RESUMO Objetivo: Descrever as práticas de ressuscitação volêmica em unidades de terapia intensiva brasileiras e compará-las com as de outros países participantes do estudo Fluid-TRIPS. Métodos: Este foi um estudo observacional transversal, prospectivo e internacional, de uma amostra de conveniência de unidades de terapia intensiva de 27 países (inclusive o Brasil), com utilização da base de dados Fluid-TRIPS compilada em 2014. Descrevemos os padrões de ressuscitação volêmica utilizados no Brasil em comparação com os de outros países e identificamos os fatores associados com a escolha dos fluidos. Resultados: No dia do estudo, foram incluídos 3.214 pacientes do Brasil e 3.493 pacientes de outros países, dos quais, respectivamente, 16,1% e 26,8% (p < 0,001) receberam fluidos. A principal indicação para ressuscitação volêmica foi comprometimento da perfusão e/ou baixo débito cardíaco (Brasil 71,7% versus outros países 56,4%; p < 0,001). No Brasil, a percentagem de pacientes que receberam soluções cristaloides foi mais elevada (97,7% versus 76,8%; p < 0,001), e solução de cloreto de sódio a 0,9% foi o cristaloide mais comumente utilizado (62,5% versus 27,1%; p < 0,001). A análise multivariada sugeriu que os níveis de albumina se associaram com o uso tanto de cristaloides quanto de coloides, enquanto o tipo de prescritor dos fluidos se associou apenas com o uso de cristaloides. Conclusão: Nossos resultados sugerem que cristaloides são usados mais frequentemente do que coloides para ressuscitação no Brasil, e essa discrepância, em termos de frequências, é mais elevada do que em outros países. A solução de cloreto de sódio 0,9% foi o cristaloide mais frequentemente prescrito. Os níveis de albumina sérica e o tipo de prescritor de fluidos foram os fatores associados com a escolha de cristaloides ou coloides para a prescrição de fluidos.
Abstract Objective: To describe fluid resuscitation practices in Brazilian intensive care units and to compare them with those of other countries participating in the Fluid-TRIPS. Methods: This was a prospective, international, cross-sectional, observational study in a convenience sample of intensive care units in 27 countries (including Brazil) using the Fluid-TRIPS database compiled in 2014. We described the patterns of fluid resuscitation use in Brazil compared with those in other countries and identified the factors associated with fluid choice. Results: On the study day, 3,214 patients in Brazil and 3,493 patients in other countries were included, of whom 16.1% and 26.8% (p < 0.001) received fluids, respectively. The main indication for fluid resuscitation was impaired perfusion and/or low cardiac output (Brazil: 71.7% versus other countries: 56.4%, p < 0.001). In Brazil, the percentage of patients receiving crystalloid solutions was higher (97.7% versus 76.8%, p < 0.001), and 0.9% sodium chloride was the most commonly used crystalloid (62.5% versus 27.1%, p < 0.001). The multivariable analysis suggested that the albumin levels were associated with the use of both crystalloids and colloids, whereas the type of fluid prescriber was associated with crystalloid use only. Conclusion: Our results suggest that crystalloids are more frequently used than colloids for fluid resuscitation in Brazil, and this discrepancy in frequencies is higher than that in other countries. Sodium chloride (0.9%) was the crystalloid most commonly prescribed. Serum albumin levels and the type of fluid prescriber were the factors associated with the choice of crystalloids or colloids for fluid resuscitation.

Inhibition of Acetylcholinesterase by Coumarin-Linked Amino Acids Synthetized via Triazole Associated with Molecule Partition Coefficient

A previous study for the identification of acetylcholinesterase (AChE) inhibitors demonstrated that the hybrid between tyrosol, the 1,2,3-triazole nucleus, and the coumarin group, namely 7-({1-[2-(4-hydroxyphenyl)ethyl]-1H-1,2,3-triazol-4-yl}methoxy)-4-methyl-2H-chromen-2-one (10), has a high enzyme inhibitory activity. Here, we synthesized analogues of 10 via triazole with pharmacophoric groups represented by tyrosine, phenylalanine, tryptophan, and glycine in addition to evaluating the impact of coumarin-linked amino acids on AChE inhibition. We obtained eight triazoles, six of which are undescribed. In general, the presence of carboxylic acid decreased the inhibitory activity, while aromatic amino acids increased enzymatic inhibition compared to glycine. The derivative containing tyrosine, structurally most similar to 10, presented the lowest inhibition percentage, indicating that phenolic hydroxyl is not the preponderant factor for inhibition. Molecular docking was not enough to explain in vitro experiments. On the other hand, MlogP (logP calculated by the Moriguchi method) was related positively to enzymatic inhibition. To increase the hydrophobicity of the molecules, we tested the esterified triazole derivatives comparatively with the enzyme. The compound ethyl 2-(4-(((4-methyl-2-oxo-2H-chromen-7-yl)oxy)methyl)-1H-1,2,3-triazol-1-yl)acetate (6) presented an increment of inhibitory activity of 46.97 ± 1.75% at 100 μmol L-1. We also associated the best activity with the lowest van der Waals volume and molar mass values.

