Abstract The present study aimed to investigate the socioeconomic and obstetric characteristics of adolescent mothers and the complications they cause to maternal and neonatal health. This baseline data analysis of the MINA-Brazil birth cohort was conducted in the municipality of Cruzeiro do Sul, state of Acre, Brazil. The chi-square test was used to compare characteristics of adolescent and adult postpartum women, and multiple Poisson regression models with robust variance were used to assess associated factors. Among the postpartum women, 26.2% (95%CI: 24.0-28.4) were adolescents. Factors associated with childbirth in adolescence included: nine years or less of schooling (adjPR:1.36; 95%CI: 1.14-1.61), belongs to the lowest quartiles of the wealth index (1st quartile: adjPR:1.40; 95%CI: 1.08-1.80) (2nd quartile: adjPR:1.37; 95%CI: 1.08-1.74), primigravidae (adjPR:3.69; 95%CI: 2.98-4.57), low pre-pregnancy BMI (adjPR:1.28; CI95%: 1.04-1.57), urinary tract infection during pregnancy (adjPR:1.25; CI95%: 1.07-1.46) and less than six prenatal consultations (adjPR:1.42; 95%CI: 1.21-1.66). Poverty, little schooling, primigravidae, low pre-pregnancy BMI, urinary tract infection during pregnancy and few prenatal consultations were associated with childbirth during adolescence in a municipality in the Northern region of Brazil.

Resumo O objetivo do estudo foi investigar as características socioeconômicas e obstétricas de parturientes adolescentes e suas complicações sobre a saúde materna e neonatal. Trata-se de uma análise de dados da linha de base da coorte de nascimentos MINA-Brasil conduzida no município de Cruzeiro do Sul, estado do Acre. Utilizou-se teste qui-quadrado para comparar características das puérperas adolescentes com as adultas e modelos múltiplos de regressão de Poisson com variância robusta para avaliar fatores associados. Entre as puérperas estudadas, 26,2% (IC95%: 24,0-28,4) eram adolescentes. Os fatores associados ao parto na adolescência foram ter nove anos ou menos de estudo (RPaj:1,36; IC95%: 1,14-1,61), pertencer aos menores quartis do índice de riqueza (1° quartil: RPaj:1,40; IC95%: 1,08-1,80) (2° quartil: RPaj:1,37; IC95%: 1,08-1,74), ser primigesta (RPaj:3,69; IC95%: 2,98-4,57), baixo IMC pré-gestacional (RPaj:1,28; IC95%: 1,04-1,57), infecção urinária na gravidez (RPaj:1,25; IC95%: 1,07-1,46) e menos de seis consultas de pré-natal (RPaj:1,42; IC95%: 1,21-1,66). Pobreza, baixa escolaridade, primigestação, baixo IMC pré-gestacional, infecção urinária na gestação e menor número de consultas de pré-natal foram associados ao parto na adolescência em município da região Norte do Brasil.

ABSTRACT Objective: Investigate the realities and challenges of Intersectoriality in cities/regions that have structured municipal intersectoral working groups (GTI-M) in the Health at School Program (PSE) in the period 2017-2021. Method: Research with documental survey and integrative review to select articles in Ingles and Portuguese. Results: Records of official documents that guide the guidelines for actions of adherence, agreement, planning, execution and monitoring of the PSE and nine articles from different regions of the country where the forms of work in front of the intersectoriality were observed: southeast (five); northeast (two); south regions (1) and north region (1). Conclusion: The perception of intersectoral work still lacks understanding, involvement and effectiveness of the GTI-M, which makes it difficult to implement mechanisms that facilitate PSE actions. In addition, there is a lack of municipal articulation, based on the official decree for GTI-M, which ends up promoting fragmented actions, team wear, reducing the positive impacts that the PSE can promote in collective health. The study can serve as a basis for reformulating strategies and control mechanisms of the federal government to execution and release of funds for PSE.

