Abstract This study aimed to determine whether the addition of green propolis extract to the diet of laying hens would improve egg quality and bird performance and decrease bacterial contamination of eggs. Forty-five brown Hy-line laying hens were used, divided into five groups with three replicates each and three animals per cage: T0 - diet without propolis; T5 - 5 grams of propolis per kg of feed; T10 - 10 grams of propolis per kg of feed; T20 - 20 grams of propolis per kg of feed and T30 - 30 grams of propolis per kg of feed. The quality of fresh eggs was made on day 21 of the experiment, and eggs were stored for 21 days. Greater specific gravity was observed in fresh eggs in T5 birds and stored eggs for T10. TBARS in fresh eggs, we found that T30 eggs had lower levels compared to other treatments. T20 eggs had the lowest total bacterial count and the lowest total coliform count in the eggshells of T10 and T20 chickens. The count of E. coli in the peel was lower for T20 and T30 than T0 and T5. The consumption of propolis did not interfere with the hens’ egg production rate but reduced food intake and consequently reduced feed conversion. We concluded that the addition of green propolis in the diet of laying birds proved efficient in reducing bacterial contamination in the eggshells and reducing the lipid peroxidation of fresh and stored eggs.

OBJECTIVES: To prospectively evaluate demographic, anthropometric and health-related quality of life (HRQoL) in pediatric patients with laboratory-confirmed coronavirus disease 2019 (COVID-19) METHODS: This was a longitudinal observational study of surviving pediatric post-COVID-19 patients (n=53) and pediatric subjects without laboratory-confirmed COVID-19 included as controls (n=52) was performed. RESULTS: The median duration between COVID-19 diagnosis (n=53) and follow-up was 4.4 months (0.8-10.7). Twenty-three of 53 (43%) patients reported at least one persistent symptom at the longitudinal follow-up visit and 12/53 (23%) had long COVID-19, with at least one symptom lasting for >12 weeks. The most frequently reported symptoms at the longitudinal follow-up visit were headache (19%), severe recurrent headache (9%), tiredness (9%), dyspnea (8%), and concentration difficulty (4%). At the longitudinal follow-up visit, the frequencies of anemia (11% versus 0%, p=0.030), lymphopenia (42% versus 18%, p=0.020), C-reactive protein level of >30 mg/L (35% versus 0%, p=0.0001), and D-dimer level of >1000 ng/mL (43% versus 6%, p=0.0004) significantly reduced compared with baseline values. Chest X-ray abnormalities (11% versus 2%, p=0.178) and cardiac alterations on echocardiogram (33% versus 22%, p=0.462) were similar at both visits. Comparison of characteristic data between patients with COVID-19 at the longitudinal follow-up visit and controls showed similar age (p=0.962), proportion of male sex (p=0.907), ethnicity (p=0.566), family minimum monthly wage (p=0.664), body mass index (p=0.601), and pediatric pre-existing chronic conditions (p=1.000). The Pediatric Quality of Live Inventory 4.0 scores, median physical score (69 [0-100] versus 81 [34-100], p=0.012), and school score (60 [15-100] versus 70 [15-95], p=0.028) were significantly lower in pediatric patients with COVID-19 at the longitudinal follow-up visit than in controls. CONCLUSIONS: Pediatric patients with COVID-19 showed a longitudinal impact on HRQoL parameters, particularly in physical/school domains, reinforcing the need for a prospective multidisciplinary approach for these patients. These data highlight the importance of closer monitoring of children and adolescents by the clinical team after COVID-19.