RESUMO O uso de cama sobreposta de suínos (CSS) na adubação de pastagens pode ser uma importante alternativa do ponto de vista ambiental e econômico. Com o objetivo de avaliar a adubação com CSS de forma exclusiva ou associada a fertilizantes minerais, na qualidade e produtividade de Panicum maximum cv. Mombaça, foi conduzido um experimento em condições de campo, em Ipameri, GO. O delineamento foi em blocos ao acaso em esquema fatorial 2 x 5 com quatro repetições, em parcelas de 24 m². Foram testadas as doses de CSS (0; 5; 10; 15 e 20 Mg ha-1), com ou sem adubação com NPK (50 kg ha-1 de P2O5, 10 kg ha-1 de K2O e 10 kg ha-1 de N). O adubo orgânico foi aplicado a lanço na implantação da pastagem, e os fertilizantes minerais parcelados no plantio e em cobertura. Foram avaliadas a massa seca, proteína bruta, fibra em detergente ácido e fibra em detergente neutro. As doses de CSS associadas à adubação mineral apresentaram incrementos lineares de 52 a 282% nas variáveis avaliadas, e melhor composição bromatológica. A aplicação de CSS é benéfica à produção de pastagens e as aplicações crescentes até à dose de 20 Mg ha-1 de CSS associadas à adubação mineral apresentaram melhores resultados quantitativos e qualitativos. Com a adubação exclusiva com CSS os ganhos de qualidade e produtividade do capim Mombaça são menores.
ABSTRACT The use of pig deep-litter (PDL) in pasture fertilization can be an important alternative from an environmental and economic point of view. This study was conducted to evaluate the fertilization with PDL exclusively or in association with mineral fertilizers on the quality and productivity of Panicum maximum cv. Mombasa. The experiment was carried out in Ipameri, GO, Brazil. The design was a randomized block design in a 2 x 5 factorial scheme with four repetitions, in plots of 24 m². The doses of PDL (0; 5; 10; 15 and 20 Mg ha-1) were tested, with or without fertilization with NPK (50 kg ha-1 of P2O5, 10 kg ha-1 of K2O and 10 kg ha-1 of N). The organic fertilizer was applied broadcast at the planting of the pasture, and the mineral fertilizers were split and applied at planting and as topdressing. Dry mass, crude protein, acid detergent fiber and neutral detergent fiber were evaluated. PDL doses associated with mineral fertilization led to linear increases from 52 to 282% in the analyzed variables and better bromatological composition. PDL application is beneficial to the production of pastures, and increasing applications up to the PDL dose of 20 Mg ha-1 associated with mineral fertilization promoted better quantitative and qualitative results. With exclusive fertilization with PDL, the gains in quality and productivity were more modest.

The benzophenones are synthetic and natural compounds presenting a variety of activities, including photoprotective, cytotoxic and antiviral. It is herein described the preparation of a series of twenty-seven benzophenone derivatives bearing 1,2,3-triazole functionalities and the evaluation of their photoprotective, cytotoxic and antiviral on Zika Virus (ZIKV) activities. The compounds were prepared in three steps, namely reduction of benzophenone, alkylation of diphenylmethanol and CuAAC reactions. The in vitro evaluation of the photoprotective activity revealed that the most active derivative 4-((benzhydryloxy)methyl)-1-(4-nitrobenzyl)-1H-1,2,3-triazole (4k) displayed UVB sun protection factor equal to 6,9±0,53, which make this compound a possible candidate to be used in formulations for photoprotective applications. In terms of cytotoxicity, the compounds were evaluated against MDA-MB-231 and B16F10 cell lines. It was observed that the compounds were more active against MDA-MB-231 cells and three of them were capable of reducing cell viability by approximately 55% at 100 µmol L-1. In the antiviral screening against ZIKV, compound 4-(3-benzhydryloxy)propyl)-1-(3-methylbenzyl)-1H-1,2,3-triazole (5j) was the most effective in mantaining Vero cell viability.

