ABSTRACT Introduction: New-onset postoperative atrial fibrillation (POAF) is a common complication following coronary artery bypass grafting (CABG) surgery. Objective: To explore predictive factors and potential mechanisms of new-onset POAF in isolated off-pump CABG patients. Methods: Retrospective observational case-control study of 233 patients undergoing isolated off-pump CABG surgery between August 2018 and July 2020 at the Department of Thoracic and Cardiovascular Surgery, the Affiliated Drum Tower Hospital of Nanjing University Medical School. Associations between predictor variables and new-onset POAF were identified. The main outcome was new-onset POAF after CABG surgery. Results: A total of 75 (32.19%) patients developed new-onset POAF after CABG surgery. The new-onset POAF patients had advanced age, higher baseline systolic blood pressure, more preoperative use of diuretic drug, more transfusion of blood products, atrial dilation and postoperative positive inotropic drug treatment. Nineteen variates entered the multivariable logistic regression model with a Hosmer-Lemeshow test score of 7.565 (P=0.477). Postoperative left atrial enlargement, postoperative drainage in the first 24 hours and total length of hospital stay were statistically significant, while postoperative right atrial enlargement (OR and 95% CI, 7.797 [0.200, 304.294], P=0.272) and left atrial enlargement (3.524 [1.141, 10.886], P=0.029) assessed by echocardiography had the largest OR value. Conclusion: Atrial enlargement is strongly associated with new-onset POAF in patients with isolated off-pump CABG, thus it highlights the advantage of echocardiography as a useful tool for predicting new-onset POAF. Careful monitoring and timely intervention should be considered for these patients.

Abstract Using Bainong 365 wheat starch as raw material, starch samples were preheated in a RVA to simulate DSC heating profiles. The thermodynamic properties, long-range orderliness, short-range orderliness, and structure of wheat starch were determined by DSC, XRD, FTIR, LF-NMR and SEM to explore the phase transformation mechanism of Bainong 365 wheat starch at different water contents. The results show that at different moisture contents, when the endothermic transition temperature of starch samples determined by DSC was reached or exceeded, the enthalpy of starch was 0, all free water was converted into uneasy flowing water and bound water, and the surface structure of starch was severely damaged. At this time, starch was completely gelatinized, but the short-range and long-range ordered structures of starch determined by FTIR and XRD still existed and gradually decreased with increasing temperature. Therefore, it was concluded that the temperature range of the starch endothermic transition does not represent the temperature range of complete gelation of starch, and the structure destroyed by complete gelation of starch may not be simple short-range and long-range ordered molecular structures.

Abstract To study the maternal and neonatal outcomes of pregnancy complicated with Systemic Lupus Erythematosus (SLE). Clinical data of 84 pregnant women complicated with SLE were collected. All patients were divided into the different groups based on the pregestational status of SLE, SLE status during pregnancy and the type of medicine administration respectively. The incidence rates of pregnancy loss and preterm labor in the pregestational and gestational stable SLE groups were significantly lower and the incidence rate of full-term labor was considerably higher than those in the active SLE groups. In the pregestational stable SLE group, the incidence of PIH was significantly lower than that in the active SLE group. The incidence of PIH and FGR in the gestational active SLE group was dramatically higher than that in the gestational stable SLE group. Among 84 women, 56 (66.7%) received cesarean section, 18 (21.4%) vaginal delivery and 10 (11.9%) labor induction. The status of SLE is intimately correlated with maternal and neonatal outcomes. PIH is a major complication of active SLE and associated with adverse pregnancy outcomes. SLE can cause congenital fetal injury, especially congenital heart block.