RESUMO O objetivo da presente pesquisa foi investigar as realidades e desafios da intersetorialidade nas cidades/região que possuem estruturação dos grupos de trabalho intersetorial municipal (GTI-M) no Programa Saúde na Escola (PSE) no período 2017-2021. Tratou-se de um pesquisa com levantamento documental e revisão integrativa para seleção de artigos nos idiomas português e inglês. Sete registros de documentos oficiais foram encontrados que norteiam as diretrizes para ações de adesão, pactuação, planejamento, execução e monitoramento do PSE e nove artigos de diferentes regiões do País onde foi observado as formas de relações de trabalho frente a intersetorialidade: sudeste (cinco); nordeste (dois); regiões sul (1) e região norte (1). A percepção sobre o trabalho intersetorial, ainda é carente de entendimento, envolvimento e efetivação dos GTI-M, o que dificulta a implementação de mecanismos facilitadores das ações do PSE. Além disso, existe uma falta de articulação municipal, a partir do decreto oficial para GTI-M, o que acaba promovendo ações fragmentadas, desgaste das equipes, diminuindo impactos positivos que o PSE pode promover na saúde coletiva. O estudo pode servir de base para reformulação de estratégias e de mecanismos de controle do governo federal para execução e liberação de verbas para o PSE.

ABSTRACT Purpose To analyze the effects of lingual frenotomy on the breastfeeding of infants, based on the electrical activity of the masseter and suprahyoid muscles and assessment of the breastfeeding. Methods Observational study developed between October 2017 and June 2018 with a sample of 20 newborns and infants who attended a dental clinic and were diagnosed with ankyloglossia. Another 20 were excluded for meeting some of the following exclusion criteria: babies more than 6 months old, who were not on exclusive or mixed breastfeeding, who had other clinical impairments that interfered with breastfeeding, who had other foods introduced into their diet, who had neurological changes and/or craniofacial deformities, and/or who did not finish all the stages of the study. Breastfeeding was assessed with the UNICEF Breastfeeding Assessment and Observation Protocol, while the muscle electrical activity was assessed with the Electrical Activity Assessment Protocol for the Masseter and Suprahyoid Muscles in Newborns During Breastfeeding. The same speech-language-hearing therapist conducted the two assessments both before the conventional frenotomy and 7 days after it. Results The signs suggestive of breastfeeding difficulties changed 7 days after the surgery, with a p-value ≤ 0.002 for general observation of the mother, position of the infant, latch, and sucking. The maximum voluntary contraction of the masseter was the only integral parameter with a difference, as the electrical activity had decreased. Conclusion Behaviors favorable to breastfeeding increased 7 days after the frenotomy in all the breastfeeding assessment categories, whereas the electrical activity of the masseter decreased.

RESUMO Objetivo Analisar os efeitos da frenotomia lingual na amamentação de recém-nascidos, com base na atividade elétrica dos músculos masseter e supra-hióideos e avaliação da mamada. Método Estudo observacional, desenvolvido entre outubro de 2017 e junho de 2018. Amostra de 20 recém-nascidos e lactentes de uma clínica odontológica, diagnosticados com anquiloglossia; 20 foram excluídos por se encaixarem em algum dos critérios de exclusão: ter mais de seis meses de vida, não fazer aleitamento materno exclusivo ou misto, apresentar outro comprometimento clínico que interfira na amamentação, ter iniciado a ingestão de outros alimentos, alterações neurológicas e/ou deformidades craniofaciais e não ter finalizado todas as etapas do estudo. A avaliação da amamentação seguiu o Protocolo de Observação e Avaliação da Mamada da UNICEF, e a avaliação da atividade elétrica muscular o Protocolo de Avaliação da Atividade Elétrica dos Músculos Masseter e Supra-hióideos em Recém-Nascidos Durante a Alimentação, ambas realizadas pela mesma fonoaudióloga antes da frenotomia convencional e sete dias após. Resultados Os sinais sugestivos de dificuldade na amamentação mudaram sete dias após a cirurgia, com valor de p≤0,002 para a observação geral da mãe, posição do bebê, pega e sucção. Houve diferença apenas no parâmetro integral da Contração Voluntária Máxima do masseter, com diminuição da atividade elétrica. Conclusão Sete dias após a frenotomia observou-se aumento de comportamentos favoráveis à amamentação em todas as categorias de avaliação da mamada e diminuição da atividade elétrica do masseter.

