Hiperglicemia no Perioperatório de Transplante de Fígado: Uma Revisão de Escopo

ABSTRACT Objectives: To find evidence on the influence of hyperglycemia in the perioperative period of liver transplantation. Methods: This is a scoping review of the PubMed, VHL, and Web of Science databases. The following descriptors were used: “Liver transplantation,” “Hyperglycemia,” and “Blood Sugar Control,” with the Boolean operator “AND,” and articles of relevance to the topic were selected. Initially, 139 articles were selected, all published in the last 20 years, in Portuguese and English. After analysis, 10 articles corresponded to the proposed objective. Results: In comparing strict control and conventional glucose control in patients undergoing liver transplantation, the average intraoperative glycemia was 143.3 mg/dL in the conventional group and 130.7 mg/dL in the strict control group, which received more insulin. The intensive care unit stay was similar with a mean of 3 days, but survival improved significantly with mean intraoperative glycemia ≤ 120 mg/dL. Patients with postreperfusion syndrome required insulin infusion more often. New-onset diabetes mellitus and early bacteremia were more frequent in the postreperfusion syndrome group. AST and ALT levels were higher in hyperglycemic patients. Surgical site infection rates correlated with postoperative hyperglycemia, with 20% for glycemia < 200 mg/dL. Patients with postoperative glucose < 200 mg/dL. Patients with diabetes had higher preoperative glycemia than patients without diabetes, with a gradual reduction after liver transplantation, whereas patients without diabetes had a peak on days 2 and 3 before returning to baseline levels. High glucose levels 48–72 h post-transplant were associated with higher mortality and a worse response to insulin treatment in the first week. Conclusion: Strict glycemic control in patients undergoing liver transplantation resulted in lower intraoperative glycemia and significantly better survival for those with glycemia ≤ 120 mg/dL, as well as lower rates of infection, rejection, and mortality, especially in patients without diabetes.
RESUMO Objetivos: Encontrar evidências sobre a influência da hiperglicemia no perioperatório de transplante de fígado. Métodos: Trata-se de uma revisão de escopo na base de dados PubMed, BVS e Web of Science. Foram utilizados os descritores: “Liver transplantation”, “Hyperglycemia” e “Blood Sugar Control” com o operador booleano “AND”, e selecionados artigos de relevância para o tema. Inicialmente, foram selecionados 139 artigos, todos publicados nos últimos 20 anos, em português e inglês. Após análise, 10 artigos corresponderam ao objetivo proposto. Resultados: Na comparação entre controle rigoroso e controle convencional da glicose em pacientes submetidos a transplante de fígado, a média de glicemia intraoperatória foi de 143,3 mg/dL no grupo convencional e 130,7 mg/dL no grupo de controle rigoroso, que recebeu mais insulina. A permanência na unidade de terapia intensiva foi similar, com média de 3 dias, mas a sobrevida melhorou significativamente com glicemia intraoperatória média ≤ 120 mg/dL. Pacientes com síndrome pós-reperfusão necessitaram mais frequentemente de infusão de insulina. O diabetes mellitus de início recente e a bacteremia precoce foram mais frequentes no grupo com síndrome pós-reperfusão. Níveis de AST e ALT foram mais elevados em pacientes hiperglicêmicos. As taxas de infecção do sítio cirúrgico correlacionaram-se com a hiperglicemia pós-operatória, sendo 20% para glicemia < 200 mg/dL. Pacientes com glicose pós-operatória < 200 mg/dL. Os pacientes com diabetes apresentaram glicemia pré-operatória mais alta que pacientes sem diabetes, com redução gradual após o transplante de fígado, enquanto os pacientes sem diabetes tiveram um pico nos dias 2 e 3 antes de retornarem aos níveis basais. Níveis elevados de glicose após 48–72 h pós-transplante foram associados a maior mortalidade e pior resposta ao tratamento com insulina na primeira semana. Conclusão: O controle rigoroso da glicemia em pacientes submetidos a transplante de fígado resultou em uma glicemia intraoperatória mais baixa e uma sobrevida significativamente melhor para aqueles com glicemia ≤ 120 mg/dL, além de menores taxas de infecção, rejeição e mortalidade, especialmente em pacientes sem diabetes.

