COVID-19 Infection Changes the Functions and Morphology of Erythrocytes: A Multidisciplinary Study

Hematological problems are associated with Coronavirus disease 2019 (COVID-19). Respiratory impairment is the higher point studied, although without experimental studies related to the oxygen transport performed by erythrocytes. Therefore, we decided to investigate if erythrocytes from COVID-19 patients have their functionality changed. The case-control study included hospitalized patients with a positive real-time polymerase chain reaction (RT-PCR) result admitted to University Hospital. Volunteers (negative RT-PCR results) were recruited as a control group. Thus, we assessed different erythrocytes parameters, oxidative stress markers, and biophysical studies using whole blood and isolated hemoglobin. We found a decrease of 51% in oxygen uptake and reduced antioxidant enzyme activity in COVID-19 patients compared to controls. Raman spectrometry showed structural changes in the hemoglobin and lipids of the erythrocytes from COVID-19 patients; thus, these results were consolidated with an increase in Young’s modulus in erythrocytes followed by morphology changes. Besides, isolated hemoglobin from COVID-19 patients has a distinct interaction profile using a ligand model compared to the control. COVID-19 leads to structural, functional, and morphological damage to human erythrocytes. Our data showed structural and molecular changes and induction of oxidative stress in erythrocytes by COVID-19, a new perspective on the contribution of erythrocytes to a respiratory commitment in COVID-19.

Penicillium meliponae, a recently described and rare species, was isolated as an endophytic fungus from the Amazonian plant Duguetia sthelechantha, and has been proven to be a pigment producer. Considering the high productivity of this species and the lack of data on its chemical composition, the present study aimed to characterize the chemical profile of P. meliponae and evaluate the influence of agitation and the use of different culture media. For this purpose, liquid chromatography coupled with mass spectrometry (LC-MS/MS) and molecular networking were used, allowing the identification of 17 azaphilone molecules with sclerotiorin-like skeletons, becoming the first chemical report of this species. In addition, the different production patterns in the tested culture media were indicative that this species is sensitive to changes in the composition of the carbon source and to the presence of agitation. Furthermore, this work contributes to the fragmentation mechanisms of the different possible structural arrangements for azaphilones of the sclerotiorin type and serves as a repository of information on the gas-phase behavior of this type of metabolite in mass spectrometry experiments and will assist future studies aimed at the discovery of azaphilones.

Agronomic characteristics and quality of lettuce cultivars in different crop seasons in western Alagoas, Brazil

ABSTRACT Growing lettuce under high temperatures and light incidence, as in the Alagoas State hinterland, Brazil, induces early tasseling and decreases the quality of the product. Thus, the objective of this work was to evaluate the production and post-harvest quality of lettuce cultivars in different crop seasons in western Alagoas, Brazil. A randomized block experimental design was used, with four replications. The treatments consisted of lettuce cultivars: Baba-de-Verão, Cinderela, Elba, Mônica, Solaris, and Veneranda. The winter crop was more favorable for plant diameter, except for Baba-de-Verão, whose performance was similar in both seasons and stood out in the summer. The winter crop resulted in better performance in plant height, stem diameter, and fresh matter yield, whereas summer plants reached larger number of leaves. The cultivar Mônica presented longer stem length, denoting lower tolerance to early tasseling and, therefore, it is not indicated to be grown in the region. The cultivar Baba-de-Verão (Lisa group) reached higher fresh matter yield, number of leaves per plant, and stem diameter, and it was the most adequate cultivar to be grown in western Alagoas. Elba, Solaris, and Veneranda were the lettuce cultivars from the Crespa group that stood out. Baba-de-Verão was the cultivar that presented the best performance for chlorophyll a, b, and total. The cultivar Solaris presented the highest carotenoid contents. Plants grown in the winter reached higher chlorophyll a and total, carotenoid contents, pH, and soluble solid (SS) to titratable acidity (TA) ratio, whereas the lettuce grown in the summer had higher SS and TA.
RESUMO A alface, quando produzida em altas temperaturas e elevada luminosidade, como as encontradas no Sertão Alagoano, é induzida ao pendoamento precoce e à baixa qualidade. Em função disso, o objetivo foi avaliar a produção e a qualidade pós-colheita de cultivares de alface em função de épocas de cultivo distintas no oeste do Estado de Alagoas. O delineamento experimental foi em blocos ao acaso, com quatro repetições, sendo os tratamentos referentes às cultivares Babá-de-Verão, Cinderela, Elba, Mônica, Solaris e Veneranda. Para diâmetro de planta, o cultivo de inverno foi mais favorável, com exceção da Babá-de-Verão que manteve o desempenho semelhante nas duas épocas e destaque da cultivar no verão. O cultivo de inverno proporcionou melhor desempenho de altura de planta, diâmetro de caule e produtividade de massa verde, enquanto no verão as plantas atingiram maior número de folhas. A cultivar Mônica obteve maior comprimento do caule, indicando menor tolerância ao pendoamento precoce, não sendo indicada para o cultivo na região. A Babá-de-Verão (lisa) atingiu maior produtividade de massa verde, número de folhas por planta e diâmetro de caule, sendo a cultivar mais adequada para o cultivo no oeste de Alagoas. Dentre as crespas, destacaram-se as cultivares Elba, Solaris e Veneranda. A cultivar que apresentou melhor desempenho para clorofilas a, b e totais foi a Babá-de-Verão. A Solaris obteve maior teor de carotenoides. As plantas de inverno alcançaram maiores clorofilas a e totais, carotenoides, pH e relação sólidos solúveis totais (SS) e acidez total titulável (TA), enquanto a alface de verão obteve maior SS e TA.

