ABSTRACT Intersectoral collaboration for health educational practices in the school environment is strengthened by the School Health Program. The objective of this study is to understand the development of educational practices on STI/HIV/AIDS carried out in schools from the perspective of the multidis-ciplinary health team, in light of complex thinking. This is a qualitative study that used constructivist grounded theory as an analysis method. Intensive interviews were carried out with 35 health professionals in two sample groups, audio-recorded, transcribed and analyzed, in three stages of coding: initial, focused and theoretical. The central category ‘Revealing the development of educational practices in STI/HIV/AIDS within the school’ emerges from nine subcategories that develop a dialogue with the work ‘The seven necessary knowledge for future education’. The STI/HIV/AIDS theme is considered a priority and pertinent knowledge for educational practices, which requires considering the complexity of the human being to understand profound issues inherent to the human condition, and which breaks the paradigm of fragmentation of knowledge. Educational practices need to go beyond lecture models, be diverse and face challenges and uncertainties, avoiding blindness in knowledge, teaching, understanding and human ethics, and bringing together young people, teachers and professionals.

RESUMO A colaboração intersetorial para as práticas educativas em saúde no ambiente escolar é fortalecida pelo Programa Saúde na Escola. O objetivo deste estudo é compreender o desenvolvimento das práticas educativas sobre IST/HIV/aids realizadas nas escolas na perspectiva da equipe multiprofissional de saúde, à luz do pensamento complexo. Trata-se de um estudo qualitativo que utilizou a teoria fundamentada nos dados construtivista como método de análise. Foram realizadas entrevistas intensivas com 35 profissionais de saúde em dois grupos amostrais, audiogravadas, transcritas e analisadas, em três etapas de codificação: inicial, focalizada e teórica. A categoria central ‘Revelando o desenvolvimento das práticas educativas em IST/HIV/aids no âmbito da escola’ emerge de nove subcategorias que desenvolvem um diálogo com a obra ‘Os sete saberes necessários à educação do futuro’. O tema IST/HIV/aids é considerado prioritário e um conhecimento pertinente para as práticas educativas, que requer considerar a complexidade do ser humano para compreender questões profundas, inerentes à condição humana, e que rompa com o paradigma da fragmentação dos saberes. As práticas educativas precisam superar os modelos de palestras, ser diversificadas e enfrentar os desafios e incertezas, evitando as cegueiras do conhecimento, ensinando compreensão e ética humanas, e aproximando jovens, professores e profissionais.

Resumo O objetivo é identificar, por meio de uma revisão integrativa, o perfil epidemiológico da mortalidade em crianças indígenas brasileiras menores de cinco anos. Foram incluídos artigos que avaliaram a mortalidade de crianças indígenas brasileiras menores de um e/ou cinco anos, entre 2000-2020, em português, inglês e/ou espanhol, nas bases LILACS, SciELO e MEDLINE. Das 3.229 publicações encontradas, 22 foram inseridas na revisão. A mortalidade infantil variou de 15,2/1.000 NV (1995-1999) para a etnia Kayabí a 307,7/1.000 NV (2000) para a etnia Hupdäh. Entre os estudos que avaliaram a mortalidade infantil pelos componentes etários, destaca-se o período pós-neonatal, com proporções que variaram de 15,2% (1990-1994), para crianças indígenas do Paraná, a 83,3% (2007-2008) para a etnia Guarani. A principal causa de mortalidade foram as doenças respiratórias, variando de 17,1% (2010) a 75% (2007-2008) entre a etnia Guarani. Crianças indígenas brasileiras menores de um ano apresentaram taxas de mortalidade elevadas, quando comparadas às crianças não-indígenas, para todas as etnias e regiões do país. Entre os componentes de TMI, observou-se elevado número de óbitos no período pós-neonatal, tendo como principal causas de óbito as doenças do aparelho respiratório.