ABSTRACT Mechanical ventilation can be a life-saving intervention, but its implementation requires a multidisciplinary approach, with an understanding of its indications and contraindications due to the potential for complications. The management of mechanical ventilation should be part of the curricula during clinical training; however, trainees and practicing professionals frequently report low confidence in managing mechanical ventilation, often seeking additional sources of knowledge. Review articles, consensus statements and clinical practice guidelines have become important sources of guidance in mechanical ventilation, and although clinical practice guidelines offer rigorously developed recommendations, they take a long time to develop and can address only a limited number of clinical questions. The Associação de Medicina Intensiva Brasileira and the Sociedade Brasileira de Pneumologia e Tisiologia sponsored the development of a joint statement addressing all aspects of mechanical ventilation, which was divided into 38 topics. Seventy-five experts from all regions of Brazil worked in pairs to perform scoping reviews, searching for publications on their specific topic of mechanical ventilation in the last 20 years in the highest impact factor journals in the areas of intensive care, pulmonology, and anesthesiology. Each pair produced suggestions and considerations on their topics, which were presented to the entire group in a plenary session for modification when necessary and approval. The result was a comprehensive document encompassing all aspects of mechanical ventilation to provide guidance at the bedside. In this article, we report the methodology used to produce the document and highlight the most important suggestions and considerations of the document, which has been made available to the public in Portuguese.

ABSTRACT Objective: To evaluate the color stability, surface roughness, and microhardness of resin-based composites (RBCs) after accelerated artificial aging (AAA) or immersion in beverages consumed by children. Material and Methods: Seventy specimens were prepared from each RBC (Z350-Z350, SS-Spectra Smart, AF-Admira Fusion, and EA-Estelite Asteria). Initial color, ∆E00, surface roughness, ∆Ra, and microhardness, ∆KHN readings were done. Specimens were separated into five groups (n=14): AAA (300 hours) and immersion (30 minutes daily/30 days) in Coca-Cola (CC), Grape juice (GJ), Chocolate milk (CM), or distilled water (DW). After the final readings, color and microhardness data were analyzed using two-way ANOVA, Tukey, and surface roughness by Kruskal-Wallis and Dunn's test. Results: SS exhibited the most significant color change in AAA, which differed from CC and GJ; the minor change occurred after CM and DW, which were distinct from each other. AF did not show a difference between the treatments. All RBCs exhibited a decrease in microhardness, except for SS, after AAA and EA in CC and DW. There was no difference in the surface roughness after treatments, except for AAA, which presented a minor change for Z350 and AF, differing from the other treatments. Conclusion: The beverages didn’t alter the surface roughness but decreased the microhardness of the RBCs, except for CC, which increased the microhardness of EA.