LUMBAR LORDOSIS VARIATION ACCORDING THE TYPE OF POSITIONER USED IN LUMBAR ARTHRODESIS

ABSTRACT Objective: Evaluate the influence of the most used surgical positioners for lumbar lordosis (LL) in asymptomatic individuals. Methods: Cross-sectional study based on demographic data and radiographic parameters of asymptomatic individuals. For this study, 16 volunteers, 15 males, and one female were selected, and the average age was 24.6 years. They were submitted to lateral radiographs of the lumbar spine in orthostasis in use of the following positioners: gel cushion, gel cushion with hip extension, four-point Relton-Hall and Wilson-type positioner. Results: The mean LL in the orthostatic position was 58.76º, whereas in the gel cushion positioner it was 52.51; on the gel cushion with hip extension of 58.23º, Relton-Hall/4points 37.63º and, finally, on the Wilson-type positioner of 40.87º. An average reduction of 5.42º of the LL was observed when positioning on the gel cushion in relation to the orthostasis. In the linear regression analysis, the data presented statistically significant results (p<0.05), demonstrating that the L4-S1 segment influences 60% in LL. Conclusion: The positioner with gel cushion and hip extension reproduces an LL similar to physiological values. Relton-Hall and Wilson-type positioners with hip flexion promote hypolordotic positioning compared to basal lordosis in orthostasis. Hip extension alone generated a 5.96º increase in the subject’s lordosis. The L4-S1 segment has a 60% influence on the LL when the individuals are in the positioners. RESUMEN: Objetivo: Evaluar la influencia de los posicionadores quirúrgicos para la lordosis lumbar (LL) en individuos asintomáticos. Métodos: Estudio transversal con datos demográficos y parámetros radiográficos de individuos asintomáticos. Utilizamos 16 voluntarios, 15 hombres y una mujer, edad media de 24,6 años. Sometidos a radiografías laterales de la columna lumbar en ortostasis en los siguientes posicionadores: almohadilla de gel, almohadilla de gel con extensión de cadera, posicionador de cuatro puntos y posicionador tipo Wilson. Resultados: El promedio de LL en posición ortostática fue de 58,76º, en el posicionador de almohadilla de gel fue de 52,51º; en almohadilla de gel con extensión de cadera de 58,23º, 4 puntos 37,63º y, por último, en posicionador tipo Wilson de 40,87º. Se observó una reducción promedio de 5,42º de LL al posicionarse sobre la almohadilla de gel en relación a ortostasis. En el análisis de regresión lineal, los datos presentaron resultados estadísticamente significativos (p<0,05), demostrando que el segmento L4-S1 tiene una influencia de 60% en LL. Conclusión: El posicionador con almohadilla de gel y con extensión de cadera reproduce una LL similar a la fisiológica. Relton-Hall y Wilson con flexión de cadera promueven el posicionamiento hipolordótico en comparación con la lordosis basal. La extensión de la cadera por sí sola fue capaz de generar un aumento de 5,96º en la lordosis. El segmento L4-S1 tiene una influencia del 60% en la LL cuando los individuos están en los posicionadores. Nivel de evidencia III; Estudio Transversal Controlado.
RESUMO: Objetivo: Avaliar a influência dos posicionadores cirúrgicos na lordose lombar (LL) em indivíduos assintomáticos. Métodos: Estudo transversal com dados demográficos e parâmetros radiográficos de indivíduos assintomáticos. Utilizamos 16 voluntários, sendo 15 do gênero masculino e uma do gênero feminino, com idade média de 24,6 anos. Foram submetidos a realização de radiografias em perfil da coluna lombar em ortostase nos seguintes posicionadores: coxim em gel, coxim em gel com extensão do quadril, Relton-Hall em quatro pontos e posicionador tipo Wilson. Resultados: A média de LL na posição ortostática foi de 58,76º, já no posicionador coxim em gel de 52,51; no coxim em gel com extensão dos quadris de 58,23º, Relton-Hall/4pontos 37,63º e, por último, no posicionador tipo Wilson, de 40,87º. Houve redução média de 5,42º da LL ao posicionar no coxim em gel em relação a ortostase, na análise de regressão linear os dados apresentaram resultados estaticamente significativos (p<0,05), demostrando que o seguimento L4-S1 apresenta uma influência de 60% na LL. Conclusão: O posicionador coxins em gel e extensão do quadril reproduz uma LL semelhante à fisiológica. Posicionadores do tipo Relton-Hall e Wilson com flexão do quadril promovem um posicionamento hipolordótico comparada a lordose basal em ortostase. A extensão do quadril por si só foi capaz de gerar um aumento de 5,96º na lordose do indivíduo. O seguimento L4-S1 apresenta uma influência de 60% na LL quando os indivíduos estão nos posicionadores. Nível de Evidência III; Estudo Transversal Controlado.