Resumen El objetivo es identificar el perfil epidemiológico de la mortalidad en niños indígenas brasileños menores de cinco años, a través de una revisión integradora. Se incluyeron artículos que evaluaron la mortalidad de niños indígenas brasileños menores de uno o cinco años, entre 2000-2020, en portugués, inglés o español, en las bases de datos LILACS, SciELO y MEDLINE. Se incluyeron 22 de las 3.229 publicaciones encontradas en la revisión. La mortalidad infantil osciló entre 15,2/1.000 NV (1995-1999) para el grupo étnico Kayabí y 307,7/1.000 NV (2000) para el grupo étnico Hupdäh. Entre los estudios que evaluaron la mortalidad infantil por componentes de edad, se destaca el período posneonatal, con proporciones que variaron desde el 15,2% (1990-1994), para los niños indígenas de Paraná, hasta el 83,3% (2007-2008) para la etnia guaraní. La principal causa de mortalidad fueron las enfermedades respiratorias, oscilando entre el 17,1% (2010) y el 75% (2007-2008) entre la etnia guaraní. Los niños indígenas brasileños menores de un año experimentaron altas tasas de mortalidad en comparación con los niños no indígenas en todas las etnias y regiones del país. Entre los componentes del TMI, se observó un elevado número de muertes en el período posneonatal, siendo las enfermedades del sistema respiratorio las principales causas de muerte.

Abstract The aim is to identify the epidemiological profile of mortality among Brazilian indigenous children under five years of age, through an integrative review. Articles that evaluated the mortality of indigenous Brazilian children under one and/or five years of age, from 2000 to 2020, in Portuguese, English, and/or Spanish, in the LILACS, SciELO, and MEDLINE databases were included. Of the 3,229 publications found, 22 were included in the review. Infant mortality ranged from 15.2/1,000 live births (LB) (1995-1999) for the Kayabí ethnic group to 307.7/1,000 LB (2000) for the Hupdäh ethnic group. Among the studies that evaluated infant mortality by age components, the post-neonatal period stands out, with proportions that varied from 15.2% (1990-1994), for indigenous children from Paraná, to 83.3% (2007- 2008) for the Guarani ethnic group. The main cause of mortality was respiratory diseases, ranging from 17.1% (2010) to 75% (2007-2008) among the Guarani ethnic group. Brazilian indigenous children under one year of age experienced high mortality rates, when compared to non-indigenous children, for all ethnicities and regions of the country. Among the IMT components, a high number of deaths were observed in the post-neonatal period, with respiratory system diseases representing the main causes of death.

We report a facile hydrothermal method for the synthesis of niobium pentoxide (Nb2O5) under two different temperatures, 120 °C (Nb2O5-120) and 150 °C (Nb2O5-150). The obtained materials were characterized by structural, optical and morphological techniques. Also, the photocatalytic properties of the Nb2O5 samples were evaluated in two reactions under ultraviolet (UVC) irradiation: degradation of methylene blue (MB) and indigo carmine (IC) dyes; and esterification of free fatty acids (FFA) present in waste cooking oil (WCO). The Nb2O5 was formed at different temperatures of synthesis, and the increase in temperature did not cause significant changes in the structural and optical characteristics, but resulted in an increased surface area of the synthesized materials. Both synthesized materials showed excellent efficiency for dye photodegradation. The sample Nb2O5-150 presented the best performance for the MB photodegradation, with almost 85% of the removal efficiency. In this case, the adsorption of MB molecules on the surface of the material was high due to the favorable electrostatic interaction and also because of its high surface area. For the IC photodegradation, the adsorption was insignificant, and both samples presented approximately 100% of the removal efficiency. These materials were also promising for the reduction of free fat acids in waste oil by photoesterification.

Resumen Marco contextual: Los niños con Necesidades Sanitarias Especiales (NSE) tienen diversas necesidades sanitarias que pueden influir en su integración y en su trayectoria escolar. La identificación precoz de las NSE es esencial para que los equipos de salud familiar y escolar planifiquen una mejor atención sanitaria e impulsen la inclusión escolar. Objetivo: Traducir, adaptar culturalmente y validar para la población portuguesa el instrumento Children with Special Health Care Needs Screener (CSHCN Screener). Metodología: Estudio metodológico que incluye la traducción y la adaptación semántica y cultural. Participaron 390 padres de niños de la región norte de Portugal. Resultados: La versión portuguesa consta de 14 ítems. El panel de expertos y los padres de niños con NSE consideraron que el instrumento era aceptable, comprensible y válido en cuanto al contenido. El CSHCN Screener tardó aproximadamente un minuto en completarse o autocompletarse. Conclusión: La versión portuguesa del CSHCN Screener demuestra ser un instrumento válido y fiable para identificar a los niños con NSE.

