Abstract Background: The Mexican Society of Pediatric Endocrinology presents recommendations for the diagnosis and treatment of precocious puberty (PP), a condition defined as the development of sexual characteristics due to an increase in pituitary gonadotropin secretion before 8 or 9 years of age in girls and boys, respectively. Methods: Three systematic reviews were conducted: controlled clinical trials on interventions for PP treatment, diagnostic tests, and observational studies on the long-term effects of PP. The quality evaluation and data extraction from the studies were conducted by two independent reviewers. The Scottish Intercollegiate Guidelines Network and the Oxford Center for Evidence-Based Medicine systems were used for grading the quality of evidence for recommendations on intervention and diagnosis, respectively. Recommendations were submitted to a consensus by a Delphi method and were validated by another 143 certified pediatric endocrinologists through an online questionnaire. Results: The group generated 12 recommendations on the diagnosis of PP, seven on the diagnosis of secondary causes of PP, eight on interventions for inhibition of puberty, five on other interventions for PP treatment, and 14 for the monitoring and follow-up of these patients. The online questionnaires submitted to certified pediatric endocrinologists showed more than 90% of approval for each one of the recommendations. Conclusions: Although a high degree of consensus for the recommendations for diagnosis, treatment, and monitoring of PP among pediatric endocrinologists was achieved, most of these recommendations showed a low level of evidence.

Resumen Introducción: La Sociedad Mexicana de Endocrinología Pediátrica presenta recomendaciones para el diagnóstico y el tratamiento de la pubertad precoz (PP), condición definida como el desarrollo de caracteres sexuales por incremento en la secreción hipofisiaria de gonadotropinas antes de los 8 años en las niñas y de los 9 años en los niños. Métodos: Se realizaron tres revisiones sistemáticas de ensayos clínicos controlados sobre intervenciones para el tratamiento de la PP, pruebas diagnósticas y estudios observacionales sobre efectos a largo plazo de la PP. La evaluación de la calidad de los estudios y la extracción de datos se realizó por pares. La evidencia se graduó con el sistema de la Scottish Intercollegiate Guidelines Network (SIGN) y del Oxford Centre for Evidence-Based Medicine (OCEBM) para las recomendaciones sobre la intervención y el diagnóstico, respectivamente. Las recomendaciones generadas se sometieron a un consenso por el método Delphi y fueron validadas por otros 143 endocrinólogos pediatras certificados mediante un cuestionario en línea. Resultados: Mediante consenso se generaron 12 recomendaciones para el diagnóstico de PP, siete sobre diagnóstico de causas secundarias de PP, ocho sobre intervenciones para inhibición de la pubertad, cinco sobre otras intervenciones en PP y 14 para la monitorización del tratamiento y el seguimiento de estos pacientes. Se obtuvo más del 90% de aprobación para cada una de las recomendaciones por el grupo de endocrinólogos certificados que respondieron el cuestionario en línea. Conclusiones: Si bien se logró un alto grado de consenso para las recomendaciones para el diagnóstico, el tratamiento y la monitorización de la PP entre los endocrinólogos pediatras, el nivel de evidencia para la mayoría de estas recomendaciones resultó bajo.

Abstract Three systematic reviews were conducted to formulate the recommendations on diagnosis, treatment and follow-up of patients with precocious puberty: interventions for the treatment of precocious puberty that included the outcomes of final or near-final height, mental health, metabolic health, health bone, or blockade success; comparative observational studies evaluating long-term outcomes in subjects with a history of precocious puberty; and diagnostic test accuracy studies for puberty.

Resumen Se realizaron tres revisiones sistemáticas para la formulación de las recomendaciones sobre diagnóstico, tratamiento y seguimiento de pacientes con pubertad precoz: intervenciones para el tratamiento de la pubertad precoz que incluyeran los desenlaces de talla final o casi final, salud mental, salud metabólica, salud ósea o éxito en el bloqueo; estudios observacionales comparativos que evaluaran desenlaces a largo plazo en sujetos con antecedentes de pubertad precoz; y por último, estudios de exactitud de prueba diagnóstica para pubertad.

Resumen Parachondria neglectus es un molusco terrestre de la familia Annulariidae, endémico de Cuba y microlocalizado en la provincia de Granma. En la presente investigación se caracterizaron la dinámica temporal del subnicho etario y algunos aspectos de la biología reproductiva de una población de P. neglectus localizada en Manzanillo, GranmaCuba. Se realizaron 30 expediciones entre diciembre 2013 y febrero 2015. Las comparaciones del largo (t = 8.05, P = 0.007) y diámetro mayor (t = 6.24, P = 0.0001) de las conchas de los juveniles truncados y los juveniles completos mostraron diferencias estadísticas significativas, indicando que ambos grupos representan dos clases de edades diferentes. La comparación del diámetro mayor de las conchas de los adultos truncados y los adultos completos (t = 1.50, P = 0.16) demostró que conforman un misma clase de edad en la población. La existencia de diferencias estadísticas significativas entre el largo (t = 19.45, P = 0.0003) y el diámetro mayor (t = 13.19, P = 0.006) de las conchas de las hembras y los machos reafirmaron el dimorfismo sexual en la especie, resultando las hembras de mayores dimensiones respecto a los machos. La mancha circular blancuzca en las conchas es una característica morfológica externa válida para diferenciar a las hembras adultas y ya apareadas en ocasiones anteriores, del resto de los miembros de la población. El período reproductivo comprendió de julio a diciembre 2014. Las cópulas se observaron de julio a octubre 2014 y estuvieron asociadas a condiciones de precipitaciones abundantes y temperaturas altas del verano. El período de reclutamiento ocurrió de septiembre a diciembre 2014.

