Resumo Objetivou-se identificar as ações desenvolvidas pelos sistemas de saúde do Brasil e da Itália para o enfrentamento da pandemia por COVID19. Trata-se de uma revisão integrativa da literatura nas bases de dados Web of Science, Scopus, Pubmed e Biblioteca Virtual de Saúde, com adição da técnica snowball, em julho de 2020. Após a coleta, 48 publicações constituíram a amostra. As ações dos sistemas de saúde dos dois países para o enfrentamento da pandemia foram agrupadas nas categorias: Adequação do normativo legal do sistema de saúde; Ampliação e fortalecimento do sistema de saúde; Uso das Tecnologias da Informação e Comunicação; Estímulo às medidas individuais e coletivas para enfrentamento da pandemia; e Limitações e desafios no enfrentamento da pandemia. As medidas adotadas são influenciadas pelos contextos político, econômico e social. Houve a reorganização da rede hospitalar nos dois países, mas identificam-se fragilidades nas ações preventivas desenvolvidas pela Atenção Primária à Saúde, predominando o modelo biomédico. A telemedicina destacou-se neste cenário e poderá perdurar no pós-pandemia. A sumarização das ações subsidiará o enfrentamento de outras pandemias com que, eventualmente, os sistemas de saúde destes e de outros países possam se deparar.

Other actions, however, do not find the same support in the national and subnational discourse, reason why the following is adopted for a more detailed analysis in this article: the organization of risk communication and community engagement; the practice of social distancing; and the use of medications to treat the disease. Risk communication is based on the premise that every public health emergency faces communication challenges and may benefit from the lessons already learned. The fact that there are different discourses among the entities of the Federation implies disorganization of information, disagreements, and a greater degree of uncertainty in the population about the disease and how to prevent it.

OBJETIVO: Identificar os padrões dos municípios do estado de São Paulo quanto à relação da tuberculose com os indicadores de oferta e serviços de saúde, contexto e iniquidade social. MÉTODOS: Estudo ecológico que considerou os 645 municípios do estado de São Paulo e oito variáveis inseridas nas dimensões indicadores de oferta de serviços de saúde, contexto e iniquidade social. Nas análises, utilizaram-se as técnicas de agrupamento nível hierárquico, não hierárquico, de componentes principais e de geoprocessamento. RESULTADOS: A investigação mostrou cinco grupos de municípios: grupo 1 (117 municípios), com coberturas equivalentes de tratamento diretamente observado (TDO) e Estratégia Saúde da Família (ESF), coeficientes de incidência e de óbito elevados e baixo índice de desenvolvimento humano (IDH), grupo 2 (142 municípios), com baixa cobertura de TDO e ESF; grupo 3 (36 municípios), com cobertura mediana de ESF e TDO e elevados coeficientes de incidência de tuberculose, demanda do teste anti-HIV e abandono do tratamento; grupo 4 (186 municípios), com IDH abaixo dos grupos 2, 3 e 5, porém alta cobertura de ESF e os menores valores de testes anti-HIV, incidência de tuberculose e cobertura de TDO; grupo 5 (164 municípios), com condição social mais favorável do que os demais grupos, com coberturas de ESF em valores medianos e de TDO mais elevadas. As duas primeiras componentes selecionadas para o estudo - indicador de oferta de serviços de saúde em função da cobertura de TDO e indicador de iniquidade social - representaram juntas 76,96% da variância. Nos municípios com alta cobertura de TDO houve evidências de melhor organização dos serviços para o controle da tuberculose. CONCLUSÕES: Observou-se cobertura da ESF nos municípios com maior iniquidade. Porém, sem TDO, o resultado da ESF para controle da tuberculose parece não ser tão eficaz.