Abstract Background: Children with special health care needs (CSHCN) have a range of health care needs that can influence their integration and their academic life. Early identification of CSHCN is essential for family and school health teams to plan better health care and promote school inclusion. Objective: To translate, culturally adapt, and validate the Children with Special Health Care Needs Screener (CSHCN Screener) for the Portuguese population. Methodology: Methodological study including translation and semantic and cultural adaptation. A total of 390 parents of children from the northern region of Portugal participated in the study. Results: The Portuguese version consists of 14 items. The panel of experts and the parents of CSHCN considered the instrument to be acceptable, understandable, and with content validity. The CSHCN Screener took about one minute to complete or self-complete. Conclusion: The Portuguese version of the CSHCN Screener proves to be a valid and reliable tool for identifying CSHCN.

Resumo Enquadramento: Crianças com Necessidades de Saúde Especiais (NSE) apresentam diversas necessidades de saúde que podem influenciar a sua integração e o seu percurso escolar. A identificação precoce das NSE é fundamental para que as equipas de saúde familiar e escolar planeiem melhores cuidados de saúde e potenciem a inclusão escolar. Objetivo: Traduzir, adaptar culturalmente e validar para a população portuguesa o instrumento Children with Special Health Care Needs Screener (CSHCN Screener). Metodologia: Estudo metodológico que inclui a tradução e adaptação semântica e cultural. Participaram 390 pais de crianças da região norte de Portugal. Resultados: A versão portuguesa é composta por 14 itens. O painel de peritos e os pais de crianças com NSE consideraram o instrumento aceitável, compreensível e com validade no conteúdo. O CSHCN Screener levou cerca de um minuto a ser preenchido ou autopreenchido. Conclusão: A versão portuguesa do CSHCN Screener demonstra ser um instrumento válido e fiável na identificação de crianças com NSE.

Production and fruit quality of Italian zucchini under brackish water irrigation strategies

ABSTRACT Semi-arid regions are subject to irregular rainfall distribution, leading to long periods of drought. Therefore, the only way to achieve and ensure production is through irrigation. However, in these regions, brackish water is predominant, requiring the adoption of irrigation strategies for better utilization. In this context, the objective of present study was to evaluate the production and fruit quality of Italian zucchini subjected to brackish water irrigation strategies during phenological stages. A completely randomized design with five treatments was used: S1 = low-salinity water (0.8 dS m-1) throughout the crop cycle; S2 = high-salinity water (3.0 dS m-1) only during germination and growth stages (0-11 DAS); S3 = high-salinity water (3.0 dS m-1) only during the pre-flowering stage (12-22 DAS); S4 = high-salinity water (3.0 dS m-1) only during the fruiting stage (23-42 DAS); S5 = high-salinity water (3.0 dS m-1) only during the harvesting stage (43-63 DAS), with eight replications. Irrigation with brackish water (3.0 dS m-1) during the pre-flowering and fruiting stages negatively affects the length and diameter of Italian zucchini fruits. The average fruit mass, skin thickness, production, and water use efficiency are reduced when zucchini plants were irrigated with water of higher electrical conductivity during the germination and initial growth stages, while the soluble solids content is increased. Under conditions of high salinity, it is possible to irrigate zucchini crop without loss of fruit quality and production throughout the harvesting stage.