Abstract Parachondria neglectus is a land snail of the Annulariidae family, endemic to Cuba and microlocalized in Granma province. In the present research, the temporal dynamics of age-related subniche and some aspects of the reproductive biology of a P. neglectus population, located in ManzanilloGranma-Cuba, were characterized. Thirty expeditions were performed from December 2013 to February 2015. The comparisons between the length (t = 8.05, P = 0.007) and biggest diameter (t = 6.24, P = 0.0001) of the shells of truncated juveniles and the shells of full juveniles showed significant statistical differences, indicating that both groups represent two kinds of different ages. The comparison between the biggest diameter of the shells of truncated adults and the shells of full adults (t = 1.50, P = 0.16) showed that both groups represent the same kind of age in the population. The existence of significant statistical differences between the length (t = 19.45, P = 0.0003) and the biggest diameter (t = 13.19, P = 0.006) of the females’ shells and the males’ shells verify the sexual dimorphism in the species, proving to be the females of bigger dimensions in relation to the males. The whitish circular spot in the shells is a valid external morphological characteristic to differentiate the adult females already mated in previous occasions from the rest of the members of the population. The reproductive period was from July to December 2014. Copulations were observed from July to October 2014 and they were associated to conditions of abundant precipitations and high temperatures of summer. The recruitment period took place from September to December 2014.

Abstract In this work, a finite element-based approach is presented to study the effective width variation in non-pre-stressed steel-concrete beams under the serviceability stage, including time dependent effects such as concrete creep, shrinkage and cracking. For this purpose, the viscoelasticity theory in conjunction with a nonlinear cracking monitoring algorithm is used to trace the nonlinear viscoelastic response of the structure along time. The present numerical model is fully three-dimensional and permits the inclusion of partial interaction at the slab-beam interface. A comprehensive study is carried out on the long-term response of a composite girder bridge previously studied by other researches. Then, previous results are revised and extended herein. Potential shortcomings of some standard codes related to the effective width evaluation are also investigated. It is demonstrated that the slab effective width varies sharply along the beam axis in the short-term, while it approaches to the actual slab width in the long-term. For the studied example, the common assumption of using only the middle layer of the reinforced concrete (RC) slab for the effective width calculation is revised with a through-thickness integration procedure. The influence of some creep and shrinkage models as well as the ultimate tensile concrete strain on the effective width response is also assessed. Finally, a simple formula is proposed to evaluate the short-term slab effective width for the studied example.

The objective of this work is to provide a reliable numerical model using the finite element method (FEM) for the static and dynamic analysis of reinforced concrete (RC) shells. For this purpose two independently computer programs based on plasticity and viscoplasticity theories are developed. The well known degenerated shell element is used for the static analysis up to failure load, while 3D brick elements are used for the dynamic application. The implicit Newmark scheme with predictor and corrector phases is used for time integration of the nonlinear system of equations. Two benchmark examples analyzed by others are solved with the present numerical model and results are compared with those obtained by other authors. The present numerical model is able to reproduce the path failure, collapse loads and failure mechanism within an acceptable level of accuracy.

Avaliou-se a atividade antimicrobiana de 13 extratos totais, 10 brandos (B) e 3 batidos (E) elaborados a partir da folha seca e fresca da Petiveria alliacea L. Para sua preparação se empregaram vários solventes: solução hidroalcoólica 30%, 80% e álcool isopropílico. O efeito antimicrobiano dos extratos foi comprovado pelo método do Kirby-Bauer, usando para isso quatro cepas bacterianas da coleção ATCC (Escherichia coli, Staphylococcus aureus, Enterococcus faecalis e Pseudomonas aeruginosa) e um fungo leveduriforme (Candida albicans). Determinaram-se parâmetros de controle da qualidade físicos, físico-químicos e químicos dos extratos mais ativos. Obtiveram-se como resultados: que 9 extratos mostraram atividade antibacteriana, sendo os mais concentrados (B8 e E3) os de maior espectro de ação frente às bactérias provadas; o efeito dos extratos batidos (E1, E2 e E3) foi maior frente a P. aeruginosa; os extratos batidos se consideram mais ativos que os brandos e não se obteve atividade antifúngica caso algum. Determinou-se a Concentração Mínima Inhibitoria e Bactericida para ambos os extratos, resultando a primeira para os extratos brandos maior a 100 mg/mL e para as batidos maior a 50 mg/mL; a segunda para os extratos brandos maior ou igual a 400 mg/mL e para as batidos maior ou igual a 200 mg/mL, em relação à droga fresca.

The antimicrobial activity of 13 total extracts was evaluated, 10 soft extracts (B) and 3 blended extracts (E) prepared from dry and fresh leaves of Petiveria alliacea L. Various solvents were used for their preparation: hydroalcoholic solution at 30%, 80% and isopropyl alcohol. The antimicrobial effect of the extracts was tested by means of the method of Kirby-Bauer, using four bacterial strains from the ATCC collection (Escherichia coli, Staphylococcus aureus, Enterococcus faecalis and Pseudomonas aeruginosa) and a leveduriform fungus (Candida albicans). The following quality control parameters were determined for most active extracts: physical, physical-chemical and chemical parameters. The results were: nine extracts showed antibacterial activity, being the most concentrated (B8 and E3), the ones with the highest activity in the presence of the bacteria tested; the effect of blended extracts (E1, E2 and E3) was greater in the presence of P. aeruginosa. Blended extracts are considered more potent and active than soft extracts. No antifungal activity was obtained for both types of extracts. The Minimum Inhibitory Concentration (MIC) and Minimum Bactericidal Concentration (MBC) were determined for both extracts, with the following results: MIC-soft extracts (>100 mg/mL), blended extracts (>50 mg/mL); MBC-soft extracts (≥400 mg/mL), blended extracts (≥200 mg/mL) based on fresh leaves.
