Abstract The combination of red and blue light emitting diodes (LEDs) is an effective light source for plant growth and development. Several species of the genus Lilium are valued as cut flowers, but the information on the effect of the quality of light on their growth is still very scarce and it is necessary to determine the optimal light spectrum that allows obtaining desirable characteristics. The objective of this paper was to evaluate the growth of Lilium spp. ‘Corvara’ with LED lighting. The proportion of red and blue LED lights in each treatment was: 20:80 (R4B); 40:60 (2R3B); 60:40 (3R2B); 80:20 (4RB) and the control (W) with 100% white light. All treatments had a photosynthetically active radiation of 90 ±10 μmol m-2 s-1 during a photoperiod of 14 h. The work was conducted in Chapingo, Mexico in 2019 and 2020. The results showed that, the lowest number of days to harvest maturity (91 days) and the plants of lower height (83 cm) occurred with the illumination of treatment 2R3B. Regarding the control, increases were achieved in the following variables: with treatment R4B, leaf area 26% and tepal color 13% in the variable Chroma; with 3R2B, vase life 6%, with 4RB, plant height 21%, flower diameter 5% and the number of days to maturity was 16. It is concluded that the proportion of red and blue light modifies the growth and development of Lilium spp. ‘Corvara’ plants.

Resumen La combinación de diodos emisores de luz (LEDs) rojos y azules es una fuente de luz efectiva para el crecimiento y desarrollo de las plantas. Diversas especies del género Lilium son valoradas como flores de corte, pero la información que se tiene del efecto de la calidad de la luz sobre su crecimiento es aún muy escasa y es necesario determinar el espectro de luz óptimo que permita obtener características deseables. El objetivo de este trabajo fue evaluar el crecimiento de Lilium spp. ‘Corvara’, con iluminación LED. La proporción de luces LED roja y azul en cada tratamiento fue: 20:80 (R4B); 40:60 (2R3B); 60:40 (3R2B); 80:20 (4RB) y el testigo (W) con 100% de luz blanca. Todos los tratamientos tuvieron una radiación fotosintéticamente activa de 90 ±10 µmol m-2 s-1 durante un fotoperiodo de 14 h. El trabajo se realizó en Chapingo, México en 2019 y 2020. Los resultados mostraron que con la iluminación del tratamiento 2R3B se registró la menor cantidad de días a madurez de cosecha (91 días) y las plantas de menor altura (83 cm). Respecto al testigo se lograron incrementos en las siguientes variables: con el tratamiento R4B, área foliar 26% y color de tépalos 13% en la variable Croma; con 3R2B la vida en florero 6%, con 4RB, altura de plantas 21%, diámetro de flor 5% y el número de días a madurez fue 16. Se concluye que la proporción de la luz roja y azul modifica el crecimiento y desarrollo de las plantas de Lilium spp. ‘Corvara’.

ABSTRACT Objective: to provide evidence based clinical recommendations for the diagnosis and therapy of the acute stage of ischemic cerebrovascular accident in EsSalud. Materials and methods : a group for producing the guideline was conveyed, including specialized physicians and methodologists. The group prepared 8 clinical questions to be answered by the guideline. Systematic searches of previous reviews were performed, and - when it was deemed necessary - primary studies in Medline and the Cochrane Controlled Registry of Trials for 2018 were reviewed. Evidence was selected aiming to respond each one of the proposed clinical questions. Certainty of the evidence was determined using the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) methodology. In periodic work sessions, the group used GRADE methodology for reviewing the evidence and generated recommendations, good clinical practice items, and designed flow charts for both diagnosis and therapy. Finally, the guide was approved with the decree N° 128-IETSI-ESSALUD-2019. Results : this clinical practice guideline focused in 8 clinical questions, which were divided into four topics: screening, diagnosis, therapy, support and rehabilitation. On the basis of these questions, 28 recommendations were formulated (8 strong and 20 conditional), 38 items for good clinical practice, 1 implementation note, and 2 flow charts. Conclusion : this paper summarizes both evidence based methodology and conclusions from a new clinical practice guide for diagnosis and therapy of the acute stage of a cerebrovascular accident in EsSalud.

RESUMEN Objetivo: proveer recomendaciones clínicas basadas en evidencia para el diagnóstico y tratamiento de la etapa aguda del ataque cerebrovascular isquémico en EsSalud. Materiales y métodos: se conformó un grupo elaborador de la guía (GEG) que incluyó médicos especialistas y metodólogos. El GEG formuló 8 preguntas clínicas a ser respondidas por la presente GPC. Se realizó búsquedas sistemáticas de revisiones sistemáticas y -cuando fue considerado pertinente- estudios primarios en Medline y Cochrane Controlled Register of Trials durante el 2018. Se seleccionó la evidencia para responder cada una de las preguntas clínicas planteadas. La certeza de la evidencia fue evaluada usando la metodología Grading of Recommendations Assessment, Development, and Evaluation (GRADE). En reuniones de trabajo periódicas, el GEG usó la metodología GRADE para revisar la evidencia y formular las recomendaciones, los puntos de buenas prácticas clínicas y el flujograma de diagnóstico y tratamiento. Finalmente, la GPC fue aprobada con Resolución N° 128-IETSI-ESSALUD-2019. Resultados: la presente GPC abordó 8 preguntas clínicas, divididas en cuatro temas: tamizaje, diagnóstico, tratamiento, soporte y rehabilitación. En base a estas preguntas se formularon 28 recomendaciones (8 fuertes y 20 condicionales), 38 puntos de buena práctica clínica, 1 nota de implementación y 2 flujogramas. Conclusión: el presente artículo resume la metodología y las conclusiones basadas en evidencias de la GPC para el diagnóstico y tratamiento de la etapa aguda del ataque cerebrovascular isquémico en EsSalud.

