O objetivo deste estudo foi comparar os efeitos clínicos da indução com propofol em nanoemulsão e em emulsão lipídica em gatas, após repetidas administrações. Utilizaram-se 12 gatas, hígidas, SRD, com peso médio de 2,9±0,6kg, distribuídas aleatoriamente em dois grupos: NANO (n=6) e EMU (n=6), que receberam propofol em nanoemulsão na dose de 9,5mg kg-1 e em emulsão lipídica na dose de 10mg kg-1, respectivamente, ambos pela via intravenosa, durante cinco dias consecutivos. Hemograma e função renal e hepática foram realizados a cada 24 horas e até 168 horas após a primeira indução. Os parâmetros clínicos de frequência cardíaca (FC) e respiratória (f) e temperatura retal (TR) foram avaliados antes dos tratamentos e 5, 10, 20 e 30 minutos após a administração de propofol. Em relação aos valores basais, observou-se diminuição da FC no EMU até o 3° dia e da f e TR em ambos os grupos até o último dia, sem diferença entre os dias. As enzimas ALT e FA diminuíram após 24 a 96 horas e 48 a 144 horas, respectivamente, no grupo EMU. Os valores de eritrograma diminuíram até o quinto dia em ambos os grupos. Os tempos para hipnose, extubação e para decúbito esternal não diferiram entre grupos e entre os dias. O tempo para recuperação total do grupo EMU foi maior no 4° e 5° dias em relação ao 1°. Conclui-se que a indução com propofol é segura e que o propofol em emulsão lipídica apresenta maior efeito cumulativo após repetidas administrações em gatas.

The aim this study was to compare the clinical effects of induction with propofol in lipid emulsion and nanoemulsion in cats after repeated administrations. Twelve healthy mongrel cats, with an average weight of 2.9±0.6kg, were randomly distributed into two groups: NANO (n=6) and EMU (n=6), who received propofol in nanoemulsion at dose of 9.5mg kg-1 and in lipid emulsion at a dose of 10mg kg-1, respectively, both intravenously for five consecutive days. Blood count and kidney and liver function were performed every 24 hours until 168 hours after the first induction. The clinical parameters of heart rate (HR), respiratory rate (RR) and rectal temperature (RT) were evaluated before treatment and 5, 10, 20 and 30 minutes after administration of propofol. At baseline, there was a decrease in HR in the EMU until the 3rd day and RR and RT in both groups until the last day, with no difference between days. The enzymes ALT and ALP decreased after 24 to 96 hours and 48 to 144 hours, respectively in EMU group. The values of blood count decreased until the fifth day in both groups. The times of hypnosis, extubation and sternal recumbency did not differ between groups and between days. The total recovery time of EMU group was higher in the 4th and 5th day in relation to the 1st. Concludes that the induction with propofol is safe and that propofol in lipid emulsion has a higher cumulative effect after repeated administration in cats.

O presente estudo objetiva avaliar os efeitos cardiorrespiratórios e metabólicos do propofol em emulsão lipídica e microemulsão em gatas. Foram utilizadas 12 gatas, hígidas, adultas, alocadas em dois grupos: microemulsão (MICRO, n=6) e emulsão lipídica (EMU, n=6), os quais receberam propofol, na respectiva formulação, em dose suficiente para intubação. Em seguida, foram intubados, fornecendo-se oxigênio 100%, em sistema sem reinalação de gases. Ato contínuo, iniciou-se a infusão de propofol na dose de 0,3mg kg-1 min-1 durante 90 minutos. A dose necessária para indução foi de 9,5±1,3mg kg-1 e 10±1mg kg-1 para MICRO e EMU, respectivamente. Os valores de pressão arterial sistólica (PAS), pressão arterial média (PAM), pressão arterial diastólica (PAD) e pH foram menores, em todos os momentos, no EMU em relação ao MICRO; a f do EMU foi menor de T30 até T75 em relação ao MICRO. A PaCO2 do EMU foi maior de T15 até T90. Os tempos de extubação, decúbito esternal, deambulação e recuperação total foram de 40,6±30,7; 91±37,5; 134,5±54,5 e 169,1±55,4 minutos no MICRO e de 68,8±37,3; 133,3±85,3; 171,3±77,1 e 233,1±60,6 minutos no EMU, respectivamente. Houve aumento da enzima Alanina Aminotransferase de 12 às 72h no EMU e de 48 às 72h no MICRO. O propofol em microemulsão apresentou características clínicas de indução e manutenção, bem como efeitos metabólicos semelhantes à formulação em emulsão lipídica. A formulação em microemulsão proporcionou maior estabilidade cardiovascular e respiratória para indução e infusão contínua em gatas hígidas.