OBJECTIVE: To identify the patterns of São Paulo state municipalities, in Brazil, regarding the relationship between tuberculosis and indicators of health care services, health care service availability, context, and social inequality. METHODS: This ecological study covered all 645 municipalities of São Paulo and eight variables belonging to three dimensions: health care service availability, context, and social inequality. Data were analyzed using hierarchical and non-hierarchical clustering, principal components analysis, and geoprocessing. RESULTS: The analysis revealed five groups of municipalities: group 1 (117 municipalities), with similar directly observed treatment (DOT) and family health strategy (FHS) coverage, high tuberculosis incidence and death coefficients, and a low human development index (HDI); group 2 (142 municipalities), with low DOT and FHS coverage; group 3 (36 municipalities), with medium DOT and FHS coverage and high tuberculosis incidence, high demand for anti-HIV testing, and treatment dropout; group 4 (186 municipalities) with lower HDI as compared to groups 2, 3, and 5, but high FHS coverage and the lowest numbers of anti-HIV testing, tuberculosis incidence, and DOT coverage; and group 5 (164 municipalities), with better social conditions as compared to the other groups, medium FHS coverage, and higher DOT coverage. Together, the first two components selected for the study-health service availability in terms of DOT coverage indicator and social inequality indicator-explained 76.96% of the variance. In municipalities with high DOT coverage there was evidence of better organized tuberculosis control services. CONCLUSIONS: Municipalities with the highest social inequality had FHS coverage. However, without DOT, FHS seem less efficient to control tuberculosis.

OBJETIVO: Analisar o trabalho em equipe na atenção primária em saúde em Portugal. MÉTODOS: Foi realizada uma pesquisa avaliativa de abordagem qualitativa, com desenho de estudo de caso. Os dados foram obtidos por entrevista semiestruturada, observação direta e análise documental. Foram entrevistados gestores, profissionais e usuários de 11 unidades de saúde familiar (USF) portuguesas, totalizando 71 participantes. Utilizou-se a técnica de análise de conteúdo na interpretação das entrevistas. RESULTADOS: Cada equipe era constituída por médico, enfermeiro e funcionário administrativo, atendendo entre 1 250 e 2 060 usuários. Uma característica marcante da experiência portuguesa foi a formação das equipes nas USF de forma voluntária, por meio de afinidades pessoais, com autonomia de composição. Observou-se nessas USF o desenvolvimento de "carteira básica de serviços" juntamente com intervenções de vigilância, promoção da saúde e prevenção de doença, cuidados em situação de doença aguda, acompanhamento clínico de doença crônica e de patologia múltipla, cuidados domiciliares, interligação e colaboração em rede com outros serviços (cuidados hospitalares). Foram relatadas dificuldades no atendimento domiciliar. A informatização era ampla nas USF. De acordo com os entrevistados, as mudanças advindas da implementação das USF foram maior acessibilidade dos usuários aos serviços, maior qualidade do cuidado prestado e trabalho em equipe com objetivos e metas, além da existência de um plano de ação. CONCLUSÕES: Mesmo não tendo assumido um papel de coordenação da rede de cuidados, a atenção primária em saúde organizada a partir de equipes foi avaliada positivamente em Portugal, como promotora de maior acesso, continuidade e humanização dos serviços.

OBJECTIVE: To analyze the work of health care teams delivering primary care in Portugal. METHODS: We performed an evaluative research project, with a qualitative case study design. Data were obtained through semi-structured interviews, direct observation, and analysis of documents. We interviewed managers, workers, and users in 11 family health units (USF) in Portugal, for a total of 71 participants. Content analysis was used to assess the interviews. RESULTS: Teams included a physician, a nurse, and an administrative employee. Each team was in charge of 1 250 to 2 060 users. A striking characteristic of the Portuguese experience was the voluntary and autonomous setup of teams at the USF, based on personal affinity. The services provided at the USF included a basic "service package" as well as activities of surveillance, health promotion, disease prevention, care of acute disease, clinical follow-up of patients with chronic or multiple diseases, home care, and networking with other services (hospital care). Difficulties in providing home care were reported. Electronic systems were widely available. According to interviewees, the changes resulting from the implementation of USF included improved user access to services, higher quality of care, and goal-oriented teamwork, guided by an action plan. CONCLUSIONS: Even without a coordinating role in the health care network, the delivery of primary health care through teams was positively evaluated in Portugal as promoting increased access, continuity, and humanization of health services.

