Abstract Introduction: We investigated the clinical course and outcomes of patients submitted to cardiovascular surgery in Brazil and who had developed symptoms/signs of coronavirus disease 2019 (COVID-19) in the perioperative period. Methods: A retrospective multicenter study including 104 patients who were allocated in three groups according to time of positive real time reverse transcriptase-polymerase chain reaction (RT-PCR) for the SARS-CoV-2 (severe acute respiratory syndrome coronavirus 2): group 1, patients who underwent cardiac surgery > 10 days after positive RT-PCR; group 2, patients with a positive RT-PCR within 10 days before or after surgery; group 3, patients who presented positive RT-PCR > 10 days after surgery. The primary outcome was mortality and secondary outcomes were postoperative complications, intensive care unit (ICU) length of stay, and postoperative days of hospitalization. Results: The three groups were similar with respect to age, the European System of Cardiac Operative Risk Evaluation score, and comorbidities, except hypertension. Postoperative complications and death were significantly higher in groups 2 and 3 than in group 1, and no significant difference between groups 2 and 3 was seen. Group 2 showed a high prevalence of surgery performed as an urgent procedure. Although no significant differences were observed in ICU length of stay, total postoperative hospitalization time was significantly higher in group 3 than in groups 1 and 2. Conclusion: COVID-19 affecting the postoperative period of patients who underwent cardiovascular surgery is associated with a higher rate of morbidity and mortality. Delaying procedures in RT-PCR-positive patients may help reduce risks of perioperative complications and death.

RESUMO O DC de Neuroimunologia da ABN e o BCTRIMS trazem, nesse documento, as recomendações sobre vacinação da população com doenças desmielinizantes do sistema nervoso central (SNC) contra infecções em geral e contra o coronavírus da síndrome respiratória aguda grave 2 (SARS-CoV-2), causador da COVID-19. Destaca-se a gravidade do atual momento frente ao avanço da COVID-19 em nosso País, o que torna mais evidente e importante a criação de guia de referência para orientação aos médicos, pacientes e autoridades de saúde pública quanto à vacinação, meio efetivo e seguro no controle de determinadas doenças infecciosa. O DCNI/ABN e o BCTRIMS recomendam que os pacientes com doenças desmielinizantes do SNC (ex., EM e NMOSD) sejam constantemente monitorados, quanto a atualização do seu calendário vacinal, especialmente, no início ou antes da mudança do tratamento com uma droga modificadora de doença (DMD). É importante também salientar que as vacinas são seguras e os médicos devem estimular o seu uso em todos os pacientes. Evidentemente, deve ser dada especial atenção às vacinas com vírus vivos atenuados. Por fim, é importante que os médicos verifiquem qual DMD o paciente está em uso e quando foi feita a sua última dose, pois cada fármaco pode interagir de forma diferente com a indução da resposta imune.
ABSTRACT The Scientific Department of Neuroimmunology of the Brazilian Academy of Neurology (DCNI/ABN) and Brazilian Committee for Treatment and Research in Multiple Sclerosis and Neuroimmunological Diseases (BCTRIMS) provide recommendations in this document for vaccination of the population with demyelinating diseases of the central nervous system (CNS) against infections in general and against the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), which causes COVID-19. We emphasize the seriousness of the current situation in view of the spread of COVID-19 in our country. Therefore, reference guides on vaccination for clinicians, patients, and public health authorities are particularly important to prevent some infectious diseases. The DCNI/ABN and BCTRIMS recommend that patients with CNS demyelinating diseases (e.g., MS and NMOSD) be continually monitored for updates to their vaccination schedule, especially at the beginning or before a change in treatment with a disease modifying drug (DMD). It is also important to note that vaccines are safe, and physicians should encourage their use in all patients. Clearly, special care should be taken when live attenuated viruses are involved. Finally, it is important for physicians to verify which DMD the patient is receiving and when the last dose was taken, as each drug may affect the induction of immune response differently.