RESUMO O propósito deste estudo foi identificar a relação entre capacidade funcional do ombro e nível de atividade física em mulheres que trataram o câncer de mama. Participaram 84 mulheres assistidas pelo Sistema Único de Saúde em Aracaju/SE com no mínimo um ano após mastectomia e terapias adjuvantes. O nível de atividade física foi mensurado pelo International Physical Activity Questionnaire (IPAQ); capacidade funcional do ombro pelo questionário Shoulder Pain and Disability Index (SPADI); qualidade de vida pelo European Organisation for Research and Treatment of Câncer Quality of Life Questionnarie (EORTCQLQ-C30) e o EORTC QLQ- BR23. Para medir a amplitude de movimento foi utilizado um flexímetro e para avaliar força muscular um dinamômetro escapular. Pela regressão logística múltipla identificou-se que uma unidade acrescida do SPADI total aumenta em 7% (p = 0,0000005) a chance de ser pouco ativo e uma unidade acrescida do SPADI dor em 4% (p = 0,0002341). Os prejuízos da capacidade funcional do ombro, a presença de dor e a fraqueza muscular, podem colaborar para barreiras na realização de atividade física, tornando estas mulheres inativas. Tais achados são úteis para desenvolver estratégias para estimular esse perfil de mulheres, permitindo melhorar os níveis de atividade física e qualidade de vida.
ABSTRACT This study aimed to identify the relationship between shoulder functional capacity and physical activity in women treated for breast cancer. The participants were 84 women assisted by the Sistema Único de Saúde in Aracaju/SE with at least one year after mastectomy and adjuvant therapies. The level of physical activity was measured by the International Physical Activity Questionnaire (IPAQ); the functional capacity of the shoulder by the Shoulder Pain and Disability Index (SPADI) questionnaire; quality of life by the European Organisation for Research and Treatment of Cancer Quality of Life Questionnaire (EORTCQLQ-C30) and the EORTC QLQ-BR23. A scapular dynamometer was used to measure the range of motion and to evaluate muscle strength. By multiple logistic regression, it was identified that a unit plus the total SPADI increases by 7% (p = 0,0000005) the chance of being little active and an increased unit of SPADI pain by 4% (p = 0,0002341). The impairment of the functional capacity of the shoulder, the presence of pain, and muscle weakness can contribute to barriers in the performance of physical activity, making these women inactive. Such findings are useful to develop strategies to stimulate this profile of women, allowing to improve the levels of physical activity and quality of life.

RESUMO: Bovinos orquiectomizados apresentam vantagens na qualidade da carne e facilidade no manejo. A quimioesterilização é uma opção à castração cirúrgica e os agentes esclerosantes podem ser administrados no parênquima testicular ou epidídimo. Estes produtos possuem menor incidência de complicações, comparados a cirurgia, especialmente quando associados ao dimetilsulfóxido (DMSO), que apresenta ação anti-inflamatória e aumenta a absorção de outros fármacos. Assim, este estudo teve como objetivo avaliar o efeito de uma única injeção intratesticular de solução de cloreto de cálcio associado com 0,5% de DMSO para a esterilização química de bovinos. Vinte e quatro touros adultos jovens foram utilizados, distribuídos em 3 grupos (G20, G30 e G40, n = 8/grupo) de acordo com a concentração de cloreto de cálcio (20, 30 e 40%), em um volume de 10mL. Foram avaliadas as concentrações séricas de testosterona, peso corporal, volume e ecotextura testicular, sinais clínicos e comportamento por 45 dias. A seguir, os animais foram submetidos à orquiectomia e os testículos avaliados histologicamente. Não foram observadas alterações do peso corporal, diminuição das concentrações de testosterona sérica (exceto no G30), sinais de sensibilidade escrotal ou alterações no comportamento no período avaliado. Porém, houve aumento significativo do volume testicular, especialmente nos 2º e 3º dia após o tratamento, com valores retornando aos iniciais aos 15 dias. Aderência e consistência firme dos testículos foram achados observados durante a orquiectomia. O exame ultrassonográfico revelou perda de integridade da rafe mediana, com formação de cavidades e alteração da ecotextura testicular. Na avaliação histológica, verificou-se necrose de coagulação de túbulos seminíferos e células intersticiais acentuada, principalmente, na porção medial em todos os grupos, sendo que em alguns animais havia ausência total das formações tubulares em todos os grupos estudados, sendo os efeitos de maior intensidade no G40. Além disso, edema foi acentuado em todos os grupos, principalmente em G40. Infiltrado inflamatório, fibroplasia e neovascularização foram achados predominantemente discretos. Com base nas condições utilizadas neste estudo, conclui-se que o cloreto de cálcio associado com o DMSO pode ser utilizado como um método de esterilização química em bovinos.
ABSTRACT: Orchiectomized bulls have advantages in the meat quality and ease of handling. Chemical castration is an option for surgical castration and the sclerosing agents can be administered into the testicular or epididymis parenchyma. These agents have a lower incidence of complications than surgery, especially when associated with dimethylsulfoxide (DMSO), which has anti-inflammatory action and increases the absorption of other drugs. Thus, this study aimed to evaluate the effect of a single intratesticular injection of calcium chloride solution associated with DMSO for the chemical sterilization of bulls. Twenty-four young adult bulls were utilized, distributed into 3 groups (G20, G30 and G40, n = 8/group), according to the calcium chloride concentration (20, 30 and 40%), in 10mL volume. Serum concentrations of testosterone, body weight, testicular volume and ecotexture, clinical signs and behavior and were evaluated for 45 days. Thus, the animals were orchiectomized and testicles were assessed histologically. There were no changes in body weight, decreased serum testosterone concentrations (except G30), signs of scrotal sensitivity or changes in behavior over the period. However, there was significant increase in testicular volume, especially on the 2nd and 3rd day after treatment, with values returning to the value initials at 15 days. Testicular adherence and firm consistency were observed during orchiectomy. Ultrasound examination revealed a loss of integrity of the median raphe, with cavity formation and an alteration of the testicular echotexture. In the histological evaluation, coagulation necrosis of seminiferous tubules and interstitial cells was observed, mainly in the medial portion in all groups. Some animals presented total absence of tubular formations in all the studied groups, being the effects of greater intensity in the G40. Additionally, pronounced edema was noted in all groups, especially in G40. Inflammatory infiltrate, fibroplasia and neovascularization were found to be predominantly discrete. Based on the conditions used in this study, we conclude that calcium chloride associated with DMSO can be used as a method of chemical sterilization in bovines.

