RESUMO Objetivo: mensurar o tempo total da mastigação e da fase oral da deglutição em idosos saudáveis, considerando diferentes consistências alimentares, a presença de dentes naturais e o tipo de reabilitação oral protética, bem como realizar comparação entre as condições dentárias e as consistências. Métodos: estudo transversal com 57 idosos saudáveis, entre 60 e 82 anos, distribuídos em grupo dentado (D) (n=15) e grupos reabilitados: próteses parciais fixas ou removíveis em uma ou ambas as arcadas (PP) (n=15); prótese total removível maxilar e mandibular (PTR) (n=15); prótese total - prótese removível superior e prótese implanto-suportada inferior (PTIS) (n=12). O tempo de duração (segundos) da mastigação e da fase oral da deglutição foi medido nas consistências: sólida (pão francês), pudim (10 ml) e líquida (10 ml). Resultados: idosos dentados saudáveis apresentaram os intervalos: líquido=1,07±0,35 s; pudim=3,48±1,76 s; e sólido=27,88±7,11 s. Foram observadas diferenças estatisticamente significativas entre os grupos D e PTIS (p=0,0056) e entre D e PTR (p=0,0056) para líquido. Para todos os grupos, houve aumento estatisticamente significante entre as consistências líquida e sólida e entre pastosa e sólida (p<0,001). Conclusões: idosos dentados apresentaram menor tempo de fase oral da deglutição para líquidos do que idosos edêntulos totais reabilitados, mas não diferente dos idosos com prótese parcial. O tempo de mastigação e deglutição do alimento sólido foi maior que o das demais consistências, independente da condição dentária.

ABSTRACT Purpose: to measure the total time of mastication and oral phase of swallowing in healthy elderly people, considering different food consistency, the presence of natural teeth, and the type of prosthetic oral rehabilitation, as well to compare dental conditions and consistencies. Methods: a cross-sectional study with 57 healthy elderly people, aged between 60 and 82 years, divided into: dentate group (D) (n=15); and rehabilitated groups; PD: fixed or removable partial dentures in one or both arches (n=15); CRD: maxillary and mandibular complete removable dentures (n=15); ISCD: complete dentures - maxillary removable denture and mandibular implant-supported dentures (n=12). The duration time (seconds) of chewing and of the oral phase of swallowing was measured in the consistencies: solid (a roll slice), pudding (10 ml) and liquid (10 ml). Results: healthy dentate elderly subjects had the following intervals: liquid=1.07±0.35 s; pudding=3.48±1.76 s; and solid=27.88±7.11 s. Statistically significant differences were observed between the D and ISCD groups (p=0.0056) and between D and CRD (p=0.0056) for liquid. For all groups, there was a statistically significant increase between liquid and solid consistencies and between pasty and solid ones (p<0.001). Conclusions: the dentate elderly had shorter oral phase time of swallowing for liquids than the rehabilitated edentulous elderly, but not different from the elderly with partial dentures. Chewing and swallowing time for solid food was longer than that for the other consistencies, regardless of dental condition.

RESUMO Objetivo: Apresentar valores para a pressão da língua em adultos jovens brasileiros, considerando provas específicas e possíveis diferenças quanto ao gênero e tipo de prova. Método: Foram avaliados 51 voluntários de 18 a 28 anos, ambos os gêneros, com boa relação dento-oclusal e sem alterações de fala. Utilizou-se o Iowa Oral Performance Instrument na avaliação da pressão da língua (Kilopascal) durante as provas de elevação, protrusão, deglutição e lateralização, além do teste de resistência (segundos). Na análise dos resultados foi utilizado o ANOVA, seguido do teste de comparações múltiplas Tukey, adotando o nível de significância de 5%. Resultados: Os valores obtidos, respectivamente para homens e mulheres, na prova de elevação foram 63,94±12,92 e 50,27±15,29, na protrusão 60,22±13,62 e 44,30±12,95, na deglutição 33,94±12,06 e 34,27±13,25, na lateralização à direita 44,15±10,47 e 31,85±8,46, na lateralização à esquerda 43,15±10,22 e 29,55±8,91, e no teste de resistência 24,85±10,95 e 17,35±6,71. Os homens apresentaram valor maior nas provas de protrusão e de lateralização; os valores das provas de protrusão e elevação não diferiram entre si para o gênero masculino, mas foram maiores que a lateralização e a deglutição em ambos os gêneros; não houve diferença na prova de lateralização em relação ao lado em ambos os gêneros. Conclusão: Valores de pressão da língua em adultos jovens brasileiros foram determinados para provas específicas; o gênero influenciou nos valores das provas de protrusão e lateralização; os valores obtidos nas provas de elevação e protrusão foram maiores que na lateralização e deglutição.