In this study, 20 different protocols for protein extraction of cowpea seeds in natura were evaluated taking into account four parameters: cell disruption method, time, temperature and solvent. Factorial design experiments were executed to determine the effects on the total protein content of extracts. The total protein content was determined and the results were in the range of 8.75 ± 1.3 and 86.6 ± 6.9 mg g-1. The richest protein extracts were obtained using Tris-HCl buffer at 50 ºC for 20 min for each assessed cell disruption method. However, the results using water were similar to buffer according to t-test at the 95% confidence level. Six protocols demonstrated higher extraction efficiency. Comparative proteomic study using size exclusion chromatography coupled to diode array detector (SEC-DAD) revealed that the one protocol extracted simultaneously high and low molecular mass biomolecules. In fact, the effect of microwave radiation promoted an increased solubility of biomolecules in water. The analysis of chromatographic profiles for principal component analysis (PCA) highlights the greater efficiency of extraction with the application of this protocol. Therefore, our study showed that the optimum condition for extraction of cowpea seed protein consists of water assisted by microwave radiation at 50 ºC for 20 min.

Resumo A febre chikungunya tem se tornado um importante problema de saúde pública nos países onde ocorrem as epidemias, visto que metade dos casos evolui com artrite crônica, persistente e incapacitante. Os dados na literatura sobre terapêuticas específicas nas diversas fases da artropatia ocasionada pela infecção pelo vírus chikungunya (CHIKV) são limitados, não existem estudos randomizados de qualidade que avaliem a eficácia das diferentes terapias. Há algumas poucas publicações sobre o tratamento das manifestações musculoesqueléticas da febre chikungunya, porém com importantes limitações metodológicas. Os dados atualmente disponíveis não permitem conclusões favoráveis ou contrárias a terapêuticas específicas, bem como uma adequada avaliação quanto à superioridade entre as diferentes medicações empregadas. O objetivo deste trabalho foi elaborar recomendações para o tratamento da febre chikungunya no Brasil. Foi feita uma revisão da literatura com seleção de artigos baseados em evidência, nas bases de dados Medline, SciELO, PubMed e Embase e de resumos de anais de congressos, além da opinião dos especialistas para dar apoio às decisões tomadas para definir as recomendações. Para a definição do grau de concordância foi feita uma metodologia Delphi, em duas reuniões presenciais e várias rodadas de votação on line. Este artigo refere-se à parte 2 das Recomendações da Sociedade Brasileira de Reumatologia para Diagnóstico e Tratamento da Febre Chikungunya, que trata especificamente do tratamento.
Abstract Chikungunya fever has become an important public health problem in countries where epidemics occur because half of the cases progress to chronic, persistent and debilitating arthritis. Literature data on specific therapies at the various phases of arthropathy caused by chikungunya virus (CHIKV) infection are limited, lacking quality randomized trials assessing the efficacies of different therapies. There are a few studies on the treatment of musculoskeletal manifestations of chikungunya fever, but these studies have important methodological limitations. The data currently available preclude conclusions favorable or contrary to specific therapies, or an adequate comparison between the different drugs used. The objective of this study was to develop recommendations for the treatment of chikungunya fever in Brazil. A literature review was performed via evidence-based selection of articles in the databases Medline, SciELO, PubMed and Embase and conference proceedings abstracts, in addition to expert opinions to support decision-making in defining recommendations. The Delphi method was used to define the degrees of agreement in 2 face-to-face meetings and several online voting rounds. This study is part 2 of the Recommendations of the Brazilian Society of Rheumatology (Sociedade Brasileira de Reumatologia - SBR) for the Diagnosis and Treatment of chikungunya fever and specifically addresses treatment.