Probiotics on performance, intestinal morphology and carcass characteristics of broiler chickens raised with lower or higher environmental challenge

ABSTRACT: This study aimed to evaluate the effect of probiotics on performance, intestinal morphology and carcass characteristics of broiler chickens housed on lower or higher environmental challenge. Three hundred male Cobb chicks were distributed into four groups in completely randomised design with treatments arranged in a 2 x 2 factorial scheme to evaluate effects of two diets (with or without probiotics) and two environmental conditions (lower or higher challenge), totaling four treatments with five replications with 15 birds per box. Probiotics were added on diets and were composed of Lactobacillus acidophilus, Bacillus subtilis, Bifidobacterium bifidum and Enterococcus faecium. The environment with lower challenge was made up of new wood shavings used as litter, low bird density (8 birds/m2) and daily-cleaned bell drinkers. The environment with higher challenge was made up of re-used wood shavings used as litter (after three broods of broilers), bird density of 16 birds/m2 with bell drinkers cleaned every two days. No significant interaction between diet and environmental challenge was found for any of the variables evaluated. Performance, slaughterhouse variables and chemical carcass composition were not affected by the use of probiotics. Chicks receiving diets without probiotics had lower intestinal crypt depth (267.1 vs. 316.6 pm, P=0.0068). Birds raised in the environment with higher challenge decreased feed intake (4,660 vs 5,020 g, P=0.0422), weight gain (2,610 vs 2,810 g, P= 0.0054), drumstick and thigh yield (21.98 vs 24.14 %, P=0.0354), and increased crypt depth (325.2 vs 258.5 pm P=0.0009). In conclusion, the probiotic does not promote satisfactory improvements, regardless of the environmental challenge.

Arsenic is considered a semimetal, and its wide distribution in the Earth's crust in different chemical forms, including organic and inorganic species, has a great deal of influence on the mechanisms of toxicity. Exposure to arsenic can be either through occupational practice (use of pesticides) or by the consumption of water and food containing the element. Rice is considered a fundamental constituent of the basic diet of Brazilians and is usually cultivated in flooded conditions. Such a plantation system results in an increased amount of As in the soil and hence a greater accumulation of As in the plant, which is highlighted by the inorganic species' classification as highly toxic. Besides the use of mitigation techniques to reduce the toxicological risk, monitoring the concentration of As and its chemical species in rice and rice products is required through the establishment of legislation in the area. Thus, some world organizations are conducting improved research to determine and establish acceptable concentrations of As and its chemical species in rice, e. g., in 2012, FDA researchers described a chemical speciation methodology for As in rice and rice products. Hence, the application of existing chemical speciation methods and the establishment of parameters for ensuring food security and exposure risk assessment deserve particular consideration.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Chlamydia trachomatis: um importante agente de infecções respiratórias em lactentes de famílias de baixa renda