Abstract Background Although the clinical features of chronic Chagas’ cardiomyopathy (CCC) have been well established, clinical data about the patients are scarce. Objectives The current analysis reports the results of the I Brazilian Heart Failure Registry (BREATHE) assessing baseline characteristics and clinical outcomes of patients with acute heart failure due to CCC. Methods BREATHE enrolled a total of 3,013 adult patients hospitalized with acute heart failure. We analyzed comparatively 261 (8.7%) patients with chronic CCC and 2,752 (91.3%) patients with other etiologies. Clinical and demographic information, cardiac structure/function data on echocardiogram and outcomes during the hospital stay and after discharge were assessed in both groups. Uni and multivariate tests were performed and a p-value <0.05 was considered statistically significant. Results Patients with CCC presented lower systolic blood pressure (108.3 ± 26.1 vs 128.3 ± 30.3 mmHg, p<0.001) and left ventricle ejection fraction (25.4 [19 - 36]% vs 37 [27 - 54] %, p<0.001) with higher rates of jugular vein distension (54.8% vs 38.9%, p<0.001), hepatomegaly (47.9% vs 25.6%, p<0.001), and “cold and wet” clinical hemodynamic profile (27.2% vs 10.6%, p<0.001). Patients with CCC presented higher rate of the composite death or heart transplantation (17.4% vs. 11.1%, p=0.004), and higher cumulative incidence of death after 3 months (16.5% vs 10.8%, p=0.017), 6 months (25.3% vs 17.2%, p=0.006), and 12 months (39.4% vs 26.6%, p<0.001). Besides, CCC was independently associated with 12-month mortality risk with odds ratio = 2.02 (95% IC: 1.47-2.77). Conclusion Patients with CCC, hospitalized due to acute heart failure, in comparison to other etiologies, presented a higher risk profile that was associated with a poorer outcome during hospital stay and after discharge.
Resumo Fundamentos Apesar de as características clínicas da Cardiomiopatia Chagásica Crônica (CCC) estarem bem estabelecidas, os dados clínicos sobre os pacientes são escassos. Objetivos Esta análise relata os resultados do I Registro Brasileiro de Insuficiência Cardíaca (BREATHE) avaliando as características basais e desfechos clínicos de pacientes com insuficiência cardíaca aguda secundária à CCC. Métodos O BREATHE incluiu 3013 pacientes adultos internados com insuficiência cardíaca aguda. Nós analisamos comparativamente 261 (8,7%) pacientes com CCC e 2752 (91,3%) pacientes com outras etiologias. Dados clínicos e demográficos, informações da estrutura e da função cardíaca no ecocardiograma e desfechos durante a internação hospitalar e após a alta foram avaliados em ambos os grupos. Testes univariados e multivariados foram realizados, e um p-valor < 0,05 foi considerado estatisticamente significativo. Resultados Pacientes com CCC apresentaram pressão arterial sistólica (108,3 ± 26,1 vs. 128,3 ± 30,3 mmHg, p<0,001) e fração de ejeção ventricular esquerda [25,4 (19 – 36)% vs. 37 (27 – 54) %, p<0,001) mais baixas, com taxas mais altas de distensão da veia jugular (54,8% vs. 38,9%, p<0,001), hepatomegalia (47,9% vs. 25,6%, p<0,001), e de perfil “frio e úmido” (27,2% vs. 10,6%, p<0,001). Pacientes com CCC apresentaram taxas mais altas do desfecho composto de morte ou transplante cardíaco (17,4% vs. 11,1%, p=0,004), e incidência cumulativa mais alta de morte após três meses (16,5% vs. 10,8%, p=0,017), seis meses (25,3% vs. 17,2%, p=0,006), e 12 meses (39,4% vs. 26,6%, p<0,001). Além disso, CCC associou-se independentemente com risco de mortalidade em 12 meses, com um odds ratio de 2,02 (IC95%: 1,47-2,77). Conclusão Em comparação a outras etiologias, os pacientes com CCC internados por insuficiência cardíaca aguda apresentaram um perfil de maior risco que foi associado com um pior desfecho durante a internação hospitalar e após a alta.

BACKGROUND Human enterovirus C99 (HEV-C99) is a member of the species Enterovirus C. Currently, three complete genomes of HEV-C99 were reported in Brazil, all obtained from children with gastroenteritis symptoms. Notwithstanding, no HEV-C99 complete genome associated with AFP cases in Brazil have been analysed so far. OBJECTIVES In light of this, molecular characterisation of an HEV-C99 isolated from a case of acute flaccid paralysis (AFP) in Brazil was carried out. METHODS In 2005, an HEV-C99 strain was isolated from a 2-year-old female child in Santa Catarina State, Brazil, showing classic symptoms of AFP. Stool sample was inoculated into specific cell cultures. Viral RNA was extracted, and polymerase chain reaction (PCR) were performed to amplify the VP1 gene; the sequence was analysed for molecular identification. Subsequently, the complete genome was sequenced and analysed, including a phylogenetic analysis of the VP1 gene. FINDINGS The isolate, denominated HEV-C99/33322/BRA/2005 presented 85.85% identity to other HEV-C99 strains also described in Brazil, subsequently. Besides, the isolate grouped together with HEV-C99 cluster C strains. To our knowledge, this was the first described HEV-C99 isolated from an AFP case in Brazil. MAIN CONCLUSIONS The data generated in this study bolster the role of HEV-C99 as an etiologic agent of AFP. Furthermore, this research enhances our knowledge regarding the HEV-C99 genetic diversity.