Resumo A febre chikungunya tem se tornado um importante problema de saúde pública nos países onde ocorrem as epidemias. Até 2013, as Américas haviam registrado apenas casos importados quando, em outubro desse mesmo ano, foram notificados os primeiros casos na Ilha de Saint Martin, no Caribe. No Brasil, os primeiros relatos autóctones foram confirmados em setembro de 2014 e até a semana epidemiológica 37 de 2016 já haviam sido registrados 236.287 casos prováveis de infecção pelo chikungunya vírus (CHIKV), 116.523 confirmados sorologicamente. As mudanças ambientais causadas pelo homem, o crescimento urbano desordenado e o número cada vez maior de viagens internacionais têm sido apontados como os fatores responsáveis pela reemergência de epidemias em grande escala. Caracterizada clinicamente por febre e dor articular na fase aguda, em cerca de metade dos casos existe evolução para a fase crônica (além de três meses), com dor persistente e incapacitante. O objetivo deste trabalho foi elaborar recomendações para diagnóstico e tratamento da febre chikungunya no Brasil. Para isso, foi feita revisão da literatura nas bases de dados Medline, SciELO e PubMed, para dar apoio às decisões tomadas para definir as recomendações. Para a definição do grau de concordância foi feita uma metodologia Delphi, em duas reuniões presenciais e várias rodadas de votação on line. Foram geradas 25 recomendações, divididas em três grupos temáticos: (1) diagnóstico clínico, laboratorial e por imagem; (2) situações especiais e (3) tratamento. Na primeira parte estão os dois primeiros temas e o tratamento na segunda.
Abstract Chikungunya fever has become a relevant public health problem in countries where epidemics occur. Until 2013, only imported cases occurred in the Americas, but in October of that year, the first cases were reported in Saint Marin island in the Caribbean. The first autochthonous cases were confirmed in Brazil in September 2014; until epidemiological week 37 of 2016, 236,287 probable cases of infection with Chikungunya virus had been registered, 116,523 of which had serological confirmation. Environmental changes caused by humans, disorderly urban growth and an ever-increasing number of international travelers were described as the factors responsible for the emergence of large-scale epidemics. Clinically characterized by fever and joint pain in the acute stage, approximately half of patients progress to the chronic stage (beyond 3 months), which is accompanied by persistent and disabling pain. The aim of the present study was to formulate recommendations for the diagnosis and treatment of Chikungunya fever in Brazil. A literature review was performed in the MEDLINE, SciELO and PubMed databases to ground the decisions for recommendations. The degree of concordance among experts was established through the Delphi method, involving 2 in-person meetings and several online voting rounds. In total, 25 recommendations were formulated and divided into 3 thematic groups: (1) clinical, laboratory and imaging diagnosis; (2) special situations; and (3) treatment. The first 2 themes are presented in part 1, and treatment is presented in part 2.

Arterial spin labeling in patients with schizophrenia: a systematic review

Abstract Background: Neuroimaging studies are an invaluable source of information about the physiopathology of schizophrenia. Arterial spin labeling (ASL) is a new magnetic resonance technique (MRI) that is able to effectively evaluate brain function without the use of radiation. Objective: To make a systematic review of studies using ASL to compare resting-state regional cerebral blood flow (rCBF) patterns in patients with schizophrenia and healthy controls. Methods: Original articles were searched for on PubMed, Scopus, Web of Science and PsycINFO electronic databases. The search terms used were ‘arterial’, ‘spin’, ‘labeling’, and ‘schizophrenia’. Only studies comparing resting-state rCBF were included, a qualitative synthesis was then performed. Results: Ten articles were included in the review among a total of 22. Decreased rCBF in schizophrenia patients was described in the anterior cingulate, cuneus, fusiform gyrus, frontal lobe, left middle frontal gyrus, inferior frontal gyrus, lingual gyrus, middle occipital gyrus, and parietal lobe. The putamen was the only region with increased rCBF in schizophrenia. Discussion: The evidence of the studies reviewed lends support to the concept of hipofrontality in schizophrenia. rCBF alterations were found in regions classically associated with schizophrenia. ASL seems to be valid, and reliable tool to assess schizophrenia.

ABSTRACT One of the reasons for the success of a productive culture is the correct sowing. Therefore, the seeds must be properly dosed, deposited and not damaged by the metering mechanism of the seed drill. So, the aim of this study was to evaluate in a simulator the sorghum seed deposition by a metering mechanism with continuous flow in different conditions of slope and sowing speed, and evaluate the quality of the deposited seeds assessing mechanical damage and germination. The experiment was carried out at the College of Agricultural Sciences, UNESP in Botucatu-SP, being used a simulator equipped with seed metering mechanism of helical channelled rotor type. The experimental design was randomized in a 3 x 2 factorial arrangement with six replications. The factors were three lateral slopes drill, 3%, 8% and 16%, and two sowing speed, 4 and 10 km h-1. For the damage and germination variables we added a control treatment whose seeds were evaluated without being distributed by the metering mechanism. The results indicate that increasing the lateral slope and working speed reduce the rate of seed deposition. The metering mechanism provides mechanical damage and contributes to the reduction of seed germination.