RESUMO As regiões semiáridas estão sujeitas à distribuição irregular das chuvas, o que ocasiona longos períodos de estiagem. Deste modo, a única maneira de obter e garantir a produção é utilizando a irrigação, porém, nessas regiões, ocorre a predominância de águas de baixa qualidade (salobras), sendo necessário adotar estratégias de irrigação para melhor aproveitamento. Neste sentido, objetivou-se avaliar a produção e qualidade de frutos da abobrinha italiana submetida a estratégias de irrigação com água salobra nos estágios fenológicos. Foi utilizado o delineamento inteiramente casualizado, com cinco tratamentos, correspondendo a cinco estratégias de irrigação (E1= água de menor salinidade (0,8 dS m-1) durante todo o ciclo da cultura; E2= água de maior salinidade (3,0 dS m-1) apenas nas fases de germinação e crescimento (0-11 DAS); E3= água de maior salinidade (3,0 dS m-1) apenas na fase de pré-floração (12-22 DAS); E4= água de maior salinidade (3,0 dS m-1) apenas na fase de frutificação (23-42 DAS); E5= água de maior salinidade (3,0 dS m-1) apenas na fase de colheita (43-63 DAS), com oito repetições. A irrigação com água salobra (3,0 dS m-1) durante as fases de pré-floração e frutificação afeta negativamente o comprimento e o diâmetro dos frutos de abobrinha italiana. A massa média dos frutos, a espessura da casca, a produção e a eficiência no uso da água são reduzidas quando irrigados com água de maior condutividade elétrica durante as fases de germinação e crescimento inicial, enquanto o teor de sólidos solúveis é aumentado. Em condições de alta salinidade é possível irrigar a cultura da abobrinha sem perda da qualidade e da produção dos frutos durante toda a fase de colheita.

Increased risk for signs and clinical symptoms associated with sedentary behaviour in people diagnosed with COVID-19: a retrospective observational study

ABSTRACT This study aimed to verify the association between sedentary behaviour (SB) and clinical signs and symptoms in individuals with COVID-19. Five hundred nine people diagnosed with COVID-19 were cross-sectionally assessed by telephone interviews. Sociodemographic characteristics, clinical signs and symptoms and SB (IPAQ-SV) were obtained. It was found an association between increased sedentary behaviour (ISB) and clinical signs/symptoms of COVID-19 (p< 0.05). Controlling for the variables age, body mass index and moderate/vigorous physical activity, men with ISB had a higher occurrence of headache (OR= 2.357; 95%CI 1.312–4.232) and cough (OR= 2.508; 95%CI 1.268–4.959), women with ISB had a reduction in fatigue or tiredness (OR= 0.574; 95%CI 0.353–0.932). Four or more hours of SB increased the risk of headache and cough in men. Women experienced reduced symptoms of fatigue/tiredness during COVID-19 diagnosis.

ABSTRACT The present study aimed to compare the quality of life on pre-diagnosis and post-recovery of people diagnosed with COVID-19 and examine the impact of its signals, symptoms, and clinical outcomes on quality of life in adults and older adults, considering sex, age groups, nutritional status, physical activity level, and family income as confounding variables. This retrospective observational study included 509 participants (39% male) ≥ 18 years old from a city in the northeastern region of São Paulo, Brazil, diagnosed with COVID-19 between March/2020 and February/2021. Participants were telephonically interviewed to provide information about COVID-19 infection, quality of life, and physical activity, considering the week pre-diagnosis and the week prior to the interview (post-recovery). Quality of life decreased in the post-COVID-19 recovery period compared to pre-diagnosis periods (Med= 5.6; Med= 3.4, respectively; Z= −19,589; p< 0.001). After controlling comparisons for age, sex, nutritional status, physical activity level, and family income, it was observed that the infection increased the frequency of respiratory distress (F= 3,956; [df= 1]; p= 0.047) and length of hospitalisation (≥ 6 days) (F= 6,538; [df= 1]; p= 0.011). COVID-19 infection induced worse respiratory distress and length of hospitalisation, compromising the survivors’ quality of life.