The aim of the present study was to evaluate the cardiorespiratory and metabolic effects from lipid emulsion and microemulsion of propofol in cats. Twelve healthy adult cats were included, and divided into two groups: microemulsion group (MICRO, n=6) and lipid emulsion (EMU, n=6), where they received propofol in the respective formulation, in a dose sufficient for intubation. The animals were then intubated and provided with 100% oxygen through a non-rebreathing circuit. Immediately after, the infusion of propofol was initiated (0.3mg kg-1 min-1) and maintained for 90 minutes. The dose required for induction was 9.5±1.3mg kg-1 and 10±1mg kg-1 in MICRO and EMU, respectively. The SAP, MAP, DAP and pH values were lower in all moments in EMU when compared to MICRO; the RR in EMU was lower from T30 to T75 in comparison to MICRO. The paCO2 was greater in the EMU from T15 to T90. The times to extubation, sternal recumbency, ambulation and total recovery were 40.6±30.7, 91±37.5, 134.5±54.5 and 169.1±55.4 minutes in MICRO and 68.8±37.3, 133.3±85.3, 171.3±77.1 and 233.1±60.6 minutes in EMU, respectively. There was an increase of the enzyme alanine aminotransferase from 12 to 72 hours in EMU and in MICRO from 48 to 72 hours. The propofol in microemulsion presents clinical characteristics of induction and maintenance, and metabolic effects similar to the formulation in lipid emulsion. The microemulsion formulation provides a better cardiovascular and respiratory stability for induction and continuous infusion in healthy cats.

Este estudo tem o objetivo de avaliar os efeitos clínicos e cardiorrespiratórios de uma formulação de propofol formada por nanopartículas, em cães. Para esse propósito, seis cães hígidos, adultos, fêmeas, com peso médio de 14,8±1,2kg foram utilizados. Cada cão recebeu dois tratamentos, sendo que entre estes foi permitido aos animais um período de washout de 15 dias. Os animais receberam propofol em microemulsão (MICRO) ou em emulsão lipídica (EMU) por via intravenosa (IV) para a indução e manutenção da anestesia. A anestesia foi mantida com velocidade de infusão constante (0,4mg kg-1 min-1). As variáveis cardiorrespiratórias foram mensuradas antes da indução (basal), imediatamente após a indução, e então a cada 15 minutos, durante 90 minutos. Amostras de sangue arterial também foram colhidas para análise de gases sanguíneos, exceto aos 45 e 75 minutos após a indução. A pressão arterial média diminuiu significativamente com a utilização de ambos os tratamentos, enquanto o índice cardíaco reduziu significativamente somente com o tratamento MICRO. O tempo necessário para extubação, decúbito esternal, deambulação e recuperação total foi semelhante em ambos os grupos. Opistótono foi observado em 33% dos animais em cada tratamento. A microemulsão de propofol apresentou parâmetros clínicos e respiratórios semelhantes àqueles obtidos com a emulsão lipídica comercialmente disponível, porém mostrou algumas características hemodinâmicas diferentes, quando utilizada para a indução e manutenção da anestesia. Com base somente nesses resultados, não são observadas vantagens na utilização da microemulsão.