OBJETIVOS: Determinar a prevalência de infecção do trato respiratório inferior (ITRI) por Chlamydia trachomatis em lactentes internados e descrever as características clínicas, laboratoriais e radiológicas da doença. MÉTODOS: Este foi um estudo do tipo corte transversal, realizado durante um período de 12 meses. Foram incluídos todos os lactentes de até 6 meses internados consecutivamente no Centro Pediátrico Professor Hosannah de Oliveira da Universidade Federal da Bahia, em Salvador, BA, com diagnóstico clínico ou clínico-radiológico de ITRI. O diagnóstico de infecção por C. trachomatis foi realizado através da pesquisa de anticorpos da classe IgM, utilizando-se o ensaio imunoenzimático (ELISA). A prevalência de ITRI por C. trachomatis foi determinada, e foram calculadas as razões de prevalência para essa infecção e variáveis clínicas e laboratoriais. RESULTADOS: Cento e cinquenta e um lactentes realizaram sorologia para C. trachomatis, das quais 15 (9,9%) foram positivas. A infecção por C. trachomatis ocorreu unicamente entre os menores de 5 meses, principalmente naqueles menores de 2 meses. Três crianças com infecção por C. trachomatis nasceram de parto cesáreo. Conjuntivite e eosinofilia ocorreram em 33,3% dos casos. As radiografias de tórax se mostraram alteradas em 92% dos casos. Demonstrou-se associação da infecção por C. trachomatis com duração de internação superior a 15 dias (p = 0,0398) e com oxigenoterapia (p = 0,0484). CONCLUSÕES: Houve alta prevalência de ITRI por C. trachomatis na população estudada. A infecção por esta bactéria foi associada a uma forma mais grave da doença, demonstrando a importância de se investigar essa infecção na gestante de forma a evitar o adoecimento de recém-nascidos.
OBJECTIVES: To determine the prevalence of lower respiratory tract infection (LRTI) due to Chlamydia trachomatis in newborn infants and to describe the clinical, laboratory, and radiological characteristics of the disease. METHODS: A cross-sectional study carried out over a 12-month period. All infants up to 6 months of age admitted consecutively at the Centro Pediátrico Professor Hosannah de Oliveira of the Universidade Federal da Bahia in Salvador, Brazil, and diagnosed with LRTI according to clinical and/or radiological criteria were included in the study. C. trachomatis infection was diagnosed by the enzyme-linked immunosorbent assay (ELISA) for the detection of IgM-class antibodies. The prevalence of LRTI by C. trachomatis was determined and the prevalence ratios for the infection and clinical or laboratory variables were calculated. RESULTS: One hundred and fifty-one infants were submitted to serology for C. trachomatis and 15 (9.9%) tested positive. Chlamydial infection was found only in infants under 5 months of age, mainly in those aged under 2 months. Three of the infants with C. trachomatis infection were born by cesarean section. Conjunctivitis and eosinophilia had occurred in 33.3% of the cases. Chest X rays were abnormal in 92.0% of cases. There was an association between C. trachomatis infection and the duration of hospitalization exceeding 15 days (p = 0.0398) and oxygen therapy (p = 0.0484). CONCLUSIONS: There was a high prevalence of C. trachomatis respiratory infection in the population studied. The infection was associated with a more severe form of the disease, emphasizing the importance of testing pregnant women for this infection to avoid infection in the newborn infant.