ABSTRACT Purpose: To present the measures for tongue pressure in Brazilian young adults, considering specific tasks, and to verify the differences regarding gender and according to the tasks. Methods: Fifty-one volunteers aged 18 to 28 years, of both genders with normal occlusion and without speech disorders were evaluated. We used the Iowa Oral Performance Instrument in the evaluation of tongue pressure (kilopascal) during specific tests of elevation, protrusion, swallowing, and lateralization, in addition to the endurance test (seconds). The analysis was conducted using analysis of variance, followed by the Tukey's multiple comparison test, adopting a 5% significance level. Results: The values obtained for men and women in the tests were as follows: elevation, 63.94±12.92 and 50.27±15.29; protrusion, 60.22±13.62 and 44.30±12.95; swallowing, 33.94±12.06 and 34.27±13.25; lateralization on the right, 44.15±10.47 and 31.85±8.46; lateralization on the left, 43.15±10.22 and 29.55±8.91; and endurance test: 24.85±10.95 and 17.35±6.71, respectively. The values were higher in men compared with women for the protrusion and lateralization tasks. The measures of the protrusion and elevation tasks did not differ for men but were higher in both genders than those of the lateralization and the swallowing tests. There was no difference in lateralization according to side in both the genders. Conclusion: The measures for the tongue pressure in Brazilian young adults were determined by specific tasks. Gender influenced the pressure of the tongue values for the protrusion and lateralization tasks. Elevation and protrusion tasks measures were higher than those of the lateralization and swallowing tasks.

Objetivo Relacionar os achados da diadococinesia (DDC) oral com a função mastigatória em idosos saudáveis.Métodos Análise de prontuários e arquivos de imagens de 35 idosos saudáveis, entre 60 e 74 anos de idade. Considerou-se o tipo mastigatório e outros comportamentos associados à mastigação de uma fatia de pão francês de 1cm de espessura, análise da apreensão do alimento, formação do bolo alimentar e mensuração do tempo de mastigação, utilizando cronômetro. Para avaliação da DDC, foram analisadas as emissões silábicas "pa", "ta", "ka" e a trissílaba "pataka", quanto aos parâmetros fornecidos automaticamente pelo programaMotor Speech Profile Advanced -KayPentax®. Para correlação entre os aspectos qualitativos da função mastigatória e a DDC, foi realizada estatística pertinente.Resultados A análise estatística demonstrou correlação entre o tempo mastigatório e os parâmetros coeficiente de variação do pico da intensidade da sílaba ‘'ta'' e perturbações do período da sílaba ‘'ka'', indicando instabilidade da DDC. Para o tipo mastigatório, apreensão e formação do bolo alimentar não houve correlação com os dados da DDC oral.Conclusão A DDC oral correlacionou-se com o tempo mastigatório em idosos saudáveis, evidenciando que, quanto maior a instabilidade na repetição dos movimentos de ponta e dorso de língua durante a produção articulatória, maior o tempo necessário para a preparação do alimento para a deglutição.

Purpose To relate the findings of oral diadochokinesis (DDK) and masticatory function in healthy elderly.Methods Analysis of medical records and images files of 35 healthy elderly subjects aged 60 to 74 years. The masticatory type and other behaviors associated with mastication of bread (1cm thick), the analysis of food seizure, bolus formation and measurement of chewing time, using a timer, were considered. For DDK assessment, the syllabic emissions "pa", "ta", "ka" and the tri-syllable "pataka", as to the parameters automatically provided by the Motor Speech Profile Advanced program (KayPentax®), were analyzed. Relevant statistics was performed for the correlation between the qualitative aspects of masticatory function and DDK.Results The statistical analysis showed a correlation between the chewing time and the parameters coefficient of peak intensity variation of syllable ‘'ta'' and disturbance time of syllable "ka'', indicating DDK instability. No correlation with the data of oral DDK was seen for the masticatory type, seizure and bolus formation.Conclusion Oral DDK correlated with the chewing time in healthy elderly, showing that the higher the instability in the repetition of movements of the tip and the back of the tongue, during articulatory production, the longer the time required to prepare the food for swallowing.

Objetivo Verificar se as características da mastigação influenciam os achados da deglutição orofaríngea em idosos. Métodos Foram analisados os dados de 47 idosos saudáveis - 29 mulheres e 18 homens - avaliados por meio da aplicação do protocolo clínico da mastigação e exame clínico funcional da deglutição orofaríngea. A avaliação da mastigação foi registrada em vídeo, tendo sido analisado o tipo mastigatório, a formação do bolo alimentar e o tempo mastigatório. Durante a avaliação clínica da deglutição foi verificado o desempenho dos pacientes quando testados alimentos de diferentes consistências, sendo que, após a realização da avaliação clínica, os indivíduos foram classificados quanto ao grau de disfunção da deglutição orofaríngea. Os dados foram analisados de modo descritivo e utilizando-se testes estatísticos de comparação e correlação. Resultados A média do tempo mastigatório foi de 32,45 segundos. A maioria dos indivíduos apresentou tipo mastigatório bilateral (62%), formação do bolo alimentar adequada (75%) e cerca de metade (47%), quadro de disfagia orofaríngea de grau moderado. De acordo com a análise estatística, não houve correlação entre o grau da disfunção da deglutição e o tipo mastigatório e a formação do bolo alimentar, porém, houve correlação positiva moderada entre o grau da disfunção da deglutição e o tempo de mastigação. Conclusão Para os idosos deste estudo, o tempo mastigatório influenciou o grau de disfunção da deglutição, não tendo sido encontrada relação para o tipo mastigatório e formação do bolo alimentar quanto aos achados da deglutição orofaríngea.