Análise econômica de milho convencional e transgênico em dois sistemas de preparos de solo

RESUMO O milho é um produto de grande importância na agricultura, em função de sua ampla utilização e das formas de produção. Tendo em vista esses cenários, o trabalho teve como objetivo a análise econômica do sistema de produção de milho utilizando diferentes híbridos de milho e preparos de solo. O estudo foi realizado na Fazenda Experimental Lageado, pertencente à Faculdade de Ciências Agronômicas/UNESP de Botucatu-SP. Foram analisados dois híbridos em dois tipos de sistemas de preparo do solo, sendo milho convencional e milho transgênico, em preparo convencional e cultivo mínimo. Para os diferentes tipos de cultivo, foram utilizados os mesmos tratores, implementos e adubação, sendo os seguintes parâmetros analisados: custos operacionais, custos de insumos, renda bruta, renda líquida e relação benefício/custo dos sistemas de cultivo. Conclui-se que a utilização de milho transgênico proporciona redução dos custos operacionais e com insumos, possibilitando, em preparo de cultivo mínimo do solo, maior produtividade, renda líquida e relação benefício/custo.
ABSTRACT Corn is an agricultural crop of major importance regarding its wide use and production forms. Given these background, this work aimed at analyzing economically the corn production chain with different hybrids and soil tillage systems. The study was carried out at the Experimental Farm of Lageado, located in the College of Agricultural Sciences/ UNESP, campus in Botucatu-SP, Brazil. We analyzed two corn hybrids, one conventional and another transgenic, on soil prepared by conventional cultivation practices and minimum cultivation. The same tractors, implements, and fertilizers were employed for both tillage systems. We evaluated operating costs, input costs, gross income, net income, as well as benefit and cost ratio of both cropping systems. Therefore, we concluded that transgenic corn provides reduced operating costs and inputs, enabling higher yield, net income and benefit/cost relationship when cropped with minimum cultivation practices.

