Abstract The aim of this study was to describe the prevalence, clinicopathological, and prognostic features of acinic cell carcinoma (AciCC) of the oral and maxillofacial region. AciCC cases were retrospectively retrieved from 11 pathology centers of three different countries. Medical records were examined to extract demographic, clinical, pathologic, and follow-up information. A total of 75 cases were included. Females (65.33%) with a mean age of 45.51 years were mostly affected. The lesions usually presented as an asymptomatic (64.28%) nodule (95.66%) in the parotid gland (70.68%). The association of two histopathological patterns was the most common finding (48.93%) and the tumors presented mainly conventional histopathological grades (86.11%). Surgical treatment was performed in the majority of the cases (59.19%). Local recurrence was observed in 20% of the informed cases, regional metastasis in 30.43%, and distant metastasis in 12.50%. The statistical analysis showed that the cases with a solid histopathological pattern (p=0.01), high-grade transformation (p=0.008), recurrence (p=0.007), and regional metastasis (p=0.03) were associated with poor survival. In conclusion, high histopathological transformation, presence of nodal metastasis, and recurrence were prognostic factors for AciCC of the oral and maxillofacial region.

RESUMO Objetivo: Descrever a implantação de um serviço de telemedicina de UTI durante a pandemia de COVID-19, assim como descrever e analisar os resultados dos primeiros quatro meses de funcionamento do programa. Métodos: Estudo observacional descritivo da implantação de um serviço de telemedicina de UTI seguido de análise retrospectiva dos dados clínicos de pacientes com COVID-19 internados em UTI entre abril e julho de 2020. Resultados: O serviço foi implantado em quatro semanas e mostrou-se viável em meio à pandemia. O treinamento foi desenhado para ser remoto e baseado em evidências, promovendo a padronização do atendimento aos pacientes com COVID-19. Mais de 100.000 visualizações foram registradas nas plataformas on-line de acesso livre e no aplicativo móvel. Durante o período do estudo, os casos de 326 pacientes com COVID-19 foram avaliados no programa. A mediana de idade foi de 60 anos (variação: 49-68 anos). Houve predomínio do sexo masculino (56%) e alta prevalência de hipertensão arterial (49,1%) e diabetes mellitus (38,4%). Na admissão na UTI, 83,7% dos pacientes estavam em ventilação mecânica invasiva, com uma mediana da relação PaO2/FiO2 < 150. Ventilação pulmonar protetora foi possível em 75% dos casos. A mortalidade na UTI foi de 65%, e a mortalidade hospitalar foi de 68%. Conclusões: A telemedicina de UTI forneceu treinamento multidisciplinar aos profissionais de saúde e acompanhamento clínico de centenas de pacientes críticos. A iniciativa na rede pública foi pioneira e mostrou-se viável em meio à pandemia de COVID-19, incentivando a criação de projetos semelhantes que combinem práticas baseadas em evidências, treinamento e telemedicina.
ABSTRACT Objective: To describe the implementation of a Tele-ICU program during the COVID-19 pandemic, as well as to describe and analyze the results of the first four months of operation of the program. Methods: This was a descriptive observational study of the implementation of a Tele-ICU program, followed by a retrospective analysis of clinical data of patients with COVID-19 admitted to ICUs between April and July of 2020. Results: The Tele-ICU program was implemented over a four-week period and proved to be feasible during the pandemic. Participants were trained remotely, and the program had an evidence-based design, the objective being to standardize care for patients with COVID-19. More than 100,000 views were recorded on the free online platforms and the mobile application. During the study period, the cases of 326 patients with COVID-19 were evaluated through the program. The median age was 60 years (IQR, 49-68 years). There was a predominance of males (56%). There was also a high prevalence of hypertension (49.1%) and diabetes mellitus (38.4%). At ICU admission, 83.7% of patients were on invasive mechanical ventilation, with a median PaO2/FiO2 ratio < 150. It was possible to use lung-protective ventilation in 75% of the patients. Overall, in-hospital mortality was 68%, and ICU mortality was 65%. Conclusions: Our Tele-ICU program provided multidisciplinary training to health care professionals and clinical follow-up for hundreds of critically ill patients. This public health care network initiative was unprecedented and proved to be feasible during the COVID-19 pandemic, encouraging the creation of similar projects that combine evidence-based practices, training, and Tele-ICU.