Abstract The “Fantastic Four,” a term coined in 2021 to refer to the four key drug pillars in the treatment of heart failure with reduced ejection fraction (beta-blockers, renin-angiotensin system and neprilysin inhibitors, mineralocorticoid receptor antagonists, and sodium-glucose cotransporter-2 inhibitors, or SGLT2 inhibitors), has demonstrated excellent performance in reducing morbidity and mortality in this setting. However, in heart failure with mildly reduced or preserved ejection fraction, the same benefits were not observed with this combined treatment where, for many years, management in this context was limited to diuretics and comorbidity control. Recently, however, new therapeutic options have emerged, demonstrating effectiveness in reducing cardiovascular outcomes in this specific group: the “Dynamic Duo”—comprising SGLT2 inhibitors and Finerenone—has shown promising results, alongside the introduction of semaglutide as a potential “wild card” treatment for patients with obesity. Despite the ongoing need for therapies that significantly reduce overall mortality, these new treatments have effectively lowered hospitalization rates and improved symptoms in such patients. As a result, a new era in heart failure management is beginning.
Resumo O “Quarteto Fantástico”, termo criado em 2021 para se referir aos quatro pilares medicamentosos no tratamento da insuficiência cardíaca com fração de ejeção reduzida (betabloqueadores, inibidores do sistema renina-angiotensina e neprilisina, antagonistas do receptor de mineralocorticoide e inibidores do cotransportador de sódio e glicose II, ou iSGLT2), apresenta excelente desempenho na redução de morbimortalidade nesse cenário. No entanto, no caso da insuficiência cardíaca com fração de ejeção levemente reduzida ou preservada, os mesmos benefícios não foram observados com esse tratamento em conjunto, restando, por muitos anos, apenas o uso de diuréticos e o controle de comorbidades como manejo recomendado nesse contexto. Contudo, recentemente, novas opções terapêuticas demonstraram eficácia na redução dos desfechos cardiovasculares nesse grupo específico da insuficiência cardíaca com fração de ejeção levemente reduzida ou preservada: a “Dupla Dinâmica” composta pelos iSGLT2 e Finerenona, além despontamento da semaglutida como tratamento “coringa” para essa condição associada à obesidade. Embora ainda seja necessária a busca por novas opções terapêuticas que reduzam, de fato, a mortalidade geral nesse contexto, esses novos tratamentos impactaram efetivamente a diminuição da hospitalização e dos sintomas desses pacientes. Por isso, inicia-se uma nova era no manejo da insuficiência cardíaca.

Abstract Swartzia langsdorffii Raddi is an endemic fruit species of Brazil, whose fruits, commonly known as “banha de galinha,” play a significant role in the diet of native fauna. In this study, different fractions of S. langsdorffii fruit (pulp, seed, and peel) were evaluated for length, width, weight, density, color, proximate composition, minerals, pH, titratable acidity, soluble solids, total carotenoids, soluble and total pectin, total phenolics, antioxidant activity, and phenolic profile, aiming at potential applications in human nutrition and health promotion. Comparisons of the fruit's fractions revealed that the pulp is notable for higher levels of lipids, soluble solids, pH, K, S, and Fe; the seed for higher contents of protein, ash, total dietary fiber, nitrogen-free extract, Mn, total and soluble pectin; and the peel for higher levels of moisture, total phenolic content (TPC), antioxidant activity, vitamin C, carotenoids, and phenolic profile. Six phenolic compounds were identified in the fruit (gallic acid, chlorogenic acid, ferulic acid, o-coumaric acid, trans-cinnamic acid, and catechin), with trans-cinnamic acid being the only one found in all three fractions. The peel exhibited the highest concentrations of phenolic compounds, with chlorogenic acid being the predominant one. These results highlight the nutritional and functional potential of S. langsdorffii fruits and their possible health-promoting effects.