No Brasil, estima-se que ocorram anualmente 1.443.350 abortos, que levam milhares de mulheres, com abortamento incompleto, a buscar atendimento nos serviços públicos de saúde. Em 2005, o Ministério da Saúde publicou a Norma Técnica de Atenção Humanizada ao Abortamento como resposta a este desafio. Este artigo objetiva discutir se o conjunto de intervenções da norma constitui um programa avaliável e propor um modelo de avaliação. A pesquisa, realizada em 2010, caracteriza-se como estudo de avaliabilidade. Utilizaram-se as técnicas de análise documental e entrevista com informantes-chave. Para validação dos indicadores, foi aplicada a técnica do grupo nominal. Como resultados, foram elaborados e pactuados o modelo lógico, a matriz dos indicadores e as perguntas avaliativas que traduzem os pilares da atenção: acolhimento, atenção clínica com qualidade técnica, ética e legal, planejamento reprodutivo, integração com a rede de saúde e com coletivos de mulheres. Conclui-se que a atenção humanizada ao abortamento é um programa avaliável com elaboração de um modelo de avaliação no momento oportuno. A pré-avaliação demonstrou ser adequada e mais bem compreendida pelo gestor e profissionais. Houve concordância quanto à clareza e pertinência das metas, necessidades de recursos e ações, questões éticas para orientar mudanças. O estudo aumentou o conhecimento dos atores envolvidos com participação decisiva da direção clínica. Para avaliações subsequentes, o estudo recomenda o foco em estrutura e processos dos cinco componentes do programa com priorização dos aspectos subjetivos da humanização, infraestrutura, além da prevalência do aborto no serviço público de saúde.

In Brazil, it is estimated that 1,443,350 abortions occur annually, leading thousands of women with incomplete abortion, to seek care in public health. In 2005, the Ministry of Health published the Technical Act for Humane Abortion in response to this challenge. This paper discusses whether the Act sets an evaluable program and proposes an evaluation model. The survey, conducted in 2010, is an evaluability study using techniques of document analysis and interviews with key informants. To validate the indicators, we applied the nominal group technique. The results were elaborated and agreed upon the logical model, the matrix of indicators and evaluative questions that reflect the pillars of care: care, quality clinical care with technical, ethical and legal, reproductive planning, integration with network health and collective women. We conclude that the humanized abortion is an assessable program with development of an evaluation model in a timely manner. The pre-assessment was suitable and better understood by managers and professionals. There was agreement about the explicitness and relevance of goals, actions, and resource needs, ethical questions to guide changes. The study has increased the knowledge of the actors involved with decisive participation of clinical direction. For subsequent evaluations, the study recommends focusing on the structure and processes of the five components of the program with prioritization of subjective aspects of humanization, infrastructure, and the prevalence of abortion in the public health service.

This study aimed to study how health professionals approach sexually-transmitted diseases. It is descriptive research, qualitative in character. Data was collected in 2008 via interviews with doctors and nurses in Basic Family Health Clinics in Campina Grande county in the state of Paraíba, and analyzed with content analysis. The approach to sexually-transmitted diseases varies from clinic to clinic; it is based around the biomedical model or health vigilance; and attention is focussed primarily on women. The article notes the need for change in the reasoning behind the approach to sexually-transmitted diseases, to include the male population.

Este estudio tuvo el objetivo de verificar de que modo ocurre el abordaje a las Enfermedades Sexualmente Transmisibles por los profesionales de salud. Es una investigación descriptiva, de carácter cualitativo. Los datos fueron recogidos en 2008, por medio de entrevista con médicos/enfermeros de las Unidades Básicas de Salud de la Familia del município de Campina Grande, estado de Paraíba, y tratados por medio del análisis de contenido. El abordaje de las enfermedades sexualmente transmisibles oscila en las distintas unidades; está centrado en modelo biomédico o de vigilancia en salud; y la atención se vuelve, de modo prioritario, para las mujeres. Se constata la necesidad de cambios en la lógica de abordaje a las enfermedades transmitidas por el sexo, haciendo la inclusión del público masculino.

Os dados coletados em 2008, por meio de entrevista com médicos/enfermeiros das Unidades Básicas de Saúde da Família do Município de Campina Grande, Estado da Paraíba, e tratados por meio de análise de conteúdo. A abordagem às doenças sexualmente transmissíveis varia nas diferentes unidades; está centrada no modelo biomédico ou de vigilância em saúde; e a atenção volta-se, prioritariamente, para as mulheres. Constata-se a necessidade de mudanças na lógica de abordagem às doenças transmitidas pelo sexo, incluindo o público masculino.
