SUMMARY OBJECTIVE: The aim of this study was to describe homocysteine concentrations in overweight and obese children and adolescents and relate them to blood pressure levels, renal function, and insulin resistance. METHODS: This is a cross-sectional and observational study with 64 overweight children and adolescents (mean age: 11.6±3.5 years) in outpatient follow-up. The following parameters were evaluated: body mass index z-score, waist-to-height circumference ratio, pubertal stage, blood pressure, serum homocysteine, glycemia, insulin, lipid profile, renal function, high-sensitivity C-reactive protein, microalbuminuria, and creatinuria. Statistical analysis: analysis of variance and logistic regression (dependent variable: homocysteine) (p<0.05). RESULTS: The mean body mass index z-score was 2.9±1.1. The mean homocysteine concentrations were 8.6±2.2 μmol/L (10th and 90th percentiles: 6.6 and 11.2 μmol/L, respectively), with no difference when compared with children with severe obesity and obesity/overweight (p=0.431). High values of waist-to-height ratio (93.8%), systolic blood pressure (18.8%), diastolic blood pressure (12.5%), glycemia (4.7%), low-density lipoprotein cholesterol (31.1%), triglycerides (35.9%), non-high-density lipoprotein cholesterol (34.4%), and microalbuminuria (21.9%) were obtained. The mean glomerular filtration rate was 122.9±24.6 mL/min/1.73 m². Homocysteine concentrations were not associated with any of the studied variables (R²=0.095). CONCLUSION: Homocysteine concentrations in overweight children and adolescents (mean 8.6±2.2 μmol/L) were not associated with body mass index z-score, blood pressure, renal function, and insulin resistance.

Abstract Background The diagnosis of cognitive disorders in Parkinson disease (PD) can be very challenging. Aiming at establishing uniform and reliable diagnostic procedures, the International Parkinson’s Disease and Movement Disorder Society (MDS) commissioned task forces to delineate diagnostic criteria for mild cognitive impairment (MCI) and dementia in PD. Objectives To investigate the applicability of the MDS recommendations for cognitive evaluation in a Brazilian sample of patients with PD with low levels of formal education. Methods A total of 41 patients with PD were subjected to a comprehensive neuropsychological evaluation based on tests proposed by the MDS, which included the Mini-Mental State Examination, the Mattis Dementia Rating Scale (MDRS), the Trail Making Test (TMT) parts A and B, in addition to language and memory skills assessment. Neuropsychiatric and daily functioning features were also evaluated. Spearman correlation analyses were used to evaluate the association between the scores obtained in the cognitive scales and demographic/clinical variables. Results Although none of the participants had a formal diagnosis of dementia, 50% presented some degree of cognitive impairment when considering the results of the MDRS. Of note, a noticeable number of patients was not able to complete the full neuropsychological assessment. The TMT part B was the most difficult task, being completed by only 22 participants (54%). As expected, the greater the educational level, the better the performance on the cognitive tests. Better motor function was also associated with better scores in cognition. Conclusions Adopting strict inclusion/exclusion criteria and a comprehensive clinical evaluation, we found remarkable limitations for the MDS recommendations when individuals with low educational levels are considered. A revision of the current guidelines is necessary considering differences among populations, especially related to formal education.

Resumo Antecedentes O diagnóstico de distúrbios cognitivos na doença de Parkinson (DP) pode ser muito desafiador. Com o objetivo de estabelecer procedimentos diagnósticos uniformes e confiáveis, a Sociedade Internacional da Doença de Parkinson e Distúrbios do Movimento (MDS, na sigla em inglês) encomendou forças-tarefa para delinear critérios diagnósticos para comprometimento cognitivo leve (CCL) e demência na DP. Objetivos Investigar a aplicabilidade das recomendações da MDS para avaliação cognitiva em uma amostra brasileira de pacientes com DP de baixa escolaridade. Métodos Um total de 41 pacientes com DP foram submetidos a uma avaliação neuropsicológica abrangente com base nos testes propostos pela MDS, que incluiu o Miniexame do Estado Mental, a Escala de Avaliação de Demência de Mattis (MDRS, na sigla em inglês), o teste de trilhas (TMT, na sigla em inglês) partes A e B, além da avaliação das habilidades de linguagem e memória. Características neuropsiquiátricas e de funcionamento diário também foram avaliadas. Análises de correlação de Spearman foram utilizadas para avaliar a associação entre os escores obtidos nas escalas cognitivas e variáveis demográficas/clínicas. Resultados Apesar de nenhum dos participantes ter diagnóstico formal de demência, 50% apresentaram algum grau de comprometimento cognitivo ao levar em consideração os resultados da MDRS. Vale ressaltar que um número notável de pacientes não conseguiu completar a avaliação neuropsicológica completa. A parte B do TMT foi a tarefa mais difícil, sendo realizada por apenas 22 participantes (54%). Como esperado, quanto maior o nível educacional, melhor o desempenho nos testes cognitivos. Melhor função motora também foi associada a melhores escores em cognição. Conclusões Adotando critérios rígidos de inclusão/exclusão e uma avaliação clínica abrangente, encontramos limitações marcantes para as recomendações da MDS quando considerados indivíduos com baixa escolaridade. É necessária uma revisão das diretrizes atuais considerando as diferenças entre as populações, principalmente relacionadas ao nível educacional.