RESUMO Objetivo: Descrever as práticas de ressuscitação volêmica em unidades de terapia intensiva brasileiras e compará-las com as de outros países participantes do estudo Fluid-TRIPS. Métodos: Este foi um estudo observacional transversal, prospectivo e internacional, de uma amostra de conveniência de unidades de terapia intensiva de 27 países (inclusive o Brasil), com utilização da base de dados Fluid-TRIPS compilada em 2014. Descrevemos os padrões de ressuscitação volêmica utilizados no Brasil em comparação com os de outros países e identificamos os fatores associados com a escolha dos fluidos. Resultados: No dia do estudo, foram incluídos 3.214 pacientes do Brasil e 3.493 pacientes de outros países, dos quais, respectivamente, 16,1% e 26,8% (p < 0,001) receberam fluidos. A principal indicação para ressuscitação volêmica foi comprometimento da perfusão e/ou baixo débito cardíaco (Brasil 71,7% versus outros países 56,4%; p < 0,001). No Brasil, a percentagem de pacientes que receberam soluções cristaloides foi mais elevada (97,7% versus 76,8%; p < 0,001), e solução de cloreto de sódio a 0,9% foi o cristaloide mais comumente utilizado (62,5% versus 27,1%; p < 0,001). A análise multivariada sugeriu que os níveis de albumina se associaram com o uso tanto de cristaloides quanto de coloides, enquanto o tipo de prescritor dos fluidos se associou apenas com o uso de cristaloides. Conclusão: Nossos resultados sugerem que cristaloides são usados mais frequentemente do que coloides para ressuscitação no Brasil, e essa discrepância, em termos de frequências, é mais elevada do que em outros países. A solução de cloreto de sódio 0,9% foi o cristaloide mais frequentemente prescrito. Os níveis de albumina sérica e o tipo de prescritor de fluidos foram os fatores associados com a escolha de cristaloides ou coloides para a prescrição de fluidos.
Abstract Objective: To describe fluid resuscitation practices in Brazilian intensive care units and to compare them with those of other countries participating in the Fluid-TRIPS. Methods: This was a prospective, international, cross-sectional, observational study in a convenience sample of intensive care units in 27 countries (including Brazil) using the Fluid-TRIPS database compiled in 2014. We described the patterns of fluid resuscitation use in Brazil compared with those in other countries and identified the factors associated with fluid choice. Results: On the study day, 3,214 patients in Brazil and 3,493 patients in other countries were included, of whom 16.1% and 26.8% (p < 0.001) received fluids, respectively. The main indication for fluid resuscitation was impaired perfusion and/or low cardiac output (Brazil: 71.7% versus other countries: 56.4%, p < 0.001). In Brazil, the percentage of patients receiving crystalloid solutions was higher (97.7% versus 76.8%, p < 0.001), and 0.9% sodium chloride was the most commonly used crystalloid (62.5% versus 27.1%, p < 0.001). The multivariable analysis suggested that the albumin levels were associated with the use of both crystalloids and colloids, whereas the type of fluid prescriber was associated with crystalloid use only. Conclusion: Our results suggest that crystalloids are more frequently used than colloids for fluid resuscitation in Brazil, and this discrepancy in frequencies is higher than that in other countries. Sodium chloride (0.9%) was the crystalloid most commonly prescribed. Serum albumin levels and the type of fluid prescriber were the factors associated with the choice of crystalloids or colloids for fluid resuscitation.

Evaluation of HIF-1α and VEGF-A expression in radiation-induced cystitis: A case-control study

ABSTRACT The standard treatment for locally advanced cervical cancer (CC) is chemoradiotherapy. Once the bladder receives part of the radiation, a typical inflammatory condition that configures radiation-induced cystitis may develop. Chronic radiation-induced cystitis is commonly characterized by the bladder new submucosal vascularization, which is typically fragile and favors hematuria. The current study aims to investigate if Hypoxia-Induced Factor (HIF-1α) and its transcriptional target Vascular Endothelial Growth Factor A (VEGF-A) could be a primary pathway leading to increased submucosal vascularization. HIF-1α and VEGF-A mRNA levels in bladder core biopsies from CC patients treated with radiotherapy versus untreated (non-irradiated) patients were analyzed using a droplet digital polymerase chain reaction technology. Gene expression results showed that HIF-1α and VEGF-A had no significant differences between bladder samples from patients previously irradiated and untreated patient samples. However, a direct relationship between the degree of late morbidity and the expression of HIF-1α and VEGF-A has been demonstrated. Despite the lack of statistical significance precludes a definitive conclusion, the data presented herein suggests that further studies investigating the role of HIF-1α in bladder neovascularization in radiation-induced cystitis are highly recommended.