Abstract Objective This study analyzed the synergistic effect between a recombinant Maquiberry (MaquiCPI-3) protein, sodium fluoride (NaF), and stannous chloride (SnCl2) against initial dental erosion in vitro. Methodology A total of 98 bovine enamel samples were prepared and allocated to seven treatment groups (n=14/group) as follows: deionized water (Water); commercial solution, 800 ppm Sn+2, 500 ppm F-, ElmexTM, Erosion, GABA, Therwil, BL, CH (Elmex); 500 ppm of fluoride (F-) from NaF (NaF); 500 ppm of F- from NaF and 800 ppm of tin (Sn+2) from SnCl2 (NaF+SnCl); 0.5 mg/mL MaquiCPI-3 (MaquiCPI-3); combination of MaquiCPI-3 and NaF (Maqui+NaF) and; combination of MaquiCPI-3, NaF and SnCl (Maqui+NaF+SnCl). Samples were treated with the respective solutions (250 μl, 2 min, 37°C, 250 rpm). After forming the acquired enamel pellicle (AEP) by adding human saliva from nine volunteers (250 μl, 1 h, 37°C, 250 rpm), the samples underwent acid challenge (1 mL, 1% citric acid, pH 3.6, 1 min, 25°C, 250 rpm). All procedures were performed in triplicate. Surface microhardness change percentage (%SMC) and relative surface reflection intensity (%SRI) were measured and analyzed by One-way ANOVA/Tukey’s tests (p<0.05). Results The Elmex, NaF, NaF+SnCl, MaquiCPI-3, Maqui+NaF, and Maqui+NaF+SnCl groups showed significantly lower %SMC compared with Water. The NaF, Sn+2, NaF+SnCl, MaquiCPI-3, Maqui+NaF and Maqui+NaF+SnCl groups exhibited significantly greater protection compared with Elmex. Maqui+NaF+SnCl revealed better enamel protection (significant) when compared with the MaquiCPI-3 and Maqui+NaF groups. Elmex, NaF+SnCl, MaquiCPI-3, Maqui+NaF, and Maqui+NaF+SnCl had a significantly higher %SRI compared to the Water and NaF groups, which did not differ between each other. Conclusion All treatment solutions provided protection against initial dental erosion in vitro. Formulations containing Maqui+NaF+SnCl2 offered superior enamel protection compared with MaquiCPI-3 alone.

Yeast cell wall in the diet of Japanese quails in the laying phase at different stocking densities