In Vitro effect of low-level laser therapy on typical oral microbial biofilms

O objetivo deste estudo foi avaliar o efeito de parâmetros específicos de irradiação com laser de baixa intensidade sobre biofilmes formados por Streptococcus mutans (S. mutans), Candida albicans (C. albicans) ou associação de ambas as espécies. Biofilmes isolados ou associados destes microrganismos foram irradiados com um dispositivo laser infra-vermelho próximo de diodos InGaAsP (LaserTABLE 780 ±3 nm, 0,04W), utilizando-se para isto o dispositivo LASERTable. Quinze horas após a irradiação, foi demonstrado, por meio da avaliação da viabilidade celular (Teste de MTT), da morfologia das células (MEV) e do crescimento do biofilme (UFC/mL), que esta terapia foi capaz de reduzir o metabolismo celular, número de microrganismos presentes no biofilme, bem como seu crescimento no local. Quanto à viabilidade celular, a resposta à irradiação do biofilme de S. mutans (SSB) foi semelhante para todas as doses de energia, sendo que o crescimento do biofilme foi dose dependente. Porém, quando associado à C. albicans, este microrganismo apresentou resistência à fototerapia. Já a C. albicans associada ao S. mutans apresentou redução de crescimento significativa, sendo este resultado também foi dose dependente. A morfologia dos microrganismos não foi alterada pelas irradiações realizadas quando em biofilmes isolados. A associação entre os microrganismos promoveu redução na formação de hifas pela C. albicans. A laserterapia de baixa intensidade apresentou efeito inibitório sobre microrganismos, sendo que esta capacidade pode ser alterada de acordo com a interação entre diferentes microrganismos.
The aim of this study was to evaluate the effect of specific parameters of low-level laser therapy (LLLT) on biofilms formed by Streptococcus mutans, Candida albicans or an association of both species. Single and dual-species biofilms - SSB and DSB - were exposed to laser doses of 5, 10 or 20 J/cm2 from a near infrared InGaAsP diode laser prototype (LASERTable; 780 ± 3 nm, 0.04 W). After irradiation, the analysis of biobilm viability (MTT assay), biofilm growth (cfu/mL) and cell morphology (SEM) showed that LLLT reduced cell viability as well as the growth of biofilms. The response of S. mutans (SSB) to irradiation was similar for all laser doses and the biofilm growth was dose dependent. However, when associated with C. albicans (DSB), S. mutans was resistant to LLLT. For C. albicans, the association with S. mutans (DSB) caused a significant decrease in biofilm growth in a dose-dependent fashion. The morphology of the microorganisms in the SSB was not altered by LLLT, while the association of microbial species (DSB) promoted a reduction in the formation of C. albicans hyphae. LLLT had an inhibitory effect on the microorganisms, and this capacity can be altered according to the interactions between different microbial species.

A ultrassonografia do segmento posterior do bulbo do olho é etapa indispensável na avaliação de cães com catarata que serão submetidos à facectomia, uma vez que a oftalmoscopia não é factível quando há opacificação da lente, notadamente nas cataratas maduras. Este estudo avaliou as condições ecográficas da lente cataratogênica e do fundo de olho de cães cursando com catarata. Foram avaliados 30 cães (56 olhos), sendo 10 machos e 20 fêmeas, cursando com catarata em diferentes estágios de desenvolvimento. Realizou-se a ecografia em modo A e B simultaneamente. Os exames revelaram catarata cortical anterior, posterior e nuclear em 12 olhos (21,4%), catarata cortical anterior, posterior, nuclear e capsular posterior em 23 olhos (41%), catarata cortical anterior, posterior e capsular posterior em um olho (1,7%), catarata cortical anterior e nuclear em um olho (1,7%), catarata cortical anterior, nuclear e capsular posterior em cinco olhos (8,9%) e catarata cortical anterior em sete olhos (12,5%). Alterações no segmento posterior foram observadas, à ultrassonografia, em 26 olhos avaliados (46,4%). Degeneração vítrea foi detectada em 12 olhos (21,4%), imagens compatíveis com exsudato ou hemorragia em sete olhos (12,5%), persistência da artéria hialóide em quatro olhos (7,1%) e subluxação da lente em três olhos (5,3%). Os resultados obtidos reiteram a importância da ultrassonografia em cães candidatos à cirurgia de catarata, uma vez que o exame do segmento posterior do bulbo do olho é de difícil execução, devido à opacificação da lente.
Ultrasonography of the lens and posterior segment is an indispensable step in the preoperative evaluation of dogs with cataracts, since ophthalmoscopy is not feasible when there is opacification of the lens. This study evaluated the echographic conditions of cataractous lens and fundus of the eye in dogs affected by cataracts. The study was conducted in 30 dogs (56 eyes), 10 males and 20 females, with different types of cataracts at different stages of development. Echography in A and B modes, simultaneously, was carried out for the examination of the lens and posterior segment. The examinations revealed anterior cortical, posterior cortical and nuclear cataract in 12 eyes (21.4%), anterior cortical, posterior cortical, nuclear and posterior capsular in 23 eyes (41%), anterior cortical, posterior cortical and posterior capsular cataract in one eye (1.7%), anterior cortical and nuclear cataract in one eye (1.7%), anterior cortical, nuclear and posterior capsular cataract in five eyes (8.9%), and anterior cortical cataract in seven eyes (12.5%). Abnormal ultrasonographic alterations were observed in the posterior segment in 26 eyes evaluated (46.4%). Vitreal degeneration was detected in 12 eyes (21.4%), images of vitreal exudate or hemorrhage in seven eyes (12.5%), persistence of hyaloid artery in four eyes (7.1%) and lens subluxation in three eyes (5.3%). The results obtained reiterate the importance of ultrasonography in canine patients presented for cataract surgery given that alterations of the posterior segment are difficult to identify in a clinical examination when the lens is opacified.