Abstract This research paper focuses on the use of murumuru husk ash (MHA), an agro-industrial waste generated specifically in the Amazon region, as partial replacement of cement in structural concrete. It also evaluates the physical, chemical and mineralogical characteristics of the ash to act as filler in concrete and its properties in the fresh and hardened states in concrete. To this end, the MHA performed physical-mechanical characterization tests, such as specific mass, pozzolanic activity with Portland cement, pozzolanic activity with lime, and BET test, also performing the mineralogical and chemical analysis of the ash. The results showed that there is technical feasibility with partial replacement of 6% of MHA in cement using plasticizer additive to improve workability, thus showing an improvement in the physical-mechanical and durability properties of the concrete.

Resumo O estudo objetiva pesquisar o uso da cinza da casca do murumuru (CCM), um resíduo agroindustrial gerado especificamente na região amazônica como substituição parcial do cimento no concreto estrutural e avaliar as características físicas, químicas e mineralógicas da cinza para atuação como filler no concreto, além de suas propriedades no estado fresco e endurecido no concreto. Com esse intuito, o CCM passou pelos ensaios de caracterização físico-mecânicas como o de massa específica, atividade pozolânica com cimento Portland, atividade pozolânica com a cal e ensaio de BET, realizando-se também as análises mineralógicas e químicas da cinza. Os resultados mostraram que existe uma viabilidade técnica com substituição parcial de 6% de CCM no cimento, utilizando-se aditivo plastificante para melhorar a trabalhabilidade, mostrando uma melhora nas propriedades físico-mecânicas e de durabilidade do concreto.

ABSTRACT: Pythiosis is the disease caused by aquatic oomycetes of the genus Pythium. In dogs the typical course of the disease involves the gastrointestinal tract, characterized by transmural thickening of the stomach or intestines. However, infection by Pythium spp. has only rarely been recognized as a cause of esophagitis in dogs. Thus, the present reports a case of esophageal and gastric pythiosis in an eight-month-old female pit bull dog. The dog was attended at the hospital after two months presenting regurgitation and dyspnea. It was reported that the dog lived in an urban area and had no previous sanitary issues. At clinical examination it was noted that the dog presented crackling sounds at pulmonary auscultation. A support therapy accompanied by antibiotics has been employed; however, 22 days after hospitalization the clinical condition worsened, and the dog died. At necropsy, the wall of the distal segment of the esophagus and the cardia and part of the fundus of the stomach were expanded by a focal extensive irregular intramural annular mass. Additionally, there was a transmural esophageal fistula. At histology, the walls of the esophagus and stomach were extensively expanded by multifocal extensive areas of necrosis, associated with a pyogranulomatous infiltrate and abundant granulation tissue containing multiple negative images of hyphae that were highlighted by silver impregnation (Grocott). Furthermore, immunohistochemistry and PCR for P. insidiosum were both positive in samples of paraffin-embedded esophageal tissue.

RESUMO: A Pitiose é uma doença causada por oomicetos aquáticos do gênero Pythium. Em cães o curso típico da doença envolve o trato gastrointestinal, caracterizado por espessamento transmural do estômago ou intestinos. No entanto, a infecção por Pythium spp. raramente foi reconhecida como causa de esofagite em caninos. Assim, o presente trabalho tem como objetivo relatar um caso de pitiose esofágica e gástrica em um canino Pit Bull fêmea de oito meses de idade. O cão foi recebido no hospital com histórico de regurgitação e dispneia durante dois meses. Foi relatado que o animal era de um domicílio de área urbana e não possuía histórico pregresso de enfermidades. No exame clínico contatou-se ainda crepitações à ausculta pulmonar. Foi instituído tratamento de suporte e antibioticoterapia, porém após 22 dias de internação, o cão evoluiu para o óbito. Na necropsia, as paredes do segmento distal do esôfago e o cárdia e parte do fundo do estômago estavam expandidos por uma massa anular intramural focalmente extensiva e irregular. Além disso, havia uma fístula esofágica transmural. À histologia, as paredes do esôfago e do estômago apresentavam-se difusamente expandidas por áreas multifocais extensas de necrose, associadas a infiltrado piogranulomatoso, abundante proliferação de fibroblastos imaturos, neovascularização e contendo múltiplas imagens negativas de hifas realçadas pela impregnação por prata (Grocott). Além disso, tanto a imuno-histoquímica como PCR para P. insidiosum foram positivas em amostras de esôfago embebidas em parafina.