Abstract Ticks parasitizing 102 wild animals in the states of Mato Grosso and Goiás, Brazil were collected between 2015 and 2018. A total of 2338 ticks (865 males, 541 females, 823 nymphs, and 109 larvae) belonging to four genera (Amblyomma, Dermacentor, Haemaphysalis, and Rhipicephalus) and at least 21 species were identified. DNA extraction and a molecular survey for rickettsial agents were performed on 650 ticks. The results revealed parasitism by the following species: Rickettsia amblyommatis in Amblyomma cajennense s.s., A. cajennense s.l., Amblyomma coelebs, Amblyomma humerale, Amblyomma longirostre, Amblyomma nodosum, Amblyomma scalpturatum, Amblyomma sculptum, and Amblyomma romitii; Rickettsia parkeri in Amblyomma nodosum, Amblyomma ovale, Amblyomma scalpturatum, and Amblyomma triste; Rickettsia rhipicephali in Haemaphysalis juxtakochi; Rickettsia sp. in A. cajennense s.s., A. nodosum, and A. sculptum, and lastly, ‘Candidatus Rickettsia andeanae’ in Amblyomma parvum and Rhipicephalus microplus. This study expands the body of knowledge about tick parasitism among wild animals, including new data concerning tick-host associations, and provides information about the epidemiology of tick-borne pathogens in the Center-West region of Brazil.
Resumo Foram coletados carrapatos parasitando 102 animais silvestres nos estados de Mato Grosso e Goiás, Brasil, entre 2015 e 2018. Um total de 2338 carrapatos (865 machos, 541 fêmeas, 823 ninfas e 109 larvas) pertencentes a quatro gêneros (Amblyomma, Dermacentor, Haemaphysalis e Rhipicephalus) e pelo menos 21 espécies foram identificadas. A extração de DNA e pesquisa molecular para agentes riquétsiais foram realizadas em 650 carrapatos. Os resultados revelaram parasitismo pelas seguintes espécies: Rickettsia amblyommatis em Amblyomma cajennense s.s., A. cajennense s.l., Amblyomma coelebs, Amblyomma humerale, Amblyomma longirostre, Amblyomma nodosum, Amblyomma scalpturatum, Amblyomma sculptum e Amblyomma romitii; Rickettsia parkeri em Amblyomma nodosum, Amblyomma ovale, Amblyomma scalpturatum e Amblyomma triste; Rickettsia rhipicephali em Haemaphysalis juxtakochi; Rickettsia sp. em A. cajennense s.s., A. nodosum e A. sculptum e, por último, ‘Candidatus Rickettsia andeanae’ em Amblyomma parvum e Rhipicephalus microplus. Este estudo amplia o conhecimento sobre o parasitismo de carrapatos em animais silvestres, incluindo-se novos dados sobre associações carrapato-hospedeiro. Fornece, ainda, informações sobre a epidemiologia de patógenos transmitidos por carrapatos na região Centro-Oeste do Brasil.