O plantio cruzado da soja consiste em semear a área em duas passadas de modo que a segunda fique perpendicular à primeira, formando um tabuleiro de xadrez. Desta forma, o objetivo do trabalho foi avaliar a produtividade e os custos de produção da cultura da soja quando semeada em diferentes arranjos de semeadura cruzada. O experimento foi realizado nas safras 2011/2012 e 2012/2013, em área cultivada em sistema de plantio direto. O delineamento experimental foi em blocos casualizados com 5 tratamentos e 8 repetições. Os tratamentos foram: semeadura em linhas paralelas com adubação e população de plantas recomendada; semeadura cruzada com população e adubo em dobro; semeadura cruzada com população recomendada e adubo em dobro; semeadura cruzada com população em dobro e adubação recomendada; semeadura cruzada com população de plantas e adubo recomendado. Os resultados mostraram maior produtividade para o tratamento de semeadura cruzada com população de plantas em dobro e adubo recomendado; apesar disto, a semeadura convencional em linhas paralelas apresenta maior receita líquida do que a semeadura cruzada da soja sendo, então, mais rentável para o produtor.
Soybean cross-seeding tillage came from better spatial arrangement of plants per area, so some producers have opted for this tillage system, which consists in seeding the area in two steps, forming a chessboard with perpendicular lines of tillage. This study aims to analyse the soybean productivity and production costs when it planted in different dispositions of crossed tillage. The experiment was conducted during the harvests 2011/2012 and 2012/2013 at Lageado Farm of the FCA/UNESP, Botucatu-SP, in an area cultivated under no-tillage. The experimental design was in randomized block with five treatments and eight replications. The treatments were: seeding in parallel rows with recommended fertilization and recommended plant population (conventional); cross-seeding with doubled fertilization and doubled plant population; cross-seeding with recommended plant population and doubled fertilization; cross-seeding with doubled plant population and recommended fertilization; cross-seeding with fertilization and recommended plant population. The results showed statistical difference among the treatments, with increased productivity for the cross-seeding with doubled plant population and recommended fertilization. However the conventional tillage with parallel lines shows higher net earnings than soybean cross-seeding, being more profitable for producer.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Introdução: O implante por cateter de prótese aórtica (TAVI, do inglês transcatheter aortic valve implantation) constitui tratamento alternativo para pacientes com estenose aórtica de alto risco cirúrgico ou inoperáveis. Para adquirir competência, o grupo multidisciplinar deve receber treinamento específico e acumular experiência na execução do TAVI. Contudo, sua curva de aprendizado não está bem estabelecida. Nosso objetivo foi analisar o impacto da curva de aprendizado na seleção de pacientes, nos aspectos técnicos e nos resultados clínicos do TAVI. Métodos: Estudo observacional e prospectivo dos primeiros 150 pacientes submetidos a TAVI por via femoral, entre janeiro de 2009 e dezembro de 2013 divididos em tercis (n = 50) de acordo com a data do procedimento. Os desfechos foram definidos conforme os critérios Valve Academic Research Consortium-2 (VARC-2). Resultados: A idade foi de 82,5 ± 6,7 anos, sendo 44% homens e 75% em classe NYHA III/IV. O EuroSCORE (24,2 ± 13% vs. 21,2 ± 10,8% vs. 23,4 ± 14,3%) e o STS Score (5,9 ± 2,9% vs. 6,7 ± 4,3% vs. 5,8 ± 3,1%) foram similares entre os grupos. Observou-se redução gradativa nos tempos do procedimento (107,2 ± 48,1 minutos vs. 90,3 ± 42,2 minutos vs. 76,6 ± 37,7 minutos; p < 0,01) e de fluoroscopia (31,3 ± 9,6 minutos vs. 25,4 ± 8,7 minutos vs. 17,2 ± 6,2 minutos; p = 0,01), e no volume de contraste (145,5 ± 70,9 mL vs. 123,2 ± 87,8 mL vs. 101,1 ± 50 mL; p = 0,01). A mortalidade reduziu-se de forma progressiva (20% vs. 10% vs. 4%; p = 0,047), e menores taxas de sangramentos e de refluxo paraprotético de grau moderado/grave foram observadas no terceiro tercil (14% e 4%, respectivamente). Não ocorreram casos de reintervenção, acidente vascular cerebral e nem outros eventos clínicos entre a alta hospitalar e o 30º dia pós-implante. Conclusões: A competência para o TAVI aumentou progressivamente com o número de pacientes tratados, associando-se a melhores desfechos clínicos.

Intraocular pressure, corneal thickness, and corneal hysteresis in Steinert's myotonic dystrophy