Marco contextual: La venopunción es un procedimiento frecuente en la atención de enfermería, y es necesario mapear/identificar los posibles problemas relacionados, considerando las especificidades de las pruebas diagnósticas, para subvencionar la atención. Objetivo: Analizar la incidencia y los tipos de manifestaciones de los traumatismos vasculares periféricos en personas sometidas a una tomografía computarizada y a una resonancia magnética. Metodología: Cohorte retrospectiva de documentos sectoriales y cohorte prospectiva con personas que se sometieron a una punción para realizarse una tomografía computarizada y una resonancia magnética de contraste. Los datos recogidos entre junio y diciembre de 2018 se trataron con la estadística descriptiva y el cálculo de la incidencia. Resultados: Participaron 76 usuarios y 2946 registros de las fichas de venopunción. En la cohorte retrospectiva, la incidencia de extravasación de contraste fue del 8,4%. En la cohorte prospectiva, la incidencia de traumatismo vascular fue del 52,6%. Las manifestaciones identificadas fueron eritema, dilatación de la vena, residuo de sangre, equimosis y endurecimiento. Conclusión: Se identificaron manifestaciones que pueden ser tratadas/minimizadas, y el enfermero es responsable de tomar decisiones basadas en las pruebas científicas que respaldan la práctica, con el objetivo de mantener la seguridad de los pacientes y la calidad de los cuidados de enfermería.
Enquadramento: A punção venosa é um procedimento frequente na assistência de enfermagem, sendo necessário mapear/identificar possíveis problemas relacionados, considerando as especificidades dos exames diagnósticos, para subsidiar a assistência. Objetivo: Analisar a incidência e os tipos de manifestações de traumatismo vascular periférico em pessoas que realizaram tomografia computadorizada e ressonância magnética. Metodologia: Coorte retrospetiva de documentos setoriais e coorte prospetiva com pessoas que foram puncionadas para realizar tomografia computadorizada e ressonância magnética com contraste. Dados colhidos entre junho e dezembro de 2018, tratados com estatística descritiva e cálculo da incidência. Resultados: Participaram 76 usuários e 2.946 registos das fichas de punções venosas. Na coorte retrospetiva, a incidência de extravasamento de contraste foi de 8,4%. Na coorte prospetiva, a incidência de traumatismo vascular foi de 52,6%. As manifestações identificadas foram eritema, dilatação da veia, resíduo de sangue, equimose e endurecimento. Conclusão: Identificadas manifestações que podem ser tratadas/minimizadas, cabendo ao enfermeiro a tomada de decisão baseada em evidências científicas que sustentem a prática, visando a segurança dos usuários e a qualificação do cuidado de enfermagem.
Background: The venipuncture is a frequent procedure in nursing care, being necessary to map/identify possible related problems, taking into account the specificities of diagnostic examinations, to support care delivery. Objective: Analyze the incidence and type of peripheral vascular trauma manifestations in people undergoing computed tomography and magnetic resonance imaging. Methodology: Retrospective cohort of sectorial documents and prospective cohort with individuals punctured for computed tomography and magnetic resonance imaging with contrast. Data collected from June to December 2018, subject to descriptive statistics and incidence measurement. Results: The study included 76 patients and 2,946 records of venipuncture evaluation forms. In the retrospective cohort, the incidence of extravasation of contrast was 8,4%. In the prospective cohort, the incidence of vascular trauma was 52,6%. The manifestations identified were erythema, vein dilation, blood residue, ecchymosis, and hardening. Conclusion: The manifestations identified can be treated/minimized, being the nurse responsible for decision-making based on scientific evidence, aiming at patient safety and quality of nursing care.

BACKGROUND Field testing required to license the combined measles, mumps, and rubella (MMR) vaccine must take into account the current recommendation of the vaccine in Brazil: first dose at 12 months and second dose at 15 months of age in combination with a varicella vaccine. OBJECTIVES This study aimed to evaluate the clinical consistency, immunogenicity, and reactogenicity of three batches of MMR vaccine prepared with active pharmaceutical ingredients (API) from Bio-Manguinhos, Fiocruz (MMR-Bio), and compare it to a vaccine (MMR produced by GlaxoSmithKline) with different API. METHODS This was a phase III, randomised, double-blind, non-inferiority study of the MMR-Bio administered in infants immunised at health care units in Pará, Brazil, from February 2015 to January 2016. Antibody levels were titrated by immunoenzymatic assays. Adverse events were recorded in diaries. FINDINGS Seropositivity levels after MMR-Bio were 97.6% for measles, 84.7% for mumps, and 98.0% for rubella. After the MMRV vaccine, seroconversion rates and GMT increased substantially for mumps. In contrast, approximately 35% of the children had no detectable antibodies to varicella. Systemic adverse events were more frequent than local events. CONCLUSION The demonstration of batch consistency and non-inferiority of the Bio-MMR vaccine completed the technology transfer. This is a significant technological achievement with implications for immunisation programs.