Formulation of diets for slow-growing broilers slaughtered at different ages on carcass characteristics and composition of commercial cuts

Abstract This study was conducted to examine the effects of a diet formulated with industrial amino acids and a commercial vitamin-mineral mixture on the characteristics of carcass and meat cuts of slow-growing broilers slaughtered at different ages. The experiment involved 600 one-day-old male and female white Naked Neck chicks. The chicks were allotted randomly to a 2 × 3 factorial arrangement consisting of two diets and three slaughter ages, in a completely randomized experimental design with five replicates of 20 birds each. The experimental period was 84 days. Six chicks were selected and slaughtered on the first day, and then at 56, 70 and 84 days of age 10 birds unit was selected and slaughtered per experimental period. Weight and yield of carcass, abdominal fat and breast, thigh and drumstick meat were determined. Additionally, moisture, protein, fat and mineral matter contents and protein and fat deposition in the breast, drumstick and thigh muscles were determined. The diet did not influence the yields of carcass, abdominal fat, or meat cuts or the nutritional composition of meat. Slaughter age influenced the yields of breast and thigh meat and abdominal fat; the protein content of breast meat; and the moisture, crude protein, fat, and mineral matter contents of drumstick and thigh meat. There was an interaction effect between diet and slaughter age for protein deposition in breast meat. The age factor influenced fat deposition in the drumstick muscles. Protein deposition in the thigh muscles was influenced by the diet. Moreover, a difference was observed between the ages for protein and fat deposition in the thigh muscle. In conclusion, the use of a commercial mixture in the diet results in decreased protein deposition in the breast and thigh muscles, although this difference does not affect the characteristics of carcass or meat cuts. Birds slaughtered at 70 days exhibit similar carcass characteristics to those of birds slaughtered at 84 days, indicating the possibility of an earlier slaughter age.

Abstract The geographical distribution of sand flies in Brazil has been the subject of some studies, yet there is no information about the phlebotomine fauna in João Pessoa, State of Paraíba, Brazil. The aim of this work is to evaluate the occurrence and distribution of sand flies in the Atlantic forest fragments and to evaluate a possible dispersion in 06 nearby districts. Light traps were used during three consecutive nights, supplemented by an aspirator during the dry period and rainy season. A total of 222 sand flies were found, 143 (130 males and 13 females) in the Atlantic forest, and 79 in urban areas. During the entire dry season, three species of phlebotomine sand flies were recorded in 11 forest fragments, Lutzomyia longipalpis, Lu. migonei and Lu. whitmani. During the rainy season, only Lu. longipalpis was found. This was the only species identified in the studied neighborhoods during both seasons. The differences in diversity of sand flies encountered between natural habitats and urban areas may thus be correlated mostly with adaptations to particular habitats and availability of food. One species (Lu. longipalpis) appears to be rapidly adapting to urban areas because of deforestation.