OBJETIVOS: Pressão intraocular (PIO) baixa medida por meio da tonometria de aplanação de Goldmann (TAG) é uma das manifestações oculares da distrofia miotônica de Steinert. O objetivo deste estudo foi avaliar a pressão intraocular compensada para as propriedades corneais (espessura corneal central e histerese corneal) em pacientes com distrofia miotônica. MÉTODOS: Um total de 12 olhos de 6 pacientes com distrofia miotônica de Steinert (grupo distrofia) e 12 olhos de 6 voluntários sadios (grupo controle) pareados para idade, raça e sexo foram incluídos no estudo. Tonometria de aplanação de Goldmann, tonometria de contorno dinâmico (TCD-Pascal) e analisador de resposta ocular (ORA) foram usados para medir a pressão intraocular. A espessura corneal central foi obtida por meio da paquimetria ultrassônica e a histerese corneal foi analizada usando o aparelho ORA. RESULTADOS: A pressão intraocular média (desvio-padrão) da TAG, TCD e compensada para a córnea do ORA no grupo distrofia foram 5,4 (1,4) mmHg, 9,7 (1,5) mmHg e 10,1 (2,6) mmHg, respectivamente. A pressão intraocular média (desvio-padrão) da TAG, TCD e compensada para a córnea do ORA no grupo controle foram 12,6 (2,9) mmHg, 15,5 (2,7) mmHg e 15,8 (3,4) mmHg, respectivamente. Houve diferença significativa nos valores da pressão intraocular entre os grupos distrofia e controle obtidas pela TAG (média, -7,2 mmHg; intervalo de confiança (IC) de 99%, -10,5 a -3,9 mmHg; P<0,001), TCD (média, -5,9 mmHg; IC de 99%, -8,9 a -2,8 mmHg; P<0,001) e ORA compensada para córnea (média, -5,7 mmHg; IC de 99%, -10,4 a -1,0 mmHg; P=0,003). A espessura corneal média (desvio-padrão) foi similar nos grupos distrofia (542 [31] µm) e controle (537 [11] µm) (P=0,65). A histerese corneal média (desvio-padrão) nos grupos distrofia e controle foram de 11,2 (1,5) mmHg e 9,7 (1,2) mmHg, respectivamente (P=0,04). CONCLUSÃO: Os pacientes com distrofia miotônica de Steinert apresentaram valores menores de pressão intraocular medidas tanto com Goldmann quanto compensadas para a córnea em comparação com indivíduos sadios. Uma vez que os valores da espessura corneal central e histerese corneal não diferiram significantemente entre os grupos, os valores baixos da pressão intraocular encontrados nos pacientes com distrofia miotônica não parecem estar relacionados com as propriedades corneais.
PURPOSE: Low intraocular pressure (IOP) measured by Goldmann applanation tonometry (GAT) is one of the ocular manifestations of Steinert's myotonic dystrophy. The goal of this study was to evaluate the corneal-compensated IOP as well as corneal properties (central corneal thickness and corneal hysteresis) in patients with myotonic dystrophy. METHODS: A total of 12 eyes of 6 patients with Steinert's myotonic dystrophy (dystrophy group) and 12 eyes of 6 age-, race-, and gender-matched healthy volunteers (control group) were included in the study. GAT, Dynamic Contour Tonometry (DCT-Pascal) and Ocular Response Analyzer (ORA) were used to assess the IOP. Central corneal thickness was obtained by ultrasound pachymetry, and corneal hysteresis was analyzed using the ORA device. In light of the multiplicity of tests performed, the significance level was set at 0.01 rather than 0.05. RESULTS: The mean (standard deviation [SD]) GAT, DCT, and corneal-compensated ORA IOP in the dystrophy group were 5.4 (1.4) mmHg, 9.7 (1.5) mmHg, and 10.1 (2.6) mmHg, respectively. The mean (SD) GAT, DCT, and corneal-compensated ORA IOP in the control group was 12.6 (2.9) mmHg, 15.5 (2.7) mmHg, and 15.8 (3.4) mmHg, respectively. There were significant differences in IOP values between dystrophy and control groups obtained by GAT (mean, -7.2 mmHg; 99% confidence interval [CI], -10.5 to -3.9 mmHg; P<0.001), DCT (mean, -5.9 mmHg; 99% CI, -8.9 to -2.8 mmHg; P<0.001), and corneal-compensated ORA measurements (mean, -5.7 mmHg; 99% CI, -10.4 to -1.0 mmHg; P=0.003). The mean (SD) central corneal thickness was similar in the dystrophy (542 [31] µm) and control (537 [11] µm) groups (P=0.65). The mean (SD) corneal hysteresis in the dystrophy and control groups were 11.2 (1.5) mmHg and 9.7 (1.2) mmHg, respectively (P=0.04). CONCLUSIONS: Patients with Steinert's myotonic dystrophy showed lower Goldmann and corneal-compensated IOP in comparison with healthy individuals. Since central corneal thickness and corneal hysteresis did not differ significantly between groups, the lower IOP readings documented in this dystrophy seem not to be related to changes in corneal properties.

O presente trabalho de pesquisa apresenta uma aplicação da relação de Poisson que pode ser utilizada em dados de magnetometria e gradiometria gravimétrica utilizando o exemplo na mesma região. A razão entre o contraste de suscetibilidade e o contraste de densidade (mapa litogeofísico) pode ser calculada. Como um teste destas técnicas, aplicamos esta relação nos dados de magnetometria e gradiometria gravimétrica 3D-FTG coletados na área do Baú-Gandarela no Quadrilátero Ferrífero, Minas Gerais, Brasil. A idéia principal foi estabelecer prioridades para prospecção de hematita na região supracitada. O mapa litogeofísico produzido mostra as regiões onde já existe uma mina de hematita e estabelece áreas para prospecção do minério.
We report here an application of the Poisson relation useful when a joint magnetic and gravity gradient survey has been made. Susceptibility contrast to density contrast ratio map (which we call the "pseudolithologic" map) can be estimated. As a test of these techniques we have applied to gravity gradiometry and magnetic airborne surveys collected over Baú-Gandarela area in Quadrilátero Ferrífero, Minas Gerais State, Brazil. The main idea here is to highlight the prospecting of hematites bodies in the area. The computed pseudolithologic map clearly distinguishes the regions of the map that we already have a hematite mine and hematites bodies.