Abstract Basketball is a sport practiced around the world in this way, performances tests especially with low-cost and accessible are suggested to adjust the training. The 00aim of present study was to evaluate and correlate the speed and jump performance of lower limbs of basketball athletes from the Praia Grande, city of São Paulo, Brazil. Twelve male athletes (6.92 ± 2.57 years of pratice) from the Basketball team of Praia Grande City, aged 18.7 ± 0.6 years, height 1.85 ± 0.06, body mass 83.66 ± 10.16 kg were evaluated. The athletes were submitted to test sessions using the protocol for evaluating the maximum speed of 20 meters and the unilateral vertical jump. The speed on 20 meter test was 3.53 ± 0.20 seconds. Although an asymmetry of 17.81 ± 14.64% was found, no statistical difference (p = 0.817) was found between the dominant (36.50 ± 7.36 cm) and non-dominant (35.92 ± 5.63 cm) of vertical jump performance. Additionally, correlation between 20-meter test and values jump was found to dominant leg (p = 0.042) but not to non-dominant leg (p = 0.704). In conclusion, although asymmetry was found between members of the dominant and non-dominant side of the pitch, did not show any difference in the vertical jump.

Resumo O basquete é um esporte praticado em todo o mundo, desta forma, testes de desempenho principalmente com baixo custo e acessíveis são sugeridos para adequar o treinamento. O objetivo do presente estudo foi avaliar e correlacionar a velocidade e o desempenho de salto de membros inferiores de atletas de basquetebol da cidade da Praia Grande, São Paulo, Brasil. Foram avaliados 12 atletas do sexo masculino (6,92 ± 2,57 anos de prática) da equipe de Basquetebol da Cidade de Praia Grande, com idade 18,7 ± 0,6 anos, estatura 1,85 ± 0,06, massa corporal 83,66 ± 10,16 kg. Os atletas foram submetidos a sessões de testes utilizando o protocolo de avaliação da velocidade máxima de 20 metros e do salto vertical unilateral. A velocidade no teste de 20 metros foi de 3,53 ± 0,20 segundos. Embora tenha sido encontrada uma assimetria de 17,81 ± 14,64%, não foi encontrada diferença estatística (p = 0,817) entre o desempenho do salto vertical dominante (36,50 ± 7,36 cm) e não dominante (35,92 ± 5,63 cm). Além disso, foi encontrada correlação entre o teste de 20 metros e os valores de salto para perna dominante (p = 0,042), mas não para perna não dominante (p = 0,704). Em conclusão, embora tenha sido encontrada assimetria entre membros do lado dominante e não dominante do campo, não houve diferença no salto vertical

Abstract Objective: To provide guidelines on the coronavirus disease 2019 (COVID-19) vaccination in patients with immune-mediated rheumatic diseases (IMRD) to rheumatologists considering specific scenarios of the daily practice based on the shared-making decision (SMD) process. Methods: A task force was constituted by 24 rheumatologists (panel members), with clinical and research expertise in immunizations and infectious diseases in immunocompromised patients, endorsed by the Brazilian Society of Rheumatology (BSR), to develop guidelines for COVID-19 vaccination in patients with IMRD. A consensus was built through the Delphi method and involved four rounds of anonymous voting, where five options were used to determine the level of agreement (LOA), based on the Likert Scale: (1) strongly disagree; (2) disagree, (3) neither agree nor disagree (neutral); (4) agree; and (5) strongly agree. Nineteen questions were addressed and discussed via teleconference to formulate the answers. In order to identify the relevant data on COVID-19 vaccines, a search with standardized descriptors and synonyms was performed on September 10th, 2021, of the MEDLINE, EMBASE, Cochrane Central Register of Controlled Trials, ClinicalTrials.gov, and LILACS to identify studies of interest. We used the Newcastle–Ottawa Scale to assess the quality of nonrandomized studies. Results: All the nineteen questions-answers (Q&A) were approved by the BSR Task Force with more than 80% of panelists voting options 4—agree—and 5—strongly agree—, and a consensus was reached. These Guidelines were focused in SMD on the most appropriate timing for IMRD patients to get vaccinated to reach the adequate covid-19 vaccination response. Conclusion: These guidelines were developed by a BSR Task Force with a high LOA among panelists, based on the literature review of published studies and expert opinion for COVID-19 vaccination in IMRD patients. Noteworthy, in the pandemic period, up to the time of the review and the consensus process for this document, high-quality evidence was scarce. Thus, it is not a substitute for clinical judgment.