This guideline aimed to update the concepts and formulate the standards of conduct and scientific evidence that support them, regarding the diagnosis and treatment of the Cardiomyopathy of Chagas disease, with special emphasis on the rationality base that supported it.nbsp;
Chagas disease in the 21st century maintains an epidemiological pattern of endemicity in 21 Latin American countries. Researchers and managers from endemic and non-endemic countries point to the need to adopt comprehensive public health policies to effectively control the interhuman transmission of T. cruzi infection, and to obtain an optimized level of care for already infected individuals, focusing on diagnostic and therapeutic opportunistic opportunities.
nbsp;
Pathogenic and pathophysiological mechanisms of the Cardiomyopathy of Chagas disease were revisited after in-depth updating and the notion that necrosis and fibrosis are stimulated by tissue parasitic persistence and adverse immune reaction, as fundamental mechanisms, assisted by autonomic and microvascular disorders, was well established. Some of them have recently formed potential targets of therapies.nbsp;
The natural history of the acute and chronic phases was reviewed, with enhancement for oral transmission, indeterminate form and chronic syndromes. Recent meta-analyses of observational studies have estimated the risk of evolution from acute and indeterminate forms and mortality after chronic cardiomyopathy. Therapeutic approaches applicable to individuals with Indeterminate form of Chagas disease were specifically addressed. All methods to detect structural and/or functional alterations with various cardiac imaging techniques were also reviewed, with recommendations for use in various clinical scenarios. Mortality risk stratification based on the Rassi score, with recent studies of its application, was complemented by methods that detect myocardial fibrosis.nbsp;
The current methodology for etiological diagnosis and the consequent implications of trypanonomic treatment deserved a comprehensive and in-depth approach. Also the treatment of patients at risk or with heart failure, arrhythmias and thromboembolic events, based on pharmacological and complementary resources, received special attention. Additional chapters supported the conducts applicable to several special contexts, including t. cruzi/HIV co-infection, risk during surgeries, in pregnant women, in the reactivation of infection after heart transplantation, and others.nbsp; nbsp;nbsp;
Finally, two chapters of great social significance, addressing the structuring of specialized services to care for individuals with the Cardiomyopathy of Chagas disease, and reviewing the concepts of severe heart disease and its medical-labor implications completed this guideline.
Esta diretriz teve como objetivo principal atualizar os conceitos e formular as normas de conduta e evidências científicas que as suportam, quanto ao diagnóstico e tratamento da CDC, com especial ênfase na base de racionalidade que a embasou.
A DC no século XXI mantém padrão epidemiológico de endemicidade em 21 países da América Latina. Investigadores e gestores de países endêmicos e não endêmicos indigitam a necessidade de se adotarem políticas abrangentes, de saúde pública, para controle eficaz da transmissão inter-humanos da infecção pelo T. cruzi, e obter-se nível otimizado de atendimento aos indivíduos já infectados, com foco em oportunização diagnóstica e terapêutica.
Mecanismos patogênicos e fisiopatológicos da CDC foram revisitados após atualização aprofundada e ficou bem consolidada a noção de que necrose e fibrose sejam estimuladas pela persistência parasitária tissular e reação imune adversa, como mecanismos fundamentais, coadjuvados por distúrbios autonômicos e microvasculares. Alguns deles recentemente constituíram alvos potenciais de terapêuticas.
A história natural das fases aguda e crônica foi revista, com realce para a transmissão oral, a forma indeterminada e as síndromes crônicas. Metanálises recentes de estudos observacionais estimaram o risco de evolução a partir das formas aguda e indeterminada e de mortalidade após instalação da cardiomiopatia crônica. Condutas terapêuticas aplicáveis aos indivíduos com a FIDC foram abordadas especificamente. Todos os métodos para detectar alterações estruturais e/ou funcionais com variadas técnicas de imageamento cardíaco também foram revisados, com recomendações de uso nos vários cenários clínicos. Estratificação de risco de mortalidade fundamentada no escore de Rassi, com estudos recentes de sua aplicação, foi complementada por métodos que detectam fibrose miocárdica.
A metodologia atual para diagnóstico etiológico e as consequentes implicações do tratamento tripanossomicida mereceram enfoque abrangente e aprofundado. Também o tratamento de pacientes em risco ou com insuficiência cardíaca, arritmias e eventos tromboembólicos, baseado em recursos farmacológicos e complementares, recebeu especial atenção. Capítulos suplementares subsidiaram as condutas aplicáveis a diversos contextos especiais, entre eles o da co-infecção por T. cruzi/HIV, risco durante cirurgias, em grávidas, na reativação da infecção após transplante cardíacos, e outros.nbsp;nbsp;nbsp;
Por fim, dois capítulos de grande significado social, abordando a estruturação de serviços especializados para atendimento aos indivíduos com a CDC, e revisando os conceitos de cardiopatia grave e suas implicações médico-trabalhistas completaram esta diretriz.nbsp;

Considering the promising antitumor effects of compounds with dual anti-inflammatory and antiproliferative activities, thus benzophenones analogs (2-7) were evaluated on in vivo anti-inflammatory assay and molecular docking analysis. Those with the best molecular docking results were in vitro evaluated on cyclooxygenase (COX) enzymes and tested regarding antiproliferative activity. All derivatives displayed in vivo anti-inflammatory activity. Among them, the substances 2’-hydroxy-4’-benzoylphenyl-β-D-glucopyranoside (4), 4-hydroxy-4’-methoxybenzophenone (5) and 4’-(4’’-methoxybenzoyl)phenyl-β-D-glucopyranoside (7) showed the best values of Glide Score in COX-2 docking evaluation and 4 and 5 selectively inhibited COX-2 and COX-1 in vitro enzymatic assay, respectively. Thus, 4 and 5 were tested against breast cancer (MCF-7, MDA MB-231, Hs578T) and non-small-cell-lung cancer (A549) cell lines. The estrogen-positive MCF-7 cell line was more responsive compared to other tested cell lines. They induced cell cycle arrest at G1/S transition in MCF-7 cell line once there was an increase in G0/G1 population with concomitant reduction of S population. The antiproliferative activity of these substances on MCF-7 was associated with their ability to inhibit cyclin E expression, a critical regulator of G1/S transition. Taken together, the data indicate that 4 and 5 have dual anti-inflammatory and antiproliferative activities and support further studies to evaluate their antitumor potential.

Abstract: COVID-19 rapidly spread across the world in an unprecedented outbreak with a massive number of infected and fatalities. The pandemic was heavily discussed and searched on the internet, which generated big amounts of data related to it. This led to the possibility of attempting to forecast coronavirus indicators using the internet data. For this study, Google Trends statistics for 124 selected search terms related to pandemic were used in an attempt to find which keywords had the best Spearman correlations with a lag, as well as a forecasting model. It was found that keywords related to coronavirus testing among some others, such as “I have contracted covid”, had high correlations (≥0.7) with few weeks of lag (≤4 weeks). Besides that, the ARIMAX model using those keywords had promising results in predicting the increase or decrease of epidemiological indicators, although it was not able to predict their exact values. Thus, we found that Google Trends data may be useful for predicting outbreaks of coronavirus a few weeks before they happen, and may be used as an auxiliary tool in monitoring and forecasting the disease in Brazil.

