor de riesgo para el inicio de la diabetes mellitus. Objetivo: Determinar la asociación entre hábitos alimentarios y el riesgo de desarrollar diabetes mellitus tipo 2 en pacientes con obesidad. Métodos: Se realizó un estudio observacional, descriptivo y transversal en el consultorio médico de familia número 6 perteneciente al Policlínico Universitario “José Machado Rodríguez”, municipio Unión de Reyes, Matanzas, durante el período comprendido entre diciembre 2013 y febrero 2015. El universo lo constituyó un total de 380 pacientes diagnosticados como obesos y se seleccionó una muestra de 186 individuos, según los criterios de selección. Se utilizó el score de riesgo Finish Diabetes Risk Score. Se solicitó el consentimiento asistido de los pacientes. Resultados: Se evidenció un predominio general del sexo femenino (51,61 %), lo que se asocia con el grupo de edad más frecuente de 35 a 54 años (49,46 %). Existió un elevado porcentaje de obesos que adicionan sal a las comidas después de cocidas o al sentarse a la mesa (93,0 %), seguido del consumo de grasa animal para cocinar (52,7 %). Hubo un escaso consumo diario de frutas (15,6 %), y de verduras/hortalizas (25,3 %). Predominaron los pacientes con riesgo alto de diabetes mellitus tipo 2 (31,2 %) asociados a 29,6 % con malos hábitos alimentarios dentro de ese grupo. Conclusiones: La obesidad asociada a patrones de alimentación inadecuados predomina en los pacientes considerados laboralmente activos. El mayor porcentaje de individuos con riesgo alto y muy alto de diabetes presenta incorrectos hábitos dietéticos. Existe una asociación porcentual entre los malos hábitos alimentarios y el riesgo estimado de desarrollar diabetes mellitus tipo 2 en individuos obesos.

ABSTRACT Introduction: Staphylococcus aureus is an important cause of morbidity and mortality in the world. In Cuba, it is a germ to be taken into account as a cause of nosocomial infection. Objective: To characterize, clinically and epidemiologically, a sample of patients with infections caused by Staphylococcus aureus. Methods: An observational, descriptive and cross-sectional study was conducted at the Military Hospital "Dr. Mario Muñoz Monroy "of Matanzas, during the period from January to December of the year 2014. The population was constituted by all the patients hospitalized with the diagnosis of infection by Staphylococcus aureus (120 cases), identified by the department of Microbiology; then the medical records from which the information was obtained were reviewed. Results: There was a predominance of males (64.2%) those over 60 were the most affected (36.7%). Diabetes mellitus was the most frequent associated disease (40.8%) and the majority of infections were of community origin (67.5%). Positivity predominated in samples taken from skin lesions and blood cultures with 50.8% and 19.2%, respectively. Low sensitivity to vancomycin (49.2%) and cotrimoxazole (38.3%) was evidenced. Conclusions: Staphylococcus aureus infections were more frequent in geriatric male patients of out-of-hospital origin. The germ showed high resistance to antibiotics and of these, the aminoglycosides showed the highest antimicrobial sensitivity.

RESUMEN Introducción: El estafilococo dorado es una causa importante de morbilidad y mortalidad en el mundo. En Cuba, es un germen a tener en cuenta como causa de infección intrahospitalaria. Objetivo: Caracterizar, clínica y epidemiológicamente, una muestra de pacientes con infecciones causadas por estafilococo dorado. Métodos: Se realizó un estudio observacional, descriptivo y transversal en el Hospital Militar "Dr. Mario Muñoz Monroy" de Matanzas, durante el período de enero a diciembre del año 2014. La población estuvo constituida por la totalidad de los pacientes hospitalizados con el diagnóstico de infección por estafilococo dorado (120 casos), identificados por el departamento de Microbiología; luego se revisaron las historias clínicas de las cuales se obtuvo la información. Resultados: Existió un predominio del sexo masculino (64,2 %), los mayores de 60 años fueron los más afectados (36,7 %). La diabetes mellitus fue la enfermedad asociada más frecuente (40,8 %) y la mayoría de las infecciones fueron de origen comunitario (67,5 %). Los aislamientos predominaron en las muestras tomadas de las lesiones en piel y los hemocultivos con un 50,8 % y 19,2 %, respectivamente. Se evidenció baja sensibilidad a la vancomicina (49,2 %) y al cotrimoxazol (38,3 %). Conclusiones: Las infecciones por estafilococo dorado fueron más frecuentes en pacientes masculinos geriátricos de su origen extrahospitalario. El germen presentó alta resistencia a los antibióticos y de estos, los aminoglucósidos mostraron la mayor sensibilidad antimicrobiana.