Abstract The objective of this study was to measure the zootechnical performance and egg quality of Japanese quails housed at different densities and fed diets containing yeast cell walls (YCWs). Five hundred and seventy-six quail (Coturnix coturnix japonica) were distributed at 43 weeks of age, and 76% were laid, with an initial weight of 158.50 ± 5.41 g, in a completely randomized design in a 3 × 2 factorial arrangement (three YCW levels: 0, 500, and 750 g.ton−1 and two housing densities: 81.5 and 92.4 cm2/quail), with six replicates of 17 and 15 quail per experimental unit, respectively. The following parameters were evaluated: feed intake, egg production/bird/day, egg production/housed quail, marketable egg production, egg mass, feed conversion per dozen eggs, egg mass and viability, egg weight, specific egg weight, percentage of yolk, albumen and shell, and shell thickness. The means of the three cycles of 21 days were subjected to analysis of variance using the statistical software Sisvar. There was no significant interaction effect between YCW inclusion level and cage density on zootechnical performance parameters or egg quality, except for egg weight, which suggested that YCW addition, regardless of cage density, did not affect the results. It was observed that the eggs of quails housed in cages with 92.4 cm2/bird feed and 500 g.ton1 YCW had greater egg weights. Shell thickness was independently influenced by cage density, and the lowest density (92.4 cm2/bird) promoted greater shell thickness. The inclusion of 500 g.ton1 of yeast cell wall material in the diet of Japanese quails housed at a density of 92.4 cm2/bird improved egg weight and shell thickness without negatively affecting the other parameters of egg quality or zootechnical performance.
Resumo Objetivou-se mensurar o desempenho zootécnico e a qualidade de ovos de codornas japonesas alojadas sob diferentes densidades e alimentadas com rações contendo parede celular de levedura (PCL). Foram utilizadas 576 codornas japonesas (Coturnix japonica) com 43 semanas de idade e 76% de postura, com peso inicial de 158,50 ± 5,41 g distribuídas em delineamento inteiramente ao acaso em esquema fatorial 3 × 2 (três níveis de PCL: 0; 500 e 750 g.ton−1 e duas densidades de alojamento: 81,5 e 92,4 cm2/ave), com seis repetições de 17 e 15 codornas por unidade experimental, respectivamente. Foram avaliados: consumo de ração, produção de ovos/ave/dia, produção de ovos/ave alojada, produção de ovos comercializáveis, massa de ovos, conversão alimentar por dúzia e por massa de ovos e viabilidade das aves; peso do ovo, peso específico, porcentagem de gema, de albúmen e de casca e espessura da casca. Não houve interação entre os níveis de inclusão de PCL e densidade de alojamento para os parâmetros avaliados, exceto para peso do ovo. Codornas alojadas em gaiolas com 92,4 cm2/ave alimentadas com 500 g.ton−1 de PCL apresentaram maior peso do ovo. A espessura de casca foi influenciada de forma independente pela densidade de alojamento, a menor densidade (92,4 cm|2/ave) promoveu maior espessura de casca. A inclusão de 500 g.ton−1 de PCL na ração de codornas japonesas alojadas sob densidade de 92,4 cm2/ave melhora o peso dos ovos e a espessura da casca.

ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. The importance of the CTFB is evidenced by the elevated number of citations in the scientific literature in diverse areas of biology, law, anthropology, education, forensic science, and veterinary science, among others.

ABSTRACT This study was conducted to evaluate the relationship among digestibility, residual feed intake (RFI), and enteric methane emission in growing Nellore cattle (Bos indicus) divergently classified based on RFI phenotype and breeding value (EBV-RFI). Animals (n = 122) subjected to performance testing (forage:concentrate ratio of 60:40) in two test groups were classified based on RFI. A sample of 80 animals classified as low (−0.748±0.076 kg DM/day) or high (0.775±0.075 kg DM/day) RFI was evaluated regarding feed compounds digestibility, fecal excretion, and methane emission (CH4, g/day). Statistical mixed models included fixed effects of RFI (or EBV-RFI), linear effect of initial age as covariate, and the random effects of the test group. There was no difference in the digestibility of feed compounds between the most and least efficient animals. However, dry matter intake (DMI; 6.92 vs. 8.66 kg DM/day) and feed conversion (7.93 vs. 9.42 kg/kg) were lesser in low RFI animals. On average, low RFI animals emitted 14.3 g less CH4 per day (174 vs. 188 g CH4/day); however, CH4 emission expressed as g/kg DMI (23.1 vs. 20.1) and the percentage of gross energy intake lost as CH4 (8.13 vs. 7.08%) were greater for these animals. These results showed the benefits of using more feed efficient animals in the beef production chain, i.e., animals that exhibit lower feed intake, lower fecal excretion, and lower enteric methane emission without differences in weight gain or body weight. However, the variations in feed efficiency among them cannot be explained by differences in dry matter or feed compounds digestibility. More efficient animals emit less enteric methane than less efficient animals, probably as a result of lower DMI.