ABSTRACT Purpose: to investigate what effects lingual frenotomy has on breastfeeding. Methods: based on the research question - “Is there a relationship between the improvement in breastfeeding and the lingual frenotomy?” -, a bibliographical survey was carried out in three databases, using the following descriptors: ankyloglossia, lingual frenum, breastfeeding and frenotomy. Frenotomy is a free term. Original articles with babies up to 6 months old who had difficulties breastfeeding due to ankyloglossia and who had been submitted to lingual frenotomy were chosen. An instrument was developed, containing the following information: name of the authors, year of publication, country of origin, the objective of the study, type of study, sample, main results, and conclusion. Literature Review: of the 243 articles screened, four met the inclusion criteria for this study. It was observed that, after lingual frenotomy, some changes may take place in breastfeeding patterns, such as an increase in the number of suctions and a decrease in the time of pause in between series of suctions. Conclusion: the lingual frenotomy can be an important procedure in the treatment of babies with ankyloglossia, possibly helping improve the latch and breastfeeding.

ABSTRACT The prevalence of immunity to Chikungunya virus (CHIKV) in pregnant women and newborns in the Western Brazilian Amazon was assessed at a time when previous studies did not report chikungunya fever in the area. In 435 asymptomatic pregnant women and 642 healthy unrelated newborns, the presence of IgM and IgG antibodies to CHIKV were determined by a commercial ELISA. All participants were negative to IgM anti-CHIKV. Anti-CHIKV IgG was identified in 41 (9.4%) pregnant women and 66 (10.3%) newborns. The presence of anti-CHIKV IgG was positively associated with the lowest socioeconomic status in pregnant women (OR 2.54, 95% CI 1.15-5.62, p=0.021) and in the newborns’ mothers (OR 5.10, 95% CI 2.15-12.09, p< 0.001). Anti-CHIKV IgG was also associated with maternal age in both, the pregnant women (OR 1.06, 95% CI 1.00-1.11, p=0.037) and the newborns’mothers (OR 1.08, 95% CI 1.03-1.12, p=0.001). Pregnancy outcomes in which the mother or the newborn was anti-CHIKV IgG positive proceeded normally. Negative CHIKV serology was associated with being positive for DENV antibodies and having had malaria during pregnancy. These findings showed that there was already a silent circulation of CHIKV in this Amazon region before the first outbreak of chikungunya fever. Furthermore, seropositivity for CHIKV was surprisingly frequent (10%) in both, pregnant women and newborns, affecting mainly low-income women.

ABSTRACT A strategy for genetic improvement of coffee Coffea canephora plants is to aggregate through artificial crossings the characteristics of the Conilon botanical variety, such as shorter height and drought resistance, with the higher average grain size and resistance to pests and diseases of the Robusta variety. Efficiently separating the clones into these two groups with the aid of appropriate analytical procedures makes field tasks easier for professionals and, thus, allows the systematic production of intervarietal hybrids. This study verifies if the non-parametric discriminant analyzes of the k-nearest neighbors (k-NN) and k-average neighbors (k-AN) would be able to correctly classify 130 coffee clones in their botanical varieties previously designated as Conilon, Robusta and Intervarietal Hybrids populations from ten quantitative agronomic characteristics, including the processed coffee beans yield, considering the existing population genetic divergence. These characteristics were found to be good discriminatory variables and the discriminant analyzes k-NN and k-AN, based on the principle of similarity by neighborhood, classified the clones with high hit rates. The k-AN discriminant analysis was able to better discriminate intervarietal hybrids from the group clones Conilon. The results correctly reflected the genetic diversity between the botanical varieties and intervarietal hybrids of Coffea canephora, allowing us to conclude that these classification methods can assist breeders in the main task of discriminating Conilon from Robusta clones.