ABSTRACT Background: Arthropod-borne viruses have recently emerged and are pathogens of various human diseases, including dengue, zika, and chikungunya viruses. Methods: We collectedAedes aegyptilarvae (N = 20) from Brumado, Bahia, Brazil, and treated and individually preserved the specimens. We analyzed the samples for dengue, zika, and chikungunya viruses using molecular biology methods. Results: We found that 25% (N = 5) and 15% (N = 3) were positive exclusively for dengue and chikungunya viruses, respectively; 15% (N = 3) were coinfected with both. Conclusions: This is the first report of dengue and chikungunya virus coinfection in A. aegypti larvae.

Caracterizar o perfil epidemiológico e clínico dos indivíduos mais propensos a se infectar com SARS-CoV-2 após o esquema vacinal completo, a fim de traçar o perfil dos grupos prioritários para receberem uma dose de reforço em situações de escassez de doses vacinais, bem como para manutenção dos cuidados de proteção individual. Métodos. Este estudo transversal utilizou dados de pacientes hospitalizados com COVID-19 com idade ≥18 anos, esquema vacinal completo e diagnóstico positivo de infecção por SARS-CoV-2, coletados do banco de dados SIVEP-Gripe (Sistema de Informação da Vigilância Epidemiológica da Gripe) de 18 de janeiro a 15 de setembro de 2021. Foram analisados dados demográficos, sintomas clínicos e problemas médicos preexistentes (comorbidades). O desfecho primário foi óbito intra-hospitalar. Resultados. A maioria dos pacientes hospitalizados com infecção devido a escape vacinal tinha ≥60 anos, era do sexo masculino e tinha um quadro de COVID-19 grave ou gravíssimo. A letalidade foi extremamente alta (50,27%) e mais pronunciada nos grupos de pessoas idosas. Os sintomas mais prevalentes foram tosse, dispneia, desconforto respiratório e baixa saturação de oxigênio no sangue. As comorbidades mais frequentes foram cardiopatia e diabetes. Altas taxas de letalidade foram observadas entre os pacientes internados em unidades de terapia intensiva (72,88%) e os que precisaram de ventilação mecânica invasiva (87,82%). Os principais fatores de risco para um desfecho desfavorável foram idade avançada, comprometimento respiratório, imunização com vacina de vírus inativado e problemas médicos preexistentes. Conclusões. Caracterizamos o perfil dos pacientes brasileiros hospitalizados com infecção por COVID-19 devido a escape vacinal e os fatores de risco para desfecho desfavorável. Esses dados permitem identificar grupos prioritários para receber dose de reforço e continuar os cuidados de proteção individual.
ABSTRACT Objective. To characterize the epidemiological and clinical profile of individuals more likely to become infected with SARS-CoV-2 after the fully vaccination schedule in order to profile priority groups to receive a booster dose in situations of vaccine doses shortage as well as for maintenance of personal protective care. Methods. This cross-sectional study used data from hospitalized COVID-19 patients aged ≥18 years, who had been fully vaccinated and had a SARS-CoV-2 infection positive diagnosis collected from the SIVEP-Gripe database (Influenza Epidemiological Surveillance Information System) from January 18, 2021 to September 15, 2021. Demographic data, clinical symptoms and preexisting medical conditions (comorbidities) were analyzed. The primary outcome was in-hospital death. Results. The majority of hospitalized patients with vaccine breakthrough infection were ≥60 years old, male, with critical or severe COVID-19. The fatality rate was extremely high (50.27%) and more pronounced in elderly groups. The most prevalent symptoms were cough, dyspnea, respiratory distress, and low blood oxygen saturation. The most frequent comorbidities were heart disease and diabetes. High fatality rates were observed among patients admitted to the intensive care units (72.88%) and those who required invasive mechanical ventilation (87.82%). The main risk factors for an unfavorable outcome were older age, respiratory compromise, inactivated virus vaccine immunization, and preexisting medical conditions. Conclusions. We characterized the profile of hospitalized Brazilian patients with COVID-19 vaccine breakthrough infection and the risk factors for an unfavorable outcome. These data allow to identify priority groups to receive a booster dose and to continue using personal protection.