Abstract Introduction: The incidence of thyroid cancer has increased during the last decades. A growing neck mass is the typical initial clinical presentation but most of these are benign in nature. Therefore, it is necessary to identify the malignant ones preoperatively; those nodules are the ones that require surgical management. Overall, we have a lack of well-conducted randomized controlled clinical trials so there is an important degree of heterogeneity in treatment on daily basis. Objectives: This Clinical Practice Guideline (CPG) contains systematically developed recommendations to support clinicians, patients and carers, policy makers and other stakeholders in the diagnostic and management of patients with early, advanced and metastatic stages of Thyroid Cancer. Methodology: This document was an initiative of the Head and Neck Cancer Research Group at the Mexican Society of Oncology (SMEO) in collaboration with Mexican Center of Clinical Excellence (CENETEC) according to International Standards. Such standards were those being developed by IOM, NICE, SIGN and GI-N. An interdisciplinary Guideline Development Group (GDG) was put together and considered oncologist, oncologic surgeons, radiation therapist, and methodologists with expertise in critical appraisssal, systematic reviews and clinical practice guidelines development. Results: 45 clinical questions were agreed between the GDG. With the evidence identified from systematic reviews, GDG developed clinical recommendations using a Modified Delphi Panel technique. Patients’ representatives validated them. Conclusions: This Clinical Practice Guideline aims to support share decision-making process for patients with different stages of Thyroid Cancer. Our goal is to improve health-care quality on these patients.

Resumen Introducción: Se ha observado un incremento mundial importante del cáncer diferenciado de tiroides. Suele presentarse clínicamente como nódulos en el cuello, y aunque la mayoría de estos son de naturaleza benigna, es importante la detección de malignidad en forma preoperatoria debido a que las lesiones malignas son las que requieren tratamiento quirúrgico. Existe todavía controversia respecto al manejo terapéutico por la falta de estudios controlados y existe además mucha heterogeneidad en la práctica clínica cotidiana. Objetivos: Esta guía de práctica clínica (GPC) contiene recomendaciones clínicas desarrolladas de forma sistematizada para asistir la toma de decisiones de médicos especialistas, pacientes, cuidadores de pacientes y elaboradores de políticas públicas involucrados en el tratamiento de pacientes con cáncer de tiroides en estadios tempranos, localmente avanzados y metastásicos. Material y métodos: El Grupo de Investigación en Cáncer de Cabeza y Cuello y la Sociedad Mexicana de Oncología, en colaboración con el Centro Nacional de Excelencia Tecnológica, conformaron un grupo multidisciplinario para desarrollar esta GPC de acuerdo con los estándares internacionales vigentes. Se desarrolló un documento de alcances y una lista de preguntas clínicas estructuradas de manera consensuada, se identificó la evidencia científica de la más alta calidad para posteriormente extraerla y elaborar las recomendaciones clínicas. Por medio de un método de consenso formal de expertos se validaron y ajustaron las recomendaciones clínicas, que tienen como objetivo apoyar la toma de decisiones clínicas en el tratamiento de esta patología para mejorar los desenlaces clínicos de nuestros pacientes. Resultados: Se consensuaron 45 preguntas clínicas que abarcaron lo establecido previamente por el grupo de desarrollo de la guía (GDG) en el documento de alcances de la guía. Se identificó y evaluó críticamente la evidencia científica que responde a cada una de estas preguntas clínicas antes de ser incorporada en el cuerpo de evidencia de la guía. El GDG acordó, mediante la técnica de consenso formal de expertos Panel Delphi, la redacción final de las recomendaciones clínicas. Conclusión: Esta GPC contiene recomendaciones clínicas para el manejo de los distintos estadios de la enfermedad para apoyar la toma de decisiones clínicas. El GDG espera que esta guía contribuya a mejorar la calidad de la atención clínica en las pacientes con cáncer de tiroides.

Abstract Introduction Indolent or chronic mucormycosis is a rare entity that affects both immunosuppressed and immunocompetent individuals. Additionally, its clinical evolution is nonspecific and there is no standardized treatment for this condition. Objective To describe the clinical characteristics and management of patients with indolent mucormycosis. Methods In the project of study with chart review in the Interinstitutional secondary care centers, patients with evidence of indolentmucormycosis, defined as pathological confirmation of nasal/paranasal sinus mucormycosis for more than 1 month, were included. All patients underwent complete laboratory workup, imaging studies, surgical treatment and adequate follow-up. No evidence of disease status was defined when patient had subsequent biopsies with no evidence of mucormycosis. Results We included seven patients, three female and four male subjects. The mean age was 53.14 years. Four patients were immunosuppressed and three immunocompetent. Among the immunosuppressed patients three had diabetes and one had dermatomyositis. The symptomswere nonspecific: facial pain/headache, mucoid discharge and cacosmiawere the ones most frequently reported. Maxillary sinus involvement was present in all patients. Two immunosuppressed subjects received amphotericin. Posaconazole was the only treatmentinoneimmunosuppressedpatient. Allimmunocompetent patientshadsingleparanasal sinus disease and received only surgical treatment. All patients are alive and free of disease. Conclusion Indolent mucormycosis is a new and emerging clinical entity in immunosuppressed and immunocompetent patients. Single paranasal sinus disease is a frequent presentation and should not be overlooked as a differential diagnosis in these patients. Immunocompetent patients should only be treated surgically.