ABSTRACT Objective To compare serum amyloid A concentrations between overweight and eutrophic children and adolescents and to relate it to lipid profiles, glucose tolerance, and carotid intima-media thickness. Methods One hundred children and adolescents (mean age: 10.8±3.16 years) were included and divided into two groups: overweight and non-overweight. The following were evaluated: Z-score body mass index, carotid intima-media thickness, lipid metabolism biomarkers (lipid profile and apolipoproteins A1 and B), inflammatory biomarkers (ultra-sensitive C-reactive protein and serum amyloid A), and glucose homeostasis model assessment of insulin resistance. Results The groups were homogeneous in age, sex, and pubertal stage. Higher levels of triglycerides, apolipoprotein B, homeostasis model assessment of insulin resistance, ultrasensitive C-reactive protein, serum amyloid A, and carotid intima-media thickness were observed in the overweight group. In the multivariate analysis, age (OR=1.73; 95%CI: 1.16-2.60, p=0.007), Z-score body mass index (OR=3.76; 95%CI: 1.64-8.59, p=0.002), apolipoprotein-B (OR=1.1; 95%CI: 1.01-1.2, p=0.030), and carotid intima-media thickness (OR=5.00; 95%CI: 1.38-18.04, p=0.014) were independently associated with serum amyloid A levels above the fourth quartile of the studied sample (>9.4mg/dL). Conclusion Overweight children and adolescents had higher serum amyloid A concentrations than eutrophic children. There was an independent association between higher concentrations of serum amyloid A and Z-score, body mass index, apolipoprotein B, and carotid intima-media thickness, indicating the importance of this inflammatory biomarker in identifying the early risk of atherosclerosis.

Abstract: Introduction: supporting the constitutional rights of people with disabilities includes meeting new curricular guidelines and requires higher education that considers diversity in different contexts. Thus, the undergraduate curricula of health professionals must meet the multifaceted needs of people with disabilities in a way that transcends the biomedical model and permeates the promotion of an inclusive culture. Attitudes and values about disability often influence people’s behavior, making it imperative to introduce this topic. Aim: the present study aim to describe an approach in the curricula and teachers’ conceptions about children and adolescents with disabilities in medical courses. Method: this exploratory, cross-sectional study with a qualitative approach included the documental analysis of official documents of the courses and official pages of the universities, published until the first semester of 2021 and interviews of teachers participating in the course. The content analysis technique was used for the interview material. Results: the documental analysis pointed in the general topics a direction towards a humanistic formation, while respecting the differences; however, when the topic is the disability, the focus was centered on biological issues. During the interviews, the following topics were identified: 1) The importance of the Unified Health System as a space for medical training; 2) The contribution of the National Curriculum Guidelines in guiding the activities of teachers and highlighting relevant points; 3) Development of teaching activities aimed at children and adolescents with disabilities, based on the teachers’ experiences; 4) Conception about the person with disabilities and teacher training; 5) Teacher training for the development of teaching activities about children and adolescents with disabilities and knowledge about public policies in the area. Conclusions: a weakness was identified in the approach to the subject, with specific insertions on the subject, usually from the perspective of the disease and from the opportunities that arise. Among the teachers, the predominance of the biomedical perspective was identified, as well as the lack of training on the subject and its fragile insertion in academic practice. However, the importance of the topic was recognized and its potential to integrate the curricular matrix in a cross-sectional way.