This research aimed to evaluate the clinical and cardiorespiratory effects of a propofol formulation with nanometer droplet diameter in dogs. Six adult healthy female dogs weighing 14.8±1.2kg were used in this study. Each dog received two treatments with a 15-day washout period. A microemulsion (MICRO) or lipid emulsion (EMU) of propofol was administered intravenously (IV) for induction and maintenance of anesthesia. Anesthesia was maintained with a constant rate infusion of propofol (0.4mg kg-1 minute-1). Cardiorespiratory variables were recorded before induction (baseline), immediately after and at 15-minute intervals for 90 minutes after treatment. Arterial blood samples were also taken for blood gas analysis, except at 45 and 75 minutes after induction. The mean arterial pressure decreased significantly during both treatments, while the cardiac index decreased significantly only in MICRO treatment. The time to extubation, sternal recumbency, ambulation and total recovery was similar in both treatments. Opisthotonos was observed in 33% of the animals in each treatment. The propofol microemulsion presented clinical and respiratory parameters similar to those obtained with the lipid emulsion commercially available, but had some significantly different hemodynamic characteristics when used for inducing and maintaining anesthesia. Based only in these results, no advantages are seen in the use of this new microemulsion.

A castração de machos e fêmeas tem sido preconizada como a principal técnica para redução do grande número de cães errantes. No entanto, vários são os entraves com relação à escolha do melhor protocolo anestésico, em relação à eficácia, segurança e redução de custos. Objetivou-se, com este trabalho, avaliar os efeitos cardiorrespiratórios, hemogasométricos e analgésicos da utilização de lidocaína em um volume maior, associada à morfina, pela via epidural em cadelas submetidas à ovariosalpingohisterectomia (OSH), com ou sem suplementação de oxigênio. Utilizaram-se 12 cadelas, com peso médio de 11,5±3,7kg e idade de um a quatro anos. Os animais receberam como medicação pré-anestésica (MPA) acepromazina (0,1mg kg-1) e meperidina (5mg kg-1), pela via intramuscular. Após 15 minutos, administrou-se tiopental (10mg kg-1), por via intravenosa, seguido de intubação endotraqueal. Nesse momento, os animais foram alocados em dois grupos: o grupo GCO (com oxigênio, n=06) recebeu suplementação de oxigênio 100% e o grupo GSO (sem oxigênio, n=06) permaneceu intubado sem suplementação de oxigênio. Após a intubação, foi administrada, pela via epidural, em ambos os grupos, morfina (0,1mg kg-1) em volume final ajustado para 1mL 3,0kg-1 de peso com lidocaína 2% sem vasoconstritor. Imediatamente após a anestesia epidural, os animais foram posicionados em decúbito dorsal com a cabeça no mesmo plano do corpo, e iniciou-se o procedimento cirúrgico, o qual foi padronizado em 30 minutos. Em ambos os grupos, foi possível realizar a cirurgia sem a necessidade de complementação analgésica e sem resposta de dor. A pressão arterial média (PAM) foi menor no GSO em todos os momentos em comparação ao basal. No GCO, a PAM foi menor após MPA e após epidural. Houve redução da f de M1 até M6 no GSO. A SaO2 e PaO2 foram maiores no GCO em comparação ao GSO. O pH foi menor no GCO 15 minutos após MPA até 40 minutos após epidural, em comparação ao GSO. Conclui-se que a anestesia epidural lombossacra com morfina e lidocaína na dose e no volume propostos é efetiva para realização de OSH em cadelas, com mínimas alterações cardiovasculares e hemogasométricas, as quais são bem toleradas em animais hígidos. Essa prática é exequível em campanhas de castração em que não há possibilidade de oxigenação dos animais.