Discrimination ability of Santa Inês and crossbred Santa Inês × Dorper lamb heavy carcasses by the Brazilian and European classification systems

ABSTRACT The ability of discriminating carcass characteristics of different fat cover scores of heavy carcasses, according to the European (EUS) and Brazilian (BRS) classification systems, was assessed. Fifty-six lambs, weighing between 26.25 and 46.15 kg, of the Santa Inês and crossbred Santa Inês × Dorper genetic groups were evaluated. The level of adiposity was assessed through color photography of the carcasses after refrigeration according to the EUS and BRS. The carcasses were assigned to four groups by cluster analysis according to 25 variables, namely, cold carcass weight; muscle; bone; fat; and muscle:fat and fat:bone ratios of the carcass and cuts (hindquarter, shoulder, back); kidney, pelvic, and inguinal fat; and subcutaneous fat thickness. Of the four groups obtained by cluster analysis, the scores according to the different classification systems only statistically differed between groups 1 and 4. The BRS had a higher number of variables well correlated with the scores by assessors than the groups classified by the EUS. The BRS was better correlated with tissue composition. However, most variables were better correlated with backfat thickness than the score obtained through the classification systems. Better results were obtained regarding the prediction of carcass fat by the BRS using backfat thickness or cold carcass weight. The Brazilian lamb carcass classification system better predicts tissue composition and is the best method to discriminate intermediate-fat classes when associated with cold carcass weight.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Objetivou-se com o presente trabalho avaliar o acúmulo de forragem e a estrutura do dossel do capim-Xaraés submetido a cinco alturas de resíduos (10; 20; 30; 40 e 50 cm) com intervalo de corte baseado na interceptação luminosa (IL) pelo dossel durante a rebrotação (95% de IL). O experimento foi conduzido no Setor de Forragicultura do Centro de Ciências Agrárias e Ambientais da Universidade Federal do Maranhão (UFMA), em Chapadinha, no período de março a novembro de 2011. Utilizou-se o delineamento inteiramente casualizado, com cinco repetições. O período de descanso baseado na estratégia de 95% de IL variou em função das alturas de resíduo e época do ano. Houve efeito das alturas de cortes para a produção de forragem, a densidade populacional de perfilhos, o número de perfilhos vivos, altura do dossel e densidade volumétrica de forragem. Apenas a relação folha/colmo e a taxa de acúmulo de forragem não foram influenciadas pelos fatores estudados, no período chuvoso de 2011. No período da seca, os pastos de capim-Xaraés foram afetados de forma negativa para todas as características avaliadas, independente das alturas de resíduo. De maneira geral, intensidades de cortes mais drásticas (10 cm), promovem maior produção de forragem, maior DPP, maior número de perfilhos mortos e maior densidade, no entanto, resulta em menor número de cortes e maior intervalo entre corte. O inverso ocorre com intensidades de cortes mais lenientes (40 e 50 cm).
The aim of this work was to evaluate the forage accumulation and sward structure of Xaraés grass subjected to five residue heights (10; 20; 30; 40 and 50 cm) with cutting interval based on light interception (LI) by sward during the regrowth (95% of LI). The experiment was conducted in forage production sector of the Agrarian and Environmental Sciences Center of the Federal University of Maranhão, in Chapadinha, in the period from March to November of 2011. A completely randomized design with five repetitions was used. The rest period based on 95% LI strategy varied according to the residue heights and time of year. There was effect of cutting heights for the production of forage, the density of tillers, the number of living tillers, sward height and volumetric density of forage. Only the leaf/stem ratio and the rate of forage accumulation were not influenced by the factors studied, in the rainy season of 2011. In the dry season, the pastures of Xaraés grass were affected in a negative way for all features evaluated, regardless of heights of residue. In General, more drastic cuts intensities (10 cm) promote higher forage production, greater density of tillers, larger number of dead tillers and higher density, however, results in less number of cuts and greater interval between cuts. The reverse occurs with more lenient cuts intensities (40 and 50 cm).