Resumo: Introdução: Amparar os direitos constitucionais das pessoas com deficiência inclui atender aos novos direcionamentos curriculares e exige um ensino superior que considere a diversidade em diversos contextos. Assim, os currículos de graduação de profissionais de saúde devem atender às necessidades multifacetadas das pessoas com deficiência de modo que transcendam o modelo biomédico e perpassem pela promoção da cultura inclusiva. As atitudes e os valores sobre a deficiência frequentemente influenciam o comportamento das pessoas, tornando imperativa a introdução desta temática. Objetivo: Este estudo teve como objetivo descrever a abordagem nos currículos e as concepções de professores sobre crianças e adolescentes com deficiência em cursos de Medicina. Método: Trata-se de um estudo exploratório, transversal e de abordagem qualitativa que incluiu a análise documental de documentos oficiais dos cursos e páginas oficiais das universidades publicados até o primeiro semestre de 2021, e entrevistas de professores participantes do curso. A análise de conteúdo foi a técnica utilizada para o material das entrevistas. Resultado: A análise documental apontou nos tópicos gerais um direcionamento para uma formação humanística, com respeito às diferenças. Entretanto, quando a temática é a deficiência, o enfoque foi centrado nas questões biológicas. Nas entrevistas, identificaram-se os seguintes eixos temáticos: 1. a importância do Sistema Único de Saúde como espaço de formação médica; 2. a contribuição das Diretrizes Curriculares Nacionais na orientação das atividades dos docentes e destaque a pontos relevantes; 3. desenvolvimento de atividades de ensino voltadas à criança e ao adolescente com deficiência, com base nas experiências dos docentes; 4. concepção sobre a pessoa com deficiência e formação docente; 5. formação docente para o desenvolvimento de atividades de ensino sobre a criança e o adolescente com deficiência e conhecimento sobre as políticas públicas da área. Conclusão: Foi identificada uma fragilidade na abordagem da temática, com inserções pontuais sobre o tema, geralmente na perspectiva da doença e a partir de oportunidades surgidas. Entre os docentes, identificaram-se a predominância do olhar biomédico, a ausência de formação para a temática e a frágil inserção desta na prática acadêmica. Entretanto, houve reconhecimento da importância do tema e seu potencial em integrar a matriz curricular de forma transversal.

Abstract Fruit ripening promoted by the exogenous application of ethylene analogs, such as calcium carbide, has commercial advantages. Thus, the knowledge of the responses of fruits treated with ethylene-inducing agents is essential to optimize the use of these substances. This work aimed to evaluate the influence of exposure to calcium carbide on the anticipation and standardization of postharvest ripening of cajá-manga fruits. Physiologically mature fruits were exposed to calcium carbide for 24 hours at concentrations 0, 20, 40, 80, and 110 g m-3. The fruits were stored at a temperature of 28±2 °C and evaluated at 0, 2, 4, and 6 days for the loss of fresh mass, color of the epidermis and pulp given by the CIELAB color space, titratable acidity, soluble solids content, the ratio between soluble solids content and titratable acidity, and vitamin C content. Cajá-manga fruits treated with different concentrations of calcium carbide had their ripening anticipated without compromising their characteristics. The concentrations of 20, 40, and 80 g m-3 of calcium carbide allowed the anticipation and standardization of fruit ripening within four days during storage, while for the highest concentration (110 g m-3), complete maturation was accelerated, occurring between two and four days of storage.

Resumo O amadurecimento de frutos promovido pela aplicação exógena de análogos de etileno, como o carbureto de cálcio, tem vantagens comerciais. Assim, o conhecimento das respostas de frutos tratados com agentes indutores de etileno é essencial para otimizar o uso dessas substâncias. Este trabalho teve como objetivo avaliar a influência da exposição ao carbureto de cálcio na antecipação e na padronização do amadurecimento pós-colheita de frutos de cajá-manga. Frutos fisiologicamente maduros foram expostos ao carbureto de cálcio nas concentrações de 0; 20; 40; 80 e 110 g m-3 por 24 horas. Os frutos foram armazenados a uma temperatura de 28±2 °C e avaliados aos 0; 2; 4 e 6 dias quanto à perda de massa fresca, cor da epiderme e polpa dada pelo espaço de cor CIELAB, acidez titulável, teor de sólidos solúveis, relação entre o teor de sólidos solúveis e a acidez titulável, e teor de vitamina C. Frutos de cajá-manga tratados com diferentes concentrações de carbureto de cálcio tiveram seu amadurecimento antecipado sem comprometer suas características. As concentrações de 20; 40 e 80 g m-3 de carbureto de cálcio permitiram a antecipação e a padronização do amadurecimento dos frutos em quatro dias, durante o armazenamento, Enquanto para a maior concentração (110 g m-3), a maturação completa foi acelerada, ocorrendo entre dois e quatro dias de armazenamento.