Purpose Verify if the masticatory characteristics influence the findings of the oropharyngeal swallowing in the elderly. Methods Data from 47 healthy elderly subjects (29 female and 18 male) were analyzed and evaluated by the application of the mastication clinical protocol and the functional clinical examination of the oropharyngeal swallowing. The mastication evaluation was recorded in video; the mastication type, the bolus formation, and the mastication time were analyzed. During the clinical evaluation of the swallowing, patients` performance was verified when food with different consistencies was tested. After this evaluation, subjects were classified according to the dysphagia level. Data was analyzed in a descriptive way using comparison and correlation statistical tests. Results The average mastication time was 32.45 seconds. Most subjects presented bilateral mastication (62%), adequate bolus formation (75%) and about half (47%) had moderate oropharyngeal dysphagia. According to the statistic analysis, there was no correlation among the level of swallowing dysfunction, the mastication type and the bolus formation. However, there was a statistically significant moderate positive correlation between the level of swallowing dysfunction and the mastication time. Conclusion It was verified that a relationship between the mastication time and the dysphagia level exists. However, no influence of mastication type and bolus formation on the swallowing of the elderly was found in this study.

OBJETIVO: investigar se há relação entre os sintomas otológicos, vestibulares, achados audiológicos e o tipo de disfunção temporomandibular (articular, muscular e misto), e verificar a distribuição do grau de disfunção da DTM nessa população. MÉTODOS: estudo retrospectivo, envolvendo 30 pacientes com deformidades dentofaciais diagnosticados com DTM, de ambos os sexos, entre 18 e 49 anos de idade, submetidos a avaliação clínica (palpação muscular, ausculta da articulação temporomandibular durante os movimentos mandibulares e mensuração da movimentação da mandíbula), exame audiológico (audiometria tonal limiar e imitanciometria) e a dois questionários, sendo um sobre sintomas otológicos e vestibulares e outro anamnético da DTM. A partir do questionário anamnético e da avaliação clínica, os sujeitos foram divididos conforme o tipo e o grau da disfunção da DTM (leve, moderado e severo), e comparados quanto à ocorrência dos sinais e sintomas auditivos, vestibulares e achados audiológicos, de acordo com o tipo de DTM. RESULTADOS: houve maior prevalência (83,33%) de DTM severa de acordo com questionário anamnético. Sujeitos com DTM mista apresentaram mais queixas de hipoacusia do que aqueles com DTM muscular (p < 0,05). Os resultados evidenciaram ausência de alterações nos exames audiológico e imitanciométrico para todos os indivíduos avaliados. CONCLUSÃO: sintomas auditivos estão presentes nos sujeitos com DTM e deformidades dentofaciais, independentemente da classificação da DTM (articular, muscular ou mista), e aqueles com DTM mista podem apresentar maior ocorrência de queixa de hipoacusia do que sujeitos com DTM muscular. Estudos futuros são necessários para investigar a relação entre a sintomatologia auditiva e os diversos tipos de DTM.

OBJECTIVE: To investigate whether there is any relationship between otological as well as vestibular symptoms, audiological findings and type of temporomandibular disorder (articular, muscular and mixed); and to check the distribution of the temporomandibular disorders (TMD) dysfunction degree in the research population. METHODS: A retrospective study involving 30 patients of both sexes, aged between 18 and 49 years old, diagnosed with TMD and dentofacial deformities, who were subject to clinical evaluation (muscle palpation, auscultation of temporomandibular joint during mandibular motion and measurement of jaw movement), audiological testing (pure tone audiometry and immittance testing) and two questionnaires, one on otological and vestibular symptoms and the other on TMD anamnesis. Based on both the anamnesis questionnaire and the clinical assessment, the subjects were divided according to the type and degree of TMD dysfunction (mild, moderate and severe), and compared regarding the occurrence of auditory signs and symptoms, vestibular symptoms and audiological findings according to TMD type. RESULTS: The anamnesis questionnaire demonstrated higher prevalence (83.33%) of severe TMD. Subjects with mixed TMD had more complaints about hypoacusis than those with muscular TMD (p < 0.05). The results showed no change in either audiological and immittance testing for all assessed individuals. CONCLUSION: Otological symptoms are present in subjects with TMD and dentofacial deformities, regardless of the classification of TMD (articular, muscular or mixed). Those with mixed TMD may have higher incidence of complaints about hypoacusis than subjects with muscular TMD. Further studies are needed to investigate the relationship between otological symptoms and the different types of TMD.