ABSTRACT: This study recorded the physicochemical properties of the marula fruits, the biometry and breaking the dormancy in the marula seeds. Using 200 ripe fruits, the physicochemical characterization was done. From a random sample of 400 seeds, an assessment of the biometrics was performed, and the weight, length, width and thickness of the fruits and seeds were evaluated. Two experiments were conducted to study dormancy breaking using pre-germination treatments, which included immersing the marula seeds five times (0, 15, 30, 45 and 60 minutes) in concentrated sulfuric acid (96%) and water, at 65 ºC temperature. The control was time zero, using plain distilled water, with 25 seeds per treatment and four replications. The marula fruits revealed average values of 21.65 ± 3.44 g, 35.5 ± 2.38 and 34.8 ± 2.73, respectively, for weight, diameter and length. The soluble solids (SS), Hydrogenonic Potential (pH), titratable acidity (AT) and ratio (AT/SS) were 14.74 ± 1.0º Brix, 3.93 ± 0.49 pH, 0.63 ± 0.22 and 27.07 ± 12.10, respectively. For the weight of one thousand seeds, the average value was 3.576 ± 188.4 g. The mean values for the dimensions of the width, length and diameter of the seeds were 22.9 ± 2.11; 23.7 ± 2.04; and 14.9 ± 2.09 mm, respectively. The physiological quality of these seeds was enhanced by immersing the seeds from freshly picked fruits in sulfuric acid for a maximum of 30 minutes and storing them for six months.

RESUMO: Objetivou-se com este trabalho estudar as características físico-químicas de frutos, biometria e superação da dormência de sementes de marula. Para a caracterização físico-química foram utilizados 200 frutos maduros. Realizou-se a biometria utilizando uma amostra aleatória de 400 sementes e foram avaliados: peso, comprimento, largura e espessura de frutos e sementes. Para o estudo da superação de dormência foram realizados dois experimentos com os tratamentos pré-germinativos constituídos pela imersão das sementes em ácido sulfúrico concentrado (96%) e água a temperatura de 65 ºC por cinco tempos (0, 15, 30, 45 e 60 minutos), sendo o tempo zero a testemunha, onde utilizou-se apenas água destilada, com quatro repetições de 25 sementes por tratamento. Os frutos de marula apresentaram valores médios de 21,65 ± 3,44 g, 35,5±2,38 e 34,8±2,73 mm de peso, diâmetro e comprimento, respectivamente e sólidos solúveis (SS), Potencial Hidrogeniônico ( pH), acidez titulável (AT) e ratio (AT/SS) foram de 14,74 ± 1,0 ºBrix, 3,93 ± 0,49 pH, 0,63 ± 0,22 e 27,07 ± 12,10 respectivamente. As sementes apresentaram valores médios de 3.576 ± 188,4 g para o peso de mil sementes. Para as dimensões, largura, comprimento e diâmetro de sementes, apresentaram valores médios de 22,9 ± 2,11; 23,7 ± 2,04; e 14,9 ± 2,09 mm respectivamente. A imersão das sementes procedentes de frutos recém colhidos em ácido sulfúrico por até 30 minutos e o armazenamento por seismeses promove incrementos na sua qualidade fisiológica.

RESUMO Objetivo Analisar as recomendações de exclusão de tecnologias em saúde no Sistema Único de Saúde (SUS), feitas pela Comissão Nacional de Incorporação de Tecnologias no Sistema Único de Saúde (Conitec) de 2012 a 2023, e identificar os critérios de desinvestimento utilizados. Métodos Análise documental, descritiva e retrospectiva, dos relatórios da Conitec que avaliaram solicitações de exclusão de tecnologias. Resultados Foram identificados 24 relatórios, sobre 74 tecnologias, em que, predominantemente, as solicitações envolveram medicamentos (95,9%). A Conitec recomendou favoravelmente 95% das exclusões, priorizando a ausência de registro na Agência Nacional de Vigilância Sanitária e a existência de alternativas terapêuticas. Conclusão A baixa demanda de exclusões em comparação às incorporações revela desafios na identificação da obsolescência e resistência à desincorporação de tecnologias. A sustentabilidade do SUS exige maior monitoramento das tecnologias incorporadas, para otimização dos recursos e promoção da eficiência do sistema de saúde.