ABSTRACT Objective: To evaluate the association of oral health-related quality of life (OHRQoL) with the longevity of ART-restorations in children after 18 months. Material and Methods: A longitudinal clinical study with 62 six- to seven-year-old children, both genders, 31 of whom underwent four weeks of oral health education strategy (OHES) followed by ART (GOHES+ART) and the others only ART (GART). The Brazilian short-version of the Child Perceptions Questionnaire (16-CPQ8-10) was used to evaluate the OHRQoL and the perception of change in oral health was assessed by a single question. Data was analyzed using Mann-Whitney, Wilcoxon and Friedman's tests. Results: After 18 months, GOHES+ART reported a greater impairment on OHRQoL, mainly in oral symptoms domain (p<0.05). In this period, greater impairment of oral symptoms was reported in children with failed restorations and with treatment considered to have a minor failure (p<0.05). In inter-group analysis, the GOHES+ART, with 2 to 4 restorations, Class I, of small size, located in the upper arch and with a smaller failure, reported more oral symptoms than the GART. Conclusion: In the long-term, children submitted to OHES followed by ART reported more oral symptoms. Factors such as number, class, size and location of the restorations need to be considered for the implementation of an efficient and effective oral health program, as well as for greater longevity of ART-restorations.

Resumo A ontogenia inicial de Triportheus albus e T. angulatus, duas espécies de peixes pertencentes a Triportheidae, é descrita usando caracteres morfológicos, merísticos e morfométricos. Essas espécies são exploradas pela pesca de subsistência e têm potencial como fonte alternativa de pescado, dado o declínio nos estoques naturais de outras espécies de peixes comercialmente importantes na Amazônia. Os espécimes foram coletados na zona limnética de águas abertas, sob bancos de macrófitas e em áreas subsuperficiais próximas a bancos de areia na bacia amazônica. Análises morfométricas, intra e interespecíficas, foram realizadas para avaliar modelos de crescimento entre as espécies. A combinação do padrão de coloração, morfologia corporal, proporções morfométricas e número de miômeros distingue as espécies entre si e de outras congêneres. Algumas relações morfométricas relacionadas à cabeça, como comprimento do focinho e diâmetro do olho, assim como o comprimento do focinho à origem da nadadeira anal e o comprimento do focinho à origem da nadadeira pélvica, relacionadas ao comprimento padrão foram diferentes entre as duas espécies de Triportheus, refletindo modelos distintos de crescimento entre elas. Abstract The early ontogeny of Triportheus albus and T. angulatus, two fish species of Triportheidae, is described using morphological, meristic, and morphometric characters. These species are exploited by subsistence fisheries and have potential as an alternative source of fish, given the decline in the natural stocks of other commercially important fish species in the Amazon. The specimens were collected in the open water limnetic zone, under of the macrophyte stands, and in subsurface areas near sandbars in the Amazon basin. Intra and interspecific morphometric analyzes were performed to evaluate growth models between species. The combination of color pattern, body morphology, morphometric proportions and myomeres number distinguishes the species from each other and from other congeners. Some morphometric relationships related to head as snout length and eye diameter as well as length from the snout to the origins of anal and length from the snout to the origins of pelvic, related with standard length were different between the two species of Triportheus, reflecting different growth models between them. RESUMEN El objetivo de este estudio es mapear la producción científica sobre las estrategias propuestas y/o utilizadas para enfrentar las infodemias con enfoque en la preservación de la salud de los adultos y ancianos en el contexto de la Pandemia del COVID-19. Esta es una revisión de alcance, desarrollada utilizando la metodología del Instituto Joanna Briggs (JBI), que incluyó 27 artículos. Estos artículos fueron analizados según el tipo de estudio, estrategias de afrontamiento utilizadas, principales resultados y limitaciones de los estudios. Las estrategias de afrontamiento encontradas frente a la infodemia se dividieron en estrategias de afrontamiento utilizadas por adultos y ancianos y estrategia sugerida por los autores frente a la infodemia. Conocer estas nuevas herramientas de afrontamiento utilizadas por adultos y ancianos es fundamental para el desarrollo de nuevas estrategias y mejora de las ya disponibles, ya que se encontraron pocos artículos específicos sobre el tema. Por lo tanto, se sugieren futuros estudios con artículos más robustos que llenen los vacíos encontrados.