Abstract The Program for Biodiversity Research (PPBio) is an innovative program designed to integrate all biodiversity research stakeholders. Operating since 2004, it has installed long-term ecological research sites throughout Brazil and its logic has been applied in some other southern-hemisphere countries. The program supports all aspects of research necessary to understand biodiversity and the processes that affect it. There are presently 161 sampling sites (see some of them at Supplementary Appendix), most of which use a standardized methodology that allows comparisons across biomes and through time. To date, there are about 1200 publications associated with PPBio that cover topics ranging from natural history to genetics and species distributions. Most of the field data and metadata are available through PPBio web sites or DataONE. Metadata is available for researchers that intend to explore the different faces of Brazilian biodiversity spatio-temporal variation, as well as for managers intending to improve conservation strategies. The Program also fostered, directly and indirectly, local technical capacity building, and supported the training of hundreds of undergraduate and graduate students. The main challenge is maintaining the long-term funding necessary to understand biodiversity patterns and processes under pressure from global environmental changes.

RESUMO: Embora raros, os micoplasmas estão incluídos entre as causas de doenças respiratórias em répteis e, na ordem Squamata, já foram realizados três relatos destes microrganismos causando doença em pítons. Este estudo teve como objetivo avaliar a ocorrência de espécies de Mycoplasma em serpentes em cativeiro. Foram avaliadas 26 serpentes das famílias Pythonidae (13), Boidae (7), Viperidae (5) e Colubridae (1) do RioZoo, Brasil. Os animais foram examinados para determinar sinais clínicos consistentes com qualquer doença infecciosa. Amostras de swab traqueal de cobras foram coletadas em meio Frey e analisadas por isolamento microbiológico e pela técnica da PCR para identificar Mycoplasma spp. As amostras positivas para o gênero Mycoplasma spp. foram submetidas ao sequenciamento genético para identificação das espécies. No isolamento, 19,23% (5/26) foram positivos, enquanto 65,38% (17/26) dos animais foram positivos por PCR. Com base nas análises das seis sequências obtidas, houve similaridade com o Mycoplasma spp. descrito anteriormente em um píton e M. agassizii e M. testudineum encontrados em quelônios. Este é o primeiro relato de Mycoplasma spp. em animais das famílias Boidae e Viperidae. Mycoplasma spp. foi detectado em serpentes com e sem sinais clínicos. Os micoplasmas encontrados apresentaram semelhança genética com outros já descritos em répteis. Não houve relação entre a presença de Mycoplasma spp. e sinais clínicos.
ABSTRACT: Although rare, mycoplasmas are included among the causes of respiratory diseases in reptiles and, in the order Squamata, three reports of these microorganisms causing diseases in pythons have already been reported. This study aimed to evaluate the occurrence of Mycoplasma species in captive snakes. A total of 26 snakes of the families Pythonidae (13), Boidae (7), Viperidae (5) and Colubridae (1) from RioZoo, Brazil, were evaluated. Animals were examined to determine clinical signs consistent with any infectious disease. Tracheal swab samples from snakes were collected in Frey medium and analyzed for the presence of Mycoplasma spp.by isolation and a genus-specific PCR. DNA sequencing analyses of six positive samples by PCR were carried out to identify the species. Using isolation 19.23% (5/26) was positive, while 65.38% (17/26) of the animals were positive by PCR. Based on the analyses of the six sequences obtained, there was similarity with a Mycoplasma spp. previously described in a phyton and, M. agassizii and M. testudineum reported in chelonians. This is the first report of Mycoplasma spp. in animals of the families Boidae and Viperidae. Mycoplasma spp. were detected in snakes with and without clinical signs. The mycoplasmas reported resented identity (range, 95% to 100%) to others already described in reptiles. There was no relationship between the presence of Mycoplasma spp. and clinical signs.