Caracterizar el perfil clínico y epidemiológico de las personas con mayores probabilidades de contraer la infección por el SARS-CoV-2 luego de tener el esquema completo de vacunación, con el fin de definir los grupos de prioridad para la aplicación de la dosis de refuerzo en situaciones de escasez de dosis de vacunación, así como para el mantenimiento de los equipos de protección personal. Métodos. En este estudio transversal se emplearon datos de pacientes mayores de 18 años hospitalizados con COVID-19 que habían recibido la pauta completa de vacunación y tenían un diagnóstico positivo de infección por SARS-CoV-2, recabados de la base de datos SIVEP-Gripe (Sistema de Información de Vigilancia Epidemiológica de la Gripe) en el periodo comprendido entre el 18 de enero del 2021 y el 15 de septiembre del 2021. Se analizaron datos demográficos, síntomas clínicos y trastornos médicos preexistentes (comorbilidades). El resultado primario fue la muerte hospitalaria. Resultados. La mayoría de los pacientes vacunados hospitalizados fueron varones mayores de 60 años con infección crítica o grave por el virus de la COVID-19. La tasa de letalidad fue extremadamente elevada (50,27 %) y más pronunciada en los grupos de edad avanzada. Los síntomas más prevalentes fueron tos, disnea, dificultad respiratoria y baja saturación de oxígeno en sangre. Las comorbilidades más frecuentes fueron las cardiopatías y la diabetes. Se observaron altas tasas de letalidad entre los pacientes ingresados en las unidades de cuidados intensivos (72,88 %) y los que necesitaron ventilación mecánica invasiva (87,82 %). Los principales factores de riesgo para un resultado desfavorable fueron la edad avanzada, dificultades respiratorias, inmunización con vacunas de virus inactivados y trastornos médicos preexistentes. Conclusiones. Se caracterizaron el perfil de los pacientes brasileños vacunados hospitalizados con COVID-19 por infección irruptiva de vacuna y los factores de riesgo de un resultado desfavorable. Estos datos permiten identificar los grupos de prioridad para recibir la dosis de refuerzo y continuar utilizando protección personal.

Abstract Introduction: We investigated the clinical course and outcomes of patients submitted to cardiovascular surgery in Brazil and who had developed symptoms/signs of coronavirus disease 2019 (COVID-19) in the perioperative period. Methods: A retrospective multicenter study including 104 patients who were allocated in three groups according to time of positive real time reverse transcriptase-polymerase chain reaction (RT-PCR) for the SARS-CoV-2 (severe acute respiratory syndrome coronavirus 2): group 1, patients who underwent cardiac surgery > 10 days after positive RT-PCR; group 2, patients with a positive RT-PCR within 10 days before or after surgery; group 3, patients who presented positive RT-PCR > 10 days after surgery. The primary outcome was mortality and secondary outcomes were postoperative complications, intensive care unit (ICU) length of stay, and postoperative days of hospitalization. Results: The three groups were similar with respect to age, the European System of Cardiac Operative Risk Evaluation score, and comorbidities, except hypertension. Postoperative complications and death were significantly higher in groups 2 and 3 than in group 1, and no significant difference between groups 2 and 3 was seen. Group 2 showed a high prevalence of surgery performed as an urgent procedure. Although no significant differences were observed in ICU length of stay, total postoperative hospitalization time was significantly higher in group 3 than in groups 1 and 2. Conclusion: COVID-19 affecting the postoperative period of patients who underwent cardiovascular surgery is associated with a higher rate of morbidity and mortality. Delaying procedures in RT-PCR-positive patients may help reduce risks of perioperative complications and death.

Resumo Hipertensão arterial sistêmica (HAS) e diabetes mellitus (DM) são dois dos principais fatores de risco para a mortalidade por COVID-19. Descrever a prevalência e o perfil clínico-epidemiológico de óbito por COVID-19 ocorridos em Pernambuco, Brasil, entre 12 de março e 14 de maio de 2020 entre pacientes que possuíam hipertensão arterial sistêmica e/ou diabetes mellitus como doenças prévias. Estudo observacional transversal. Foram analisadas as seguintes variáveis: município de procedência, sexo, faixa etária, tempo entre o início dos sinais/sintomas e o óbito, sinais/sintomas, tipo de comorbidades e hábitos de vida. Variáveis categóricas foram descritas por meio de frequências e variáveis contínuas por meio de medidas de tendência central e de dispersão. Os testes de Mann-Whitney e Kruskal-Wallis foram utilizados. Dos 1.276 registros incluídos no estudo, 410 apresentavam HAS e/ou DM. A prevalência de HAS foi 26,5% (n=338) e de DM foi 19,7% (n=252). Dos registros, 158 (12,4%) eram de pacientes que possuíam somente HAS, 72 (5,6%) somente DM e 180 (14,1%) apresentavam HAS e DM. Dos indivíduos com HAS, 53,3% apresentavam DM e 71,4% dos diabéticos apresentam HAS. A mediana (em dias) do tempo entre o início dos sinais/sintomas e o desfecho óbito foi 8,0 (IIQ 9,0), sem diferença significativa entre os grupos de comorbidades (p=0,633), sexo (p=0,364) e faixa etária (p=0,111). Observou-se maior prevalência de DM e HAS na população masculina (DM — 61,3% eram homens e 38,9% mulheres; HAS — 53,2% eram homens e 46,8% mulheres). Os sinais/sintomas mais frequentes foram dispneia (74,1%; n=304), tosse (72,2%; n=296), febre (68,5%; n=281) e saturação de O2<95% (66,1%; n=271). Dos hipertensos, 73,3% (n=100) apresentavam outras comorbidades/fatores de risco associados, e 54,2% (n=39) dos diabéticos apresentavam outras comorbidades/fatores de risco associados. Destacaramse as cardiopatias (19,5%; n=80), obesidade (8,3%; n=34), doença respiratória prévia (7,3%; n=30) e nefropatia (7,8%; n=32). A prevalência de tabagismo foi 8,8% (n=36) e de etilismo alcançou 3,4% (n=14). O estudo mostrou que a prevalência de HAS foi superior à prevalência de DM nos indivíduos que foram a óbito por COVID-19. Em idosos, a prevalência foi superior à observada em indivíduos não idosos.