ABSTRACT The objective of this study is to map the scientific production on the strategies proposed and/or used to face infodemics with a focus on preserving the health of adults and the elderly in the context of the COVID-19 Pandemic. This is a Scope review, developed using the Joanna Briggs Institute (JBI) methodology, which included 27 articles. These articles were analyzed according to the type of study, coping strategies used, main results and limitations of the studies. The coping strategies found against the infodemic were divided into coping strategies used by adults and the elderly and authors' suggested strategy against the infodemic. Knowing these new coping tools used by adults and the elderly is essential for the development of new strategies and improvement of those that are already available, since few specific articles on the subject were found. Therefore, future studies are suggested with more robust articles that fill in the gaps found.

RESUMO O objetivo desse estudo é mapear a produção científica sobre as estratégias propostas e/ou utilizadas para o enfrentamento a infodemias com foco à preservação da saúde adultos e idosos no contexto da Pandemia de COVID-19. Trata-se de uma revisão de escopo, desenvolvida com a metodologia do Joanna Briggs Institute (JBI), que incluiu 27 artigos. Esses artigos foram analisados de acordo com tipo de estudo, estratégias de enfrentamento utilizadas, principais resultados e limitações dos estudos. As estratégias de enfrentamento encontradas contra a infodemia foram divididas entre estratégias de enfrentamento utilizadas pelos adultos e idosos e sugestões de estratégia de autores contra a infodemia. Conhecer essas novas ferramentas de enfrentamento utilizadas pelos adultos e idosos são essenciais para o desenvolvimento de novas estratégias e aprimoramento das que já estão disponíveis, já que foram encontrados poucos artigos específicos sobre o tema. Sendo assim, sugere-se estudos futuros com artigos mais robustos que preencham as lacunas encontradas.

Resumen Se presenta un informe de experiencia de un Curso de Extensión en formación continua dirigido a psicólogos escolares que actuán en contextos de educación pública y se discuten sus impactos en el desempeño profesional. Las reuniones del curso tuvieron lugar quincenalmente, de marzo a diciembre de 2019, y contaron con la participación de 12 psicólogas de Paraíba, un estado del nordeste de Brasil. El curso tiene como objetivo asesorar y monitorear la actividad de los profesionales y favorecer la apropiación de herramientas teóricas, metodológicas y técnicas para el contexto educativo escolar. Las reuniones fueron mediadas por textos sobre psicología escolar y elementos estéticos y fueron registradas en actas que contenían los relatos de los participantes sobre los impactos de la profundización ética, teórica, metodológica y técnica en sus prácticas. Al final del curso, se verificó: la construcción colaborativa de procesos de acción-reflexión-acción en el desarrollo de actividades; el reconocimiento del protagonismo y valor del saber y de las estrategias utilizadas por las profesionales en su cotidiano institucional; y la motivación para seguir con la formación continua.

Resumo Apresenta-se um relato de experiência de um Curso de Extensão em formação continuada direcionada a psicólogas escolares que atuam em contextos públicos de educação e discutem-se seus impactos na atuação profissional. Os encontros ocorreram quinzenalmente, entre os meses de março e dezembro do ano de 2019, e contaram com a participação de 12 psicólogas escolares da Paraíba, um estado do Nordeste brasileiro. O curso objetiva assessorar e acompanhar as atividades das profissionais e favorecer a apropriação de ferramentas teóricas, metodológicas e técnicas para o contexto escolar educacional. As reuniões foram mediadas por textos sobre a psicologia escolar e elementos estéticos. Os encontros foram registrados em atas que continham os relatos das participantes sobre os impactos do aprofundamento ético, teórico, metodológico e técnico em suas práticas. Ao final do curso, verificou-se: a construção colaborativa de processos de ação-reflexão-ação no desenvolvimento de atividades; o reconhecimento do protagonismo e do valor do saber e das estratégias utilizadas pelas profissionais em seu cotidiano institucional; e a motivação para dar seguimento à formação continuada.