ABSTRACT In spite of the chemical, physical, and biological benefits that Crotalaria spectabilis can provide to the soil, it is little used as a green manure crop by farmers. Inoculation with strains of legume-nodulating nitrogen-fixing bacteria that are efficient and competitive may be a strategy to enhance accumulation of N in C. spectabilis and stimulate adoption of this green manure crop. The aims of this study were i) to evaluate the symbiotic and agronomic efficiency of new strains of Bradyrhizobium on C. spectabilis in an oxisol (red latosol) compared to that of noninoculated controls (without and with mineral N) and with the approved strain BR2811, seeking to corroborate possible recommendation as inoculants for this species; and ii) to determine, the contribution of these treatments to N accumulation in the plant of C. spectabilis in four periods of cutting for determining possible and ideal periods for its incorporation in the soil. Experiments were carried out in pots and field. Inoculation with the new strains UFLA05-03, UFLA05-09, and UFLA05-14 and with BR2811 on C. spectabilis is effective since it increases the production of N-enriched plant biomass when compared to the control without N mineral. However, UFLA05-03 stands out among these strains because it behaves similarly to the control with mineral N both in relation to shoot N accumulation and dry matter just after 150 days in the field.

Resumo Objetivo Comparar as rotações medial e lateral dos ombros e as distâncias entre o processo coracoide e a fossa cubital de indivíduos não atletas e de jogadores profissionais de squash. Método O estudo transversal foi realizado entre março e agosto de 2017. Não atletas do sexo feminino e masculino (n = 628) foram selecionados no Serviço de Emergência Ortopédica da nossa instituição. Os critérios de inclusão foram: idade entre 18 e 60 anos, ausência de deficiências físicas ou cognitivas e ausência de dor nos membros superiores. Jogadores profissionais de squash (n = 30) de várias nacionalidades foram selecionados em um evento realizado em nossa cidade. Todos os atletas praticavam seu esporte em alto nível há > 10 anos e/ou 10.000 horas, e todos eram assintomáticos. Os dados demográficos e clínicos foram coletados por entrevista, enquanto os exames físicos e de ombro foram realizados por um único consultor ortopédico. Resultados Em comparação com os não atletas, os jogadores profissionais de squash apresentaram perdas médias significativas (p < 0,001) de 23º34' na rotação interna e significativos (p < 0,003) ganhos médios de 10º23' na rotação externa dos ombros dominantes. Houve diferença significativa (p < 0,008) entre não atletas e atletas quanto à distância entre o processo coracoide e a fossa cubital no braço dominante. Conclusão A participação intensiva no squash provoca alterações adaptativas que dão origem ao déficit de rotação interna glenoumeral, acompanhadas de significativo ganho de rotação externa, e podem gerar alterações patogênicas no ombro.
Abstract Objective To compare the medial and lateral rotations of the shoulders and the distances between the coracoid process and the cubital fossa of non-athletic individuals to those of elite squash players. Method The cross-sectional study was performed between March and August 2017. Male and female non-athletes (n = 628) were selected at the Orthopedic Emergency Service of our institution. The inclusion criteria were: age between 18 and 60 years, no physical disabilities or cognitive impairments and absence of pain in the upper limbs. Elite squash players (n = 30) of various nationalities were selected at an event held in our city. All of the athletes had practiced this sport under high performance requirements for > 10 years and/or 10.000 hours, and all were asymptomatic. Demographic and clinical data were collected through interviews, while physical examinations and shoulder assessments were performed by a single orthopedic practitioner. Results If compared with non-athletes, elite squash players presented significant (p < 0.001) mean losses of 23º34' in medial rotation and significant (p < 0.003) mean gains of 10º23' in lateral rotation of the dominant shoulders. There was a significant difference (p < 0.008) between non-athletes and athletes regarding the distance between the coracoid process and the cubital fossa in the dominant arm. Conclusion Intensive squash practice causes adaptive changes that trigger glenohumeral medial rotation deficit, accompanied by significant lateral rotation gain, and can generate pathogenic alterations in the shoulder.