RESUMEN Objetivo Investigar las solicitudes de exclusión de tecnologías sanitarias en el Sistema Único de Salud (SUS) analizadas por National Committee for Health Technology Incorporation in the Unified Health System (Conitec) de 2012 a 2023, e identificar los criterios de desinversión utilizados. Métodos Análisis documental, descriptivo y retrospectivo de los informes de Conitec que evaluaron solicitudes de exclusión de tecnología. Resultados se identificaron 24 informes, sobre 74 tecnologías, en los cuales, predominantemente, las solicitudes involucraron medicamentos (95,9%). Conitec recomendó favorablemente el 95% de exclusiones, priorizando la ausencia de registro ante Anvisa y la existencia de alternativas terapéuticas. Conclusión La baja demanda de exclusiones en comparación con las incorporaciones revela desafíos en la identificación de obsolescencia y resistencia a deshacerse de tecnologías. La sostenibilidad del SUS requiere un mayor monitoreo de tecnologías incorporadas para optimizar recursos y promocionar la eficiencia del sistema de salud.

ABSTRACT Objective To analyze the recommendations for exclusion of health technologies in the Brazilian National Health System (SUS), made by the National Commission for the Incorporation of Technologies in the Brazilian National Health System (CONITEC) from 2012 to 2023, and to identify the disinvestment criteria used. Methods Documentary, descriptive and retrospective analysis of CONITEC reports that assessed technology exclusion requests. Results We identified 24 reports on 74 technologies, whereby the requests predominantly involved medications (95.9%). CONITEC favorably recommended 95% of the exclusions, prioritizing the absence of registration with the National Health Surveillance Agency and the existence of therapeutic alternatives. Conclusion Low demand for exclusions compared to incorporations reveals challenges in identifying obsolescence and resistance to exclusion of technologies. The sustainability of the SUS requires greater monitoring of incorporated technologies, to optimize resources and promote the efficiency of the health system.

Resumo Objetivo: Estabelecer protocolo para realização de autópsias minimamente invasivas (AMIs) na detecção de óbitos por arboviroses e relatar dados preliminares desse protocolo no Ceará, Brasil. Métodos: Médicos patologistas foram treinados em AMI. Resultados: Estabeleceu-se protocolo para AMI, definindo-se critérios para amostras a serem coletadas, suas formas de armazenamento e diagnóstico, segundo o tipo de amostra biológica; em três meses, foram realizadas 43 AMIs, das quais 21 (48,8%) chegaram ao Serviço de Verificação de Óbito (SVO) com hipótese diagnóstica de alguma arbovirose e sete (16,3%) foram confirmados (seis de chikungunya; uma de dengue); também foram confirmados casos de covid-19 (n = 9), tuberculose (n = 5), meningite (n = 4), criptococose (n = 1), doença de Creutzfeldt-Jakob (n = 1), neoplasia de mama (n = 1) e raiva humana (n = 1). Conclusão: O protocolo implantado permitiu a captação de um maior número de óbitos suspeitos de arboviroses, além da confirmação de outras patologias de interesse da vigilância.

Resumen Objetivo: Estabelecer un protocolo utilizado para la realización de autopsias mínimamente invasivas (AMI) para la detección de muertes por arbovirus y presentar datos preliminares de este protocolo en Ceará, Brasil. Métodos: Se llevó a cabo la capacitación de médicos patólogos en AMI. Resultados: Se estableció un protocolo para la realización de AMI, que define los criterios para la toma de muestras, métodos de almacenamiento y diagnóstico; en tres meses se realizaron 43 AMI; de estas, 21 (48,8%) llegaron al Servicio de Verificación de Óbito (SVO) con una hipótesis diagnóstica de alguna arbovirosis y siete (16,3%) fueron confirmadas (seis casos de chikungunya y uno de dengue); también se confirmaron casos de Covid-19 (n = 9), tuberculosis (n = 5), meningitis (n = 4), criptococosis (n = 1), enfermedad de Creutzfeldt-Jakob (n = 1), neoplasia de mama (n = 1) y rabia humana (n = 1). Conclusión: El protocolo implementado permitió la identificación de un mayor número de muertes sospechosas de arbovirus, además de la confirmación de otras patologías de interés.