Abstract Spondyloarthritis (SpA) is a group of chronic inflammatory systemic diseases characterized by axial and/or peripheral joints inflammation, as well as extra-articular manifestations. Over some decades, nonsteroidal anti-inflammatory drugs (NSAIDs) have been the basis for the pharmacological treatment of patients with axial spondyloarthritis (axSpA). However, the emergence of the immunobiologic agents brought up the discussion about the role of NSAIDs in the management of these patients. The objective of this guideline is to provide recommendations for the use of NSAIDs for the treatment of axSpA. A panel of experts from the Brazilian Society of Rheumatology conducted a systematic review and meta-analysis of randomized clinical trials for 15 predefined questions. The Grading of Recommendations, Assessment, Development and Evaluation methodology to assess the quality of evidence and formulate recommendations were used, and at least 70% agreement of the voting panel was needed. Fourteen recommendations for the use of NSAIDs in the treatment of patients with axSpA were elaborated. The purpose of these recommendations is to support clinicians’ decision making, without taking out his/her autonomy when prescribing for an individual patient.

Abstract Aims: This study aimed to present a technical evaluation proposal for the crawl stroke that can be used with large groups of swimmers, based on an observation sheet. Methods: Fifteen healthy university students aged between 18 and 30 years were chosen to participate in the study. The subjects were recorded swimming at a distance of 50 meters using the crawl technique at a comfortable and self-determined speed. The recordings simulated docent observation capacity. Five swimming teachers were selected to evaluate the proposed checklist and the subjects’ technique. An observation sheet was created based on references present in literature containing ten items that are considered fundamental for swimming movements. The study was divided into (i) checklist validation, (ii) intra-evaluator consistency, and (iii) inter-evaluator consistency. Results: The proposed checklist fulfilled the validity criteria, with intra-evaluator consistency varying between reasonable and substantial, with k varying between 0.36 and 0.73 respectively, while inter-evaluator consistency was deemed reasonable (k = 0.24). Conclusion: According to the results that were obtained, the suggested list is valid and adequate for what it proposes to do.

ABSTRACT Growth performance, organ biometrics, bone characteristics, and intestinal morphometry were evaluated in broilers fed a diet containing orange (Citrus sinensis L.) essential oil. A completely randomized design was used, with five treatments with orange essential oil (0, 100, 200, 300, and 400 mg kg−1 diet) and six replications with 20 birds per experimental unit. In the pre-starter phase, feed intake and weight gain of all birds linearly increased, while feed conversion decreased with the addition of orange essential oil in the feed. At day 21, bone density (Seedor Index) and body weight were higher in the birds that received the maximum level of essential oil (400 mg kg−1) compared with those not treated with essential oil. The observed effects resulted from the better functioning of the physiological mechanisms of digestion and absorption of nutrients, characterized by the increase in villus height. Glycemia and weights of gastrointestinal tract organs of broilers at 21 days of age were not influenced by the evaluated essential oil. The results show that the addition of phytogenic additives to the diet does not cause any physiological impairment in birds.

Abstract Psoriatic arthritis (PsA) is a chronic and systemic immune disease characterized by inflammation of peripheral and/ or axial joints and entheses in patients with psoriasis (PsO). Extra-articular and extracutaneous manifestations and numerous comorbidities can also be present. These recommendations replace the previous version published in May 2013. A systematic review of the literature retrieved 191 articles that were used to formulate 12 recommendations in response to 12 clinical questions, divided into 4 sections: diagnosis, non-pharmacological treatment, conventional drug therapy and biologic therapy. These guidelines provide evidence-based information on the clinical management for PsA patients. For each recommendation, the level of evidence (highest available), degree of strength (Oxford) and degree of expert agreement (interrater reliability) are reported.

Abstract Introduction: The efficacy of 20-minute whole blood clotting (WBCT20) and the Lee-White clotting time (LWCT) tests in diagnosing coagulation alterations from snakebites were compared. Methods: We evaluated 89 snakebite cases treated at the Hospital Regional do Juruá em Cruzeiro do Sul, Acre, Brazil. Results: WBCT20 results were normal in 33.7% and unclottable in 66.3% of cases, while LWCT results were normal in 23.6% and altered (prolonged or unclottable) in 76.4% of cases, with no significant differences. Conclusions: The WBCT20 is important for rapidly diagnosing coagulation alterations from snakebites. Furthermore, it is efficient, inexpensive, and can be deployed in isolated hospitals.