Male and female sterilization have been established as the main technique to reduce the huge number of mongrel dogs. However, there are several barriers regarding to the choice of the best anesthetic protocol, in terms of efficacy, security and cost reduction. The aim of this study was to evaluate the analgesic, cardiorespiratory and hemogasometric effects of epidural anesthesia with a large volume of lidocaine in combination with morphine in female dogs submitted to ovariosalpingohisterectomy (OSH), with or without oxygen supplementation. Twelve adult female dogs were used, with average weight of 11.5±3.7kg and age of 1 to 4 years. The animals received acepromazine (0.1mg kg-1) and meperidine (5mg kg-1) as premedication, administered by the intramuscular route. Fifteen minutes later, thiopental (10mg kg-1) was administered by intravenous route, followed by endotracheal intubation. The animals were allocated into two groups: GCO (group with oxygen, n=06), where the animals received 100% oxygen supplementation and GSO (group without oxygen supplementation, n=6), mantained with endotracheal intubation, but without oxygen supplementation. After intubation, epidural with morphine (0,1mg kg-1), adjusted to 1mL 3kg-1 in lidocaine 2% without epinepherine was administered. Immediately after epidural administration, the animals were positioned in dorsal recumbency, with the head in the same level of the body. In both groups, surgery was performed without the need of additional analgesia and without signs of pain. Mean arterial pressure (MAP) was lower in GSO in all moments compared to baseline values. In GCO, MAP was lower after premedication and epidural anesthesia. Respiratory rate was lower from M1 to M6 in GSO. The SaO2 and PaO2 were higher in GCO when compared to GSO. The pH was lower in GCO after 15 minutes after premedication, until 40 minutes after the epidural administration, compared to GSO. It was concluded that the epidural administration using morphine and lidocaine in the dosage and volume proposed are effective to OSH procedure in dogs. The lack of oxygen supplementation promoted minimal cardiovascular and hemogasometric alterations, which is feasible in sterilizations campaigns where there is no possibility of oxygen supplementation of the animals.

O objetivo deste estudo foi comparar os efeitos da expansão volêmica produzida pelo hidroxietilamido 130/0,4 (HES 130/0,4) ou pelo sangue em gatas com hipovolemia induzida. Foram utilizadas 12 gatas adultas, sem raça definida (SRD), com peso médio de 2,85±0,28kg e hígidas. Os animais foram induzidos à anestesia geral com isofluorano a 5V%, intubados e conectados a um sistema sem reinalação de gases. Após a instrumentação, os animais foram mantidos sob anestesia com isoflurano em 1,3V% e mantidos em ventilação mecânica, ciclada a pressão. Em seguida, foi induzida a hipovolemia por meio da retirada de 30ml kg-1 de sangue da artéria femoral. Após 60 minutos da estabilização do paciente, os tratamentos foram iniciados. No grupo hidroxietilamido (GH, n=06), os animais receberam, como reposição volêmica, o hidroxietilamido 130/0,4 no mesmo volume de sangue retirado e, no grupo sangue (GS, n=06), os animais receberam o próprio sangue retirado, sendo considerado grupo controle. A pressão arterial sistólica, a diastólica e a média e a pressão venosa central aumentaram após a reposição volêmica em ambos os grupos. Observou-se, para o GH, aumento da PaCO2 no T15, no T30 e no T60. Houve redução do pH no T30 e no T45 e de íons Na+ no T90 para GH. A restauração das pressões arteriais com a administração de HES 130/0,4 foi similar ao grupo controle. A reposição volêmica com HES 130/0,4 produz aumento acentuado da PVC; e o uso do HES 130/0,4 em gatas submetidas à hipovolemia não produziu alterações clinicamente significativas no equilíbrio ácido-básico.

The aim of this study was to compare the volemic expansion effects produced by hydroxyethyl starch 130/0.4 (HES 130/0.4) or blood, in female cats with induced hypovolemia. Twelve healthy adult female cats, crossbreed and weighing an average of 2.85±0.28kg were used. They were induced into general anesthesia with isofluorane at 5V%, intubated and connected to a non-rebreathing system. After instrumentation, the animals were maintained under anesthesia with isofluorane at 1.3V% and maintained on pressure cycled mechanic ventilation. Afterwards, hypovolemia was induced by withdrawal of 30ml kg-1 of blood from the femoral artery. After 60 minutes of stabilization of the patient, the treatments were initiated. In the hydroxyethyl starch group (GH, n=06) the animals received hydroxyethyl starch 130/0.4 as volemic expansion at the same volume of blood withdrawed, in the blood group (GS, n=06) the animals received their own withdrawed blood, being considered the control group. The systolic, diastolic and mean arterial pressures and central venous pressure increased after volemic expansion in both groups. An increase of PaCO2 at T15, T30 and T60 in GH was observed. In addition, there was reduction of pH at T30 and T45 and Na+ ions at T90 in GH. The arterial pressure restoration with the use of HES 130/0.4 was similar to the control group; the volemic expansion with HES 130/0,4 produces accentuated increase of CVP; the use of HES 130/0,4 in female cats submitted to hypovolemia did not produce clinically significant alterations in acid-base equilibrium.