Abstract Objective: To create a protocol for performing minimally invasive autopsies (MIA) in detecting deaths from arboviruses and report preliminary data from its application in Ceará state, Brazil. Methods: Training was provided to medical pathologists on MIA. Results: A protocol was established for performing MIA, defining criteria for sample collection, storage methods, and diagnoses to be carried out according to the type of biological sample; 43 MIAs were performed in three months. Of these, 21 (48.8%) arrived at the Death Verification Service (SVO) with arboviruses as a diagnostic hypothesis, and seven (16.3%) were confirmed (six chikungunya cases and one dengue case); cases of COVID-19 (n = 9), tuberculosis (n = 5), meningitis (n = 4), cryptococcosis (n = 1), Creutzfeldt-Jakob disease (n = 1), breast cancer (n = 1), and human rabies (n = 1) were also confirmed. Conclusion: The protocol implemented enabled identification of a larger number of suspected arbovirus-related deaths, as well as confirmation of other diseases of interest for surveillance.

O Programa Alimentar Brasileiro Cardioprotetor (Dica Br) consiste em uma estratégia de aconselhamento nutricional com o objetivo de prevenção da incidência de novos eventos cardiovasculares. Esta investigação qualitativa é um estudo de caso do Dica Br e visa compreender o processo de escolhas alimentares relacionadas à adesão dos participantes às estratégias orientadas. Aplicou-se a análise de conteúdo temática para explorar as entrevistas semiestruturadas realizadas com dez participantes. Os pacientes relataram incorporar as orientações nutricionais e como negociam suas escolhas alimentares para alcançar uma alimentação saudável mantendo o consumo de alimentos prazerosos, sendo alguns recomendados a serem evitados pelo Dica Br. As contínuas tensões e conflitos entre comer por saúde e comer por prazer que permeiam as escolhas alimentares dos pacientes devem ser abordados no aconselhamento nutricional, possibilitando maior e duradoura adesão ao Dica Br.

El Programa Alimentario Brasileño Cardioprotector (Dica Br) consiste en una estrategia de prevención de nuevos eventos cardiovasculares. Esta investigación cualitativa es un estudio de caso del Dica Br con el objetivo de comprender el proceso de elección alimentaria relacionada con la adhesión a las estrategias orientadas. Se aplicó el análisis de contenido temático para explorar las entrevistas semiestructuradas realizadas con 10 participantes. Los pacientes informaron incorporar pautas nutricionales y cómo negocian sus elecciones alimentarias para lograr una dieta saludable mientras mantienen el consumo de alimentos placenteros, algunos de los cuales se recomienda evitar. Las tensiones y conflictos continuos entre comer para la salud y comer por placer que impregnan las elecciones alimentarias deben abordarse en el asesoramiento nutricional para una mayor y duradera adherencia al Dica Br.

The Brazilian Cardioprotective Nutritional Program (Balance)(f) consists of a nutritional counseling strategy aiming to prevent new cardiovascular events. This qualitative investigation is a case study of Balance aiming to understand the process of food choices related to the adherence of participants to the nutritional counseling. Semi-structured interviews were conducted with 10 participants and analyzed according to thematic content analysis. Patients reported incorporating nutritional advice and how they negotiate food choices to achieve a healthy diet while maintaining the consumption of pleasurable foods, some of them recommended avoiding. The continuous tensions and conflicts between healthy and pleasurable eating which permeate patients’ food choices should be addressed in nutritional counseling, enabling greater and long-lasting adherence to Balance.