Objetivo Avaliar o efeito da Reeducação Postural Global (RPG) sobre o sistema cardiovascular por meio da variabilidade da frequência cardíaca (VFC), pressão arterial (PA) e frequência cardíaca (FC). Materiais e métodos Dezessete homens saudáveis (22,47± 3,02 anos) foram submetidos às posturas rã no chão, rã no ar, sentada, de pé contra a parede e de pé inclinada, sendo duas posturas por sessão. A pressão arterial sistólica e diastólica (PAS, PAD) e FC foram registradas. Os intervalos entre os batimentos cardíacos foram coletados durante toda a sessão (Polar S810i). O domínio da frequência foi analisado (transformada Wavelet), as bandas de baixa (LF) e a alta frequências (HF) foram obtidas. Os dados foram analisados por ANOVA e Tukey (p < 0,05). Resultados Houve aumento da relação LF/HF nas posturas rã no chão (1 ± 0,1 vs. 2 ± 0,3 p < 0,05); rã no ar (1 ± 0,1 vs. 2 ± 0,2 p < 0,01). Observou-se aumento da PAS nas posturas rã no chão (123 ± 2 vs. 136 ± 4 p < 0,05), rã no ar (122 ± 2 vs. 133 ± 3 p < 0,05), de pé na parede (123 ± 2 vs. 136 ± 4 p < 0,05), de pé inclinada (124 ± 3 vs. 146 ± 5 p < 0,05). Houve aumento da PAD nas posturas rã no chão (69 ± 2 vs. 81 ± 2 p < 0,01), rã no ar (72 ± 2 vs. 83 ± 3 p < 0,05), sentada (85 ± 2 vs. 102 ± 3 p < 0,01). Houve aumento da FC nas posturas rã no ar (67 ± 2 vs. 77 ± 3 p < 0,05) e de pé inclinada (88 ± 3,5 vs. 101 ± 3 p < 0,05). Conclusão O aumento da relação LF/HF e também da PA e da FC aponta para maior atividade simpática, possivelmente relacionada ao trabalho isométrico desenvolvido durante a realização do método de RPG.
Objective To evaluate the effect of Global Postural Reeducation (GPR) on cardiovascular system by heart rate variability (HRV), blood pressure (BP) and heart rate (HR). Materials and methods Seventeen healthy men (22.47 ± 3.02 years) were submitted to the postures frog on the floor, frog on the air, sitting, standing against the wall and inclined standing, two postures per session. The systolic and diastolic blood pressure (SBP and DBP) and HR were recorded. The intervals between heartbeats were collected during the whole session (Polar S810i). The frequency domain was analyzed (Wavelet Transform), the low frequency (LF) and high frequency (HF) were obtained. The data were analyzed by ANOVA and Tukey (p < 0.05). Results Increased LF/HF ratio was observed in the frog on the floor (1 ± 0.1 vs. 2 ± 0.3 p < 0.05) and on the air postures (1 ± 0.1 vs. 2 ± 0.2 p < 0.01). There was an increase in SBP in the postures frog on the floor (123 ± 2 vs. 136 ± 4 p < 0.05), frog on the air (122 ± 2 vs. 133 ± 3 p < 0.05), standing against the wall (123 ± 2 vs. 136 ± 4 p < 0.05), inclined standing (124 ± 3 vs. 146 ± 5 p < 0.05). There was increase of DBP in the postures frog on the floor (69 ± 2 vs. 81 ± 2 p < 0.01), frog on the air (72 ± 2 vs. 83 ± 3 p < 0.05), sitting (85 ± 2 vs. 102 ± 3 p < 0.01). There was increase in HR in the postures frog on the air (67 ± 2 vs. 77 ± 3 p < 0.05) and inclined standing (88 ± 3.5 vs. 101 ± 3 p < 0.05). Conclusion The increase in LF/HF ratio and also the BP and HR indicates high sympathetic activity, possibly related to the work isometric developed during GPR method. [P]

A model of genetic guidance for hemoglobinopathy patients and laboratory diagnosis of family members as educational and preventive measures

BACKGROUND: The high frequency of hemoglobinopathies in Brazil constitutes a public health problem and thus educational and preventive measures are necessary to reduce the incidence. Genetic guidance, a modality of genetic counseling, and family screening are measures that can assist in reproductive decisions and mitigate clinical, psychological and social problems of families with these disorders. OBJETIVE: The objective of the current study was to evaluate the effectiveness of educational and preventive measures for hemoglobinopathies using genetic guidance and laboratory screening of families. METHODS: The diagnoses of patients with hemoglobinopathies were confirmed and then the level of knowledge about their disease was evaluated and genetic guidance was provided. Three months later, the level of assimilated information of these patients was evaluated. In addition, laboratory diagnosis of family members was carried out. RESULTS: Diagnosis of sickle cell anemia was confirmed for most patients. Moreover, the majority of the patients who had a low level of knowledge before genetic guidance (68.8%) demonstrated a higher level of assimilated information after the process (81.8%). Almost 70% of the family members had hemoglobin changes and some had hemoglobinopathies (2.6%). They were duly informed about the results of the examinations, which made it possible to investigate further. CONCLUSION: Genetic guidance and family screening were effective preventive and educational measures that improved the quality of life of patients, preventing complications and sequels and allowed the referral of those who may transmit altered genes for clinical diagnosis and to genetic counseling services.