Abstract An experience report of an Extension Course in continuing education aimed at school psychologists working in public education contexts is presented, and its impacts on professional performance are discussed. The course took place fortnightly, from March to December of 2019, and were attended by 12 psychologists from Paraíba, a northeastern Brazilian state. The course aims to advise and monitor the activities of the professionals and to favor the appropriation of theoretical, methodological and technical tools for the educational school context. The meetings were mediated by texts on school psychology and aesthetic elements and were recorded in minutes containing the participants' reports on the impacts of ethical, theoretical, methodological and technical deepening in their practices. At the end of the course, there was: a collaborative construction of action-reflection-action processes in the development of activities; the recognition of the protagonism and value of knowledge and of the strategies used by professionals in their institutional daily life; and the motivation to carry on with continuing education.

ABSTRACT Introduction: Systemic Mastocytosis comprises a group of neoplastic diseases characterized by clonal expansion and infiltration of mast cells into several organs. The diagnosis and treatment of this disease may be challenging for non-specialists. Objective: Make suggestions or recommendations in Systemic Mastocytosis based in a panel of Brazilian specialists. Method and results: An online expert panel with 18 multidisciplinary specialists was convened to propose recommendations on the diagnosis and treatment of Systemic Mastocytosis in Brazil. Recommendations were based on discussions of topics and multiple-choice questions and were graded using the Oxford Centre for Evidence-Based Medicine 2011 Levels of Evidence Chart. Conclusion: Twenty-two recommendations or suggestions were proposed based on a literature review and graded according to the findings.

SUMMARY OBJECTIVE: This study aimed to compare neutrophil-to-lymphocyte ratio and platelet-to-lymphocyte ratio of overweight children and adolescents with the eutrophic ratios and to verify whether these ratios are associated with age, inflammation, Z-score of body mass index, and waist-to-height ratio. METHODS: This is a cross-sectional study involving 64 overweight and 106 eutrophic children and adolescents. Data on weight, height, and waist circumference (body mass index and waist-to-height ratio), blood count (neutrophil-to-lymphocyte ratio and platelet-to-lymphocyte ratio), and high-sensitivity C-reactive protein were collected. RESULTS: The mean age of participants was 8.4±3.2 years. The ratios did not differ between the overweight and non-overweight groups. The platelet-to-lymphocyte ratio has shown a direct and independent association with body mass index (p=0.031) and waist-to-height ratio (p=0.018), a fact not observed for neutrophil-to-lymphocyte ratio. The ultrasensitive C-reactive protein level was higher in the obesity group (p=0.003). Both ratios had a direct and independent association with age. CONCLUSION: The ratios did not differ between the overweight and non-overweight groups. There was a direct and independent association of platelet-to-lymphocyte ratio with overweight, not observed in neutrophil-to-lymphocyte ratio. The ratios have significantly increased according to the age of the participants.

ABSTRACT Among the less exploited Brazilian native fruit trees, the Spondias dulcis (cajá-manga) has many uses, being their fruits appreciated in fresh consumption and its pulp used in popsicles, ice-creams, and jellies production. However, there is a lack of research on this species, being necessary more studies. This work aimed to characterize the physical and chemical quality and the genetic diversity of Spondias dulcis fruits harvested in different accessions in the municipality of Jataí - GO. The experiment was carried out in a completely randomized design with six treatments (accessions), ten replications with ten fruits per plot, totaling a sample of 600 fruits. The fruits were evaluated for pulp and peel color, pulp yield, soluble solids content, titratable acidity, soluble solids content/titratable acidity ratio, and vitamin C content. The fruits from accessions M4 and M5 presented physical and chemical characteristics higher than the other plants. The evaluated accessions presented genetic variability, being the methodology employed efficiently to indicate this variability, in which the accession M2 was the most divergent.

Abstract Introduction Leg ulcers (LUs) are relatively common in patients with sickle cell anemia (SCA). The role of inflammation and nitric oxide (NO) pathways in the pathophysiology of the LU is not understood. Objective The aim of this study was to verify the association between inflammatory molecules and nitric oxide metabolites (NOx) and the occurrence of the LU in patients with SCA. Method It was a cross-sectional study on adult participants with SCA followed at Fundação Hemominas, a public blood center in Brazil. Eligible participants were recruited and included in one of two groups: Group 1, comprised of cases with SCA (Hb SS) and at least one LU at the time of inclusion in the study and Group 2, comprised of controls with SCA without a history of LU, matched by sex and age to cases. Participants were interviewed to obtain sociodemographic data and blood samples were collected. Clinical and laboratory data were abstracted from medical records. Nitric oxide metabolites (NOx) and inflammatory molecules were quantified using an immunoassay and Multiplex xMAP® technology, respectively. Eighty-seven individuals were included, ranging in age from 17 to 61 years (mean 40 ± 10.7 years); 30 had LU and 57 were controls without LU. Results Participants with LU had significantly higher levels of interleukin 8 (IL-8), IL-10, IL-15, NOx and platelet and white blood cell (WBC) counts, when compared to those without LU. Participants with LU had a significantly higher risk of having a history of osteomyelitis and a higher use of antiseptic soap in bathing, when compared to those without LU. Conclusion In conclusion, our results showed that NOx, inflammatory molecules and hematological features were associated with LU in Brazilian adults with SCA.