ABSTRACT: Currently, the Brazilian savanna (Cerrado) represents the main agricultural area of the country, comprising a great variety of landscapes and soils, geological formations and vegetation patterns, as well as the major watershed. We studied the hydropedology and morphometry of a representative catchment (Frutal river), on a high tableland (Chapada) in the Triângulo Mineiro region, Brazil, describing the soil-water-landscape relationships to understand land use and water resources. To this end, we applied physical, chemical, micromorphological, and morphometric methods. When dry, compaction was observed in well-structured Ferralsols (Latossolos) with medium texture under intensive agriculture, reducing the water recharge capacity. The soil carbon stock was highest in hydromorphic savannas (veredas), reaching an organic matter content of 316.8 g kg-1 in the studied Umbric Gleysols, representing poorly drained lowlands. Physical and micromorphological properties were relevant parameters to understand the water recharge in soil; in agricultural fields, bulk density tended to increase and hydraulic conductivity to decrease, particularly under long-term sugarcane; morphometric parameters in the Frutal catchment indicated a low flooding risk and high flow capacity. This reinforces the need for soil conservation strategies to enhance water infiltration and groundwater recharge, with a view to maintain the water longer in the catchment. For surface water dynamics, slope morphology is an important property, affecting soil erosion, water retention and crop productivity.

RESUMO: A artrite encefalite caprina causa perdas consideráveis para a produção caprina. A principal forma de transmissão do vírus da artrite encefalite caprina é a ingestão de colostro ou leite de fêmeas infectadas. No entanto, algumas transmissões não podem ser explicadas por esta via. Dessa forma, este estudo teve como objetivo avaliar a transmissão do vírus da artrite encefalite caprina por via transplacentária (vertical). Foram realizadas coletas de sangue em 283 crias recém-nascidas das raças Anglo-Nubiana e Saanen, provenientes de progenitores soropositivos e soronegativos. As amostras foram coletadas logo após o nascimento e analisadas pelas técnicas de imunodifusão em gel de agarose e western blot. No teste de imunodifusão em gel de agarose, nenhum cabrito foi detectado reagente. Porém, no teste de western blot, quatro cabritos nasceram soropositivos. Esse resultado indica que, apesar de baixa frequência (1,4%), existe a possibilidade de transmissão via transplacentária do lentivírus de pequenos ruminantes.
ABSTRACT: Caprine arthritis encephalitis causes considerable losses in goat production. The main form of the caprine arthritis encephalitis virus transmission is through the ingestion of colostrum or milk from infected females. However, some transmissions cannot be explained in this manner. Therefore, this study aimed to evaluate transplacental transmission of caprine arthritis encephalitis virus. Blood samples were collected from 283 newborn kids of Anglo-Nubian and Saanen breeds born from seropositive and seronegative goats. Samples were collected immediately after birth and analyzed with agarose gel immunodiffusion and western blot. All samples were negative in the agarose gel immunodiffusion. However, the western blot test demonstrated that four kids were born positive for caprine arthritis encephalitis virus. This result indicates that although in a low frequency (1.4%), there is a possibility of transplacental transmission of small ruminant lentivirus.

The objective of this work was to evaluate the performance of piglets in the nursery phase submitted to activation of immune system with vaccine against Haemophilus parasuis and to different levels of metabolizable energy in the diets. It was used 160 barrows and females at 30 days of age, weaned at 23 days of age, with initial weight of 7.97 ± 0.54 kg, distributed in 2 × 4 factorial arrangement (with vaccination and without vaccination with four levels of metabolizable energy - 3,200; 3,300; 3,400 and 3,500 kcal/kg). Each combination was evaluated with five repetitions of four animals per experimental plot (two males and two females). Energy levels reduced linearly the intake, feed conversion and the total number of leukocytes in blood in the experimental period, regardless of the use of vaccine which in turn reduced daily average weight gain only in the first week after second application and it increased the number of limonocytes and spleen weight after three weeks. There was no difference in the C-reactive protein quantity in the blood in the periods when it was evaluated. The use of vaccine reduces performance immediatelly after its application but after it occurs recovery of performance with changes in the number of monocytes and spleen weight. Dietary energy levels do not smooth this reduction in the performance, but they influence intake, feed conversion and blood parameters in the total period of use.