A commercial corrosion inhibitor used in petroleum production was characterized by means of infrared spectroscopy and energy dispersive spectroscopy (EDS). Predicting the adsorption behavior of corrosion inhibitor onto steel, sandstone and esmectite is the key to improve working conditions. In this study, the adsorption kinetics of inhibitor formulations in HCl 15% or in Mud Acid (HCl 13,5% and ammonium bifluoride) onto steel, sandstone and esmectite was determined by means of spectrophotometry. Kinetic parameters indicated that adsorption of inhibitor in the presence of bifluoride was favored. Moreover, the adsorption constant rate was the largest when the substrate was esmectite.

A acromegalia é uma síndrome causada pela hipersecreção do hormônio de crescimento (GH). No entanto, alguns pacientes podem apresentar níveis séricos limítrofes, ou mesmo dentro da normalidade, dificultando o diagnóstico. Relatamos um caso de acromegalia cuja investigação diagnóstica inicial evidenciou níveis séricos basais de GH dentro dos limites da normalidade e tomografia computadorizada (TC) de sela túrcica normal. A confirmação da doença só foi estabelecida através de testes dinâmicos que avaliam a regulação do GH (dosagem de GH durante o teste oral de tolerância à glicose e após TRH) e confirmada pela ressonância nuclear magnética (RNM) de sela túrcica que evidenciou imagem sugestiva de microadenoma. A paciente foi submetida à cirurgia hipofisária pela via de acesso oronaso esfenoidal e a área tumoral encaminhada para estudo imunohistoquímico, corroborando o diagnóstico de acromegalia. A avaliação dinâmica no pós operatório evidenciou regulação normal do GH. Discutimos os critérios diagnósticos da acromegalia, com ênfase na importância de se prosseguir com a investigação sempre que houver uma forte suspeita clínica, mesmo que os exames iniciais se apresentem dentro dos limites da normalidade.
Acromegaly is a syndrome caused by growth hormone (GH) hyper-secretion. However, some patients can present normal or near normal levels of GH, difficulting diagnosis. We report an acromegalic patient with normal levels of GH and negative imaging at initial investigation. Acromegaly was confirmed by dynamic tests (glucose tolerance test and GH-TRH), and RMI was suggestive of a microadenoma. The patient was submitted to a transesphenoidal pituitary surgery and imunohistochemistry study, corroborating the diagnosis of acromegaly. Dynamic tests performed after surgery evidenced normal GH regulation. Considerations on the diagnosis of acromegaly were also done, emphasizing the importance of continuing the investigation whenever clinical suspicion persists, even though the initial tests are within the normal range.

An exanthematic disease epidemic associated with coxsackievirus B3 infection in a day care center

É descrita uma epidemia de doença exantemática em uma creche, acometendo 10 crianças de 7 a 13 meses de idade. O quadro exantemático caracterizou-se por lesões máculo ou maculopapulares não confluentes, que atingiam a face, tronco e pernas. 50% das crianças infectadas apresentaram febre de até 39ºC, no início da doença. Foi isolado das fezes de uma criança doente o coxsackievírus B3 (CB3). Foram examinados soros pareados de 8 das 10 crianças doentes e em 6 delas demonstrou-se soroconversão para CB3. Duas crianças que apresentaram sintomatologia e que tiveram amostras de sangue colhidas somente 7 meses após a epidemia, apresentaram anticorpos específicos contra o CB3, em títulos superiores a 16. Entre os contatos das crianças doentes, verificou-se que 71,0% do total apresentaram anticorpos neutralizantes específicos contra o CB3.
An epidemic of exanthematic illness in a day care center is described. Ten children aged 7 to 13 months were affected by the illness. The exanthem was characterized by nonconfluent macular or maculopapular lesions that appeared on the face, body and limbs. Fifty percent of the infected children had fever of up to 39ºC at the beginning of the disease. Coxsackievirus B3 (CB3) was isolated from the stool of one ill child. Paired serum samples were obtained from eight ill children and six of them presented seroconversion to CB3. Antibodies to CB3 were detected at titers higher than 16 in a single serum sample collected from the other two patients. Neutralizing antibodies to CB3 were detected in 71.0% of the contact children.