Resumo Fundamento A espirometria é subutilizada na insuficiência cardíaca (IC) e não está claro o grau de associação de cada defeito com a capacidade de exercício e com o prognóstico desses pacientes. Objetivo Determinar a relação da %CVF prevista (ppCVF) e do VEF1/CVF contínuos com: 1) pressão inspiratória máxima (PImáx), fração de ejeção do ventrículo esquerdo (FEVE) e desempenho ao exercício; e 2) prognóstico, para o desfecho composto de morte cardiovascular, transplante cardíaco ou implante de dispositivo de assistência ventricular. Métodos Coorte de 111 participantes com IC (estágios AHA C/D) sem pneumopatia; foram submetidos a espirometria, manovacuometria e teste cardiopulmonar máximo. As magnitudes de associação foram verificadas por regressões lineares e de Cox (HR; IC 95%), ajustadas para idade/sexo, e p <0,05 foi considerado significativo. Resultados Com idade média 57±12 anos, 60% eram homens, 64% em NYHAIII. A cada aumento de 10% no VEF1/CVF [β 7% (IC 95%: 3-10)] e no ppCVF [4% (2-6)], foi associado à reserva ventilatória (VRes); no entanto, apenas o ppCVF associado à PImáx [3,8cmH2O (0,3-7,3)], à fração de ejeção do ventrículo esquerdo (FEVE) [2,1% (0,5-3,8)] e ao VO2 pico [0,5mL/kg/min (0,1-1,0)], considerando idade/sexo. Em 2,2 anos (média), ocorreram 22 eventos; tanto FEV1/FVC (HR 1,44; IC 95%: 0,97-2,13) quanto ppCVF (HR 1,13; 0,89-1,43) não foram associados ao desfecho. Apenas no subgrupo FEVE ≤50% (n=87, 20 eventos), VEF1/CVF (HR 1,50; 1,01-2,23), mas não ppCVF, foi associado a risco. Conclusão Na IC crônica, ppCVF reduzido associou-se a menor PImáx, FEVE, VRes e VO2 pico, mas não distinguiu pior prognóstico em 2,2 anos de acompanhamento. Entretanto, VEF1/CVF associou-se apenas com VRes, e, em participantes com FEVE ≤50%, o VEF1/CVF reduzido mostrou pior prognóstico proporcional. Portanto, VEF1/CVF e ppFVC contribuem para melhor fenotipagem de pacientes com IC.

Abstract Background Spirometry is underused in heart failure (HF) and the extent to which each defect associates with exercise capacity and prognosis is unclear. Objective To determine the distinct relationship of continuous %predicted FVC (ppFVC) and FEV1/FVC with: 1) maximal inspiratory pressure (MIP), left ventricular ejection fraction (LVEF), exercise performance; and 2) prognosis for the composite of cardiovascular death, heart transplantation or left ventricular assist device implant. Methods A cohort of 111 HF participants (AHA stages C/D) without diagnosed pneumopathy, spirometry, manovacuometry and maximum cardiopulmonary test. The association magnitudes were verified by linear and Cox (HR; 95% CI) regressions, age/sex adjusted. A p<0.05 was considered significant. Results Age was 57±12 years, 60% men, 64% in NYHA III. Every 10%-point increase in FEV1/FVC [β 7% (95% CI: 3–10)] and ppFVC [4% (2-6)] associated with ventilatory reserve (VRes), however only ppFVC associated with MIP [3.8 cmH2O (0.3-7.3)], LVEF [2.1% (0.5-3.8)] and VO2peak [0.5 mL/kg/min (0.1–1.0)], accounting for age/sex. In 2.2 years (mean), 22 events occurred, and neither FEV1/FVC (HR 1.44; 95% CI: 0.97–2.13) nor ppFVC (HR 1.13; 0.89–1.43) was significantly associated with the outcome. Only in the LVEF ≤50% subgroup (n=87, 20 events), FEV1/FVC (HR 1.50; 1.01–2.23), but not ppFVC, was associated with greater risk. Conclusions In chronic HF, reduced ppFVC associated with lower MIP, LVEF, VRes and VO2peak, but no distinct poorer prognosis over 2.2 years of follow-up. Distinctively, FEV1/FVC was associated only with VRes, and, in participants with LVEF ≤50%, FEV1/FVC reduction proportionally worsened prognosis. Therefore, FEV1/FVC and ppFVC add supplementary information regarding HF phenotyping.