RESUMO Objetivo: Descrever as práticas de ressuscitação volêmica em unidades de terapia intensiva brasileiras e compará-las com as de outros países participantes do estudo Fluid-TRIPS. Métodos: Este foi um estudo observacional transversal, prospectivo e internacional, de uma amostra de conveniência de unidades de terapia intensiva de 27 países (inclusive o Brasil), com utilização da base de dados Fluid-TRIPS compilada em 2014. Descrevemos os padrões de ressuscitação volêmica utilizados no Brasil em comparação com os de outros países e identificamos os fatores associados com a escolha dos fluidos. Resultados: No dia do estudo, foram incluídos 3.214 pacientes do Brasil e 3.493 pacientes de outros países, dos quais, respectivamente, 16,1% e 26,8% (p < 0,001) receberam fluidos. A principal indicação para ressuscitação volêmica foi comprometimento da perfusão e/ou baixo débito cardíaco (Brasil 71,7% versus outros países 56,4%; p < 0,001). No Brasil, a percentagem de pacientes que receberam soluções cristaloides foi mais elevada (97,7% versus 76,8%; p < 0,001), e solução de cloreto de sódio a 0,9% foi o cristaloide mais comumente utilizado (62,5% versus 27,1%; p < 0,001). A análise multivariada sugeriu que os níveis de albumina se associaram com o uso tanto de cristaloides quanto de coloides, enquanto o tipo de prescritor dos fluidos se associou apenas com o uso de cristaloides. Conclusão: Nossos resultados sugerem que cristaloides são usados mais frequentemente do que coloides para ressuscitação no Brasil, e essa discrepância, em termos de frequências, é mais elevada do que em outros países. A solução de cloreto de sódio 0,9% foi o cristaloide mais frequentemente prescrito. Os níveis de albumina sérica e o tipo de prescritor de fluidos foram os fatores associados com a escolha de cristaloides ou coloides para a prescrição de fluidos.
Abstract Objective: To describe fluid resuscitation practices in Brazilian intensive care units and to compare them with those of other countries participating in the Fluid-TRIPS. Methods: This was a prospective, international, cross-sectional, observational study in a convenience sample of intensive care units in 27 countries (including Brazil) using the Fluid-TRIPS database compiled in 2014. We described the patterns of fluid resuscitation use in Brazil compared with those in other countries and identified the factors associated with fluid choice. Results: On the study day, 3,214 patients in Brazil and 3,493 patients in other countries were included, of whom 16.1% and 26.8% (p < 0.001) received fluids, respectively. The main indication for fluid resuscitation was impaired perfusion and/or low cardiac output (Brazil: 71.7% versus other countries: 56.4%, p < 0.001). In Brazil, the percentage of patients receiving crystalloid solutions was higher (97.7% versus 76.8%, p < 0.001), and 0.9% sodium chloride was the most commonly used crystalloid (62.5% versus 27.1%, p < 0.001). The multivariable analysis suggested that the albumin levels were associated with the use of both crystalloids and colloids, whereas the type of fluid prescriber was associated with crystalloid use only. Conclusion: Our results suggest that crystalloids are more frequently used than colloids for fluid resuscitation in Brazil, and this discrepancy in frequencies is higher than that in other countries. Sodium chloride (0.9%) was the crystalloid most commonly prescribed. Serum albumin levels and the type of fluid prescriber were the factors associated with the choice of crystalloids or colloids for fluid resuscitation.