Abstract The study was designed to investigate the effects of cryopreservation on bovine, ovine, and goat sperm motility, acrosome structure, enzyme activity, and fertilization ability. Percentage of sperm with hyaluronidase enzyme (HYD) activity was detected by a modified sodium hyaluronate-gelatin membrane. The N-α-benzoyl-DL-arginine-p-nitroanilide (BNPNA) method was used to assess the sperm acrosome enzyme (ACE). The mean percentage of sperm acrosome integrity dropped significantly (P < 0.01) after cryopreservation. The ACE activity of bovine sperm (100.48) was higher (P < 0.01) than that of ovine (57.88) or goat sperm (50.30), while the percentage of sperm with HYD activity of bovine (71.10%) and ovine (67.60%) sperm was higher than that of goat sperm (58.52%) after cryopreservation (P < 0.01). Sperm motility was positively correlated with the activity of the two acrosome enzymes before and after cryopreservation (P < 0.01). Cryopreservation had a negative effect on acrosomal morphology, motility, and acrosomal enzyme activity in their sperm. The fertilization ability of ovine and goat sperm decreased significantly after cryopreservation, but that of frozen bovine sperm did not differ significantly when compared with fresh sperm. There was no significant difference between ovine and goat sperm indices, except for percentage of sperm with HYD activity.

ABSTRACT Objective: The aim was to characterize blood glucose fluctuations in patients with fulminant type 1 diabetes (FT1DM) at the stable stage using continuous blood glucose monitoring systems (CGMSs). Subjects and methods: Ten patients with FT1DM and 20 patients with classic type 1 diabetes mellitus (T1DM) (the control group) were monitored using CGMSs for 72 hours. Results: The CGMS data showed that the mean blood glucose (MBG), the standard deviation of the blood glucose (SDBG), the mean amplitude glycemic excursions (MAGE), the blood glucose areas and the percentages of blood glucose levels below 13.9 mmol/L were similar between the two groups. However, the percentage of blood glucose levels below 3.9 mmol/L was significantly higher in the FT1DM group compared to the T1DM group (p < 0.05). The minimum (Min) blood glucose level in the FT1DM group was significantly lower than that of the T1DM group (p < 0.05). Patients with FT1DM had severe dysfunction of the islet beta cells and alpha cells compared to patients with T1DM, as indicated by lower C-peptide values and higher glucagon/C-peptide values. Conclusion: In conclusion, patients with FT1DM at the stable stage were more prone to hypoglycemic episodes as recorded by CGMSs, and they had a greater association with severe dysfunction of both the beta and alpha islet cells compared to patients with T1DM.

OBJECTIVE: The angiotensin-converting enzyme gene is one of the most studied candidate genes related to atrial fibrillation. Among the polymorphisms of the angiotensin-converting enzyme gene, the 2350 G/A polymorphism (rs4343) is known to have the most significant effects on the plasma angiotensin-converting enzyme concentration. The aim of the present study was to investigate the association of the angiotensin-converting enzyme 2350 G/A polymorphism with atrial fibrillation in Han Chinese patients with essential hypertension. METHODS: A total of 169 hypertensive patients were eligible for this study. Patients with atrial fibrillation (n = 75) were allocated to the atrial fibrillation group, and 94 subjects without atrial fibrillation were allocated to the control group. The PCR-based restriction fragment length polymorphism technique was used to assess the genotype frequencies. RESULTS: The distributions of the angiotensin-converting enzyme 2350 G/A genotypes (GG, GA, and AA, respectively) were 40.43%, 41.49%, and 18.08% in the controls and 18.67%, 46.67%, and 34.66% in the atrial fibrillation subjects (p = 0.037). The frequency of the A allele in the atrial fibrillation group was significantly greater than in the control group (58.00% vs. 38.83%, p = 0.0007). Compared with the wild-type GG genotype, the GA and AA genotypes had an increased risk for atrial fibrillation. Additionally, atrial fibrillation patients with the AA genotype had greater left atrial dimensions than the patients with the GG or GA genotypes (p<0.01 and p<0.05, respectively). CONCLUSIONS: The results obtained in this study indicate that the angiotensin-converting enzyme 2350 G/A polymorphism is associated with atrial fibrillation and that the A allele shows an increased risk for atrial fibrillation in Han Chinese patients with essential hypertension.

The effect of heat treatment on the microstructures and tensile properties of a powder metallurgical (PM) TiAl based alloy has been investigated in this paper. The near gamma (NG) microstructure is transformed to a full lamellar (FL) microstructure with an average grain size of 100 μm by heat treatments. The lamellar spacing of FL structure decreases with the increase of cooling rate. For cooling rates of 5, 10 and 50 °C/min, the lamellar spacing is 1.9, 1.0 and 0.8 μm respectively. The room temperature tensile properties exhibit an increasing trend with decrease of lamellar spacing.