SUMMARY: The complete petrosphenoidal foramen, or canal, is an eventual and atavistic bony formation at the boundary between the posterior and middle cranial fossa, by occurrence of ossification of the superior petrosphenoidal ligament. This ligament ossification, which has important clinical and surgical significance, can be complete or incomplete, in variable degrees, and is associated with the passageway of neurovascular structures, such as the abducens nerve and the inferior petrosal sinus. This study, conducted with 175 dry skulls that belong to the University of São Paulo’s collection (USP), São Paulo, Brazil, established criteria for a morphological classification of the incomplete petrosphenoid foramen in nine types. In addition, anatomical parameters were established for the morphometric determination of two diameters: the Oblique Diameter (ObDi) and the Maximum Transverse Diameter (MTD). Thus, of the 175 skulls, 146 (83.42 %) presented some of the incomplete forms of the petrosphenoid foramen, and 43 skulls (29.45 %), due to their conservation characteristics, were habilitated to the morphological study, in the classification and in the morphometry (the types I and II of our classification). The type II (incomplete foramen with bony projections of the petrosal tubercle, of the margin of the dorsum of the hypophyseal fossa or of the posterior clinoid process with a distance between them greater than 1mm) and type V (incomplete foramen with a bony projection only in one of the referential structures - posterior clinoid process) were the most common in this study (50 % of the 86 hemiskulls). Morphometry was attributed only to the types: I selar (incomplete foramen with bony projections from the petrosal tubercle and the margin of the dorsum of the hypophyseal fossa with a distance between them less than or equal to 1mm) and to the type II of this classification. The type I selar (9.3 % of the 43 skulls) resulted in an average of 3.25 mm of MTD and 4.63 mm, on average, of ObDi. The type II (25.58 % of the 43 skulls) showed, on average, 4.93 mm of MTD and 7.01 mm of ObDi.

RESUMEN: El foramen o canal petroesfenoidal completo es una formación ósea eventual y atávica en el límite entre las fosas craneal posterior y media, por osificación del ligamento petroesfenoidal superior. Esta osificación del ligamento, que tiene un importante significado clínico y quirúrgico, puede ser completa o incompleta, en grados variables, y está asociada al paso de estructuras neurovasculares, como el nervio abducente y el seno petroso inferior. Este estudio se realizó en 175 cráneos secos pertenecientes a la colección de la Universidad de São Paulo (USP), São Paulo, Brasil. Se establecieron criterios para una clasificación morfológica del foramen petrosfenoidal incompleto en nueve tipos. Además, se establecieron parámetros anatómicos para la determinación morfométrica de dos diámetros: el Diámetro Oblicuo (ObDi) y el Diámetro Transversal Máximo (MTD). Así, de los 175 cráneos, 146 (83,42 %) presentaron alguna de las formas incompletas del foramen petrosfenoidal, y 43 cráneos (29,45 %), por sus características de conservación, fueron habilitados para el estudio morfológico, en la clasificación y en la morfometría (los tipos I y II de nuestra clasificación). El Tipo II (foramen incompleto con proyecciones óseas del tubérculo petroso, del margen del dorso de la fosa hipofisaria o del proceso clinoides posterior con una distancia entre ellos mayor de 1 mm) y el Tipo V (foramen incompleto con proyección ósea solamente en una de las estructuras referenciales - proceso clinoides posterior) fueron los más comunes en este estudio (50 % de los 86 hemiscráneos). La morfometría se atribuyó únicamente al Tipo I selar (foramen incompleto con proyecciones óseas desde el tubérculo petroso y el margen del dorso de la fosa hipofisaria con una distancia entre ellos menor o igual a 1mm) y al Tipo II de esta clasificación. El Tipo I selar (9,3 % de los 43 cráneos) resultó en un promedio de 3,25 mm de MTD y 4,63 mm, en promedio, de ObDi.
